Description
- Es ist viele Jahre hindurch postuliert worden, daß die Herzvorhöfe als Sensoren dienen, die für den Nachweis von Änderungen im extrazellulären Flüssigkeitsvolumen wichtig sind (Gauer et al., Physiol.Rev. 43: 423, 1963). Eine solche Rezeptorfunktion für die Herzvorhöfe ist im Falle des Vasopressins bekannt, dem hypothalamischen Hormon, das wichtig ist für die Regulierung der osmotischen Konzentration der Körperflüssigkeiten.
- Die postulierte Existenz einer Substanz, welche die Natriumausscheidung im Harn steigert und daher an der Regulierung des extrazellulären Flüssigkeitsvolumens beteiligt ist, wurde kürzlich bewiesen. De Bold et al., Life Sci. 28: 89, 1981, injizierten einen teilweise gereinigten Extrakt von Herzvorhöfen von Ratten in andere anästhetisierte Ratten und beobachteten eine starke Steigerung des Harnflußes und der Natriumausscheidung im Harn. Dieser relativ rohe Extrakt besitzt die geeigneten Eigenschaften für eine endogene natriuretische Substanz.
- Zusätzlich zu den starken diuretischen und natriuretischen Wirkungen, Eigenschaften, welche die Stoffe besonders dafür geeignet machen, eine Hauptwirkung auf die Regulierung des Volumens der Körperflüssigkeiten auszuüben, wurde auch gefunden, daß diese Extrakte von Herzvorhöfen eine starke die glatte Muskulatur relaxierende Wirkung aufweisen (Currie et al., Science 221: 71, 1983). Eine solche Wirkung läßt auf eine potentielle direkte Rolle bei der Regulierung des Blutdrucks und auch auf eine Rolle bei der Regulierung des extrazellulären Flüssigkeitsvolumens schließen.
- Wegen der sofort erkannten Bedeutung dieser Entdeckung für das Verständnis der Regulierung des Volumens der Körperflüssigkeiten und des Blutdrucks und wegen des offensichtlichen therapeutischen Potentials eines solchen Naturstoffs bei der Behandlung von dekompensierter Herzinsuffizienz und von Hypertonie, nahmen zahlreiche Laboratorien die Isolierung, Charakterisierung und chemische Identifizierung der Wirksubstanz(en) in Extrakten aus Herzvorhöfen in Angriff. Die Wirksubstanz(en) in den Herzvorhöfen wurde(n) atrialnatriuretischer Faktor oder ANF genannt, sie wurden aber auch als Cardionatrin (de Bold et al., Life Sci. 33: 297-302, 1983) und Atriopeptin (Currie et al., Science 111: 67, 1984) bezeichnet. Es wurde gezeigt, daß der atrialnatriuretische Faktor eine Familie von Peptiden ist, die alle eine gemeinsame Aminosäurensequenz aufweisen, aber sich in der Länge durch das Vorhandensein oder Fehlen von 1-8 Aminosäuren an dem Amino- oder Carboxylterminus unterscheiden.
- Die Peptidchemiker stellten schnell vollständig synthetische Stoffe her, welche die biologische Wirkung der Familie von Peptiden, welche aus den Herzvorhöfen isoliert worden waren, nachahmten.
- Die biologische Wirkung von ANF weist auf eine Verwendbarkeit bei dekompensierter Herzinsuffizienz hin, bei welcher die Standardtherapie starke Diuretica in Kombination mit peripher vasodilatorischen Arzneimitteln verwendet. Der atrialnatriuretische Faktor vereinigt beide dieser Wirkungen in einem Molekül, welches auf natürliche Weise im Körper hergestellt wird. Es ist möglich, daß das Zurückhalten von Salz und Wasser, mit dem eine dekompensierte Herzinsuffizienz verbunden ist, ein Resultat einer unzureichenden Produktion von ANF ist. Wenn das stimmt, würde die Verabreichung von ANF den Ersatz von adäquaten Mengen der Stoffe ermöglichen.
