Sistema para proporcionar drenaje continuo en terapia de heridas de presión negativa
Antedecentes
1. Campo técnico
La presente divulgación en general se refiere a tratar una herida con presión negativa o reducida. En particular, la divulgación se refiere a un sistema para proporcionar drenaje continuo de fluidos desde un sitio de herida a un recipiente de recolección.

Diversas técnicas para promover la curación de una herida implican proporcionar la succión de la herida. El protocolo general de NPWT prevé cubrir la herida con una capa de cubierta flexible tal como una película polimérica, por ejemplo, para establecer un depósito de vacío sobre la herida, donde u puede aplicarse una presión reducida mediante los procedimientos de evacuación individuales o cíclicos. Varias realizaciones de la presente divulgación proporcionan sistemas (o aparatos) de terapia de herida de presión negativa que incluyen un recipiente de recolección que tiene una cámara para recoger fluidos de la herida. La capa de cubierta 24 puede estar conformada por una membrana flexible, por ejemplo, una película polimérica o elastomérica, que puede incluir un adhesivo biocompatible en al menos una porción de la capa de cubierta 24, por ejemplo, en la periferia 26 de la capa de cubierta 24. Alternativamente, la capa de cubierta 24 puede ser una membrana sustancialmente rígida. En una realización, la capa de cubierta 24 se forma a partir de una membrana permeable al vapor de humedad, por ejemplo, para promover el intercambio de oxígeno y humedad entre el lecho de herida "w" y la atmósfera. Un ejemplo de una membrana que puede proporcionar una velocidad de transmisión de vapor de humedad adecuado (MVTR) es una membrana transparente disponible en el mercado bajo el nombre comercial POLYSKIN® II ofrecida por Tyco Healthcare Group LP (d / b/a Covidien). Una membrana transparente puede ayudar a permitir que se haga una inspección visual del estado de la herida sin requerir la retirada de la capa de cubierta 24.
El apósito para herida 12 puede incluir un puerto de vacío 30 que tiene un reborde 34 para facilitar la conexión de la cámara de vacío 14 a un sistema de vacío. El puerto de vacío 30 puede estar configurado como un componente rígido o flexible, de bajo perfil y puede adaptarse para recibir un conducto 36 en una manera liberable y hermética a fluidos. Un adhesivo en al menos una porción del lado inferior del reborde 34 puede utilizarse para proporcionar un mecanismo para fijar el puerto de vacío 30 a la capa de cubierta 24. Las posiciones relativas, tamaño y/o forma del puerto de vacío 30 y el reborde 34 pueden variar de una realización representada en la FIG. 1. Puede utilizarse cualquier conducto apropiado para el conducto 36, que incluye conducto fabricado a partir de materiales elastoméricos o poliméricos flexibles. Al menos una parte del depósito 38 puede ser transparente o semi-transparente, por ejemplo, para permitir una evaluación visual del exudado de la herida para ayudar en la evaluación del color, la calidad y / o cantidad de exudado. El recipiente de recolección 38 está en comunicación fluida con el apósito para herida 12 a través de la primera y segunda secciones de conducto 36A, 36B. La tercera y cuarta secciones de conducto 36C, 36D conectan el recipiente de recolección 38 a la fuente de vacío 40 que genera o de otro modo proporciona a presión negativa al recipiente de recolección 38. La fuente de vacío 40 puede incluir una bomba peristáltica, una bomba de diafragma u otro mecanismo adecuado. La fuente de vacío 40 puede ser una bomba en miniatura o microbomba que puede ser biocompatible y adaptada para mantener o extraer los niveles de vacío terapéuticos y adecuados. El nivel de vacío de la presión subatmosférica alcanzado puede estar en el intervalo de aproximadamente 20 mmHg a aproximadamente 500 mmHg. El conducto de ventilación 37 está configurado para un flujo inferior de aire desde el depósito 14 al recipiente de recolección 38. El conducto de ventilación 37 incluye un diámetro más pequeño que el conducto de exudado 36 y puede estar conformado por cualquier conducto adecuado incluyendo aquellos fabricados a partir de materiales elastoméricos o poliméricos flexibles. El puerto 230 está además configurado para comunicar de forma fluida la segunda abertura 225 del apósito para herida 212 con el recipiente de recolección 40 a través del conducto de ventilación 237. Como se debatió anteriormente, la diferencia de tamaño entre el conducto de exudado 236 y conducto de ventilación 237 resulta en la acción capilar que hace que el fluido fluya solamente a través del mayor conducto de exudado 36.
