Description
Oppfinnelsen vedrører en farmasøytisk formulering for anvendelse ved behandling av brystkreft ved intramuskulær injeksjon, hvor den farmasøytiske formuleringen omfatter fulvestrant, en farmasøytisk akseptabel alkohol som er en blanding av 10 vekt % etanol pr. volum av formuleringen og 10 vekt % benzylalkohol pr. volum av formuleringen og formuleringen inneholder 15 vekt % benzylbenzoat pr. volum av formuleringen og en tilstrekkelig mengde av ricinoleatkonstituent for fremstilling av en formulering med minst 45 mgml-<1>fulvestrant, hvor ricinoleatkonstituenten er ricinusolje og hvor det totale volumet av formuleringen er 6 ml eller mindre.
Østrogensvinn er fundamentalt for behandling av mange godartede og ond-artede sykdommer i bryst og reproduksjonskanal. Hos premenopausale kvinner, oppnås dette ved fjerning av ovariefunksjonen via kirurgiske, radioterapeutiske eller medisinske midler, og, hos postmenopausale kvinner, ved anvendelse av aromatase-inhibitorer.
En alternativ løsning på østrogen-tilbakegang er å antagonisere østrogener med antiøstrogener. Dette er droger som binder seg til og konkurrerer om østrogenreseptorer (ER) som er tilstede i kjernene i østrogenresponsivt vev. Konvensjonelle ikke-steroidale antiøstrogener, for eksempel tamoksifen, konkurrerer effektivt om ER-binding, men deres effektivitet blir ofte begrenset av den partielle agonisme som de viser, noe som resulterer i en ufullstendig blokade av østrogenmediert aktivitet (Furr og Jordan 1984, May og Westley 1987).
Potensialet for ikke-steroidale antiøstrogener til å vise agonistiske egenskaper påskyndet forskningen etter nye forbindelser som ville binde ER med høy affinitet uten å aktivere noen av de normale transkripsjonene hormonresponser og påfølgende manifestasjoner av østrogener. Slike molekyler ville være "rene" anti-østrogener, klart skjelnet fra tamoksifen-lignende ligander og med evne til å fremkalle fullstendig fjerning av de trofiske effekter av østrogener. Slike forbindelser refereres til som Estrogen Receptor-Downregulators (E.R.D.). Rasjonalet for design og testing av nye, rene antiøstrogener er beskrevet hos Bowler et al. 1989, Wakeling 1990a, 1990b, 1990c. Wakeling og Bowler 1987, 1988.
Steroidale analoger av østradiol med en alkylsulfinylsidekjede i 7<x-posisjon, tilveiebragte de første eksempler på forbindelser blottet for østrogenaktivitet (Bowleret al. 1989). Én avdisse, 7a-[9-(4,4,5,5,5-pentafluorpentylsulfinyl)nonyl]- østra-1,3,5(10)-trien-3,17p-diol, ble valgt for intensivt studium på basis av sin rene østrogen-antagonist-akivitet og signifikant øket antiøstrogenpotens i forhold til andre tilgjengelige antiøstrogener. In wfro-funn og tidlig klinisk erfaring med 7a-[9-(4,4,5,5,5-pentafluorpentylsulfinyl)nonyl]østra-1,3,5(10)-trien-3,17p-diol har fremkalt interesse i utvikling av drogen som et terapeutisk middel for østrogenavhengige indikasjoner slik som brystkreft og visse godartede gynekologiske tilstander.
7a-[9-(4,4,5,5,5-pentafluorpentylsulfinyl)nonyl]østra-1,3,5(10)-trien-3,170-diol eller IC1182 780 er blitt allokert det internasjonale ikke-proprietære navn fulvestrant, som anvendes i det følgende. Når det refereres til fulvestrant, inkluderer vi farmasøytisk akseptable salter derav og eventuelle solvater av hvert av dem.
Fulvestrant binder seg til ER med en affinitet lik den for østradiol og blokkerer fullstendig den vekststimulerende virkning av østradiol på humane brystkreftceller in vitro; det er mer potent og mer effektivt enn tamoksifen i dette henseende. Fulvestrant blokkerer fullstendig den uterotrofe virkning av østradiol hos rotter, mus og aper, og blokkerer også den uterotrofiske aktivitet hos tamoksifen.
