Den foreliggende oppfinnelsen er relatert til en kombinasjon av 2 S. pneumoniae proteiner, deres fremstilling og anvendelse i medisin som en vaksine. Slike kombinasjoner er spesielt nyttig for beskyttelsen av spedbarn og eldre mot streptokokkinfeksjon.
BAKGRUNN FOR OPPFINNELSEN
Streptococcus pneumoniae er en Gram-positiv bakterie ansvarlig for anselig sykelighet og dødelighet (spesielt hos yngre og eldre), som forårsaker invasive sykdommer slik som pneumoni, bakteremi og meningitt og sykdommer assosiert med kolonisering slik som akutt Otitis media. Mengden av pneumokokkpneumoni i USA for personer over 60 år er estimert til å være 3 til 8 pr 100.000.120 % av tilfeller fører dette til bakteremi og andre utslag slik som meningitt, med en dødelighetsrate nærmere 30 % selv med antibiotikabehandling.
Pneumokokker er innkapslet med et kjemisk forbundet polysakkarid som konfererer serotypespesifisitet. Det er 90 kjente serotyper av pneumokokker og kapselen er den hovedsakelige virulensterminanten for pneumokokker ettersom kapselen ikke bare beskytter den indre overflaten til bakteriene fra komplement, men er i seg selv dårlig immunogen. Polysakkarider er T-uavhengige antigener, og kan ikke bli prosessert eller presentert på MHC-molekyler for å interagere med T-celler. De kan imidlertid stimulere immunsystemet gjennom en alternativ mekanisme som involverer kryssbinding av overflatereseptorer på B-celler.
Det ble vist i flere eksperimenter at beskyttelse mot invasiv pneumokokksykdom er sterkest korrolert med antistoff spesifikt for kapselen, og beskyttelsen er serotypspesifikk.
Streptococcus pneumoniae er den mest alminnelige årsaken til invasiv bakteriell sykdom og Otitis media hos spedbarn og yngre barn. På lignende måte har eldre personer dårlige responser til pneumokokkvaksiner [Roghmann et al., (1987), J. Gerontol. 42:265-270], herav den økende forekomsten av bakteriell pneumoni i denne populasjonen [Berghese and Berk, (1983) Medicine (Baltimore) 62:271-285].
WO 0037105, WO 9741151 og Ogunniyi David A. et al., Infection and Immunity, vol. 68, nr.5, 2000, s. 3028-3033 beskriver proteiner for bruk i vaksiner.
En 23-valent ukonjugert pneumokokkvaksine har vist en stor variasjon i klinisk effektivitet, fra 0 % til 81 % (Fedson et al. (1994) Arch Intern Med. 154:2531-2535). Effektiviteten viser seg å være relatert til risikogruppen som har blitt immunisert, slik som eldre, Hodgkin's sykdom, splenektomi, sigdcellesykdom og gammaglobulinemi (Fine et al. (1994) Arch Intern Med. 154:2666-2677), og også til sykdomsmani-festasjonen. Den 23-valente vaksinen demonstrerer ikke beskyttelse mot pneumokokkpneumoni (i enkelte høy-risikogrupper slik som hos eldre) og Otitis media sykdommer.
Strategier som har blitt utpekt for å overvinne denne mangelen på immunogenisitet hos spedbarn inkluderer forbindelsen av polysakkaridet til store immunogene proteiner, som tilveiebringer tilskuer T-cellehjelp og som induserer immunologisk hukommelse mot polysakkaridantigenet som det er konjugert til.
Imidlertid er det fortsatt et behov for forbedrede pneumokokkvaksinesammensetninger, spesielt de som vil være mer effektive i beskyttelsen eller lindringen av pneumokokksykdom (spesielt pneumoni) hos de eldre og hos yngre barn.
Den foreliggende oppfinnelsen tilveiebringer en slik forbedret vaksine.
SAMMENDRAG AV OPPFINNELSEN
I ett aspekt er den foreliggende oppfinnelsen en immunogen sammensetning som omfatter minst 2 S. Pneumoniae proteiner hvori et av proteinene er PhtD og et annet protein er detoksifisert pneumolysin (Ply).
I et relatert aspekt tilveiebringer den foreliggende oppfinnelsen en vaksine for å behandle eller lindre Otitis media hos spedbarn eller pneumoni hos eldre. Eventuelt omfatter vaksinene ytterligere en adjuvans som fortrinnsvis er fremkaller av en TH1-respons.
Et ytterligere aspekt er relatert til en fremgangsmåte for å lage vaksinen ifølge oppfinnelsen ved å velge og å isolere 2 ulike S. pneumoniae proteiner som er PhtD og detoksifisert pneumolysin (Ply) og blande begge proteinene med en farmasøytisk aksepterbar bærer.
BESKRIVELSE AV OPPFINNELSEN
Den foreliggende oppfinnelsen tilveiebringer en forbedret vaksine for forebyggingen eller lindringen av pneumokokkinfeksjon hos eldre (f.eks. pneumoni) og/eller hos spedbarn (f. eks. Otitis media), ved å være avhengig av en pneumokokkprotein basert metode. I en foretrukket utførelsesform er vaksinen egnet for forebyggingen eller lindringen av pneumokokkinfeksjon hos eldre. Ettersom de fleste voksne har blitt eksponert for Streptococcus pneumoniae, er den foreliggende vaksinen tilsiktet til å forsterke den underliggende immunresponsen hos voksne og de eldre til beskyttende nivåer ved administrasjon av minst to pneumokokkproteiner identifisert i den foreliggende oppfinnelsen. Pneumokokkproteinet er administrert under fraværet av S. pneumoniae polysakkaridet
I konteksten av den foreliggende oppfinnelsen er en pasient betraktet som eldre om de er 55 år eller mer, vanligvis over 60 år og mer generelt over 65 år. I en utførelsesform tilveiebringer oppfinnelsen derved for en vaksinesammensetning som omfatter pneumokokkproteiner for forebyggingen av pneumoni hos de eldre.
I en annen utførelsesform tilveiebringer den foreliggende oppfinnelsen en vaksinesammensetning, egnet for anvendelse hos spedbarn (vanligvis 0 til 2 år), som omfatter to eller flere pneumokokkproteiner identifisert i den foreliggende oppfinnelsen.
Pneumokokkproteiner ifølge oppfinnelsen
De beskrevne Streptococcus pneumoniae proteinene er enten overflateeksponert, i det minst under en del av livssyklusen til pneumokokken, eller de er proteiner som er utskilt eller frigjort av pneumokokken. Fortrinnsvis er kombinasjonen av proteiner ifølge oppfinnelsen valgt fra 2 ulike kategorier slik som proteiner som har et type II signalsekvensmotiv av LXXC (hvor X er enhver aminosyre, f.eks. polyhistidintriadefamilien (PhtX)), kolinbindingproteiner (CbpX), proteiner som har et type I signalsekvensmotiv (f.eks. SplOl), proteiner som har et LPXTG-motiv (hvor X er enhver aminosyre, f.eks. Spl28, Spl30), toksiner (f.eks. Ply), osv. Foretrukkede eksempler innenfor disse kategoriene (eller motiver) er de følgende proteinene eller immunologiske funksjonelle ekvivalenter derav.
