本发明涉及超声显影剂,它由包裹不溶性气体的蛋白质的(Proteinaceous)微球组成并涉及其制备和使用方法。
诊断超声显像是根据下述原理,即可以将声波的能量集中在要检查的区域并以在该区域产生图像的方式表现。将超声扫描仪放到要显像区域的身体表面上并将声波形式的超声能对准该区域。扫描仪检测反射的声波并将数据转化为电视图像。当超声能通过一种物质传播时,反射的能量的数量依赖传播的速度和物质的声学性质。在不同声学密度的界面(如液-固或液-气),物质声学性质的改变(例如声阻抗的改变)是最明显的。所以,当超声能直接通过组织时,器官结构产生通过超声扫描仪检测的声反射信号。可以通过适当使用造影剂来增强这些信号。
超声显影剂特殊的重要性在于使用了气体,因为气体具有作为超声反射层的功效。共振气泡散射声音的功效是大小相同固体颗粒的上千倍。Ophir和Parker叙述了两种含气体的显影剂即(1)游离气泡,和(2)用胶囊包裹的气泡(Uetra sound in Medicine andBiology 15(4):319—333,1989)。然而,适宜大小的游离气泡存活时间太短以至于不能有效地长时间在体内应用(Meltzer,等Ultrasound in Medicine and Biology 6:263—269,1980)。Ophir和Parker指出开发用胶囊包裹的气泡是克服该问题的尝试。

Bichon等(EPA90/810367)和Schneider等(Inv.Radiol.27:134—139,1992)描叙了多孔(5—2000nm孔径大小)聚合“微球”的制备。他们在该欧洲专利申请中报告道“微球外壳的微孔结构是弹性因素,即微球可欣然接受压力的变化而不损坏。”
Erbel和Zotz(美国专利5,190,982)描述了其中包裹空气的交联聚合微囊。
Schneider等(EPA554,213)指出,因含有被包裹的气体中至少部分是具有S气/MW气≤0.0031的气体而可以改善微球的压力阻力,其中S气是以升/升表示的气体的水溶性并且MW气是以道尔顿表示的气体的平均分子量。该文献的表1中列示了符合该标准的N2,SF6,CBrF3和CF4。该文献教导这些微球可以按两种方法中的任一方法来制备。第一种方法是两步法,其中,按已知的方法来制备含空气的微球并且通过气体交换法来用不溶性气体取替空气,例如,通过在一大气压不溶气体下,将填充气体的微球孵育足够长的时间。
第二种方法是一步法,其中按EPA324,938(参见该文献实例)的方法,用不溶性气体代替空气来制备微球。在该方法中,将气体通过壳形成物质溶液(例如白蛋白溶液),同时将超声波仪杨声器放到容器内,然后取出。
遗憾的是,这些方法中没有一种特别适用于制备稳定的蛋白质包裹不溶性的填充气体微球的悬浮液。使用第一种(二步)方法,只有少量填充空气的白蛋白微球在不溶性气体环境中不受损坏(二步法中的第二步)。可溶性气体(空气)从微球内向外流出的量大于不溶性气体向微球内流入的量,这导致微球完全萎陷,只剩下外壳的废弃物。这种结果尤其可用更不溶性气体如全氟乙烷来判断。第二种方法制备的微球,当施加压力时失去容积并且在压力解除后不恢复原状。认为这两种方法都制备劣等的微球,因为微球含有大量的空气并且在制备过程中含有空气可减小在仅含不溶性气体时制备微球的好处。关于此事,注意到先前的研究者已考虑了通过空化作用制备微球中,含有氧是必需的。
Suslick报告与超声有关的空化作用适于作为在仅含氧下制备微球的方法,在详细的研究中,Suslick等(Proc.Natl.Acad.Sci.88:7708—7710,1991;J.Am.Chem.Soc.112:7807-7809,1990)报告为制备稳定的蛋白质壳,需要氧参与由空化作用引起的分子内二硫键的重新排列。Suslick阐叙道“我们发现由于缺氧,强烈抑制微球的形成。”他继续阐叙道“如果在惰性气体环境(He,Ar或N2)下进行反应,不形成微胶囊”。“根据试验,只有在O2或空气下进行反应,才能合成高浓度的微球。”也见美国专利4,774,958。在Holmes(PCT WO92/17213)中也特别提到前面确认的空气是形成白蛋白微球必需的这种观点。其中公开了制备含各种低分子量气体的微球。然而,当叙述通过声处理制备白蛋白微球时,作者阐述道,“所述制备含气体微球的另一种公认的方法(即US—A—4,774,958)是在空气存在下(进一步强调),将混合物进行声处理。”
