本発明は、Aβ関連疾患若しくは障害の予防および処置のための、とりわけAβ関連疾患若しくは障害を有するか若しくはその危険性がある患者での認知の改善(例えば認知の迅速な改善)を治療上遂げるための免疫学的試薬、とりわけ治療的抗体試薬を特徴とする。本発明は、少なくとも部分的に、Aβペプチド内のエピトープに特異的に結合する数種のモノクローナル抗体の同定および特徴付けに基づく。本発明は、特定の活性、とりわけ、可溶性オリゴマーAβに優先的に結合する能力、および/若しくはAβ関連の認知障害の適切な動物において測定されるところの認知を迅速に改善する能力を有するAβ抗体の選択を特徴とする。
本発明を記述する前に、下で使用されるべきある用語の定義を示すことがその理解に有用でありうる。
本明細書で使用されるところの「Aβ関連疾患若しくは障害」という用語は、Aβペプチドの発生若しくは存在と関連するか若しくはそれらを特徴とする疾患若しくは障害を指す。一態様において、Aβ関連疾患若しくは障害は可溶性Aβの存在と関連するか若しくはそれを特徴とする。別の態様において、Aβ関連疾患若しくは障害は、不溶性Aβの存在と関連するか若しくはそれを特徴とする。別の態様において、Aβ関連疾患若しくは障害は、向神経活性Aβ種(NAβ)の存在と関連するか若しくはそれを特徴とする。別の態様において、Aβ関連疾患若しくは障害はまたアミロイド形成障害でもある。別の態様において、Aβ関連疾患若しくは障害は、Aβ関連の認知障害(deficit)若しくは障害(disorder)、例えばAβ関連痴呆障害を特徴とする。例示的Aβ関連疾患若しくは障害は、アルツハイマー病(AD)、ダウン症候群、脳アミロイド血管症、ある種の血管性痴呆および軽度認知障害(MCI)を包含する。

本発明の治療薬はAβに特異的に結合する抗体を包含する。好ましい抗体はモノクローナル抗体である。いくつかの態様において、受動免疫療法での使用のための本発明の抗体は、単量体の可溶性および/若しくはオリゴマーの可溶性Aβポリペプチド(例えば可溶性Aβ二量体、三量体など)を包含する可溶性Aβペプチドを結合する。他の態様において、該抗体は、単量体の可溶性ならびにオリゴマーの可溶性Aβポリペプチド(例えば可溶性Aβ二量体、三量体など)を包含する可溶性Aβを捕捉し、かつ、Aβの蓄積を予防しかつ/若しくはCNSからのAβの除去を促進することが可能である。とりわけ好ましい一態様において、本発明の抗体は可溶性および不溶性双方のAβペプチド若しくはそれらのフラグメントに結合することが可能であり、ならびに、既存のアミロイド斑の大きさおよび密度もまた減少させつつ付加的なアミロイド斑の形成を予防することが可能である。他の例示的態様において、Aβ関連の認知障害の適切な動物モデルで有効性を示す抗体を、本発明の治療方法での使用のための試薬として選択する。
本発明はヒト以外の抗体、例えば本発明の好ましいAβエピトープに対する特異性を有する抗体を特徴とする。こうした抗体は、本発明の多様な治療的組成物の処方において使用し得るか、または、好ましくはヒト化若しくはキメラ抗体(詳細に下述される)の製造のための相補性決定領域を提供し得る。ヒト以外のモノクローナル抗体、例えばマウス、モルモット、霊長類、ウサギ若しくはラットの製造は、例えばAβで動物を免疫することにより達成し得る。Aβ若しくはAβの免疫原性フラグメントを含んでなるより長いポリペプチド、またはAβに対する抗体に対する抗イディオタイプ抗体もまた使用し得る。HarlowとLane、上記(全部の目的上引用することにより組み込まれる)を参照されたい。こうした免疫原は天然の供給源から、ペプチド合成若しくは組換え発現により得ることができる。場合によっては、免疫原は下述されるとおり担体タンパク質に融合若しくは別の方法で複合体形成して投与し得る。場合によっては、免疫原はアジュバントともに投与し得る。「アジュバント」という用語は、抗原とともに投与される場合に該抗原に対する免疫応答を増強するが、しかし単独で投与される場合に該抗原に対する免疫応答を生成しない化合物を指す。アジュバントは、リンパ球動員、Bおよび/若しくはT細胞の刺激、ならびにマクロファージの刺激を包含する数種の機構により免疫応答を増強し得る。数種の型のアジュバントを下述されるとおり使用し得る。フロイントの完全アジュバント、次いで不完全アジュバントが実験動物の免疫化に好ましい。
本発明は、βアミロイドペプチドに特異的なキメラおよび/若しくはヒト化抗体(例えばキメラおよび/若しくはヒト化免疫グロブリン)もまた特徴とする。キメラおよび/若しくはヒト化抗体は、キメラ若しくはヒト化抗体の構築のための出発原料を提供するマウス若しくは他のヒト以外の抗体と同一若しくは類似の結合特異性および親和性を有する。
「キメラ抗体」という用語は、そのLおよびH鎖遺伝子が典型的には遺伝子工学により、異なる種に属する免疫グロブリン遺伝子セグメントから構築された抗体を指す。例えば、マウスモノクローナル抗体からの遺伝子の可変(V)セグメントを、IgG1およびIgG4のようなヒト定常(C)セグメントに結合しうる。ヒトアイソタイプIgG1およびIgG4が例示的である。典型的なキメラ抗体は、従って、マウス抗体からのVすなわち抗原結合ドメインおよびヒト抗体からのCすなわちエフェクタードメインよりなるハイブリッドタンパク質である。
「ヒト化抗体」という用語は、実質的にヒト抗体鎖(アクセプター免疫グロブリンすなわち抗体と称される)からの可変領域フレームワーク残基、および実質的にマウス抗体(ドナー免疫グロブリンすなわち抗体と称される)からの最低1個の相補性決定領域を含んでなる最低1本の鎖を含んでなる抗体を指す。Queenら、Proc.Natl.Acad.Sci.USA 86:10029−10033(1989)、米国特許第5,530,101号、同第5,585,089号、同第5,693,761号、同第5,693,762号、Selickら、第WO 90/07861号、およびWinter、米国特許第5,225,539号明細書(全部の目的上そっくりそのまま引用することにより組み込まれる)を参照されたい。定常領域(1個若しくは複数)もまた、存在する場合、実質的に若しくは完全にヒト免疫グロブリンからである。

