Description
Bakgrunn for oppfinnelsen
Koronar hjertesykdom (CHD) forblir den ledende døds-årsak i de industrialiserte land. Til tross for nylige reduk-sjoner i CHD-dødelighet, er CHD fremdeles ansvarlig for mer enn 500 000 dødsfall i USA årlig. Det er anslått at CHD, direkte og indirekte, koster USA mer enn 100 milliarder dollar pr. år. Den primære årsak til CHD er aterosklerose, en sykdom som er karakterisert ved avsetning av lipid (kolesterol og triglyserider) i den arterielle karvegg, noe som resulterer i en forsnevring av det arterielle lumen og sluttelig årefor-kalkning av arteriene.
Aterosklerose som manifestert i sin hovedkliniske komplikasjon, koronar hjertesykdom (CHD) eller iskemisk hjertesykdom, fortsetter å være en hovedårsak til død i industrialiserte land. Det er nå vel akseptert at aterosklerose kan begynne med lokal skade på det arterielle endotelium etter-fulgt av penetrering av sirkulerende monocytter inn i intima av den arterielle vegg hvor de blir fylt med lipoprotein-avledede lipider. Ved tilnærmet samme tid synes det å være en vandring av arterielle glattmuskelceller fra det mediale lag til det intimale lag og deres proliferering der sammen med avsetning av lipid og akkumulering av skumceller i lesjonen. Ettersom den aterosklerotiske plakk utvikles, okkluderer den progressivt mer og mer av det angrepne blodkar og kan sluttelig føre til iskemi, trombose eller infarkt. Det er derfor ønskelig å tilveiebringe metoder for inhibering av progre-sjonen av aterosklerose hos pasienter med behov for det.
National Institutes of Health Consensus Development Conference Panel konkluderte med at nedsettelse av plasma-kolesterolnivåer (spesifikt blodnivåer av lipoproteinkoles-terol med lav densitet) definitivt vil redusere risikoen for hjerteangrep på grunn av CHD. Serumlipoproteiner er bærerne for lipider i sirkulasjonen. De er klassifisert i henhold til deres densitet; dvs. chylomikroner, lipoproteiner med meget lav densitet (VLDL), lipoproteiner med intermediær densitet (IDL), lipoproteiner med lav densitet (LDL) og lipoproteiner med høy densitet (HDL). Ca. 50% til 70% av kolesterolet som sirkulerer i blodet, bæres som LDL. Derimot finnes ca. 25% av totalt kolesterol i HDL, mens VLDL bærer det meste av plasma-triglyseridene og bare ca. 10% til 15% av det totale kolesterol .
Chylomikroner samles i den intestinale vegg fra prod-ukter av lipidfordøyelsen og transporteres deretter inn i den perifere sirkulasjon via det torakikolymfatiske system. I sirkulasjonen brytes de ned av lipoproteinlipase (LPL) til frie fettsyrer og triglyserider som primært anvendes av muskler for energi eller lagres i adiposevev. De andre serumlipoproteiner er involvert i transporten av endogent syntetisert lipid. Endogen lipidtransport begynner når leveren utskiller triglyserider og kolesterol i plasmaet som VLDL. Triglyseridene av VLDL spaltes i kapillarene av LPL til IDL og sluttelig LDL. Enkelte av disse partikler renses hurtig av leveren ved resep-torformidlet endocytose. Resten sirkulerer hovedsakelig som
LDL.
Ettersom celler dør og cellemembraner tar over, fri-gis kolesterol kontinuerlig inn i plasmaet og blir HDL. HDL aktiverer fjerningen av kolesterol fra perifere celler og let-ter dets transport tilbake til leveren.
