“GEL POLIFUNZIONALE CONTRO LA SECCHEZZA VAGINALE AD EFFETTO DIRETTO E RITARDATOâ€
La presente invenzione ha per oggetto composizioni topiche vaginali in forma di gel comprendenti fattori mediatori dell’immunità , fattori di crescita, fattori chemiotattici e fattori antibatterici/antivirali estratti da colostro bovino ed eventualmente altri ingredienti ad attività complementare.
Stato della tecnica
L’ecosistema vaginale in condizioni fisiologiche à ̈ un equilibrio dinamico di flora microbica modulato dall’assetto ormonale, dal pH e dalla risposta immunitaria, fattori tra loro strettamente interdipendenti. Le cellule della parete vaginale hanno un importante ruolo nel mantenimento di tale equilibrio, in particolare gli strati superficiale ed intermedio dell’epitelio, la cui proliferazione e maturazione à ̈ ormono-correlata.
Normalmente la mucosa vaginale à ̈ umettata dal fluido, visibile all’ispezione come secreto chiaro costituito da trasudato del plasma, da muco cervicale e dalle secrezioni delle ghiandole vestibolari. Tale fluido contiene anche sostanze antimicrobiche che gli conferiscono una funzione di interfaccia protettiva. Il secreto vaginale à ̈ costituito per il 5% da trasudato vaginale e per il 95% dal muco cervicale e dalle secrezioni che in questo confluiscono. Il muco ostacola la colonizzazione microbica sia per un’azione meccanica, mediante le sue proprietà di viscosità e di eluizione, sia perché in esso sono presenti anticorpi (IgG e IgA), citochine e fattori antibatterici.
La cosiddetta “secchezza vaginale†à ̈ una condizione frequente nell’età della menopausa; diventa, invece, meno comune ma altrettanto fastidiosa quando si presenta in pazienti di età fertile.
Mentre nella donna anziana prevalgono come fattori favorenti l’ipoestrogenismo menopausale e le abitudini minzionali, nell’età fertile le cause possono essere rappresentate anche dall’uso di contraccettivi estroprogestinici a basso dosaggio, assorbenti, stress ecc..
I sintomi consistono in dolore, bruciore e prurito, particolarmente durante il rapporto sessuale; i disturbi tendono, poi, a protrarsi nel tempo. Le ghiandole di Bartolini e di Skene dovrebbero produrre muco, essenziale quale “liquido lubrificante†, utile al normale svolgimento dell’atto stesso. In presenza del disturbo, però, tale produzione non avviene, causando così il disagio alla paziente. La carenza di lubrificazione vaginale e vulvare à ̈ spesso causa, per le donne, di una serie di disturbi quali bruciore, prurito e dispareunia. Vi à ̈ anche una maggiore sensibilità a stimoli irritativi anche modesti e una maggior frequenza di episodi di vaginosi e vaginiti. Non à ̈ infrequente l’associazione con disturbi della minzione, sotto forma di bruciore urinario. All’esame colposcopio à ̈ frequente il riscontro di un quadro di irregolare captazione dello iodio al test di Schiller.
Numerosi sono i farmaci su base ormonale proposti per curare queste affezioni. Malauguratamente per tutti questi esiste il sospetto di un’aumentata incidenza di neoplasie mammarie, in particolare il comedocarcinoma. Sono stati proposti molti prodotti a base di fitoestrogeni ma anche per questi esistono le prove scientifiche di rischio di carcinoma mammario: gli isoflavoni di soia presentano il 97% di proliferazione della linea tumorale murina MCF-7 rispetto ai controlli.
Esiste pertanto la necessità di disporre di un prodotto a base di sostanze naturali che:
- non alterino l’ecosistema vaginale ma aiutino a ripristinare e mantenere l’equilibrio potenziandone le difese;
- siano in formulazione gel simile alla secrezione vaginale fisiologica e in grado di dare immediato sollievo, favorendo nel contempo la ricostruzione e il mantenimento dell’integrità della mucosa;
- sia in grado di rilasciare parte degli ingredienti attivi e parte in modo protratto nel tempo “lento rilascio†così da assicurare un effetto terapeutico prolungato e tale coprire il periodo che intercorre fra somministrazioni successive.
