Abstract
Description
ファンまたはエアコンから顔に吹きつけられる空気は、倦怠感を減らし、注意力を高めることが可能である。額に当てた濡れタオルは熱または頭痛による不快感を和らげることが可能である。これらの方法は、物理的に組織温度を下げ、外部環境は涼しいというメッセージで脳への信号を活性化することによりその効果を得る。
Rowsellら(1978)は、皮膚および身体の粘膜、特に、鼻、口、喉および消化管に対し、生理的冷却効果を有する様々なホスフィンオキシドについて記載する。例えば、その中の欄3および欄4の表を参照のこと。そこで示された化合物のうち10個(以下の表を参照)は1つのイソプロピル基(iso-C3H7と示される)を有する。該化合物はDIPA-1-6、DIPA-1-7、DIPA-1-8およびDIPA-1-9ではない。実際、該化合物は2つのイソプロピル基を有しない。
本発明の化合物は、(以下の一般式を有する)ホスフィンオキシドの例であり、特に、ジ−アルキル−ホスフィノイル−アルカン(R1、R2、R3のそれぞれはアルキル基である)の例である。
DIPA化合物は以下の汎用的方法で作製した。100mL(23.7g、〜200mmol)のイソプロピルマグネシウムクロリド(または、ジ−sec−ブチル誘導体の場合、sec−ブチルマグネシウムクロリド)を、Acros社からテトラヒドロフラン(THF)に溶けた25%溶液として入手し、500mLフラスコ(攪拌棒付き)に入れ窒素下に置いた。THFに溶けたホスホン酸ジエチル溶液(Aldrich社より取得、D99234、8.25g、50mL中60.6mmol)を滴下して加えた。約30分後、反応混合物を沸騰するまで温めた。反応混合物を30分間追加して攪拌し、続いて、適切なTHFに溶けたn−ヨウ化アルキル溶液(TCI社より、20mL中60mmol)を滴下して加えた。反応混合物はその後、室温で一晩攪拌した。反応混合物を水で希釈し、別の漏斗に移し、酢酸(〜10mL)で酸性にし、エーテルで2度抽出した。エーテル層は水で洗浄し、蒸発させた(ロータリーエバポレータBuchi、浴温40℃)。薄茶の油を高真空下で蒸留した。最終産物は質量分析により求めた質量で検証したが、それは無色またはかすかに淡黄色の透明液体であった。

本発明の別の態様は、治療法、例えば本明細書に記載の不調(例えば、疾病)の治療法に関し、該治療法には、治療の必要な対象に、本明細書に記載のDIPA化合物を好適には医薬品組成物の形で、治療効果のある量で投与することを含む。
(例えば、療法で役立つ、薬剤の製造で役立つ、治療法で役立つ)一実施形態において、治療とは、(例えば、炎症、かゆみ、または痛みにより引き起こされる)感覚的不快感、皮膚の異常感覚、皮膚炎、乾癬、眼球の不快感、熱不快感、熱ストレス、閉経後の女性における紅潮および/または寝汗(血管運動性症状)、術後の低体温、麻酔後振戦、疲労、倦怠感、うつ病、および、認知機能障害の治療、ならびに、認知機能を向上するための治療である。

本明細書で、不調を治療するという文脈で使用する「治療」という用語は、概して、人間または動物(例えば、獣医学的適用)の治療に関する。ここにおいて、いくつかの望ましい治療効果は、例えば、不調の進行の阻止により達成され、進行速度の低減、進行速度の停止、不調症状の軽減、および、不調の治癒を含む。予防手段(つまり、予防法)としての治療も含む。例えば、不調はまだ現れていないが、不調が現れる危険のある患者に使用することは、「治療」という用語に包含する。平均的、または健康と考えられる人の認知または身体パフォーマンスの基礎レベルを向上させるための治療も含む。

DIPA化合物またはDIPA化合物を含んだ医薬品組成物は、例えば、本明細書に記載の対象に局所的に適切に投与することができる。
対象/患者は、哺乳動物、例えば、有袋類(例えば、カンガルー、ウォンバット)げっ歯類(例えば、モルモット、ハムスター、ネズミ、ハツカネズミ)、ネズミ科の動物(例えば、ハツカネズミ)、ウサギ目の動物(例えば、ウサギ)、鳥類(例えば、トリ)、イヌ科の動物(例えば、イヌ)、ネコ科の動物(例えば、ネコ)、ウマ科の動物(例えば、ウマ)、ブタのような動物(例えば、ブタ)、羊のような動物(例えば、羊)、ウシ亜科の動物(例えば、ウシ)、霊長類、サル(例えば、サルもしくは類人猿)、サル(例えば、マーモセット、ヒヒ)、類人猿(例えば、ゴリラ、チンパンジー、オラウータン、テナガザル)、または人間とすることができる。