一方面,本发明涉及未预料到的发现,即在不含氧气的不溶性气体存在下,通过超声或机械空化方法,可以制备包裹相对不溶性气体的高浓度蛋白质微球。这种微球的表现是出人意料的并大大改善对使用压力的稳定性和弹性,保持较好的或相当的回声(echogenicity)。蛋白质外壳预防聚结并阻止因溶解的周围环境气体的扩散而引起的膨胀。
另一方面,本发明涉及一种新的方法,即利用剪切力形式的机械能来制备以蛋白质为外壳的微球。这些力通过机械方法剪切液-气混合物,形成微球悬浮液,并且也引起释放能量的流体力学空化作用。这种能量可以由周围的液体吸附来影响局部蛋白质的变性和在气-液界面沉积,产生离散的微球。根据各空化作用产生的液体***压力变化(它导致能量的释放)的方法,可以区分流体力学空化作用和超声波(声学)空化作用。前者,由通过小孔或穿过表面的快速流动的液体产生压力变化,而后者由高频声波的循环产生快速的局部压力变化。(见F.Ron Young.1989 Cavitation Page4-5,McGraw-Hill Book Co.London)。另外,流体力学空化作用是在流动的液体中产生的,即液体是通过或穿过固定的物体流动的。相比之下,声空化作用是在经过增加和减小压力的充分循环(正压和吸引压力)来显示空化作用后必须保持静止的液体中产生的。甚至在连续流动的声处理***(如美国专利4,957,656中所述)中,声空化过程的滞留时间比真实的单向流体力学空化***(如本发明中所述)的滞留时间更难控制。
通过机械剪切力制备的微泡悬浮液本来是用作造影剂的或通过进一步的加工制成微球。例如,PCT公开号WO92/05806叙述了微泡悬浮液的制备,其中被称为成膜(filmogenic)蛋白质的“泡沫”,是在低于使蛋白质沉淀的某恒定温度下,将含有粘滞剂(Viscosifiers)的蛋白质溶液搅打成粗泡沫来制备的。然后将产生的泡沫用机械方法剪切成需要大小的泡,该泡由所含的粘滞剂来稳定。通过加热沉淀或通过加入交联剂使包围小泡的蛋白膜***可以进一步将小泡加工成微球。
欧洲专利申请公开0450745A1中描述了通过机械剪切和同时或随后加入在界面沉淀的水不溶性聚合物而形成水包油乳剂来制备微球的方法。然后将疏水相蒸发掉形成空气或气体填充的微球。
因此,本发明也涉及改进的方法,即通过使蛋白质溶液受到机械剪切力的作用而由加热变性的蛋白质来制备微球。这些剪切力产生微泡的悬浮液,同时或随后由分离的外壳来包裹。由于空化的加热性,蛋白质变性局部化并通过在液-气界面沉淀形成外壳。与先前以声学为基础制备微球的方法相比,这种新方法更容易扩大并改善产品的产量。
一方面,本发明提供一种制备作用超声显影剂的包裹气体的微球的方法,它包括在无氧条件下,将成膜(filngenic)蛋白质的水溶液和药理学可接受的水不溶性气体的混合物进行超声或机械空化。
另一方面,本发明提供一种制备用作超声显影剂的包裹气体的微球的方法,它包括在与环境隔绝的装置中,使成膜蛋白质的水溶液和药理上可接受的不溶性气体的混合物受到超声或机械空化作用。
另一方面,本发明提供一种超声显影剂组合物,它包含用加热而变得不溶解的成膜蛋白质来包裹气体的微球的水性悬浮液,其中所包裹的气体全部是药理上可接受的水不溶性气体。
另一方面,本发明提供一种超声显影剂组合物,它包含用加热而变得不溶解的成膜蛋白质来包裹全氟丙烷气体的微球的水性悬浮液。
另一方面,本发明提供一种制备用作超声显影剂的包裹气体的微球的方法,它包括:
a)制备蛋白质水溶液,该蛋白质是在随后的机械乳化期间对完成早期变性所必需的温度下可变性的。
b)将溶液和气体合并;
c)通过机械方法剪切蛋白质溶液和气体的混合物使混合物乳化,由此形成平均直径约为0.1—10微米的气体微泡悬浮液;和