Con referencia ahora a la FIG. 4, en aún otra realización, un apósito para herida 312 similar a aquellos que se describen más arriba que incluye un ensamblaje de ventilación 340 formado en una capa de cubierta 322. El ensamblaje de ventilación 340 incluye un elemento de filtro 342 y una solapa o elemento de cubierta 344. El elemento de filtro 342 puede estar integralmente formado con, o de otra manera fijado a, la capa de cubierta 322. En una realización, el elemento de filtro 342 está fijado a la capa de cubierta 322 con un adhesivo. El elemento de filtro 342 está configurado para depósito 314 del apósito para herida 312 con aire filtrado. Para evitar que se moje, el elemento de filtro 342 puede ser hidrófobo. El elemento de filtro 342 puede estar dimensionado en función del flujo pasante deseado. Un mayor elemento de filtro 342 proporcionaría una mayor cantidad de flujo de aire; sin embargo, si el elemento de filtro 342 es demasiado grande, se puede reducir la eficacia de la NWPT.
La solapa 344 puede estar formada integralmente con capa de cubierta 322. Alternativamente, la solapa 344 se puede asegurar de forma liberable sobre el elemento de filtro 342. La solapa 344 puede estar unida a o separable del elemento de cubierta 322. La solapa 344 puede estar configurada para descubrir selectivamente parcial o completamente el elemento de filtro 342. De esta manera, un clínico puede afectar el flujo de aire en el depósito 314. Aunque se muestra incluyendo la solapa 344, se contempla que el apósito para herida 312 pueda estar provisto del elemento de filtro 342 expuesto.
En su uso, el apósito para herida 312 se aplica a una herida "w" de una manera convencional. La activación de la fuente de vacío 40 (Figs. 1 y 2) inicia el drenaje del depósito 314 del apósito para herida 312. En cualquier momento antes o durante el proceso de drenaje, la solapa 344 puede ser tirada parcial o completamente hacia atrás para exponer el elemento de filtro 342. Como se describió anteriormente, cuanto más expuesto está el elemento de filtro 342, más flujo de aire en el depósito 14 es posible. El flujo de aire proporcionado al depósito 14 a través del elemento de filtro 342 actúa de una manera similar a la acción de sumidero descrita anteriormente. De esta manera, el ensamblaje de ventilación 340 permite el flujo de fluido continuo a través del conducto de exudado 336, evitando de ese modo el estancamiento de fluido y sus complicaciones.
Con referencia a las FIGS. 5Ay 5B, en aún otra realización, se muestra un puerto de herida 500. El puerto de herida 500 es adecuado para su uso con los apósitos para herida descritos anteriormente. El puerto de herida 500 tiene una cubierta de plástico 510 que incluye un puerto de vacío 512. Además del puerto de vacío 512, la cubierta de plástico 510 tiene el tubo 520. El tubo 520 puede estar hecho de un acero inoxidable de calibre pequeño o de plástico rígido. El tubo 520 se utiliza para proporcionar una fuga controlada o fija por la admisión de aire en el apósito para herida. El tubo 520 puede estar dispuesto para permitir la inserción del tubo 520 en el puerto de herida 500 de modo que es posible el ajuste de la profundidad y la colocación dentro del material de embalaje de la herida, como se indica mediante las flechas en las figuras. 5A y 5B. Como tal, el aire puede ser inyectado en el material de embalaje para dirigir el movimiento de exceso de exudado hacia el puerto de vacío y fuera de la herida. El tubo 520 puede tener una válvula (no mostrada) para ajustar la velocidad de flujo de aire en el lecho de herida. La válvula puede ser una válvula de aguja pequeña que se puede conectar al tubo 520 para permitir el ajuste infinito de flujo de aire en el apósito para herida..
El extremo del tubo 520 que puede estar expuesto a la atmósfera ambiente o a una fuente de aire puede incluir un filtro 522. El filtro 522 puede ser un filtro de aire como de punta q para evitar la obstrucción del tubo y también evitar que la suciedad y otros contaminantes entren el sitio de la herida. Alternativamente, el filtro 522 puede incluir un filtro de carbón para prevenir el mal olor, un filtro hidrófobo, o cualquier material sinterizado o poroso. La punta del tubo 520 que se inserta en el material de embalaje de la herida puede estar equipado con una punta de perforación 524 para permitir la inserción fácil en el material de embalaje de la herida.