DIPA化合物を単独で投与することは可能だが、それを、本明細書に記載の少なくとも1つのDIPA化合物と、当業者には周知である医薬的に受容可能な1つ以上の他の成分をともに含んだ医薬品製剤として提供するのが好適である。該医薬的に受容可能な成分には、医薬的に受容可能な担体、希釈剤、賦形剤、補助剤、充填剤、緩衝剤、保存剤、酸化防止剤、潤滑剤、安定剤、溶解剤、界面活性剤(例えば、湿潤剤)、マスキング剤、着色剤、着香料、および甘味剤を含むが、これらに限定されるものではない。製剤はさらに、他の活性剤を含んでよい。

実験により、疲労を和らげ、最大の感覚的効果を達成する薬の最適な局所送達標的は、目または上顎の三叉神経枝の受容野にあることを発見した。顔における好適な部位は、眼窩周囲≧頬骨=眼窩舌であり、これらは図1でそれぞれ(f)、(c)、および(a)と表記する。(f)と表記した眼窩周囲部位には、瞼およびまつ毛の皮膚を含む。
DIPA化合物の送達は、化合物をクリームもしくは軟膏などの固形もしくは半固形媒体、または、溶液、ローションなどの液体媒体に溶解し、綿棒、ウェットワイプに乗せて、または、エアロゾル化したミストとして、達成することが可能である。
DIPA-1-7およびDIPA-1-8は抗疲労効果を生み、適用部位で「動的に涼しい」感覚を引き起こすことにより、熱ストレスおよび皮膚の不快感の緩和を提供する。感覚は、しっかりとした、涼しい、冷たい、または、氷のように冷たい感覚ではないが、突然爽やかで涼しいそよ風が(例えば、顔の)皮膚に吹きつけられたかのような、強く爽やかな感覚である。この効果は強烈である。この感覚の生理学的根拠、起こり得るレセプターメカニズム、ならびに、抗疲労、抗熱ストレス、および止痒性作用に関する動的冷却の重要性について、本明細書でさらに記載する。
小さい有髄線維(Aδ)および無髄線維(C線維)は、皮膚温度が例えば35℃〜15℃の間まで低下すると、求心性線維の発火頻度を増加する。放熱を検知するこれらの神経信号は中枢神経系に送られ、涼しいおよび冷たいという意識的知覚を生む。皮膚温度が35℃〜40℃に上昇すると、発火頻度はC線維において増加し、これらの線維は温かいという信号を送る(例えば、Hutchisonら(1997)を参照)。涼しさ/冷たさおよび温かさについての受容メカニズムおよび「ケーブル線」は、分離し別個であるが、脳において、および、おそらく末端部においても、相互に互いを阻害する。感覚的レセプターは様式特異性を持ち、機械的刺激には反応しない。分子レベルで、冷却剤の標的結合部位は、温度の降下に反応して脱分極するイオンチャネルレセプターにあると考えられる。放熱はレセプターの放電のしきい値を下げ、促進された脱分極は神経信号を発生する軸索反応を始める。
動的な冷却は、(静的な冷却/寒さに比較して)抗疲労効果に不可欠である。例えば、人に倦怠感があり、車両を運転している場合、空調をつけ空気を顔に吹き付けることは疲労を和らげるだろう。しかし、ただ空調をつけて周囲の温度を低くし車両内を冷却しても、大した違いは生まないだろう。
「TRP−」イオンチャネルレセプター(A1、M8、およびV1〜4)は、生理的温度の検知にとって最も重要な生体要素であるという一般的見解がある。TRPM8受容値は、メントールおよびイシリンなどの感覚/冷却剤に反応するものである(例えば、McKemy(2002)を参照)。TRPM8は1104−アミノ酸残基付きタンパク質であり、6つの貫膜ドメインを有する。周囲温度の低下によるこのレセプターの活性化は、5番目と6番目の貫膜ループの間に穴を開け、非特異的陽イオンの細胞内への侵入を引き起こす。感覚ニューロンにおけるTRPM8レセプターの脱分極は、その後、主にAδ線維(およびC線維の一部)を介して信号を送ることができる。
テスト化合物のインビトロでの効果を、(CHO細胞に発現する、ヒトTRPM8遺伝子によりコードした)クローンhTRPM8チャネルにおいて、Fluo−8カルシウムキットおよび蛍光イメージングプレートリーダー(FLIPRTETRA(登録商標))機器を用いて評価した。テスト化合物の特異性を調べるため、さらなるテストをTRPV1チャネル(HEK293細胞で発現するヒトTRPV1遺伝子)およびTRPA1チャネル(CHO細胞で発現するヒトTRPA1遺伝子)において実行した。アッセイは、ChanTest社(アメリカ合衆国、オハイオ州44128、クリーブランド、ネオ・パークウェイ14656)が行った。
理想的には、角化皮膚に送達するように配合された有効医薬品成分(API)は、安定し、有毒でなく、抗疲労効果、抗熱効果、または抗侵害受容効果をもたらすメカニズムを活性するのに十分なほど長く作用し、強くあるべきである。APIは、製造時に製剤が一定の濃度を維持するように、組成物に溶解して均一に分散させるべきである。最終製品は清潔度および無菌性の標準規格を満たすべきである。製剤のため、APIを標準の温度と圧力の状態(STP)で液体とすることが可能であり、それは中性pHおよび/または等浸透圧で水溶液に均一に溶解する。最終製品の無菌性は、微小孔フィルタ、加熱、または放射線照射を通し、精製試薬を使用することにより最適に達成することが可能である。乳化剤、生理食塩水、溶剤、安定剤、および保存剤など標準の賦形剤を添加して製剤を最適化することができるが、重要な成分は、好適には、精製水または標準皮膚溶剤などの水媒体に可溶であるべきである。
