Foreliggende oppfinnelse vedrører enzymatisk spaltbare promedikamenter med redusert Michaelis-Menten konstant (Km), og farmasøytiske preparater inneholdende disse.
Et promedikament kan defineres som en kjemisk forbindelse som er ikke-toksisk og farmakodynamisk inert, men som kan overføres in vivo til et farmakologisk aktivt legemiddel.
Oppfinnelsen vedrører feltet legemiddel-målretting, som vedrører posisjons-spesifikk avlevering av legemidler in vivo. Posisjons-spesifikk avlevering øker fordelaktig selektiviteten av legemidler og reduserer deres uønskede bivirkninger.
En potensiell fremgangsmåte for å oppnå en posisjons-spesifikk avlevering består i anvendelse av utoksiske promedikamenter som kan aktiveres posisjons-spesifikt til cytotoksiske legemidler ved anvendelse av prelokaliserte promedikamentspaltningskatalysatorer som enzymer, muteiner avledet fra enzymer, katalytiske antistoffer, antistoffenzymkonjugater eller fusjonsproteiner.
Denne fremgangsmåten kombinerer fordelen med legemiddel-avgivelse via promedikamenter (dvs. forøket stabilitet, regulert oppløselighet, forbedret administreringsmåte, mer fordelaktig fordeling, forbedrede farmakokinetiske forhold, by-passing motstand; T. A. Connors, Xenobiotica 16, 975-988, 1986) med den foretrukne tumorspesifikke aktiveringen formidlet ved hjelp av et katalytisk prinsipp. Anvendelsen av eksogene enzymer eller polyklonale antistoffenzymkonjugater for promedikamentaktivering ble først beskrevet av Graffi (tysk utlegningsskrift 22 12 014) og Philpott et al. (J. Immunol. 111, 921, 1973).
I den senere tid ble Graffi og Philpotts opprinnelige lære eksemplifisert og forbedret ved anvendelsen av monoklonale antistoffenzymkonjugater som promedikamentaktiverende katalysatorer (Bagshawe et al., Brit. J. Cancer, 58, 700,
1988; Senter et al., Bioconjugate Chem. 4, 3-9, 1993) eller fusjonsproteiner (Bosslet et al., Brit. J. Cancer, 65, 234-238, 1992; Goshorn et al., Cancer Res. 53, 2123-2127, 1993).
På tross av disse forbedringene har systemene som hittil er beskrevet visse hovedulemper for kliniske anvendelser: a) monoklonale antistoffenzymkonjugater produsert ved kjemisk kobling har som en hovedulempe en sterk immunogenisitet i mennesket på grunn av det xenogeniske opphavet av antistoffenheten og enzymet (Bagshawe et al., Disease Markers 9: 233-238, 1991). Som en følge av denne høye immunogenisiteten er gjentatte anvendelser i mennesket mulig bare i meget begrenset omfang;
b) fusjonsproteiner bestående av ikke-humaniserte bindende enheter og xenogeniske enzymer produsert ved
rekombinant DNA teknologi vil også være immunogeniske 1 mennesket med ulemper sammenlignbare med monoklonale antistoffenzymkonjugater dersom gjentatte anvendelser er påkrevet;
c) fusjonsproteiner bestående av humaniserte bindende enheter og humane enzymer vil sannsynligvis ikke være
svært immunogeniske i mennesket, og sannsynligvis tillate gjentatte behandlingssykler i mennesket. Imidlertid er to hovedulemper ved humane fusjonsproteiner den sannsynligvis lavere turnover-raten (Vmax) av den humane enzymenheten såvel som den sannsynligvis høyere promedikament (substrat) konsentrasjonen som er påkrevet for å oppnå signifi-kant katalyse sammenlignet med xenogeniske enzymer som har en høy turnover-rate og en lav Michaelis-Menten konstant (Km).
