Description
- HINTERGRUND DER ERFINDUNG
- 1. Gebiet der Erfindung
- Die Erfindung betrifft die Verwendung von Konjugaten aus Antikörper-cytotoxischem Mittel zur Ablation gesunder Zellen oder Gewebe.
- 2. Beschreibung des Stands der Technik
- Gesunde Gewebe und Organe wurden bisher durch magnetische Resonanzabbildungstechniken abgebildet, allerdings nicht unter Verwendung von bildverstärkenden Kontrastmitteln und nicht mit Antikörper-konjugierten Abbildungsmitteln.
- Verfahren zur Abbildung von Tumoren und infektiösen Läsionen unter Verwendung von markierten Antikörpern und Antikörperfragmenten, die Marker, die von Tumoren oder infektiösen Läsionen produziert werden oder damit im Zusammenhang stehen, spezifisch binden, wurden bereits offenbart, inter alia bei Hansen et al., U.S.-Pat. Nr. 3,927,193 und Goldenberg, U.S.-Pat. Nrn. 4,331,647; 4,348,376; 4,361,544; 4,468,457; 4,444,744; 4,460,459 und 4,460,561 und in den verwandten anhängigen Anmeldungen mit den U.S.-Serien-Nrn. 609,607 und 633,999, siehe auch DeLand et al., J. Nucl. Med., 20, 1243-50(1979).
- Die Verfahren verwenden radioaktiv markierte Antikörper, die spezifisch an Marker binden, die von Tumoren oder infektiösen Läsionen produziert werden oder damit im Zusammenhang stehen, und ein "positives" Bild ergeben, d. h. Aufnahme der an den Antikörper gebundenen Radioaktivität in die an dem Tumor oder der infektiösen Läsion beteiligten Struktur, und die das entsprechenden Antikörper-Ziel aufweisen und somit eine Sichtbarmachung der betroffenen Struktur erlauben. Weitere Verbesserungen in der Spezifität und Auflösung dieser Verfahren werden durch die Verwendung verschiedener Subtraktionstechniken erzielt, die ebenfalls in den zuvor genannten Druckschriften offenbart sind, und durch die sich der Hintergrund, die nicht spezifische Radioaktivität, von der spezifischen Aufnahme durch den Tumor oder die Läsion unterscheiden lässt.
- Weiterhin besteht Bedarf an therapeutischen Verfahren, die empfindlicher und spezifischer sind, und an organtherapeutischen Reagentien und Verfahren mit hoher Spezifität zur Differenzierung von bestimmten Organen und Geweben von den umliegenden Strukturen.
- AUFGABEN DER ERFINDUNG
- Eine Aufgabe der Erfindung besteht in der Bereitstellung von Mitteln zur Ablation gesunder Zellen und Gewebe als Teil eines therapeutischen Eingriffs.
- Beim weiteren Studium der Spezifikation und der dazugehörigen Ansprüche werden der Fachwelt weitere Aufgaben und Vorteile der Erfindung klar.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Die vorliegende Erfindung stellt die Verwendung eines Konjugats aus einem Antikörper oder Antikörperfragment und einem cytotoxischen Mittel bei der Herstellung eines Medikaments zur parenteralen Anwendung bei der Ablation von gesunden Zellen oder Geweben bereit:
bei der Knochenmarksablation bei einem Patienten vor der Reimplantation gesunder Knochenmarkszellen;
bei der Milz-Ablation zur Behandlung von thrombocytopenischer Purpura; oder
bei der Behandlung von Endometriose
wobei:
der Antikörper oder das Antikörperfragment auf einen Marker spezifisch ist, der mit der zu ablatierenden Zelle im Zusammenhang steht oder von ihr produziert wird; und
wobei das cytotoxische Mittel, wie in Anspruch 1, definiert ist. - Bei einer Ausführungsform stellt die Erfindung die Verwendung des Konjugats aus Antikörper-cytotoxischem Mittel zur Herstellung eines Medikaments zur Zerstörung von Knochenmarkszellen in einem Patienten vor der Reimplantation von gesunden Knochenmarkszellen bereit. Der Antikörper oder das Antikörperfragment ist auf einen Marker spezifisch, der mit den Knochenmarkszellen im Zusammenhang steht oder von ihnen produziert wird, und ist mit einem cytotoxischen Mittel konjugiert.
