Пациентка П. обратилась в отделение челюстно-лицевой хирургии им. Н.А. Семашко с жалобами на нарушение носового дыхания слева, периодические головные боли, чувство тяжести в левой половине лица, ощущение прохождения воды и воздуха через лунку удаленного 2.5 зуба. Из анамнеза: экстракция 2.5 зуба около 2х месяцев назад по поводу хронического периодонтита, альвеола 2.5 зуба не зажила, через 4 недели повторно обратилась к стоматологу по месту жительства, направлена в ОКБ им. Н.А. Семашко с целью консультации челюстно-лицевого хирурга, проведено КТ-обследование, установлен DS: Хронический одонтогенный верхнечелюстной синусит слева, ороантральное сообщение в области лунки 2.5 зуба. Назначена плановая госпитализация в ОКБ в отделение челюстно-лицевой хирургии с целью проведения оперативного вмешательства в объеме: эндоскопическая гайморотомия, устранение ороантрального сообщения (пластика раны местными тканями. Проведено оперативное вмешательство, местнопластический этап осуществлен по описанной ранее схеме: под местным обезболиванием (инфильтрационная и проводниковая анестезия) произведен разрез слизистой оболочки неба по десневому краю. Разрез продлен дистально и мезиально перфорации, произведен дополнительный разрез к небному шву в области клыка. Отслоен эпителиальный небный лоскут, после чего с помощью скальпеля сформирован субэпителиальный васкуляризированный лоскут (с обязательным сохранением надкостницы в интактном состоянии). Далее сформированы мягкотканные тоннели в области ороантрального свища в вестибуло-оральном направлении, первым слоем уложена PRF-мембрана над сообщением, после чего субэпителиальный лоскут проведен между альвеолярным отростком и десной, уложен на перфорацию, расправлен и подшит под вестибулярный край десны со щечной стороны П-образным швом по типу матрацного, небный дефект закрыт PRF-мембранами, после чего эпителиальный лоскут уложен на прежнее место и фиксирован по периферии узловыми швами. Послеоперационный период прошел гладко - швы сняты на 7 сутки, пациентка выписана к труду на _ сутки.
