Изобретение относится к области медицины, а именно к оториноларингологии, челюстно-лицевой хирургии и хирургической стоматологии, и предназначено для использования при эндоназальной субантральной аугментации.The invention relates to medicine, namely to otorhinolaryngology, maxillofacial surgery and dental surgery, and is intended for use in endonasal subantral augmentation.
Известен способ поднятия слизистой оболочки дна верхнечелюстного синуса доступом через латеральную стенку верхней челюсти. Производят разрез на небной части альвеолярного гребня на протяжении всей области отсутствия зубов, формируют трапециевидный слизисто-надкостничный полнослойный лоскут с широким основанием, отслаивают его, чтобы обнажить латеральную стенку верхней челюсти, в которой бором формируют окно. По краю окна вводят кюрету, отслаивая и поднимая слизистую, формируя подслизистое пространство, в которое вносят остеотропный материал. Область окна на латеральной стенке верхней челюсти закрывают коллагеновой мембраной, слизисто-надкостничный лоскут укладывают на место и фиксируют швами (1). Недостатками операции являются: ее травматичность, отсутствие фиксации мембраны к альвеолярному отростку, что приводит к ее смещению, изменению анатомии послеоперационного дефекта, миграции остеотропного материала из верхнечелюстного синуса в подслизистое пространство альвеолярного отростка верхней челюсти; обратимые и необратимые воспалительные процессы, развитие иммунных реакций на чужеродный материал.The known method of raising the mucous membrane of the bottom of the maxillary sinus access through the lateral wall of the upper jaw. Для эндоскопического обзора используют естественное отверстие верхнечелюстной пазухи в среднем носовом ходу, куда вводят жесткий эндоскоп диаметром 1.7 мм с углом обзора 70 градусов. В пазухе эндоскоп устанавливают срезом к альвеолярной стенке. На уровне нижнего носового хода выкраивают П-образный лоскут слизистой оболочки, бором производят перфорацию медиальной костной стенки с продвижением бора параллельно дну пазухи. Максимально близко к дну полости носа высверливают отверстие в кости, размером 10/10 мм. В отверстие фиксируют трубку диаметром 9,9 мм, через которую вводят инструменты для поднятия слизистой оболочки и введения биоматериала. Вводят элеватор в костное окно, продвигают его от дна носа к дну пазухи. После образования пространства между костным дном и слизистой оболочкой пазухи начинают гидропрепаровку, эндоскопически контролируя целостность слизистой ВЧ пазухи, поднимая слизистую выстилку на протяжении всего дна пазухи. After the formation of a space between the bone bottom and the sinus mucosa, hydropreparation begins, endoscopically controlling the integrity of the HF sinus mucosa, lifting the mucous lining throughout the entire sinus floor. Естественное соустье верхнечелюстной пазухи в 90% случаев прикрыто крючковидным отростком, поэтому сначала нужно удалить его, чтобы была возможность ввести туда эндоскоп, что сопряжено с Техническими трудностями. Перфорация медиальной стенки верхнечелюстной пазухи со стороны полости носа бором, чревата разрывом слизистой оболочки выстилающей пазуху. Фиксация в нижнем носовом ходу трубки диаметром 9,9 мм далеко не всегда выполнимая задача, потому, что диаметр ноздри не всегда ему соответствует, а пространство, ограниченное передним концом нижней носовой раковины, дном полости носа, перегородкой носа гораздо меньше 9.9 мм. В образовавшуюся полость плотно укладывают пластический материал так, что он ограничен снаружи боковой стенкой верхнечелюстной пазухи, сверху костной створкой боковой стенки носа и мембраной Шнейдера, снизу дном вернечелюстной пазухи. Пациентка Л. 56 лет, обратилась в стоматологическое отделение поликлиники с жалобами на неэффективность выполненного ранее синуслифтинга, миграцию дентальных имплантов. При осмотре ротовой полости отмечается выраженный спаечный процесс в проекции послеоперационного рубца. После санации ротовой полости, последующего наблюдения пациентка направлена к оториноларингологу для выполнения операции эндоназального синуслифтинга. Пациентке выполнена операция по заявляемому способу. Инраоперационно и в послеоперационном периоде осложнений не было. Состояние пациентки соответствовало тяжести и сроку проведенного вмешательства. Нежелательных воспалительных и иммунных реакций в послеоперационном периоде не наблюдали. На компьютерной томограмме через 6 месяцев после операции высота костной ткани в области зоны мигрировавших имплантов не менее 10 мм. Пациентке вновь установлены 4 дентальных имплантата, целостность зубных рядов восстановлена. При наблюдении в течение трех лет миграции дентальных имплантатов не наблюдали.There were no intraoperative and postoperative complications.
