Sistema portátil de terapia de heridas
ANTECEDENTES
- 1.
- Campo Técnico La presente invención se refiere al tratamiento de una herida abierta, y, más concretamente, se refiere a un sistema portátil de terapia de heridas que incluye un vendaje para la herida en combinación con una presión subatmosférica para favorecer la cicatrización de la herida abierta.
- 2.
- Descripción de la Técnica Relacionada El cierre de la herida implica la migración de tejido epitelial y subcutáneo adyacente a la herida hacia el centro y fuera de la base de la herida hasta que la herida se cierre. Desgraciadamente, el cierre es difícil con las grandes heridas, las heridas crónicas o las heridas que se han infectado. En tales heridas, se forma una zona de éxtasis (es decir, un área en la cual la hinchazón del tejido restringe la circulación de sangre en los tejidos) cerca de la superficie de la herida. Sin suficiente circulación de sangre, los tejidos epiteliales y subcutáneos que rodean la herida no solamente reciben oxígeno y nutrientes disminuidos, sino que también son menos capaces de enfrentarse satisfactoriamente con una infección microbiana y, por tanto, son menos capaces de cerrar la herida naturalmente. Esas heridas han presentado dificultades al personal médico durante muchos años.
La terapia de presión negativa conocida también como terapia de vacío o aspiración ha sido usada durante muchos años en el tratamiento y cicatrización de heridas. Una diversidad de dispositivos de presión negativa ha sido desarrollada para drenar el exceso de fluido de la herida, es decir, los exudados, de la herida para proteger la herida y, consecuentemente, mejorar la cicatrización. Diversos vendajes de la herida han sido empleados con los dispositivos de presión negativa para aislar la herida y mantener el ambiente subatmosférico.
Un dispositivo de la técnica anterior que comprende las características del preámbulo de la reivindicación 1 se describe en US 2004/0073151.
SUMARIO Según la reivindicación 1, el sistema portátil para la terapia de presión subatmosférica en relación con la cicatrización quirúrgica o herida crónica, incluye entre otras cosas un vendaje de herida dimensionado para ser posicionado con relación a un lecho de la herida de un sujeto, un mecanismo de presión subatmosférico portátil dimensionado para ser transportado o usado por el sujeto y un receptáculo para recoger exudados del lecho de la herida extraídos a la presión subatmosférica suministrada por el mecanismo de presión subatmosférico. El mecanismo de presión subatmosférico portátil incluye un alojamiento, una fuente de presión subatmosférica dispuesta dentro del alojamiento y en comunicación fluida con el vendaje de la herida para suministrar presión subatmosférica al vendaje de la herida y una fuente de potencia montada en o dentro del alojamiento para suministrar potencia para accionar la fuente de presión subatmosférica. La fuente de presión subatmosférica incluye un miembro de bomba. El miembro de bomba es seleccionado del grupo compuesto de una bomba de diafragma, una bomba de doble diafragma, Una bomba de fuelle y una bomba peristáltica.
Un accionador para activar el miembro de bomba puede ser proporcionado también. El accionador puede estar montado en el alojamiento. El accionador puede ser seleccionado del grupo compuesto de motores de AC (corriente alterna), motores de DC (corriente continua), accionadores de bobina de voz y solenoides. La fuente de potencia puede incluir baterías desechables o baterías recargables y puede estar montada de modo liberable en el alojamiento. La fuente de potencia puede ser reutilizada con nuevos componentes del mecanismo de presión subatmosférico.
El receptáculo puede estar montado en o dentro del alojamiento. El receptáculo puede ser relativamente flexible. Alternativamente, el receptáculo flexible puede ser remoto del alojamiento y usado por el paciente. El receptáculo flexible incluye al menos una trayectoria de recogida o una pluralidad de trayectorias de recogida definidas en el interior para contener los exudados. La pluralidad de trayectorias de recogida puede definir una serpentina o disposición de trayectoria de fluido paralela.
El mecanismo de presión subatmosférico puede ser descartado después de un periodo único de utilización. Alternativamente, algunos de los compartimentos del mecanismo de presión subatmosférico pueden ser reutilizados.
El vendaje de la herida incluye un miembro de contacto con la herida para ser posicionado contra el lecho de la herida; incluye un miembro de guarnición de la herida y una cubierta de la herida para envolver un perímetro del lecho de la herida. El miembro de contacto con la herida puede incluir una sección porosa para permitir el paso de los exudados. El miembro de contacto con la herida puede ser sustancialmente no adherente al lecho de la herida. El miembro de empaquetado de la herida incluye una pluralidad de fibras o filamentos en una disposición de arrastre. La cubierta de la herida puede estar adaptada para permitir el paso de humedad.

