fabricación de un medicamento para reducir el dolor asociado a un
trastorno muscular.

La presente invención se refiere a toxinas
botulínicas de tipo B. De manera importante, la presente invención
se refiere a mitigar el dolor relacionado con la actividad muscular
o contractura y por tanto, es ventajosa en el tratamiento de, por
ejemplo, espasmos musculares tales como enfermedad de la mandíbula,
dolor lumbar bajo, dolor miofascial, dolor relacionado con la
espasticidad y distonía, así como lesiones deportivas y dolor
relacionado con contracturas en artritis.
Hasta ahora, las toxinas botulínicas, en
particular la toxina botulínica tipo A, han sido utilizadas en el
tratamiento de varios trastornos neuromusculares y estados que
implican espasmos musculares; por ejemplo, estrabismo,
blefaroespasmos, tortícolis espasmódica (distonía cervical),
distonía oromandibular y disfonía espasmódica (distonía laríngea).
La toxina se une rápida y fuertemente a las terminales presinápticas
colinérgicas del nervio e inhibe la exocitosis de acetilcolina
mediante la disminución de la frecuencia de liberación de
acetilcolina. Esto da como resultado una parálisis local y por lo

botulínica tipo A.
La expresión toxina botulínica es una expresión
genérica que abarca las familias de las toxinas producida por la
bacteria anaeróbica Clostridium botulinum y, hasta la fecha,
7 neurotoxinas inmunológicamente distintas han sido identificadas.
Estas han recibido las denominaciones A, B, C, D, E, F y G. Para más
información referente a las propiedades de las distintas toxinas

Hatheway en el capítulo 1 del libro titulado Botulinum Neurotoxin
and Tetenus Toxin, L.L. Simpson, Ed., publicado por Academic
Press Inc. de San Diego, California 1989. cuyas descripciones se
incorporan aquí como referencia.
El componente neurotóxico de la toxina
botulínica tiene un peso molecular de aproximadamente 150
kiloDaltons y se piensa que comprende una corta cadena
polipeptídica de alrededor de 50 kD que se considera responsable de
las propiedades tóxicas de la toxina, por ejemplo, al interferir en
la exocitosis de la acetilcolina, disminuyendo la frecuencia de
liberación de acetilcolina, y en una cadena polipeptídica más larga
de cerca de 100 kD que se cree es necesaria para permitir la unión
de la toxina a la membrana presináptica.
Las cadenas "corta" y "larga" están
unidas entre sí por medio de puentes simples de disulfuro. (Cabe
destacar que ciertos serotipos de toxina botulínica, por ejemplo,
del tipo E, pueden existir en forma de proteína de una sola cadena
y sin cortes, en lugar de una doble cadena. La forma de una sola
cadena es menos activa, pero puede convertirse en la
correspondiente cadena doble mediante cortes con una proteasa, por
ejemplo, tripsina. Ambas, la cadena sencilla y la doble, son útiles
en el método de la presente invención).
En general, se conocen cuatro grupos
fisiológicos de C. Botulinum (I, II, III, IV). Los organismos
capaces de producir una toxina serológicamente distinta pueden
provenir de más de un grupo fisiológico. Por ejemplo, las toxinas
de tipo B y F pueden ser producidas por cepas del Grupo I o II.
Además, han sido identificadas otras cadenas de especies de
clostridia (C. Baratii, tipo F; C. Butyricum, tipo E;
C. Novyi, tipo C_{1} o D) que pueden producir neurotoxinas
botulínicas.
Los conjugados de inmunotoxina con ricina y
anticuerpos, que se caracterizan por tener aumentada su
citotoxicidad mediante la mejora de la afinidad a la superficie
celular, están descritos en la memoria de Patente Europea 0129434.
Los inventores advierten que la toxina botulínica puede ser
utilizada en lugar de la ricina.
La toxina botulínica se obtiene comercialmente
mediante la creación y crecimiento de cultivos de C.
botulinum en un fermentador y recogiendo y purificando más
tarde la mezcla fermentada de acuerdo con técnicas conocidas.
La toxina botulínica tipo A, el tipo de toxina
generalmente utilizado para tratar estados neuromusculares, está en
la actualidad comercialmente disponible en distintas compañías; por
ejemplo, en Porton Products Ltd. Reino Unido, bajo el nombre
comercial "DYSPORT", y en Allergan, Inc., Irvine, California,
bajo el nombre comercial BOTOX®.

