배경

상세한 설명
녹내장은 망막절세포(retinal ganglion cell) (“RGC”)의 사멸, 특정 시야 손실, 및 시신경 위축으로 특징지어지는 질병군이다. 녹내장은 세계적으로 실명의 주된 원인이다. 안압 (“IOP”)을 감소시키는데 효과적인 다양한 치료 옵션이 질병의 진행을 조절, 및 아마도, 늦출 수 있다. 치료 옵션은 예를 들어, 약제 요법 (즉, IOP-저하 약물), 레이저 눈 수술, 및/또는 녹내장 여과 수술 (또는 여과 수술 또는 섬유주절제술로도 알려진)과 같은 통상적인 수술 방법을 포함한다.
선진국에서 국소적 IOP-저하 약물의 광범위한 사용에도 불구하고, 특히 폐쇄-각 녹내장(closed-angle glaucoma)인 경우, 여전히 세계의 다른 곳에서 녹내장 수술이 통상적으로 시행되고 있다. 녹내장 수술은 시간이 지남에 따라 비용이 낮은 장점을 가지며 하루에 다회 적용해야하는 국소 점안과 관련된 준수 문제를 다룰 필요가 없다. 전형적인 녹내장 여과 수술 및 섬유주절제술은 높은 실패율을 가지며 (Schlunck et al. 제제 및 투여 요법(Dosing Regimen)

