Description
本出願は、2012年12月21日に出願された米国特許仮出願第61/740,538号の利益を主張するものであり、この仮出願は、参照により本明細書に組み込まれる。
本発明は、腹部大動脈瘤用の装置に関し、より詳細には、生体内分岐ステント又は分岐ステントグラフトの対側性脚部の挿入を容易にする送達システムのガイド装置及び使用法に関する。
(1) 腔内送達装置であって、
近位領域及び遠位領域を有し、ガイドワイヤにわたって係合するように構成される細長い内側管状部材であって、前記遠位領域が第1の管状脚部及び第2の管状脚部を有する分岐腔内装置を受容するように構成されており、前記第1及び第2の管状脚部がそれぞれ遠位開口部を有し、前記細長い内側管状部材の前記遠位領域が前記第1の脚部内に位置付けられている、細長い内側管状部材と、
前記細長い内側管状部材よりも短い長さを有し、前記内側管状部材に接続され、前記第2の脚部内に位置付けられてそこを貫通する、最初から装着されているガイドワイヤセグメントと、を備える、腔内送達装置。
(2) 近位端及び遠位端を有し、前記細長い内側管状部材の少なくとも一部の周りに同心円状に位置付けられたシースを更に備え、前記シースが前記腔内装置の拡張を抑制し、そのため、前記シースと前記内側管状部材との間の相対運動によって前記腔内装置の送達が可能になる、実施態様1に記載の腔内送達装置。
(3) 前記シースが、潤滑コーティングを更に備える、実施態様1に記載の腔内送達装置。
(4) 前記最初から装着されているガイドワイヤセグメントが、前記第2の脚部の前記遠位開口部を超えて延在する、実施態様1に記載の腔内送達装置。
(5) 前記分岐腔内装置が、分岐点において前記第1及び第2の脚部に分岐する近位管状幹部領域を有し、前記最初から装着されているガイドワイヤセグメントが、前記分岐点に近位の前記細長い内側管状部材に接続されている、実施態様1に記載の腔内送達装置。
(7) 分岐ステント及び拡張エンドレッグを送達する方法であって、
第1の細長いガイドワイヤにわたって第1の血管を通って分岐ステントを送達する工程であって、前記第1の細長いガイドワイヤが近位端及び第1の遠位端を有し、前記分岐ステントが第1及び第2の端部を有し、前記第2の端部が分岐して第1及び第2の管腔開口部をもたらし、各開口部が前記第1の端部への不断の流路をもたらし、前記第1のガイドワイヤが、前記第1の管腔開口部内に位置付けられ、前記不断の流路を貫通し、前記第1の端部から出ており、前記第1のガイドワイヤが、前記遠位端に接続されている補助長さを有し、前記第1の遠位端から離れて位置付けられ、前記開口部を超えて延在する、前記第2の管腔開口部内に位置付けられる、第2の遠位端をもたらす、工程と、
第2のガイドワイヤにわたって第2の血管を通ってスネア装置を送達する工程であって、前記スネア装置が前記第1のガイドワイヤの前記第2の遠位端に動作可能に取り付けられ得る、工程と、
前記第1のガイドワイヤの前記第2の遠位端に前記スネア装置を取り付ける工程と、
前記ステントの前記第1の端部を超えて前記第1のガイドワイヤを前進させて、前記第2のガイドワイヤが前記第2の管腔開口部に位置付けられるようにする工程と、
前記第2のガイドワイヤにわたって第2の血管を通って、第1の端部及び第2の端部を有する拡張エンドレッグを送達する工程であって、前記拡張エンドレッグの前記第1の端部が前記分岐ステントの前記第2の管腔開口部内に位置付けられ、前記拡張エンドレッグの前記第2の端部が前記第2の血管に位置付けられて、前記不断の流路の拡張をもたらす、工程と、を含む、方法。
(8) 腔内装置を体腔内の位置に送達する方法であって、前記腔内装置が、近位幹部と、第1及び第2の遠位管状脚部と、単独の分離したエンドレッグ部と、を含む、本体を有し、前記方法が、