・疲労に対抗するため、眼窩および頬骨周りで「動的に涼しい」感覚を使用することの根拠の明確化、および、この作用の神経生理学およびメカニズムを記載すること。この感覚的効果は普通でなく、DIPA化合物では見られるが構造的に類似した化合物には見られない。
・角膜の侵害受容器に接触することを避けるAPI送達方法を考案すること。これは、それは刺痛/痛みを招き、有害で、実用的ではないためである。
・実験による理想の化合物(API)を発見すること。DIPA-1-7およびDIPA-1-8は水溶性であり(透明な溶液は蒸留した水に最大20mg/mLで得られる)、熱に安定的で、1〜10mg/mLの適用濃度で5〜7時間の間「動的に涼しい」感覚を働かせる。過耐性が反復適用に発展することはない。
・これら化合物のレセプター標的をインビトロで明確にすること、および、選んだAPIの選択性を明確にすること。
・DIPA-1-7の抗侵害受容作用を示す、インビトロ摘出神経の作製を明確にすること、および、この効果はTRPM8ノックアウトマウスに由来する神経では無効であることを示すこと。
・「動的に涼しい」性質を示し、作用のメカニズムのさらなる研究を可能にする、(「身震い」の)動物モデルを明確にすること。
・人間の志願者において、慢性疾患および熱ストレスにより引き起こされる疲労を低減し、平均的な人間の精神的能力レベルを高めることに対するDIPA化合物の有効性を示すテストを実行すること。
・人間の志願者において、DIPA-1-7は皮膚の感覚的不快感を緩和するのに効果的であるため、抗侵害受容薬もしくは止痒薬、または、皮膚の異常感覚を評価する診断ツールとして使用できることを示すテストを実行すること。
DIPA化合物は、角化皮膚に適用する際、放熱を再現するが、組織温度は変化させない感覚的/冷却効果を有する。これらの化合物、特に、DIPA-1-6、DIPA-1-7、およびDIPA-1-8は皮膚バリアを貫通し体循環に入って冷却作用を働かせることもできる。これらの効果は、少量、例えば、0.1〜0.5mLを、1〜20mg/mLつまり0.1〜2%wt/volの濃度で適用することで得られる。効果の発現は迅速で、5分未満であり、涼感は強力で、爽快で、強い。角化皮膚に対し類似の生物活性を有する化合物は、化粧用途または療法用途において現在使用されていない。
熱的快適性とは、空調技術者が使用する、「周囲の環境に満足の意を示す人間の精神状態」を定義する技術用語である。ビルまたは他の囲い地の占有者に対し熱的快適性を維持することは、建築家および設計技術者の重要な目標の一つである。ほとんどの人々にとって、熱的快適性のある室温は25℃(77°F)である。徹底した研究が、労働パフォーマンスおよび生産性(生産量/投入量)は、25℃を上回り33℃まで、+1℃ずつ増大するごとに2%ずつ低下することを記録に残した。28〜30℃(82〜86°F)のオフィス温度で、発汗、ならびに、頭痛、眠気・無気力、集中のしづらさ、および身体的不快感の訴えが増える。例えば、研究は、コールセンターの室内気温が25℃から26℃に上昇すると、電話の反応率が7.79電話/時間から7.64電話/時間へと1.9%低下することを示した。(例えば、Tanabeら(2007)を参照)。したがって、25℃を超える周囲温度は熱ストレスの一形態である。
身体的、または精神的のいずれかでより良く行いたいというのは人間の自然な願いである。最近、運動パフォーマンスを向上するため、寒冷療法を使用することへの興味が熱狂的に高まってきている。寒冷療法は、「療法目的を達成するため身体からの熱を撤去することにより(局所的に、または、全体的に)組織温度を下げること」と定義される。放熱により、例えば、氷に浸すまたは氷を詰めたベストを着用することにより外部を予め冷却することは、熱い環境での労働耐久性を向上させることが可能である(例えば、Marinoら(2002)を参照)。肉体労働生産量の〜5%の増加を約30分の仕事において見ることができる(例えば、Grahnら(2005)を参照)。熱疲労は労働を制限し、これは中心身体温度が40℃(104°F)に近づく場合に起きる。予め冷却すること(または、例えば冷たいスラリーを飲むことにより内部を冷却すること)は、蓄熱速度を緩める。
