Description
The useful model belongs to the field of medicine, namely to gastroenterology, therapy and other fields of clinical medicine, and can be used for the prevention of progression and treatment of liver fibrosis in non-alcoholic steatohepatitis with comorbidities with obesity and osteoarthritis.
М.The problem of development and progression of non-alcoholic fatty liver disease (NAFLD) is currently one of the most important in modern internal medicine, it has general medical and social significance |Babak O.Ya., 2009; Kharchenko V.V., 2013; Amosova K.M., 2008). In Ukraine, from 1995 to 2005, mortality from cirrhosis of the liver of non-viral etiology among men aged 40 to 55 increased 3.5 times, and among men in general - 2 times (O. Ya. Babak, 2009) . At the same time, obesity is a very important problem in Ukraine and the world today (V.M.
Д. М.Kovalenko, 2012). In Ukraine, one in four women and one in six men is overweight. In total, about 15,95% of the population of our country suffers from obesity (G.D. Fadeyenko, 2009; V.M. Hvorostinka, 2008). The social significance of the increase in the incidence of obesity is that it is the most important component of the metabolic syndrome and a risk factor for the development of vascular and various dysmetabolic complications (M.D. Tronko, P.M.
2006).Karabun, 2006).
М., 2013).According to epidemiological studies, the prevalence of NAFLD in the population is 10-4095, and it has been established that the number of patients increases with age. The increase in the incidence of nonalcoholic steatohepatitis (NASH) occurs against the background of an increase in the frequency of obesity, metabolic syndrome, and diabetes (Kharchenko V.V., 2013; Rudenko T.M., 2013).
С. Г. 2012).NASH is the most aggressive form of NAFLD (A.S. Svintsitsky, O.G. Kuryk, 2012).
Due to the multifactorial nature of the development and progression of non-alcoholic steatohepatitis, therapeutic approaches should be complex.
The prototype of a useful model is a method of treatment and prevention of progression of liver fibrosis in patients with non-alcoholic steatohepatitis | Method of treatment and prevention of progression of liver fibrosis in patients with non-alcoholic steatohepatitis: patent of Ukraine
Theoretical prerequisites for the implementation of a useful model.
Fibrosis of the liver is an excessive diffuse development of connective tissue in the liver due to the action of damaging factors, which leads to the development of portal hypertension and impaired liver function.
Nonalcoholic fatty liver disease is a syndromic complex of interconnected nosologies, the components of which are: steatosis of the liver, that is, the accumulation of triacyl glycerides and fatty inclusions in the liver over 5 95 of its mass.
Steatohepatitis - inflammatory infiltration of the liver against the background of fatty dystrophy of hepatocytes and fibrosis, which can contribute to the development of steatogenic liver cirrhosis. At the moment, the distribution at the stage of development of fatty liver damage remains relevant.
Livery IS is a drug used for liver diseases, a detoxification agent. Hepatotropic drug.
The active substance methadoxin is a hepatoprotective detoxifying drug. According to the structure, metadoxine is a salt of pyridoxine with I-5-oxo-pyrrolidine-2-carboxylate (pyroglutamate), which can dissociate into pyridoxine (vitamin Bb) and pyroglutamate in human tissues.
At the cellular level, metadoxine: increases the level of reduced glutathione; increases the synthesis of nicotinamide adenine dinucleotide (NAD), improving the energy supply of the cell; normalizes the level of markers of liver damage: alanine aminotransferase (ALT), aspartate aminotransferase (AST), GHTP - due to inhibition of lipid peroxidation (POL) of cell membranes; normalizes the processes of decarboxylation, reamination and deamination of amino acids.
Pyridoxine in the liver plays a role in the neutralization and detoxification of bile acids by synthesizing their conjugates with taurine.
Guar gum is a dietary fiber obtained from the endosperm of seeds.
Suatorzivs Ieidopoiobry5 contains up to 7095 of the higher polysaccharide galactomannan. It is not absorbed from the digestive tract, is partially decomposed by bacteria and increases the volume of fecal masses by approximately 20 95.
The drug, thanks to its gel-forming ability, significantly increases the viscosity of liquid food and, due to the thickening of the diffusion layer of carbohydrates near the villi of the mucous membrane of the small intestine, slows down their absorption. By increasing the volume of chyme and slowing down the emptying of the stomach, guar provides a quick feeling of satiety. Thus, the drug reduces the feeling of hunger and the amount of food consumed. The decrease in the level of cholesterol in the serum is due to its reduced absorption, increased secretion of bile acids, as well as increased metabolism of cholesterol in the liver.
To control the treatment, the patient is comprehensively examined before the start of the treatment and 30 days after the treatment.