(2)CDR領域に隣接するか、
(3)CDR領域と別の方法で相互作用する(例えばコンピュータモデル化により決定されるところのCDR領域の約3〜6Å以内である)か、若しくは
(4)VL−VH界面に参画する
ことが合理的に期待される場合に、マウス抗体からの同等のフレームワークアミノ酸により、通常は置換すべきである。
決定するのに重要(例えばCDRと相互作用することが可能)であることが既知である(ChothiaとLesk、上記、Chothiaら、上記およびTramontanoら、J.Mol.Biol.215:175(1990)(それらの全部は引用することにより本明細書に組み込まれる))。これらの著者らは、数種の既知の抗体の構造の分析によりCDRコンホメーションに重要な保存されたフレームワーク残基を同定した。分析した抗体は、CDRのコンホメーションに基づき、制限された数の構造若しくは「正準」分類に属した。正準分類のメンバー内の保存されたフレームワーク残基は「正準」残基と称される。正準残基は、L鎖の残基2、25、29、30、33、48、64、71、90、94および95、ならびにH鎖の残基24、26、29、34、54、55、71および94を包含する。付加的な残基(例えばCDR構造決定残基)は、MartinとThorton(1996)J.Mol.Biol.263:800の方法論に従って同定し得る。注目すべきことに、L鎖の位置2、48、64および71ならびにH鎖の26−30、71および94のアミノ酸(Kabatによる番号付け)は、多くの抗体中でCDRと相互作用することが可能であることが既知である。L鎖の位置35ならびにH鎖の93および103のアミノ酸もまたCDRと相互作用することがありそうである。CDRのコンホメーションを遂げうる付加的な残基は、FooteとWinter(1992)J.Mol.Biol.224:487の方法論に従って同定し得る。こうした残基は「バーニア」残基と命名され、そしてCDRのすぐ下の(すなわちその下の「基盤」を形成する)フレームワーク領域中の残基である。全部のこれらの番号付けられた位置で、ヒト化免疫グロブリン中にあるべきアクセプターアミノ酸よりむしろドナーアミノ酸(それらが異なる場合)の選択が好ましい。他方、L鎖の最初の5アミノ酸のようなCDR領域と相互作用することが可能なある残基を、ときに、ヒト化免疫グロブリンでの親和性の喪失を伴わずにアクセプター免疫グロブリンから選ぶことができる。

本発明は、Aβ関連疾患若しくは障害を処置するための、とりわけ、Aβ関連疾患若しくは障害を有する若しくはその危険性がある患者で認知の迅速な改善を遂げるための免疫学的試薬および改良された方法を特徴とする。とりわけ、該試薬および方法は、AD若しくは他のアミロイド形成疾患を有する若しくはその危険性がある患者の処置において有用である。本発明は、少なくとも部分的に、とりわけin vitroおよび/若しくはin vivo活性アッセイで測定されるところの特徴的な生物学的活性を有する多様なモノクローナル免疫グロブリンの同定および特徴付けに基づく。例示的態様において、単量体Aβに比較して可溶性オリゴマーAβを優先的に結合する(若しくはそれに対する増大された親和性を有する)抗体を、本発明の治療方法での使用のため試薬として選択する。他の例示的態様において、Aβ関連の認知障害の適切な動物モデルで有効性を示す抗体を、本発明の治療方法での使用のため試薬として選択する。抗体は、以下の活性、すなわち、βアミロイドタンパク質(Aβ)の結合(例えば可溶性および/若しくは凝集型Aβの結合)、(例えば凝集型Aβの)食作用の媒介、斑負荷量の低下、神経炎性ジストロフィーの低下および/または認知の改善(例えば被験体における)で有効の最低1種をさらに有しうる。

上述されたとおり製造した抗体の可変セグメント(例えばキメラ若しくはヒト化抗体のHおよびL鎖可変領域)は、典型的には免疫グロブリン定常領域(Fc領域)(典型的にはヒト免疫グロブリンのもの)の少なくとも一部分に連結する。ヒト定常領域のDNA配列は多様なヒト細胞、しかし好ましくは不死化B細胞から公知の手順に従って単離し得る(Kabatら、上記、およびLiuら、第W087/02671号明細書(それらのそれぞれは全部の目的上そっくりそのまま引用することにより組み込まれる)を参照された

