Description
Foreliggende oppfinnelse angår en fremgangsmåte for fremstilling av et intravenøst, farmasøytisk preparat omfattende cyklosporin som aktiv bestanddel.
Cyklosporiner er cykliske oligopeptider dannet av mikroorganismer. Cyklosporin A, C og G utviser signifikant immunoundertrykkende effekt. Cyklosporin A er vidt anvendt når organer (nyre, hjerte, lunger, lever, pancreas) transplanteres for å inhibere den avstøtning som fremkalles av det transplanterte organ, og når benmarg transplanteres i cyklosporin A for å forhindre transplantat-versus-vertsykdom. Cyclosporin A anvendes, også med hell for behandling av autoimmune sykdommer (f.eks. diabetes juvenilis, reumatoid arthritis, uveitis, psoriasis).
Cyklosporiner dannes fra nøytrale aminosyrer med hydrofob karakter og er uløselige i vann. Deres dårlige løse-lighet i vann forårsaker at de ikke absorberes eller bare delvis absorberes i organismen når de administreres sammen med vanlige farmasøytiske eksipienter, hvorfor disse preparater ikke er egnet for terapeutisk bruk.
Cyklosporiner må oppløses eller dispergeres i et kolloidsystem hvis de skal kunne anvendes i terapeutisk praksis.
For å gjøre cyklosporiner anvendbare ved behandling, er følgende løsninger blitt utarbeidet: 1. Såkalte "faste løsninger" fremstilles under anvendelse av polyethylenglycoler med høy molekylvekt (Chion, W. L., Riegelman, S.: J. Pharm. Sei. 60, 1281 (1971)). 2. Cyklosporin oppløses i naturlige oljer, og den således erholdte løsning innkapsles (van Hoff, J. P. et al.: 1987, II. 1456). 3. Cyklosporin oppløses i blandingen av et om-estringsprodukt av et naturlig vegetabilsk olje-triglycerid, en polyalkylenglycol, ethanol og en vegetabilsk olje (øste rriksk patentskrift nr. 375.828, US patentskrift nr. 4.388.307). 4. Cyklosporin oppløses i en blanding av ethanol og "Chremophor" EL (polyoxyethylert castorolje, fremstilt av
BASF, Ludwigshafen, Tyskland) (Sandimmun Product Information, kapittel XII, Sandoz-rPharma, Basel, 1984) .
Produktene fremstilt i henhold til metode 1, er ikke egnet for parenteral bruk idet de er pelletpartikler som kan administreres oralt.
Produktene fremstilt i henhold til metode 2, er også bare egnet for oral administrering.
Produktene fremstilt i henhold til metode 3, er ikke egnet for intravenøs administrering på grunn av deres olje-innhold og kan derfor administreres subkutant eller intra-muskulært.
Den intravenøse administrering av preparater fremstilt i henhold til metode 4, er velkjent innen faget. Imidlertid lider de av den ulempe at de er ikke godt tolerer-bare av pasientene, dvs. etter administrering forårsaker de ofte anafylaktiske reaksjoner som er alvorlige ut fra pasientens synspunkt (Kahan et al.: Lancet, 1984, 1:52; Leunissen, K. M. L. et al.: Lancet, 1985, 1:636; Howrie, D. L. et al.: Drug Intell. Clin. Pharm. _19, 425 (1985)).
Den anafylaktiske reaksjon finner ikke sted når cyklosporin administreres i forskjellige sammensetninger, hvorfor det har blitt angitt at den polyoxyethylerte castorolje utelukkende er ansvarlig for å forårsake den anafylaktiske reaksjon ("Chremophor" EL, Technical Leaflet MEF 074e, BASF, Ludwigshafen, 1984). Selv om aktiviteten av "Chremophor" ble grundig studert, er det ingen litteratur-referanse som beskriver den del av molekylet som er ansvarlig for den alvorlige bivirkning.
Det var derfor et mål å utarbeide et intravenøst farmasøytisk preparat omfattende cyklosporin som aktiv bestanddel som er mer tolererbart enn de kjente intravenøse preparater, dvs. dets anafylaktiske, hypersensibiliserende effekt er mindre enn den av de kjente formuleringer.
Det er nå funnet at hvis cyclosporin oppløses i blandingen av en alkohol egnet for intravenøs administrering og en monoester av en mettet, hydroxylert fettsyre dannet med polyethylenglycol, kan tilsynekomst og grad av toksiske bivirkninger signifikant reduseres eller fullt ut elimineres.