DESCRIPCIÓN DE LAS REALIZACIONES PREFERIDAS El sistema de terapia de la herida de la presente descripción favorece la cicatrización de una herida por medio del uso de un apósito de la herida y un mecanismo de presión subatmosférico portátil. El sistema de terapia de la herida es completamente portátil, es decir, puede ser usado o transportado que el sujeto de modo que el sujeto puede ser completamente ambulatorio durante el periodo de la terapia. El sistema de terapia de la herida que incluye el mecanismo de presión subatmosférica y componentes del mismo puede ser completamente desechable después de un periodo predeterminado de utilización o puede ser desechable individualmente de modo que algunos de los componentes sean reutilizados en una aplicación de terapia subsiguiente. Generalmente, el mecanismo de presión subatmosférica portátil aplica presión subatmosférica a la herida para desplazar fluidos o exudados capturados por el apósito de la herida compuesto, e, incrementar la circulación de sangre en el lecho de la herida y mejorar la estimulación celular del tejido epitelial y subcutáneo.
El sistema de terapia de la herida de la presente exposición favorece la cicatrización de una herida conjuntamente con la terapia de presión negativa subatmosférica. El sistema puede incorporar una diversidad de vendajes de herida y fuentes y bombas de presión subatmosférica.
Haciendo referencia ahora a la Figura 1, en ella se ilustran diversas realizaciones del sistema 100 de terapia de la herida de la presente descripción. El sistema 100 de terapia de la herida incluye el apósito 102 de herida compuesto y el mecanismo 104 de presión subatmosférica en comunicación fluida con el apósito 102 de la herida a través de un conducto, identificado esquemáticamente con el número 106 de referencia. En la Figura 1, se muestran tres mecanismos 104a, 104b, 104c de presión subatmosférica alternativa. Los mecanismos 104a, 104b, 104c de presión subatmosférica comparten componentes similares como se apreciará. Cualquiera de los mecanismos 104a, 104b, 104c de presión subatmosférica es contemplado para ser usado con el vendaje 102 de la herida.
El vendaje 102 de la herida incluye varios componentes, especialmente, una capa o miembro 108 de contacto con la herida, un miembro o relleno 110 de empaquetado de la herida soportado por el miembro 108 de contacto y una capa exterior o miembro 112 de cubierta. El miembro 108 de contacto con la herida está destinado a configurar la topografía de un lecho “w” de la herida. El miembro 108 de contacto con la herida es sustancialmente poroso para permitir que los exudados pasen desde el lecho “w” de la herida a través del miembro 108 de contacto con la herida. La porosidad del miembro 108 de contacto puede ser ajustada variando el tamaño de las aberturas o perforaciones de diámetro o tamaño y en la distribución alrededor del miembro 108 de contacto. Por tanto, la circulación de fluido desde la herida puede ser optimizada y la adherencia del miembro 108 de contacto al lecho de la herida puede ser minimizada. El miembro 108 de contacto con la herida puede ser también no adherente. Esta configuración permite que el fluido y los exudados circulen sin inhibiciones a través del miembro 108 de contacto con la herida con una “adherencia” mínima del miembro 108 de contacto de la herida con el lecho “w” de la herida mientras mantiene un equilibrio húmedo adecuado de la herida. La Figura 1A ilustra el miembro 108 de contacto con los poros 108a. Los poros 108a pueden ser de igual tamaño o diámetro o tener tamaños y dimensiones que varíen o aleatorios.
El paso de exudados de la herida a través del miembro 108 de contacto es preferiblemente unidireccional de modo que los exudados de la herida no retroceden al lecho “w” de la herida. Esta característica de circulación
unidireccional podría ser originada por aberturas direccionales impartidas dentro de la capa de material, una estratificación de los materiales de diferente capacidad de transferencia de fluido o a la capacidad de la mecha o de una selección de material concreta que provoque la circulación direccional de exudados. No obstante, una capa bidireccional para los propósitos de suministro de medicinas o antiinfecciosos en el lecho “w” de la herida es también contemplada y se describirá más adelante.