Proteins of Immunological Interest、米国保健福祉省刊行物(U.S.Department of Health Human Services Publication)を参照されたい)。IgG分子中の2本のH鎖のそれぞれは、例えば位置297のアスパラギン残基のアミド基に連結されている単一の分枝状鎖炭水化物基を有する。例えばアスパラギンのアラニンでの置換は、例えば米国特許第6,706,265号明細書(引用することにより本明細書に組み込まれる)に記述されるとおり抗体のグリコシル化を予防する。特定の一態様において、位置297のアミノ酸残基Asnをアラニンに突然変異する。
キメラおよびヒト化抗体は典型的に組換え発現により製造する。場合によっては定常領域に連結されたLおよびH鎖可変領域をコードする核酸を発現ベクターに挿入する。LおよびH鎖は同一若しくは異なる発現ベクターにクローン化し得る。免疫グロブリン鎖をコードするDNAセグメントは、免疫グロブリンポリペプチドの発現を確実にする、発現ベクター(1種若しくは複数)中の制御配列に作動可能に連結される。発現制御配列は、限定されるものでないが、プロモーター(例えば天然に関連する若しくは異種のプロモーター)、シグナル配列、エンハンサー要素および転写終止配列を挙げることができる。好ましくは、発現制御配列は、真核生物宿主細胞(例えばCOS若しくはCHO細胞)を形質転換若しくはトランスフェクトすることが可能なベクター中の真核生物プロモーター系である。ベクターが適切な宿主に一旦組み込まれれば、該宿主を、ヌクレオチド配列の高レベル発現、ならびに交差反応性抗体の収集および精製に適する条件下で維持する。
抗体フラグメントもまた本発明の範囲内で企図している。一態様において、ヒト以外のおよび/若しくはキメラの抗体のフラグメントが提供される。別の態様において、ヒト化抗体のフラグメントが提供される。典型的には、これらのフラグメントは最低107、およびより典型的には108若しくは109M−1の親和性での抗原への特異的結合を表す。ヒト化抗体フラグメントは、別個のH鎖、L鎖、Fab、Fab’、F(ab’)2、FabcおよびFvを包含する。フラグメントは組換えDNA技術、または無傷の免疫グロブリンの酵素的若しくは化学的分離により製造する。
エピトープマッピングを実施して、Aβのどの抗原決定基すなわちエピトープが該抗体により認識されるかを決定し得る。一態様において、エピトープマッピングは置換NET(rNET)分析に従って実施する。rNETエピトープ地図アッセイは、該抗体の全体的結合活性への該エピトープ内の個々の残基の寄与についての情報を提供する。rNET分析は合成された体系的単一置換ペプチドアナログを使用する。試験されている抗体の結合を、天然のペプチド(天然の抗原)および19種の代替の「単一置換」ペプチド(各ペプチドは第一の位置でその位置の19種の非天然のアミノ酸の1種で置換されている)に対し測定する。多様な非天然の残基でのその位置の置換の影響を反映したプロファイルが生成される。プロファイルは同様に、抗原ペプチドに沿った連続する位置でも生成される。組み合わせたプロファイル、すなわちエピトープ地図(全19種の非天然のアミノ酸での各位置の置換を反映する)をその後、第二の抗体について同様に生成した地図と比較し得る。実質的に類似若しくは同一の地図は、比較されている抗体が同一若しくは類似のエピトープ特異性を有することを示す。
7〜9月齢のPDAPPマウスの群それぞれに、PBS中0.5mgのポリクローナル抗Aβ若しくは特異的抗Aβモノクローナル、ヒト化またはキメラ抗体を注入する。全抗体調製物は低エンドトキシンレベルを有するように精製する。モノクローナル抗体は、Aβのフラグメント若しくはより長い形態をマウスに注入すること、ハイブリドーマを調製すること、およびAβの他の重ならないフラグメントに結合することなくAβの所望の1フラグメントに特異的に結合する抗体について該ハイブリドーマをスクリーニングすることにより、1フラグメントに対し製造し得る。ヒト化および/若しくはキメラ抗体は本明細書に記述されるとおり製造する。
ELISA力価により測定される循環抗体濃度を維持ため、マウスに4か月間にわたり必要とされるように腹腔内注入する。力価をモニターし、そしてマウスを6か月の注入の終了時に安楽死させる。組織化学、Aβレベルおよび毒物学を死後に実施する。10匹のマウスを1群あたりに使用する。
本発明はまた、生化学的アッセイでの可溶性のオリゴマーAβに結合する抗体の能力の試験方法も提供する。該生化学的アッセイは、少なくとも部分的に、単量体Aβへの抗体の結合に比較した1種若しくはそれ以上の形態の可溶性オリゴマーAβ(例えばAβ二量体、Aβ三量体、Aβ四量体、Aβ五量体など)への該抗体の結合の比較に基づく。この比較を使用して、単量体Aβに比較しての可溶性オリゴマーAβへの抗体の相対的結合を決定し得る。多様な態様において、この相対的結合を、単量体Aβに対する1種若しくはそれ以上の可溶性オリゴマーAβへの対照試薬の対応する相対的結合と比較する。他の局面において、1種若しくはそれ以上のオリゴマーAβ種に対する抗体の親和性を、Aβ調製物中の単量体Aβに対する該抗体の親和性と比較する。可溶性オリゴマーAβ種を優先的に結合するAβ抗体の能力と、詳細に下述されるところの適切なモデル動物でのCFCアッセイにより評価されるところの認知を迅速に改善する該抗体の能力の間に、強い相関が存在することが発見された。CFCアッセイでの認知を改善する抗体の能力は、該抗体の最終的なヒト治療の有効性(とりわけ患者での認知の迅速な改善における有効性)の強力な指標若しくは予測因子であるとさらに考えられる。従って、Aβ抗体結合の優先性および/若しくは親和性の比較は、本発明の治療法での使用、とりわけ患者で認知の迅速な改善を遂げる方法での使用のための候補としてのある種の抗体の同定に至る。
いずれを使用しても実施し得る。適する標識は、限定されるものでないが、蛍光標識、放射活性標識、常磁性標識、若しくはそれらの組合せを挙げることができる。
