방사선 빔(106)을 방출할 수 있는 방사선 소스(104)를 지지하는 갠트리(112)를 포함하는 예시적인 방사선 치료 장치(101)가 도 1에 도시되어 있다.
시준 장치(102)는 방사선 빔(106)의 경로 내에 위치될 수 있으며, 방사선 빔(106)이 타겟(108)을 향해 이동할 때 방사선 빔(106)을 선택적으로 감쇠시키도록 구성될 수 있다. 방사선 소스(104)는 예를 들어 방사성 동위 원소, 중 이온 가속기, 전자 또는 광자 빔을 생성하기 위한 선형 가속기 등일 수 있다. 본 개시 내용의 기술은 방사선 빔이 이용되는 임의의 분야에서 사용될 수 있지만, 본 명세서에 기술된 실시예는 의료 환자(P)를 타겟(108)으로 도시한다.
도 2는 다엽 시준기[Multi-Leaf Collimator(또는 MLC)]로 알려진 특정한 유형의 시준 장치를 도시한다. 도시된 예시적인 MLC(200)는 가동 잎의 제2 뱅크(204)에 대향하는 가동 잎의 뱅크(202)를 포함한다. 이러한 장치에서, 각각의 잎(206)은 치료를 위해 원하는 형상으로 빔을 시준하도록 하는 개구(212)의 형성을 가능하게 하기 위해 독립적으로 조정 가능하다.
MLC(200)의 각각의 잎은 폭(208) 및 높이(110)(높이는 도 1에 도시됨)를 갖는 것으로 설명될 수 있다. 높이(110)는 또한 잎의 "두께"로 설명될 수 있으며, MLC(200)에 의한 빔(106)의 감쇠량을 결정하는 데 중요하다. 감쇠량은 또한 MLC의 잎이 만들어지는 재료에 의해 영향을 받기 때문에 텅스텐, 텅스텐 합금, 탄탈, 탄탈 합금, 납, 납 합금 등과 같은 고 감쇠 재료가 사용된다.
본 개시 내용에 의해 고려되는 예시적인 시준 시스템이 도 3에 도시되어 있으며 다중 "적층된(stacked)" MLC를 포함한다. 예를 들어, 도시된 실시예는 제1 MLC(302) 및 제2 MLC(304)를 포함한다. MLC들은 이들의 감쇠 값들이 방사선 빔(106)에 대해 가산적(additive)이도록 적층된다. 제1 MLC(302)는 제2 MLC(304)보다 방사선 소스(104)에 더 근접하게 위치되어, 방사선 빔(106)은 제2 MLC(304)를 통과하기 전에 제1 MLC(302)를 통과한다. 본 명세서에 도시된 실시예는 2개의 적층된 MLC를 도시하지만, 본 개시 내용의 일반적인 교시에 따라 추가의 MLC가 추가(예를 들어, 3개의 적층)될 수 있는 것으로 고려된다.
시준 장치가 방사선 소스(104)에 근접하게 위치하는 것이 일반적이지만, 본 개시 내용은 시준 장치를 타겟 또는 환자에 더 가깝게 이동시키는 실시예를 고려한다. 예를 들어, 본 개시 내용의 바람직한 구현예는 타겟/환자에 의해 점유되는 바람직한 보어(bore) 또는 체적을 제한하지 않으면서 가능한 한 타겟에 근접하게 시준 장치를 이동시킨다. 하나의 바람직한 구현예에서, 타겟(108)에 가장 근접한 시준 장치의 에지(edge)[즉, 방사선 소스(104)로부터 가장 먼 제2 MLC(304)의 에지]는 등중심으로부터 60cm 미만, 바람직하게는 등중심으로부터 약 50cm이다. 이러한 설계는 조립 동안 시준 장치를 위치시키는 것을 용이하게 하고 빔 반음영을 감소시키는 것으로 고려된다.
도 4a 및 도 4b는 예시적인 이중 적층 MLC 시스템과 함께 빔이 어떻게 시준될 수 있는지에 대한 단순화된 도면이다. 양 도면에 도시된 바와 같이, 제1 MLC(302) 및 제2 MLC(304)의 잎은 잎의 폭의 절반, 또는 잎의 폭의 약 절반만큼 오프셋된다. 제1 MLC(302) 및 제2 MLC(304)의 잎은 서로 독립적으로 이동될 수 있다. 도 4a에서, 제1 MLC(302)의 하나의 잎과 제2 MLC(304)의 하나의 잎은 빔(106)이 통과할 수 있는 최소 개구(잎의 폭에 상응하는 차원에서)를 생성하도록 수축될 수 있다. 결과적으로, MLC의 잎은 제1 및 제2 다엽 시준기 각각의 잎의 폭보다 얇은 빔의 시준을 허용하는 방식으로 오프셋된다.