Denne begrensningen ved humane fusjonsproteiner (lav Vmax og høy Km) som er gitt ved den intrinsiske naturen av den humane enzymenheten kan overvinnes ved metodologi ifølge teknikkens stand bare i meget begrenset omfang (faktor 4) ved tilfeldig mutagenese i den aktive posisjonen av enzymet (Munir et al., PNAS USA 90:4012-4016, 1993).
Overraskende er det funnet at begrensningen ved en høy Km, en intrinsisk egenskap for de fleste humane enzymer som er anvendelige ved in vivo promedikamentaktivering, kan overvinnes ved hjelp av nye promedikamenter.
Ved foreliggende oppfinnelse tilveiebringes følgelig forbindelser som er kjennetegnet ved formelen
hvor R er CH0H-C00H, CH2-C00H eller COOH.
Promedikamentene ifølge oppfinnelsen har den felles egenskap at de spaltes ved hjelp av enzymer ved betydelig lavere molar promedikamentkonsentrasjon enn de naturlige eller standard substratene som anvendes for enzymatisk analyse eller tilsvarende promedikamenter ifølge teknikkens stand (WO 92/19639). De betegnes derfor Km-reduserte promedikamenter. Foreliggende oppfinnelse tilveiebringer videre et farma-søytisk preparat, som er kjennetegnet ved at det omfatter en forbindelse som omtalt ovenfor og en farmasøytisk akseptabel bærer.
Ikke innbefattet er p<->D-glukuronid-Z-antracyklinforbindelser:
Enzym i foreliggende søknad kan også bety et katalytisk antistoff. Forbindelsene som her er beskrevet kan fremstilles ved tic.ligere kjente fremgangsmåter.
Km-reduserte promedikamenter, selektive for human p-glukuro-nidasej er beskrevet i de følgende avsnittene. Promedikament A (eksempel 1) kan betraktes som avledet fra det kompetitive p<->gluktronidaseinhibitorsakkarolaktonet:

Eksperimentell fremgangsmåte for promedikament A:
Fremstilling av 1,2,5-tri-0-acetyl-aldehydo-D-glukurono-3,6-1akton (forbindelse 7).
3,6-glukarolakton (forbindelse 5) (45 g) ble langsomt tilsatt til en avkjølt (0-5°C) blanding av tørt pyridin (225 ml) og AC2O (185 ml). Den indre temperaturen ble holdt ved 5°C under
tilsatsen og etter at alt laktonet var oppløst ble reaksjonsblandingen tillatt å omrøres i ytterligere 2 timer. Den fargeløse oppløsningen ble deretter helt i 3 liter av en blanding av vann og knust is og omrørt kraftig i ca. 3 timer. Utfellingen ble samlet og vasket med vann, og etter tørking ble det isolert et fast stoff som inneholdt 70 g av en blanding av a og p tri-O-acetyl-glukuronolakton (forbindelse 7). Denne blandingen ble anvendt direkte i det neste trinnet.
Fremstilling av 2,5-di-0-acetyl-oc-D-glukurono-3,6-lakton-cx-furanosyl bromid (forbindelse 8).
Titanbromid (16,6 g, 45 mmol) ble tilsatt til en omrørt oppløsning av forbindelse 7 (70 g, 23,3 mmol) i diklormetan (200 ml) holdt i mørke og under nitrogenatmosfære. Etter omrøring over natten ble ytterligere TiBr4 tilsatt (8,3 g, 22 mmol). Etter ytterligere 24 timer ble reaksjonsblandingen fortynnet med diklormetan (150 ml) og den organiske oppløs-ningen ble helt i knust is-vann. Det organiske laget ble separert, vasket med vann, tørket og inndampet under redusert trykk. Dette ga forbindelse 8 (65 g) ren nok for det neste trinnet.
Fremstilling av (2-nitro-4-formylfenyl )-2,5-di-0-acetyl-p-D-glukurono-3,6-lakton furanosid (forbindelse 9).