Da monoesterne av mettede, hydroxylerte fettsyrer dannet med polyethylenglycol er strukturelt lik "Chremophor", kunne det ikke forventes av fagmannen at tilsynekomsten av toksiske bivirkninger kan overvinnes ved anvendelse av disse forbindelser.
Oppfinnelsen angår således en fremgangsmåte for fremstilling av et intravenøst, farmasøytisk preparat omfattende cyklosporin som aktiv bestanddel, hvilken fremgangsmåte er kjennetegnet ved at det blandes
a) 1 vektdel av ett eller flere cyclosporiner,
b) 8 til 13 vektdeler av en monoester av en mettet, hydroxylert fettsyre dannet med polyethylenglycol eller
blandingen av angitte monoestere, med
c) 4 til 10 vektdeler av én eller flere intravenøst administrerbare mono- eller polyvalente alkoholer.
De farmasøytiske preparater fremstilt ifølge oppfinnelsen er egnet for intravenøs administrering av det hydrofobe cyklosporin A, G, C eller blandingen derav som er uløselig eller dårlig løselig i de vanlige farmasøytiske eksipienter, og muliggjør administrering av angitte cyklosporiner i en vandig løsning. Cyklosporinene kan anvendes i et hvilket som helst vektforhold i forhold til hverandre i preparatet.
Som komponent b) av det farmasøytiske preparat fremstilt ifølge oppfinnelsen kan monoestrene av C10_22, fortrinnsvis C14.22, og helst C16.20 mettede, hydroxylerte fettsyrer dannet med polyethylenglycol (PEG) med en molekylvekt på 600 til 1300, fortrinnsvis 750 til 1100, eller blandingen av slike monoestere i et hvilket som helst vektforhold, anvendes. Spesielt foretrukne monoestere er PEG-9-hydroxymyristat, PEG-9-hydroxypalmitat og PEG-12-hydroxystearat hvori PEG-delen har en molekylvekt på 750 til 1150, eller blandinger derav i ethvert vektforhold.
Disse forbindelser kan fremstilles eksempelvis ved metoden ifølge Chandrasekhara Rao, T. et al. (J. Am. Oil Chem. Soc, 5_4, 18, (1977)). Polyethylenglycol-12-hydroxy-stearat er kommersielt tilgjengelig. Disse komponenter b) kan oppløse de hydrofobe cyklosporiner i nærvær av co-løsningsmidler selv ved romtemperatur.
Som co-løsningsmiddel, komponent c), kan mono- eller polyvalente alkoholer egnet for intravenøs administrering, eller blandingen av slike alkoholer i et hvilket som helst vektforhold, anvendes. Eksempler på slike alkoholer innbe-fatter eksempelvis ethylalkohol, propylenglycol, polyethylenglycol eller blandingen derav.
Det farmasøytiske preparat fremstilles ved blanding av cyklosporinet med komponent c), hvoretter komponent b) opp-løses i den således erholdte løsning. I henhold til en annen utførelsesform kan komponent b) blandes med komponent c), og den aktive bestanddel kan oppløses i den således erholdte løsning. Blandingen utføres fortrinnsvis ved 10 til 50°C.
De således fremstilte preparater kan formuleres med eller uten ytterligere farmasøytiske eksipienter egnet for parenteral bruk, men de må fortynnes til det ønskede innhold av aktiv bestanddel før bruk ved behandlingen, etter som de således fremstilte farmasøytiske preparater er kon-sentrater som ikke direkte kan anvendes for intravenøs administrering.
Som parenteralt administrerbare egnede eksipienter kan nevnes vann eller vandige løsninger, f.eks. infusjons-løsninger slik som saltvann, glucose, dextran, fructose og mannitolløsninger. Fortynningen avhenger av det ønskede innhold av aktiv bestanddel av injeksjonsløsningen, slik at preparatene ifølge oppfinnelsen generelt fortynnes i et vektforhold på 1:20 til 1:100 med vann eller en egnet løs-ning. Mengden av cyklosporin som skal administreres i det farmasøytiske preparat, vil naturlig nok avhenge av den sykdom som skal behandles, alderen og tilstanden av pasienten, og strengheten av sykdommen, etc.