- Bei einer weiteren Ausführungsform stellt die Erfindung die Verwendung des Konjugats aus Antikörper-cytotoxischem Mittel zur Herstellung eines Medikaments, wobei der Marker mit Milzzellen im Zusammenhang steht oder von ihnen produziert wird, zur Verwendung bei der Milz-Ablation bei der Behandlung von thrombocytopenischer Purpura bereit.
- Bei einer weiteren Ausführungsform stellt die Erfindung die Verwendung des Konjugats aus Antikörper-cytotoxischem Mittel zur Herstellung eines Medikaments, wobei der Marker ein Hormon- oder ein Wachstumsfaktor-Rezeptor ist, zur Ablation von Endometriumzellen bei der Behandlung von Endometriose bereit.
- Bei der Endometriose, die ektopiertes Endometiumgewebe umfasst, schließt die derzeitige Arzneimittel-Standardtherapie die Verabreichung eines synthetischen Steroids ein, das aus Ethisteron (Markenname DANOCRINE von Danazol) stammt, das chemisch betrachtet ein 17-α-Pregna-Z,3-dien-20-yno[2,3,3-d]-isoxazol-17-ol ist. Dieses wirkt wahrscheinlich, mindestens teilweise, auf den Sexualsteroidmetabolismus und mit Sexualhormon-Rezeptoren, insbesondere dem Follikel-stimulierenden Hormon (FSH) und dem luteinisierenden Hormon (LH) am Zielorgan. Es ist nun möglich, einen Antikörper gegen diese Gonaden-Steroidrezeptoren allein oder als Immunkonjugat mit Isotopen, Arzneimitteln, Toxinen, Hormonantagonisten, Cytokinen, Autocrinen, etc. zu verwenden, um das ektopierte Endometrium atrophisch zu machen.
- Der Fachwelt sind mehrere Verfahren zur Produktion von mit Organ- oder Gewebe zusammenhängenden oder -spezifischen Antikörpern bekannt, wenn die existierenden Antikörper als ungeeignet betrachtet werden oder wenn unterschiedliche oder stärker diskriminierende Spezifitäten erwünscht sind. In der Regel werden ganze Zellen, Gewebeproben und/oder Zell- oder Gewebefraktionen, Membranen, Antigenextrakte oder gereinigte Antigene zur Provokation des Immunsystems eines geeigneten Tiers, z. B. Maus, Kaninchen, Hamster, Ziege oder dergleichen, verwendet, wobei das Antigen durch Aggregation, falls notwendig, und/oder durch Co-Verabreichung mit einem geeigneten herkömmlichen Hilfsstoff immunogen gemacht wird. Hyperimmun-Antiserum kann isoliert werden, und polyklonale Antikörper können durch herkömmliche Verfahren hergestellt werden. Alternativ können Milzzellen mit unsterblichen Myelomzellen unter Bildung von Hybridomzellen, die monoklonale Antikörper durch eines der nun bereits gebräuchlichen Verfahren produzieren, verschmolzen werden. Siehe z. B. die Verfahren in der oben zitierten U.S.-Patentanmeldung mit der Serien-Nr. 609,607 zur Erläuterung der Techniken. Hybridome unter Verwendung von Tieren, z. B. der Maus, oder menschlichen Myelomzelllinien und tierischen oder menschlichen Milz- oder Lymphzellen sind alle aus der Technik bekannt und können für die vorliegende Erfindung hergestellt und verwendet werden. Siehe beispielsweise Glassy et al., „Human Monoklonal Antibodies to Human Cancers" in „Monoklonal Antibodies and Cancer", Boss et al., Hrsg., 163–170 (Academic Press, 1983). Die spezifischen Antiseren oder monoklonalen Antikörper werden auf die Spezifität mit Verfahren gescreent, die zum Screening der Antilymphozytenklone in den hier vorstehend zitierten Druckschriften verwendet werden, wobei die Verfahren nun ebenfalls bei dieser Technik üblich sind.