하나의 특정 구현예에서, 이러한 빔의 폭은 제1 MLC(302) 및 제2 MLC(304) 둘 다에서 잎의 폭이 약 8.3mm일 때 4.15mm일 수 있다. 도 4b는 MLC들 중 하나의 2개의 잎이 수축되고 다른 MLC의 중첩 잎이 수축될 때, 방사선 빔(106)이 통과할 수 있는 두 번째로 작은 개구, 예를 들어 8.3mm의 폭을 갖는 빔이 생성되는 것을 도시한다.
하나의 구현예에서, MLC는 적층되고, 각각의 MLC 내의 잎은 대략 동일한 폭이고, 제1 MLC(302)의 잎은 제2 MLC(304)의 잎으로부터 그 폭의 약 절반만큼(도 4에 도시된 바와 같이) 오프셋된다. 이러한 구현예에서의 MLC 잎은 여전히 대략적으로 동일한 해상도를 달성하면서도 전형적인 MLC의 약 2배 폭이 되도록 설계될 수 있다. 예를 들어, 등중심에서 5mm 해상도를 달성하기 위해, 전형적인 단일 MLC는 약 2.5mm의 폭을 필요로 하고, 오프셋을 갖는 이중 적층 설계에서 잎은 약 5mm의 폭을 가지며 동일한 해상도를 달성할 수 있다. 이러한 설계는 기계 가공을 용이하게 하고 잎에 연결되거나 잎과 결부되는 장비에 더 많은 재료를 제공하는 데 바람직할 수 있다.
도 5는 이중 적층 MLC(302 및 304)를 도시하는 도 3의 예시적인 시준 시스템의 등각도이다. 예시적인 시준 시스템이 가산적인 빔 감쇠 영향을 갖도록 배열된 다중 MLC를 포함하기 때문에, 개별 MLC 각각의 잎은 표준 단일 MLC 시준 시스템의 잎과 비교하여 감소된 높이 또는 두께를 가질 수 있다. 예로서, 2개의 MLC가 이용되는 경우, 각각의 MLC 내의 잎은 동일한 재료로 제조된 전형적인 단일 MLC에서의 잎의 높이의 대략 절반일 수 있다. 이러한 것은 개별 잎의 무게를 감소시킬 수 있어, 개별 잎을 제어하기 쉽고, 보다 신속한 이동을 가능하게 하여, 전체 치료 시간을 단축시킬 수 있다. 또한, 시준기가 포커싱형 또는 이중 포커싱형이도록(바람직하게는 아래에서 더 설명되는 바와 같이) 설계되면, 빔에 노출된 MLC의 에지는 더 큰 감쇠를 가질 것이고, MLC 각각의 잎은 높이가 더 감소될 수 있다.
도 4에 도시된 빔 시준 특성부 및 본 명세서에 설명된 빔 감쇠의 중요성을 고려해 볼 때, 본 개시 내용의 바람직한 구현예는 제1 MLC(302) 및 제2 MLC(304)에 대해 동일하거나 대략적으로 동일한 잎 높이를 이용한다. 제1 MLC(302) 및 제2 MLC(304) 모두는 방사선 빔(106)을 성형하는 역할을 하기 때문에, 바람직하게는 제1 MLC(302) 및 제2 MLC(304) 모두는, 예를 들어 의료 방사선 치료를 위해, 방사선 빔(106)을 완전히 감쇠시키기에 충분한 잎 높이로 각각 설계된다. 하나의 특정 구현예에서, 제1 MLC(302) 및 제2 MLC(304) 모두의 잎은 17.5gm/cc 이상의 밀도의 텅스텐 합금(예를 들어, 5:5:90 Cu:Ni:W)으로 설계되고 각각은 약 5.5cm 두께를 갖는다. 바람직한 예시적인 시준 시스템은 제1 MLC(302)의 각각의 뱅크에 34개의 잎을 포함할 수 있고, 제2 MLC(304)의 각각의 뱅크에 35개의 잎을 포함할 수 있지만, 각각의 뱅크에서 상이한 해상도 및 잎의 수를 고려할 수 있다.