En adición, agentes tales como hidrogeles y medicamentos podrían ser ligados o revestidos en el miembro 108 de contacto para reducir la biocarga en la herida, favorecer la cicatrización, incrementar la circulación de sangre en el lecho de la herida y reducir el dolor asociado con los cambios o eliminación de apósitos. Los medicamentos incluyen, por ejemplo, agentes antimicrobianos, factores de crecimiento, antibióticos, analgésicos, agentes de desbridado de oxido nítrico, enriquecimiento de oxígeno y similares. Además, cuando sea usado un analgésico, el analgésico podría incluir un mecanismo que permitiera la liberación de ese agente antes de la retirada o cambio del apósito.
El miembro 108 de contacto puede ser construido de una diversidad de materiales diferentes. Estos pueden incluir pero sin limitarse a fibras de polímeros sintéticas no absorbibles tales como polímeros carbonizados, polietileno (PE), polipropileno (PP), politetrafluoretileno (PTFE), nailon, aramidas, Kevlar, tereftalato de polietileno PET) o fibras naturales tales como celulosa. Se contempla que el miembro 108 de contacto pueda ser transparente para permitir una capacidad visual mejorada y una mejor visión del lecho “w” de la herida. Además, el miembro 108 de contacto puede ser construido de una tela que podría ser tejida, no tejida (incluyendo soplada en fusión) que incluya, estructuras de punto o compuestas tales como tejidos ligados por centrifugación. Materiales a modo de ejemplo usados como miembro 108 de contacto son vendidos bajo la marca comercial EXCILON® o XEROFLOW® ambas por la Kendall Corp., una división de TycoHealthcare.
El miembro 110 de acondicionamiento del apósito 102 de la herida está destinado a absorber y transferir fluido y exudados de la herida. Materiales absorbentes a modo de ejemplo usados en la técnica anterior incluyen espumas, telas compuestas no tejidas, hidrogeles, telas celulósicas, polímeros superabsorbentes y combinaciones de los mismos. Típicamente, el miembro 110 para empaquetar puede contener o absorber hasta alrededor de 100 centímetros cúbicos o más de fluido de la herida. Preferiblemente, el material absorbente incluye el vestido antimicrobiano vendido bajo la marca comercial KERLIX® AMD por la Kendall Corp., una división de TycoHealthcare. En una realización preferida, el miembro 110 de empaquetado podría ser preconfigurado o conformado para variar la forma del lecho de la herida. Los expertos en la técnica reconocerán que el miembro 110 de empaquetado puede ser configurado de cualquier manera adecuada. Según la técnica anterior, el miembro 110 de empaquetado puede incluir múltiples capas. En otra realización preformada, el miembro empaquetado puede ser construido por capas de materiales absorbentes que varían para ayudar en la circulación direccional de exudados fuera de la herida.
Adicionalmente, con referencia a la Figura 1B, el miembro 110 de empaquetado puede incluir materiales hidrófobos tales como fibras sintéticas continuas destinadas a transferir fluidos de la herida bajo una presión negativa. Las fibras sintéticas pueden ser construidas a partir de materiales polímeros tales como polipropileno, polietileno, poliéster y otros tipos de polímeros. Las fibras continuas pueden ser dispuestas en mazos o de múltiples fibras para ayudar a facilitar el ángulo o forma en el miembro 110 de empaquetado de la herida. Según la presente invención, las fibras están en una disposición para ser remolcadas representada en la Figura 1B. Además, los mazos de fibras pueden ser enlazados con longitudes dadas usando tiras o alguna forma de adhesivo, dentro de una zona 110b fundida. La zona fundida 110b puede ser un área para cortar el mazo de fibras en el mazo de fibras con una longitud seleccionada para adaptar el miembro 110 de empaquetado de la herida óptimamente para diversos tamaños de la herida.
Alternativamente, el miembro 110 de empaquetado de la herida podría ser de materiales hidrófobos/no absorbentes para minimizar los fluidos de la herida cerca de la herida. Tales materiales son fibras en una disposición para ser remolcadas, compuestas de PTFE, PE, PET o materiales hidrofílicos tratados con silicona o una solución de PTFE.
Adicionalmente, el miembro 110 de empaquetado absorbente o no absorbente podría ser tratado con medicamentos. Los medicamentos incluyen, por ejemplo, un agente antiinfeccioso tal como un antiséptico u otro antimicrobiano o combinación de antimicrobianos adecuada, biguanidina de polihexametileno (aquí en adelante, “PHMB”), antibióticos, analgésicos, factores de cicatrización tales como vitaminas, factores de crecimiento, nutrientes y similares, así como un agente de fusión tal como una solución salina isotónica.