Den terapeutisk effektive mengde av cyklosporiner er velkjent. Ved anvendelse av preparatet fremstilt ifølge oppfinnelsen er en daglig dose på fra 3 mg/kg til 50 mg/kg an-tydet for å behandle kroniske inflammasjoner eller for å frem-kalle en immunoundertrykkende effekt.
Løsningen sammensatt av de ovenfor angitte komponenter, kan fylles direkte i ampuller eller kan suppleres med en egnet mengde av destillert vann, og den vandige løsning kan fylles i ampuller.
Før fylling i ampuller må løsningene filtreres til de er bakteriefrie. Dette kan foretas under anvendelse av f.eks. en regenerert cellulosemembran med en porestørrelse på 0,2 nm. Fyllingen av løsningen i ampuller utføres generelt i nærvær av luft eller en inert gass, f.eks. nitrogen, hvorpå ampullene lukkes.
Preparatene er stabile over lange perioder ved lavere temperatur, f.eks. ved 5°C, hvilket er vanlig anvendt for lagring av farmasøytiske midler i sykehus, og ved forhøyede temperaturer, f.eks. ved 75°C. Preparatene har således en god levetid, deres innhold av aktiv bestanddel utfelles ikke selv etter lang lagring, og de stiller også reduserte krav til temperatur.
Det farmasøytiske preparat omfatter kjente og lett tilgjengelige eksipienter og kan lett fremstilles. Samtidig elimineres hovedsakelig ulempen ved de kjente preparater omfattende cyklosporin som aktiv bestanddel når preparatene fremstilt ifølge oppfinnelsen anvendes i terapeutisk praksis.
Oppfinnelsen illustreres ytterligere ved de etter-følgende eksempler.
Eksempel 1
Parenteralt preparat omfattende cyklosporin A som aktiv bestanddel 65 g "Solutol" HS 15 (kjemisk navn: polyethylenglycol-660-12-hydroxystearat, fremstilt av BASF, Ludwigshafen, Tyskland) ble blandet med 30 ml 96% ethanol (USP XXI-kvalitet), hvorpå 5 g cyklosporin A ble oppløst ved romtemperatur i den således erholdte blanding.
Den således erholdte løsning ble brakt til 100 ml
med 96% ethanol (USP XXI-kvalitet). Væsken ble homogenisert ved omrøring og filtrert bakteriefritt gjennom et cellulose-membranfilter med en porestørrelse på 0,2 ym av "Sartorius" SM 11607-type. Den filtrerte løsning ble fylt i ampuller under nitrogenatmosfære, og ampullene ble fylt med 5,3 ml løsning og ble lukket. Det således erholdte preparat kan anvendes for intravenøs administrering etter fortynning med eksempelvis isotonisk saltvannsløsning, glucose, dextran, fructose eller mannitolløsning.
Eksempel 2
Parenteralt preparat omfattende cyklosporin A som aktiv bestanddel 35 g polyethylenglycol-800-9-hydroxymyristat og 30 g polyethylenglycol-850-9-hydroxypalmitat ble blandet med 20 ml 1,2-propandiol (av USP XXI-kvalitet), hvorpå 5 g cyklosporin A ble oppløst under omrøring i den således erholdte blanding mens temperaturen på blandingen ble holdt ved 50°C.
Den således erholdte løsning ble supplert til 100 ml med destillert vann egnet for injeksjon. Væsken ble filtrert til den var bakteriefri som beskrevet i eksempel 1, og ble fylt i ampuller (5,3 ml væske i hver ampulle) under argon-atmosfære.
Preparatet kan anvendes for intravenøs administrering etter fortynning som beskrevet i eksempel 1.
Eksempel 3
Parenteralt preparat omfattende cyklosporin C som aktiv bestanddel 55 g polyethylenglycol-850-9-hydroxypalmitat ble blandet med 20 ml polyethylenglycol 300 (av USP XXI-kvalitet), hvorpå 5 g cyklosporin C ble oppløst i den således erholdte blanding mens løsningen ble oppvarmet til en temperatur på 50°C.
Videre ble løsningen bearbeidet som beskrevet i eksempel 2.
Preparatet kan anvendes for intravenøs administrering etter fortynning i henhold til eksempel 1.
