Abstract

図1と図2(A)を参照すると、本発明の第一実施例が示されている。図1と図2(A)は、従来の気管内チューブである気管内チューブ10を示す。気管内チューブ10は、管10を患者の気管内に配置するのを容易にするために、遠位端に近く配置された膨張性カフ12を有するか、あるいは、気管内チューブ10は膨張性カフの無いタイプであつてもよい。膨張性カフ12は、気管内チューブ10内の分離された膨張ルーメンを経て、膨張ガス源(図示せず)へ接続のための近位継ぎ手13へ接続されている。気管内チューブ10は、分岐管継ぎ手14へ接続された近位端を有する。継ぎ手14は、送風器回路(図示せず)への接続に適切に構成された接続口15を有する。継ぎ手14は、分離した噴霧カテーテルの近位端から気管内チューブへの挿入を行う他の接続口16も有する。継ぎ手14は、米国特許第5,078,131号明細書に記載されたルボ継ぎ手と同様の構造を有し、この全開示を本明細書に援用する。図1において、噴霧カテーテル20は、近位継ぎ手14を経て気管内チューブ10の送風ルーメン22へ挿入される位置に置かれている。図2(A)において、噴霧カテーテル20は、気管内チューブ10内に完全に位置決めされ、近位端が近位継ぎ手14の接続口16から延びる。

エーロゾル噴霧の形状は、薬剤の供給の速度と効率に影響する重要な要因であることが明らかにされた。一般に、噴霧カテーテルの遠位端の位置とその回りの空気流の状態とが与えられている場合、噴霧カテーテルの遠位端近くにおける密着を最小にする形状を有するエーロゾルを発生させることが望ましい。例えば、エーロゾル噴霧が広い場合、薬剤の一部は、気管内チューブの端部、または、気管かほかの気道の壁に密着して浪費される。他方、噴霧の幅があまり狭いか、または、その速度が速い場合、薬剤の一部は、患者の気管竜骨に過度に密着する。一般に、遅いエーロゾル粒子速度が望ましい。この理由の一つは、速い速度のエーロゾル粒子の供給によって気管竜骨に密着することを防止することである。その上、可能な限り広いエーロゾル噴霧を有し、気管内チューブまたは気道のいずれかの壁と著しく突き当たるのを防止することが望ましい。エーロゾル噴霧の速度と幾何学的形状との影響は、解剖学的要因と関係がある。ある状況において、例えば、気管竜骨から離れている場合、狭く速いエーロゾル噴霧は、遅く広い噴霧より好ましい。
上述のように、噴霧化粒子サイズのコントロールと噴霧形状とは、治療の効果に影響する重要な考慮すべき要件である。多くの適用において、可能な限り小さい前進速度と組み合わせて可能な限り小さな粒子サイズを有することが望ましい。以下で説明される態様の幾つかは、向流配置の使用を通じてこれらの目的を達成する。
上述した実施例において、噴霧の速度は逆方向のガスの向流を用いることにより低減された。図31と図32の実施例においては、噴霧粒子の速度は他の方法で低減される。図31において、噴霧カテーテル496は遠位液体オリフィス504で終端する液体のルーメン500と、一つ以上の遠位ガスオリフィス512で終端するガスルーメン508とを有する。その液体のルーメン500を通じて送られた液体は複数のガスオリフィス512から流れ出す加圧ガスにより噴霧化される。その噴霧カテーテル496はまた、追加の遠位オリフィス520で終端する一つ以上の追加のルーメン516を含む。これらのルーメン516は遠位オリフィス520に真空(負圧)を送るために使用される。その真空は、追加のルーメン516の近位端部に連結された適当な真空源(図示されない)により提供される。追加のルーメン516により送られたその真空は、ルーメン508により送られた加圧ガスが液体のルーメン500により送られた液体を噴霧化した後に、それを引き込むのを支援する。