En la alternativa, el miembro 110 de empaquetado absorbente o no absorbente, puede incluir una disposición nervada como se describe en la Publicación de Patente Nº 2007/0185463. Las espumas descritas en la solicitud ‘921 se prefiere que sean sustancialmente rígidas para mantener así sus formas durante al menos un periodo predeterminado de tiempo durante la cicatrización. Las espumas cuando están dispuestas dentro del lecho “w” de la herida definen espacios o pasajes entre ellas para permitir que los exudados de la herida pasen a través de los pasajes. Los tamaños de las espumas pueden variar, pero estas deberían ser dimensionadas para conseguir el tamaño de poro correcto a través de la disposición de la espuma para facilitar la proliferación de celdas y permitir que el fluido y el aire sean evacuados de la herida. Una porosidad en el rango de 10-1000 μm ha sido hallada
beneficiosa en la estimulación y proliferación de celdas de estimulación y en permitir que el fluido y el aire sean evacuados de la herida. Las espumas pueden trabajar en combinación con el miembro de 108 contacto para conformar el lecho “w” de la herida mientras permite el drenaje de los exudados de la herida y la liberación de aire del lecho “w” de la herida sin obstrucciones, A medida que es aplicada la presión negativa, los bordes se mueven y reajustan sus respectivas posiciones para evitar el crecimiento interior que puede ocurrir con diseños de vestidos de espuma corrientes.
Haciendo referencia de nuevo a la Figura 1, el miembro exterior o cubierta 112 de la herida que rodea el perímetro del apósito 100 de la herida para rodear el lecho “w” de la herida y para proporcionar una obturación hermética a líquidos alrededor del perímetro “p” del lecho “w” de la herida. Por ejemplo, el mecanismo de obturación puede ser cualquier adhesivo enlazado con una capa que rodee el lecho “w” de la herida. El adhesivo debe proporcionar una adhesión aceptable al tejido que rodea el lecho “w” de la herida, por ejemplo, el área del perímetro de la herida, y ser aceptable para ser usado sobre la piel sin deterioración de contacto (por ejemplo, el adhesivo deberá ser preferiblemente no irritante y no sensibilizador). El adhesivo puede ser permeable o semipermeable para permitir que la piel en contacto con este respire y transmita la humedad. Adicionalmente, el adhesivo podría ser activado o desactivado mediante un estímulo externo tal como calor, una solución fluida dada o una reacción química dada. Los adhesivos incluyen, por ejemplo, acrílicos de grado médico como el adhesivo usado con el vendaje CURAFOAMISLAND® del Grupo TycoHealthcare, LP o cualquier silicona o caucho basados en adhesivos médicos que sean amigables para la piel y no irritantes. El miembro 112 de cubrición de la herida puede ser proporcionado con un respaldo adhesivo y/o alternativamente, un adhesivo puede ser aplicado a la cubierta 112 de la herida y/o la piel durante el procedimiento. Como una alternativa más, un miembro 114 adhesivo de forma anular puede ser interpuesto entre la periferia de la cubierta 112 de la herida y solapando la periferia del miembro 108 de contacto para asegurar la cubierta 112 de la herida alrededor de la herida “w”.
La cubierta 112 de la herida es típicamente de un material flexible, por ejemplo, elástico o elastómero, que obtura la parte superior del vendaje 102 de la herida para impedir el paso de líquidos o contaminación al y desde el vendaje 102 de la herida. Los materiales flexibles a modo de ejemplo incluyen el vendaje transparente semipermeable fabricado bajo la marca comercial Polyskin II® de la Kendall Corp., una división del Tyco Healthcare Group L.P. El Polyskin II® es un material semipermeable transparente que permite el paso de la humedad o vapores de agua desde el lugar de la herida a través del vendaje 102, mientras proporciona al mismo tiempo una barrera a los microbios y la contención del fluido. Películas alternativas podrían ser fabricadas de poliuretanos, poliolefinas respirables, copoliésteres, o estratificados de estos materiales. La transparencia de la cubierta 112 de la herida permite una revisión visual del estado del vendaje 102 de la herida y del nivel de saturación del miembro 110 de empaquetado. Como una alternativa, el miembro 112 de cubrición exterior puede ser impermeable a los vapores húmedos.
Un vendaje de heridas adecuado exterior se describe en común asignado a los Núms. de Publicación de Patente de EE. UU. 2007/0078366, 2007/0066946 y 2007/0055209.