Arterielt veggkolesterol er avledet praktisk talt utelukkende fra LDL [Brown og Goldstein, Ann. Rev. Biochem. 52, 223 (1983); Miller, Ann. Rev. Med. 31, 97 (1980)]. Fram-ingham-forskere fant at jo høyere nivåene av LDL var, jo høyere var risikoen for å utvikle CHD [Am. J. Med. 80 (Suppl. 2A) 23-32, 1986] . I pasienter med lave nivåer av LDL er utvik-ling av aterosklerose sjelden [Patton et al., Clin. Chem. 29, 1890 (1983)]. Følgelig er det ønskelig å tilveiebringe en metode for redusering av plasmakolesterol i pasienter med hyperkolesterolemi eller med risiko for å utvikle hyperkolesterolemi.
Forhøyede kolesterolnivåer er også assosiert med et utall sykdomstilstander, innbefattende koronar arteriesykdom, angina pectoris, halsarteriesykdom, slag, cerebral arterio-sklerose og xantom. Det er ønskelig å tilveiebringe en metode for reduksjon av plasmakolesterol i pasienter med, eller med risiko for å utvikle sykdomstilstander assosiert med forhøyede kolesterolnivåer.
Hypertriglyseridemi er en tilstand hvori det er en forhøyet mengde av triglyserid (>500 mg/dl) i plasma. Det kan spille en rolle i aterogenese og i utviklingen av koronar hjertesykdom. [Vega og Grundy, Adv. Exp. Med. 243, 311 (1989)]. I tillegg er alvorlig hypertriglyseridemi (>1 000 mg/dl) assosiert med chylomikronemi og forårsaker akutt pankreatitt [se K. Alvorlige forøkn-inger i chylomikroner fremkaller pankreatitt direkte, og det kan forhindres ved triglyseridreduksjon [U.S. Department of Health and Human Services, NIH-publikasjon nr. 89-2925, s. 74-77, januar 1989, "Report of the Expert Panel on Detection, Evaluation and Treatment of High Blood Cholesterol in Adults"]. Foreliggende oppfinnelse angår anvendelse av visse merkaptoacetylamidderivater som er kjent for å være anvendbare for inhibering av enkefalinase (EC 3.4.24.1) og ACE (EC 3.4.15.1) [Flynn, Warshawsky, Mehdi, Bey, Beight, Giroux og Burkholder, europeisk patentsøknad, publikasjonsnr. 0 481 522 Al, publisert 22. april 1992] ved behandling av pasienter som lider av hyperkolesterolemi.

Som anvendt her, angir uttrykket "pasient" varmblod-ige dyr eller pattedyr, innbefattende kaniner, gnagere, aper og mennesker som har behov for behandling for hypertriglyseridemi, aterosklerose eller hyperkolesterolemi, slik som f.eks. når det gjelder en pasient som lider av familiær hyperlipid-emi. Pasienter har behov for behandling for hypertriglyseridemi f.eks. når det gjelder en pasient som lider av hyperlipo-proteinemi type IV (som indikerer forhøyet VLDL) i henhold til Fredrickson-klassifiseringen [Fredrickson og Levy, FAMILIAL HYPERLIPOPROTEINEMIA, i The Metabolic Basis of Inherited Disease, 3. utg. Hyperkolesterolemi er en sykdomstilstand karakterisert ved nivåer av serumkolesterol eller LDL-kolesterol som er forhøyet med en klinisk signifikant mengde over den som betraktes som normal av fagmannen. Identifiseringen av de pasienter som har behov for behandling for hyperkolesterolemi, hører til fagmannens kunnskap og evne. Eksempelvis er individer som har serumkolesterolnivåer eller LDL-kolesterolnivåer som bestemt ved kliniske laboratorietester som er vesentlig og kronisk forhøyet over hva som betraktes som normalt av fagmannen, pasienter med behov for behandling for hyperkolesterolemi. Som ytterligere eksempel er individer som har risiko for å utvikle hyperkolesterolemi, også pasienter med behov for behandling for hyperkolesterolemi. En dyktig lege innen faget kan lett identifisere, ved anvendelse av kliniske tester, fysikalsk eksaminasjon og medisinsk/familiehistorie, de pasienter som lider av hyperkolesterolemi og de som har risiko for å utvikle hyperkolesterolemi, og kan således lett bestemme om et individ er en pasient med behov for behandling for hyperkolesterolemi .