본 개시 내용의 기술과 함께 사용되는 MLC는 도면에 도시된 바와 같이(선형 잎 움직임 및 둥근 잎 단부를 갖는 것들 같은 비-포커싱 시준기를 이용하는 것과는 대조적으로), 이중 포커싱형인 것이 바람직하다. MLC는 잎의 모든 빔 한정 표면이 방사선 소스로 역방향 투영(project back)될 때 이중 포커싱형이다. 예를 들어, 도 1을 참조하면, 방사선 빔(106)은 방사선 소스(104)로부터 밖으로 확산된다. 예시적인 시준 시스템은 아크를 따라 수축하는 만곡된 잎을 사용하기 때문에(예를 들어, 도 1 및 도 3에 도시된 바와 같이), 잎의 에지는 수축하면서 항상 방사선 소스(104)로 역방향 투영되는 라인을 나타낸다. 이러한 설계로, 잎의 전체 두께는 빔이 시준 장치를 통과할 때 빔(106)을 감쇠시킬 것이고, 잎이 얼마나 많이 수축되었는지 여부에 관계없이 적은 반음영으로 더 날카로운 빔 에지를 제공한다.
빔(106)을 시준하는 4개의 잎 표면 모두가 방사선 소스로 역방향 투영될 때, 시준 시스템은 이중 포커싱형이다. 도 5는 방사선 소스(104)로부터의 거리에 따라 잎의 폭이 증가함에 따라, MLC가 빔(106)을 상이한 차원에 포커싱할 수 있는 방식을 도시한다. 도 5에서, 예를 들어, MLC(302)의 상부에서의 잎의 폭이 가장 얇다. 폭은 MLC(302)의 잎의 하부에서 더 크고, 제2 MLC(304)에서의 잎의 상부에서 더욱 더 크며, MLC(304)에서 잎의 하부에서 가장 크다. 이러한 설계는 또한 도 6에 도시되어 있다.
하나의 구현예에서, 잎 설계의 포커싱은 의도적으로 약간 탈초점화(defocused)된다. 예를 들어, 잎 표면은 실제 방사선 소스의 1 내지 2 센티미터 위 또는 아래 포인트로 투사하도록 설계될 수 있다. 이러한 약간의 탈초점화는 빔 반음영에 작은 영향만을 가지면서, 잎 사이의 공간(즉, 잎 사이의 간격)을 통한 방사선 누출을 상당히 감소시킬 수 있다.
다른 구현예에서, 제1 MLC(302) 및 제2 MLC(304)는 상이한 초점 포인트를 갖는다. 따라서, MLC가 이동하는 아크들은 어떤 포인트에서 교차하지만 그들의 경계 내에서 서로간에 충분한 여유 공간을 갖도록 설계될 수 있다. 제1 다엽 시준기 및 제2 다엽 시준기가 소스로부터 상이한 거리에 있더라도, 제1 다엽 시준기와 제2 다엽 시준기 사이의 반음영의 일치를 개선하기 위해 초점 포인트들을 상이하게 하는 것이 선택될 수 있다. 예를 들어, 제1 MLC의 초점은 유효 소스 포인트에 위치될 수 있고 제2 MLC의 초점은 유효 소스 포인트로부터 벗어날 수 있다. 이러한 예시적인 설계는 하부 MLC의 반음영을 증가시켜 상부 MLC의 반음영과 더 잘 일치시키고 제1 MLC 및 제2 MLC에 의해 형성된 빔 에지의 더 양호한 선량 일치를 제공한다.
종래의 비-포커싱 MLC의 경우, 빔 개구 외부의 방사선 누출을 방지하기 위해 시준기 죠(jaw)가 필요하다. 종래의 MLC의 둥근 잎 단부는 완전히 폐쇄된 경우에도 방사선의 차단에 열악하므로, 폐쇄된 잎 단부들은 이들이 종종 종래의 시준기 죠에 의해 차단되는 위치로 이동된다. 이중 포커싱형 잎의 사용은 합리적인 두께(잎 교차 위치의 오프셋을 가짐)의 인접한 적층 MLC가 전송을 차단시키기에 충분할 정도로 잎 단부 누출 및 반음영을 제한하여, 종래의 시준기 죠가 필요하지 않도록 한다. 따라서, 본 개시 내용은 시준기 죠를 포함하지 않는 시준 시스템을 고려한다.