疲労は、重度の進行性疾患、特にがんを患う患者にとって重要な問題として認識される。これは、疲労は身体的、心理的、社会的、ならびに精神的満足感、および、生活の質(QOL)に悪影響を与えるためである(例えば、Mintonら(2010)を参照)。この症状は管理と優先的な研究を必要とする状態と断定される。がんに関連した疲労についての一致した定義は、「がん、または、がん治療に関する、通常の機能を妨げる、よくある、持続的で主観的な倦怠感」である。
身体的、または精神的のいずれかでより良く行いたいというのは人間の自然な願いである。パフォーマンスを向上するように設計された化学物質は、2つのカテゴリーに属する。身体的能力を増強するもの、例えば、アナボリックステロイドまたはビタミン、および、認知機能を増強するものである。「認知エンハンサー」(CE)である薬物は向知性薬または神経エンハンサーとも呼ばれ、カフェイン、アンフェタミン、メチルフェニデート、ニコチン、ドネペジル、およびモダフィニルが含まれる。CEは、抽象的思考、注意力、態度、ブレーンストーミング、理解、認識、創造的思考、批判的思考、好奇心の増加、実行機能、意思決定、直感像記憶、感情ならびに記憶、目標ならびに目標設定、想像力、知性、内省、水平思考、学習、記憶、暗算、モチベーション、知覚、人格、および回想(追想)など、仕事に関する個々の能力を向上するように設計されている。
DIPA-1-7およびDIPA-1-8により生成される強い「動的に涼しい」感覚は、皮膚のかゆみ止め(および他の抗侵害受容)効果のためさらに評価された。本明細書に記載の事例研究で示すように、綿棒で適用した20mg/mL溶液は、3人の接触性皮膚炎により引きこされたかゆみおよび不快感を強く止めた。
(a)種々の形態の皮膚炎(アトピー性、接触性、および刺激性)による炎症、かゆみ、および痛みの軽減、

(d)乾燥性、乾癬、または脂漏性皮膚炎による掻痒、
(e)粘膜炎、口内炎、***炎、ヘルペスまたは歯肉炎による唇のかゆみ、
(f)肛門掻痒、痔の不快感、裂肛による痛み、痔瘻による痛みまたは痒み、痔核切除による痛み、会陰炎症、肛門性器の皮膚の炎症、および、失禁、おむつかぶれ、会陰炎症など種々の局所的原因による不快感、
(g)(例えば、外陰部前庭炎または外陰部痛など、カンジダ症による、または、突発性の)外陰部掻痒および痛み、***疼痛、いぼおよび性病などの肛門性器感染症、真菌感染症、(特に免疫不全患者における)皮膚のウイルス性感染症、
(h)呼吸閉塞による鼻孔および鼻または上気道の不快感、例えば、充血、鼻炎、喘息、気管支炎、肺気腫および慢性閉塞性肺疾患、呼吸困難、睡眠時無呼吸およびいびき、ならびに、
(i)結膜炎、眼球表面の炎症、角膜の擦過傷による痛み、および、目の手術による痛み、である。
脳の対応調節システムが体温を低下させる必要性を感じると、紅潮(血管拡張)および発汗が身体に起きる。閉経後、少なくとも3分の1の女性が「ホットフラッシュ」(つまり、温かさおよび紅潮しているのを感じる、短時間だが反復的な症状の出現、および、昼間と夜間の発汗)を経験する。代替エストロゲンが症状を軽減し得るが、ホルモン補充療法(HRT)が安全かどうかは不確実である。夜および早朝の時間帯で起きる発汗発症は、ベッドシーツが濡れ、ベッドシーツを毎日または頻繁に交換することは負担になることから、特に不都合である。「ホットフラッシュ/寝汗」の発症は、週に平均14回の頻度で起きる可能性がある。HRTとは別として、ヨガ、鍼治療および植物性エストロゲンなどの現在の代替治療法は、有効であるとは示されていない。
神経障害痛のある患者は、通常は無害な皮膚冷却に誘発される痛みに頻繁に苦しみ、これは冷感異痛と呼ばれる状態である(例えば、Wasnerら(2008)を参照)。冷感異痛は痛みのある一部の糖尿病患者に見られるが、神経障害痛と体性痛を識別する簡易な診断ツールは不足している。皮膚に適用するDIPA-1-7などの薬は、そのような診断に有用であり、最適な治療方法を選択する際の手助けとなる。アルコールに溶けた40%メントール溶液がチャレンジ薬として使用されてきたが、診療所での結果は曖昧であった(例えば、Binderら(2011)を参照)。
軽度の手術前後の低体温(33〜36.4℃)および麻酔後振戦を有する外科患者は、創傷治癒の減少、出血の増加、および病気による心事故などの事象を含め、有害転帰のリスクがより大きい(例えば、Buggyら(2000)を参照)。研究は、メントールなどのTRPM8アゴニストは、冷たい感覚を生成することにより中心温度を上昇させることができることを示唆している(例えば、Tajnoら(2011)を参照)。DIPA-1-7などの薬は、冷たさに対する感度を向上させることにより、術後低体温に対する薬物治療として有用であり得る。ラットでは、DIPA-1-7の注入は、震え、体温の上昇、および、立ち直り反射の回復により測定される、ペントバルビタール麻酔の持続期間の短縮を誘発する。これらの薬理作用は体温に対する麻酔の抑制効果に対抗するだろう。