Den immunogene sammensetningen omfatter minst 2 proteiner valgt fra gruppen som består av polyhistidintiradefamilien (PhtX), kolinbindingproteinfamilien (CbpX),
avkortede CpbX, LytX familie, avkortede LytX, CpbX avkortet-LytX avkortede kimære proteiner (eller fusjoner), pneumolysin (Ply), PspA, PsaA, Spl28, SplOl, Spl30, Spl25 og Spl33. Om CbpX er PspC så er det andre proteinet imidlertid ikke PspA eller PsaaA. Fortrinnsvis omfatter den immunogene sammensetningen 2 eller flere proteiner valgt fra gruppen som består av polyhistidintriadefamilien (PhtX), kolinbindingproteinfamilien (CbpX), avkortede CbpX, LytX-familien, avkortede LytX, CbpX avkortet-LytX avkortede kimære proteiner (eller fusjoner), pneumolysin (Ply), PspA, PsaA og Spl28. Mer foretrukket er det at den immunogene sammensetningen omfatter 2 eller flere proteiner valgt fra gruppen som består av polyhistidintriade-familien (PhtX), kolinbindingproteinfamilien (CbpX), avkortede CbpX, LytX familien, avkortede LytX, CbpX avkortet-LytX avkortede kimære proteiner (eller fusjoner), pneumolysin (Ply) og Spl28.
Pht (polyhistidintriade) familien omfatter proteiner PhtA, PhtB, PhtD og PhtE. Familien erkarakterisert veden lipideringssekvens, to domener separert av en prolin-rik region og flere histidintriader, sannsynligvis involvert i metall eller nukleosidbinding eller enzymatisk aktivitet, (3-5) coiled-coil-regioner, en konservert N-terminal ende og en heterogen C-terminal ende. Det er tilstede i alle stammer av pneumokokker testet. Homologe proteiner har også blitt funnet i andre streptokokker og neisseria. Foretrakkede medlemmer av familien omfatter PhtA, PhtB og PhtD. Mer foretrakket omfatter den PhtA eller PhtD. Det er imidlertid forstått at betegnelsene Pht A, B, D og E refererer til proteiner som har sekvenser angitt i henvisningene nedenfor, såvel som naturlig forekommende (og menneskelagede) varianter derav som har en sekvens homologi som er minst 90 % identisk til de henviste proteinene. Fortrinnsvis er det minst 95 % identisk og mest foretrukket er det 97% identisk.
Med hensyn på PhtX-proteinene er PhtA angitt i XO 98/18930, og er også referert til som Sp36. Som bemerket ovenfor er det protein fra polyhistidintriadefamilien og har type II signalmotivet av LXXC.
PhtD er angitt i WO 00/37105 og er også referert til som Sp036D. Som notert ovenfor er det også et protein fra polyhistidintriadefamilien og har type II LXXC signalmotivet. PhtB er angitt i WO 00/37105, og er også referert til som Sp036B. Et annet medlem av PhtB-familien er det C3-degraderende polypeptidet, som angitt i WO 00/17370. Dette proteinet er også fra polyhistidintriadefamilien og har type II LXXC signalmotivet. I en foretrukket immunologisk funksjonell ekvivalent er proteinet Sp42 angitt i WO 98/18930. Et avkortet PhtB (omtrentlig 79kD) er angitt i WO 99/15675 som også er betraktet som et medlem av PhtX-familien.
PhtE er angitt i WO 00/30299 og er referert til som BVH-3.
Angående kolinbindingproteinfamilien (CbpX) ble medlemmene av den familien opprinnelig identifisert som pneumokokkproteiner som kunne bli renset ved kolin-affinitetskromatografi. Alle av kolinbindingproteinene er ikke-kovalent bundet til fosforylkolinhalvdeler av cellevegg-teikoinsyre og membran-assosiert lipoteikoinsyre. Strukturelt har de flere regioner alminnelige for hele familien selv om den eksakte naturen av proteinene (aminosyresekvens, lengde, osv) kan variere. Generelt omfatter kolinbindingproteiner en N-terminal region (N), konserverte repeterte regioner (RI og/eller R2), en prolinrik region (P) og konservert kolinbindingregion (C), laget av multiple repetisjoner som omfatter omtrentlig en halvdel av proteinet. Som benyttet i denne søknaden er betegnelsen "kolinbindingproteinfamilie (CbpX)" valgt fra gruppen som består av kolinbindingproteiner som identifisert i WO 97/41151, PbcA, SpsA, PspC, CbpA, CbpD og CbpG. CbpA er angitt i WO 97/41151. CbpD og CbpG er angitt i WO 00/29434. PspC er angitt i WO 97/09994. PbcA er angitt i WO 98/21337. SpsA er et kolinbindingprotein angitt i WO 98/39450. Fortrinnsvis er kolinbindingproteinene valgt fra gruppen som består av CbpA, PbcA, SpsA og PspC.
En annen foretrukket utførelsesform er avkortede CbpX hvor "CbpX" er definert ovenfor og "avkortede" refererer til CbpX-proteiner som mangler 50 % eller mer av kolinbindingregionen (C). Fortrinnsvis mangler slike proteiner hele kolinbindingregionen. Mer foretrukket mangler slike avkortede proteiner (i) kolinbindingregionen og (ii) også en del av den N-terminale halvdelen av proteinet, likevel beholde minst en repetisjonsregion (RI elelr R2). Mer foretrukket har det avkortede proteinet 2 repetisjonsregioner (RI og R2) eksempler på slike foretrukkede utførelsesformer er NRlxR2 og RlxR2 som illustrert i WO 99/51266 eller WO 99/51188, imidlertid er også andre kolinbindingproteiner som mangler en lignende kolinbindingregion også betraktet.
LytX-familien er membran-assosierte proteiner assosiert med cellelysering. Det N-terminale domenet omfatter kolinbindingdomene(r), imidlertid har ikke LytX-familien alle kjennetegnene funnet i CbpA-familien notert ovenfor og derved for den foreliggende oppfinnelsen er LytX-familien betraktet som forskjellig fra CbpX-familien. I kontrast til CbpX-familien inneholder det C-terminale domenet det katalytiske domenet til LytX-proteinfamilien. Familien omfatter LytA, B og C. Med hensyn på LytX-familien er LytA angitt i Ronda et al., Eur J Biochem, 164:621-624 (1987). LytB er angitt i WO 98/18930, og er også referert til som Sp46. LytC er også angitt i WO 98/18930, og er også referert til som Sp91. Et foretrukket medlem av den familien er LytC.
En annen foretrukket utførelsesform er avkortede LytX hvor "LytX" er definert ovenfor og "avkortede" referer til LytX-proteiner som mangler 50 % eller mer av kolinbindingregionen. Fortrinnsvis mangler slike proteiner hele kolinbindingregionen. Et eksempel på slike avkortede proteiner kan bli funnet i eksempeldelen til denne oppfinnelsen.