- Eine zweite wichtige Krankheit, bei welcher die biologische Aktivität von ANF auf eine Verwendbarkeit hinweist, ist die essentielle Hypertonie. Die Standardtherapie für Hypertonie verwendet Diuretica und peripher vasodilatorische Arzneimittel. Der. atrialnatriuretische Fakt6r vereinigt beide dieser Charakteristika. Eine spezielle Verwendung kann auch bei der akuten Behandlung der hypertensiven Krise, wie bei maligner Hypertension, gefunden werden, bei welcher die starke vasodilatorische Wirkung von ANF überragend sein würde.
- Zusätzlich zu diesen beiden sehr umfangreichen Kategorien der therapeutischen Verwendbarkeit ist es möglich, daß es aufgrund der günstigen Wirkung von ANF auf die renale Hämodynamik, insbesondere auf die Steigerung des medulären Blutflusses, bei solchen Krankheiten, die duch eine Verringerung der Nierenfunktion gekennzeichnet sind, von Vorteil sein kann.
- US-A-4 609 725 von Brady et al. zeigt, daß synthetische Fragmente von ANF am Carboxylterminus in dem Tyrosinrest durch elektrophiles Markieren mit Iod markiert werden können, und daß diese radioaktiv markierten Derivate für die Untersuchung des in vivo- und des in vitro-Metabolismus von ANF verwendbar sind.
- Radioaktive Indiumkomplexe mit Chelat-Systemen vom EDTA-Typ werden von Meares und Wensel in Acc. Chem. Res. 1984, 17, 202-209 beschrieben. EHPG- und HBED-Chelate von Gallium und Indium werden von F.C. Hunt in Nucl. Med. Biol., 1988, 15, 659- 664, als potentielle Radiopharmazeutika vorgeschlagen. In keiner dieser Veröffentlichungen werden jedoch die Modifikationen vom ANF-Typ der vorgeschlagenen Komplexe entweder beschrieben oder vorgeschlagen.
- Es ist das Ziel der vorliegenden Erfindung, radioaktive Derivate von synthetischen Fragmenten von ANF zur Verfügung zu stellen, in denen das radioaktive Atom an den N-Terminus des Peptids mittels eines Chelat-Moleküls gebunden ist.
- Es ist ein anderes Ziel der vorliegenden Erfindung, einen leichteren Zugang zu radioaktiv markierten Formen von ANF zur Verfügung zu stellen, als sie gegenwärtig mittels elektrophiler Markierung mit Iod verfügbar sind, da die Chelatchemie die Synthese der radioaktiven Form von ANF durch bloßes Mischen der chelatisierten Form von ANF mit einem radioaktiven Metall ermöglicht.
- Ein weiteres Ziel der Erfindung ist es, eine Verbindung zur Verfügung zu stellen, die für die Untersuchung des Metabolismus und der Halbwertszeit von ANF in vivo und in vitro brauchbar ist.
- Diese und andere Ziele der Erfindung werden aus der folgenden Beschreibung ersichtlich sein.
- Radioaktiv markierte Formen von ANF können hergestellt werden, indem ein synthetisches Derivat vom atrialnatriuretischen Faktor eines Säugetiers, das am N-Terminus eine Chelat- Funktionalität aufweist, mit einem geeigneten metallischen Radioisotop wie Tc-99m, Ga-67 oder In-111 verbunden wird.
- Gemäß der vorliegenden Erfindung weist die Chelatform von ANF die folgende Aminosäurensequenz auf:

- worin X Ile oder Met bedeutet,

- und worin Ch ein Chelat bedeutet, ausgewählt aus der Gruppe, die aus besteht, worin R&sub3; bedeutet,
- und worin n 2 bis 6 ist und das Peptid linear oder mittels covalenter Bindungen zwischen den beiden Cysteinresten cyclisiert ist, und R&sub1;R&sub2;, die gleich oder verschieden sein können, aus Wasserstoff, C&sub1;-C&sub7; Alkyl, C&sub1;-C&sub7; Alkoxy, C&sub3;-C&sub7; Cycloalkyl, Amino, Halogen, d.h. Chlor, Brom, Fluor und Iod, Trifluormethyl und dergleichen ausgewählt sind.
