Больному проводят местную анестезию 1% раствором дикаина. Пациента укладывают на специальный подвижный операционный стол, имеющий 3 степени свободы. Совмещают центр роговицы с центром луча лазера, перемещая пациента при помощи стола. Накладывают блефаростат. Больной фиксирует взгляд на гелий-неоновом луче. Устанавливают перилимбально одно из вакуумных колец, входящих в набор. Включают вакуумный насос для обеспечения присасывания этого кольца и повышения внутриглазного давления. С помощью аппланационного тонометра контролируют внутриглазное давление, которое должно равняться 60 мм рт.ст. Далее проводят измерение диаметра будущей срезаемой крышки клапана с помощью набора аппланационных линз. При этом нужно срезать не менее 1/2 длины кератотомического рубца от центра. Таким образом, диаметр крышки клапана составляет 9,0 -9,5 мм и достигается путем подбора вакуумного кольца. Затем устанавливают в пазы вакуумного кольца головку микрокератома. Оросив предварительно роговицу физиологическим раствором, проводят поверхностную кератэктомию, формируя крышку клапана толщиной 130-150 мкм и захватывая не менее 1/2 длины кератотомического рубца от центра. Оставляют ножку клапана длиной 1 мм. Остановив микрокератом, выводят его из пазов сочленения с вакуумным кольцом. Убирают последнее с глазного яблока, выключив вакуумный насос. С помощью шпателя поднимают крышку клапана и отворачивают ее в сторону ножки. Под визуальным контролем с помощью операционного микроскопа производят эксимерлазерное испарение стромы ложа роговой оболочки излучением УФ-лазера, пропущенным через миопическую формирующую систему с Гауссовым профилем распределения плотности энергии с образованием на роговой оболочке плавного перехода от зоны лазерного воздействия к интактной в виде прямой. При этом энергия и число импульсов, требуемых для испарения стромы роговицы, заранее рассчитываются с помощью специальной компьютерной программы. Для эксимерлазерного испарения стромы роговой оболочки с кератотомическими рубцами требуется на 10-15% больше энергии по сравнению с интактным глазом такой же степени миопии. После завершения испарения стромы роговой оболочки с помощью шпателя укладывают крышку клапана на прежнее место на ложе роговицы. Промывают ложе роговой оболочки и внутреннюю часть клапана физиологическим раствором через канюлю. С помощью микротупфера удаляют лишнюю влагу из-под крышки клапана и добиваются адгезии последнего к ложу в течение 3 - 5 минут. Снимают блефаростат. Глаз промывают 0,25% раствором левомицетина и накладывают асептическую повязку. The patient is given local anesthesia with 1% dicaine solution.