Descrizione dell’invenzione
Si à ̈ ora trovato che questi scopi sono raggiunti grazie a una formulazione che contiene come ingrediente attivo una frazione di colostro arricchita in immunoglobuline, fattori di crescita, chemiotattici e antibatterici/antivirali, in parte in forma libera a pronto rilascio e in parte in forma a rilascio controllato, preferibilmente incapsulato in microsfere mucotrope.
Le microsfere mucotrope sono disponibili commercialmente con la denominazione di SPHERULITE®, oppure possono essere preparate secondo WO 2010060886.
Le composizioni dell’invenzione contengo preferibilmente altri ingredienti, in particolare pantenolo, betaina, sericina, vitamina E, estratto di Lepidium mehenii (Maca), Aloe vera, argento metallico (ad esempio in forma colloidale).
Le composizioni dell’invenzione sono vantaggiosamente impiegate per il trattamento di donne in premenopausa, in menopausa, isterectomizzate, adolescenti, donne in puerperio o in allattamento, donne affette da secchezza da stress o che abusano di trattamenti locali squilibrati.
La frazione di colostro impiegabile nel gel dell’invenzione à ̈ ottenibile da colostro bovino, in particolare da mucche di razza Holstein (Frisona) e Guernsey. E’ stato provato che tali mucche producono colostro con la maggior concentrazione di fattori di crescita, modulatori dell’immunità , fattori chemiotattici e antibatterici/antivirali. Le mucche sono preferibilmente al secondo o terzo parto. Il colostro à ̈ preferibilmente raccolto fra la 5° e la 6° ora dal parto (colostro 5H), in quanto in questo periodo si assiste alla maggior concentrazione di attivi. Il colostro raccolto nella prima ora ha alcuni attivi più bassi, mentre a partire dalla sesta ora i fattori attivi diminuiscono rapidamente (dopo 24 ore dal parto ne sono presenti solo il 15%).
Il colostro 5H raccolto à ̈ sottoposto a test per tubercolosi, citotossicità su culture cellulari, controllo di micoplasmi, prioni e di virus umani e bovini.
Il colostro nella cisterna mammaria à ̈ praticamente sterile, ma una volta munto, nonostante ogni precauzione, per l’alta concentrazione di fattori di crescita, la sua carica batterica sale molto rapidamente durante la gelatura e la sgelatura, processi piuttosto lenti data l’alta densità del colostro nelle prime ore (Fig. 1).
La concentrazione dei conservanti ammessi per uso alimentare e quelli ammessi per uso parenterale e/o endovenoso, non ferma la carica batterica. L’utilizzo di raggi γ permette di ottenere un colostro sterile solo se si utilizzano radiazioni superiori a 10 Kgy che tuttavia distruggono gran parte dei fattori attivi, e d’altronde questo metodo non impedisce la formazione di pirogeni il cui utilizzo à ̈ proibito per via endovenosa e/o topica in aree a contatto con il sangue e i linfatici. E’ stata pertanto messa a punto una filiera di raccolta, innovativa, per ottenere un composto anallergico sterile, senza conservanti e senza pirogeni.
Al colostro raccolto in taniche sterili (sterilizzate a vuoto a 25 Kgy) si aggiungono agenti antisettici in quantità tali da garantire la sterilità e l’assenza di pirogeni (Figura 2). Si impiegano preferibilmente potassio sorbato e sodio benzoato, ciascuno alla concentrazione pari al 12,5%, (concentrazione ben maggiore di quella normalmente utilizzata, pari allo 0,2%), oppure, in alternativa, fenossietanolo alla concentrazione del 2,5% o diazolidinil-urea a una concentrazione dell’1% (Figura 3).
Il colostro così trattato non deve essere conservato gelato prima dei processi di estrazione dei fattori attivi, con evidente risparmio dei costi industriali.
Il colostro viene quindi diluito con soluzione salina: questa diluizione permette non solo una miglior filtrazione senza intasamento di pori filtranti ma soprattutto permette la liberazione di fattori attivi legati ai grassi e alla caseina. Il colostro così diluito viene sottoposto a microfiltrazione tangenziale (membrane ceramiche con cut-off compreso tra 2 e 6 µm, temperatura 5/20°C, pressione transmembranica compresa fra 0,2 e 2 bar) anche ripetuta, ottenendo una soluzione opalescente, libera da caseina, dalla matrice grassa e dalle proteine del latte. Tutte queste sostanze costituiscono oltre il 90% della carica allergica del colostro e del latte bovino. La soluzione viene quindi passata su membrane o in alternativa su setaccio molecolare con cut-off a 300.000 daltons per un’ulteriore purificazione dei fattori attivi, tutti con peso inferiore a 200.000 daltons.