本発明は、消失活性が望ましいアミロイド沈着物若しくはいずれかの他の抗原、または関連する生物学的実体の消失における活性についての抗体のスクリーニング方法もまた提供する。アミロイド沈着物に対する活性についてスクリーニングするため、アルツハイマー病を伴う患者、若しくは特徴的なアルツハイマーの病理学を有する動物モデルの脳からの組織サンプルを、小膠細胞のようなFc受容体を担持する食細胞および試験中の抗体と媒体中でin vitroで接触させる。食細胞は初代培養物若しくは細胞株であり得、そしてマウス(例えばBV−2若しくはC8−B4細胞)またはヒト起源(例えばTHP−1細胞)のものであり得る。いくつかの方法において、顕微鏡モニタリングを容易にするために成分を顕微鏡用スライドガラス上で組合せる。いくつかの方法において、複数の反応をマイクロタイター皿のウェル中で同時に実施する。こうした形式では、別個の小型顕微鏡用スライドガラスを別個のウェルにマウントし得るか、若しくはAβのELISA検出のような非顕微鏡的検出形式を使用し得る。好ましくは、一連の測定は、反応が進行する前の基礎値から出発するin vitro反応混合物中のアミロイド沈着物の量、および反応中の1個若しくはそれ以上の試験値から構成される。抗原は、例えばAβ若しくはアミロイド斑の他成分に対する蛍光標識抗体での染色により検出し得る。染色に使用する抗体は、消失活性について試験されている抗体と同一であってももしくはなくてもよい。アミロイド沈着物の反応中の基礎に関しての低下は、試験中の抗体が消失活性を有することを示す。こうした抗体はアルツハイマー病および他のアミロイド形成疾患の予防若しくは処置において有用であることがありそうである。アルツハイマー病および他のアミロイド形成疾患を予防若しくは処置するためのとりわけ有用な抗体は、稠密および拡散双方のアミロイド斑を消失させることが可能なもの、例えば本発明の12A11抗体、またはそのキメラ若しくはヒト化バージョンを包含する。
多様な局面において、本発明の抗体は、認知を改善する能力について適切な動物モデルで試験し得る。例えば、コンテクチュアル・フィアー・コンディショニング(CFC)アッセイを介して評価されるところのADの動物モデルで認知を改善する抗体の能力を使用して、本発明の治療方法、とりわけ患者での認知の迅速な改善を遂げる方法での使用のための候補として該抗体を選択し得る。
定常領域(Fc領域)を含んでなる本発明の上述された抗体について、該分子のエフェクター機能を変えることもまた望ましいことがある。一般に、抗体のエフェクター機能は、多様なエフェクター分子、例えば補体タンパク質若しくはFc受容体への結合を媒介し得る該分子の定常すなわちFc領域に存する。Fc領域への補体の結合は、例えば細胞病原体の捕食および溶解ならびに炎症応答の活性化で重要である。例えばエフェクター細胞の表面上のFc受容体への抗体の結合は、例えば抗体で被覆された病原体若しくは粒子の貪食および破壊、免疫複合体の消失、キラー細胞による抗体で被覆された標的細胞の溶解(すなわち抗体依存性の細胞媒介性細胞傷害すなわちADCC)、炎症メディエーターの放出、抗体の胎盤通過、ならびに免疫グロブリン産生の制御を包含する多数の重要かつ多彩な生物学的応答を誘発し得る。
13.親和性成熟
本発明の抗体(例えばヒト化抗体)は、多数の親和性成熟技術のいずれかを使用して、改良された機能のため改変し得る。典型的には、所定の標的分子に対する結合親和性をもつ候補分子を同定し、そしてその後、標的分子とのより望ましい結合相互作用を有する1種若しくはそれ以上の関係する候補をもたらす突然変異誘発技術を使用してさらに改良すなわち「成熟」させる。典型的には、それは改変される抗体の親和性(affinity)(若しくは親和性(avidity)、すなわち標的抗原に対する抗体の結合した親和性)であるが、しかしながら、安定性、エフェクター機能、消失、分泌若しくは輸送機能のような該分子の他の特性もまた、親和性成熟技術を使用して別個に若しくは親和性と同時にのいずれかで改変しうる。
アミロイド沈着物に対する免疫応答は、受動免疫に使用される抗体およびそれらの成分鎖をコードする核酸の投与によってもまた導入し得る。こうした核酸はDNA若しくはRNAであり得る。免疫原をコードする核酸セグメントは、典型的には、患者の意図される標的細胞中での該DNAセグメントの発現を可能にするプロモーターおよびエンハンサーのような調節エレメントに連結する。免疫応答の誘導のため望ましいところの血液細胞中での発現のためには、例示的プロモーターおよびエンハンサー要素は、L若しくはH鎖免疫グロブリン遺伝子ならびに/またはCMV主要前初期プロモーターおよびエンハンサー(Stinski、米国特許第5,168,062号および同第5,385,839号明細書)からのものを包含する。連結した調節エレメントおよびコーディング配列はしばしばベクターにクローン化される。二本鎖抗体の投与のためには、2本の鎖を同一若しくは別個のベクターにクローン化し得る。
本発明は、とりわけ、可溶性Aβを特徴とするアミロイド形成障害および疾患(例えばアルツハイマー病)を包含するAβ関連疾患若しくは障害の処置に向けられる。本発明は、Aβ関連疾患若しくは障害またはアミロイド形成疾患若しくは障害の処置若しくは予防のための医薬品の製造における開示される免疫学的試薬(例えばヒト化免疫グロブリン)の使用にもまた向けられる。本発明の処置方法は、例えば、Aβ関連疾患若しくは障害またはアミロイド形成疾患若しくは障害の予防若しくは処置のための患者における、患者で有益な治療応答(例えば認知の迅速な改善、Aβの食作用の誘導、斑負荷量の減少、斑形成の阻害、神経炎性ジストロフィーの低減、および/または認知低下の逆転、処置若しくは予防)を生成させる条件下での患者への開示される免疫学的試薬(例えばAβ内の特定のエピトープに対するヒト化免疫グロブリン)の投与を含んでなる。こうした疾患は、アルツハイマー病、ダウン症候群および軽度認知障害を包含する。後者はアミロイド形成疾患の他の特徴を伴い若しくは伴わずに発生し得る。
患者に治療上の利益を提供することが示されているいかなる障害も処置しうることが、当業者により認識されるであろう。例えば、障害はいかなる認知障害、例えば痴呆障害でもありうる。こうした認知障害は、多数の起源、すなわち、機能的機序(不安、うつ)、生理学的加齢(加齢による記憶障害)、薬物若しくは解剖学的病変を有しうる。本発明の免疫治療薬が有用であり得る適応症は、毒物曝露による学習障害若しくは記憶障害、健忘に至る脳傷害、加齢、統合失調症、癲癇、精神遅滞、アルコール性ブラックアウト、コルサコフ症候群、薬物誘発性健忘(例えばハルシオン)、脳底動脈偏頭痛、若しくは単純ヘルペス脳炎と関連する記憶喪失を包含する。