d)通过机械方法使悬浮液产生空化作用,引起蛋白质变性并因此在气-液界面沉淀来包裹气体微泡,形成微球。

在含有不溶性气体和实际不含氧的条件下,即在厌氧(封闭***)条件下,通过成膜蛋白质水溶液的超声或机械空化作用在水性悬浮液中形成本发明新的微球。该微球可反射回声并且其大小适于穿过肺通道,其平均直径小于10微米并且大于0.1微米。通过分成大的或小的微球群体,可以改变大小的分布。可以或可以不通过除去过量的水相,将微球浓缩或收集并重新悬浮在第二种水性溶液中。
用于制备这些新微球的气体只需是药理上可接受的并且不溶于它们所存在的水性介质中(即开始是在其中制备微球的介质,当使用时是指血液)。在该介质中的溶解度近似于在水中的溶解度。术语“气体”是指任何气态的或能在进行显像的温度下(典型地为正常生理温度)形成气态的化合物。适宜的气体包括但不限制于含氟气体,如六氟化硫,全氟乙烷,全氟丙烷,全氟甲烷和全氟丁烷。通过测定气体的本生系数(Bunsen Coefficient)可以确定该气体的溶解度。该值是单位体积的溶剂所吸收的气体的体积。(见Wen,W-Y,Muccitelli,JA.J.Sol.Chem.8:225-240(1979))。适用于本发明的气体应该具有小于0.01ml/ml溶液的本生系数(25℃下,在水中)。表1给出几种气体的本生系数。

药理上可接受的指所选择的气体必须是生物相容的和具有最小的毒性。
全氟丙烷是优选的,因为它是一种不溶性气体,该气体(1)在生产和使用温度下不凝结,(2)设有异构体,(3)可制备显示极好加压阻力的微球,和(4)是药理上可接受的。
用成膜蛋白质外壳包裹气体微泡。术语成膜的(filmgenic)指蛋白质在所包裹的气体外面形成壳的能力,根据蛋白质变得不溶解(由加热变性引起)时的特征,其亲水基向外并且疏水基向里。蛋白质必须同时具有亲水性和疏水性氨基酸。适宜的蛋白质包括天然存在的蛋白质,如白蛋白,y-球蛋白(人),去铁铁传递蛋白(人),b-乳球蛋白和脲酶。尽管天然存在的蛋白质是优选的,但可以使用显示三级结构并容易加热变性的合成蛋白质(均聚的或杂聚的)。特别适用于本发明的是白蛋白并且更特别适用的是人体白蛋白。蛋白质在溶液中存在的浓度范围约为0.1—10%W/V,优选约1—5%W/V,并最优选约1%W/V。
为了器官靶向或抑制致免疫活性,可以通过化学方法修饰适用于本发明的蛋白质或产生的微球(例如,用聚乙二醇修饰)。然而,本发明不包括加入化学交联剂。或为形成微球,对蛋白质的其它修饰。
在含有不溶性气体并且不含氧的条件下(即在一个避免空气污染的闭封***中)进行空化作用,由溶液中的变得不溶解的蛋白质来形成本发明微球。此蛋白质变得不溶解的特征主要在于局部蛋白质变性和在气体核心外定位,其中后者可因含有不溶性气体而增强。
用于加热使蛋白质变得不溶解来制备本发明微球的***必须是厌氧的,即与环境隔绝的,也称作“密闭***”。相比之下,“开放***”是与环境相通的***。在这种密闭***中制备的微球中所包裹的气体必须只含制备所用的不溶性气体。可以通过用氧电极测定***流出物中氧的含量来监测环境气体的污染。在本发明中,制备微球初期,微球中只含制备用的气体。然而,当根据实验测定气体含量时,在实验过程中,有一定量不可避免的环境气体污染,这样,所测定的产物中气体的量小于100%。因此,气体的测定结果大于85%就代表开始气体组分都是不溶性气体的微球。
微球制成后,在包装期间,应避免暴露到大气中。例如,应在离开密闭***5-30秒内将微球密封在小瓶或其它密封的容器中,另外,在包装期间应该抽空小瓶中液面上的空间并用制备中所用的气体代替。
用不溶性气体填充的这些蛋白质微球显示显著的稳定性,在约1.0×109微球/ml的浓度下,受到40Psig(72000mmHg)压力作用可不被损坏。在稀释的悬浮液中,微球也显示弹性,表现为在3-10Psig压力下压缩,并且当撤去压力时,恢复到原来的容积。加入化学交联剂是不利的,因为产生的微球太硬而不显示增加压力稳定性。