- Bevorzugt ist X Ile oder Met.
- Wenn X im obigen Ile bedeutet, A ein Chelat, wie es definiert wurde, z.B. HBED-Arg-Arg-Ser-Ser und B nicht vorhanden ist, ist es ein bevorzugtes Chelat,und HBED ist N,N'- Bis-[2-hydroxybenzyl]-ethylendiamin-N,N'-diessigsäure.
- Die ANF-Chelatform wird synthetisiert, indem zuerst das ANF-Derivat durch eine geeignete Ionenaustauscher-Säule durchlaufen gelassen wird, um es in die Tetrahydrochloridform zu überführen. Dies ist besonders wichtig, wenn ANF ursprünglich in der Tetraacetatform vorliegt. Die Tetrahydrochloridform wird dann mit einem aktiven Esterderivat des Chelatmoleküls in einer solchen Weise umgesetzt, daß die Reaktion am N-Terminus des Peptids vonstatten geht. Die erhaltene Mischung von ANF- Chelatform, von chelatfreiem ANF und von nicht umgesetztem aktiven Chelatester wird dann mittels präparativer HPLC gereinigt, um das reine ANF-Chelatderivat zu erhalten. Diese ANF-Chelatform wird dann in ionenfreiem Wasser mit einem metallischen Radioisotop, wie &sup6;&sup7;Ga, Galliumcitrat, ¹¹¹In, Indiumchlorid oder 99mTc-Technetiumionen im IV-oder V Stadium, zur Reaktion gebracht.
- In der Praxis werden diese modifizierten ANF-Chelatformen in einem Pufferträger gelöst, lyophilisiert und an den Endverbraucher in Einzeldosen mit 10 bis 300 Mikrogramm ANF pro Ampulle abgegeben. Der Verbraucher wird eine Lösung eines metallischen Radioisotops zu dem lyophilisierten Chelat zugeben, und nach einer Zeit von Minuten, während welcher sich der Metallchelatkomplex bildet, ist der markierte ANF-Komplex fertig zum Gebrauch.
- Radioisotopen, für welche diese Chelate geeignet sind, sind 99mTechnetium, &sup6;&sup7;Gallium, &sup6;&sup8;Gallium, &sup5;¹Chrom, &sup5;&sup7;Kobalt, &sup6;&sup0;Kobalt, ¹¹¹Indium, 113mIndium, ¹&sup8;&sup6;Rhenium, ¹&sup8;&sup8;Rhenium, &sup9;&sup0;Yttrium und dergleichen.
- Um die Praxis der vorliegenden Erfindung weiter zu erläutern, werden die nachfolgenden Beispiele aufgenommen.
- Ein humanes ANF-Fragment (Methionin in 10-Stellung) wurde mittels 300 MHz-NMR-Spektrometrie untersucht, um das Vorhandensein der Tetraacetatgruppen zu bestätigen. Diese Gruppen werden bei 1,94 in der Figur 1 gefunden. 10 mg dieser Probe wurden in 2 ml von ultrareinem Wasser (Widerstanswert > 18 MOhm) gelöst und durch eine kleine Kolonne durchlaufen gelassen, die 1 g Dowex AG3-X4 (Chloridform) enthielt. Das Eluat wurde gewonnen und die Kolonne weiter mit ultrareinem Wasser gewaschen. Das ANF-tetrahydrochlorid wurde aus dem Eluat duch Lyophilisierung erhalten. Ausbeute 9,6 g. Das NMR-Spektrum dieses Materials bestätigt die Abwesenheit von Acetatresten bei 1,9 bis 2,1 (Figur 2).
