Abstract
Description
The invention relates to medicine, in particular to ophthalmology, can be used to treat acute inflammatory diseases of the orbit in children.
A known method of treatment of acute infectious inflammatory diseases of the orbit, including opening the cavity of the orbit, followed by washing the retrobulbar space (Brovkina A.F. Surgical interventions for diseases of the orbit // Guide to eye surgery Edited by M.L. Krasnov. - M .: Medicine, 1988 .-- 624s.). The known method has the following disadvantages:
- значительная травматичность из-за широких и глубоких разрезов мягких тканей орбиты для оттока гноя, и как следствие этого, обильное кровотечение и большая вероятность повреждения сосудисто-нервных образований орбиты;- significant trauma due to wide and deep incisions of the soft tissues of the orbit for the outflow of pus, and as a result of this, profuse bleeding and a high probability of damage to the neurovascular formations of the orbit;
- затруднение дренирования из-за пассивного характера оттока патологического содержимого вследствие анатомического строения орбиты и вынужденного лежачего положения больного;- difficulty in drainage due to the passive nature of the outflow of pathological contents due to the anatomical structure of the orbit and the forced lying position of the patient;
- необходимость ежедневных болезненных манипуляций замены дренажа и промывания ретробульбарного пространства, нанесение связанной с этим психоэмоциональной травмы ребенку;- the need for daily painful manipulations to replace drainage and rinse retrobulbar space, causing a related psychoemotional injury to the child;
- недостаточность однократного введения лекарственного вещества в сутки в ткани орбиты для поддержания терапевтической концентрации, а повторное его введение практически невозможно;- insufficiency of a single injection of a drug substance per day into the tissues of the orbit to maintain therapeutic concentration, and its repeated administration is almost impossible;
- наличие одного канала в ретробульбарном пространстве, что затрудняет отток патологического содержимого;- the presence of one channel in retrobulbar space, which complicates the outflow of pathological contents;
- техническая сложность и значительная продолжительность операции и наркоза;- technical complexity and significant duration of surgery and anesthesia;
-косметический дефект, связанный с образованием грубого рубца после заживления послеоперационной раны;- cosmetic defect associated with the formation of a rough scar after healing of a postoperative wound;
The technical result of the invention:
- меньшая травматичность мягких тканей орбиты при пункции;- less traumatic soft tissue of the orbit during puncture;
- возможность дренирования ретробульбарного пространства;- the possibility of drainage retrobulbar space;
- установленные в ретробульбарном пространстве катетеры используются в течение нескольких дней для повторных, безболезненных манипуляций (вымывание гнойного и некротического содержимого);- catheters installed in retrobulbar space are used for several days for repeated, painless manipulations (leaching of purulent and necrotic contents);
- возможность и простота многократного введения лекарственных веществ по катетерам в ретробульбарное пространство в течение суток для поддержания необходимой концентрации;- the possibility and simplicity of repeated administration of drugs through catheters into retrobulbar space during the day to maintain the required concentration;
- создание необходимых условий для ирригации и аспирации путем установления катетеров в двух противоположных углах орбиты;- creating the necessary conditions for irrigation and aspiration by establishing catheters in two opposite corners of the orbit;
- упрощение технологии и сокращение времени операции и наркоза;- simplification of technology and reduction of operation time and anesthesia;
- избежание косметического дефекта.- avoidance of a cosmetic defect.
The technical result is achieved by a method of treating acute inflammatory diseases of the orbit, including penetration into the cavity of the orbit and subsequent washing of the retrobulbar space, characterized in that the penetration is made by puncture with a puncture needle in the upper-outer or upper-inner sectors of the orbit, a catheter is passed through the needle channel to the pathological focus, likewise, a second catheter is placed in the opposite corner of the orbit, the retrobulbar space is washed by simultaneous irrigation and aspiration when using antiseptic solutions followed by the introduction of medicinal substances, and at the end of the operation, the catheters are closed with sterile valves and left in the cavity of the orbit for repeated manipulations.
The installation of catheters in two opposite corners of the orbit ensures active, daily flushing of the retrobulbar space and complete aspiration of the pathological contents until the inflammatory process subsides.
Performing a puncture for introducing catheters into the retrobulbar space instead of an incision provides less trauma and a better cosmetic effect; simplifies technology, reduces surgery time and anesthesia.
Leaving the catheters for a certain period of time avoids the painful manipulations of replacing the drainage and causing a related psychoemotional injury to the child.
The installation of catheters in two opposite corners of the orbit requires only a single and short-term anesthesia in children.
The method is as follows.
The operation is performed under general anesthesia. After processing the surgical field, a puncture in the upper-outer or upper-inner sector is performed transpalpebrally. The choice of the installation site of the first catheter depends on the topography of the pathological focus in retrobulbar space. A puncture is made with a special needle, which allows a catheter with an internal diameter of at least 1 mm to be inserted into the retrobulbar space. The catheter is carried out along the channel of the needle to the pathological focus. The second catheter is inserted in the same way in the opposite corner of the orbit. The inner diameter of the second catheter must be at least 1.6 mm. After fixing the catheters, for example, using an adhesive plaster, an aspiration-irrigation system is connected to them. The retrobulbar space is washed and the pathological contents (purulent exudate, blood) are aspirated until it is completely removed under visual control. After washing, drugs are introduced into the retrobulbar space. At the end of the operation, the catheters are closed with sterile valves. Subsequently, the washing of the retrobulbar space with the irrigation of medicinal substances is usually carried out daily up to 4 times a day until the inflammatory process subsides.
After clinical recovery, the catheters are removed, the puncture sites are treated with alcohol, diamond greens.