Haciendo referencia ahora al diagrama esquemático de la Figura 2, en combinación con la Figura 1, será examinado el mecanismo 104 de presión subatmosférica. El mecanismo 104 de presión subatmosférica incluye el alojamiento 116, la fuente o bomba 118 de vacío dispuesta dentro del alojamiento 116, el accionador o motor 120 dispuesto con el alojamiento 116a para activar la fuente 118 de vacío y la fuente 122 de potencia montada relativa al alojamiento
114. La fuente o bomba 118 de vacío puede ser cualquier bomba miniatura o microbomba que sea biocompatible y está destinada a mantener u obtener niveles de vacío adecuados y terapéuticos. Preferiblemente, el nivel de vacío que se ha de lograr está en el margen de alrededor de 75 mm Hg y alrededor de 125 mm Hg. La fuente de vacío o bomba 118 puede ser una bomba del tipo diafragmático, peristáltico, de fuelle o similar, en la cual la parte (s) que se mueve arrastra exudados fuera del lecho “w” de la herida dentro del vendaje 102 de la herida creando áreas o zonas de presión disminuida, por ejemplo, zonas de vacío con el vendaje 100 de la herida. Esta área de presión disminuida comunica preferiblemente con el lecho “w” de la herida para facilitar la retirada de los fluidos del mismo y dentro del miembro 110 de empaquetado absorbente o no absorbente. La fuente de vacío o bomba 118 es accionada por el accionador 120 que puede ser cualquiera de los medios conocidos por los expertos en la técnica, incluyendo por ejemplo, motores de AC, motores de DC, accionadores de bobina de voz, solenoides, etc. El accionador 120 puede estar incorporado dentro de la bomba 118.
La fuente 122 de potencia puede estar dispuesta dentro de la bomba 116 o montada separadamente en el alojamiento 116. Una fuente 122 de potencia adecuada incluye baterías alcalinas, baterías de celda húmeda, baterías de celda seca, baterías de cadmio níquel, medios generados solares, baterías de litio, baterías de NiMH (de metal níquel hídridas), cada una de las cuales puede ser de variedad desechable o recargable.
Componentes adicionales del mecanismo de presión subatmosférica pueden incluir sensores 124 de presión para vigilar la presión adyacente a la fuente o bomba 118 de vacío o en lugares seleccionados desplazados de la bomba 118, y medios 126 de regulación o control. Los medios 126 de control pueden incorporar un controlador/accionador 128 de motor que incluya circuitos de proceso y accionamiento para controlar o variar la tensión de accionamiento en el motor de la fuente o bomba 118 de vacío sensible a la presión detectada por el sensor 124 de presión. La potencia de salida del motor de la bomba 118 de vacío puede ser aumentada o disminuida, o iniciada o interrumpida,
como controlada por los medios 126 de control. El sensor 124 de presión deberá proporcionar también información para ayudar en la detección de una fuga en el sistema 100 de terapia de la herida si la presión óptima subatmosférica no se consigue. Los medios 126 de regulación y control pueden tener también una alarma tal como una alarma sensorial visual, de audio o táctil (por ejemplo, vibratoria, etc.) para indicar al usuario cuando han sido encontradas condiciones específicas (por ejemplo, el nivel de vacío deseado o la pérdida de vacío). El sensor 124 de presión puede ser sustituido o suplementado con cualquier otro tipo de sensor o detector para medir o detectar una característica o condición del lecho de la herida “w”. Sensores adicionales contemplados incluyen sensores térmicos, sensores bacteriales, sensores de oxígeno, sensores de humedad, etcétera, que proporcionarían al clínico información de diagnóstico adicional.
Haciendo referencia de nuevo a la Figura 1, el sistema 100 de terapia de la herida incluye además la recogida del receptáculo 132 que almacena los exudados recogidos de la herida “w” durante la terapia a través del entubado 106. El receptáculo 132 de recogida está asociado preferiblemente con el alojamiento 116 y puede ser incorporado dentro del alojamiento 116 o conectado de modo liberable al alojamiento 116 mediante medios convencionales. El receptáculo 132 es sustancialmente rígido y define una cámara interna en comunicación fluida con el entubado 106. Como una alternativa, el receptáculo 132 de recogida puede incluir una unidad flexible contenida dentro del alojamiento 116 y desmontable para facilitar el desecho de los fluidos de la herida.