En ytterligere foretrukket utførelsesform ifølge oppfinnelsen er CbpX avkortet-LytX avkortede kimære proteiner (eller fusjoner). Fortrinnsvis omfatter dette NRlxR2 (eller RlxR2) av CbpX og den C-terminale delen (Cterm, dvs som mangler kolinbindingdomenene) av LytX (f.eks. LytCCterm eller Sp91 Cterm). Mer foretrukket er CbpX valgt fra gruppen som består av CbpA, PbcA, SpsA og PspC. Mer foretrukket er det CbpA. Fortrinnsvis er LytX LytC (også referert til som Sp91).
En annen utførelsesform ifølge oppfinnelsen er et avkortet PspA eller PsaA som mangler kolinbindingdomenet (C) og uttrykt som et fusjonsprotein med LytX. Fortrinnsvis er LytX LytC.
Pneumolysin er et multifunksjonelt toksin med en distinkt cytolytisk (hemolytisk) og komplementaktiveringsaktiviteter (Rubins et al., Am. Respi.Cit Care Med, 153:1339-1346 (1996)). Toksinet er ikke utskilt av pneumokokki, men det er frigjort ved ly sering av pneumokokker under influensen av autolysin. Dets effekter inkluderer f.eks. stimuleringen av produksjonen av inflammatoriske cytokiner ved human monocytter, inhiberingen av bevegelsen av flimmerhår på humant respiratorisk epitel, og nedgangen av bakterisidal aktivitet og migrering av neutrofiler. Det mest tydelige effekten av pneumolysin er i lyseringen av røde blodceller, som involverer binding til kolesterol. Fordi det er et toksin trenger det å bli detoksifisert (dvs ikke-toksisk for et menneske når tilveiebrakt ved en dosering egnet for beskyttelse) før det kan bli administrert in vivo. Ekpresjon og kloning av villtype eller naturlig pneumolysin er kjent innen fagområdet. Se for eksempel Walker et al (Infect Immun, 55:1184-1189)), Mitchell et al. (Biochim Biophys Acta, 1007:67-72 (1989) og Mitchell et al (NAR, 18:4010 (1990)). Detoksifisering av Ply kan bli utført ved kjemiske midler, f.eks. ved formalin eller glutaraldehydbehandling eller kombinasjon av begge. Slike fremgangsmåter er vel kjent innen fagområdet for ulike toksiner. Alternativt kan Ply bli genetisk detoksifisert. Derved omslutter oppfinnelsen derivater av pneumokokkproteiner som for eksempel kan være muterte proteiner. Betegnelsen "mutert" betyr her et molekyl som har gjennomgått delesjon, addisjon eller substitusjon av en eller flere aminosyrer ved å benytte vel kjente teknikker for sete-rettede mutageneser eller hvilke som helst andre konvensjonelle fremgangsmåter. Som for eksempel beskrevet ovenfor kan et mutant Ply protein bli endret slik at det er biologisk inaktivt mens det fortsatt beholder sine immunogene epitoper, se for eksempel WO 90/06951, Berry et al. (Infect Immun, 67:981-985 (1999)) og WO 99/03884.
Som benyttet her er det forstått at betegnelsen "Ply" refererer til mutert eller detoksifisert pneumolysin egnet for medisinsk anvendelse (dvs ikke-toksisk).
Med hensyn på PsaA og PspA er begge kjent innen fagområdet. PsaA og transmembrandelesjonsvarianter derav har for eksempel blitt beskrevet av Berry & Paton, Infect Immun 1996 (Dec;64(12):5255-62. PspA og transmembrandelesjonsvarianter derav har blitt angitt i for eksempel US 5804193, WO 92/14488 og WO 99/53940.
Sp 128 og Spl30 er angitt i WO 00/76540.
Spl25 er et eksempel på et pneumokokkoverflateprotein med celleveggforankret motiv av LPXDTG (hvor X er enhver aminosyre). Ethvert protein innenfor denne klassen av pneumokokkoverflateprotein med dette motivet har blitt funnet å være nyttig innenfor konteksten av denne oppfinnelsen, og er derfor betraktet som et ytterligere protein ifølge oppfinnelsen. SP 125 er angitt i WO 98/18930, og er også kjent som ZmpB - en sinkmetalloproteinase.
SplOl er angitt i WO 98/06734 (hvor det har referansenummeret y85993). Det erkarakterisert veden type I signalsekvens.
Spl33 er angitt i WO 98/06734 (hvor det har referansenummeret y85992). Det er ogsåkarakterisert veden type I signalsekvens.
De beskrevne proteinene kan også bli fordelaktig kombinert. Foretrakkede kombinasdjoner inkluderer, men er ikke begrenset til PhtD + NRlxR2, PhtD + NRlxR2-Sp91Cterm kimære eller fusjonsproteiner, PhtD + Ply, PhtD + Spl28, PhtD + PsaA, PhtD + PspA, PhtA + NRlxR2, PhtA + NRlxR2-Sp91 Cterm kimære eller fusjonsproteiner, PhtA + Ply, PhtA + Spl28, PhtA + PasaA, PhtA + PspA, NRlxR2 + LytC, NRlxR2 + PspA, NRlxR2 + PsaA, NRlzxR2 + Spl28, R2xR2 + LytC, RlxR2 + PspA, RlxR2 + PsaA, RlxR2 + Spl28, RlxR2 + PhtD, RlxR2 + PhtA. Fortrinnsvis er NRlxR2 (eller RlxR2) fra CbpA eller PspC. Mer foretrukket er den fra CbpA.
En spesielt foretrukket kombinasjon av pneumokokkproteiner omfatter Ply (eller en avkortet variant eller immunologisk funksjonell ekvivalent derav) + PhtD (eller en avkortet variant eller immunologisk funksjonell ekvivalent derav) + NRlxR2 (eller RlxR2). Fortrinnsvis er NRlxR2 (eller RlxR2) fra CbpA eller PspC. Mer foretrakket er den fra CbpA.
Den foreliggende oppfinnelsen omslutter også "immunologisk funksjonell ekvivalent(er)" til proteinene ifølge oppfinnelsen. "Immunologisk funksjonell ekvivalent(er) er definert som et peptid eller protein som omfatter minst en beskyttende epitop fra proteinene ifølge oppfinnelsen. Slike epitoper er karakteristisk nok overflateksponerte, sterkt konserverte, og kan frembringe en bakterisidal antistoffrespons i en vert eller hindre toksiske effekter. Fortrinnsvis har den funksjonelle ekvivalenten minst 15 og fortrinnsvis 30 eller flere tilstøtende aminosyrer fra proteinet ifølge oppfinnelsen og kan bli benyttet med forbehold om at de er i stand til å hovedsakelig reise den samme immunresponsen som det naturlige proteinet. Posisjonen for potensielle B-celleepitoper i en proteinsekvens kan enkelt bli bestemt ved å identifisere peptider som er både overflate-eksponerte og antigene ved å benytte en kombinasjon av to fremgangsmåter: 2D-strakturberegning og antigenindeksberegning. 2D-strukturberegningen kan bli gjort ved å benytte PSIPRED-programmet (fra David Jones, Brunei Bioinformatics Group, Dept. Biological Sciences, Brunei University, Uxbridge UB8 3PH, UK). Antigenindeksen kan bli kalkulert på basisen av fremgangsmåten beskrevet ved Jameson og Wolf (CABIOS 4:181-186 [1988]).