Fordi fulvestrant ikke har noe av den østrogenlignende stimulerende aktivitet som er karakteristisk for klinisk tilgjengelige antiøstrogener, for eksempel tamoksifen eller toremifen, kan det tilby forbedret terapeutisk aktivitetkarakterisertved hurtigere, mer fullstendig eller langvarigere tumorregresjon; en lavere forekomst eller rate av utvikling av resistens mot behandling; og en reduksjon av tumor-angrep.
I intakte voksne rotter, oppnår fulvestrant maksimal regresjon av uterus ved en dose som ikke har ugunstig innvirkning på bentetthet eller fører til øket gonodotrofinsekresjon. Om det også er tilfelle hos mennesker, kunne disse funn være av ekstrem viktighet klinisk sett. Redusert bentetthet begrenser varigheten av østrogen-fjernende behandling for endometriose. Fulvestrant blokkerer ikke hypotalamisk ER. Østrogenfjerning forårsaker eller forverrer også varmebølge og andre menopausale symptomer; fulvestrant vil ikke forårsake slike effekter fordi det ikke krysser blod/hjernebarrieren.
Europeisk patentsøknad nr. 0 138 504 åpenbarer at visse steroid-derivater er effektive antiøstrogene midler. Beskrivelsen inkluderer informasjon vedrørende fremstilling av steroid-derivatene. Spesielt er det beskrivelsen i eksempel 35 av forbindelsen 7a-[9-(4,4,5,5,5-pentafluorpentylsulfinyl)nonyl]østra-1,3,5(10)-trien-3,17p-diol, den forbindelse som er spesifikt nevnt i krav 4. Det beskrives også at forbindelsene i henhold til nevnte oppfinnelse kan tilveiebringes for bruk i form av et farmasøytisk preparat som omfatter et steroid-derivat i henhold til oppfinnelsen sammen med et farmasøytisk akseptabelt fortynningsmiddel eller en slik bærer. Det angis deri at preparatet kan være i en form egnet for oral eller parenteral administrering.
Fulvestrant viser, sammen med andre steroid-baserte forbindelse, visse fysikalske egenskaper som gjør at det er vanskelig å sette sammen disse forbindelser. Fulvestrant er et spesielt lipofilt molekyl, selv i sammenligning med andre steroidale forbindelser, og dets vannløselighet er ekstremt lav ved rundt 10 ngrnl-1 (dette er en antagelse fra en vann/løsningsmiddelblandingssolute siden målinger som går så lavt ikke kunne oppnås i en solute av bare vann).
For tiden er det et antall injiserbare steroidale blandinger med forsinket frigivelse som er blitt kommersialisert. Vanligvis benytter disse blandinger olje som løsningsmiddel, og ytterligere eksipienter kan være tilstede. I tabell 1 nedenunder, er det beskrevet noen få kommersielle injiserbare blandinger med forsinket frigivelse.
I blandingene i tabell 1, anvendes et antall forskjellige oljer for å solubilisere forbindelsen og ytterligere eksipienter så som benzylbenzoat, benzylalkohol og etanol er blitt anvendt. Volumer av olje som trenges for å solubilisere den steroid-aktive ingrediens er lave. Utstrakt frigivelse er oppnåelig for tidsrom på 1-8 uker.
I US 5 183 814, eksempel 3, beskrives en oljebasert injeksjonsblanding av fulvestrant som omfatter 50 mg fulvestrant, 400 mg benzylalkohol og tilstrekkelig ricinusolje til å bringe løsningen til et volum på 1 ml. Produksjon i kommersiell målestokk av en blanding som beskrevet i US 5 183 814, vil bli komplisert av den høye alkoholkonsentrasjon. Derfor er det nødvendig å senke alkohol-konsentrasjonen i fulvestrantblandinger mens man forhindrer utfelling av fulvestrant fra blandingen.
Tabell 2 viser løseligheten av fulvestrant i et antall forskjellige løsnings-midler.