追加のルーメン516により提供される真空が無しで、遠位ガスオリフィス512により送られた加圧ガスは遠位液体オリフィス504により送られた噴霧化液体粒子にエネルギーを伝達し続け、それにより、それらを前進速度で推進させてもよい。又、その真空は加圧ガスが液体を噴霧化した後に、少なくともその一部を掃気して、液体粒子の前進速度は低減することができる。加圧ガスの掃気を容易にするために、遠位液体オリフィス504、遠位ガス・オリフィス512、そして遠位真空オリフィス520の全てが、噴霧カテーテル496の外部管状延長部528により形成された遠位キャビティ524内に開口する。その遠位延長部528は、低前進速度で噴霧化液体粒子を放出するために細口部536をその中に設置した閉口遠位端部532を有する。取り除かれた噴霧化ガスで、そのエーロゾル粒子は主にそれらの慣性によるだけで前方に運ばれる。
上述の態様は最適噴霧プルームを発達させることに向けられる。噴霧の効率に貢献する他の要因は解剖学的環境に関する噴霧カテーテルの位置であることが更に認識される。例えば、たとえ使用されている噴霧カテーテルが最適プルームを発達させても、カテーテルの投与効率は、もしそのプルームが気管内チューブ、気管壁、または他の気道内に向けられるならば非常に緩和されるかも知れない。故に、人体内の噴霧カテーテルの正しいロケーション、向き、そしてアライメントは、噴霧カテーテルを介しての薬剤の投与に貢献する重要な要因である。一般に、カテーテルがその中に位置する気道内にそのカテーテルを同軸に一直線に一致させることが望ましい。
上述のように、ガスルーメンを加圧するために使用される駆動ガスは、圧力35−50psiの純粋(例えば100%)酸素でもよい。他のガスや圧力を、所望の粒子サイズを準備するために適切に調整して使用してもよい。加圧ガスを、必要ならば、バブラー、または他の適当な手段により湿らせて、暖めてもよい。
噴霧カテーテルで薬剤を投与する時、肺の他方ではなく、一方の気管支にのみに、または他のものではなく特定の気管支にのみ投薬することが望ましい。このタイプの選択的療法の理由は、肺の一部のエリアのみが薬物治療を必要とすることからであろう。図56に示された発明の態様は一方の気管支のみに噴霧カテーテルを介しての薬物の選択的投与を容易にする。図56において、気管内チューブ904は患者の気管908内に位置決めされる。噴霧カテーテル912はその気管内チューブ内に位置決めされる。この噴霧カテーテル904は上述の態様と似かよったものでもよい。この噴霧カテーテル904は、気管内チューブ内に固定的に組み込まれているタイプのものであってもよい。第二カテーテル916は気管内チューブ904の遠位方向に延びる。その第二カテーテル916は気管内チューブ904の換気ルーメン920内に位置決めされるのがよい。その第二カテーテル916は、低い流れ加圧ガスが通されるルーメンを含む。第二カテーテル916の近位端部は気管内チューブ904の近位端部から、米国特許第5,078,131号明細書で説明される器具などの、適当な器具の中を延びる。加圧ガスの適当な供給源は第二カテーテル916の近位端部に設置される。このガス供給源は噴霧カテーテル912の加圧ガスルーメンのために使用されたのと同じガス供給源であってもよい。第二カテーテル916の遠位端部928は、噴霧投薬することが望まれる気管支以外の気管支内に位置決めされる。加圧ガスは第二カテーテル916を通じてその遠位端部内のオリフィス936から送られる。第二カテーテル916の遠位端部936からの加圧ガスの送りは、気管支932内の圧力レベルを他の気管支におけるよりも僅かに高くさせる。故に、その噴霧カテーテル912が液状薬剤のエーロゾルを発生する時、それは気管内チューブ904から第二カテーテル916と供にある気管支以外の方に吸入流と供に流れる傾向がある。このように、肺の気管支の一方、または選択された気管支は、気管内チューブ内に位置決めされた単一噴霧カテーテルを用いて選択的に投薬できる。