- Mit einem Organ im Zusammenhang stehende und Organ-spezifische Antikörper können durch Immunisieren eines geeigneten tierischen Wirts mit bestimmten Säugertumoren oder gesunden Organ/Gewebeextrakten und/oder Zellen sowie mit gereinigten Hormon- oder Wachstumsfaktor-Rezeptoren entwickelt werden. Es ist bekannt, dass die Verwendung von Tumoren als Immunogene Antikörper ergeben kann, die nicht nur mit Neoplasie- sondern auch mit gesunden Gewebekomponenten reagieren, die manchmal eine Organ-begrenzte Natur zeigen. Histogenetische und funktionelle Unterschiede zwischen verschiedenen Geweben und Organen des Körpers legen natürlich nahe, dass distinkte Antigene vorhanden und identifizierbar sind. Es existiert bereits eine Sammlung von wissenschaftlicher Literatur, die die Identifizierung von organspezifischen Antigenen entweder unter Verwendung klassischer Immunisierungswege oder durch Immunisierung mit spezifischen Tumoren beansprucht. Diese sind in einem Übersichtsartikel von Goldenberg et al., Cancer-Res., 3455(1976) zusammengefasst, der zeigt, dass solche Antigene bekannt und verfügbar sind.
- Ähnliche, mit Organ und Gewebe zusammenhängende und -spezifische Antigene sind durch Hybridomverfahren identifizierbar, die monoklonale Antikörper produzieren. Eine neue Entwicklung ist die Produktion von menschlichen monoklonalen Hybridom-Antikörpern durch Sicherstellen von Lymphozyten oder Plasmazellen aus Patienten, die bestimmte Organ-begrenzte Au toimmunerkrankungen zeigen, z. B. Thyroiditis, Gastritis, ulzerative Colitis, Myositis und dergleichen. Die Antikörper-produzierenden Zellen werden sodann mit menschlichen Myelom- oder Maus-Myelomzellen verschmolzen, und Hybridome des entsprechenden Antiorgans und die Antigewebe-Antikörperbildung werden unter Anwendung gut bekannter Verfahren produziert und propagiert. Auch Patienten mit spezifischen Tumortypen können als Quelle für solche Lymphozyten oder Plasmazellen verwendet werden, oder solche Patienten können weiterhin mit solchen Tumorzellen zur Stimulierung der Produktion von Antiorgan- und Antigewebe-Antikörpern immunisiert werden. Das lymphatische Gewebe, das entfernt wird, wird anschließend zur Verschmelzung mit geeigneten Myelomzellen durch Verfahrensweisen, die nun in der Technik gut bekannt und üblich sind, verwendet.
- Organ-zusammenhängende und Organ-spezifische Antigene können zur Immunisierung von anderen Spezies, z. B. subhumanen Primaten, Nagern, Kaninchen, Ziegen etc., durch eine Anzahl von aus der Technik bekannten Verfahren, wie Isolierung von Zellmembranen oder Aufbrechen der Zellen, z. B. durch Zentrifugation, Ultraschall, etc., unter Erhalt intrazellulärer Antigene isoliert werden. Es ist für diese Zwecke bevorzugt, intrazelluläre Antigene im Gegensatz zu Oberflächen-Antigenen und extrazellulären Antigenen zu verwenden. Auf diese Weise können Organ-zusammenhängende und Organ-spezifische Antigene aus einer großen Anzahl von Geweben und Organen des Körpers erhalten werden.