본 개시 내용의 바람직한 구현예는 이중 포커싱형 MLC를 이용하지만, 단일 포커싱형 또는 비-포커싱형 MLC가 또한 이용될 수 있거나, 포커싱 유형의 혼합이 다중 적층 MLC 전반에 사용될 수 있다는 것이 고려된다.
포커싱형 구현예에서 제1 MLC(302) 및 제2 MLC(304)의 잎의 폭을 비교할 때, 잎 폭은 방사선 소스(104)로부터의 거리에 따라 계속적으로 증가한다는 것이 위에 언급되었다. 즉, 본 개시 내용의 바람직한 구현예는 제2 MLC(304)에서와 같이 제1 MLC(302)에서 대략적으로 동일한 폭을 갖는 잎 설계를 포함한다. 이러한 방식으로 기술될 때, "대략적으로 동일한 폭"은 제1 MLC(302)에서의 잎의 하부 폭이 제2 MLC(304)에서의 잎의 상부 폭과 거의 동일하다(즉, 약간 작다)는 것을 의미한다. 다른 말로 표현하면, 제1 MLC 및 제2 MLC의 포커싱형 잎은 거의 동일한 폭을 갖는 것으로 생각될 수 있으며, 이 동일한 폭은 포커싱형 설계를 제공하는 데 필요한 바에 따라(예를 들어, 도 5 및 도 6에 도시된 바와 같이) 제1 MLC 및 제2 MLC의 포커싱형 잎이 방사선 소스(104)로부터 더 멀리 연장됨에 따라 잎을 따라서 추가되는 작은 추가 폭을 포함한다.
바람직한 구현예는 제1 MLC(302) 및 제2 MLC(304)의 잎 폭이 거의 동일한 잎 설계를 이용하지만, 본 개시 내용은 잎 폭이 적층된 MLC간에 상이할 수 있는 설계를 고려한다.
본 개시 내용의 바람직한 구현예에서, 제1 MLC(302)의 잎들은 서로 바로 인접하거나 접촉하고, 제2 MLC(304)의 잎들은 서로 바로 인접하거나 접촉한다. 이러한 구현예에서, 제1 MLC(302) 및 제2 MLC(304) 모두에서의 인접한 잎들 사이의 간격은 잎들 사이의 방사선 누출을 최소화하면서도 여전히 상대적인 움직임을 허용하는 방식으로 최소화된다. 이러한 유형의 구현예는, 예를 들어 도 4, 도 5, 및 도 6에서 설명된다.
MLC의 잎은 독립적으로 움직일 수 있기 때문에, 약간의 방사선이 지나갈 수 있는 작은 간격이 반드시 필요하다. 본 개시 내용의 시준 시스템은 제1 MLC(302)의 잎과 제2 MLC(304)의 잎이 바람직하게 배치되어 잎들 사이의 간격이 정렬되지 않아, 방사선 빔(106)이 제1 MLC(302)의 잎 간격을 통해 그리고 곧바로 제2 MLC(304)의 잎 간격을 통해 전송되지 않을 수 있도록 고려한다. 대신에, 제1 MLC(302)의 잎은 제2 MLC(304)의 잎으로부터 바람직하게 오프셋되어, 빔이 MLC들 둘 다의 잎 사이의 간격을 통과하여 이동하는 직선 경로가 없다. 예를 들어, 도 4, 도 5, 및 도 6 를 참조하라.
예시적인 실시예에서, 제1 MLC(302)와 제2 MLC(304)의 잎 사이 간격 사이에서 가장 큰 분리를 제공하기 위해, 제1 MLC(302)와 제2 MLC(304)의 잎은 그 폭의 약 50%만큼 오프셋된다. 잎 폭의 50% 미만의 오프셋이 본 개시 내용에 의해 고려되지만, 오프셋이 이용되는 것이 바람직하며, 바람직하게는 잎 폭의 10%보다 크다.