前述のように、噴霧薬剤を肺に投与するために薬剤は患者の吸入により運ばれることが望ましい。多数の要因が、この方法で投与される薬の効率に影響を及ぼす。次の態様はこれらの要因の幾つかを考慮に入れた投薬効率の向上に向けられる。
A.気管内チューブ内に組み込まれた噴霧カテーテル
上述された噴霧カテーテルの種々の態様は、独立した気管内チューブ提携して使用するように適応されるか、または気管内チューブなしで使用されるように適応されるかの何れかで説明されてきた。もし気管内チューブと供に使用されるならば、上記噴霧カテーテルの態様は、もし提案されるならば、気管内チューブから脱着可能であるのが望ましい。ここで開示された本発明の態様の多くは気管内チューブから脱着できない、即ちその噴霧カテーテルはその中に組み込まれて、気管内チューブの一部を形成する噴霧カテーテルと連係して使用してもよい。噴霧化投薬用の気管内チューブはDr. R. MacIntyreにより1992年3月10日に「Endortracheal Tube Adapted for Aerosol Generation at Distal End
Thereof(その遠位端部でのエーロゾル発生のために適応された気管内チューブ)」の
名称で提出された特許出願で説明される。Dr. MacIntyreにより開発されたシステムによれば、その遠位端部において薬剤の噴霧化を準備する気管内チューブが提供される。Dr. MacIntyreのシステムによれば、気管内チューブは患者の呼吸気流のために使用される主換気ルーメンに加えて、二つの追加の、独立したルーメンを含む。液状の薬剤は追加的ルーメンの一方を経て運ばれる、そして加圧ガスが他方のルーメンを経て運ばれる。その二つの追加的ルーメンは気管内チューブ気流ルーメンの遠位端部の近くに遠位開口部を有する。加圧ガスルーメンの遠位開口部は、加圧ガスを遠位薬剤ルーメン開口部の向こう側に方向付ける、それにより液状薬剤を噴霧化できるので、患者の肺に投与できる。本発明は、気管内チューブに関して非脱着可能である噴霧カテーテルの態様をカバーすることは意図されている。
図60はエーロゾルを生成するための他のカテーテル1060を示す。そのカテーテル1060はカテーテルのルーメンに位置する多孔性材料、またはスポンジの使用によりエーロゾル、またはエーロゾル状プルームを発生する。そのカテーテル1060は、液状薬剤が加圧下でその中を運ばれるルーメン1068と、ガスが加圧下でその中を運ばれるルーメン1072とを備えた主軸1064を有する。多孔性材料1076は軸1064の遠位端部に位置するので、両方のルーメン1068と1072とはそれらの内容物をその多孔性材料1076内に運び入れる。その多孔性材料1076はポレックス(Porex)製の多孔性のポリエチレンであってもよい。又は、その多孔性材料は、ポリマー・スポンジか、または他のポリマー材料であってもよい。その多孔性材料1076の遠位の主軸1064内に位置するのは、オリフィス1084をその中に備えたエンドキャップ1080である。そのオリフィスは小さく、且つカテーテル軸と多孔性材料エリアで正の背圧を維持する。そのエンドキャップ1080は小さなギャップ1082により多孔性材料1076の遠位側から分離される。圧力下で多孔性材料1076に送られた液体とガスとは多孔性材料を経て、ギャップ1082を横切って開口部1084に向って移動する。液体とガスは圧力下で混合状態となり、そしてそれらが細い先端部から吐出されると、そのガスは液体粒子を膨張させて、小さな滴で撒き散らす。開口部1084を通じての発散時には、その薬剤は小さな滴、例えばエーロゾルを形成する。そのエーロゾルは、上記態様で説明されたものと似た方法で患者の肺に運ばれる。遠位液体オリフィスに多孔性材料、またはスポンジを使用する利点は、それが液だれを低減することにある。