Con referencia a las FIGS. 6A y 6B, en aún otra realización, se muestra un puerto de herida 600. Como se muestra en la FIG. 6A, el puerto de herida 600 tiene el tubo 610 en lugares separados alrededor de una circunferencia del puerto de herida 600. Cada tubo puede incluir una punta perforada o un filtro como se ha descrito anteriormente. Como se muestra en la FIG. 6B, la distancia "a" entre el tubo 610 y el tubo 612 puede ser una distancia y la distancia “b” entre el tubo 610 y 614 puede ser una distancia diferente de la distancia "a". Por otro lado, la diferencia entre cada tubo puede ser similar como en la distancia "c" entre el tubo 620 y 622 y el tubo 620 y 624. Aunque las FIG. 6A y 6B muestran un número determinado de tubos, cualquier número de tubos puede estar dispuesto fuera de una circunferencia del puerto de herida 600.
Con referencia a las FIGS. 7A y 7B, en aún otra realización, se muestra un puerto de herida 700 que tiene un tubo 710 que es similar a los tubos descritos anteriormente. El tubo 710 puede ser ligeramente más grande en diámetro para permitir que los fluidos entren en el sitio de la herida. Los fluidos pueden incluir una solución para limpiar la herida tal como solución salina o puede ser un anestésico para anestesiar la zona de la herida. El tubo 710 puede estar equipado con la válvula 712 para abrir y cerrar la vía en el lugar de la herida. Adicionalmente, el extremo del tubo 710 puede estar equipado con un conector tipo luer 714 para crear una conexión estanca a los fluidos con un tubo adicional, jeringas, o cualquiera de otros conductos. Alternativamente, en lugar de una válvula, podría ser utilizado un tapón (no mostrado) para cerrar el conector tipo luer. Con referencia a la FIG. 7B, una aguja hipodérmica 716 puede insertarse en el tubo 710. La aguja hipodérmica 716 se podría utilizar para suministrar una solución a un área específica de la herida o podría ser utilizado para obtener una muestra de sangre, exudado o tejido de la zona de la herida.
Con referencia a las FIGS. 8A y 8B, en aún otra realización, se muestra un puerto de herida 800. En lugar de utilizar tubos como se describe anteriormente para permitir una fuga controlada o fija, se puede proporcionar una serie de pequeños orificios dispuestos en una circunferencia alrededor del puerto de herida 800. Los orificios pueden tomar la forma de una simple punción 810 de un tamaño dado como se muestra en la FIG. 8A. Como alternativa, los orificios 822 pueden estar formados en una placa 820 que está soldada por radiofrecuencia (RF) al puerto de herida 800.
Con referencia a la FIG. 9, en aún otra realización, se muestra una sección transversal del puerto de herida 900. El puerto de herida 900 está operativamente conectado al apósito para herida 910 e incluye un reborde 912. El reborde 912 puede tener una forma circular o cualquier forma poligonal. Un cuerpo 914 está conectado al reborde 912 que está fluidamente conectado al conducto 916. El conducto 916 lleva directamente o indirectamente al recipiente de recolección. El cuerpo 914 tiene como orificio pequeño 920 utilizado para proporcionar una fuga controlada en el sitio de la herida. El diámetro del orificio 920 y la diferencia de presión entre el exterior del puerto de herida 900 y el interior del puerto de herida 90 generan una fuga de aire controlable en el puerto de herida 900 a través del orificio. El orificio pequeño 920 se puede crear de varias maneras. El orificio 920 puede ser integral con el diseño de puerto, tal como un moldeado en el dispositivo. Puede ser creado mediante micro-perforación posterior al moldeo en el puerto mediante una aguja o una jeringa. Como alternativa, el ensamblaje o la inserción de un pequeño tubo que permite la comunicación de aire desde el exterior del puerto de herida 900 al interior del puerto de herida 900 se pueden utilizar para crear el orificio 920.