La soluzione à ̈ quindi dializzata per ultrafiltrazione (cut-off 1000/2000 daltons) ad alta pressione per eliminare totalmente i conservanti (peso molecolare inferiore a 150 daltons) e quindi immediatamente liofilizzata (Fig. 4). Si ottiene una polvere sterile, priva di pirogeni, libera da conservanti, anallergica (caseina e lattoalbumina sono i responsabili di oltre il 95% dell’allergia al latte bovino) di altissima solubilità e con la massima concentrazione possibile di fattori attivi.
Questa frazione, di seguito identificata con la denominazione LIFEINSIDEâ„¢ Mucosa, contiene i seguenti fattori:
FATTORI MEDIATORI DELL’IMMUNITA’ Immunoglobuline della classe IgG2 e IgA (γ/mg), in proporzione pari a circa il 60% del contenuto di Lifeinside™ Mucosa (circa 50% IgG2 e circa 10% IgA), dotate di una specificità naturale contro molti batteri e virus, alcuni dei quali responsabili della sovrapposizione infettiva della NEC.
COMPLEMENTO C3/C4: Il complemento à ̈ costituito da proteine circolanti capaci di interagire con le membrane biologiche e con specifici recettori situati sulla superficie di diversi tipi cellulari e che hanno la capacità di indurre reazioni infiammatorie che aiutano a combattere le infezioni.
FATTORI ANTIBATTERICI/ANTIVIRALI
Transferrina;
Lactoferrina;
Lisozima;
Lattoperossidasi.
FATTORI DI CRESCITA TGF-β1- TRASFORMIG GROWTH FACTOR: stimola la produzione di immunoglobuline della classe A, responsabili delle difese immunitarie a livello mucoso. Modula la proliferazione cellulare e stimola la deposizione di matrice extracellulare.
EGF-EPIDERMAL GROWTH FACTOR: regola lo sviluppo della mucosa. Promuove la formazione di cellule epiteliali.
IGF 1- INSULIN-LIKE GROWTH FACTOR: modula la proliferazione, l’adesione e la migrazione cellulare e induce la maturazione delle mucose.
VEGF- VASCULAR ENDOTHELIAL GROWTH FACTOR: stimola la produzione di vasi sanguigni. Presenta attività mitogenetica e di attivazione della permeabilità vascolare.
FGF- b- FIBROBLAST GROWTH FACTOR BASIC: stimola la proliferazione delle cellule di origine mesenchimale come fibroblasti, cellule endoteliali, astrociti, cheratinociti. Agisce come fattore chemiotattico e fitogenetico.
GH-GROWTH HORMONE: fattore generale di crescita di ogni tessuto. GHRF: GROWTH HORMONE RELEASING FACTOR.
NGF-NERVE GROWTH FACTOR: stimola l’attività e la regolazione della crescita e della differenziazione del sistema simpatico.
PRP- Proline Rich Protein: Stimola la maturazione delle cellule T, ha azione di ripristino sul sistema nervoso, sul sistema osseo e sulle mucose.
FATTORI CHEMIOTATTICI
EOTAXIN: si lega ai recettori delle chemochine per richiamare gli eosinofili nei tessuti infiammati.
IP-10-Chemochin ligand 10: indotta dall’interferone gamma aggrega le cellule infiammatorie.
MCP-1 Monocyte chemotactic factor-1: favorisce l’aggregazione dei monociti ai tessuti infiammati.
CITOCHINE IL-2: induce la proliferazione dei linfociti T.
IL-4: ha attività antiinfiammatoria.
IL-6: stimola l’immunità innata e adattativa.
IL-9: regolatore delle cellule emopoietiche, stimola la proliferazione cellulare e previene l’apoptosi.
IL-17: Regola le attività dell’NF-KB e potenzia la produzione di ossido nitrico (NO).
IL-10: presenta effetti pleiotropici nell’immunoregolazione e nell’infiammazione. Migliora la sopravvivenza delle cellule B e pertanto la produzione di anticorpi. Studi sui topi Knockout dimostrano che questa proteina à ̈ essenziale nell’immunoregolazione delle mucose. Interferongamma: presenta note attività antivirali, antitumorali e immunoregolatorie. E’ un potente attivatore dei macrofagi e attiva l’attività cellulo-mediata contro batteri e virus.