本発明は、Aβ関連疾患若しくは障害またはアミロイド形成疾患若しくは障害(例えばAD)を有するか若しくはその危険性がある患者での認知の迅速な改善を遂げる方法を提供する。好ましい局面において、該方法は、認知の迅速な改善が達成されるような有効用量の免疫学的試薬(例えばAβ抗体)を投与することを特徴とする。本発明の例示的局面において、患者での1種若しくはそれ以上の認知障害(例えば手続き学習および/若しくは記憶障害)の改善が達成される。認知障害は、意識に利用可能でありかつ従って言語により表現され得る特定の情報を保存かつ想起する能力(例えば特定の事実若しくは事象を思い出す能力)と定義される顕在記憶(「陳述」若しくは「作業」記憶としてもまた知られる)の障害であり得る。あるいは、認知障害は、意識に利用可能でなくかつ表現されるために技術、連合、習慣若しくは複雑な反射の学習を必要とする、一般情報若しくは知識を取得、保持および想起する能力、例えば特殊な課題をどのように実行するかを思い出す能力と定義される手続記憶(「潜在」若しくは「文脈」記憶としてもまた知られる)の障害であり得る。手続記憶障害に罹患している個体は正常に機能する彼らの能力がはるかにより損なわれている。であるから、手続記憶の障害の改善において有効である処置が高度に望ましくかつ有利である。
処置に従いやすい患者は、Aβ関連疾患若しくは障害またはアミロイド形成疾患若しくは障害の危険性があるがしかし症状を示していない個体、ならびに現在症状を示している患者を包含する。アルツハイマー病の場合、事実上誰でも彼若しくは彼女が十分に長く生きる場合にアルツハイマー病に罹患する危険性がある。従って、本方法は、主題の患者の危険性のいかなる評価に対する必要性も伴わずに一般集団に予防的に投与し得る。