不象游离的微泡,本发明微球抑制聚结并抑制扩散驱动的膨胀。在各温度下,将含不溶性气体的微球在空气或氧气饱和的溶液中孵育,不增加平均直径或总体积。尽管蛋白质壳在受到压力时是弹性的,但它足以抵抗膨胀或由气体扩散或气体交换引起的撕裂。蛋白质壳可预防聚结并且类似于空气填充的蛋白质微球,保持气体是小的,独立的小泡达数月。不溶性气体填充的微球不能由溶剂化的环境气体交换引起膨胀是利用该物质作超声显影剂的新的和关键的性质。
当暴露到脱气水溶液中时,包裹不溶性气体的蛋白质微球表现出抑制萎陷。不象游离的微泡或用空气填充的囊化微球。可以将不溶性气体填充的微球加到真空脱气的水中并在高度稀释下保持完整。空气填充的微球在血中萎陷,这是由于气相中氧组分的流出。用不溶性气体填充的微球在部分排气或加压环境下抵制蒌陷的能力显著增加体内超声造影的时间。
可通过超声或机械空化作用来制备本发明微球。Cerny(USP4,957,656)已叙述了超声制备空气填充的微球的方法。
机械空化作用是制备本发明新的不溶性气体填充的微球的优选方法。它也可用于制备空气填充的或可溶性气体(例如N2,H2,氩)填充的微球。
在本发明新的机械空化作用过程中,提供在随后机械乳化过程中产生早期变性必需的温度下能加热变性的蛋白质水溶液。溶液中蛋白质的变性温度通常在50—100℃。这可以从文献中蛋白质受热变性的表中查到。或通过任何已知的方法进行实验获得。例如,为了通过实验确定变性温度,可以在水浴上将蛋白质在搅拌下加热。变性温度是最初看到不溶性物质的温度。注意,变性温度受蛋白质的性质,纯度和来源,溶液中蛋白质的浓度,PH值,缓冲剂,离子强度,稳定剂的含量和化学变性剂或去垢剂的影响。因此,在制备微球的环境下测定蛋白质的变性温度是必要的。如果需要,可以使用添加剂如去垢剂或极性溶剂来改变发生变性的温度。

用于剪切蛋白质溶液/气体混合物的每种装置都会使蛋白质溶液产生一定量的附加热,这是由于机械剪切力对溶液发挥作用的结果。该热必须足以在气—液界面引起蛋白质局部变性。确定由装置引起的温度增加量是重要的,这样,可以调节将蛋白质溶液引入装置时的温度以便获得这种局部的热变性。尤其,在空化作用即将开始前,装置中溶液的温度必须与最初变性温度吻合。空化作用产生使蛋白质局部变性必需的附加热。最初变性温度是指蛋白质在变性的边缘,但溶液不含任何变性的蛋白质时的温度。该温度低于变性温度(典型地低1—5℃)。如果必要,可以在将初始蛋白质溶液引入装置之前,将其预热到最初变性的温度。
当获得蛋白质溶液适宜的初始温度后,将溶液与适宜的气体合并,例如,通过在乳化步骤前或在乳化过程中,在气/液的体积比约为5%—200%(优选约为20%—100%)下,将气体引入蛋白质溶液中。适宜的气/液比依赖于装置的几何形状,气体的物理性质(溶解度、密度,分子量等)并且可将其调至使产率最佳化。
气体和蛋白质溶液混合后,将混合液乳化并在制备微球的条件下进行空化作用。同时使用能产生机械剪切力和流体力学空化作用的装置如高速混合器,碾磨机,流化机等。优选的仪器是胶体磨,它是指“一种由高速转子和定子组成的机器,相反的磨面影响分散或乳化。”见Advanced Filtration and separation Technology,P.108—110。如下为可使用的具体碾磨装置的实例:

当用来制备不溶性气体填充微球时,胶体磨应该与环境隔绝以便避免将空气引入混合物中。
图1-3进一步描述可以在机械空化方法中使用的几种碾磨机。
图1描绘Craulin磨的基本元件,有:可操作地连接到发动机(未显示)上的旋转轴(10);附加在旋转轴(10)末端的圆盘状转子(12);和定子(11)。定子(11)具有中心开孔(18)和将转子容纳在其中的沉孔(16)。在该磨中,经过“三通”(15),将蛋白质溶液和气体加入磨中。在转子和定子表面之间将蛋白质溶液/气体混合物乳化并空化。该磨的“缝隙”是在定子沉孔的径向表面(17)和转子周围的径向表面之间。当存在的混合物通过定子(11)时,会产生(例如用热电偶,未显示)微球生产的温度。