- Antikörper können unter Verwendung von Zellen produziert werden, die aus Gewebe, das aus einer Autopsie erhalten wurde, isoliert wurden. Beispielsweise können Mäuse mit solchen Geweben für eine Dauer immunisiert werden, die notwendig ist, um anti-spezifische Organ- oder Gewebeantikörper hervorzurufen. Die Milz dieser Mäuse wird entfernt und sodann durch Standardverfahren mit einer Maus-Myelomzelllinie, die zur Hybridomproduktion geeignet ist, verschmolzen. Unter Verwendung von Verfahren, die in der Technik bereits als Standard gelten, werden monoklonale Antikörper- produzierende Hybridomzellen selektiert und propagiert, und diejenigen mit Organ- oder Gewebe-zusammenhängender Antikörperproduktion werden als Quelle von Organ- oder Gewebeantikörpern kloniert und expandiert. Eine absolute Gewebespezifität ist nicht erforderlich, da nennenswerte quantitative Unterschiede in der Regel zur operativen Spezifität für Abbildungszwecke ausreichen.
- Antikörper und -fragmente, die bei der vorliegenden Erfindung geeignet sind, umfassen diejenigen gegen Antigene, die mit gesundem Gewebe und Zellen im Zusammenhang stehen oder davon produziert werden und mit bestimmten neoplastischen Geweben kreuzreaktiv sein können oder nicht.
- Bevorzugt sind diejenigen, die vor ihrer Markierung oder Konjugation eine spezifische Immunreaktivität gegenüber Ziel-Zellen oder Ziel-Gewebe von mindestens 60% und eine Kreuzreaktivität gegenüber anderen Antigenen von weniger als 35% aufweisen.
- Spezielle Beispiele umfassen Antikörper und -fragmente gegen Knochenmarkszellen, insbesondere gegen Hämatopoietin-Vorläuferzellen, Milzzellen, Uterus-Endometrium, Hormon-Rezeptoren, wie FSH, LH, TSH, und Wachstumsfaktor-Rezeptoren, wie epidermaler Wachstumsfaktor.
- Ein Beispiel für einen monoklonalen Antikörper, der mit Granulozyten reagiert und der auf Knochenmarkszellen gerichtet ist, ist NP-2, der bei Sharkey et al., Cancer Res. 50: 2823–283 (1990) offenbart ist. Dieser Antikörper ist bei Konjugation mit einem cytotoxischen Mittel zur Ablation des Knochenmarks geeignet. Ein weiterer Granulozyten-Antikörper, MN3, besitzt ähnliche Eigenschaften. Weitere Antikörper, die mit anderen Knochmarkszellen, insbesondere Vorläuferzelltypen, reagieren, und im Handel erhältlich sind, sind ebenfalls zur Knochenmarksablation geeignet.
- Antikörper, die auf die Milz gerichtet sind, umfassen den monoklonalen Antikörper LL2 (auch bekannt als EPB-2), der bei Pawlak-Byczkowska, Cancer Research, 49: 4568–4577 (1989) offenbart ist und gegen gesunde und maligne B-Zellen gerichtet ist und zur Behandlung gesunder Milzzellen bei Patienten mit Immunerkrankungen verwendet werden kann.
- Antikörper, die gegenüber dem Uterus-Endometrium spezifisch sind, sind bei Konjugation mit geeigneten therapeutischen Mitteln zur Therapie der Endometriose bevorzugt.