Invest Ophthalmol Vis Sci. 2003; 44(3):1097-1103)에 기재된 바와 같다. 간략하게, 부분 두께 8-0 실크 각막 견인 봉합사를 우선적으로 배치하고, 눈을 아래로 당겼다. 뇌궁(fornix) 기반 결막 플랩(conjunctival flap)을 올린 후, 연곽(limbus)을 따라 약 5mm 및 뒤쪽으로 8mm의 결막하 공간의 비절개 박리(blunt dissection) 수행하였다. 미세유리체망막 (microvitreoretinal, MVR) 도를 연곽 뒤로 3 내지 4mm의 부분-두께 공막 절개를 만드는데 사용되고, 각막 기질(corneal stroma)에 공막 터널(scleral tunnel)이 형성된다. 투명 각막에서 캐뉼라 바늘이 보일 때까지 22-게이지, 25-mm 정맥 캐뉼라 (Venflon 2; Beckton Dickinson, Oxford, UK)가 앞쪽 공막 터널을 통과한다. 전방(anterior chamber)으로의 진입은 캐뉼라 바늘로 이루어지며, 그 후 캐뉼라가 동공-중심(mid-pupillary) 영역에 진출하면 후퇴시켰다. 삽입점으로부터 1 mm 돌출되도록 캐뉼라를 손질하고 공막 끝에서 비스듬히 잘랐으며, 공막 표면에 튜브를 고정하기 위해 10-0 나일론 봉합사를 배치한다. 결막 절개는 두 개의 단속 봉합 및 바늘 (B/V 100-4; Ethicon)에 부착된 중앙의 매트리스-유형 10-0 나일론 봉합사로 봉합되어 방수 봉합(water-tight closure)이 된다. 구태 클로람페니콜(guttae chloramphenicol) 및 베트네솔-N(Betnesol-N) (Glaxo Wellcome, Uxbridge, UK) 연고 각각 한 방울을 수술 끝에 주입한다.
20 마리의 암컷 뉴질랜드 백색 토끼 (2-2.4 kg, 12-14 주령; Charles River)는 실험 시작 전에 5일 동안 순응될 것이다. 기재된 바와 같이 왼쪽 눈에 녹내장 수술이 수행될 것이다. 수술 후에, 토끼를 두 개의 군으로 나누고, 한 군을 비히클 및 또 다른 군은 닌테다닙 0.2% 용액을 처리할 것이다. 처리는 수술 직후에 시작되며 처리는 2 주 동안 TID될 것이다. 수술에 의해 형성된 수포의 생존 및 안압(intraocular pressure, IOP)가 28일 동안 추적될 것이다. 반흔 조직의 조직학적 분석이 연구 끝에 수행될 것이다.
결과
수술 성공 결과는 비히클 군에 비해 닌테다닙 군에서 현저히 연장될 것이다. 도 2는 수술 후 수포의 생존 커브를 나타내는 그래프를 제공한다. 도 2에 나타난 바와 같이, 닌테다닙 군은 비히클 군에 비해 현저히 연장된 수포 생존을 나타낼 것이다. 28일째 연구가 끝날 때까지, 닌테다닙 군에서 거의 모든 수포가 생존한 반면 비히클 군에서 수포가 생존하지 못할 것이다. 도 3은 수술 후 후속 기간 동안의 IOP 커브를 나타내는 그래프이다. IOP는 닌테다닙 군에서 낮게 유지되었고 (즉, 20 mm Hg 미만) 비히클 군에서는 점차 증가하였다. 차이는 통계적으로 유의미할 것이다. 수포 생존 및 IOP 변화 외에, 수술 부위에서 반흔 조직의 조직학적 분석은 비히클 군보다 닌테다닙 군에서 더 적은 반흔 조직을 나타낼 것이다.
이 실험의 결과는 닌테다닙 0.2% 용액이 녹내장 수술의 성공 (즉, 연장된 수포 생존, 수술 후 낮은 IOP의 기간의 연장 및/또는 감소된 섬유증/반흔 형성)을 증가시키는 것을 나타낼 것이다.
실시예 2: 녹내장 여과 수술의 보조 요법으로서 닌테다닙의 국소적 안구 제제
임상 연구에서 섬유주절제술의 성공을 증가시키기 위한 보조 요법으로서의 국소적 닌테다닙 0.2%. 섬유주절제술의 성공률에 대한 국소적 닌테다닙 0.2%의 효과를 시험하기 위해 랜덤화된, 이중-마스킹된, 플라시포-조절된, 12-주 실험 시험. 연구 설계는 Vandewalle et al (Vandewalle et al. Br J Ophthalmol. 2014, Jan; 98(1):73-8)에 기재된 바와 같을 것이다.
닌테다닙 또는 플라시보 용액의 한방울 TID를 받기 위해, 일차 섬유주절제술로 예정된 의학적으로 통제되지 않은 개방각 녹내장 환자를 등록하고 랜덤화할 것이다. 치료는 수술 직후 시작될 것이며 한달 동안 지속될 것이다. 약 150명의 환자가 본 연구에 등록될 것이다.
수술은 일반적 또는 구후 마취(retrobulbar anaesthesia) 하에 변형된 무어 필드(Moorfields) 기술을 이용하여 숙련된 외과 의사에 의해 수행될 것이다. IOP는 골드만 압평 안압계(Goldmann applanation tonometry)로 측정될 것이다. 마스킹될 관찰자에 의해 두 가지 측정이 수행되었고 두 가지 값이 2 mm Hg 이내인 경우 평균 IOP를 결정하기 위해 평균이 계산되었다. 처음 두 측정들 간의 차이가 >2 mm Hg이면 세 번째 측정이 수행될 것이다.
환자들은 섬유주절제술 1일; 1, 2, 및 4 주; 및 3, 6, 및 12 달 후에 검사될 것이다. 모든 환자들은 최대-교정 시력의 측정, 자이델 시험(Seidel test)을 포함하는 세극-등 시험, IOP 측정, 및 90-디옵터 렌즈를 이용한 안저 생체현미경 검사(fundus biomicroscopy)를 포함한 포괄적인 안과 검사를 받게 될 것이다. 수술 후 IOP-저하 약물, 수술 중 및 수술 후 합병증, 및 외과 처치(surgical intervention) 또한 기록될 것이다.
절대적 성공은 1차 종료점(primary endpoint)일 것이며 기준치로부터 20% 이상 감소 및 광감수(light perception)의 소실 없이, 안압 (intraocular pressure, IOP) ≤21 mm Hg 및 >5 mm Hg로 정의된다.
결과
12-개월 방문에서 기준치와 비교했을 때 IOP는 닌테다닙 군 및 플라시보 군 모두에서 효과적으로 감소할 것이다. 녹내장 수술의 절대적 성공률(absolute success rate), 즉, 수술 후 12개월 이상 동안 약 20 mm Hg 미만으로 IOP 유지하는 것은 도 4에 나타난 바와 같이 플라시보 군 대비 닌테다닙 군에서 더 높을 것이다. 6개월 후 시점에, 차이는 통계적으로 유의미할 것이다.
실시예 3: 제제
닌테다닙 안과 용액(Ophthalmic Solution)
약물 제품은 2-하이드록시프로필 베타 사이클로덱스트린(2-hydroxypropyl beta cyclodextrin) 또는 다른 유사한 사이클로덱스트린, 및 완충액으로 제조된, pH 범위 5.5 내지 8.0의 등장 안과 용액이다. 다른 점도 조절제, 윤활제, 보존제는 제제의 기능성을 향상시키기 위해 첨가될 수 있다. 안과 용액의 조성물은 표 1에 개시되어 있다.

약물 제품은 카복시메틸셀룰로오스 소듐 및 완충액으로 제조된, pH 범위 5.5 내지 8.0의 등장 안과 현탁액이다. 약물 입자 크기는 40 마이크론 이하로 감소된다. 다른 점도 조절제, 윤활제 및 보존제는 제제 현탁액의 기능성을 향상시키기 위해 첨가될 수 있다. 조성물은 표 2에 개시되어 있다.