Den foreliggende oppfinnelsen har fordeler over S. penumoniae polysakkaridvaksiner ved at multiple S. pneumoniae protein (immunogen) sammensetninger kan inkludere stor kryss-beskyttelse over flerfoldige serotyper, og kan videre inhibere tilkopling og kolonidannelse, og kan potensielt reise antistoffer som kan nøytralisere de toksiske/enzymatiske funksjonene til et patogen. Videre tilveiebringer ytterligere overflateantigener et middel for å videre stimulere opsonofagocytose.
Den foreliggende oppfinnelsen betrakter også kombinasjonsvaksiner som tilveiebringer beskyttelse mot et bredt spekter av ulike patogener. Mange pediatriske vaksiner er nå gitt som en kombinasjonsvaksine for å redusere antallet av injeksjoner som et barn må motta. For pediatriske vaksiner kan andre antigener fra andre patogener derved bli formulert med vaksinene ifølge oppfinnelsen. For eksempel kan vaksinene ifølge oppfinnelsen bli formulert med (eller administrert separat, men ved samme tidspunkt) den vel kjente trivalente kombinasjonsvaksinen som omfatter difteria toksoid (DT), tetanus toksoid (TT), og pertussikomponenter [vanligvis detoksifisert pertussi toksoid (PT) og filamentøs hemagglutin (FHA) med eventuelt pertaktin (PRN) og/eller agglutinin 1+2], for eksempel den markedsførte vaksinen INFANRK-DTPa™ (SmithKlineBeecham Biologicals) som inneholder DT, TT, PT, FHA og PRN antigener, eller med en hel celle pertussikomponent for eksempel som markedsført av SmithKlineBeecham Biologicals s.a., som Tritanrix™. Den kombinerte vaksinen kan også omfatte annet antigen, slik som hepatitt B overflateantigen (HBsAg), poliovirusantigener (for eksempel inaktivert trivalent poliovirus - IPV), Moraxella catarrhalis ytre membranproteiner, ikke-typbare Haemophilus influenzae proteiner, N. meningitidis B ytre membranproteiner.

I en annen utførelsesform kan ulike antigener beskrevet ovenfor bli inkludert i den immunogene sammensetningen ifølge oppfinnelsen som antigener presentert på overflaten av ytre membranvesikler (blebs) laget fra bakteriene de er utledet fra.
Andre kombinasjoner som er betraktet er S. pneumoniae proteinene ifølge oppfinnelsen i kombinasjon med virale antigener, for eksempel fra influensa (svekkede, kløyvd eller sub-enhet [f.eks. overflateglykoproteiner neuraminidase (NA) og hemagglutinin (HA). Se f.eks. Chaloupka I. Et al, Eur. Journal Clin. Microbiol. Infect. Dis. 1996, 15:121-127], RSV (f.eks. F og G antigener eller F/G fusjoner, se f.eks. Schmidt A. C. Et al, J Virol, mai 2001, s 4594 - 4603), PIV3 (f.eks. HN og F proteiner, se Schmidt et al. Supra), Varcella (f.eks. svekket, glykoproteiner I-V, osv), og hvilket som helst (eller alle) komponent(er) av MMR (meslinger, kusma, røde hunder).
Polvsakkairdantigener ifølge oppfinnelsen
Den foreliggende søknaden betrakter også kombinasjonsvaksiner med 2 eller flere S.
pneumoniae proteiner kombinert med polysakkarider andre enn fra S. pneumoniae. Slike polysakkarider kan bli isolert fra for eksempel H. influensa, H. influensa type B (Hib), N. meningitidis grupper A, C, W, Y, streptokokker andre enn S. pneumoniae (f.eks. gruppe B streptokokkus, S. pyogenes, osv), stafylokokkus (f.eks. S. aureus, S. epidermidis), E. coli, enterokokkus (f.eks. E. faecalis og E. faecium), osv. Fortrinnsvis er polysakkaridene fra H. influensa type B (Hib) og/eller N. meningitidis grupper A, C, W135 og/eller Y.
Som nevnt ovenfor er et problem assosiert med polysakkaridmetoden for vaksinasjon det faktum at polysakkarider per se er dårlige immunogener. For å overvinne dette kan polysakkarider bli konjugert til proteinbærere, som tilveiebringer tilskuer T-cellehjelp. Det er derfor foretrukket at polysakkaridene benyttet i oppfinnelsen er forbundet til en slik proteinbærer. Eksempler på slike bærere som nå er alminnelig benyttet for produksjonen av polysakkaridimmunogener inkluderer difteria og tetanus toksiodene (DT, DT CRM197, andre DT mutanter, f.eks. posisjon Glu-148, osv [se f.eks. US 4.709.017, WO 93/25210, WO 95/33481, osv] og TT (og TT fragment C)), Keyhole Limpet Hemocyanin (KLH), OMPC fra N. meningitidis og det rensede proteinderivatet fra tuberkulin (PPD).
En annen bærer for de polysakkaridbaserte immunogene sammensetningene (eller vaksiner er protein D fra Haemophilus influenzae (EP 594610-B), eller fragmenter derav. Fragmenter egnet for anvendelse inkluderer fragmenter som omslutter T-hjelperepitoper. Nærmere bestemt vil et protein D-fragment fortrinnsvis inneholde den N-terminale 1/3 av proteinet.
Polysakkaridet kan bli forbundet til bærerproteinet ved enhver kjent fremgangsmåte (for eksempel fra Likhite, U.S. patent 4.372.045 og fra Armor et al, U.S. patent 4.474.757). Fortrinnsvis er CDAP-konjugering utført (WO 95/08348). For å forsterke immunogenesitet kan polysakkaridene være av størrelse (depolymerisert), blandet med adjuvans, lyofilisert eller være konjugert til ulike bærerproteiner.
TH1 adjuvans ifølge oppfinnelsen
Vaksinene ifølge foreliggende oppfinnelsen er fortrinnsvis blandet med adjuvanser. Egnede adjuvanser inkluderer et aluminiumsalt slik som aluminiumhydroksidgelé (alum) eller aluminiumfosfat, men kan også være et salt av kalsium, magnesium, jern eller sink, eller kan være en uløselig suspensjon av acylert tyrosin eller acylerte sukkere, kationisk eller anionisk derivatiserte polysakkarider eller polyfosfasener.
Det er foretrukket at adjuvansen blir valgt til å være en preferensiell induserer av en TH1 type av respons. Slike høye nivåer av Thl-type cytokiner tjener til å favorisere induksjonen av celleformidlede immunresponser til et gitt antigen, mens høye nivåer av Th2-type cytokiner tjener til å favorisere induksjonen av humorale immunresponser til antigenet.