Som det kan sees, er fulvestrant signifikant mer løselig i ricinusolje enn
noen av de andre oljer som er testet. Den større solvateringsevne hos ricinusolje for steroidale forbindelser er kjent og tilskrives det høye antall hydroksygrupper av ricinolsyre, som er hovedbestanddelen i fettsyrene i de triglycerider som er tilstede i ricinusolje - se (Riffkin et al., J. Pharm. Sei., (1964), 53, 891).
Imidlertid har vi, selv ved anvendelse av det beste oljebaserte løsnings-middel, ricinusolje, funnet at det ikke er mulig å oppløse fulvestrant i et oljebasert løsningsmiddel alene for å oppnå en konsentrasjon som er høy nok til å dosere en pasient i en lav-volum-injeksjon og oppnå en terapeutisk signifikant frigivelsesrate. For å oppnå en terapeutisk signifikant frigjøringsrate, ville mengden av fulvestrant som trenges kreve at blandingsvolumet var stort, minst 10 ml. Dette krever at legen injiserer et overordentlig stort volum av blanding for å administrere en dose som er signifikant høy nok for humanterapi.
For tiden anbefaler veiledninger at ikke mer enn 5 ml væske blir injisert intramuskulært i en enkelt injeksjon. Farmakologisk aktive doser som kreves for en 1 måneds langtvirkende depotblanding av fulvestrant er rundt 250 mg. Derfor, når det oppløses i akkurat ricinusolje, vil fulvestrant måtte bli administrert i minst 10 ml ricinusolje.
Tilsetning av organiske løsningsmidler som fulvestrant er lettløselige i, og som er blandbare med ricinusolje, kan anvendes, for eksempel en alkohol. Med tilsetning av høye konsentrasjoner av en alkohol, er det oppnåelig med konsentrasjoner av >50 mgml-<1>av fulvestrant i en ricinusoljeblanding, noe som gir injeksjonsvolumer på <5 ml - se tabell 3 nedenunder. Vi har overraskende funnet at innføring av et ikke-vandig esterløsningsmiddel som er blandbart i ricinusolje og en alkohol overraskende gjør solubiliseringen av fulvestrant i en konsentrasjon av minst 50 mgml-<1>lettere - se tabell 3 nedenunder. Funnet er overraskende siden løseligheten av fulvestrant i ikke-vandige esterløsningsmidler - se tabell 2 ovenfor - er signifikant lavere enn løseligheten av fulvestrant i ikke-vandige esterløsnings-midler - se tabell 2 ovenfor - er signifikant lavere enn løseligheten av fulvestrant i en alkohol. Løseligheten av fulvestant er også lavere i ikke-vandige ester-løsningsmidler enn hva løseligheten av fulvestrant er i ricinusolje.
Oppfinnelsen vedrører en farmasøytisk formulering for anvendelse ved behandling av brystkreft ved intramuskulær injeksjon, hvor den farmasøytiske formuleringen omfatter fulvestrant, en farmasøytisk akseptabel alkohol som er en blanding av 10 vekt % etanol pr. volum av formuleringen og 10 vekt % benzylalkohol pr. volum av formuleringen og formuleringen inneholder 15 vekt % benzylbenzoat pr. volum av formuleringen og en tilstrekkelig mengde av ricinoleatkonstituent for fremstilling av en formulering med minst 45 mgml-<1>fulvestrant, hvor ricinoleatkonstituenten er ricinusolje og hvor det totale volumet av formuleringen er 6 ml eller mindre.
Foretrukket av en farmasøytisk formulering som over hvor den totale mengden fulvestrant i formuleringen er 250 mg eller mer og det totale volumet av formuleringen er 6 ml eller mindre.
Ytterligere foretrukket er en farmasøytisk formulering som over hvor den totale mengden av fulvestrant i formuleringen er 250 mg og det totale volumet av formuleringen er 5 til 5,25 ml.
Den farmasøytisk akseptable alkohol er en blanding av to alkoholer, 10 vekt/vol.% etanol og 10 vekt/vol.% benzylalkohol.
Det farmasøytisk akseptable ikke-vandige esterløsningsmiddel er benzylbenzoat.
Ricinoleatvehikkelen bør fortrinnsvis være tilstede i preparatet i en andel av minst 30 vekt% pr. volumdel av blandingen, ideelt minst 40% eller minst 50% i vekt pr. volumdel av blanding.