En el mecanismo 104 de presión subatmosférica de la Figura 1, la fuente o bomba 118 de vacío, el motor 120, el sensor 124 de presión y los medios 126 de control están incorporados dentro del alojamiento 116. El sensor 124 de presión puede ser desplazado también del alojamiento de la microbomba 118, por ejemplo, adyacente al miembro 110, a un lugar desplazado del alojamiento 120, y conectado a los medios 126 de control a través de una conexión eléctrica. La fuente 122 de potencia puede estar incorporada dentro del alojamiento 116 o puede estar conectada de modo liberable al alojamiento 116 a través de medios convencionales.
En la realización del mecanismo 104a de presión subatmosférico, el mecanismo 104a subatmosférico está destinado a una aplicación de uso único, es decir, el mecanismo subatmosférico 104a se dispone después de un periodo predeterminado de tiempo. Ese periodo de tiempo puede variar de alrededor de un día a alrededor de siete días o más. Una aplicación contemplada es un periodo de tiempo de tres días. Por tanto después de tres días de terapia, el mecanismo 104a subatmosférico que incluye los componentes (fuente de vacío o bomba 118, accionador o motor 120, fuente 122 de potencia, sensor 124 de presión y medios 126 de control) así como el apósito 102 de la herida, el receptáculo 132 de recogida, y el entubado 106 de recogida son desechados. En la realización del mecanismo subatmosférico 104b, todos los componentes (fuente o bomba 118 de vacío, accionador o motor 120, sensor 124 de presión, medios 126 de control, y receptáculo 132 de recogida) son desechados después del periodo predeterminado de tiempo, por ejemplo, de alrededor de un día a alrededor de siete días, con la excepción de la fuente 122 de potencia. A este respecto, la fuente 122 de potencia tiene una mayor longitud de vida, por ejemplo, una duración de alrededor de veinte a alrededor de cuarenta días, o de más de alrededor de 30 días. Por tanto la fuente 122 de potencia puede ser montada de modo desmontable como se muestra en la Figura 1, y reconectada en el alojamiento 116 para una aplicación subsiguiente con los nuevos componentes. Cualquier medio para montar de modo desmontable la fuente 122 de potencia en el alojamiento puede ser apreciado por un experto en la técnica. La fuente 122 de potencia puede ser recargable.
En la realización del mecanismo 104c de presión subatmosférica, los componentes eléctricos tendrán una mayor esperanza de vida, por ejemplo, entre alrededor de veinte y alrededor de cuarenta días, más de treinta días. Por tanto, estos componentes pueden ser reutilizados para una aplicación subsiguiente después de la recogida del receptáculo 132, el vendaje 102 de la herida y el entubado 106 que son desechadas.
Con referencia ahora a la Figura 3A, en ella se ilustra una bolsa 134 de soporte de cuerpo para soportar al menos el mecanismo 104 de presión subatmosférica y al menos el receptáculo 132. Como se ha expuesto, el sistema 100 de terapia de la herida de la presente descripción está destinado a ser montado en el cuerpo del paciente para que sea una unidad portátil autocontenida. A este respecto, el mecanismo de bomba subatmosférica y el receptáculo pueden ser al menos parcialmente transportados o soportados por la bolsa 134 de soporte de cuerpo. La bolsa 134 de soporte de cuerpo incluye generalmente una bolsa 136 y al menos una brida 138, preferiblemente dos bridas, para asegurar la bolsa 136 al cuerpo del paciente. La bolsa 134 de soporte de cuerpo está destinada a recibir y almacenar al menos el mecanismo 104 de bomba subatmosférica y el receptáculo 132 de recogida. La bolsa 134 de soporte de cuerpo puede ser llevada alrededor de la cintura del paciente tal como si fuese una cinturón. Esto es conveniente porque puede reducir la longitud de los tubos necesarios dependiendo del lugar de la herida. En adición, la bolsa 136 puede estar situada adyacente al abdomen del paciente lo cual puede presentar una posibilidad mejorada significativamente de ocultar el sistema. El tubo 106 puede estar asegurado al cuerpo con cinta, ligaduras,
o similares, u, opcionalmente, puede no estar asegurado y dispuesto entre la ropa del paciente. Por tanto, la bolsa 134 de soporte de cuerpo le permite al paciente moverse sin restricciones o limitaciones, y proporciona capacidad completamente portátil al paciente durante el drenaje y cicatrización de la herida.
La Figura 3B ilustra una realización alternativa de la bolsa de soporte de cuerpo. De acuerdo con esta realización, la bolsa 140 de soporte de cuerpo está destinada a ser montada en el hombro del paciente y tiene una bolsa 142. En
otros aspectos, la bolsa 140 que funciona de una manera similar a la de la bolsa de soporte de cuerpo de la Figura
3.