ある状況においては、噴霧カテーテルにより発生された一次エーロゾル・スプレーを修正することが望まれるかも知れない。これが達成できる一つの方法は、一次エーロゾルスプレーをその通路内に置かれたバフルに衝突させることによるものである、そのスプレーの速度と方向とが変更されて、そしてそのエーロゾルの分布のサイズが修正されて、二次エーロゾルを作る。正しく位置したバフル上への衝突は、大きなエーロゾル粒子をばらばらにして、より細かいエーロゾルの霧を作る。そのバフルはまた、粒子を運ぶ気流を偏向、または拡散して、それらの前進速度を低減し、且つそれらの方向を変える。これは気管竜骨、または気道上への衝撃を低減でき、そして吸入気流内への粒子の搬入性を高めることが出来る二次エーロゾルを提供するために衝突バフルを組み入れる噴霧カテーテルの態様が図61乃至図64で示される。
上述の態様において、薬剤は液状で遠位液体オリフィスに送られる。図65で例示された、他の態様においては、薬剤は推進薬と混合され、圧力下で保持され、そして噴霧カテーテル1198の遠位先端部に加圧下で送ることもできる。加圧された薬剤/液体推進混合は、計量された、または計量されていない服用量の吸入器の成分として使用されるこれらのように、加圧された容器1200から供給できる。推進薬即ち噴射剤を使用することにより、エーロゾルは、加圧された噴霧ガスを加えなくてもカテーテルの遠位端部1202から発生できる。しかしながら、噴霧カテーテルの遠位端部からの加圧ガス1204の送りは、大きな薬剤粒子を分解するのを助け、そしてエーロゾルプルームを形作るのを助けるだけでなく、カテーテルを通じて送られた噴霧化された薬剤溶液を散乱させるのを助けるために使用される。例えば、加圧された噴霧ガスの送りは、薬剤液体推進混合により発生されたエーロゾルを保護するのを助け、そして衝突に因る損失を回避するのを助けることもできる。
上述のように、噴霧カテーテルは、脱着可能、または固定的に、気管内チューブなどの、他の装置内に組み込むことが出来る。噴霧カテーテルがそれに適応できる他のそのような装置は吸入、または吸引カテーテルである。吸入カテーテルは、器官内チューブされている患者との関連で時々使用される。吸入カテーテルは、気管内チューブの換気ルーメンを通じて挿入出来るような、外径と長さを有する。その吸入カテーテルは、器官内チューブされる患者の呼吸管内、または気管内チューブ内で収集する流体やムチン分泌物を吸引により取り除くために使用される。従来型の吸入カテーテルは、気管内チューブの換気ルーメンを下降して、そしてその遠位端部から挿入される。粘液溶解剤が、気管、または気管支からムチンを回収するのを助けるために吸入カテーテルのルーメンを経て液体として点滴注入してもよい。その吸入カテーテルは気管内チューブから取り出され、そして気管内チューブの近位端部に接続された無菌容器内に置かれる、または保持されるので、再度気管内チューブ内に再挿入できる。
噴霧カテーテルの遠位先端部に位置した振動オリフィス、複数オリフィス、または目打を有するスクリーン、または振動ワイヤーが、微細エーロゾル粒子の発生を助けるためにも採用できよう。その震動は、電機的、液圧的、空圧的、または圧電的手段により発生させてもよい。その振動は軸内、または体外で、カテーテルの先端部において発生されるかも知れない。
上記態様は、噴霧化されるべき液体がそれから出てくるオリフィス近くのカテーテルを通じて、加圧ガスを方向付けることによりエーロゾルが発生される噴霧カテーテルを説明する。呼吸管に薬剤を投与するためのエーロゾルを発生するために他の手段、または作用を用いることはここで説明された発明の範囲内であると見なされる。例えば、上記実施例を、液状薬剤のエーロゾルを発生するために他の手段を利用する装置と共に使用してもよい。単一液体ルーメンにより送られる液体は、超音波エネルギー、電子スプレー、蒸気、またはインクジェットタイプのプリンターで使用されるこれらのものと同様のマイクロポンプをその液体に適用することにより噴霧化することもできよう。噴霧化に対するこれらの他のアプローチを、液状薬剤のエーロゾルを生成するために上述の態様の幾つかのための加圧ガスの使用の代りとしてもよく、または加圧ガスと組合わせても、または互いに組み合わせてもよい。