Un objetivo de la invención es proporcionar
nuevos tratamientos de trastornos y afecciones neuromusculares con
diversos tipos de toxina botulínica. Otro objetivo de la presente
invención es mitigar el dolor con diversos tipos de toxina
botulínica.
La presente invención se refiere al uso de
toxina botulínica de tipo B en un medicamento para mitigar el dolor
asociado a contracciones musculares y a trastornos musculares leves,
incluyendo, pero sin limitarse a ellos, espasmos en el esfínter de
la arteriola cardiovascular, sistema gatrointestinal, urinario,
vesícula biliar y recto.
Cada serotipo de toxina botulínica ha sido
identificado como proteínas inmunológicamente diferentes a través
del uso de anticuerpos específicos. Por ejemplo, si el anticuerpo
(antitoxina) reconoce, esto es, neutraliza la actividad biológica
de, por ejemplo, el tipo A, no reconocerá los tipos B, C, D, E, F o
G.
Aunque todas las toxinas botulínicas parecen ser
zinc-endopeptidasas, el mecanismo de acción de los
distintos serotipos, por ejemplo, A y E dentro de la neurona,
parecen ser diferentes al del tipo B. Además, el "receptor" de
la superficie neuronal para la toxina, parece ser diferente en cada
serotipo.
En el área de uso de la toxina botulínica de
acuerdo con la presente invención, con respecto a los sistemas
orgánicos que implican la liberación de neurotransmisor, se espera
introducir la toxina tipo B directamente mediante inyección
local.
La toxina botulínica utilizada de acuerdo con la
presente invención es la toxina botulínica tipo B.
Los grupos fisiológicos de los tipos de
Clostridium botulinum aparecen en la lista de la Tabla I.

Estos tipos de toxinas pueden ser producidos por
selección del grupo fisiológico apropiado de organismos
Clostridium botulinum. Los organismos designados como Grupo
I, se denominan normalmente proteolíticos y producen toxinas
botulínicas de los tipos A, B, y F. Los organismos designados como
Grupo II son sacarolíticos y producen toxinas botulínicas de los
tipos B, E y F. Los organismos designados como Grupo III producen
sólo toxinas botulínicas de los tipos C y D, y se distinguen de los
organismos de los Grupos I y II por la producción de cantidades
significativas de ácido propiónico. Los organismos del Grupo IV sólo
producen neurotoxina tipo G. La producción de una cualquiera y de
todos los tipos de toxina botulínica A, B, C, D, E, F y G está
descrita en el Capítulo I de "Botulinum Neurotoxin and Tetanus
Toxin", citado anteriormente, y/o las referencias allí
citadas. Los tipos de toxina botulínica B, C, D, E, F y G también
se pueden obtener de varias especies de clostridia.
Actualmente, catorce especies de clostridia son
consideradas patógenas. La mayoría de las cepas patógenas producen
toxinas que son responsables de varios signos y síntomas
patológicos. Han sido aislados organismos que producen toxinas
botulínicas a partir de epidemias de botulismo en humanos (tipos A,
B, E y F) y en animales (tipos C y D). Sus identidades fueron
descritas a través del uso de antitoxinas (anticuerpos) específicas,
desarrolladas contra unas primeras toxinas. La toxina de tipo G se
encontró en el suelo y es de toxigenicidad baja. Sin embargo, se ha
aislado de especimenes de autosia, aunque hasta ahora no ha habido
evidencia adecuadad de que el botulismo de tipo G haya ocurrido en
seres humanos.
Preferentemente, la toxina se administra por
medio de inyección intramuscular, directamente en una zona local
tal como el músculo espástico, preferentemente en la región de unión
neuromuscular, aunque puedan ser empleadas cuando se considere
apropiado, formas alternativas de administración (por ejemplo,
inyección subcutánea), que pueden transportar la toxina
directamente a la región afectada. La toxina puede ser presentada
como una solución o dispersión acuosa estéril apirética, y como
polvo estéril para reconstitución en una solución o dispersión
estéril.
Si se desea, pueden ser añadidos agentes
ajustadores de la tonicidad tal como cloruro sódico, glicerol y
distintos azúcares. Pueden también ser incluidos estabilizadores
tales como albúmina sérica humana. La formulación se puede
conservar por medio de un conservante adecuado, farmacéuticamente
aceptable, tal como un parabeno, aunque es preferible que no tenga
conservantes.