La Figura 4 ilustra una disposición en la que las bolsas 134, 140 de soporte de cuerpo son eliminadas. En esta disposición, una correa 144 está conectada al alojamiento 114 mediante medios convencionales tales como por ejemplo, una hebilla de deslizamiento, para permitir que el alojamiento 114 se deslice con relación a la correa 144, o, alternativamente, la correa 144 puede ser fijada directamente al alojamiento 114 mediante medios convencionales tales como, por ejemplo, una hebilla de deslizamiento para permitir que el alojamiento 114 se deslice con relación a la correa 144, o, alternativamente, la correa 144 puede ser fijada directamente al alojamiento 114. Múltiples disposiciones de la correa son también contempladas.
En uso, el vendaje de la herida se coloca adyacente al lecho “w” de la herida como se muestra en la Figura 1. El mecanismo 104 de presión subatmosférico se activa entonces creando un estado de presión reducida dentro del vendaje 102 de la herida. A medida que el bombeo progresa, los exudados son recogidos y dirigidos al receptáculo 132 de recogida. Cuando la fuente o bomba 118 de vacío es activada y establecida en un punto de establecimiento específico, la bomba 118 empezará a reducir la presión hasta que esta alcance el punto establecido. El vacío que se lee en la bomba permanecerá con este nivel hasta que el punto establecido sea cambiado, la bomba sea detenida, o se produzca una fuga importante en el sistema que supere la capacidad de la bomba para continuar manteniendo este nivel. La terapia de presión subatmosférica puede ser continua o intermitente.
Las Figuras 5A-5C ilustran realizaciones alternativas del sistema 200 de terapia de la herida de la presente exposición. De acuerdo con estas realizaciones, el receptáculo 202 de recogida se retira del alojamiento 204 y se dispone en línea entre el vendaje 206 de la herida y el mecanismo 208 de presión subatmosférica. Diversos receptáculos 202 se contemplan. En una realización, el receptáculo 202 es relativamente flexible y expandible, y define una cámara interna para recoger los exudados. Por tanto, como los exudados son recibidos dentro del receptáculo 202, el receptáculo 202 se expande para acomodar el volumen de exudados. El receptáculo 202 puede contener múltiples cámaras. En una realización mostrada en las Figuras 5A-5C y las figuras 6-7, el receptáculo 202 incluye múltiples canales o trayectorias 210 de recogida en comunicación fluida unas con otras. Los canales 210 pueden estar dispuestos en una relación de lado con lado como se muestra para definir de ese modo una disposición sinusoidal general. El receptáculo 202 puede incluir materiales superabsorbentes dentro de la cámara interna o trayectorias 210 de recogida tales como polímeros o geles superabsorbentes, es decir, un polímero que tenga la capacidad de absorber líquido en una cantidad varias veces mayor que su propio peso. Antimicrobianos para controlar el crecimiento de bacterias pueden ser también añadidos al receptáculo 202. El uso de tales polímeros mejorará significativamente la capacidad absorbente y el volumen de exudados contenido dentro del receptáculo 202. El receptáculo 202 puede incluir también una membrana 212 de filtración adyacente a la boca de salida que conduce a la bomba o alojamiento 204 para minimizar el pasaje de exudados a la bomba. Membranas 212 de filtración adecuadas incluyen filtros de membrana que incorporan películas polímeras con especificaciones de poro concretas. Tales películas polímeras pueden incluir nitrocelulosa, acetato de celulosa, PTFE hidrofílico, PTFE hidrofóbico, nailon, policarbonato. La Figura 5A ilustra una porción de depósito 202 eliminada para representar el lugar de la membrana 212 de filtración. La circulación fluida de exudados se indica mediante flechas direccionales

La Figura 8 ilustra una disposición alternativa en la que los canales 210 se extienden en una disposición general paralela dentro del depósito 202 junto con una porción importante de los canales 210. Los canales 210 están en comunicación con los conductos 216, 218 de entrada y salida que están en comunicación con el respectivo vendaje 102 de la herida y la bomba 118. También pueden ser añadidos materiales no absorbentes. Tales materiales pueden incluir TOW, fieltro o espuma.
El depósito 202 puede estar soportado por medio de cualquiera de las bolsas de soporte de cuerpo ilustradas en las Figuras 3-4. Alternativamente, como se muestra en la Figura 9, el depósito 202 pueden ser adheridos directamente al sujeto mediante medios convencionales que incluyen la cinta quirúrgica 220 o tiras de pierna (tiras elásticas). El deposito 202 puede ser adherido al área de pierna, área abdominal, área de la espalda, o a cualquier lugar sobre el cuerpo, o sobre o cerca del vendaje de la herida.