- Antikörper, die bei der vorliegenden Erfindung geeignet sind, können ein vollständiges Immunglobulin einer beliebigen Klasse, z. B. IgG, IgM, IgA, IgD, IgE, chimäre oder hybride Antikörper mit dualer oder multipler Antigen-Spezifität oder mit Epitop-Spezifität sein. Es kann ein polyklonaler Antikörper, vorzugsweise ein Affinitäts-gereinigter Antikörper aus einem Menschen oder einem entsprechenden Tier, z. B. ein Primat, eine Ziege, ein Kaninchen, eine Maus oder dergleichen sein. Auch monoklonale Antikörper sind zur Verwendung bei der vorliegenden Erfindung geeignet und sind auf Grund ihrer hohen Spezifitäten bevorzugt. Sie werden leicht durch eines der Verfahren, die nun zur Immunisierung von Säugern als gebräuchlich angesehen werden, mit einer immunogenen Antigen-Präparation, Verschmelzung von Lymph- oder Milz-Immunzellen mit einer unsterblichen Myelomzelllinie und Isolierung der spezifischen Hybridomklone hergestellt. Unkonventionellere Verfahren zur Herstellung monoklonaler Antikörper sind nicht ausgeschlossen, wie Interspezies-Verschmelzung und gentechnische Manipulationen der hypervariablen Regionen, da es hauptsächlich die Antigenspezifität der Antikörper ist, die ihre Brauchbarkeit bei der vorliegenden Erfindung beeinflusst. Es wird davon ausgegangen, dass auch neuere Techniken zur Produktion von monoklonalen Antikörpern, z. B. von menschlichen monoklonalen Antikörpern, monoklonalen Interspezies-Antikörpern, chimären (z. B. Mensch/Maus) monoklonalen Antikörpern, gentechnisch hergestellten Antikörpern und dergleichen, eingesetzt werden können.
- Antikörperfragmente, die bei der vorliegenden Erfindung geeignet sind, sind F(ab')2, F(ab)2, Fab', Fab, Fv und dergleichen, einschließlich von Hybridfragmenten. Ebenfalls geeignet sind alle Subfragmente, die die hypervariable Antigen-bindende Region eines Immunglobulins beibehalten und mit einer Größe, die kleiner ist als ein Fab'-Fragment. Diese schließen gentechnisch hergestellte und/oder rekombinante Proteine ein, gleich ob einkettig oder mehrkettig, die eine Antigen-Bindungsstelle einschließen und ansonsten in vivo als zielgerichtete Vehikel auf im Wesentlichen dem gleichen Weg wie natürliche Immunglobulinfragmente funktionieren. Solche einkettigen Bindungsmoleküle sind in der U.S.-Patentschrift 4,946,778 offenbart. Fab'-Antikörperfragmente können zweckmäßigerweise durch reduktive Spaltung von F(ab')2-Fragmenten hergestellt werden, die ihrerseits durch Pepsinabbau von intaktem Immunglobulin hergestellt werden können. Fab-Antikörperfragmente können durch Papainabbau von intaktem Immunglobulin unter reduzierenden Bedingungen oder durch Spaltung von F(ab)2-Fragmenten hergestellt werden, welche einem vorsichtigen Papain-Abbau von vollständigem Ig entstammen. Die Fragmente können auch gentechnisch hergestellt werden.
- Es sollte angemerkt werden, dass Gemische von Antikörpern, Isotopen und Immunglobulinklassen sowie Hybrid-Antikörper verwendet werden können. Die Hybride können zwei verschiedene Antigen-Spezifitäten aufweisen, z. B. einen Arm, der an ein Organ-Antigen bindet, und einen anderen Arm, der an ein anderes Antigen bindet, oder ein Arm könnte an ein Epitop auf dem Antigen binden, und der andere Arm könnte an ein anderes Epitop binden. Das Vorgenannte ist hauptsächlich erläuternd. Hybrid-Antikörperfragmente mit dualen Spezifitäten können entsprechend den Antitumor-Markerhybriden hergestellt werden, die in U.S.-Pat. Nr. 4,361,544 offenbart sind. Weitere Techniken zur Herstellung von Hybrid-Antikörpern sind z. B. offenbart in U.S.-Pat. Nr. 4,474,893; U.S.-Pat. Nr. 4,479,895; U.S.-Pat. Nr. 4,714,681; U.S.-Pat. Nr. 4,474,893 und bei Milstein et al., Immunol. Today, 5,299 (1984).
- Wird hier der Begriff "Antikörper" verwendet, sind darin alle obigen Typen von Antikörpern und -fragmenten eingeschlossen.
- Isotope, die bei der vorliegenden Erfindung verwendet werden können, sind: Iod-125, Iod-131, Rhenium-186, Rhenium-188, Silber-111, Platin-197, Palladium-109, Kupfer-67, Phosphor-32, Phosphor-33, Yttrium-90, Scandium-47, Samarium-153, Lutetium-177, Rhodium-105, Praseodym-142, Praseodym-143, Terbium-161, Holmium-166 und Gold-199.