Det ble fremstilt fra forbindelse 8 (15 g, 50 mmol) og fra 4-hydroksy-3-nitrobenzaldehyd ifølge fremgangsmåten som allerede er beskrevet i WO 92/19639. Dette ga 12 g (61,6 %) av forbindelse 9.
Fremstilling av (2-nitro-4-formylfenyl )-2,3,5-tri-0-acetyl-3-D-glukuronat (forbindelse la).
Til en oppløsning av fast natriumhydroksid (50 mg) i metanol (125 ml), ble forbindelse 9 (10 g) tilsatt. Oppløsningen ble omrørt ved romtemperatur i 4 timer og Inndampet under redusert trykk. Dette resulterte i en rå blanding som straks ble oppløst i vannfritt pyridin (50 ml). Etter avkjøling til 0°C ble eddiksyreanhydrid (40 ml) tilsatt og reaksjonsblandingen ble deretter omrørt i ytterligere 18 t. Ekstraksjon med diklormetan etterfulgt av vanlig opparbeidelse resulterte i 6,6 g av forbindelse la (65 % samlet utbytte).
Fremstilling av (2-nitro-4-hydroksymetylfenyl)-2,3,5-tri-0-acetyl-p<->D-glukuronat (forbindelse 2a).
Den ble fremstilt ved natriumborhydridreduksjon av forbindelse la (6 g) ifølge fremgangsmåten som allerede er beskrevet i WO 92/19639. Dette ga 5,6 g (95 %) av forbindelse 2a.
Fremstilling av 4-(2,3,5-tri-0-acetyl-p-D-metylglukurono-furanosyl )-3-nitro-p-nitrobenzyloksykarbonat (forbindelse 3a).
Den ble fremstilt ved kobling av forbindelse 2a (6 g) med 4-nitrofenylkloroformiat (utbytte 75 %) ifølge fremgangsmåten som allerede er beskrevet i WO 92/19639.
Fremstilling av promedikament Å:
Promedikament A ble fremstilt fra forbindelse 3a og doksorubicin (utbytte 83 <$ >) etterfulgt av behandling med natriummetoksid i metanol og deretter natriumhydroksid.
Eksempel 2;
Fremstilling av 4-metylumbelliferyl (5R)-5-fosfonyl-p<->D-xylopyranosid (16):
Allyl 6-C-trityl-a-D-glukopyranosid (2). En oppløsning av allyl cx-I-glukopyranosid (1) (fremstilt ifølge R.E. Wing, J.N. BeMiller, Carbohydr. Res., 1969, 10, 441) (12,5 g, 56,8 mmol) og trifenylmetylklorid (20,0 g, 71,7 mmol) I tørr pyridin (120 ml) ble omrørt ved romtemperatur i 12 timer og ved 60°C i 1 time. Etter tilsetningen av trifenylmetyl-kloridet (12,0 g, 43,0 mmol) ble oppløsningen omrørt ved 60°C inntil alt utgangsmateriale hadde forsvunnet (3-4 timer). H2O (120 ml) ble tilsatt til den fremdeles varme oppløsningen. Ekstraksjon med EtOAc, ekstraksjon av de kombinerte organiske lagene med 1 M aq. E2SO4 og saltvannsoppløsning, fordampning av det organiske laget og FC (400 g Si02» toluen/aceton 2:1 -t toluen/aceton 1:1) ga 23,3 g (90 <t>) av 2. Grått glassaktig faststoff.