Det er viktig å huske at forskjellen på Thl og Th2-type immunrespons ikke er absolutte. I virkeligheten vil et individ støtte en immunrespons som er beskrevet til å være overveiende Thl eller overveiende Th2. Imidlertid er det ofte passende å betrakte familiene av cytokiner på vilkår av det beskrevet i murine CD4 +ve T cellekloner ved Mosmann og Coffman (Mosmannn, T.R. og Coffman, R.L. ()1989) TH1 og TH2 cells: different patterns of lymphokine secretion lead to different functional properties. Annual Review of Immunology, 7, s 145-173). Tradisjonelt er Thl-type responser assosiert med produksjonen av INF-y og IL-2 cytokinene ved T-lymfocytter. Andre cytokiner ofte direkte assosiert med induksjonen av Thl-type immunresponser er ikke produsert av T-celler, slik som IL-12.1 motsetning er Th2-type responser assosiert med utskillelsen av IL-4, IL-5, IL-6, IL-10. Egnede adjuvanssystemer som hovedsakelig fremmer en Thl respons inkluderer: monofosforyl lipid A eller et derivat derav, fortrinnsvis 3-de-O-acylert monofosforyl lipid A (3D-MPL) (for dets preparering se GB 2220211 A); og en kombinasjon av monofosforyl lipid A, fortrinnsvis 3-de-O-acylert monofosforyl lipid A, sammen med enten et aluminiumsalt (for eksempel aluminiumfosfat eller aluminiumhydroksid) eller en olje-i-vann emulsjon. I slike kombinasjoner er antigen og 3D-MPL holdt i de samme partikulære strukturene, som tillater mer effektiv levering av antigenet og immunostimulatoriske signaler. Studier har vist at 3D-MPL er i stand til å ytterligere forsterke immunogenesiteten av et alum-adsorbert antigen [Thoelen et al. Vaccine (1998) 16:708-14; EP 689454-B1].
Et forsterket system invovlerer kombinasjonen av et monofosforyl lipid A og et saponinderivat, spesielt kombinasjonen av QS21 og 3D-MPL som angitt i WO 94/00153, eller en mindre reaktogen sammensetning hvor QS21 er undertrykt med kolesterol som angitt i WO 96/33739.
En spesielt potent adjuvansformulering involverer QS21, 3D-MPL og tokoferol i en olje-i-vann-emulsjon som beskrevet i WO 95/17210, og er en foretrukket formulering.
Fortrinnsvis omfatter vaksinen ytterligere et saponin, mer foretrukket QS21. Formuleringen kan også omfatte en olje-i-vann-emulsjon og tokoferol (WO 95/17210).
Den foreliggende oppfinnelsen tilveiebringer også en fremgangsmåte for å produsere en vaksineformulering som omfatter å blande et protein av den foreliggende oppfinnelsen sammen med en farmasøytisk aksepterbart bindemiddel slik som 3D-MPL.
Umetylert CpG som inneholder oligonukleotider (WO 96/02555) er også preferensiell induserer av en TH 1-respons og er egnet for anvendelse i den foreliggende oppfinnelsen.
I et ytterligere aspekt av den foreliggende oppfinnelsen er det tilveiebrakt en vaksine som her beskrevet for anvendelse i medisin.
Vaksinepreparater ifølge oppfinnelsen
Vaksinepreparatene av den foreliggende oppfinnelsen kan bli benyttet for å beskytte eller behandle et pattedyr (fortrinnsvis en human pasient) mottakelig for infeksjon, ved midler av å administrere nevnte vaksine via systemisk eller mukosal rute. Disse administreringene kan inkludere injeksjon via de intramuskulære, intraperitonale, intradermale eller subkutane veier; eller via mukosal administrasjon til de orale/alimentære, respiratoriske, urogenitale systemene. Intranasal administrasjon av vaksiner for behandlingen av pneumoni eller Otitis media er foretrukket (ettersom nasofaryneal transport av pneumokokker kan bli mer effektivt forhindret, som på denne måten svekker infeksjon på et tidlig stadium). Selv om vaksinen ifølge oppfinnelsen kan bli administrert som en enkel dose kan komponentene derav også bli co-administrert sammen ved det samme tidspunktet eller ved ulike tidspunkter (for eksempel om polysakkarider er tilstede i en vaksine kan disse bli administrert separat ved det samme tidspunktet eller 1-2 uker etter administrasjonen av den bakterielle proteinkombina-sjonen for optimal koordinering av immunresponsene med hensyn til hverandre) i tillegg til en enkel rute for administrasjon kan 2 ulike ruter for administrasjon ble benyttet. For eksempel kan viralantigener bli administrert ID (intradermalt), mens bakterielle proteiner kan bli administrert IM (intramuskulært) eller IN (intranasalt). Om polysakkarider er tilstede kan de bli administrert IM (eller ID) og bakterielle proteiner kan bli administrert IN (eller ID). I tillegg kan vaksinene ifølge oppfinnelsen bli administrert IM for forberedende doser og IN for påfyllende doser.
Mengden av konjugert antigen i hver vaksinedose er valgt som en mengde som inkluderer en immunobeskyttende respons uten signifikante motsatte bieffekter i typiske vaksiner. Slik mengde vil variere avhengig av hvilket spesifikt immunogen som er benyttet og hvordan det er presentert. Innholdet av proteinantigener i vaksinene vil vanligvis være i området av 1-100 \ ig, fortrinnsvis 5-50 ug, mest vanlig i området av 5-25 ug. Om polysakkarider er inkludert er det generelt forventet at hver dose vil omfatte 0,1-100 \ ig av polysakkarid, fortrinnsvis 0,1-50 \ ig, mer foretrukket 0,1-10 \ ig, hvor 1 til 5 \ ig er det mest foretrakkede området.
Optimale mengder av komponenter for en bestemt vaksine kan bli fastslått ved standard studier som involverer observasjon av hensiktsmessige immunresponser hos individer. Etter en initiell vaksinering kan individer motta en eller flere påfyllingsimmuniseringer med passende mellomrom. Vanligvis vil en vaksine omfatte antigen (proteiner), et adj uvant og bindemidler eller en farmasøytisk aksepterbar bærer.
Vaksinepreparering er generelt beskrevet i Vaccine Design ("The subunit and adjuvant approach" (utg. Powell M.F. % Newman MJ.) (1995) Plenum Press New York). Innkapsling i liposomer er beskrevet av Fullerton, US patent 4.235.877.
Selv om vaksinene av den foreliggende oppfinnelsen kan bli administrert ved enhver rute former administrasjon av de beskrevne vaksinene i huden (ED) en utførelsesform av den foreliggende oppfinnelsen. Hud hos mennesker omfatter en ytre "hornaktig" overhud, kalt stratum corneum, som overdekker epidermis. Under dette epidermis er et lag kalt dermis, som videre dekker det subkutane vevet. Forskere har vist at injeksjon av en vaksine til huden, og nærmere bestemt til dermis, stimulerer en immunrespons som også kan bli assosiert med et antall av ytterligere fordeler. Intradermal vaksinasjon med vaksinene beskrevet her former et foretrukket moment av den foreliggende oppfinnelsen.