TNF-α- Tumour necrosis factor: stimola la migrazione di neutrofili e monociti nella sede delle infezioni.
La frazione contenente le sostanze sopra elencata può essere vantaggiosamente associata ad altri composti ad attività complementare o comunque utile. Sono preferite le seguenti sostanze:
Betaina: La Betaina o trimetilglicina à ̈ una sostanza di origine vegetale estratta dalla barbabietola da zucchero. Interviene nella trans-metilazione, processo biochimico indispensabile per il metabolismo cellulare per mezzo del quale i gruppi metilici (CH3) sono trasferiti da una molecola ad un’altra. Nell’organismo la betaina perde il metile e si trasforma in dimetilglicina (vitamina B15): in questo passaggio si ha produzione di energia che si ritiene responsabile degli effetti benefici propri di questo composto: scomparsa della stanchezza immotivata, azione favorevole sul sistema circolatorio, sul cuore, sulle allergie, sulla respirazione cellulare. La betaina à ̈ un ingrediente ideale per prodotti destinati a pelli e mucose particolarmente sensibili e delicate, grazie anche alle sue proprietà idratanti e umettanti, utili per favorire l’immediata idratazione della mucosa vaginale.
Pantenolo: L’acido pantotenico e la forma ridotta corrispondente, pantenolo, à ̈ una tra le vitamine che trova maggiore utilizzo in applicazioni topiche. Si à ̈ rivelato efficace come ammorbidente, idratante e condizionante. Svolge differenti e fondamentali funzioni:
• dopo la penetrazione nelle mucose agisce da idratante attivo internamente, poiché ha un’eccellente capacità di trattenere umidità , permettendo alla pelle di essere morbida e elastica;
• stimola la proliferazione delle cellule ed aiuta la riparazione dei tessuti;
• promuove una normale cheratinizzazione;
• promuove la guarigione di piccole ferite, leggere abrasioni e piccole bruciature grazie alle sue proprietà lenitive.
Argento metallico: L’argento metallico puro al 99,9%, agisce da antimicrobico e antibatterico a lungo termine e basse concentrazioni (0,1%). E’ utile per normalizzare la flora mucosale, contrastare l’infiammazione causata da agenti patogeni, arrestare la proliferazione di microrganismi dannosi.
Vitamina E o Tocoferolo, vitamina liposolubile avente marcata attività antiossidante capace di impedire la decomposizione di vitamina A e acidi grassi con la formazione conseguente di cataboliti tossici e la formazione di radicali liberi e perossidi. La vitamina E entra inoltre nei processi di respirazione cellulare, fortifica le pareti dei vasi sanguigni, ha importanti effetti positivi sull’apparato riproduttivo.
Sericina Integra (MW 400.000): idratante, filmante, ha capacità tamponanti dell’acidità . Riduce l’aggressività dei tensioattivi. Presenta una notevole affinità verso le proteine idrofobe e un’ottima capacità di trattenere l’acqua, caratteristiche che aumentano con l’aumentare del peso molecolare. L’effetto filmogeno non à ̈ occlusivo, come avviene invece per l’acido ialuronico spesso utilizzato in prodotti per l’idratazione vaginale. Nel caso della sericina integra il gradevole e terapeutico effetto filmante non impedisce agli altri fattori di essere assorbiti dalla mucosa vaginale.
Maca (Lepidium Meyenii Extract): à ̈ un rimedio vegetale poco conosciuto ed anche un alimento altamente energetico. La sua popolarità à ̈ in aumento a causa degli effetti energizzanti, sulla fertilità e afrodisiaci. L’estratto di Maca contiene glucosinolati e isotiocianati ad attività chemio preventiva. Nelle donne l’estratto di Maca favorisce la fecondità , stimola la libido, allevia i sintomi associati alla menopausa, attenua i dolori mestruali e ha un effetto energizzante.
I componenti della Maca sono privi di attività estrogenica come dimostrato sul carcinoma mammario estrogeno-dipendente (cellule tumorali MCF-7).
L’Aloe Vera ha utili proprietà rigeneranti, proteolitiche, cicatrizzanti, antiinfiammatoria, antipiretica, analgesica, umettante, batteriostatica, virustatica, fungicida e antiprurito.