图2显示Bematek磨的基本元件。该磨在结构和功能上与图1Gaulin磨类似。其主要差别在于转子和定子的沉孔的结构。它包括带有frustroconical转子(21)和定子(23)的旋转轴(20),其中转子具有螺纹状的前端(22),定子具有中央圆筒开孔(25)和用来容纳转子的frustroconical沉孔(24)。经过孔(25),将蛋白质溶液/气体混合物加入该磨中。当通过旋转轴(22)上的螺纹(22)时,气体和溶液被混合并且混合物被乳化,当通过磨的缝隙时,气体和溶液受到空化作用。将缝隙定义为转子和定子圆锥形表面之间的空间。

当使用图1—3中所述的那些磨时,转速,缝隙大小和气/液比是方法的主要参数,它们影响微球产品的特性(平均粒度,粒度分布和微球的浓度)可以根据经验调节参数来产生具有所要求特性的产品对于任何特定的产品,根据临床来确定其性质例如,用于心肌灌注的全氟丙烷微球的公认的规格是:平均粒度,4微米;粒度分布,10微米以下的占90%,浓度7×108—2×109个微球/ml

实施例1
对人血清白蛋白进行机械空化时
其加工温度的监测及控制
如上所述,在开始加工前,将蛋白质溶液预热,这样可以达到加工温度并保持最初变性温度
典型的实施方法如下:
将21/2″型Bematek胶体磨(图2;Bematek Systems,BeverlyMA)装上管子,这样将入口部分与热交换器相连用不透气的管子使热交换器软管barbs之间软连接。
工艺顶端出口部分连接不锈钢的后处理冷却装置。
在三个位置(T1、T2、和T3)监测溶液的温度。将T1热电偶安置在预热的热交换器和磨顶之间的Swagelok“三通”上来测定加入蛋白质溶液的温度。第二个导入气体的“三通”也安置在进料部。将T2热电偶放在工艺顶端出口的内侧,距转子约1cm,并且距旋转轴约2cm,这样可以准确地测定加工温度。用这种方法,可以分别测定两个温度,进料温度(T1)和加工温度(T2),并且可以在加工过程中与测定溶液的供热量相比较。
在该实施例中,用生理盐水稀释U.S.P白蛋白制备1%(W/V)的溶液。按所述方法,通过实验测定变性温度为78℃。以200ml/min的速度将白蛋白溶液加到磨中,接着用全氟丙烷,以100ml/min(50%V/V)的速度脱气。记录T1和T2之间的差为10℃—15℃。为了使加工温度为77℃(低于变性温度1℃),将进料温度调至62—67℃。因为产生的热量随不同的碾磨参数变化,所以为了确定加工温度以避免蛋白质大量变性同时成功地用变性的蛋白质外壳来包裹气体微泡,测定每种碾磨参数(磨的选择,磨的安装,流速,气/液比等)改变引起的T1和T2之间的差值是必要的。
也监测从冷却装置出来的温度(T3),并且最好的结果为20℃。
实施例2
制备含有不同气体的微球的机械空化方法
如下制备含有各种气体的微球:在真空条件下,将5%人血清血蛋白溶液(USP)脱气2小时。通过向抽空的容器中充入特定的气体,解除真状态。所用的不溶性气体包括六氟化硫,全氟乙烷和全氟丙烷。也可以制备含多种可溶性气体,空气,氮气,氧气和氩气的微球。使用氩气代表高分子量但相对可溶性的气体。通过在管线中的热交换器将白蛋白溶液温度调至68℃并以100ml/min的速度泵入21/2″胶体磨中(Greerco,Hudson,NH,model W250v或AF Gaulin,Everett,mA,model2F)。在室温下,以120—220ml/min的流速将特定气体加到刚好在入口上游的液体进料器中。将转子和定子之间的缝隙调至2/1000英寸(0.005cm)并在加工温度为73℃下,以约7000rpm的转速连续碾磨白蛋白溶液。
通过热交换器,将如此制得稠密白色微球溶液立即冷却到10℃,并收集在玻璃小瓶中。立即将小瓶密封。用Coulter计数器确定物质的浓度和粒度分布特性。结果见下表3。

实施例3
转子的速度和缝隙大小的影响
以50%的气/液(V/V)比,将1%白蛋白溶液(200ml/min)与全氟丙烷(100ml/min)混合。按照实施例1所述的方法,通过改变转子的速度和缝隙的大小来制备微球。所得的数据见表4。