予防的応用において、製薬学的組成物若しくは医薬品を、アルツハイマー病に感受性の、若しくは別の方法でその危険性がある患者に、該危険を排除もしくは低下、重症度を低下、または、該疾患の生化学的、組織学的および/若しくは行動的症状、その合併症、ならびに該疾患の発症の間に現れる中間の病理学的表現型を包含する該疾患の発症を遅らせるのに十分な量で投与する。治療的応用においては、組成物若しくは医薬品を、こうした疾患に感受性の、若しくはすでに罹患している患者に、その合併症および該疾患の発症での中間の病理学的表現型を包含する該疾患の症状(生化学的、組織学的および/若しくは行動的)を治癒若しくは少なくとも部分的に抑止するのに十分な量で投与する。

本発明は、アルツハイマー病に罹患しているか若しくはそれに感受性の患者における処置のモニター方法、すなわち患者に投与されている処置の経過のモニター方法を提供する。該方法を使用して、症候性患者での治療的処置および無症候性患者での予防的処置の双方をモニターし得る。とりわけ、該方法は受動免疫をモニターする(例えば投与された抗体のレベルを測定する)のに有用である。

本発明は患者中のアミロイド沈着物のin vivo画像化方法を提供する。こうした方法はアルツハイマー病若しくはそれに対する感受性を診断若しくはその診断を確認するのに有用である。例えば、該方法を痴呆の症状で診察を受ける患者で使用し得る。患者が異常なアミロイド沈着物を有する場合には、該患者はアルツハイマー病に罹患していることがありそうである。該方法は無症状の患者でもまた使用し得る。アミロイドの異常な沈着物の存在は将来の症候性疾患に対する感受性を示す。該方法は、以前にアルツハイマー病と診断された患者での疾患の進行および/若しくは処置に対する応答をモニターするのにもまた有用である。
単回投与第I相試験をヒトでの安全性を決定するため実施し得る。治療薬(例えば本発明の抗体)を、想定される有効性のレベルの約0.01から開始しかつ有効なマウス投薬量の約10倍のレベルに達するまで3の係数により増大させる、増大する投薬量で多様な患者に投与する。
てアルツハイマー病・関連障害協会(Alzheimer’s disease and
Related Disorders Association)(ADRDA)の基準を使用して定義される早期ないし中期アルツハイマー病を伴う患者を選択する。適する患者は、ミニメンタルステート検査(MMSE)で12〜26の範囲で得点する。他の選択基準は、患者が試験の期間中生存しかつ妨害しうる付随する医薬品の使用のような複雑にする問題を欠くことがありそうであることである。患者機能の基礎評価は、MMSE、およびADAS(アルツハイマー病の状態および機能を伴う患者を評価するための包括的尺度である)のような古典的精神測定尺度を使用して行う。これらの精神測定尺度はアルツハイマー病の状態の進行の尺度を提供する。適する質的生活尺度もまた処置をモニターするのに使用し得る。疾患の進行はMRIによってもまたモニターし得る。免疫原特異的抗体およびT細胞応答のアッセイを包含する患者の血液プロファイルもまたモニターし得る。

PDAPPマウスをmAb 12B4若しくはmAb 3D6(双方ともIgG1アイソタイプのもの)いずれかで受動免疫した。12B4は10mg/kgで試験した。mAb 3D6は3種の異なる用量すなわち10mg/kg、1mg/kgおよび10mg/kg月1回(1×4)で試験した。無関係のIgG1抗体(TY 11/15)およびPBS注入が対照としてはたらいた。Aβペプチドでの能動免疫が比較としてはたらいた。20と35の間の動物を各群で分析した。アッセイした神経病理学的エンドポイントはアミロイド負荷量および神経炎性負荷を包含する。