Den konvensjonelle teknikken for injeksjon, "mantoux-prosedyren" omfatter trinnene av å rense huden og så strekke den med en hånd, og med den nedslipte overflaten av en smal justert nål (26-31 justert) som vender oppover er nålen stukket inn med en vinkel på omkring 10-15°. Med en gang den slipte overflaten av nålen er stukket inn er løpet på nålen senket og ytterligere fortsatt mens det gis et lett trykk for å levere det under huden. Væsken er så injisert veldig sakte som derved former en boble eller ujevnhet på hudoverflaten, etterfulgt av sakte tilbaketrekking av nålen.
I den senere tid har anordninger som er spesifikt designet for å administrere væskeagens til eller gjennom huden blitt beskrevet, for eksempel anordningene beskrevet i WO 99/34850 og EP 1092444, også stråleinjeksjonsanordningene beskrevet for eksempel i WO 01/13977; US 5.480.381, US 5.599.302, US 5.334.144, US 5.993.412, US 5-649.912, US 5.569.189, US 5.704.911, US 5.383.851, US 5.893.397, US 5.466.220, US 5.339.163, US 5.312.335, US 5.503.627, US 5.064.413, US 5.520.639, US 4.596.556, US 4.790.824, US 4.941.880, US 4.940.460, WO 97/37705 og WO 97/13537. Alternative fremgangsmåter for intradermal administrasjon av vaksinepreparatene kan inkludere konvensjonelle sprøyter og nåler eller anordninger fremstilt for ballistisk levering av vaksiner i fast form (WO 99/27961), eller transdermale lapper (WO 97/484440; WO 98/28037); eller påført til overflaten av huden (transdermal eller transkutan levering WO 98/120734; WO 98/28037).
Når vaksinene av den foreliggende oppfinnelsen skal bli administrert til huden, eller mer spesifikt til dermis er vaksinen i et lite væskevolum, nærmere bestemt et volum av mellom omkring 0,05 ml og 0,2 ml.
Innholdet av antigener i hud eller intradermalvaksinene av den foreliggende oppfinnelsen kan være lignende konvensjonelle doser som funnet i intramuskulære vaksiner. Følgelig kan proteinantigenet tilstede i de intradermal vaksinene være i området 1-100 \ ig„ fortrinnsvis 5-50 ug. Om polysakkaridkonjugatantigen er tilstede vil mengden i hver vaksinedose på lignende måte generelt omfatte 0,1-100 ug av polysakkarid, fortrinnsvis 0,1-50 ug, fortrinnsvis 0,1-10 ug, og kan være mellom 1 og 5 \ ig. Imidlertid er det et kjennetegn for hud eller intradermale vaksiner at formuleringene kan være "lav dose". Følgelig er proteinantigenet i "lav dose" vaksiner fortrinnsvis tilstede i så lite som 0,1 til 10 ug, fortrinnsvis 0,1 til 5 \ ig pr dose; og om tilstede kan polysakkaridkonjugatantigene bli presentert i området av 0,01-1 \ ig, og fortrinnsvis mellom 0,01 til 0,5 \ ig av polysakkarid pr dose.
Som benyttet her betyr betegnelsen "intradermal levering" levering av vaksinene til regionen av dermis i huden. Imidlertid vil vaksinen ikke nødvendigvis bli lokalisert eksklusivt i dermis. Dermis er laget i huden lokalisert mellom omkring 1,0 og omkring 2,0 mm fra overflaten i hud hos mennesker, men det er en bestemt mengde av variasjon mellom individer og i ulike deler av kroppen. Generelt kan det forventes å nå dermis ved å gå 1,5 mm under overflaten av huden. Dermis er lokalisert mellom stratum corneum og epidermis ved overflaten og det subkutane laget under. Avhengig av måten for levering kan vaksinen omsider bli lokalisert utelukkende eller først og fremst i dermis, eller den kan utelukkende bli distribuert innenfor epidermis og dermis.
I et annet aspekt ifølge oppfinnelsen kan den foreliggende oppfinnelsen inneholde DNA som koder for ett eller flere S. pneumoniae proteiner, slik at proteinet er generert in situ. DNAet kan være tilstede innenfor ethvert av et utvalg av leveringssystemer kjent for de med ordinær dyktighet innen fagområdet, som inkluderer nukleinsyreekspresjonssystemer, bakterielle og virale ekspresjonssystemer. Tallrike genleveringsteknikker er vel kjent innen fagområdet slik som de beskrevet av Rolland, (CritRev. Therap. Drug Carrier Systems 15:143-198,1998) og referanser sitert der. Hensiktsmessige nukleinsyreekspresjonssystemer inneholder de nødvendige DNA-sekvensene for ekspresjon i pasienten (slik som en egnet promoter og termineringssignal). Når ekspresjonssystemet er en rekombinant levende mikroorganisme, slik som et virus og en bakterie, kan genet av interesse bli satt inn i genomet til en levende rekombinant virus eller bakterie. Inokulering og in vivo infeksjon med denne levende vektoren vil føre til in vivo ekspresjon av antigenet og induksjon av immunresponser. Virus og bakterier benyttet for dette formålet er for eksempel: poks-virus (f.eks. vaksinia, fjærfédifteri, kanaridifteri), alfa-virus (Sindbis-virus, Semliki Forest-virus, venezuelsk hesteenkefalitt-virus) adeno-virus, adenoassosierte virus, pikorna-virus (polio-virus, rino-virus), herpes-virus (varicella zoster-virus, osv), Listeria, Salmonella, Shigella, Neisseria, BCG. Disse virusene og bakteriene kan være virulente eller attenuerte på ulike måter for å oppnå levende vaksiner. Slike levende vaksiner danner også del av oppfinnelsen.
I et ytterligere aspekt av den foreliggende oppfinnelsen er det tilveiebrakt en fremgangsmåte for fremstilling av en vaksineformulering som beskrevet her, hvor fremgangsmåten omfatter å blande en kombinasjon av proteiner i henhold til oppfinnelsen.
Fortrinnsvis er de antigene sammensetningene (og vaksinene) som inneholder polysakkarider beskrevet her tidligere lyofilisert inntil de skal bli benyttet, på dette tidspunktet er de umiddelbart rekonstituert med fortynningsmiddel. Mer foretrukket er de lyofilisert under tilstedeværelsen av 3D-MPL, og er umiddelbart før bruk rekonstituert med saltoppløsning.
Lyofiliseringen av vaksiner er vel kjent innen fagfeltet. Vanligvis er væskevaksinene frysetørket under tilstedeværelsen av et antiklumpemiddel som for eksempel sukkere slik som sukrose eller laktose (tilstede ved en initiell konsentrasjon på 10-200 mg/ml). Lyofilisering foregår vanligvis over en serie av trinn, for eksempel en syklus som starter ved -69°C, gradvis justering til -24°C over tre timer, så opprettholde denne temperaturen i 18 timer, så gradvis justering til -16°C over 1 time, så opprettholde denne temperaturen i 6 timer, så gradvis justere til +34°C over 3 timer, og til slutt opprettholde denne temperaturen i 9 timer.