实施例4
气/液比的影响
通过使用具有约0.012的缝隙和9950ft/min的转子周缘速度的Gaulin磨,将0.5%的白蛋白溶液(100ml/min)分别与20,50,70,或100ml/min流速(气液比(V/V)分别为20,50,70或100%)的全氟丙烷混合。所得的数据见表5。

实施例5
通过声空化作用制备不溶性
气体填充的微球的方法
通过间歇和连续超声空化处理制备空气,六氟化硫和全氟乙烷微球。在真空下,将5%的人白蛋白溶液(USP)脱气并贮存在特定的气体中。如Cermy(USP4,957,656)所述方法。通过用不溶性气体代替空气来进行连续超声处理。通过使用3/4″液体处理扬声器(Sonics and Materials,Danbury CT)进行间歇超声处理。气体通过扬声器并进入白蛋白中,这样,在整个处理过程中,空气被排除在外。通过使用Branson压电转化器和电源(BransonUltrasonics,Danbury CT)将该白蛋白温热至73℃并在20KHz频率,在60微米双振幅下,声处理5秒钟。将产品立即转移至玻璃小瓶中并在气体环境下密封。
产品由平均粒度为2.5—3.3微米,浓度为1.4×108-1.0×109微球/ml的粘稠的、乳白色微球悬浮液组成。
实施例6
微球的显微观察
如实施例2或5所述,制备含多种气体的白蛋白微球。显微观察产品可见球形微球的单分散悬浮液。用注射器施加高压,微球萎陷直至悬浮液澄清。在整个产品中,显微再观察可见透明、膜状外壳而没有萎陷的微球。
实施例7
微球的耐压性
如实施例2所述方法制备含多种气体的白蛋白微球。将10ml的各悬浮液加到安装有压力表的10ml不漏气的玻璃注射器中(Hamilton,Reno NV)。除去全部液面上的空间并密封该装置。连续施加40psig的压力3分钟。然后用Coulter计数器来测定样品的颗粒浓度及分布。加压前后的数据比较(图4a—4e)表明,不溶性气体微球的相对耐力为40psig。
实施例8
微球的稀释悬浮液的耐压性
如实施例2所述方法制备含多种气体的微球。将微球的各样品稀释到每毫升磷酸盐缓冲的盐水(0.15M)包裹等体积的气体,约1∶60稀释。稀释后的溶液在具有足够大液面上空间的密封容器中,立即受到0.5psig—7.7psig的静压力。图5显示压力对微球浓度的影响。含不溶性气体全氟丙烷,全氟乙烷和六氟化硫的微球的耐压性比相同浓度和粒度分布下的高分子量氩气填充的微球的加压阻力大得多(图6)。血流中生理压的范围为1.5psig的外周静脉压到2.5psig的心肌壁压。
实施例9
脱气缓冲剂对微球的影响
如实施例2所述方法制备含多种气体的白蛋白微球。通过在使用前煮沸,将磷酸盐缓冲的盐水(PBS)脱气。将0.05ml—1.5等分试样的热缓冲剂加到13×100试管中并在水浴上冷却1分钟至室温。每个试管中加入恒定体积的微球。混合后,用PBS将最终体积调至3.0ml并测定微球浓度。图7显示在脱气的溶液中,含不溶性气体全氟丙烷,全氟乙烷和六氟化硫的微球的未萎陷数得到改善。
将含空气,六氟化硫或全氟乙烷的微球稀释到全血中。空气填充的微球表现为萎陷。不溶性气体填充的微球在新鲜全血中稀释而不蒌陷。
实施例10
弹性
如实施例2所述方法制备由多种气体制备的微球。如实施例8所述方法,将微球稀释到磷酸盐缓冲的盐水中并加到放在显微镜边缘的透明的测定池中。将该测定池与氮源连接,其中该氮源用于观察快速施加和解除生理压力对微球的影响。
向含可溶性气体的微球中施加1.5psig或更大的压力,观察完全丧失的球形体。当解除压力时,微球不再形成,表明是不可逆性破坏。施加小于1.5psig的压力,结果,未完全丧失微球的壳可再形成并出现绉纹。当解除所施加的压力时,不能再恢复球形外观和粒子数。
向含不溶性全氟化碳气体的微球悬浮液中施加不超过几个psig的压力,引起微球直径的减小。当解除压力时,微球的直径恢复到原来的直径。
在施加生理压力下,六氟化硫微球相对于空气填充的微球来说也表现弹性增大,但相对于全氟化碳微球来说表现弹性减小。
