Перфорация дна верхнечелюстной пазухи возникает как по вине врача (при травматическом или неправильном удалении зуба долотом, щипцами или элеватором, неосторожном обследовании лунки кюретажной ложечкой или удалении грануляций с ее дна), так и в результате индивидуальных особенностей строения верхней челюсти (пневматический тип верхнечелюстной пазухи с низким расположением ее дна или предшествующими патологическими процессами в области верхушки корня зуба).Perforation of the bottom of the maxillary sinus occurs both through the doctor’s fault (with traumatic or improper tooth extraction with a chisel, forceps or elevator, careless examination of the hole with a curette spoon or removal of granulations from its bottom), as well as as a result of individual structural features of the upper jaw (pneumatic type of the maxillary groove low location of its bottom or previous pathological processes in the region of the apex of the tooth root).
Диагностика перфорации верхнечелюстной пазухи (ороантрального сообщения) основывается на следующих признаках:Diagnosis of maxillary sinus perforation (oroantral communication) is based on the following symptoms:

- положительная носовая проба - больной закрывает пальцами обе ноздри и пытается выдохнуть воздух через нос, а воздух со свистом (с шумом и пузырьками или кровью) выходит из лунки. Отрицательная проба не исключает наличия соустья, т.к. оно может закрываться полипами. Если перфорация верхнечелюстной пазухи (без наличия корня зуба) осложнена острым гнойным гайморитом, то следует через лунку промыть полость верхней челюсти антисептическим раствором (в течение нескольких дней) для снятия воспалительных явлений. В последующем производят закрытие ороантрального сообщения общепринятым способом. Способ заключался в том, что под инфильтрационной анестезией 4% раствором артикаина в количестве 3-5 мл выкраивался слизисто - надкостничный лоскут трапециевидной формы с вестибулярной поверхности альвеолярного отростка верхней челюсти в области ороантрального сообщения основанием к переходной складке. Лоскут с помощью распатора смещался вверх. Для увеличения подвижности лоскута у его основания рассекалась надкостница. Производилась ревизия и антисептическая обработка лунки удаленного зуба, освежались края дефекта, при наличии иссекался ороантральный свищ, отслаивалась десна с небной стороны на 3-5 мм. Устье лунки покрывалось мембраной ТахоКомб. Мембрана вводилась под десну с небной стороны и накладывалась таким образом, что она перекрывала дефект со всех сторон на 3-5 мм, затем плотно прижималась к кости в течение 3-5 мин. Выкроенный слизисто-надкостничный лоскут перемещался на область дефекта дна пазухи и фиксировался к небному краю лунки зуба узловыми и «П»-образными швами нитью «викрил». Рана в области альвеолярного отростка с вестибулярной стороны ушивалась узловыми швами. Линия швов укрывалась йодоформным тампоном, который фиксировался к зубам лигатурной проволокой на срок до 7 дней. Швы снимались на 10-е сутки. В послеоперационном периоде проводилась стандартная противовоспалительная и антибактериальная терапия, гипотермия местно, полоскания раствором гипохлорита натрия, УФО полости рта. Для хорошего дренирования верхнечелюстной пазухи через естественное отверстие назначались сосудосуживающие капли в нос в течение 2 недель.The method consisted in the fact that under infiltration anesthesia with a 4% solution of articaine in an amount of 3-5 ml, a mucous - periosteal trapezoidal flap was cut out from the vestibular surface of the alveolar process of the upper jaw in the area of the oroantral communication with the base to the transition fold. The mouth of the hole was covered with a TahoeKomb membrane. The membrane was inserted under the gum from the palatine side and was applied in such a way that it overlapped the defect from all sides by 3-5 mm, then tightly pressed to the bone for 3-5 minutes. The cut out mucoperiosteal flap moved to the area of the sinus fundus defect and was fixed to the palatine edge of the tooth socket with interrupted and “P” -shaped sutures with a “vikril” thread. A wound in the alveolar ridge from the vestibular side was sutured with interrupted sutures. The line of sutures was covered with an iodine swab, which was fixed to the teeth with ligature wire for up to 7 days. Sutures were removed on the 10th day. In the postoperative period, standard anti-inflammatory and antibacterial therapy was performed, topical hypothermia, rinsing with sodium hypochlorite solution, and oral ultraviolet radiation. For good drainage of the maxillary sinus through a natural opening, vasoconstrictive drops in the nose were prescribed for 2 weeks.

Способ осуществляют следующим образом. По краям ороантрального соустья проводят окаймляющий разрез слизистой, отслаивают слизисто-надкостничные лоскуты отступя от края дефекта на 1.5-2 мм, проводят ревизию верхнечелюстной пазухи. Далее ороантральное сообщение закрывают нерезорбируемой мембраной с титановым усилением, края которой заправляют под надкостницу по краям дефекта, рану ушивают нерезорбируемым материалом. Удаление мембраны производят через 1 месяц, дождавшись восстановления слизистой оболочки верхнечелюстной пазухи. После удаления мембраны восстановление слизистой полости рта проходит самопроизвольно в течении 5-7 дней.The method is as follows.
