動物および細胞株
6〜8週齢のCB17/lcr SCID雌マウスをCharles River Laboratories (Sulzfeld、ドイツ)から入手した。すべてのマウスをマイクロアイソレーターユニット(microisolator unit)に入れ、試験期間を通して無菌の食餌と水を自由に与えた。EBV陰性ヒトB細胞リンパ腫細胞株Ramos44はアメリカン・タイプ・カルチャー・コレクション(ATCC、Manassas、バージニア州)から購入した。細胞をRPMI 1640培地(2mM L-グルタミン、10mM HEPES、1mM ピルビン酸ナトリウム、4.5g/L グルコース、1.5g/L 重炭酸塩、10%熱不活化ウシ胎仔血清、100U/mL ペニシリン、および100μg/mL ストレプトマイシンを含むように調整した)中で対数増殖期にて維持した。ヒト濾胞性リンパ腫細胞株DoHH-2はドイツ生物材料資源センター(German Resource Centre for Biological Material) (DSMZ、Braunschweig、ドイツ)から入手した。
L19は、フィブロネクチンのEDBドメインに対して誘導された血管ターゲッティング抗体である。SIP (small immunoprotein:小免疫タンパク質)フォーマットでのL19およびL19-IL2融合タンパク質の発現、精製、特性解析は、Borsiら(Int J Cancer. 2002;102:75-85)およびCarnemollaら(Blood. 2002;99:1659-1665)によってすでに記載されている。組換えヒトIL-2 (プロロイキン(Proleukin)、18×106IU)はProrero Pharma社(Liestal、スイス)から入手し、キメラ型ヒトIgG1抗CD20モノクローナル抗体であるリツキシマブ(MabThera)はRoche社(Reinach、スイス)から入手した。
Ramos異種移植腫瘍に対する免疫組織化学のために、凍結サンプルの10μmクリオスタット切片を氷冷アセトン中で固定し、TBS (50mmol/L Tris、100mmol/L NaCl pH7.4)中で再水和させて、20%FCS (Invitrogen社、Basel、スイス)を用いてブロッキングした。その切片にL19-SIPを加えて10μg/mLの最終濃度とした。結合した一次抗体は、ウサギ抗ヒトIgE抗体(Dako社、Glostrup、デンマーク)、続いてビオチン化ヤギ抗ウサギIgG抗体(Biospa社、Milan、イタリア)およびストレプトアビジン-アルカリホスファターゼ複合体(Biospa社)を用いて検出した。ホスファターゼ基質としてFast Red TRSalt (Sigma社)を用いた。ヒトリンパ腫サンプルでのEDB発現の免疫組織化学的解析は、ビオチン化L19-SIPとストレプトアビジン-アルカリホスファターゼ(SAP)を用いておこなった。切片をヘマトキシリンで対比染色し、グリセルゲル封入剤(Glycergel mounting medium) (Dako社)により封入して、Axiovert S100 TV顕微鏡(Zeiss社、Feldbach、スイス)を用いて解析した。ヒトリンパ腫サンプルでの免疫組織化学はリンパ腫異種移植片の場合と同様におこなったが、ビオチン化L19-SIPを一次抗体として用いて、ストレプトアビジン-アルカリホスファターゼ複合体(Biospa社)により検出した。
L19-SIPを蛍光シアニン化合物のCy3-NHSエステルで、メーカー(Amersham Pharmacia社、Duebendorf、スイス)の推奨に従って、標識した。120μgのL19-Cy3コンジュゲートをリンパ腫担持マウスの外側尾静脈に静脈内(i.v.)注入した。注入の24時間後、マウスを犠牲にして腫瘍を切除し、凍結包埋コンパウンド(cryoembedding compound) (Microm社、Walldorf、ドイツ)中に包埋して、−80℃で貯蔵した。10μmの切片を切り出し、37℃で15分間乾かし、4%パラホルムアルデヒドを用いて室温で15分間固定した。ラット抗CD31抗体(BD Pharmingen社)を加え、二次抗体としてAlexa Fluor 488ウサギ抗ラットIgG (Invitrogen社)を用いて内皮細胞を浮かび上がらせた。AxioCam MRcカメラを装備したAxioskop 2 Mot plus顕微鏡(Zeiss社)で画像を取得した。