ヒト化3D6v1の機能試験を、完全にキメラの抗体で一過性にトランスフェクトしたCOS細胞からの馴化培地、キメラH鎖+ヒト化L鎖若しくはキメラL鎖+ヒト化H鎖いずれかの混合物、および最後に完全にヒト化した抗体を使用して実施した。馴化培地をELISAアッセイにより凝集型Aβ1−42への結合について試験した。ヒト化抗体は実験誤差内の良好な活性を示し、また、キメラ3D6参照サンプルと識別不可能な結合特性を表した。ヒト化3D6v2の類似の試験は、該抗体が3D6v1のものにほぼ同一のAβ結合特性を有したことを示した。3D6v2はマウス3D6抗体と比較して事実上同一のエピトープ地図を有することもまた示された。ヒト化3D6v1抗体はPDAPPマウスから調製したクライオスタット脳切片中のAβを認知した(上述されたとおり実施した免疫組織化学)。同一の実験で、h3D6v2は、3D6v1に類似の様式で(例えば高度に装飾された斑)PDAPPおよびAD脳切片を染色した。
凝集させた合成Aβ1−42へのモノクローナル抗体6C6、10D5、2C1、12B4、3A3および12A11の結合を、Schenkら(Nature 400:173(1999))に記述されるとおりELISAにより実施した。可溶性Aβ1−42(本実施例において)はジメチルスルホキシド(DMSO)中で超音波処理した合成Aβ1−42ペプチドを指す。20μg/mlの抗体の連続希釈を50,000cpmの[125I]Aβ1−42(190μCi/μmol;IodogenTM試薬、Pierceで標識)とともに室温で一夜インキュベートした。75mg/mlプロテインAセファロース(Amersham Pharmacia)および200μgのウサギ抗マウスIgG(H+L)(Jackson ImmunoResearch)を含有する50マイクロリットルのスラリーを、希釈した抗体とともに室温で1時間インキュベートし、2回洗浄し、そしてWallacガンマカウンター(Perkin−Elmer)でカウントした。全段階は、10mMトリス、0.5M NaCl、1mg/mlゼラチンおよび0.5%Nonidet P−40、pH8.0よりなるRIA緩衝液中で実施した。親和性試験からの結果を下の表19に示す。
多様なN末端Aβ抗体のin vivo有効性を決定するために、12A11、12B4および10D5を、Bardら(2000)Nat.Med.6:916に記述されたとおり、別個のマウス群に10mg/kgで週1回腹腔内注入により6か月間投与した。試験の終了時に皮質のAβの総レベルをELISAにより測定した。抗体のそれぞれは総AβレベルをPBS対照と比較して有意に低下させた(P<0.001)。すなわち、12B4は69%の低下を示し、10D5は52%の低下を示し、そして12A11は31%の低下を示した。
以前の研究は、ex vivoで(例えばPDAPP若しくはAD脳切片で)斑を結合するおよび/若しくはex vivo食作用アッセイで斑消失を誘発する抗体の能力により、AD関連の神経病理学(例えば斑負荷量)の低減における多様なAβ抗体のin vivo有効性を予測することが可能であることを示した(Bardら(2000)Nat.Med.6:916−919)。該相関は、小膠細胞および/若しくはマクロファージによるFc依存性の食作用がin vivoでの斑消失の過程に重要であるという概念を裏付ける。しかしながら、抗体の有効性はFc相互作用に依存しない機構によりin vivoでもまた得ることができることもまた報告された(Bacskaiら(2002)J.Neurosci.22:7873−7878)。研究は、アミロイド斑を認識し得ない、Aβの中央部分に向けられた抗体が、可溶性Aβに結合しかつ斑沈着を低下させるようであることを示した(DeMattosら(2001)Proc.Natl.Acad.Sci.USA 98:8850−8855)。
本実施例では、Aβ調製物は、実質的に後に続くとおり合成Aβオリゴマーに由来した。
(1)凍結乾燥したAβ1−42ペプチドを100%ヘキサフルオロイソプロパノール(HFIP)に1mMまで溶解し(混合し、その後室温で1時間インキュベートし)、そして微小遠心管中でアリコートに分離し(各チューブは0.5mgのAβ1−42ペプチドを含有した);
(2)HFIPを蒸発により除去し、次いで凍結乾燥して残余のHFIPを除去し;
(3)生じるAβペプチド薄膜/残渣を−20℃で乾燥保存し;
(4)Aβペプチド残渣を5mMのペプチドの最終濃度までDMSOに再懸濁し、その後氷冷ハムF12(フェノールレッド不含)培地に添加して、ペプチドを100μMの最終濃度にし;
(5)ペプチドを4℃で24時間インキュベートして、およそ100μM濃度の合成Aβオリゴマーを生じ;ならびに
(6)合成Aβオリゴマーをペルオキシ亜硝酸で処理した。
野性型およびTg2576マウスに、腹腔内注入により単一用量のリン酸緩衝生理的食塩水(PBS)若しくは処置抗体を投与した。処置抗体はAβペプチドのN末端、中央およびC末端部分に対し生じさせた抗体を包含した。認知(すなわち文脈的および手掛かり依存的記憶)の迅速な改善を評価するため、各マウスにCFC訓練活動を処置直後に、およびCFC試験活動を処置24時間以内(すなわち処置後第1日)に実施した。治療的有効性は、記憶障害の逆転および記憶障害状態双方に関して表した。「記憶障害の逆転」は、mAb対PBS対照処置Tg2576動物のフリージング行動を比較することにより決定した。「記憶障害状態」は野性型対Tg2576 mAb処置動物のフリージング行動を比較することにより決定した。
11が、対照処置に関してTg2576マウスの文脈的記憶の有意の改善(**)を引き起こし(p値<0.05)、かつ、野性型マウスに関して有意の記憶障害を引き起こさなかった(##)(p値>0.1)ことを示す。さらに、抗体1C2および2B1は記憶障害の逆転への傾向(*)を表した(0.1>p値>0.05)。
N末端のマウス12A11抗体(「mu12A11」)での処置後24時間以内に観察された認知改善の持続期間を、第二の長時間CFC試験で評価した。Tg2575および野性型マウスに、再度、PBS対照若しくは低用量の12A11抗体(1mg/kg ip)を投与し、そしてそれらの認知状態を、処置後第0〜1、9〜10および16〜17日のCFCアッセイにより評価した(すなわち、CFC訓練活動を第0、9、16日に実施しかつCFC試験活動を第1、10および17日に実施した)。
N末端のマウス12A11抗体(「mu12A11」)での処置後24時間以内に観察された認知改善の持続期間を、中央領域のマウス266抗体(「mu266」)を用いる比較長時間CFC試験で評価した。Tg2575および野性型マウスに、再度、PBS対照若しくは低用量の12A11抗体(1mg/kg ip)を投与し、そしてそれらの認知状態を処置後第0〜1、4〜5、9〜10および16〜17日のCFCアッセイにより
評価した(すなわち、CFC訓練活動を第0、4、9、16日に実施しかつCFC試験活動を第1、5、10および17日に実施した)。

マウス12A11抗体中の重要な構造フレームワーク残基を同定するため、12A11
HおよびL鎖に対する相同性を有する解明されたマウス抗体の三次元モデルを研究した。12A11 L鎖に対する緊密な相同性を有する1KTRと呼称される抗体を選び、また、12A11 H鎖に対する緊密な相同性を有する1ETZおよび1JRHと呼称される2種の抗体を選んだ。これらのマウス抗体は12A11との強い配列保存を示す(Vkについて112アミノ酸で94%の同一性およびVhについてそれぞれ126アミノ酸で83%の同一性および121アミノ酸で86%の同一性)。1ETZのH鎖構造を1KTRのものに重ねた。加えて、Vkについて、選択した抗体のCDRループは、12A11 VLのCDRループが属すると同一の正準Chothia構造分類に属する。これらの抗体の結晶構造を、抗体の機能に重要であると予測される残基(例えばCDRのコンホメーションに重要なFR残基など)について、および比較して類似の12A11抗体の機能を検査した。