- Es sind viele Arzneimittel und Toxine mit cytotoxischen Wirkungen auf Zellen bekannt. Sie werden in den Kompendien der Arzneimittel und Toxine, wie im Merck-Index, bei Goodman und Gilman und dergleichen und in den oben zitierten Druckschriften gefunden. Ein Antikörper kann mit jedem derartigen Arzneimittel durch die herkömmlichen Mittel, die aus der Technik bekannt und in Analogie zu denjenigen, die vorstehend beschrieben sind, erläutert sind, konjugiert oder befrachtet werden.
- Cytotoxische Mittel, die bei der vorliegenden Erfindung verwendet werden können, sind Taxol, Mechlorethamin, Cyclophosphamid, Melphalan, Uracil-Lost, Chlorambucil, Thiotepa, Busulfan, Carmustin, Lomustin, Semustin, Streptozocin, Dacarbazin, Methotrexat, Fluorouracil, Cytarabin, Azaribin, Mercaptopurin, Thioguanin, Vinblastin, Vincristin, Dactinomycin, Daunorubicin, Docorubicin, Bleomycin, Mithramycin, Mitomycin, L-Asparaginase, cis-Platin, Hydroxyharnstoff, Procarbazin, Mitotan, Prednison, Hydroxyprogesteroncaproat, Medroprogesteronacetat, Diethylstilbestrol, Ethinylestradiol, Tamoxifen, Testosteronpropionat und Fluoxymesteron.
- Die Antikörper und Fragmente, die bei der Erfindung verwendet werden, können durch eine Vielzahl von aus der Technik bekannten Verfahren radioaktiv markiert werden. Viele dieser Verfahren sind in den oben zitierten U.S.-Patentschriften und Patenanmeldungen offenbart und umfassen die direkte radioaktive Iodierung. Siehe auch Rayudu, op. Cit.; und Childs et al., J. Nuc. Med., 26, 293 (1985). Jedes herkömmliche Verfahren der radioaktiven Markierung, das zur Markierung von Isotopen zur in vivo Anwendung geeignet ist, ist im Allgemeinen erfindungsgemäß zur Markierung bildgebender Mittel geeignet.
- Die Antikörper und -fragmente können mit den obigen cytotoxischen Mitteln durch der Fachwelt bekannte Verfahren konjugiert werden. Die U.S.-Patentschrift Nr. 5,057,313; Shih et al., lehrt ein solches Verfahren.
- Die Befrachtung eines Trägers mit Arzneimitteln, wie in USP 5,057,313 offenbart, hängt von der Potenz des Arzneimittels, der Wirksamkeit der Antikörper-Ausrichtung und der Wirksamkeit des Konjugats nach Erreichen seines Ziels ab. In den meisten Fällen ist es erwünscht, den Träger mit mindestens 20, vorzugsweise 50 und oft mit 100 oder mehr Molekülen eines Arzneimittels zu befrachten. Die Möglichkeit zum teilweisen oder vollständigen Entgiften eines Arzneimittels als Konjugats, während es zirkuliert, kann die systemischen Nebenwirkungen des Arzneimittels reduzieren und erlaubt seine Verwendung, wenn die systemische Verabreichung des unkonjugierten Arzneimittels nicht akzeptabel wäre.
- Toxine sind als Fracht oft weniger konzentriert als Arzneimittel, allerdings ist es immer noch zweckmäßig, einen Träger mit mindestens 1, vorzugsweise 5 und in einigen Fällen mit 10 oder mehr Molekülen des Toxins zu befrachten und mindestens eine Trägerkette auf dem Antikörper zur gezielten Abgabe zu befrachten.
- Das Konjugat wird im Allgemeinen als sterile wässrige Lösung in Phosphatgepufferter Salzlösung verabreicht. Die Dosierung hängt von dem verwendeten Therapeutikum, der gewünschten Wirkung und den Nebenwirkungen, die der Patient erfährt, ab.