第1の送達装置を前記体腔内の位置に送達する工程であって、前記第1の送達装置が第1の細長い部材と、そこに接続されている、最初から装着されているワイヤと、を含み、前記第1の細長い部材が前記腔内本体部の前記近位幹部及び第1の遠位脚部を貫通するように前記腔内装置の前記本体を運ぶ、工程と、
前記腔内装置の前記本体を前記体腔内の前記位置に留置する工程と、
前記最初から装着されているワイヤの自由端が前記腔内本体部の前記第2の脚部内を延在するように、前記留置された腔内装置の前記本体内に前記第1の送達装置を位置付ける工程と、
第2の細長い部材を有する第2の送達装置を前記体腔内の前記位置に送達する工程であって、前記第2の細長い部材が、それに付随し、前記最初から装着されているワイヤを把持するように構成されている捕捉機構を有し、前記第2の送達装置が、前記腔内装置の前記エンドレッグを運ぶ、工程と、
前記第2の送達装置に付随する前記捕捉機構を使用して、前記最初から装着されているワイヤを把持する工程と、
前記第2の管状脚部の内腔部内へと前記第2の細長い部材を前進させて、所望の位置に前記エンドレッグを位置付ける工程と、
前記エンドレッグを留置する工程と、を含む、方法。
(9) 第1の血管を通って、前記体腔内の前記位置に第1のガイドワイヤを送達する工程を更に含む、実施態様8に記載の方法。
(10) 前記体腔内の位置に前記第1の送達装置を送達する工程が、前記第1のガイドワイヤにわたって前記位置まで前記第1の送達装置を前進させることを含む、実施態様9に記載の方法。
(12) 前記第2の管状脚部の遠位端を超えて遠位方向に前記最初から装着されているワイヤを延ばす工程を更に含む、実施態様8に記載の方法。
(13) 第2の血管を通って、前記体腔内の前記位置に第2のガイドワイヤを送達する工程を更に含む、実施態様8に記載の方法。

Claims (6)
- 第1の腔内送達装置であって、
近位領域及び遠位領域を有し、ガイドワイヤにわたって係合するように構成される細長い内側管状部材であって、前記遠位領域が、同じ長さの第1の管状脚部及び第2の管状脚部を有する分岐腔内装置を受容するように構成されており、前記第1及び第2の管状脚部がそれぞれ遠位開口部を有し、前記細長い内側管状部材の前記遠位領域が前記第1の管状脚部内に位置付けられている、細長い内側管状部材と、
前記細長い内側管状部材よりも短い長さを有し、前記内側管状部材に接続され、前記第2の管状脚部内に位置付けられてそこを貫通する、最初から装着されているガイドワイヤセグメントと
を備える、第1の腔内送達装置、
ならびに
第2の腔内送達装置であって、
前記最初から装着されているガイドワイヤセグメントを把持することができる捕捉機構を含み、前記第2の管状脚部の遠位内腔部に固着されるエンドレッグを留置する、第2の腔内送達装置
を含む、送達システム。 - 前記第1の腔内送達装置が、
近位端及び遠位端を有し、前記細長い内側管状部材の少なくとも一部の周りに同心円状に位置付けられたシースを更に備え、前記シースが前記腔内装置の拡張を抑制し、そのため、前記シースと前記内側管状部材との間の相対運動によって前記腔内装置の送達が可能になる、
請求項1に記載の送達システム。 - 前記シースが、潤滑コーティングを更に備える、請求項2に記載の送達システム。
- 前記最初から装着されているガイドワイヤセグメントが、前記第2の管状脚部の前記遠位開口部を超えて延在する、請求項1〜3のいずれか一項に記載の送達システム。
- 前記分岐腔内装置が、分岐点において前記第1及び第2の管状脚部に分岐する近位管状幹部領域を有し、前記最初から装着されているガイドワイヤセグメントが、前記分岐点に近位の前記細長い内側管状部材に接続されている、請求項1〜4のいずれか一項に記載の送達システム。
- 前記最初から装着されているガイドワイヤセグメントが、前記幹部内で前記細長い内側管状部材に接続されている、請求項5に記載の送達システム。