En effektiv hypokolesterolemisk mengde av en forbindelse av formel (I) eller (II) er en mengde som er effektiv til å redusere serumkolesterolnivåer eller LDL-kolesterolnivåer i en pasient med behov for det. Som sådan skal vellykket behandling av en pasient for hyperkolesterolemi forstås å innbefatte en reduksjon av pasientens serumkolesterol eller LDL-kolesterolnivå. Vellykket behandling for hyperkolesterolemi skal også forstås å innbefatte profylakse ved forhind-ring av klinisk signifikante forøkninger i serumkolesterol eller i LDL-kolesterolnivåer i en pasient som har risiko for å utvikle hyperkolesterolemi.
En effektiv hypokolesterolemisk dose kan lett bestemmes ved anvendelse av konvensjonelle teknikker og ved observering av resultater erholdt under analoge omstendigheter. Ved bestemmelse av den effektive dose tas et utall faktorer i betraktning, innbefattende, men ikke begrenset til: arten av pasient; dens størrelse, alder og generelle helse; den spesifikke sykdom som er involvert; graden av eller innbefattelsen eller strengheten av sykdommen; responsen av den individuelle pasient; den bestemte forbindelse som administreres ; administreringsmåte; biotilgjengelighetskarakter-istikaene for det administrerte preparat; det valgte dose-regime og anvendelse av ledsagende medikamentering.
En effektiv hypokolesterolemisk mengde av en forbindelse av formel (I) vil generelt variere fra ca. 1 milligram pr. kilogram kroppsvekt pr. dag (mg/kg/dag) til ca. 1 000 milligram pr. kilogram kroppsvekt pr. dag (1 g/kg/dag). En daglig dose på fra ca. 2 mg/kg til ca. 200 mg/kg er foretrukket .
Ved utførelse av behandling av en pasient kan en forbindelse av formel (I) administreres i enhver form eller på enhver måte som gjør forbindelsen biotilgjengelig i effektive mengder, innbefattende orale og parenterale ruter. Eksempelvis kan forbindelsen administreres oralt, subkutant, intramusku-lært, intravenøst, transdermalt, intranasalt, rektalt og lignende . Oral administrering er generelt foretrukket. Fagmannen vedrørende fremstilling av formuleringer, kan lett velge den riktige form og administreringsmåte avhengig av den sykdomstilstand som skal behandles, graden av sykdommen og andre relevante omstendigheter.
Forbindelsene av formel (I) kan administreres i form av farmasøytiske preparater eller medikamenter som fremstilles ved kombinering av forbindelsene av formel (I) med farmasøyt-isk akseptable bærere eller eksipienser hvis mengde og art bestemmes av den valgte administreringsrute og standard far-masøytisk praksis.
De farmasøytiske preparater eller medikamenter fremstilles på kjent måte innen det farmasøytiske fag. Bæreren eller eksipiensen kan være et fast, halvfast eller væskeformig materiale som kan tjene som en bærer eller et medium for den aktive bestanddel. Egnede bærere eller eksipienser er vel-kjente innen faget. Det farmasøytiske preparat kan være til-passet for oral eller parenteral bruk og kan administreres til pasienten i form av tabletter, kapsler, stikkpiller, løs-ninger, suspensjoner eller lignende.