Allyl 2,3,4-tri-0-benzyl-6-0-trityl-cx-D-glukopyranosid (3). En oppløsning av 2 (15,9 g, 34,3 mmol) i tørr THF (390 ml) ble behandlet med en suspensjon av NaH (6,9 g, ca. 150 mmol) ved romtemperatur i 10 min. BnBr (25,0 ml, 211 mmol) og BU4NI (1,9 g, 5,1 mmol) ble tilsatt. Oppløsningen ble oppvarmet under tilbakeløp inntil TLC indikerte avslutning av reaksjonen (ca. 12 timer). Et20 ble tilsatt, og oppløsningen ble filtrert gjennom silisiumoksid. Filtratet ble fordampet og resten ble underkastet FC (600 g Si02, Et20/heksan 1:9 -* Et20) for å gi 21,4 g (85 %) av 3. Allyl 2 ,3 ,4-tri-O-benzyl-cx-D-glukopyranosid (4). En oppløs-ning av BF3-0Et2 (5,0 ml, 39,8 mmol) i MeCN (90 ml) ble tilsatt dråpevis til en avkjølt (0°C) oppløsning av 3 (13,4 g, 18,3 mmol) og Et3SiH (14,5 ml, 91,5 mmol) i tørr CH2C12 (150 ml). Etter 10 minutter ble en mettet vandig oppløsning av NaHC03 (100 ml) og H20 (200 ml) tilsatt. Blandingen ble ristet kraftig, det vandige laget ble ekstrahert med CE2C12, de kombinerte organiske lagene ble ekstrahert med saltvanns-oppløsning, tørket (MgSO^ og inndampet. FC (400 g Si02, EtOAc/heksan 1: 5 -+ EtOAc/hek san 1:1) ga 8,35 g (93 %) av 4. Rf (EtOAc/heksan 1:2) 0,20.
Allyl 2 ,3,4-tri-O-benzyl-cx-D-glukopyranuronid tert.-butyl-ester (5). En oppløsning av 4 (8,35 g, 17,0 mmol) i DMF/CH2C12 4:1 (45 ml) ble tilsatt til en oppløsning av Cr03 (6,8 g, 6,8 mmol) i DMF/CH2C12 4:1 (180 ml) og pyridin (11,0 ml, 142 mmol) som var omrørt kraftig 1 30 minutter ved romtemperatur. Etter tilsetning av Ac20 (13,0 ml, 11,8 mmol)
og tert.-BuOH (34,0 ml, 362 mmol) ble oppløsningen omrørt i 9 timer ved romtemperatur før MeOH (30 ml) ble tilsatt. Etter 30 minutter ble blandingen konsentrert til en fjerdedel av
volumet og fortynnet med Et20 (250 ml). Filtrering gjennom Na2SC>4 og Si02 (300 g), eluering med Et20, inndampning og FC (330 g Si02, AcOEt/heksan 1:9) ga 6,30 g (66 %) av 5. Rf (EtOAc/heksan 1:2) 0,63. <13>C-NMR (50 MHz, CDC13): 27,94 (q, trippel intensitet); 68,69 (t); 71,44 (d); 73,43 (t);

Allyl ( 5R )-5-acetoksy-2 ,3 ,4-tri-O-benzyl-oc-D-xylopyranosid (7) . En omrørt oppløsning av 6 (5,60 g, 11,1 mmol) i C^H^ (50 ml) og pyridin (5 ml) ble behandlet med Pb(0Ac)4 (16,80 g, ca. 32 ramol) under N2 ved 60"C i 25 minutter. Filtrering gjennom Si02, eluering med Et20, fordampning og FC (300 g Si02, AcOEt/heksan 1:6) ga 4,1 g (71 %) av 7. Rf (EtOAc/heksan 1:4) 0,29. Allyl (5S )-5-hydroksy-2 ,3,4-tri-O-benzyl-cx-D-xylopyranosid (8) . Ved -78°C ble DIBAH (2,8 ml av en 20 % oppløsning i toluen, ca. 2,9 mmol) dråpevis tilsatt til en oppløsning av 7 (553 mg, 0,96 mmol) i CH2C12 (20 ml). Etter 15 minutter ble en mettet oppløsning av NH4CI (2 ml) tilsatt. Blandingen ble oppvarmet til romtemperatur, fortynnet med H20 og en 1 M oppløsning av H2S04 (10 ml). Det vandige laget ble ekstrahert med CH2C12 (3 x), de kombinerte organiske lagene ble ekstrahert med saltvannsoppløsning (2 x), tørket (MgS04) og inndampet for å gi 499 mg (98 <&) av krystallinsk 8 som ble anvendt uten ytterligere rensing i det neste trinnet. Allyl ( 5S)-5-trikloracetimidyloksy-2 ,3 ,4-tri-O-benzyl-cx-D-xylopyranosid (9). MTBD (66 pl, 0,46 mmol) ble tilsatt til en avkjølt (-30°C) oppløsning av rå 8 (200 mg, ca. 0,42 mmol) og C13CCN (0,63 ml, 6,3 mmol) i tørr C1CH2CH2C1 (6 ml). Etter 10 minutter ble oppløsningen filtrert gjennom Si02, SI02 ble eluert med Et20, og de kombinerte filtratene ble fordampet for å gi rå 9 som var tilstrekkelig rent (^-H-NMR, TLC) til å anvendes i det neste trinnet. Rf (EtOAc/heksan 1:2) 0,53.