오직 하나의 MLC를 갖는 통상적인 시준 시스템에서, 잎이 서로 정합하거나 맞닿는 위치에서 잎의 복잡한 기계 가공을 통해 잎 사이의 누출을 방지해야 한다. 예를 들어, 텅(tongue) 및 그루브(groove) 또는 계단형(stepped) 설계가 현저한 빔 누출을 허용할 수 있는 선형 잎 사이 간격을 차단하기 위해 사용될 수 있다. 본 개시 내용의 시준 시스템은 직선 에지형 잎이 이용되더라도, 시준 시스템을 통한 누설 경로가 이전에 기술된 제1 MLC(302)와 제2 MLC(304) 사이의 잎의 중첩 또는 오프셋에 의해 차단될 것이기 때문에 이러한 추가 기계 가공을 제거할 수 있는 능력을 고려한다. 바람직한 구현예는 추가적인 기계 가공 또는 잎 사이 간격을 차단할 특성부가 없는, 단순한 직선 에지형 잎을 포함한다. 이러한 설계는 또한 보다 균일한 잎 에지 및 감소된 빔 반음영으로 이어질 수 있다.
현재 기술된 시준 시스템의 대안적인 실시예에서, 다중 MLC 및 잎 오프셋을 가지지만, 누설 경로를 더 감소시키고 MLC의 높이를 감소시킬 수 있도록 잎의 정합 표면이 기계 가공될 수 있다. 텅 및 그루브 설계 등과 같은 비선형 표면의 임의의 구성이 유익한 것으로 입증될 수 있다. 도 7에 도시된 예시적인 실시예에서, 잎의 정합 표면에 계단이 기계 가공된다. 도 7은 제1 MLC(302)에 대응하는 제1 부분 잎 뱅크(702) 및 제2 MLC(304)에 대응하는 제2 부분 잎 뱅크(706)를 도시한다. 도시된 실시예에서, 잎은 폭(709) 및 높이(704, 708)를 갖는다. 예시적인 실시예에서, 부분 잎 뱅크(702)의 잎 높이(704) 및 부분 잎 뱅크(706)의 잎 높이(708)는 동일하고 약 5.5cm이다. 그러나 잎 뱅크 각각의 높이가 동일할 필요는 없다.

본 명세서에서 고려되는 갠트리의 하나의 구현예는 적어도 하나의 방사선 치료 빔 소스를 지닐 수 있고 동일 평면 빔의 전달을 허용하는 방식으로 치료 중에 빔 소스를 재배향하는 데 사용되는 링 갠트리(ring gantry)이다. 용어 링 갠트리가 본 명세서에서 사용되는 경우, 갠트리는 반드시 순전히 링 형상일 필요는 없는 것으로 고려된다. 원형 모양에서 벗어나거나, 그 구조에 단절(들)을 포함하는 갠트리도 고려된다.

본 명세서에 개시된 방사선 치료 장치의 실시예는 소스가 이동하는 동안(예를 들어, 갠트리의 회전 중에) 방사선 치료 빔 소스가 방사선 빔을 방출하는 아크 치료(또한 VMAT이라고도 함)를 수행하는 데 사용될 수 있다. 그러나, 상술한 시준 장치 개념을 이용하는 특정 유익한 실시예는 방사선 치료 장치가 아크 치료를 하도록 구성되지는 않지만 그럼에도 불구하고 단시간 내에 고품질의 치료 계획을 전달할 수 있도록 설계될 수 있다.
본 개시 내용은 개시된 방사선 치료 시스템과 관련하여 진단 품질 CT 스캐너의 사용을 고려한다. 진단 품질 CT 스캐너는 전형적으로 단일 또는 다중 슬라이스 검출기 능력을 가지며 축 방향 또는 나선형 데이터 획득 및 영상 재구성이 가능한 '슬립 링 기술(slip-ring technology)'에 기초하여 연속적으로 회전하는 CT 시스템이다. 이들은 수십 내지 수백 개의 영상 슬라이스를 획득하도록 구성된 다중 소스 및 검출기 배열을 가질 수 있다. 이들은 종종 병원 또는 클리닉의 진단 X-선 부서에서 사용되며, 전형적으로 환자 주위를 회전하는 팬(fan) 빔 형상의 킬로 볼트 에너지 X-선을 사용한다. 진단 품질 CT 스캐너는 방사선 치료 환자의 치료 계획에 사용하기 위해 고품질 CT 영상을 획득하기 위해 종종 사용된다. 고품질의 CT 영상은 향상된 선량 계산을 위해 조직 밀도에 대한 Hounsfield 수치의 교정을 허용한다.
