Verfahren und Reagens zur spezifischen Bestimmung von
Oreatinin
Die Erfindung betrifft ein neues Verfahren und ein Reagens zur spezifischen enzymatischen Bestimmung von Creatinin.
In der klinischen Chemie, insbesondere für die Punktionsdiagnostik
der Niere, spielt die Bestimmung des Creatinins eine wichtige Rolle. Die Bestimmung des Creatinins im Serum, bzw.
im sogenannten Clearance-Test, die gleichzeitige Bestimmung im Serum und Harn gehört zu den Standarduntersuchungsmethoden
im klinischen Labor.
Die ;bisher übliche Bestimmungsmethode beruht auf der von M. Jafte gefundenen Farbreaktion von Creatinin mit Pikrinsäure
im alkalischen Milieu. Hierbei wird nach saurer Enteiweißung der Probe (z.B. mit Trichloressigsäure oder mit
Pikrinsäure) im überstand nach Zugabe von Pikrinsäure und Alkalisieren eine Färbung entwickelt und photometrisch gemessen.
Ein wesentlicher Nachteil dieses an. sich einfachen
Verfahrens besteht jedoch darin, daß es nicht für Creatinin spezifisch ist.

Eine Reihe von Modifikationen der Jaffe-Reaktion verbessert zwar
die Präzision und die Durchführbarkeit, ohne diesen prinzipiellen Mangel zu beheben. Auch blieb die Jaffe-Methode
sehr störanfällig und schon geringe Verschiebungen in der H- bzw. OH-Ionenkonzentration führen zu einer Verän-'
derung der Farbtiefe. ' -
Eine weitere bekannte Methode wandelt Creatinin unter Zusatz von o-Nitrobenzaldehyd in Methylguanidin um, welches dann
nach der Sakaguchi-Reaktion bestimmt wird. Weiter wurde auch eine Farbreaktion zwischen Creatinin und Kalium-Quecksilber-"
Thiocyanat beschrieben. Beide Methoden erwiesen sich jedoch für das klinische Labor als ungeeignet.
Von Dubos und Miller (J.biol.Chem. 121, 457 (1937)) wurde
. eine Modifikation der Jaffe-Reaktion beschrieben, bei der
mit einem Rohextrakt eines bestimmten Bakteriums in einem Aliquot einer Probe Creatinin abgebaut, während der Rest der
Probe unbehandelt blieb. In beiden Probeteilen wurde dann die Bestimmung nach Jaffe durchgeführt und aus der Differenz
der Extinktionen der Creatinin-Gehalt bestimmt. Diese Methode ist zwar sehr spezifisch, die Durchführung ist jedoch recht
umständlich und erfordert es, das Bacterium laufend zu züch- \ ten.
Es besteht daher Bedarf an einem einfachen, aber spezifischen Test für Creatinin.
Die Erfindung löst dieses Problem durch ein Verfahren zur spezifischen Bestimmung von Creatinin, welches darin besteht,
daß eine wäßrige, Creatinin enthaltende Lösung mit Creatininamidohydrolase bei einem pH-Wert zwischen etwa 7>5
und 9 inkubiert und entweder gebildetes Creatin oder die Abnahme an Cr.eatinin in an sich bekannter Weise bestimmt wird.
Das für das erfindungsgemäße Verfahren verwendete Enzym Creatininamidohydrolase und ein Verfahren zu seiner Gewinnung

sind in der gleichzeitig unter der internen Nummer ,1749 eingereichten Patentanmeldung P

noch nicht "beschrieben"wurde, katalysiert die Reaktion!

Das gebildete Creatin kann nach bekannten Methoden direkt
gemessen werden, beispielsweise durch Phosphorylierung mit Creatinkinase und Adenosintriphosphat (ΑΪΡ) unter Bildung von
Adenosindiphosphat, welches in bekannter Weise bestimmt wird.
Eins andere Möglichkeit besteht darin, die Abnahme des Creatinine nach Jaffe zu messen.
Das erfindungsgemäße Verfahren wird vorzugsweise bei einem
pH-Wert zwischen 7,8 und 8,5 durchgeführt. Zur Einstellung des pH-Wertes können beliebige Puffer verwendet werden. Es
ist jedoch darauf zu achten, daß der verwendete Puffer die angeschlossene Bestimmungsmethode nicht stört. Wenn beispielsweise
anschließend nach Jaffe die Creatiniii-Differenz
bestimmt wird, soll ein Puffer verwendet werden, welcher die Jaffe-Reaktion nicht herabsetzt.Ungünstig sind beispielsweise
Glycin- und Glycylglycin-Puffer; nicht störend sind Phosphat-
und Pyrophosphat.-Puf.fer. Falls jedoch nicht nach Jaffe bestimmt
wird, sondern das gebildete C^eatin mit Creatinkinase und ATP bestimmt wird, so können auch G-lycir.-Puffer
und ähnliche Puffer verwendet werden.
Beim erfindungsgemäßen Verfahren ist eine vorherige En'teiweißung
der Probe nicht erforderlich. Eine Enteiweißung kann
jedoch für die nachfolgende Bestimmung des Creatins zweckmäßig sein.
Gemäß einer besonderen Au sführungs forin des erf j ndungs gemäßen
Verfahrens wird gebildetes Groatin unter Verwendung des Enzyms