Eksempel 4
Parenteralt preparat omfattende cyklosporin G som aktiv bestanddel
5 g cyklosporin G ble oppløst i 20 ml 96% ethanol (USP XXI-kvalitet) og ble blandet med 65 g polyethylenglycol-950-12-hydroxystearat, fortrinnsvis "Solutol" HS 15. Den således erholdte løsning ble brakt til 100 ml med 96% ethanol (USP XXI-kvalitet). Væsken ble homogenisert ved om-røring og filtrert til den var bakteriefri som beskrevet i eksempel 1, og ble fylt i ampuller (5,3 ml væske i hver ampulle) under nitrogenatmosfære.
Preparatet kan anvendes for intravenøs administrering etter fortynning som beskrevet i eksempel 1.
Eksempel 5
Parenteralt preparat omfattende cyklosporin A som aktiv bestanddel 60 g polyethylenglycol-950-12-hydroxystearat, fortrinnsvis "Solutol" HS 15 (BASF, Tyskland) ble oppløst i 10 ml 96% ethanol (av USP XXI-kvalitet), hvorpå 5 g cyklosporin A ble oppløst i den således erholdte blanding. Ytterligere bearbeidelse ble foretatt som beskrevet i eksempel 1.
Preparatet kan anvendes for intravenøs administrering etter fortynning som beskrevet i eksempel 1.
Stabilitetstest
Stabiliteten av preparatene ifølge eksempel 1 til 5 ble undersøkt. 10 ampuller hver omfattende løsningene fremstilt i henhold til eksempel 1 til 5, ble lagret ved en temperatur på 0°C, 5°C, 25°C, 60°C og 75°C, og forandringen i sammensetningen av formuleringen ble testet ved høytrykks-væskekromatografi.
Etter 9 måneders henstand kunne ingen utfelling observeres i noen av prøvene. Etter undersøkelse av innholdet i ampullene ved HPLC kunne ingen spor av spaltning av noen komponent finnes.
Den potensielle anafylaktiske-hypersensibiliserende effekt og veneskadende effekt av preparatet fremstilt ifølge eksempel 1 (C+S) og det kommersielt tilgjengelige "Sandimmun"
(Sandoz) (C+C) (cyklosporin + "Chremophor" + ethanol) ble sammenlignet for å demonstrere den mer fordelaktige effekt av preparatet fremstilt ifølge oppfinnelsen overfor det kjente preparat.
1. Testing av hypersensibiliserende effekt
Metoden ifølge W. Lorenz og A. Schmal (Agents and Actions 12, 1/2 (1982)) egnet for bestemmelse av den anafylaktiske effekt av oppløseliggjørende midler, ble fulgt.
Testene ble utført på beaglehunder bedøvet med "Nembutal". Dyrene hadde spontan pusting. To polyethylen-kanyler ble ført inn i blodkarene til hundene: én i den abdominale aorta via den høyre lårarterie for å måle det systemiske (systoliske og diastoliske) blodtrykk, den andre i caral-vene inferior for å administrere preparatet i kroppen og for å ta blodprøver for å bestemme histaminnivået. Parallelt med målingen av blodtrykk ble hjertehastighet og pustefrekvens også målt. Tilsynekomst av andre kliniske symptomer (erythema, ødem, kutane allergiske reaksjoner) som fremkom under administreringen av preparatene, ble også studert. Etter en 20-minutters ekvilibreringstid etter operasjonen ble grunnverdiene bestemt, og testene ble gjen-tatt 2, 5, 10 og 20 minutter etter administrering av preparatene (i.v. bolusinjeksjon uten fortynning). Preparatene ble administrert i en dose på 3 og 10 mg/kg kroppsvekt.
Effekten av preparatene på blodtrykk og hjertehastighet ble bestemt ved variansanalyse. Ødemet og hudfargnings-reaksjoner ble klassifisert i fire kategorier. Effekten av preparatene på det systoliske og diastoliske blodtrykk (figur 1 og 4), på hjertehastighet (figur 2 og 5) og på pustefrekvens (figur 3 og 6) og hudsymptomer (tabell I) er opp-ført i figur 1 til 6 og i tabell I.
På basis av resultatene av testen kan det fastslås at preparatene fremstilt ifølge oppfinnelsen (C+S) administrert ved lav dose (3 mg/kg kroppsvekt), ikke hadde noen signifikant kardiovaskulær effekt (ikke forårsaket signifikant hypotensjon og bradycardia), mens sammenligningspreparatet (C+C) signifikant nedsatte blodtrykket og hjertehastigheten selv ved denne lave dose.