- Das Konjugat wird vorzugsweise auf systemischem Weg, z. B. intravenös, intraarteriell, intramuskulär oder subkutan, oder durch eine Kombination von systemischen Wegen, die seine Zunahme im Gewebe oder Organ von Interesse sicherstellen, injiziert.
- Das Konjugat wird im Allgemeinen als sterile wässrige Lösung in einer gepufferten Salzlösung verabreicht. Dosierungseinheiten von etwa 1–200 mg Konjugat werden für eine Dauer der Behandlung, wie vom Fachmann bestimmt, verabreicht. Es kann notwendig sein, die Dosis zu verringern und/oder Antikörper von anderen Spezies und/oder hypoallergene Antikörper, z. B. Maus/Mensch-Chimäre, CDR-gepfropfte ("humanisierte") Antikörper oder Primaten-Antikörper zu verwenden, um die Patientenempfindlichkeit herabzusetzen.
- Die Verabreichungswege umfassen intravenös, intraarteriell, intrapleural, intraperitoneal, intrathekal, subkutan oder durch Perfusion.
- Ohne weitere Ausführung wird angenommen, dass ein Fachmann unter Anwendung der vorhergehenden Beschreibung die vorliegende Erfindung zu ihrem vollsten Ausmaß anwenden kann. Die folgenden bevorzugten speziellen Ausführungsformen sind darum hauptsächlich zur Erläuterung konzipiert. In den folgenden Beispielen sind sämtliche Temperaturen in unkorrigierten °C angegeben, wenn nicht anderweitig angegeben, sind sämtliche Teile und Prozentangaben auf das Gewicht bezogen.
- BEISPIELE
- Beispiel 1 – Knochenmarksablation und Krebstherapie
- Einer Frau mittleren Alters mit fortgeschrittenem Brustkrebs, einschließlich Knochen- und Knochenmarksinvasion, wird ein Aliquot ihres Knochenmarks entfernt und nach Säubern des Marks von den Krebszellen in vitro zur Reimplantation geerntet. Sodann wird das Knochenmark in der Patientin durch i. v. Infusion von 20 mg monoklonalem NP-2-Antikörper F(ab')2, der mit 200 mCi Rhenium-188 markiert ist, nach dem Verfahren von Griffiths et al. (Cancer Res. 51: 4594, 1991) zerstört. Etwa 3 Wochen später zeigt sich eine schwere Knochenmarkstoxizität, die die Infusion des autologen, zuvor von Krebszellen gereinigten Knochenmarks in Kombination mit der Verabreichung von Hämatopoietin-Wachstumsfaktor, in diesem Fall mit GM-CSF, der vor und nach der Mark-Implantation wiederholt verabreicht wird, erfordert. Sechs Wochen später hatte sich bei der Patientin die Knochenmarksfunktion erneuert, und ein MN3-Fab'(Tc-99m)-Knochenmark-Screening zeigt ein gutes Knochenmarksbild, ohne Hinweis auf metastatische Fehler. Sie ist nun eine Kandidatin zur Behandlung von anderen Stellen ihres metastatischen Brustkrebses.
- Beispiel 2 – Endometriose-Therapie
- Bei einer Frau wird Endometriose diagnostiziert, und sie wird zur Behandlung an ihren Gynäkologen überwiesen. Ein mit Endometriumgewebe im Zusammenhang stehender monoklonaler IgG-Antikörper und ein monoklonaler IgG-Antikörper gegen den FSH-Rezeptor werden durch das Chloramin-T-Verfahren mit I-131 mit einer spezifischen Aktivität von 10 mCi/mg markiert. Die Kombination wird sodann zur Abgabe einer Dosis von 100 mCi I-131 i. v. infundiert. Nach Überwachen ihrer peripheren Blutzellen während des darauf folgenden Monats wird 6 Wochen später eine Wiederholungstherapie verabreicht. Nach weiteren 6 Wochen zeigt die Patientin eine vollständige Remission ihrer Symptome.