Allyl (5R)-5-dimetylfosfonyl-2,3,4-tri-O-benzyl-a-D-xylopyranosid (10) og allyl (5S)-5-dimetylfosfonyl-2,3,4-tri-O-benzyl-cx-D-xylopyranosid (11). TMSOTf (83 pl, 0,46 mmol) ble tilsatt til en avkjølt (-17°C) oppløsning av rå 9 (350 mg) og P(0Me)3 (240 pl, 1,26 mmol) i tørt MeCN (6 ml). Oppløsningen ble oppvarmet til 0°C, holdt ved denne temperaturen i 3 timer og filtrert gjennom Si02. Si02 ble eluert med Et20, og de kombinerte filtratene ble inndampet. Resten (396 mg) ble utsatt for FC (22 g Si02, EtOAc/heksan 1:1) for å gi en blanding av 10 og dets (5S) isomer 11 (147 mg, 62 $ fra 7). Denne blandingen ble ytterligere renset ved hjelp av HPLC (EtOAc/heksan 2:1) for å gi 49 mg (21 $ > fra 7) av 10 og 52 mg (22 1o fra 7) av 11. Prop-l-enyl (5R )-5-dimetylfosf onyl-2 ,3 ,4-tri-O-benzyl-cx-D-xylopyranosid (12). En oppløsning av aktivert 1,5-cyklo-oktadien-bis[metyldifenylfosfin]-iridium heksafluorfosfat (15 mg) i tørr THF (5 ml) ble tilsatt til en oppløsning av 10 (257 mg; 0,452 mmol) i tørr THF (10 ml). Etter 2 timer indikerte TLC at reaksjonen var fullført og oppløsningen ble inndampet for å gi 57 mg av rå 12 som ble anvendt uten rensing I det neste trinnet. Rf (EtOAc(heksan 3:1) 0,39.
(5R)-5-dimetylfosfonyl-2 ,3,4-tri-0-benzyl-a-D-xylopyranose (13). En omrørt oppløsning av rå 12 (57 mg) og gul HgO (118 mg; 0,54 mmol) i H20/aceton 1:10 (10 ml) ble behandlet med en oppløsning! av HgCl2 (148 mg; 0,55 mmol) i H20/aceton 1:10 (5 ml). Etter at reaksjonen var fullført, ble Et20 tilsatt. Et20 laget ble vasket med en halvmettet oppløsning av Kl og med saltvannsoppløsning. S102 (2 g) ble tilsatt, blandingen ble inndampet og underkastet FC (15 g SI02, EtOAc/heksan 3:1 -► EtOAc/heksan 5:1) for å gi 216 mg (90 £ fra 10) av 13. Rf (EtOAc/heksan 3:1) 0,16.