Det vil forstås av fagmannen at den farmasøytisk akseptable alkohol vil være av en kvalitet som er slik at den vil tilfredsstille farmakopestandarder (slik som beskrevet i US-, britisk, europeisk og japansk farmakope) og vil som sådanne inneholde noe vann og eventuelt andre organiske løsningsmidler, for eksempel etanol i US-farmakope inneholder ikke mindre enn 94,9 volum% og ikke mer enn 96,0 volum% etanol målt ved 15,56°C. Dehydratisert alkohol i US-farmakope inneholder ikke mindre enn 99,5% etanol i volum målt ved 15,56°C.
Det vil forstås av fagmannen på området at det farmasøytisk akseptable ikke-vandige esterløsningsmiddel vil være av en kvalitet slik at det vil tilfredsstille farmakopestandarder (for eksempel som beskrevet i US-, britisk, europeisk og japansk farmakope).
Ved anvendelse av betegnelsen ricinoleatvehikkel, mener vi en olje som har som en andel (minst 20%, 30%, 40%, 50%, 60%, 70%, 80%, 90% og 95 vek/vol.%) av sin sammensetning som triglycerider av ricinolsyre. Ricinoleatvehikkelen kan være en syntetisk olje eller er bekvemt ricinusolje, ideelt av farmakopestandard som beskrevet ovenfor.
Vi har overraskende funnet at de ovenstående blandinger i henhold til oppfinnelsen tilveiebringer, etter intramuskulær injeksjon, tilfredsstillende frigjøring av fulvestrant over et utstrakt tidsrom.
Dette funn er virkelig overraskende av de følgende grunner.
1. Tidligere testet av oss er intramuskulære injeksjoner av fulvestrant av en vandig suspensjon. Vi har funnet utstrakt lokal vev-irritasjon ved injeksjonsstedet så vel som en dårlig frigjøringsprofil. Det menes at vevirritasjonen/inflammasjonen skyldtes nærvær av fulvestrant i form av faste partikler. Frigjøringsprofilen viste seg å være bestemt av graden av inflammasjon/irritasjon som er tilstede i injeksjonsstedet, og dette var variabelt og vanskelig å styre. Dessuten var fulvestrantfrigjøringsraten ikke tilstrekkelig høy til å være klinisk signifikant. 2. Våre funn fra studier som anvender<14>C-merket benzylalkohol viser at den sprer seg hurtig fra injeksjonsstedet og blir borte fra kroppen i løpet av 24 timer fra administreringen.
Det ville forventes at etanol vil spre seg like hurtig, om ikke hurtigere, fra injeksjonsstedet.
Det er kjent at benzylbenzoat blir metabolisert ved konjugasjon til glycin for å danne hippursyre i menneskets lever og skilles ut i urinen - Martindale : The Extra Pharmacopoeia 32. utgave, side 1103, og derfor er det usannsynlig at benzylbenzoat, når det anvendes, er tilstede ved injeksjonsstedet i hele den utstrakte frigjøringsperiode.
Vi har funnet at til tross for den hurtige eliminering av de ytterligere solubiliserende eksipienter, dvs. alkoholen og det farmasøytisk akseptable ikke-vandige esterløsningsmddel, fra preparatvehikkelen og injeksjonsstedet etter injeksjon av preparatet, kan utstrakt frigjøring ved terapeutisk signifikante nivåer av fulvestrant over et utstrakt tidsrom fremdeles oppnås med preparatet i henhold til oppfinnelsen.
Ved anvendelse av uttrykket "terapeutisk signifikante nivåer" menes at blodplasmakonsentrasjoner på minst 2,5 ngml"<1>, ideelt minst 3 ngml"<1>, minst 8,5 ngrnl-1 og opp til 12 ngml-<1>av fulvestrant oppnås hos pasienten. Fortrinnsvis bør blodplasmanivåer være mindre enn 15 ngml-<1>.
Ved anvendelse av betegnelsen "utstrakt frigjøring" mener vi minst to uker, minst tre uker og fortrinnsvis minst fire uker med kontinuerlig frigjøring av fulvestrant som oppnås. Det vil forstås at den behandlende lege kan ønske å administrere den intramuskulære injeksjon som en oppdelt dose, dvs. en 5 ml blanding blir sekvensielt administrert i to separate injeksjoner på 2,5 ml, noe som er et ytterligere trekk ifølge beskrivelsen.
Ganske enkelt å solubilisere fulvestrant oljebasert flytende blanding er ikke forutsigelig for en god frigjøringsprofil eller mangel på utfelling av droge eter injeksjon ved injeksjonsstedet.
Tabell 3 viser løseligheten av fulvestrant i en ricinusoljevehikkel som i tillegg inneholder alkoholer, etanol og benzylalkohol, med eller uten benzylbenzoat. Resultatene viser tydelig den positive effekt av benzylbenzoat på fulvestrant-løselighet i ricinusolje, til tross for at fulvestrant har lavere løselighet i benzylbenzoat enn i enten alkohol eller ricinusolje.
Følgende tabell 4 viser løseligheten av fulvestrant i et område av oljebaserte preparater som inneholder de samme mengder av alkohol og benzylbenzoat, men hvor oljen er endret. Dataene viser også løselighet av fulvestrant etter fjerning av alkoholene. Effekt av blanding på utfelling av fulvestrant ved injeksjonsstedet
Utfelling av fulvestrant og frigjøringsprofilen ble bestemt med de ovennevnte blandinger i et in wVo-kaninstudium.
Fig. 1 viser frigjøringsprofilen in vivo av de fire blandinger fra den annen del av tabell 4 og viser effekten av den fikserte oljekomponent på fulvestrant-plasmaprofil over fem dager etter intramuskulær administrering i kaniner (data normalisert til 50 mg pr. 3 kg; gjennomsnitt gitt; antall dyr pr. tidspunkt = 8, plasmaprøver analysert for fulvestrantinnhold ved anvendelse av 1c-ms/ms-påvisning etter løsningsmiddelekstraksjon). Som det kan sees, viste ricinusoljeblandingen en spesielt jevn frigjøringsprofil uten noe tegn på utfelling av fulvestrant ved injeksjonsstedet.
Et trekk ved oppfinnelsen er anvendelse av fulvestrant i fremstilling av et farmasøytisk preparat, for behandling av brystkreft. Ytterligere eksipienter som vanligvis anvendes på dette preparatområde inkluderende for eksempel et antioksidant-konserveringsmiddel, et fargemiddel eller et overflateaktivt middel, kan anvendes. En foretrukken valgfri eksipient er et overflateaktivt middel.
Som beskrevet ovenfor, er fulvestrant nyttig ved behandling av østrogenavhengige indikasjoner slik som brystkreft og gynekologiske lidelser, for eksempel endometriose.
I tillegg til fulvestrant, er en annen lignende type molekyl for tiden under klinisk utprøving. SH-646 (11p-fluor-7a-(14,14,15,15,15-pentafluor-6-metyl-10-tia-6-azapentadecyl)estra-1,3,5(10)-trien-3,17p-diol) er også formentlig en forbindelse med den samme handlingsmodus som fulvestrant og har en svært lik kjemisk struktur. Det menes at forbindelsen også vil dele lignende fysikalske egenskaper med fulvestrant, og derfor vil foreliggende oppfinnelse også ha anvendelse sammen med denne forbindelse.
Preparateksempel
Fulvestrant blandes med alkohol og benzylalkohol, og det røres inntil fullstendig oppløsning. Benzylbenzoat tilsettes, og løsningen lages opp sluttvekt med ricinusolje og omrøres (for bekvemmelighetsskyld anvendes vekt istedenfor volum ved å benytte vekt:volum-forholdet). Bulkløsningen dekkes med nitrogen. Løsningen destilleres ved filtrering under anvendelse av ett eller to filtre med porøsitet 0,2 nm. Det sterile filtrat holdes under et nitrogendekke mens det fylles under aseptiske betingelser inn i vaskede og depyrogeniserte, sterile primær-beholdere, for eksempel glassbeholdere eller fornhåndsfylte sprøyter. Et overskudd inkluderes i den primære pakke for å gjøre det lettere å fjerne dosevolumet. De primære pakker dekkes med sterilt nitrogen før aseptisk forsegling.