Creatinase in Sarcosin und Harnstoff überführt und letzterer nach bekannten Methoden bestimmt. Das Enzym Creatinase ist
in der oben erwähnten, gleichzeitig eingereichten Anmeldung beschrieben und kann systematisch auch als Creatinamidinohydrolase
bezeichnet werden.
Diese Ausführungsform des erfindungsgemäßen Verfahrens wird
vorzugsweise durchgeführt, indem ein Creatininase und Creatinase gemischt enthaltendes Enzympräparat für die Bestimmung
eingesetzt wird.
Wird gebildetes Creatin in an sich bekannter Weise unter Verwendung
von Creatinase und ATP bestimmt, so ist eine Enteiweißung vorteilhaft, da hierdurch die Empfindlichkeit des
Tests erhöht wird. Diese größere Empfindlichkeit beruht darauf j daß bei Enteiweißung konzentrierter gearbeitet werden
kann, so daß die Extinktionsdifferenz, bezogen auf eine bestimmte Creatinmenge, vergrößert wird.
Die Verwendung eines Kombinationsenzyms, enthaltend Creatininamidohydrolase
und Creatinase ist auch dann von Vorteil, wenn die Bestimmung des Creatini-ns nach Jaffe erfolgt, da durch
die Creatinase das Gleichgewicht zugunsten der Bildung von ) Creatin verschoben wird.
Die Erfindung schafft auch eine neue Reagentienkombination zur spezifischen Bestimmung von Creatinin. Die erfindungsgemäße
Heagentienkoinbination besteht aus
1. Creatininstandard
2. Pikrinsäure
3. Katronlauge und
4. Creatininamidohydrolase allein oder zusammen mit Puffer sowie gegebenenfalls Creatinase,
in vor Gebrauch unvermischbarem Zustand.
Eine weitere ei-findungsgemäße Reagentienkombination besteht

1. Puffer, reduz.Nicotinamid-adenin-dinucleotid
(NADH), ATP und Phosphoenolpyruvat (PEP)
2. Lactatdehydrogenase (IDH), Pyruvatkinase (PK) und MgCl2,
3. Creatinkinäse (CK) und 4· Creatininaniidohydroläse
in vor Gebrauch unvermischbarem Zustand.
Das erfindungsgemäße Verfahren und die erfindungsgemäße Reagentienkombination
schaffen erstmals die Möglichkeit einer spezifischen Creatinin-Bestimmung, die sich auch zur Routineanwendung
im klinischen labor eignet und eine ausreichende Empfindlichkeit für praktische Zwecke besitzt. Die einzige
bisher bekannte spezifische Bestimmungsmethode nach Miller und..Dubos war hingegen für die Verwendung zur Routineuntersuchung
unbrauchbar (vgl. R.J.Henry, Clinical Chemistry, 1964, Seite 288).
Die folgenden Beispiele erläutern das erfindungsgemäße Vorfahren
und die erfindungsgemäße Testkombination weiter.
Bestimmung unter anschließender Anwendung der Jäffe-Reaktion,
1,0 ml einer Creatinin-haltigen Probe (Serum oder verdünnter
Urin) werden in 0,050 bis 0,2M Puffer (besonders geeignet
Pyrop]"osphat-, Phosphat-, Diäthanolamin/HOl- oder Glycylglyej.u/
HCl-Puffer) mit Creatininamidohydrolase in einer Menge von
mindestens 5 U/Test (1 U= lyuMol Substratumsatz/min) und
Creatinase in einer Menge von mindestens 0,8 U/Terst bei 370C 45 bis 60 Minuten inkubiert. Dann wird mit 3M 'friehlcu—
essigsäure enteiweißt, zusammen mit einer Probe, der kein
Enzyingemisch zugesetzt wurde. Mit dem Überstand der beiden
Proben wirr3 die Jaffo-Reaktion durchgeführt. Aus der Extinktionsdifferftns
der beiden Proben bei 546 nm ergibt, sich d.u-Creatinin-Menge