- Die vorgenannten Beispiele können mit ähnlichem Erfolg durch Ersatz der allgemein oder spezifisch für die Erfindung beschriebenen Reaktanten und/oder Arbeitsbedingungen für diejenigen wiederholt werden, die in den vorgenannten Beispielen verwendet werden.
- Selbstverständlich ist die Erfindung nicht auf die Verwendung bekannter Antikörper oder Marker beschränkt, sondern kann mit Antikörpern auf jeden beliebigen Marker, der von einem Organ oder Gewebe produziert wird oder damit im Zusammenhang steht, praktiziert werden kann.
- An der vorgenannten Beschreibung kann ein Fachmann leicht die wesentlichen Merkmale der Erfindung feststellen.
Claims (6)
- Verwendung eines Konjugats aus einem Antikörper oder Antikörper-Fragment und einem cytotoxischen Mittel bei der Herstellung eines Medikaments zur parenteralen Anwendung bei der Ablation von gesunden Zellen oder Geweben bei der Knochenmarksablation bei einem Patienten vor der Reimplantation gesunder Knochenmarkszellen; bei der Milz-Ablation zur Behandlung von thrombocytopenischer Immunpurpura; oder bei der Behandlung von Endometriose wobei: der Antikörper oder das Antikörperfragment auf einen Marker spezifisch ist, der mit der zu ablatierenden Zelle im Zusammenhang steht oder von ihr produziert wird; und das cytotoxische Mittel eines von folgenden ist: Iod-125, Iod-131, Rhenium-186, Rhenium-188, Silber-111, Platin-197, Palladium-109, Kupfer-67, Phosphor-32, Phosphor-33, Yttrium-90, Scandium-47, Samarium-153, Lutetium-177, Rhodium-105, Praseodym-142, Praseodym-143, Terbium-161, Holmium-166 oder Gold-199; oder Taxol, Mechlorethamin, Cyclophosphamid, Melphalan, Uracil-Lost, Chlorambucil, Thiotepa, Busulfan, Carmustin, Lomustin, Semustin, Streptozocin, Dacarbazin, Methotrexat, Fluoruracil, Cytarabin, Azaribin, Mercaptopurin, Thioguanin, Vinblastin, Vincristin, Dactinomycin, Daunorubicin, Doxorubicin, Bleomycin, Mithramycin, Mitomycin, L-Asparaginase, cis-Platin, Hydroxyharnstoff, Procarbazin, Mitotan, Prednison, Hydroxyprogesteroncaproat, Medroprogesteronacetat, Diethylstilbestrol, Ethinylestradiol, Tamoxifen, Testosteronpropionat oder Fluoxymesteron.
- Verwendung nach Anspruch 1, wobei der Antikörper oder das Antikörper-Fragment ein Fv, ein einkettiger Antikörper, ein Fab-, Fab'- oder F(ab')2-Fragment oder ein intakter Antikörper ist.
- Verwendung nach Anspruch 1 oder Anspruch 2, wobei das Medikament eine sterile injizierbare Zusammensetzung ist.
- Verwendung nach einem der Ansprüche 1 bis 3, wobei der Marker mit Knochenmarkszellen im Zusammenhang steht oder von ihnen produziert wird und das Medikament zur Verwendung bei der Knochenmarksablation in einem Patienten vor der Reimplantation gesunder Knochenmarkszellen ausgelegt ist.
- Verwendung nach einem der Ansprüche 1 bis 3, wobei der Marker mit Milzzellen im Zusammenhang steht oder von ihnen produziert wird und das Medikament zur Verwendung bei der Milzablation bei der Behandlung der thrombocytopenischen Immunpurpura ausgelegt ist.
- Verwendung nach einem der Ansprüche 1 bis 3, wobei der Marker ein Hormonrezeptor oder ein Wachstumsfaktorrezeptor ist, die zu ablatierenden Zellen Endometrium-Zellen sind und das Medikament zur Verwendung bei der Behandlung von Endometriose ausgelegt ist.