De farmasøytiske preparater kan administreres oralt, f.eks. med et inert fortynningsmiddel eller med en spiselig bærer. De kan være. innelukket i gelatinkapsler eller sammen-presset til tabletter. For oral, terapeutisk administrering kan en forbindelse av formel (I) inkorporeres med eksipienser og anvendes i form av tabletter, pastiller, kapsler, elik-sirer, suspensjoner, siruper, kjeks, tyggegummi og lignende. Disse preparater skal inneholde minst 4% av forbindelsen av formel (I), den aktive bestanddel, men kan varieres avhengig av den bestemte form og kan hensiktsmessig være mellom 4% til ca. 70% av vekten av enheten. Mengden av aktiv bestanddel til-stedeværende i preparatene, er slik at en enhetsdoseringsform egnet for administrering, vil bli erholdt. .Tablettene, pillene, kapslene, pastillene og lignende kan også inneholde én eller flere av følgende adjuvanser: bin-demidler, slik som mikrokrystallinsk cellulose, gummitragant eller gelatin; eksipienser slik som stivelse eller laktose, oppbrytende midler slik som alginsyre, "Primogel", maisstiv-else og lignende; smøremidler slik som magnesiumstearat eller "Sterotex"; glidemidler slik som kolloidalt silisiumdioksid; og søtningsmidler slik som sukrose eller sakkarin, kan tilset-tes, eller smaksgivende midler slik som peppermynte, metyl-salisylat eller appelsinsmak. Når doseringsenhetsformen er en kapsel, kan den i tillegg til materialer av den ovenfor an-gitte type, inneholde en væskeformig bærer slik som polyetyl-englykol eller en fettolje. Andre doseringsenhetsformer kan inneholde andre forskjellige materialer som modifiserer den fysikalske form av doseringsenheten, f.eks. som belegg. Således kan tabletter eller piller belegges med sukker, skjel-lakk eller andre enteriske belegningsmidler. En sirup kan inneholde, i tillegg til den aktive bestanddel, sukrose som et søtningsmiddel og visse konserveringsmidler, fargestoffer og smaksgivende midler. For parenteral administrering kan en forbindelse av formel (I) inkorporeres i en løsning eller suspensjon. Disse preparater skal inneholde minst 0,1% av en forbindelse ifølge oppfinnelsen, men kan varieres til å være mellom 0,1 og ca. 50% av vekten derav. Mengden av den aktive bestanddel til-stedeværende i slike preparater, er slik at en egnet dose vil bli erholdt.
Løsningene eller suspensjonene kan også innbefatte én eller flere av følgende adjuvanser: sterile fortynningsmidler slik som vann for injeksjon, saltvannsløsning, fettoljer, polyetylenglykoler, glyserol, propylenglykol eller andre syn-tetiske løsningsmidler; antibakterielle midler slik som benz-ylalkohol eller metylparaben; antioksidanter slik som askor-binsyre eller natriumbisulfitt; chelaterende midler slik som etylendiamintetraeddiksyre; buffere slik som acetater, sit-rater eller fosfater og midler for justering av tonisiteten slik som natriumklorid eller dekstrose. Det parenterale preparat kan være innelukket i ampuller, engangssprøyter eller multiple doseampuller fremstilt av glass eller plast.
Forbindelsene av formel (I) kan fremstilles som beskrevet i europeisk patentsøknad, publikasjon nr. 0 481 522 Al, publisert 22. april 1992, herved inkorporert ved refer-anse .

Kanintest for hypokolesterolemisk, antiaterosklerotisk og hypotriglyseridemisk aktivitet
For kaniner med en høykolesterol- (1%) diett i
8 uker, suppler diettene for visse kaniner med det angjeldende middel. Avliv kaninene ved slutten av den 8 uker lange peri-ode, oppsamle serum og bestem kolesterol- og triglyseridnivåer ved standardmetoder [Hypertension 15:327-331, 1990].
Disseker aorta av hver kanin fra den oppadstigende bue til hofteforgreningen, rens og preparer for beising med
Sudan IV for å bestemme områder av aterosklerotisk lesjon ved anvendelse av bildeanalyse.
Foreta statistiske sammenligninger mellom de kon-troll- og legemiddelbehandlede grupper for å bestemme aktiviteten av det angjeldende middel.