0-[(5R)-5-dimetylfosfonyl-2,3,4-tri-O-benzyl-a-D-xylopyranosid]-trikloracetimidat (14). Ved -30°C ble MTBD (1,1 ekvivalent) tilsatt til en oppløsning (0,05 M) av 13 og CI3CCN (15 ekvivalenter) i tørr CH2C12. Etter at reaksjonen var fullfcirt, ble oppløsningen filtrert gjennom Si02, Si02 ble eluert med Et20, og de kombinerte eluatene ble inndampet
for å gi rå 14 som ble anvendt uten rensing i det neste trinnet.
4-metylumbelliferyl (5R)-5-dimetylfosfonyl-2,3,4-tri-O-benzyl-p<->D-xylopyranosid (15). 4-metylumbelliferyl (5R)-5-fosfonyl-p<->D-xylopyranosid (17). En oppløsning av 16 i CH2C12 (0,05 M) ble behandlet under N2 ved 0°C med Me3SiBr (30 ekvivalenter) [CE. McKenna, Tetrahedron Lettr. 1977, 155]. Etter at reaksjonen var fullført, ble MeOH tilsatt, blandingen ble konsentrert i.v., resten ble opptatt i H20, og blandingen ble lyofilisert. Rensing av resten ved hjelp av anionbytterkromatografi (Dowex 1x8 (HC00~): 0-0,7 M HCOOH) [K. Wallimann, Heiv. Chim. Acta 1990, 73, 1359] ga 17 som straks ble transformert til dens Na-salt ved anionbytterkromatografi (Dowex 50 W x 4 (Na<+>)).

Sammenligning av Km- og Vmax- verdier for naturlig og forbedret substrat for antistoff g- glukuronidase fusjonsprotein
For Km- og Vmax-bestemmelse bør 3'-N-[4-(beta-D-glukuronyloksy)-3-nitro-benzyloksykarbonyl]-doksorubicin og promedikament A fortynnes i området på 10-10.000 pM i 100 mM fosfatbuffer + 1 mg/ml BSA, pH 7,2. Enzymatisk spaltning bør utføres med konstante mengder av fusjonsprotein ved 37°C. Spaltning kan overvåkes ved hjelp av HPLC analyse. Km- og Vmax-verdier kan beregnes med datamaskinprogrammet Grafit 2.0 (Erithacus Software Ltd.).
HPLC analyse:
HPLC apparaturen besto av en "autosampler" (Abimed, modell 231), et automatisk prøveekstraksjonssystem (AASP, Varlan) utstyrt med minipatroner inneholdende C 18 reversert fase silikagel (Analytichem), en gradientpumpe (Gynkotek, modell 480), en fluorescensdetektor (Shimazdu RF 535, eksitasjon: 495 nm, emisjon: 560 nm). Før prøveinjeksjon ble minipatronene forkondisjonert med 2,5 ml metanol og 1,5 ml fosfatbuffer, pH 6. Analytter som ble holdt tilbake på silikagelen med reversert fase ble deretter eluert ved hjelp av ventIIomkobl ing og forbindelse til minipatronene til den mobile fasen. Kromatografi ble utført på reversert fase materiale ("Nucleosil C 18", 5 pm partikkelstørrelse, 120 mm lengde, 4,5 mm I.D.) og gradienteluering. Eluering ble utført ved hjelp av en gradient bestående av 2 komponenter (A: 20 mM fosfat, pH 3, B: acetonitril). Gradienten ble kjørt med følgende tid-konsentrasjonsprofil:
Før det neste forsøket ble startet, ble kolonnen tillatt å ekvilibrere ved utgangsbetingelser i 5 minutter.
Eksempel 4:
Promedikament A kan innkapsles i henhold til D. Papahadjo-poulos et al. (PNAS, USA 88: 11460-11464, 1991) i skjulte liposomer. Etter i.v. injeksjon i CD1 nu/nu mus skulle plasma clearance av promedikament A innkapslet i skjulte liposomer være forlenget fra 2 20 minutter for det frie promedikament A til £ 40 timer for det innkapslede promedikament A. Den betydelige tl/2<p> forlengelsen fører til forbedret farmakologisk virkningsfullhet.