- 1,0 mg des Tetrahydrochlorids von humanem ANF (Methionin in 10-Stellung) wurde in 40 Mikrolitern eines 0,1M NaHCO&sub3;-Puffers bei pH 8,2 mit einer Lösung von N,N-Bis-[2-(2,6- dioxomorpholino)-ethyl]-glycin (160 Mikrogramm, 448 mMole, in 4 Mikrolitern Dimethylsulfoxid (DMSO)) zur Reaktion gebracht. Die Mischung wurde über Nacht stehen gelassen und dann mit CO&sub2;- Gas gesättigt. Von dieser Lösung wurde eine radiochromatographische Analyse (ITLC-Silicagel/mit CO&sub2; gesättigte physiologische Kochsalzlösung) mit ¹¹¹InCl&sub3; durchgeführt, die anzeigte, daß 82% der Radioaktivität an das ANF-Molekül gebunden waren.
- Diese Mischung des rohen ANF-Derivats wurde mittels präparativer HPLC-Chromatographie unter Verwendung einer Waters C8 Reverse-Phase-Säule und 81:19 Wasser/Acetonitril als Lösungsmittel gereinigt. Die Eluierungszeit des nicht chelatierten ANF unter Verwendung dieses Systems betrug 2,18 Minuten und die der Chelatform 3,28 Minuten. Die isolierte Ausbeute an gereinigtem Diethylentriaminpentaessigsäure-ANF betrug 319 Mikrogramm. Das 300-MHz-NMR-Spektrum dieser Substanz wird in der Figur 3 gezeigt.
- 25 Mikroliter von ultrareinem Wasser wurden zu 25 Mikroliter einer Stammlösung von ¹¹¹Indiumchlorid (Atomic Energy of Canade Limited) zugegeben. Füge 100 Mikroliter einer 0,01M Natriumcitrat-Pufferlösung vom pH 5 hinzu, gefolgt 10 Mikrolitern einer Lösung, die 16 Mikrogramm des Diethylentriaminpentaessigsäure-Derivats von ANF enthält. Die Reaktion erfolgt fast augenblicklich und die Analyse mittels Radiochromatographie über ITLC zeigte eine Markierungsausbeute von 83% an.
- 106 mg N,N'-Bis[2-hydroxybenzyl]-ethylendiamin-N,N'- diessigsäure (HBED) wurden in 1 ml Acetonitril gelöst und die Lösung wurde mit Trimethylamingas gesättigt. Unlösliches wurde mittels Filtration entfernt und das Gelöste bei 90ºC im Hochvakuum zur Trockene eingedampft. Der Rückstand wurde in 5 ml Acetonitril, das 205 mg Di-(succinimidyl)-carbonat enthielt, gelöst. Nach einer Stunde wurde Acetonitril im Vakuum entfernt. Der Rückstand wird schnell zwischen Wasser und Chloroform verteilt und die Chloroformschicht wird abgetrennt, über Natriumsulfat getrocknet und Chloroform im Vakuum enfernt, um den rohen Ester zu erhalten. ¹HMR bestätigt die Anwesenheit von 2 Succinimidylresten pro Molekül HBED. Da es sich um einen sehr reaktionsfähigen Ester handelt, wird er ohne weitere Reinigung verwendet.
- 0,38 g des obigen aktiven Esters wurden mit 1 mg des Tetrahydrochlorids von humanem atrialnatriuretischen Faktor in 110 Mikrolitern von trockenem Dimethylsulfoxid und 2,7 Mikrolitern von trockenem Pyridin zur Reaktion gebracht. Die Analyse mittels Dünnschichtchromatographie zeigte die Überführung von ANF (O-Phthalaldehyd positiv, UV negativ) zu einem neuen Derivat (O-Phthalaldehyd negativ, UV positiv). Nach 16 Stunden wurde die Lösung zur Trockene eingedampft und der Rückstand in Methanol aufgenommen, die unlösliche Proteinfraktion wurde isoliert, chromatographisch gereinigt und in Anteilen von 0,5 mg lyophilisiert.
- 2 mCi von ¹¹¹InCl&sub3; in 1 ml eines 0,1M Natriumcitratpuffers (pH 5,4) wurden zu einem Vial gegeben, das 0,5 mg von lyophilisiertem ANF enthielt. Nach 5 Minuten wurde die Lösung mittels ITLC analysiert und zeigte eine Markierungsausbeute von 90%.