in vivoターゲッティング能力を定量的に評価するために、放射性標識抗体調製物を用いた生体内分布解析を以前に記載されたとおりに実施した(Carnemolla et al., 2002)。簡単に説明すると、精製したSIP(L19)を125Iで放射性ヨウ素化し、皮下移植Ramosリンパ腫異種移植片を担持するSCIDマウスまたは全身的同系A20リンパ腫を担持するBalb/cマウスに静脈内注入した(マウスあたり10μg、12.2μCi)。注入の24時間後または48時間後にマウスを犠牲にしたが、各時点につき少なくとも3匹の動物を用いた。臓器を秤量し、Cobra γカウンター(Packard社、Meriden、コネチカット州)を用いて放射能を計測した。代表的な臓器の放射能含有量を、組織1gあたりの注入線量(ID)に対する百分率(%ID/g±SE)として表した。
1×107個のRamosリンパ腫細胞または1×107個のDoHH-2リンパ腫細胞を6〜8週齢のCB17/lcr SCID雌マウスの横腹に皮下注入した(0日目)。腫瘍が確立されて、明らかに触知可能になったとき(50〜100mm3、注入後8日目)、マウスを病期ごとに分類してグループ間の均一性を最大にし、外側尾静脈に20μgのL19-IL2 (6.6μgまたは118000IUのrIL-2に相当する)、6.6μgの非標的化rIL-2、200μgのリツキシマブ、または対照の生理食塩水のいずれかを100μLの容量で8、11、14および17日目に注入した(Q3D×4)。併用療法試験では、L19-IL2 (6.6および20μg、それぞれ「フリー」のrIL-2の2.2および6.6μgに相当する)、または非コンジュゲート化rIL-2 (2.2および6.6μg)をリツキシマブ(200μg)と組み合わせて、8、11、14および17日目に別々の静脈内注入により投与した。L19抗体単独が治療的に活性であるかを調べるため、SIP (x.xμg)またはIgG (x.xμg)フォーマットでのL19(等モル量)を単独でまたはフリーのrIL-2 (6.6μg)と組み合わせて用いて、マウスを処置した。すべての薬剤(単剤療法と併用療法で用いたもの)の治療スケジュールは、3日ごとに合計4回(Ramos)または3回(DoHH-2)の注入とした(それぞれQ3D×4またはQ3D×3)。
全身性疾患のモデルにするため、SCIDマウスに、200μLのPBS中に再懸濁した2×106個のRamosリンパ腫細胞を静脈内注入した。治療を開始する前に、B細胞リンパ腫の播種と増殖を8日間を行った。マウスを無作為に6グループ(n>6)に分けて、20μgのL19-IL2、6.6μgの非コンジュゲート化rIL-2、もしくは200μgのリツキシマブ(単剤療法)、または20μgのL19-IL2と組み合わせた200μgのリツキシマブ、もしくは6.6μgの非コンジュゲート化rIL-2と組み合わせた200μgのリツキシマブ(併用療法)、または生理食塩水のいずれかを8、11、14および17日目に静脈内注入した(Q3D×4)。後肢麻痺の存在または悪化しつつある状態の兆候についてマウスを毎日観察し、そのときにはすぐにマウスを犠牲にした。麻痺の発症または死亡をエンドポイントとして設定し、マウスの生存を治療効果の解析のために記録した。播種性リンパ腫モデルを用いた動物実験は、プロジェクトライセンス「腫瘍ターゲッティング」の補正1に従って実施した。
データは平均±SEとして表される。異なるマウスグループ間の腫瘍体積の差異は、スチューデントの両側t検定を用いて比較した。播種性リンパ腫モデルにおける治療効果を示すためにKaplan-Meier生存曲線を実施し、log-rank検定を用いて比較した。両側P値<0.05を有意とみなした。
in vitro局在確認:異種移植腫瘍への免疫組織化学
Ramosリンパ腫異種移植片の切片への免疫組織化学的解析は、フィブロネクチンのEDBドメインに特異的なL19抗体を用いて実施した。図1(左のパネル)に示したように、リンパ腫組織中の血管構造物の特異的染色がL19について観察されたが、これはヒトU87グリア芽腫異種移植片(右のパネル)により例示される固形腫瘍でのL19の染色パターンによく似ていた。リンパ腫異種移植片におけるEDBの発現パターンは、このアイソフォームがin vivoでリンパ腫部位への生物活性化合物の選択的送達のための標的として役立ちうることを示している。
次のステップでは、リンパ腫異種移植片に発現されたEDBフィブロネクチンがin vivoで血流からのL19抗体にとって接近可能であるか否かを検討した。そのために、皮下Ramosリンパ腫を担持するマウスに、フルオロフォアCy3で化学的に標識したL19-SIPを静脈内注入した。24時間後、動物を犠牲にして、腫瘍切片を「材料および方法」に記載したとおりに処理した。図2aは、リンパ腫切片の2色蛍光顕微鏡画像を示しており、腫瘍の血管構造物への抗体局在が確認される。