医薬品または薬用化粧品において、「補助剤」という用語は、主要物質、治療、もしくは処置の有効性または安全性を向上させるため、または、そのパフォーマンスを助けるために用いる、追加の物質、治療、もしくは処置である。DIPA化合物は皮膚の感覚的不快感を緩和し、抗侵害受容作用を有し、適用後1分未満で活性する。それらは皮膚に適用する医薬品および化粧品として理想的な補助剤である。
〔毒性〕
予備的な毒性研究をDIPA-1-7について行った。それは、エームズ試験において変異原性ではなかった(菌株TA98およびTA100、肝臓活性化あり、および、なし)(テストは、Apredica社(アメリカ合衆国、マサチューセッツ州、ウォータータウン)により実行)。
〔組織温度〕
本発明の化合物は、放熱の感覚を模倣するが、組織の温度は変化させない。対象(N=5)の平均の額皮膚温度を、DIPA-1-7を(蒸留水中20mg/mLの濃度で、ワイプを用いて)適用した後に測定した。結果は以下の表にまとめる。対象は皮膚におけるDIPA-1-7の冷却効果が30〜45分間続いたことを指摘したが、皮膚温度は影響を受けなかった。
〔顔の皮膚に対する化合物の感覚的効果〕
テスト化合物を皮膚に適用する際、得られる感覚を特徴づけすることが可能である。個々の化合物によって生成された感覚の質はある特定の特性を支持し、該特性は別個である。引き起こされる感覚の質、それらの記述子、作用の提案メカニズムを以下の表にまとめる。任意の化合物について、作用が一部重複するかもしれないが、通常、1つの化合物は1つまたは2つのみの感覚カテゴリーを占める。例えば、イシリンは非常に冷たく、「冷たい」はほとんどない。DIPA-1-6およびDIPA-1-7は快適で強い「動的な涼しさ」の生成において例外的である。DIPA-1-8、2-6、および2-7は強い冷たさを生成する薬である。
〔TRPM8における化合物のアゴニスト活性〕

〔摘出迷走神経についての研究:直接の抗侵害受容作用〕
DIPA-1-7が直接感覚神経に作用するかどうか明らかにするため、Imperial College(イギリス国、ロンドン)で開発された摘出神経モデルでテストした(例えば、Birrellら(2009)、Patelら(2003)を参照)。このインビトロアッセイにおいて、マウスの迷走神経のセグメントをプラットフォームに置き、電気的活動をカプサイシンの局所適用後に記録する。カプサイシンは皮膚に置いた場合痛みを引き出す既知の刺激物であり、摘出迷走神経を脱分極する。このカプサイシンが誘発する脱分極を阻止する物質の能力を測定した。
〔実験動物における生物活性〕
毛皮に包まれた動物および羽の生えた動物は、濡れて寒い場合、濡れたイヌのように震える。(例えば、Dickersonら(2012)、Ortega-Jimenezら(2012)、Wei(1981)を参照)。これらの震えは脊髄軸まわりの回外筋および回内筋の素早い変動収縮であり、容易に観察および集計することが可能である。「身震い」は、動物において詳細に研究され、この行動は生存価を有すると解釈される。これは、震えることは、水をその皮膚から除去することにより、水分を除去するために蒸発エネルギーを消費する必要性を小さくするからである。したがって、震えの引き金となる感覚は、毛包または羽の間に閉じこめられた水があることである。人間は皮膚に体毛がほとんどないため震えない。人間における、震えとおそらく同等の行動は、振戦、つまり、涼しい/冷たいおよび水分の一般化された感覚により引き起こされる状態である。
〔頭蓋骨の局所箇所に対する影響〕
DIPA-1-7、つまり、動的な冷却に関し最も強力な化合物を頭蓋骨の他の局所箇所でテストした。20mg/mL溶液を、コットンワイプを用いてバッカル頬、耳下腺−咬筋頬およびこめかみの上の皮膚、ならびに、耳周囲領域および下顎後方部の上の皮膚に、適切な頭蓋計測点(それぞれ、プテリオン、コロニオン、コンジリオン、ゴニオン)を目印として使用して適用した。驚くべきことに、これらの部位のうちバッカル頬以外の全てにおいて、観察された冷却はあるとしてもほんのわずかだった。軽い冷却が約30分間バッカル頬で観察されたが、この効果は溶液が眼窩下神経の受容野に広がったためとみられる。したがって、眼窩および頬骨/額の皮膚への作用は選択的であり、頭の皮膚の重要な送達標的を識別する。
事例研究を以下に記載する。これは、DIPA-1-7を、(a)認知を向上し、精神的疲労および疲労を低減し、パフォーマンスを活性化するため、(b)慢性疾患からくる倦怠感および疲労を和らげるため、(c)熱ストレスからくる疲労および/または不快感を和らげるため、(d)皮膚のかゆみおよび痛みを和らげるため、および、(e)「寝汗」の重症度を低減するために使用することを説明する。