Le formulazioni in gel dell’invenzione sono preparate secondo tecniche note, utilizzando eccipienti convenzionali. La concentrazione della frazione di colostro in forma libera potrà variare tra 1 e 10% mentre quella in forma incapsulata a rilascio controllato tra 0,1 e 5%.
Si riportano di seguito intervalli di concentrazione indicativi per gli altri componenti, quando presenti:
Sericina: 0.5-5%
Pantenolo: 0.1-1%
Betaina: 1-10%
Estratto di Aloe vera: 1-5%
Vitamina E: 0.1- 2%
Argento metallico: 0.01-0.1%
Estratto di Lepidium meyenii: 1-5%

Esempio 2 - Prove cliniche
Il gel vaginale dell’Esempio 1 à ̈ stato utilizzato su 30 donne in etÃ
fertile, in premenopausa e in menopausa, affette da secchezza vaginale per
cause ormonali, meccaniche, da stress, da trattamenti locali squilibrati,
allattamento, contraccettivi ecc.
La tabella 1 riporta i risultati della colposcopia con il test di Schiller su
un gruppo di 30 pazienti in terapia e 28 di controllo. La differenza fra i due
gruppi al termine dello studio à ̈ statisticamente significativa (p < 0,05).

statisticamente significativa (p < 0,05, terapia con una singola
somministrazione giornaliera per 20 giorni).

L’uso del gel vaginale a rilascio combinato pronto e ritardato ha consentito un netto miglioramento del quadro clinico e della sintomatologia legata alla secchezza vaginale. Il gruppo di controllo registra dopo 20 giorni soltanto il 16,7% di casi migliorati mentre il gruppo in terapia à ̈ migliorato/guarito nell’86,7% dei casi.
L’esame citologico dopo trattamento (Figure 4-7) dimostra:
• Aumentata cellularitÃ
• Aumento relativo delle cellule squamose mature
• Diminuzione relativa delle basali e delle para basali
• Presenza di flogosi e di istiociti multinucleati indicanti un aumento del tropismo.

- RIVENDICAZIONI 1. Composizioni topiche vaginali in forma di gel comprendenti una frazione di colostro arricchita in immunoglobuline, fattori di crescita, chemiotattici e antibatterici /antivirali in parte in forma libera e in parte incapsulate in microsfere mucoadesive.
- 2. Composizioni secondo la rivendicazione 1 comprendenti Immunoglobuline della classe IgG2 e IgA, Complemento c3/c4, Transferrina, Lactoferrina, Lisozima, Lattoperossidasi, TGF-β1- Trasforming Growth Factor, EGF-Epidermal Growth Factor, IGF 1- Insulin-like Growth Factor, VEGF- Vascular Endothelial Growth Factor, FGF-b- Fibroblast Growth Factor BASIC, GH-Growth Hormone, GHRF: Growth Hormone Releasing Factor, NGF-Nerve Growth Factor, Proteine ricche in prolina, Eotaxina, IP-10-Chemochin ligand 10, MCP-1 Monocyte chemotactic factor-1, IL-2, IL- 4, IL-6, IL-9, IL-17, IL-10, Interferon-gamma, TNF-α- Tumour necrosis factor.

Composizioni secondo la rivendicazione 1 o 2 in cui la frazione di colostro à ̈ ottenibile da colostro bovino munto tra la quinta e la sesta ora dopo il parto e trattato con un processo che comprende: - aggiunta al colostro di agenti antisettici in concentrazioni sufficienti per garantire sterilità e apirogenicità ; - diluizione con soluzione salina; - uno o più stadi di microfiltrazione tangenziale su membrane ceramiche con cut-off compreso tra 2 e 6 µm; - passaggio su membrane o su setaccio molecolare con cut-off di 300.000 daltons; - dialisi per ultrafiltrazione ad alta pressione fino a completa rimozione degli agenti antisettici e liofilizzazione finale.
- 4. Composizioni secondo la rivendicazione 3 in cui il colostro à ̈ prelevato da mucche di razza Guernsey o Holstein.
- 5. Composizioni secondo una o più delle rivendicazioni precedenti contenenti inoltre uno o più dei seguenti ingredienti: a) Sericina; b) Pantenolo; c) Betaina; d) Estratto di aloe vera; e) Vitamina E; f) Argento metallico; g) Estratto di Lepidium meyenii.