抗体-サイトカイン融合タンパク質L19-IL2はさまざまな固形腫瘍モデルにおいて強力な抗癌活性を示すことが以前に明らかにされている (Menrad et al. (Expert Opin Ther Targets. 2005;9:491-500)、Carnemolla et al. (Blood. 2002;99:1659-1665))。B細胞リンパ腫におけるL19-IL2の単剤療法効果を評価するために、SCIDマウスに1×107個のRamos細胞を皮下注入した。腫瘍細胞を移植してから8日目に、腫瘍の大きさが50〜100mm3に達した時点で、マウス(n>4)に20μgのL19-IL2 (6.6μgのrIL-2に相当する)、6.6μgの非コンジュゲート化rIL-2、200μgのリツキシマブ、または生理食塩水のいずれかを静脈内投与した(Q3D×4)。図3は、単剤L19-IL2および単剤リツキシマブが、生理食塩水で処置した対照マウスと比較して、リンパ腫増殖を実質的に抑制した(それぞれP=0.024およびP=0.004)ことを示している。これとは対照的に、等モル量の非コンジュゲート化rIL-2は顕著な治療効果を示さず(P=0.383)、固形癌の動物モデルについて以前に報告されていた治療効果と同様であった。これらは、サイトカインの抗体介在血管ターゲッティングが治療効果に貢献する(L19-IL2対IL-2: P=0.044)ことを実証するものである。しかしながら、L19-IL2もリツキシマブも、単剤療法として用いた場合には、腫瘍増殖を遅らせただけであり、この実験ではすべての動物に進行性の疾患が認められた。融合タンパク質L19-IL2は再現性よくリンパ腫増殖を抑制した(P=0.031)が、SIPまたはIgGフォーマットでの裸のL19は、等モル量を単独でまたはフリーのrIL-2と組み合わせて投与したとき、治療的に不活性であった。このことは、L19-IL2の治療活性がリンパ腫部位へのサイトカインの標的化送達に依存する(図7)という概念をさらに強めるものである。
リツキシマブの臨床上の有効性を高めるための方法がいろいろ報告されており、例えば、リンパ腫細胞のADCC介在死滅を増強するためのrIL-2の投与が報じられている(Cartron et al., Blood. 2004;104:2635-2642)。こうして、本発明者らは、血管標的化IL-2と抗CD20療法の組合せがいずれか一方の治療手法よりも治療上有効であるか、特に、リンパ腫組織へのIL-2の抗体介在蓄積が非コンジュゲート化サイトカインとリツキシマブの組合せの有効性を果たして超えるか、を検討した。そのために、次のスキームに従って併用療法実験をおこなった(>5匹/グループ):200μgのリツキシマブ+2.2μgの非コンジュゲート化rIL-2 (「低用量」)、200μgのリツキシマブ+6.6μgの非コンジュゲート化rIL-2 (「高用量」)、200μgのリツキシマブ+6.6μgのL19-IL2 (「低用量」、2.2μgのrIL-2に相当する)、200μgのリツキシマブ+20μgのL19-IL2 (「高用量」、6.6μgのrIL-2に相当する)、または対照の生理食塩水。単剤療法実験と同様に、腫瘍細胞の接種後8日目(触知可能な皮下異種移植片が現われたとき)に注入を開始し、3日ごとに合計4回の注入を繰り返した。
播種性リンパ腫異種移植片に対する単剤およびリツキシマブとの組合せとしてのL19-IL2の治療活性
ヒトの進行NHL(非ホジキンリンパ腫)は一般に播種性疾患として発症する。全身性リンパ腫に対するL19-IL2の活性を検討するために、本発明者らは播種性SCID/Ramosリンパ腫モデルを選んだ。リンパ腫細胞を静脈内に接種されたSCIDマウスは通常、後肢の麻痺をおこすが、この麻痺は脊髄でのリンパ腫症状から生じ、この疾患の終末期を示している。公表された観察によれば、Ramos細胞の静脈内注入によって、パイロット実験のすべてのケースで26日までに後肢の麻痺が現われて、移植した細胞の100%の生着率を示した(データは示さず)。どのケースでも麻痺が死に先行したので、後肢麻痺の出現を生存率解析のためのエンドポイントとして設定した。リンパ腫細胞の生着と増殖を確実にするため、治療開始を8日間延期した。薬剤の投与およびスケジュールは限局性Ramosリンパ腫モデルで採用したものと同じであり、単剤療法(rIL-2、L19-IL2、リツキシマブ)と併用療法(リツキシマブ+rIL-2、リツキシマブ+L19-IL2)双方の活性をこの実験で同時に評価した。
最後に、免疫組織化学的解析によって、びまん性大細胞型B細胞リンパ腫およびバーキットリンパ腫を含めて、ヒトB細胞悪性疾患におけるEDBフィブロネクチンの存在と血管発現パターンを確認した(図6)。

- 前記癌がリンパ腫である、請求項21に記載の医薬。
- リンパ腫が非ホジキンリンパ腫(NHL)を含むB細胞リンパ腫である、請求項22に記載の医薬。
- 請求項1〜20のいずれか1項に記載の組合せ物を含む自己免疫疾患の治療用医薬。
- 自己免疫疾患が、関節リウマチ、クローン病、潰瘍性大腸炎および自己免疫性溶血性貧血から選択される、請求項24に記載の医薬。