65歳の男性は熱心なスヌーカー選手で、ロンドンと香港のスヌーカーパーラーに足繁く通うのが好きである。彼は友人と少額の賭けをするが、高齢になるにつれ彼の腕前は落ち、1日に約8フレームしかプレイできない。彼は、ゲーム中の自分の助けとなるよう顔に氷のように冷たいタオルと度付きメガネを使用する。しかし、それは集中力を欠き、ゲームを妨げ、彼が「ブレイク」(「ラン」におけるポイントの連続集積)を完了できないようにする一連のショット計画であると感じる。彼はDIPA-1-7を含んだワイプを試すことを申し出た。彼のゲームには驚くべき変化があった。彼はショットからショットの間より早く動き、計画および実行はきびきびしていた。セッション毎のフレーム数はプレイの頻度と同様に増加した。彼は80ポイントというキャリア中最高のポイントを得て、有頂天だった。彼はスヌーカーゲームの補助としてワイプを使用することを継続している。彼はまた、認知機能の向上が、氷のように冷たいタオルを顔に適用することで復活し、活性化されると述べた(「貯蔵器効果」の例)。しかしながら、DIPA-1-7は時として、その使用が頻繁過ぎる場合は特に、炎症を引き起こすため、DIPA-1-7が眼球表面に過度に入るのを避けることが重要であると述べた。訓練により、彼はゲームの認知向上は送達工程を最適化することにより調節および制御可能であると述べた。
48歳の女性アカウント・エグゼクティブは大手金融機関で働く忙しい専門家である。彼女の夫は成功した建築家だった。彼女には2人のティーンエージャーの子どもがおり、家事をする時間が絶えず足りなかった。一日の終わりに、彼女はしばしば身体的に、精神的に疲れ果て、夕食後の早い時間に眠りについていた。最近の結婚生活の問題から、彼女はほとんど常に倦怠感と疲れを感じ、服装および礼儀において、家庭内および仕事での態度が悪くなり始めた。彼女は慢性的な身体的疾患を患っていたが、数回のインタビュー後、簡易疲労一覧表(BFI)で「中程度の疲労」と評価され、医師により「鬱状態」と判断された。彼女はDIPA-1-7を含んだワイプを使用することを申し出て、1日に2回以上使用しないように指示を受けた。使用して2日後、彼女は、ワイプは気分と外部の出来事に対する興味を向上させたと報告した。彼女はより活発で前向きだった。彼女は職場で与えられた仕事をすぐに完了し、より多くのスタミナがあった。彼女はより闘争的で自己主張が強かった。彼女に一番近い人々、つまり子どもと職場の同僚も彼女の態度と人柄の良い変化を口にした。彼女は必要に応じてワイプを使用することを続けている。
他の一連の研究では、コットンワイプの代わりにタオレットを使用した。タオレットは、ラップ(重さ1.1g)、不織レース(重さ3.4〜3.5g)でできた23cm×26cmのタオル、および、液体組成物(14〜15mL)から成り、これは包紙に自動的に加えられ、封をされる。タオレットを生成するための自動化機械は当業者によく知られている。本明細書では、タオレットはKank Factor社、サンフランシスコ(カリフォルニア州94108、サンフランシスコ、コマーシャル・ストリート721、www.3LWipes.com)が製造した。(プラシーボ対照としての)蒸留水または(1〜5mg/mLの濃度で)蒸留水に溶解したDIPA-1-7をタオレットに組み込んだ。1回の自己適用当たりの量は、適用部位に左右されるが、顔および額には約0.3mL〜0.5mLだった。ただし、胴を拭くことも含む場合はより多くすることも可能だった。
研究室で働く2人の科学者の手には、洗剤および石鹸に反応するアレルギー性皮膚炎があった。手は炎症し、極度にかゆみがあった。20mg/mLのDIPA-1-7を綿棒で適用するとかゆみは即時に止まり、この効果は少なくとも2時間は続き、抑制は繰り返し適用することにより復活させることが可能だった。1人の科学者はかゆみについて多くの刊行物がある有名な皮膚科医だったが、該科学者は、DIPA-1-7は炎症した皮膚に「氷のように冷たい」感覚を生成し、かゆみをこれほど早く止めるのにこれほど効果的な化合物には出会ったことがないと言った。
66歳の女性は、ホットフラッシュ/寝汗が時折、2週間に約1回起きていた。彼女はホルモン補充療法(HRT)(1日に1回、エストラジオール1mgおよびメドロキシプロゲステロン2.5g)を受けていたが、2人の友人が乳がんに、1人が子宮がんになった後、HRTを止めることを決めた。寝汗の発生は2日に約1回へ増加し、彼女と彼女の夫はベッドシーツを頻繁に交換する必要があるため、いらいらしていた。彼女はDIPA-1-6を1%含んだローションを試すことを承諾した。このローションは夜眠りに就く前に彼女の首元および胸の中央の皮膚に適用した。夜目が覚めた場合は、適用を繰り返した。彼女は、ローションは冷たく感じるが不快ではないと言った。寝汗の発生は3週間観察されなかった。彼女は医師とさらに協議してHRTに戻るよう説得され、彼女は最低でもこの9ヶ月間、寝汗は経験していない。