Claims (7)
- 薬剤が供給される近位端と、患者の体内に挿入されて薬剤を噴霧する遠位端とを備えるカテーテル装置(416、448)であって、
該カテーテル装置内を該近位端から遠位端に延び、該遠位端で薬剤を第1の方向に噴霧するための薬剤オリフィス(419、453,454)を有する薬剤ルーメンを備える薬剤噴霧手段と、
該カテーテル装置内を該近位端から該遠位端に延び、該薬剤噴霧手段から噴霧される薬剤に向けて、該第1の方向と反対方向にガスを放出する向流ガスオリフィス(440、460)を有するガスルーメンを有する向流ガス放出手段と、
を有するカテーテル装置。 - 該向流ガス放出手段の該ガスルーメン(428)が、該遠位端においてJ字形に曲がっており、該遠位端の先端の該向流ガスオリフィス(440)が該近位端に向けて開口するようにし、
該薬剤オリフィス(419)が、該向流ガスオリフィスよりも該近位端側で該向流ガスオリフィスに向けて開口している
ことを特徴とする請求項1に記載のカテーテル装置。 - 該薬剤噴霧手段が、少なくとも該遠位端において、該近位端から遠位端方向での当該カテーテル装置の長手方向に対して平行に延びる第1のガスチューブ(417)及び薬剤チューブ(418)を備え、該薬剤ルーメンは該薬剤チューブにより形成され、該薬剤オリフィスが該薬剤チューブの先端に形成され、該薬剤オリフィス(418)から放出される薬剤が該第1のガスチューブ(417)から放出されるガスによって噴霧化され、
該向流ガスオリフィスが、該噴霧手段を通って延びる第2のガスチューブ(428)を備え、該第2のガスチューブ(428)の遠位端(423)が、該第1のガスチューブ及び該薬剤チューブを超えて延び、J字形とされてその先端が該第1のガスチューブ及び該薬剤チューブ側に向けられ、該ルーメンが該第2のガスチューブにより形成され、該向流ガスオリフィスが該第2のガスチューブの該先端に形成されている請求項2に記載のカテーテル装置。 - 該薬剤チューブ及び該第2のガスチューブが、該第1のガスチューブ内を延びている請求項3に記載のカテーテル装置。
- 該薬剤チューブと第1のガスチューブが同軸状にされている請求項4に記載のカテーテル装置。
- 該薬剤噴霧手段の該薬剤ルーメン(453,454)が、該遠位端においてJ字形に曲がっており、該遠位端の先端の該薬剤オリフィスが該近位端側に向けて開口し、
該向流ガスオリフィス(460)が、該薬剤オリフィスよりも該近位端側で該薬剤オリフィス側に向けて開口している
ことを特徴とする請求項1に記載のカテーテル装置。 - 該薬剤噴霧手段が、該カテーテル装置の近位端から遠位端に延びてその遠位端(452)がJ字形にされているカテーテル軸であって、該カテーテル軸を長手方向に延びてJ字形にされた前記遠位端の先端端面に開口し前記近位端に向けられた薬剤オリフィス(453,454)を有する薬剤ルーメンと、該カテーテル軸を長手方向に延びてJ字形にされた前記遠位端の先端端面に開口し前記近位端に向けられたガスオリフィス(453,454)を有するガスルーメンとを有し、前記薬剤オリフィスからの薬剤を該ガスオリフィスからのガスによって噴霧化するようにしたカテーテル軸を有し、
前記向流ガス放出手段が、該カテーテル装置の近位端から遠位端に延び、該ガスルーメンを形成しているガスチューブを有し、
カテーテル軸が該ガスチューブを通り、該カテーテル軸の前記遠位端が該ガスチューブの遠位端を超えて延びるようにされている請求項6に記載のカテーテル装置。