De immunogene sammensetningene og vaksinene ifølge oppfinnelsen kan bli evaluert i ulike dyremodeller eller med humant serum. Som en illustrasjon kan de følgende dyremodellene bli benyttet for å evaluere pneumokokkinfeksjon. C3H/HeJ mus (6 til 8 uker gamle) kan bli immunisert s.c. med 15 \ ig protein hjulpet med 50 \ il CF A, etterfulgt 3-4 uker senere av påfylling med 15 \ ig protein med IFA. For å demonstrere passiv og aktiv beskyttelse fra systemisk infeksjon kan mus bli administrert intraperitonalt med immunserum eller proteiner før utfordring ved intraperitonal injeksjon med 15 til 90 LD50 pneumokokki på uke 8-10.1 tillegg kan proteiner blir testet i en muse-nasofarynks kolonisasjonsmodell av (Wu et al Microbial Pathogenesis 1997; 23:127-137).
I tillegg til mus er yngre rotter mottakelige for kolonisasjon og infeksjon ved S. pneumoniae. I passive beskyttelsesstudier kan administrasjon av immunt serum fra mus (100 ul i.p. eller 10 \ il i.n.) bli utført før utfordring med intranasal administrasjon av S. pneumoniae (10 i 2-5 dager gamle rottebarn. Koloniasjon kan bli bestemt ved å plate nasale skyllinger (20-40 jxl dryppet inn, 10 jxl trukket ut).
Fordelaktige interaksjoner mellom proteinkomponenter av kombinasjonsvaksinen kan bli demonstrert ved å administrere en dose av hvert protein i vaksinen som vil være sub-beskyttende i en monovalent vaksine. Økt beskyttende effektivitet av kombinasjonsvaksinen sammenlignet til monovalente vaksiner kan bli tilskrevet til en fordelaktig interaksjon mellom komponentene.
Oppfinnelsen er illustrert i de vedlagte eksemplene. Eksemplene er utført ved å benytte standard teknikker, som er vel kjent og rutine for dem med dyktighet innen fagområdet, med unntak hvor på annen måte beskrevet i detalj. Eksemplene er ment å illustrere, men ikke begrense oppfinnelsen.
EKSEMPLER
Eksempel 1. Konstruksjon og ekspresjon av antigener
NRlxR2
CbpA er et 75kDa overflateeksponert protein som består av flere domener. Det N-terminale domenet omfatter 2 sterkt konserverte repetisjoner (RI og R2) og det C-terminale domenet omfatter 10 tandem, direkte repeterte sekvenser av 20 aminosyrer. Et avkortet CbpA ble preparert til å produsere NRlxR2, dvs uten det kolinbindende domenet.
NRlxR2 genet ble amplifisert via PCR fra DNA ervervet fra en serotype 4 stamme og S. pneumoniae (se f.eks. "WO 97/41157 eller WO 99/51266). PCR ble utført med Expand High Fidelity PCR-systemet eller Hi-Fi (Roche). Det er komponert av en blanding som inneholder Taq-polymerase og en korrekturlesende polymerase. På grunn av en iboende 3'-5' eksonuklease korrekturlesingsaktiviteten resulterer anvendelsen av Hi-Fi i en 3 doblet økt kvalitet av DNA-syntese sammenlignet med Taq-polymerase.
PCR-fragmenter ble klonet i pGEM-T-vektor fra pGEM-T Vector Systems (Promega). Dette trinnet er nødvendig for å fremme restriksjonsenzymfordøying av PCR-fragment for videre ligering. pGEM-T-vektor er tilveiebrakt lineær og inneholder 3'-T-overheng. Disse overhengene fremmer innsetting av PCR-produkter generert ved termostabile polymeraser som tilfører en enkel deoksyadenosin på en templat-uavhengig måte til de 3' endene av det amplifiserte fragmentet.
Fragmenter og vektorer ble renset etter enzymatiske fordøyinger { Ndel og Xbal fordøyinger) i henhold til artikkelen til Benore-Parsons et al. (Nucleic Acids research, 23, 4926-4927, 1995). Agaroseskive ble ferdig lyofilisert i løpet av 3-4 timer. En 1:1 etanol-TE-løsning ble tilsatt til den lyofiliserte gelen. Prøven ble forsiktig blandet i 1 time, agarosen ble sammenpresset og fullstendig fjernet ved sentrifugering. DNA ble gjenvunnet fra eluatet ved etanol presipitering.
DNAet som kodet for NRlxR2 ble klonet i en vektor som inneholdt lang promoter L fra fag X. Proteinet av interesse kunne bli indusert ved varme når tilstede i AR 58 E . coli - stamme, eller ved nalidiksinsyre i AR 120 E . co/ Z-stamme.
En forkultur av bakterier ble laget over natt ved 30°C. Denne forkulturen ble fortynnet omkring 40 ganger i et totalt volum på 20 ml og satt ved 30°C inntil en O.D. på 0,4-0,6. Så ble varmeinduksjon utført ved 42°C. Prøver ble tatt ved ulike tidspunkt. En ml av kultur ble sentrifugert 5 minutter ved 7000 rpm. Kultursupernatant ble konseervert ved - 20°C og pellet (totalt ekstrakt) ble resuspender i 500 \ i\ av prøvebuffer (westert blot eller SDS-PAGE-analyse), eller i 500 [ il av lysisbuffer og inkubert 30 minutter ved 37°C (ELISA). Sammensetningen av lysisbuffer er: SDS 0,1 %, Deoksykolat 0,1 %, Na-sitrat; 0,015 M.
Prøver ble kjørt på en SDS-PAGE, lastet på 4-20 % gel (Novex, Invitrogen). Migrering ble utført ved 200 V. Coomassie blå-farging ble utført. Prøver ble lastet på 4-20 % gel (Novex, Invitrogen) for Western Blotting. Migrering ble utført ved 200 V. Gel ble overført på nitrocellulose og punkter ble avslørt med kanin oc-NRlxR2 polyklonale antistoffer (første antistoff) og et oc-kanin antistoff koplet til alkalisk fosfatase (andre antistoff).
En bånd på omtrentlig 55kDa ble observert ved SDS-PAGE-analyse. Klonen 28B2 ble valgt på basis av SDS-PAGE-analysen og overført til fermentering. Denne klonen ble sekvensert og dens sekvens ble konfirmert (aminosyrer 39 (dvs etter signalsekvens) til 446 = 406 aminosyrer).
Løseligheten av NRlxR2 ble også studert etter lysering av over-natt-induserte bakterier etterfulgt av sentrifugering. En SDS-PAGE-analyse og en ELISA-test ble utført. NRlxR2 viste seg å hovedsakelig (> 95 %) gjenvunnet i den løselige fraksjonen.