3人の対象は、DIPA-1-6、DIPA-1-7、DIPA-1-8、およびDIPA-1-9を、それらの眼球表面に対する感覚的効果について全身的に比較することを決めた。核化合物を蒸留水中1mg/mLで作製した。55〜75mgの綿の玉を3インチのポリスチレン棒に巻いた特異的なサイズの綿棒(Puritan 803-PCL)を溶液に浸した。先端をその後、瞼を閉じたまま、上まぶたの下側面のまつ毛に、外側から内側に2回塗る動作をすることで適用した。対象はその後、まばたきするよう指示された。まばたきすることにより、溶液はその後、前角膜に均等に分散する。この「綿棒」送達方法は両目の表面に合計〜36μLの液体を落とした。DIPA-1-6は著しい刺痛と不快感を引き起こしたため、さらなる研究はしなかった。DIPA-1-7およびDIPA-1-8は強く爽快な冷却を生成し、これは目の炎症を和らげ認知機能を向上した。例えば、対象は遠くの目的物に焦点を合わせ、眺めを楽しむことが可能だと感じた。彼らは精神的に冴え、爽快だった。しかし、DIPA-1-7およびDIPA-1-8の両方で瞼にわずかな残留物が残った。顔を洗うためにタオルをその後使用することで目の炎症を引き起こし得る。驚くべきことに、DIPA-1-9は瞼の上に塗った際、目の炎症を生成せず、残留物も残さなかった。それも爽快な冷却を生成したが、DIPA-1-7またはDIPA-1-8と同じ強度ではなかった。一方、DIPA-1-9は眼球の不快感、例えば、眼精疲労により引き起こされる不快感、目の疲労、目の手術、目の表面と相互作用する浮遊刺激物または汚染物質、コンタクトレンズの長時間の装着、太陽に過度にさらされること、結膜炎、または、ドライアイ症候群の治療に理想的な性質を有する。
2歳のメスのウエスト・ハイランド・テリアが、夏の間かゆい状態を発症し、耳および下腹部を継続してひっかくようになっていた、獣医はその症状をイヌアトピーと診断し、経口抗ヒスタミン薬を処方した。この薬物はかゆみの進行を制御せず、毛が抜け、生皮の斑点が尾の付け根および後肢に現れた。アトピー性抗炎症ステロイド、トリアムシノロンは成功が少しの成功を提供し、イヌはまだつらそうだった。驚くべきことに、DIPA-1-7クリーム(1%wt/vol)を炎症した皮膚部位に適用すると即時にひっかきが減少し、皮膚部位は治り始めている。イヌの行動からかゆみは軽症化していることは明らかだった。さらに、イヌの屋外へのアクセスの削減と、ノミおよびイエダニへの暴露の可能性の制御が、イヌの皮膚の不調の制御の成功をもたらした。

- 請求項1〜5のいずれか一項に記載の化合物と、医薬的に受容可能な担体または希釈剤を含む組成物。
- 前記化合物を0.005〜2.0%wt/volの濃度で含む、請求項6に記載の組成物。
- 液体組成物であり、前記化合物を0.5〜20mg/mLの濃度で含む、請求項6に記載の組成物。
- 液体組成物であり、前記化合物を1〜5mg/mLの濃度で含む、請求項6に記載の組成物。
- 液体組成物であり、前記化合物を5〜10mg/mLの濃度で含む、請求項6に記載の組成物。
- 液体組成物であり、前記化合物を10〜20mg/mLの濃度で含む、請求項6に記載の組成物。
- 請求項1〜5のいずれか一項に記載の化合物と、医薬的に受容可能な担体または希釈剤とを混ぜるステップを含む、組成物の作製方法。
- 請求項1〜5のいずれか一項に記載の化合物、または、請求項6〜12のいずれか一項に記載の組成物を運ぶ綿棒、ワイプ、パッド、またはタオレット。
- 請求項1〜5のいずれか一項に記載の化合物、または、請求項6〜12のいずれか一項に記載の組成物を運ぶ、皮膚への適用に適切な、放出制御当て布。
- 請求項1〜5のいずれか一項に記載の化合物、または、請求項6〜12のいずれか一項に記載の組成物を含んだエアロゾル化した噴霧を送達するための加圧容器。
- 請求項1〜5のいずれか一項に記載の化合物、または、請求項6〜11のいずれか一項に記載の組成物を含んだ、貯蔵器につなげた手動で作動させる噴霧器。
- 療法による人間または動物の身体の治療法で用いる、請求項1〜5のいずれか一項に記載の化合物。