Når preparatene ble administrert ved høyere doser
(10 mg/kg kroppsvekt), har preparatet ifølge foreliggende oppfinnelse (C+S) en vesentlig mer moderat kardiovaskulær effekt enn sammenligningspreparatet (C+C).
Det er ikke noen signifikant forskjell mellom den pustefrekvensøkende effekt av de undersøkte preparater.
Når preparatene ble administrert i en dose på 3 mg/kg, forårsaket begge preparater ødem og erythema, men når preparatet fremstilt ifølge oppfinnelsen (C+S) ble administrert i en dose på 10 mg/kg, forsvant imidlertid hudreaksjonene praktisk talt. I betraktning av den hypersensibiliserende effekt er følgelig preparatet fremstilt ifølge oppfinnelsen mer fordelaktig enn det kjente sammenligningspreparat (C+C).
2. Testing av lokal veneskadende effekt
Den lokale veneskadende effekt ble testet på New-Zealand-kaniner. De to preparater ble fortynnet til det 20-dobbelte volum med fysiologisk saltvannsløsning, og 0,2 ml ble injisert i venen av ett øre, mens fysiologisk saltvannsløsning ble injisert i venen av det andre øre.
I den tredje og fjerde gruppe av testdyr ble de opp-løseliggjørende midler ("Solutol" og "Chremophor") og bærere (ethanol) testet. Preparatene omfattende "Solutol" pluss ethanol og "Chremophor" pluss ethanol, ble fortynnet til det 20-dobbelte volum med fysiologisk saltvannsløsning, og de ble administrert i samme dose (0,2 ml) i venen av øret til testdyrene, mens bare fysiologisk saltvannsløsning ble injisert i venen av det andre øre.
De lokale reaksjoner ble observert 24, 48 og 72 timer etter behandlingen. Ved dette sistnevnte tidspunkt ble dyrene tappet for blod, og vevet ble undersøkt.
Preparatet fremstilt ifølge oppfinnelsen (C+S) ble like godt tolerert som sammenligningspreparatet (C+C).
Som en oppsummering kan det angis at preparatet fremstilt ifølge foreliggende oppfinnelse forårsaker lavere hypersensibiliserende effekt enn sammenligningspreparatet. For-skjellen er signifikant spesielt med hensyn til den blod-trykksnedsettende effekt. Med hensyn til lokal toleranse er preparatet fremstilt ifølge oppfinnelsen ekvivalent med sammenligningspreparatet, mens det er mer fordelaktig med hensyn til systemisk toleranse (hypersensibiliserende effekt).
Claims (7)
1. , Fremgangsmåte for fremstilling av et intravenøst, farmasøytisk preparat omfattende cyklosporin som aktiv bestanddel,
karakterisert ved at det blandes a) 1 vektdel av ett eller flere cyclosporiner, b) 8 til 13 vektdeler av en monoester av en mettet, hydroxylert fettsyre dannet med polyethylenglycol eller blandingen av angitte monoestere, med c) 4 til 10 vektdeler av én eller flere intravenøst administrerbare mono- eller polyvalente alkoholer.
2. Fremgangsmåte ifølge krav 1, karakterisert ved at det anvendes ett eller flere medlemmer av gruppen bestående av cyklosporin A, cyklosporin C og cyklosporin G som cyklosporin.
3. Fremgangsmåte ifølge krav 1, karakterisert ved at det anvendes ett eller flere medlemmer av gruppen av monoestere av ^ 2. 0- 22 mette^e' hydroxylerte fettsyrer dannet med polyethylenglycol (PEG) med en molekylvekt på 600 til 1300.
4. Fremgangsmåte ifølge krav 1, karakterisert ved at det anvendes ett eller flere medlemmer av gruppen bestående av monoestere av C14-22 mettede' hydroxylerte fettsyrer dannet med polyethylenglycol (PEG) med en molekylvekt på 750 til 1100 som komponent b).
5. Fremgangsmåte ifølge krav 1, karakterisert ved at det anvendes ett eller flere medlemmer av gruppen bestående av polyethylenglycol-9-hydroxymyristat, polyethylenglycol-9-hydroxypalmitat eller polyethylenglycol-12-hydroxystearat som komponent b).
6. Fremgangsmåte ifølge krav 1, karakterisert ved at det anvendes polyethylenglycol-6 60-12-hydroxystearat som komponent b).
