В настоящее время при лечении пациентов с адентией используют дентальные имплантаты в качестве опор для ортопедических конструкций, замещающих собой отсутствующие зубы. Для успешной интеграции имплантатов и длительного их функционирования необходимо создать в области выхода супраструктур имплантатов (в периимплантатной зоне) так называемый тканевой барьер - неподвижные мягкие ткани достаточной толщины для формирования биологической ширины в коронарном направлении. Данную проблему решает способ одномоментного изготовления направляющего хирургического шаблона для установки дентальных имплантатов и индивидуальных постоянных абатментов (заявка RU №2018103320, А61С 8/00, 29.01.2018, Лысов А.Д. и др.). Авторы предлагают использовать протокол лечения, предусматривающий установку постоянного индивидуального абатмента в день операции и больше не извлекать его, тем самым, не травмируя сформированную после операции мягкотканную буферную зону и в дальнейшем проводить протезирование с уровня абатмента. Сущность способа заключается в том, что в области альвеолярного отростка отступив в апекальном направлении от слизисто-десневой границы на 6-12 мм делают разрез и формируют расщепленный лоскут слизистой оболочки, при этом ткани рассекают от линии разреза к челюсти. Сущность способа заключается в том, что производят разрезание мягких тканей до надкостницы по центральной продольной оси челюсти. Проводят забора биоптата толщиной 2-3 мм и длиной, превышающей величину радиуса имплантата на 3 мм. Придают биоптату форму равнобедренного треугольника, основание которого имеет длину 3-4 мм, а высота имеет размер, который на 2-3 мм превышает размер радиуса имплантата. Производят вначале сквозной поперечный разрез биоптата длиной, превышающей величину радиуса имплантата на 1 мм. Эпителиальный слой этого лоскута иссекают шаровидным алмазным бором, надежно удерживая сам лоскут атравматичным пинцетом. Затем окончатый лоскут ротируют на внутреннюю поверхность основного лоскута и фиксируют одиночным П-образным швом (Фиг. 5). В последующем вестибулярный лоскут укладывают по альвеолярному гребню, плотно охватывая шейку постоянного индивидуального абатмента, тем самым создают необходимый объем мягкотканной буферной зоны. At the same time, the mucous membrane from the lingual or palatine side, after preliminary excision of the epithelial layer with a spherical diamond bur, is squeezed in the lingual or palatine direction by an individual abutment, turning the edge of the mucous membrane under itself, forming a roller, in order to increase the volume of soft tissues (Fig. 3) . Then, at the coronal end of the vestibular flap, two vertical sections are made parallel to each other, the distance between which is equal to 2/3 of the diameter of the permanent individual abutment in the mesio-distal direction, and their length is 2/3 of the diameter of the same abutment in the vestibular-lingual or vestibular-palatal direction thereby forming a fenestrated flap (Fig. 4). The epithelial layer of this flap is excised with a spherical diamond bur, reliably holding the flap with atraumatic forceps. Then, the fenestrated flap is rotated on the inner surface of the main flap and fixed with a single U-shaped suture (Fig. 5). Subsequently, the vestibular flap is laid along the alveolar ridge, tightly covering the neck of a permanent individual abutment, thereby creating the necessary volume of soft tissue buffer zone. The wound is sutured with simple interrupted sutures (Fig. 6).

С помощью компьютерной томографии определено достаточное количество костной ткани в позиции 1.2 для установки дентального имплантата.Conducted a comprehensive survey.