Con referencia a las FIGS. 10A y 10B, en aún otra realización, se muestra un puerto de herida 1000. El puerto de herida 1000 está operativamente conectado al apósito para herida 1010 e incluye un reborde 1012. El reborde 1012 puede tener una forma circular o cualquier forma poligonal.. Un cuerpo 1014 está conectado al reborde 1012 que está fluidamente conectado al conducto 1016. El conducto 1016 lleva directamente o indirectamente al recipiente de recolección. El conducto 1016 incluye un lumen principal 1110 utilizado para proporcionar una vía para que el exudado entre la herida "w" y el recipiente de recolección. Un lumen secundario o lumen de ventilación se proporciona en el conducto 1016 para proporcionar una fuga controlada en el sitio de la herida. El exudado entra al
lumen 1110 en el área 1114 y el aire sale del lumen 1112 en el área 1116. El lumen secundario 1112 se expone al entorno ambiental o una fuente de aire para proporcionar una fuga controlada en el puerto de herida 1000. Aunque la FIG. 10B representa los lúmenes 1110 y 1112 en un solo conducto 1016, los lúmenes 1110 y 1112 se pueden proporcionar como conductos separados.
Con referencia a la FIG. 11A, en aún otra realización, se muestra un apósito para herida 1200 que tiene un puerto de herida 1210. El apósito para herida 1200 y puerto de herida 1210 son similares al apósito para herida 12, 112, y 212 y puerto de herida 1210 son similares al puerto de herida 30, 130 y 230 que se describen más arriba. Un conducto de ventilación 1220 puede insertarse en la parte superior del puerto de herida 1210 para proporcionar una fuente de aire filtrado al apósito para herida 1200 a través del conducto de ventilación 1220. El conducto de ventilación 1220 puede ser una aguja de acero inoxidable con un lumen que se extiende a través de la aguja. El final del conducto de ventilación 1220 tiene filtro 1225 para filtrar el aire de la atmósfera ambiente. El aire filtrado de del flujo inferior que fluye de la atmósfera ambiente a través del conducto de ventilación 1220, en combinación con el drenaje de flujo superior que se produce a través de un conducto de exudado, crea una acción de sumidero entre la herida y un recipiente de recolección. Esta acción de sumidero asegura un flujo continuo a través del conducto de exudado 36, evitando de este modo el estancamiento de líquidos y sus complicaciones. Como se debatió anteriormente, la diferencia de tamaño entre el conducto de exudado y conducto de ventilación 1220 resulta en la acción capilar que hace que el fluido fluya solamente a través del mayor conducto de exudado. Las figuras 11B-11D representan un puerto de herida 1210similar al puerto de herida en la FIG. 11A. En las figuras 11B-11D, el conducto de ventilación 1220 se coloca en el lado del puerto de herida 1210 en lugar de la parte superior del puerto de herida 1210 como se muestra en la FIG. 11A. La FIG. 11D representa el final del conducto de ventilación 1220 que está situado en el puerto de herida 1210 por encima de un orificio de exudado 1230.
Aunque la descripción anterior se ha descrito con cierto detalle a modo de ilustración y ejemplo, para fines de claridad o comprensión, será obvio que ciertos cambios y modificaciones pueden ser practicados dentro del alcance de las reivindicaciones adjuntas. Por ejemplo, el líquido y conductos de ventilación puede estar sustituidos por un conducto con un doble lumen individual. Para asegurar la acción capilar, un lumen debe ser mayor que el otro; sin embargo, los lúmenes pueden ser coaxiales o paralelos.
Claims (2)
1. Un sistema para terapia de presión subatmosférica en conexión con la curación de una herida, que comprende:
una cubierta con apósito para herida (12,112,212,312) dimensionada para su posicionamiento con respecto a un lecho de herida de un sujeto para establecer un depósito (14,114,314) sobre el lecho de herida en el que puede mantenerse presión subatmosférica;
un mecanismo de presión subatmosférica (10) que incluye:
una carcasa;
una fuente de vacío (40) dispuesta en la carcasa; y
un recipiente de recolección (38) en comunicación fluida con la fuente de vacío (40);
un puerto de herida (800) operativamente conectado al apósito para herida (12,112,212,312) en comunicación fluida con el depósito (14,114,314), caracterizado porque el puerto de herida (800) incluye: un puerto de vacío; y
una pluralidad de orificios dispuestos en forma circunferencial alrededor del puerto de herida, donde la pluralidad de orificios (822) es operable para permitir aire ambiental en el depósito (14,114,314) en donde dicha pluralidad de orificios se forma en una placa (820); y en el que el sistema además comprende un conducto de exudado en comunicación fluida con el puerto de herida (800) y el recipiente de recolección (38) para recolectar exudado removido del depósito (14,114,314) y depositado en el recipiente de recolección (38) bajo la influencia de la fuente de vacío (40).
2. El sistema de acuerdo a la reivindicación 1, en donde la placa (820) es soldada por radiofrecuencia al puerto de herida (800)..