En la realización de la Figura 5A, el depósito 202, el vendaje 206 de la herida y el entubado 222 están destinados a ser reemplazados después de un periodo predeterminado de tiempo que se extiende entre uno y alrededor de siete días, más alrededor de tres días. El mecanismo 204 de presión subatmosférica que incluye fuente o bomba de vacío, accionador o motor, fuente de potencia, sensor de presión, medios de control (similares a los componentes correspondientes examinados aquí anteriormente) está destinado a una vida extendida, por ejemplo de entre alrededor de veinte a alrededor de cuarenta días, más de alrededor de treinta días. Por tanto, después, por ejemplo, cada periodo de tres días, el depósito 202, el entubado 222, y el vendaje 222 de la herida son desechados y reemplazados con nuevos componentes para la conexión al mecanismo de presión subatmosférica. En la realización de la Figura 5B, el depósito 202, el entubado 222 y el vendaje 206 serán reemplazados como se ha expuesto aquí anteriormente en relación con la realización de la Figura 5A. Similarmente, los componentes del mecanismo de presión atmosférica serán sustituidos de una manera similar. No obstante, el mecanismo 204 de presión subatmosférico es un sistema más avanzado y puede incluir una bomba de doble diafragma activada por medio de un accionador de bobina de voz. Capacidades de amortiguación pasiva tales como un aislamiento de espuma para reducir los niveles de ruido pueden ser también incorporadas dentro del alojamiento. En la realización de la Figura 5C, el depósito 202, entubado 222 y vendaje 206 de herida así como la fuente o bomba de vacío y el sensor son desechables tras una duración más corta de alrededor de uno a alrededor de siete días, o de alrededor de tres días.
5 La fuente de potencia, el accionador o motor y los medios de control están destinados a ser reutilizados con nuevos componentes que sustituyan los componentes que hayan sido descartados anteriormente.
Aunque la exposición ha sido ilustrada y descrita, no está destinada a ser limitada por los detalles mostrados, puesto que diversas modificaciones y sustituciones pueden hacerse sin salirse en forma alguna del alcance de las
10 presentes declaraciones. Como tales, más modificaciones y equivalencias de la invención aquí descrita se les pueden ocurrir a personas expertas en la técnica usando no más que una experimentación rutinaria, y todas esas modificaciones y equivalencias se considera que están dentro del alcance de la descripción tal como es definida por las reivindicaciones siguientes.

- REIVINDICACIONES1. Un sistema portátil para la terapia de presión subatmosférica en conexión con la cicatrización de una herida quirúrgica, comprendiendo el sistema:un vendaje (102) de herida dimensionado para el posicionamiento relativo en un lecho (w) de herida de un sujeto, comprendiendo el vendaje de herida una cubierta (112) de la herida adaptada para envolver el perímetro del lecho de la herida, y un miembro (108) de contacto con la herida para se colocado contra el lecho (w) de la herida, y un miembro (110) de empaquetado debajo de la cubierta de la herida; un mecanismo (104) de presión subatmosférica portátil dimensionado para ser transportado o llevado por el sujeto, comprendiendo el mecanismo de presión subatmosférica portátil:un alojamiento (116); una fuente (118) de presión subatmosférica dispuesta dentro del alojamiento y en comunicación fluida con el vendaje de la herida para suministrar presión subatmosférica al vendaje de la herida; una fuente (122) de potencia montada en o dentro del alojamiento para suministrar potencia para accionar la fuente de presión subatmosférica; y un receptáculo (132, 202) para recoger los exudados del lecho de la herida removidos bajo la presión subatmosférica suministrada por la fuente de presión subatmosférica; caracterizado porque:el miembro de empaquetado incluye una pluralidad de fibras en una disposición de arrastre destinada a facilitar la transferencia de los exudados fuera del lecho de la herida.
- 2. Un sistema portátil según la reivindicación 1, en el que la fuente de presión subatmosférica incluya un miembro(118) de bomba y un accionador (120) para activar el miembro de bomba.

- Un sistema portátil según la reivindicación 2, en el que el miembro (118) de bomba es seleccionado del grupo compuesto de una bomba de diafragma, una doble bomba de diafragma, una bomba de fuelle y una bomba peristáltica.