这些观察表明,含不溶性气体的微球不仅抵抗加压,而且在压力解除后可再恢复。这表示是弹性蛋白质壳。
实施例11
比较在开放***和封闭***中制备的微球
处理方法:
A)手工声处理:开放***(相当于EPA554,213一步法)
如下采用美国专利4,844,882和欧洲专利申请554,213中所述的方法来制备微球:
用通过顶端附加物***的T型热电偶来装备20cc注射器圆筒并将其安置支架上。用Swiss Red Cross5%的人血清白蛋白填充注射器至16cc刻度。将气体(全氟丙烷(C3F8)或六氟化硫(SF6))引入注射器圆筒的顶部并在液体表面上流动。将超声扬声器放到100cc刻度处,低于溶液的表面并以5%的功率开始工作直到溶液的温度升至72.8—73℃;大约1分钟。立即将扬声器提起到弯月面±1mm并将功率水平增至65%。连续声处理5秒钟,温度另外增加1.2—2℃。将产品倾入玻璃小瓶中至小瓶的容量并密封。
B)连续声处理:密闭***
如下采用美国专利4,957,656来制备全氟丙烷和六氟化硫微球;
用无菌盐水将人血清白蛋白稀释到1%W/V的溶液。将溶液加热到最初变性温度,大约76℃。***与外界环境隔绝并将全氟丙烷或六氟化硫气体(代替空气)引入液体流(1∶1)中。通过让气体/白蛋白混合物以大约100ml液体/分钟的流速连续流过超声波器的扬声器来制备产品。当产品通过热交换器的通道从声处理室中流出时被冷却并作为一批微球的液体悬浮液收集。处理与贮存条件与手工制备微球的条件类似。
C)机械空化作用:密闭***
与实施例2所述的方法类似,在密闭***中,通过碾磨1%人血清白蛋白和气体的混合物,也制备含全氟丙烷或六氟化硫的白蛋白微球。将加热到足以通过指定磨的机械空化作用形成微球的温度下的白蛋白溶液与气体1∶1(V/V)混合并引入胶体磨中。液体的流速依赖于磨的容积或大小,典型地为100—500ml/min。在本评估中使用Silverson L4R磨和Bematek3″生产胶体磨。通过热交换器***的通道将从磨流出的物质冷却并批量收集产生的白蛋白微球悬浮液。与其它方法类似,将产品装入玻璃小瓶中。
分析方法
A)粒子数动力学
用50微米孔径的Coulter MultisizerII来评估粒子数动力学。将按处理方法中所述的方法制备的白蛋白微球以1∶10,000的比例稀释到Isoton中并分析500μl的样品。得到原微球悬浮液的浓度,平均粒度和每毫升原微球悬浮液中包裹的气体的体积。
B)气体含量
通过气相色谱法,用Hewlett Packard 5890来测定按处理方法中所述的方法重复制备的两批微球所包裹的全氟丙烷的百分数。用不漏气的注射器抽取微球悬浮液的样品。通过使用消泡剂的乙醇液排出微球中的气体并且通过热导性测定所包裹的气体。
C)耐压性
按类似于欧洲专利申请554,213中Sintetica报道的方法来评估白蛋白微球的耐压性。在3ml耐压测定杯中,用通气的磷酸盐缓冲的盐水将微球稀释到在600nm约为1吸收度单位。将颈部与压源连接并将比色杯放到自记分光光度计中。测定杯中的压力经150秒从0到5或10psig线性增加,然后解除压力。通过配比的电磁阀(Hondywell)和放在20Psi压源(N2罐)和5升不锈钢储压器之间的压力转换器(Omega)产生压力梯度。通过数字压力表将测定杯与不锈钢储压器相连接。配备有模拟数字变换器和模拟变换器板上数字的PC—型计算机(National Instruments)控制阀门的打开并显示压力转换器读数。以选择的速率加压储压器和测定杯直至获得需要的压力。监测微球悬浮液的光密度并作为时间和压力的函数。校正测定杯中微球的天然漂浮率。
结果
A)粒子数动力学
通过手工声处理,连续声处理和机械空化作用制备的白蛋白微球,在制备后24小时内进行浓度,平均粒度,所包裹气体的体积和粒度分布的分析。所有的测定都要至少进行两次并以其平均值表示。测定结果见表6。

在该项研究期间,按所有方法制备的微球都是稳定的,在4℃下至少可稳定几星期。