RlxR2, PhtD, Sp91, ( N) RlxR2- Sp91[ C- terminalt domene], og Ply
Disse genene ble også klonet, sekvensert og uttrykt på en lignende måte som NRlxR2. RlxR2 inneholder aminosyrer 177 til 443 av CbpA (av S. pneumoniae serotype 4N), PhtD inneholder aminosyrer 21 (dvs etter signalsekvens) til enden (aminosyre 839 av S. pneumoniae serotype 4N), Sp91 starter ved aminosyre 20 (VAA) til enden. For fusjonsproteiner inneholder RlxR2-Sp91 Cterm aminosyrer 177-446 av CbpA og aminosyrer 271 til translasjonsstopp; NRlxR2-Sp91 Cterm inneholder aminosyrer 39-446 av CbpA og aminosyrer 271 til translasjonsstopp. For begge fusjonsproteiner er 2 ytterliere aminosyrer (GS) funnet mellom (N)RlxR2 og Sp91Cterm-sekvensene. For alle konstruksjoner har en ATG blitt introdusert 5' for starten av genet for å tillate transkripsjon og translasjon, som betyr at det er en ytterligere N-terminal metionin foran hver sekvens nevnt ovenfor.
Eksempel 2. Serologi
Ved å benytte serum fra kliniske studier ble ELSIAer målt for antistoffresponsen som naturlig utviklet seg til S. pneumoniae proteiner).
2.1 EKSPERIMENTELL PROSEDYRE
• Serumprøver
□ Parede serum fra barn samlet når de var henholdsvis 2 til 4 måneder gamle og 6
til 12 måneder gamle (N = 20, studier DTPa HB V).
□ Serum av~20 år gamle voksne (N = 50).
□ Serum av > 65 år bamle voksne (N=140).
• ELISA-prosedyrer
Immunoplater ble belagt over-natt ved 4°C med 1 ug/ml av hvert protein. To-doblede fortynninger av serum (som starter med en 1/10 fortynning) i rekke ble så inkubert i 1 time ved romtemperatur (RT) under risting. Immunodeteksjon ble gjort ved å benytte et peroksydasekoplet anti-humant IgG monoklonalt antistoff (Strateck, HP6043) fortynnet 4000 ganger og inkubert i 30 minuter ved RT under risting. Avsløring ble midtpunkttitere kalkulert ved SoftMaxPro. Serum med titere > 10 ble betraktet som positive. For mellomproporsjonalledd-kalkulasjon ble en titer på 5 (halvparten av avskjæringen) vilkårlig tillagt negative serum.
IgG-konsentrasjoner uttrykt som ug/ml ble etablert ved å sammenligne optiske tettheter (OD) av prøver til OD-kurven for kromo-ren IgG (Jackson) fanget på platen av polyklonale anti-humane IgG geit-antistoffer og avslørt ved det samme peroksydase-merkede antistoffet som ovenfor.
2.2 RESULTATER
2.2.1 Strep-proteinserologi hos barn
Den høyste antistofftiteren og seropositivitetsrater målt i serum fra 2 til 4 måneder gamle barn ble oppnådd med PhtD, PsaA, Sp 128, NRlxR2 og i mindre utstrekning med Sp91 og Ply. Ingen eller lave responser til SplOl og Spl30 ble detektert. Sp46 og PhtA ble ikke testet (resultat av materialtilgjengelighet).
Antistoffresponsene minket generelt i serum fra de samme individene samlet når disse var 6 til 12 måneder gamle, som antyder at de høye titrene hovedsakelig var et resultat av passivt overførte antistoffer fra mor.
Imidlertid hos enkelte barn økte immunresponsen til noen proteiner med alder, sannsynligvis som en konsekvens av naturlig eksponering for pneumokokker. Antigenet hovedsakelig implisert ved denne sero-omdannelsen var tydelig PsaA. Avhengig av individet ble også forhøying av antistoffnivåer til PhtD, NRlxR2, Sp 128, Sp91 og Ply også vist. Bare marginal variasjon i den humorale responsen til SplOl og Spl30 ble observert. (Se figurer 1 og 2).
2.2.2 Strep-proteinserologi hos yngre voksne
I henhold til den geometriske middelverdititeren er PhtD, PhtA og NRlxR2 de mest immunogene proteinene i den yngre voksne populasjonen evaluert, så fulgt av Spl28, Ply og Sp91. Alle individer hadde detekterbare antistoffer til disse proteinene. Lavere responser ble målt til Sp46, og spesielt til Spl30 og SplOl. PsaA ble ikke testet (ikke nok serum tilgjengelig). (Se figurer 3 og 4).
2.2.3 Strep-proteinserologi hos eldre voksne
Det var en klar nedgang i antistoffnivåer til strep-proteiner hos eldre mennesker sammenlignet med yngre voksne. I eldre mennesker er det beste immunogenet PhtD, etterfulgt av Sp 128, NRlxR2, Sp91, Ply og PsaA. Bare marginale responser ble målt til SplOl og Spl30. Sp46 og PhtA ble ikke testet (resultat av materialtilgjengelighet). (Se figurer 5 og 6).
Claims (13)

Immunogen sammensetningkarakterisert vedat den omfatter minst 2 S. pneumoniae proteiner hvori et av proteinene er PhtD og et annet protein er detoksifisert pneumolysin (Ply).
2.
Immunogen sammensetning ifølge krav 1, videre omfattende NRlxR2 eller RlxR2 fra CbpA eller PspC.
3.
Immunogen sammensetning ifølge krav 1-2, hvor pneumolysinet er kjemisk detoksifisert.
4.
Immunogen sammensetning ifølge krav 1-2, hvor pneumolysinet er genetisk detoksifisert.
5.
Immunogen sammensetning ifølge krav 1-4, som i tillegg inneholder et annet antigen valgt fra gruppen bestående av hepatitis B overflate antigen (HBsAg), poliovirus antigener, Moraxella catarrhalis ytre membranproteiner, ikke-typbare Haemophilus influenzae proteiner og N. meningitidis B ytre membranproteiner.

Immunogen sammensetning ifølge krav 5, hvor det ytterligere antigenet er et ikke-typbart Haemophilus influenzae protein.

Immunogen sammesetning ifølge krav 6, hvor det ytterligere antigenet er Protein D fra ikke-typbart Haemophilus influenzae.

Immunogen sammensetning ifølge krav 1-7, som ytterligere omfatter en adjuvant.

Immunogen sammensetning ifølge krav 8, hvor adjuvanten er en preferensiell induser av en THl type respons.

Vaksine,karakterisert vedat den omfatter den immunogene sammensetningen ifølge krav 1-9.

Anvendelse av vaksine ifølge krav 10, for fremstilling av et medikament for forebyggelse av pneumonia i pasienter over 55 år.

Anvendelse av vaksine ifølge krav 10, for fremstilling av et medikament for forebyggelse av Otitis media hos spedbarn.

Fremgangsmåte for fremstilling av en vaksine ifølge krav 10,karakterisert vedat den omfatter trinnene av: å velge og å isolere to ulike S. pneumoniae proteiner; og å blande nevnte proteiner sammen med en farmasøytisk aksepterbar bærer.