Es preferible que la toxina se formule en una
única dosis; por ejemplo, se puede proporcionar en forma de una
solución estéril en un vial o como un vial o sobres que contengan
polvo liofilizado para reconstituir con un vehículo adecuado tal
como solución salina para inyección.
En una realización, la toxina botulínica se
formula en una solución que contiene albúmina sérica humana
pasteurizada y solución salina, que estabiliza la toxina y minimiza
las pérdidas mediante adsorciones no específicas. La solución se
filtra en condiciones estériles (filtro de 0,2 micrómetros), se
carga en viales individuales y se seca al vacío, para dar un polvo
liofilizado estéril. Para su uso, el polvo puede ser reconstituido
mediante la adición de suero salino normal estéril sin conservantes
(cloruro sódico al 0,9% para inyección).
La dosis de toxina administrada al paciente
dependerá de la gravedad de los estados; por ejemplo del número de
grupos de músculos que requieren tratamiento, de la edad y talla del
paciente y de la potencia de la toxina. La potencia de la toxina se
expresa como múltiplo del valor DL_{50} para el ratón,
definiéndose una unidad (U) de la toxina como la cantidad de toxina
equivalente que provoca la muerte al 50% de un grupo de 18 a 20
ratones Swiss-Webster hembras, que pesan
aproximadamente 20 gramos cada uno.
Las dosis utilizadas en aplicaciones
terapéuticas humanas son aproximadamente proporcionales a la masa
del músculo que se inyecta. Típicamente, la dosis administrada al
paciente puede ser desde alrededor de 0,01 hasta alrededor de 1.000
unidades; por ejemplo, hasta alrededor de 500 unidades, y
preferiblemente en un intervalo desde alrededor de 80 hasta
alrededor de 460 unidades por paciente y por tratamiento, aunque
dosis menores y mayores pueden ser administradas en circunstancias
apropiadas, tales como hasta 50 unidades para el alivio del dolor y
el control de las secreciones colinérgicas.
Conforme los médicos se familiaricen con el uso
de este producto, la dosis puede cambiarse. La potencia de la
toxina botulínica y su largo tiempo de acción, hacen que las dosis
tiendan a ser administradas de forma poco frecuente. Finalmente,
sin embargo, tanto la cantidad de toxina administrada como la
frecuencia en su administración será responsabilidad del médico
encargado del tratamiento y estará de acuerdo con las cuestiones de
seguridad y los efectos producidos por la toxina.
El invento será ilustrado ahora con referencia a
los siguientes ejemplos no limitantes.
En cada uno de los ejemplos, las zonas
apropiadas de cada paciente son inyectadas con una solución estéril
que contiene la confirmación de la toxina botulínica. Las dosis
totales del paciente varían desde alrededor de 0,01 unidades hasta
460 unidades. Antes de inyectar en un grupo de músculos, se
considera cuidadosamente la anatomía del grupo de músculos, con el
propósito de inyectar, si se conoce, en la zona con la mayor
concentración de uniones neuromusculares. Antes de inyectar en el
músculo, la posición de la aguja en el músculo se confirma poniendo
el músculo en toda su gama de movimientos, y observando el
movimiento resultante del extremo de la aguja. La anestesia
general, la anestesia local y la sedación se usan de acuerdo con la
edad del paciente, el número de puntos que serán inyectados, y las
necesidades particulares de cada paciente. Puede ser necesaria más
de una inyección y/o puntos de inyección para alcanzar el resultado
deseado. Además, algunas inyecciones, dependiendo del músculo
inyectado, pueden requerir el uso de agujas finas, huecas y
revestidas con teflón, guiadas por electromiografía.
Tras la inyección, cabe destacar que no hay
efectos secundarios sistémicos ni locales y en ninguno de los
pacientes se ha visto que desarrollase hipotonicidad local
extensiva. La mayoría de los pacientes muestra una mejora en la
función tanto subjetivamente y, cuando se mide, también
objetivamente.
Se trata un varón, de 30-40 años
de edad, con una válvula pilórica estrechada que impide que se vacíe
el estómago, administrando 1-50 unidades de toxina
botulínica. La administración es a la válvula pilórica (que controla
la eliminación de los contenidos del estómago al intestino)
dividida en inyecciones en 2 a 4 cuadrantes hechas con cualquier
dispositivo endoscópico o durante la cirugía. En aproximadamente
1-7 días, tiene lugar el vaciado normal del
estómago, eliminación o reducción drástica de la regurgitación.