- 感覚的不快感、皮膚の異常感覚、皮膚炎、乾癬、眼球の不快感、熱不快感、熱ストレス、閉経後の女性における紅潮および/または寝汗(血管運動性症状)、術後の低体温、麻酔後振戦、疲労、倦怠感、うつ病、もしくは、認知機能障害の治療、または、認知機能を向上するための治療で使用する、請求項1〜5のいずれか一項に記載の化合物。
- 感覚的不快感の治療法で使用する、請求項18に記載の用途の化合物。
- 前記感覚的不快感とは、炎症、かゆみ、または痛みである、請求項19に記載の用途の化合物。
- 前記感覚的不快感は、皮膚の異常感覚により引き起こされる、請求項19に記載の用途の化合物。
- 前記感覚的不快感は、皮膚炎症、かゆみのある皮膚、または、痛みのある皮膚により引き起こされる、請求項19に記載の用途の化合物。
- 前記感覚的不快感は皮膚炎により引き起こされる、請求項19に記載の用途の化合物。
- 前記感覚的不快感は、アトピー性皮膚炎により引き起こされる、請求項19に記載の用途の化合物。
- 前記感覚的不快感は、イヌのアトピー性皮膚炎により引き起こされる、請求項19に記載の用途の化合物。
- 前記感覚的不快感は、乾癬により引き起こされる、請求項19に記載の用途の化合物。
- 皮膚の異常感覚の治療法で使用される、請求項18に記載の用途の化合物。
- 前記皮膚の異常感覚は、膚炎症、かゆみのある皮膚、または、痛みのある皮膚である、請求項27に記載の用途の化合物。
- 皮膚炎の治療法で使用される、請求項18に記載の用途の化合物。
- 前記皮膚炎はアトピー性皮膚炎である、請求項29に記載の用途の化合物。
- 前記皮膚炎はイヌのアトピー性皮膚炎である、請求項29に記載の用途の化合物。
- 乾癬の治療法に使用される、請求項18に記載の用途の化合物。
- 眼球の不快感の治療法に使用される、請求項18に記載の用途の化合物。
- 前記眼球の不快感は、眼精疲労、目の疲労、目の手術、眼球表面と相互作用する浮遊刺激物または汚染物質、コンタクトレンズの長時間の装着、太陽に過度にさらされること、結膜炎、または、ドライアイ症候群により引き起こされる、請求項33に記載の用途の化合物。
- 熱不快感の治療法に使用される、請求項18に記載の用途の化合物。
- 熱不快感の治療は、運動パフォーマンスを向上させる目的のためである、請求項35に記載の用途の化合物。
- 熱ストレスの治療法で使用される、請求項18に記載の用途の化合物。
- 閉経後の女性における紅潮および/または寝汗(血管運動性症状)の治療法で使用される、請求項18に記載の用途の化合物。
- 術後の低体温または麻酔後振戦の治療法で使用される、請求項18に記載の用途の化合物。
- 疲労、倦怠感、または、うつ病の治療法で使用される、請求項18に記載の用途の化合物。
- 疲労の治療法で使用される、請求項18に記載の用途の化合物。
- 前記疲労は、慢性疾患、廊下、神経機能障害、または、精神機能障害により引き起こされる、請求項41に記載の用途の化合物。
- 前記疲労は、がんまたはがんに関連した治療により引き起こされる、請求項41に記載の用途の化合物。
- 前記疲労は、不安、うつ病、熱ストレス、認知機能障害、過度の身体運動、または、過度の知的労働により引き起こされる疲労である、請求項41に記載の用途の化合物。
- 前記疲労は、考える、集中する、研究する、または労働を行う能力の低下に関連する疲労である、請求項41に記載の用途の化合物。
- 認知機能障害の治療法で使用される、請求項18に記載の用途の化合物。
- 認知機能を向上するための治療で使用される、請求項18に記載の用途の化合物。
- 前記認知機能の向上は、スポーツでの手と目の連携の向上である、請求項47に記載の用途の化合物。
- 前記認知機能の向上は、運または知的技能がものを言うゲームにおけるパフォーマンスの向上である、請求項47に記載の用途の化合物。
- 前記治療は局所適用による治療である、請求項17〜49のいずれか一項に記載の用途の化合物。
- 前記治療は、皮膚への局所適用による治療である、請求項17〜49のいずれか一項に記載の用途の化合物。
- 感覚的不快感、皮膚の異常感覚、皮膚炎、乾癬、眼球の不快感、熱不快感、熱ストレス、閉経後の女性における紅潮および/または寝汗(血管運動性症状)、術後の低体温、麻酔後振戦、疲労、倦怠感、うつ病、もしくは、認知機能障害の治療、または、認知機能を向上するための治療向けの薬剤の製造における、請求項1〜5のいずれか一項に記載の化合物。
- 感覚的不快感、皮膚の異常感覚、皮膚炎、乾癬、眼球の不快感、熱不快感、熱ストレス、閉経後の女性における紅潮および/または寝汗(血管運動性症状)、術後の低体温、麻酔後振戦、疲労、倦怠感、うつ病、もしくは、認知機能障害の治療、または、認知機能を向上するための治療方法であって、治療の必要な患者に、請求項1〜5のいずれか一項に記載の化合物を治療効果のある量で投与することを含む、治療方法。