这些结果表明,在开放***中,用手工声处理制备的微球所包裹的用于制备微球的气体的量比在密闭***中(连续声处理或机械空化作用)少得多。在无氧条件下(用氧电极测定)制备在密闭***中制备的微球。用三种方法制备的微球在处理和取样期间暴露到外界环境的量是相同的(这就是在用两种密闭方法制备的微球中测定全氟丙烷气体少于100%的原因),因此,在开放***中制备微球期间,有氧(和其它环境气体)存在,这减低了气体包裹的功效。
C)耐压性
随着压力的增加,由于粒度减小及表面积相应的变化,气体填充微粒悬浮液的光密度减小。绉缩是由两个因素引起的;按照气体定律的可逆性压缩和按照Henry's定律增加溶解度引起气体核心不可逆地流失到外界液体中。当解除施加的压力时,只有压缩引起体积丢失的部分再恢复,并且这可以通过光密度增加而观察到。当解除压力时,丢失到外界溶液中的所包裹的气体不再进入到微球中,但流失到溶液上方的顶部空间中。
图8显示在10D的白蛋白微球悬浮液上施加不超过10psi线性压力梯度的结果,其中微球是通过手工声处理(开放***)方法及连续声处理和机械空化作用(密闭***)方法,用全氟丙烷气体制备的。两种密闭***方法产生随压力增加表现为压缩的微球,在梯度终点解除压力时,恢复原来的总体积。不能看到所包裹的气体丢失到外界溶液中。在开放***(手工声处理方法)制备的白蛋白微球,随着压力的施加表现出更大的压缩,并且当解除压力时,由于不可逆地丢失气体核心,只有部分体积恢复,引起40%的微球破坏。
Claims (17)
1.一种超声显影剂组合物,它含有微球的水性悬浮液,该微球是用加热变得不溶解的成膜蛋白质包裹气体来制备的,其中所包裹的气体全部是药理上可接受的水不溶性气体。
2.权利要求1的组合物,其中气体在水中的溶解度在25℃时为每毫升水中少于0.01ml气体。
3.权利要求1的组合物,其中蛋白质是人血清白蛋白。
4.权利要求1的组合物,其中蛋白质是人血清白蛋白且不溶性气体为全氟丙烷。
5.一种超声显影剂组合物,它含有微球的水性悬浮液,其中微球是用加热变得不溶解的成膜蛋白质包裹全氟丙烷气体来制备的。
6.权利要求3的组合物,其中蛋白质是人血清白蛋白。
7.一种超声显影剂组合物,它含有微球的水性悬浮液,该微球是在含所包裹气体并且不含氧气的条件下,用加热变得不溶解的成膜蛋白质包裹气体来制备的,其中所包裹的气体全部是药理上可接受的水不溶性气体。
8.权利要求1的组合物,其中气体在水中的溶解度在25℃时为每毫升水中少于0.01ml气体。
9.权利要求7的组合物,其中蛋白质是人血清白蛋白。
10.制备包裹气体的可用作超声显影剂的微球的方法,它包括在不含氧气的条件下,使成膜蛋白质的水溶液与药理上可接受的不溶性气体的混合物受到超声或机械空化作用。
11.制备包裹气体的可用作超声显影剂的微球的方法,它包括在与外界环境隔绝的装置中,使成膜蛋白质的水溶液与药理上可接受的不溶性气体的混合物受到超声或机械空化作用。
12.权利要求5或6的方法,其中蛋白质是人血清白蛋白并且气体是全氟丙烷。
13.制备用作超声显影剂的,包裹气体的微球的方法,它包括:
a)制备蛋白质水溶液,该蛋白质是在随后的机械乳化期间,在获得早期变性所必需的温度下可变性的;
b)将溶液和气体合并;
c)通过机械方法剪切蛋白质溶液和气体的混合物使混合物乳化,由此形成平均直径约为0.1—10微米的气体微泡的悬浮液;和
d)通过机械方法使悬浮液产生空化作用引起蛋白质变性并因此在气/液界面沉淀来包裹气体微泡,形成微球。
14.权利要求8的方法,其中蛋白质是人血清白蛋白并且气体的全氟丙烷。
15.权利要求8或9的方法,其中通过使混合物经过碾磨机来实现步骤(c)和(d)。
16.通过权利要求10,11,12,13,14或15的方法来制备的包裹气体的微球。
17.在准备超声显像的病人的组织和/或器官增强其反差的方法,包括:(a)给病人注射权利要求1,2,3,4,5,6,7,8,9或16的组合物或微球,(b)给组织和/或器官提供超声能,(c)测定从组织和/或器官反射回来的超声能和(d)将反射回来的能量转换成图像。