진단 품질 CT 스캐너는 콘 빔 X-선 CT를 생성하기 위해 수축 가능한 X-선 튜브 및 평면 패널 영상기(flat panel imager)를 사용할 수 있는 콘 빔 CT 시스템과 구별된다. 콘 빔 CT(일명 CBCT)에 의해 생성된 CT 영상화 데이터는 표준 CT 유닛보다 낮은 영상 품질, 낮은 연조직 대비도, 장기 움직임의 아티팩트, 및 영상화된 조직의 전자 밀도를 정확하게 반영하지 않는 Hounsfield 수치로부터 어려움을 겪는다. 진단 품질 CT 스캐너는 또한 낮은 연조질 대비도로 화질이 좋지 않은 영상으로 이어지는 메가 볼트 CT 영상을 생성하기 위해, 평면 패널 영상기를 사용하여 메가 볼트 방사선 치료 빔을 영상화 소스로서 사용할 수 있는 메가 볼트 시스템과 또한 구별된다.

결합된 CT/RT 시스템에서, 진단 품질 CT 스캐너는 영상화 등중심을 가지며, 방사선 치료 장치는 영상화 등중심과 분리된 방사선 치료 등중심을 갖는다. 분리되었다는 것은, 예를 들어, 침상이 영상화와 치료 사이에서 환자를 움직여야하는 것과 같이, 등중심들이 상당히 이격되어 있음을 의미하는 것으로 이해된다. 하나의 구현예에서, 등중심들이 서로 약 80cm 떨어져 있다. 바람직한 구현예에서, CT 스캐너 및 방사선 치료 장치는 서로에 대해, 또한 치료실에 대해 고정된 위치에 있으며, 이는 그들이 이동될 수 없는 방식으로(마치, 예를 들어, 그들이 레일이나 턴테이블에 있는 것처럼) 장착된다는 것을 의미한다.

침상은 결합된 CT/RT 시스템을 위해 특별히 설계될 수 있다. 하나의 구현예에서, 침상은 위아래로 움직이고, 시스템의 보어(들)를 통해 이동하도록 설계될 수 있지만, 상술한 바와 같이 회전하지 않도록 구성될 수 있다. 대안적으로, 기성품 CT 시뮬레이터 침상이 사용되어 CT/RT 시스템에 가능한 한 가깝게 위치될 수 있으므로 두 등중심들 모두를 통해 연장될 수 있다. 또 다른 구현예에서, 다중 등중심 시스템에서 사용하기 위해 설계된 기성품 PET/CT 스캐너 침상이 사용될 수 있다. 본 명세서에서 "기성품(off-the-shelf)" 시스템이라는 용어가 사용되는 경우, 이는 사용 준비가 되어있는 구성으로 구입되거나 사소한 수정만으로 사용될 수 있는 시스템을 의미한다.

결합된 CT/RT 시스템은 방사선 치료 직전에 치료 침상에서 환자의 진단 품질 CT 영상을 획득할 수 있는 능력을 가지며, 이는 다수의 이점을 제공할 수 있다. 이점 중 하나로서, 환자는 치료 전의 영상화 및 치료 자체에 대해 정확히 동일한 방식으로 위치되어, 환자의 신체가 영상화와 치료 사이에서 상이한 방식으로 위치되거나 지지될 때 발생할 수 있는 치료 오류를 감소시킬 것이다.
결합된 CT/RT 시스템에 대한 다른 이점은 그의 제어 시스템 및 관련 소프트웨어의 사용을 통해 실현된다. 예를 들어 치료 계획을 재최적화하고 진단 품질 CT 영상화를 기반으로 온-테이블(on table) 적응 치료를 수행하도록 시스템을 구성할 수 있다.

따라서, 제어 시스템 및 소프트웨어의 기능은 CT 영상 획득, 변형 가능한 영상 정합, 자동 조직 분할/컨투어링, 선량 계산, 치료 계획 최적화 및 방사선 치료 전달을 포함하지만 이에 한정되지는 않는다.
도 14는 결합 CT/RT 시스템에 대한 예시적인 배치의 추가적인 도면을 포함한다. RF 차폐부는 본 명세서에 개시된 CT/RT 시스템의 특정 실시예에 포함될 수 있다. 예로서, 방사선 치료 빔 소스가 선형 가속기인 경우, 다양한 선형 가속기 구성 요소로부터의 RF 방사선은 실내 또는 환자 내의(예를 들어, 맥박 조정기, ICD 등) 장치와 간섭할 수 있다. 간섭을 줄이기 위한 한가지 방법은 선형 가속기 구성 요소(1102)에 대한 컨테이너에서 RF 차폐부를 이용하는 것이다. 이러한 컨테이너의 예는 도 14에서 볼 수 있고, 이는 본 양수인에게 양도된 미국 특허 제8,836,332호 및 제9,446,263호에 상세히 논의되어 있으며, 이는 본 명세서에 참조로서 인용된다.