닌테다닙 안과 에멀젼
약물 제품은 등장 안과 에멀젼이다. 약물을 오일상 혼합물 및 유화 부형제에 용해한 후 유화하고 pH 범위 5.5 내지 8.0의 수상과 혼합한다. 다른 점도 조절제, 윤활제, 가용화제, 및 보존제는 에멀젼 제제의 기능성을 향상시키기 위해 첨가될 수 있다. 조성물은 표 3에 개시되어 있다.

닌테다닙 서방형 반-고체 제제

닌테다닙 서방형 이식물
약물 제품은 고체 이식물이다. 약물을 하나 이상의 폴리머와 혼합 및 배합한다. 약물 및 폴리머 혼합물을 소정의 온도에서 녹이고 소정의 직경 크기의 필라멘트로 압출한다. 필라멘트 제제를 안구 조직에 이식할 수 있는 소정의 크기의 세그먼트로 자른다. 조성물은 표 5에 개시되어 있다.

Claims (26)
- 녹내장 수술 성공률을 개선하기 위한 치료에 사용하기 위한 조성물로서, 혈관 내피 성장 인자 수용체(VEGFR 1-3), 혈소판-유래 성장 인자 수용체(PDGFR α 및 β), 및 섬유아세포 성장 인자 수용체(FGFR 1-4)로부터 선택된 하나 이상의 수용체 타이로신 카이네이즈(receptor tyrosine kinase)의 억제제를 포함하고,
이 때 상기 치료는 상기 억제제를 포함하는 조성물을 이를 필요로 하는 대상의 눈에 투여하는 것을 포함하고,
이 때 조성물은 수술 부위에서 섬유증 및 비정상적 혈관 분포(vascularity)를 약화시킴으로써 반흔 형성을 감소시키는데 유효한 양의 상기 억제제를 포함하는, 조성물. - 제 1항에 있어서, 조성물은 국소적 안구 제제 또는 이식물의 형태인, 조성물.
- 제 1항에 있어서, 조성물은 녹내장 수술 후 적어도 10일 동안 보다 낮은 안압(IOP)의 기간을 연장시키고, 절대적 성공률 또는 제한적 성공률을 증가시키는데 효과적인 양의 상기 억제제를 포함하거나; 또는 조성물은 수포(bleb) 생존을 연장시키는데 효과적인 양의 상기 억제제를 포함하는, 조성물.
- 제 1항에 있어서, 조성물은 대상의 눈에 주입가능한 반-고체 또는 고체 서방형 이식물의 형태인, 조성물.
- 제 1항에 있어서, 조성물은 용액, 현탁액, 및 에멀젼으로 이루어진 군으로부터 선택된 국소 안구 제제인, 조성물.
- 제 1항에 있어서, 조성물 투여는 녹내장 수술 전, 중 또는 후에 수행되는, 조성물.
- 제 1항에 있어서, 녹내장 수술은 고전적인 섬유주절제술(trabeculectomy), 또는 트라벡톰(Trabectome), 우각경검사법(Gonioscopy)-보조 경혈관(transluminal) 섬유주대 절개술(trabeculotomy), 엑시머 레이저 섬유주대 절개술, 및 내시경(Endoscopic) 섬모체광응고술(cyclophotocoagulation)로 이루어진 군으로부터 선택된 방법을 이용하여 수행되는, 조성물.
- 제 1항에 있어서, 수행되는 녹내장 수술은 안구 여과 장치를 이식하기 위한 것인, 조성물.
- 제 8항에 있어서, 안구 여과 장치는 안구 스텐트인, 조성물.
- 제 1항에 있어서, 조성물은 세포-증식-억제 항대사 약물과 조합되어 투여되는, 조성물.
- 제 10항에 있어서, 항대사 약물은 미토마이신(Mitomycin) C, 5-플루오로우라실(Fluorouracil), 플록슈리딘(Floxuridine), 사이타라빈(Cytarabine), 6-아자우라실(6-Azauracil), 아자티오프린(Azathioprine), 메토트렉세이트(Methotrexate), 미코페놀레이트 모페틸(Mycophenolate Mofetil), 및 티오테파(Thiotepa)로 이루어진 군으로부터 선택되는, 조성물.
- 필요로 하는 대상에서 녹내장 수술과 관련된 보조 치료로서 사용하기 위한 조성물로서, 혈관 내피 성장 인자 수용체(VEGFR 1-3), 혈소판-유래 성장 인자 수용체(PDGFR α 및 β), 및 섬유아세포 성장 인자 수용체(FGFR 1-4)로부터 선택된 하나 이상의 수용체 타이로신 카이네이즈(receptor tyrosine kinase)의 억제제를 포함하고,