- 認知の迅速な改善が達成されるような、有効用量のAβ抗体を被験体に投与すること(該抗体はAβの残基1−10内の1エピトープに特異的であり、かつ、単量体Aβに比較して可溶性オリゴマーAβに優先的に結合する)を含んでなる、被験体における認知の迅速な改善を遂げる方法。
- 認知の迅速な改善が達成されるような、有効用量のAβ抗体を被験体に投与すること(該抗体はAβの残基1−10内の1エピトープに特異的であり、かつ、コンテクチュアル・フィアー・コンディショニング(CFC)アッセイで測定されるところのAβ関連障害の動物モデルにおける認知の迅速な改善を遂げる)を含んでなる、被験体における認知の迅速な改善を遂げる方法。

- Aβ抗体が、3D6抗体、6C6抗体、10D5抗体および121A11抗体よりなる群から選択される、請求項1〜3のいずれか1つに記載の方法。
- 請求項1〜3のいずれか1つに記載の方法であるが、但しAβ抗体が3D6抗体でない、上記方法。
- 認知の迅速な改善が達成されるような、有効用量のAβ抗体を被験体に投与すること(該抗体はAβの残基13−28内の1エピトープに特異的であり、かつ、単量体のAβに比較して可溶性オリゴマーAβに優先的に結合する)を含んでなる、被験体における認知の迅速な改善を遂げる方法。

- Aβ抗体が、2B1抗体、1C2抗体および15C11抗体よりなる群から選択される、請求項7〜9のいずれか1つに記載の方法。
- 請求項7〜9のいずれか1つに記載の方法であるが、但しAβ抗体が266抗体でない、上記方法。
- 動物モデルでの認知の改善が、記憶障害状態の改善若しくは記憶障害の逆転である、先行する請求項のいずれか1つに記載の方法。
- 被験体がAβ関連疾患若しくは障害を有するか若しくはその危険性がある、先行する請求項のいずれか1つに記載の方法。
- Aβ関連疾患若しくは障害が可溶性Aβと関連するか若しくはそれを特徴とする、請求項14に記載の方法。
- Aβ関連疾患若しくは障害が不溶性Aβと関連するか若しくはそれを特徴とする、請求項15に記載の方法。
- Aβ関連疾患若しくは障害がアミロイド形成疾患である、請求項15に記載の方法。
- Aβ関連疾患若しくは障害がアルツハイマー病である、請求項17に記載の方法。
- Aβ関連疾患若しくは障害がAβ関連認知障害である、請求項15に記載の方法。
- Aβ関連認知障害が軽度認知障害である、請求項19に記載の方法。
- 被験体にアミロイド沈着物が実質的にない、先行する請求項のいずれか1つに記載の方法。
- Aβ抗体が被験体での実質的斑沈着前に該被験体に投与される、先行する請求項のいずれか1つに記載の方法。
- 被験体がアルツハイマー病を伴うと診断されている、先行する請求項のいずれか1つに記載の方法。
- Aβ抗体が被験体での実質的斑沈着後に該被験体に投与される、請求項1〜21のいずれか1つに記載の方法。
- Aβ抗体が単一用量として被験体に投与される、先行する請求項のいずれか1つに記載の方法。
- Aβ抗体が複数用量で被験体に投与される、請求項1〜24のいずれか1つに記載の方法。
- Aβ抗体の用量が患者体重1kg当り約100μgから100mgまでである、先行する請求項のいずれか1つに記載の方法。
- Aβ抗体の用量が患者体重1kg当り約300μgから30mgまでである、請求項1〜26のいずれか1つに記載の方法。
- Aβ抗体の用量が患者体重1kg当り約1mgから10mgまでである、請求項1〜26のいずれか1つに記載の方法。
- 認知の迅速な改善が抗体の投与後1か月以内に達成される、先行する請求項のいずれか1つに記載の方法。
- 認知の迅速な改善が抗体の投与後1週以内に達成される、先行する請求項のいずれか1つに記載の方法。
- 認知の迅速な改善が抗体の投与後1日以内に達成される、請求項1〜30のいずれか1つに記載の方法。
- 認知の迅速な改善が、抗体の投与後12時間以内に達成される、請求項1〜30のいずれか1つに記載の方法。
- 被験体がヒトである、先行する請求項のいずれか1つに記載の方法。
- 被験体で認知を迅速に改善させるのに有効な量のAβ抗体を含んでなる組成物であって、該抗体がAβの残基1−10内の1エピトープに特異的であり、かつ、単量体Aβに比較して可溶性オリゴマーAβに優先的に結合する、上記組成物。
- 被験体で認知を迅速に改善させるのに有効な量のAβ抗体を含んでなる組成物であって、該抗体がAβの残基1−10内の1エピトープに特異的であり、かつ、コンテクチュアル・フィアー・コンディショニング(CFC)アッセイで測定されるところのAβ関連障害の動物モデルにおける認知の迅速な改善を遂げる、上記組成物。

- 請求項35〜37のいずれか1つに記載の組成物であるが、但しAβ抗体が3D6抗体でない、上記組成物。
- 認知の迅速な改善が達成されるような、有効用量のAβ抗体を含んでなる、被験体での認知の迅速な改善を遂げるための組成物であって、該抗体はAβの残基13−28内の1エピトープに特異的であり、かつ、単量体Aβに比較して可溶性オリゴマーAβに優先的に結合する、上記組成物。