특정 실시예에서, 선택된 치료 침상은 제한된 자유도를 가질 수 있다. 예를 들어, 침상은 보어의 내외 및 상하로의 이동만이(통상적인 기성품 CT 시스템의 경우와 같이) 가능할 수 있다. 이러한 측방향 이동의 부족은 타겟이 환자의 세로축의 측면에 있거나 정중면(midsaggital plane)으로부터 멀어지면 방사선 치료를 위해 환자를 위치시키는 데 문제를 일으킬 수 있다. 다수의 설계가 이 제한을 극복할 수 있다고 고려된다. 예를 들어, 기성품 CT 침상은 측면 운동이 가능한 플랫폼상에 장착될 수 있다. 대안적으로, 침상은 추가적인 자유도를 포함하도록 변경되거나 재설계될 수 있다. 도 15에 도시된 실시예에서, 방사선 치료 장치(101)[여기서 선택적 피트(1504)의 내부에 도시됨]는 침상 및 그의 보어 내에 위치된 환자에 대해 그 자체가 측방향으로 이동하도록 시프트되도록 구성될 수 있다. 하나의 실시예에서, 갠트리는 치료 전에 환자에게 방사선 치료 등중심을 위치시키는 것을 용이하게하기 위해, 적어도 8cm의 범위에 걸쳐서 침상 움직임에 직각으로 이동하도록 구성될 수 있다.

시스템은 MRI 가이던스 또는 X-선 CT 가이던스와 함께 설치되도록 설계될 수 있으며, 다용도 기초판 또는 다중 다용도 기초판을 통해 상이한 유형의 가이던스 사이의 변환을 용이하게 하도록 설계될 수도 있다(예를 들어 도 16을 참조하라). 기초판(들)은 단단히 장착하고 정렬하기에 충분한, 시스템 아래의 영역의 적어도 일부를 덮는다. 하나의 예시로서, 베이스 플레이트는 예를 들어, 1) RT 장치, 2) CT 스캐너 또는 MRI, 및 3) CT 침상, PET/CT 침상 또는 MRI 침상을 수용하기 위한 다수의 드릴 패턴(drill patterns)으로 설계될 수 있다. 이와 관련하여 시스템은 방사선 치료 장치 자체를 제거하거나 방해하지 않으면서 CT 가이던스에서 MRI 가이던스로 변환될 수 있다.

Claims (23)

환자의 영상화(imaging)를 위한 진단 품질 CT 스캐너(diagnostic-quality CT scanner)로서, 영상화 등중심을 가지는 진단 품질 CT 스캐너;
상기 진단 품질 CT 스캐너에 인접하여 위치된 방사선 치료 장치로서, 방사선 치료 빔 소스를 지지하며 상기 진단 품질 CT 스캐너의 상기 영상화 등중심과 분리된 방사선 치료 등중심을 가지는 갠트리(gantry)를 포함하는 방사선 치료 장치; 및
영상화 및 방사선 치료를 위해, 상기 진단 품질 CT 스캐너와 상기 방사선 치료 장치 사이에 상기 환자를 이동시킴으로써 환자를 위치시키도록 구성된 침상(couch)
을 포함하고,
동일 평면 방사선 치료만을 하도록 구성된, 시스템. - 제1항에 있어서,
상기 방사선 치료 장치는 캔틸레버형이 아닌, 시스템. - 제1항에 있어서,
상기 갠트리는 링 갠트리(ring gantry)인, 시스템. - 제1항에 있어서,
상기 갠트리가 상기 소스를 평면 내의 상이한 위치로만 이동시키도록 구성된, 시스템. - 제1항에 있어서,

상기 진단 품질 CT 스캐너가 RT 시뮬레이션을 위해 설계된, 시스템. - 제1항에 있어서,

상기 제1 다엽 시준기의 상기 잎 및 상기 제2 다엽 시준기의 상기 잎이 서로 독립적으로 움직이도록 구성되며, 상기 제1 다엽 시준기 및 상기 제2 다엽 시준기 중 적어도 하나가 이중 포커싱형(double focused)인, 시스템. - 제13항에 있어서,