Se trata una hembra, de 35 años de edad, por
administración de 0,1 a 50 unidades totales de toxina botulínica.
La administración es a los músculos, controlando el cierre de la
mandíbula. Los músculos hiperactivos se pueden identificar con guía
EMG (electromiografía). El alivio del dolor asociado con los
espasmos musculares, posible reducción en el rechinamiento de
dientes tiene lugar en aproximadamente 1-3 días.
Se trata una hembra, de 35 años de edad, con
colitis espástica con 1-100 unidades de toxina
botulínica dividida en varias áreas, se proporciona enema
(1-5 unidades) en el volumen de enema estándar,
dosis de valoración, partiendo de la dosis más baja. La inyección
es en el recto o colon sigmoide o se puede emplear un enema de baja
dosis. Los calambres y el dolor asociado al colon espástico se
mitigan en 1-10 días.
Se inyecta a un varón, de 70 años de edad, tras
un ictus o accidente cerebro vascular, 50 a 300 unidades de toxina
botulínica en los músculos mayores implicados en el cierre de las
manos y giro de muñecas y antebrazos, o los músculos implicados en
el cierre de las piernas tal que el paciente y su asistente tienen
dificultades con la higiene. Estos síntomas se mitigan en 7 a 21
días.
Claims (26)
1. Uso de la toxina botulínica tipo B para la
fabricación de un medicamento para reducir el dolor asociado a una
contracción o actividad muscular.
2. Uso según la reivindicación 1, en el que la
contracción o actividad muscular es un espasmo muscular o una
afección de espasticidad.
3. Uso según la reivindicación 2, en el que la
afección de espasticidad es secundaria a un ictus o un accidente
cerebro vascular.
4. Uso según la reivindicación 2, en el que la
afección de espasticidad es secundaria a un traumatismo
craneoencefálico.
5. Uso según la reivindicación 2, en el que la
afección de espasticidad es secundaria a una lesión en la espina
dorsal.
6. Uso según la reivindicación 2, en el que el
espasmo muscular se asocia con un trastorno de la mandíbula.
7. Uso según la reivindicación 1,en el que el
dolor se asocia con un trastorno muscular leve.
8. Uso según la reivindicación 7, en el que el
trastorno muscular leve es una válvula pilórica estrechada.
9. Uso según la reivindicación 7, en el que el
trastorno muscular leve es colitis espástica.
10. Uso según la reivindicación 7, en el que el
trastorno muscular leve es un músculo espasmódico.
11. Uso según la reivindicación 10, en el que el
músculo espasmódico está en el sistema gastrointestinal.
12. Uso según la reivindicación 10, en el que el
músculo espasmódico está en el colon.
13. Uso según la reivindicación 10, en el que el
músculo espasmódico es un esfínter de una ateriola
cardiovascular.
14. Uso según la reivindicación 10, en el que el
músculo espasmódico es un esfínter en el sistema
gastrointestinal.
15. Uso según la reivindicación 10, en el que el
músculo espasmódico es un esfínter en el sistema urinario.
16. Uso según la reivindicación 10, en el que el
músculo espasmódico es un esfínter de la vesícula biliar.
17. Uso según la reivindicación 10, en el que el
músculo espasmódico es un esfínter rectal.
18. Uso según la reivindicación 2, en el que el
dolor está asociado con un espasmo muscular en una afección
secundaria a una lesión deportiva.
19. Uso según la reivindicación 18, en el que el
espasmo muscular tiene lugar en el muslo del paciente y la toxina
botulínica se formula para su administración en el muslo.
20. Uso según la reivindicación 1, en el que el
dolor se relaciona con contracturas en artritis.
21. Uso según la reivindicación 1, en el que el
dolor es dolor lumbar bajo.
22. Uso según la reivindicación 1, en el que el
dolor es dolor miofascial.
23. Uso según la reivindicación 1, en el que el
dolor está relacionado con la espasticidad.
24. Uso según la reivindicación 1, en el que la
toxina botulínica se formula para administración en una cantidad de
al menos aproximadamente 1000 unidades.
25. Uso según la reivindicación 1, en el que la
toxina botulínica se formula para inyección intramuscular.
26. Uso según la reivindicación 1, en el que la
contracción o actividad muscular es una contracción o actividad
muscular influenciada por factores colinérgicos.