Die Farbreaktion nach Jaffe wird durchgeführt durch Zusatz
von Pikrinsäure und NaOH, 15 Minuten Stehenlassen bei Raumtemperatur
und Messen in einer Küvette mit 2 cm Schichtdicke bei 5.46 nni.
In der beigefügten Zeichnung zeigt Pig. 1 in graphischer Darstellung
die Linearität zwischen mg fo Creatinin und der Extinktionsdifferenz
bei 546 nm.
Messung des Creatine mittels Creatinkinase
Das Prinzip dieser Ausführungsform besteht darin, daß mit Hilfe
von Creatininase das Greatinin zu Creatin hydrolysiert und
gebildetes Creatin nach M.L. Tanzer (J.of Biol.Ohem. 2J5£>
S.3201 .(1959)) mit Creatinkinase und ATP phosphoryliert, gebildetes
ADP mit PEP und PK in ATP umgewandelt und dabei entstandenes Pyruvat mit IiADH und lactatdehydrogenase zu Lactat
reduziert wird unter Bildimg von IAD. Die Umsetzung von 1 Mol WADH (Messung bei 334? 340 oder 366nm). entspricht der Gegenwart
von 1 Mol Creatinin.
Zur Durchführung wird ein Reagentiengemisch, das HADH, ATP,
PEP und Magnesiumchlorid in 0,11M Puffer bei pH 9,0 enthält,
" mit Lactatdehydrogena.se (LDH), Pyruvatkinase (PK) gemischt
und auf 25°C temperiert. Dann werden Serum und Creatinkinase zugesetzt und bei der angegebenen Temperatur maximal 30 Minuten
inkubiert. Durch Zugabe von mindestens 5 U Creatininase wird die Reaktion gestartet. Der Extinktionsabfall wird registriert.
Die Reaktionsdauer beträgt je nach Creatinin-Gehalt 40 biß 90 Minuten. Der Creatinin-G-ehalt errechnet sich
direkt Uher dori molaren Extinktionskoeffizienten von IJADH.

Vorteile dieser Ausführungsform sind.Einsparung an Untersuchungsmaterial
um den Faktor 10 gegenüber der Jaffe-Methode, ein Serum-Leerwert ist nicht erforderlich, keine Enteiweißung,
kein -Zentrifugieren nötig.

- PatentansprücheVerfahren zur spezifischen Bestimmung von Creatinin, dadurch gekennzeichnet, daß eine wäßrige, Creatiniii enthaltende Lösung mit Creatininamidohydrolase bei einem pH-Wert zwischen etwa 7,5 und 9 inkubiert und entweder gebildetes Creatin oder die Abnahme an Creatinin in an sich bekannter Weise bestimmt wird.
- 2. Verfahren nach Anspruch 1, dadurch gekennzeichnet, daß Phosphat- oder Pyrophosphat-Puffer verwendet und die Creatinin-Differenz durch Messung der mit Pikrinsäure und" Natronlauge gebildeten Färbung bestimmt wird.
- 3. Verfahren nach Anspruch' 1, dadurch gekennzeichnet, daß gebildetes Creatin durch Phosphorylierung mit Creatinkinase und Adenosintriphosphat und Messung des .gebildeten Adenosindiphosphats in bekannter Weise bestimmt wird.
- 4. Verfahren nach Anspruch 1, dadurch gekennzeichnet, daß gebildetes Creatin in Gegenwart von Creatinase unter Harnstoffbildung gespalten und gebildeter Harnstoff nach be-. kannten Methoden bestimmt wird.)
- 5. Verfahren nach.einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, daß die Umsetzung mit Creatinase bei pH 7,8 bis 8,3 durchgeführt wird.
- 6. Verfahren nach Anspruch 3, dadurch gekennzeichnet, daß die zu bestimmende Probe enteiweißt wird vor der Messung des Creatins unter Verwendung von Creatinkinase, -
- 7. Verfahren nach Anspruch 2, dadurch gekennzeichnet, daß Creatininaraj-dohydrolase zusammen mit Creatinaae zugesetzt wird.209847/0981 BAD ORIGINAL21222S5
- 8. Reagentienkombination zur spezifischen -Beatlmmung con Creatinin, dadurch gekennzeichnet, daß sie aus1. Creatinin-Standard2. Pikrinsäure3. Natronlauge .4. - 9. Reagentienkorabination zur spezifischen Bestimmung von Creatinin, dadurch gekennzeichnet, daß eie aus1. Puffer, NADH, ATP und Phosphoenolpyruvat2.
