Description
- Die vorliegende Erfindung betrifft eine pharmazeutische Zusammensetzung zum Schutz von Gehirnzellen.
- Nachdem ein System der Erste Hilfe Behandlung Fortschritte gemacht hat, ist neuerdings die Todesrate bei cerebraler Infarzierung oder cerebraler Blutung vermindert. Diese verminderte Todesrate führt jedoch zu dem anderen Problem, daß cerebrale Infarzierung oder cerebrale Blutung zu Demenz als Folgeeffekt führt. Es ist bekannt, daß zerebrovaskulare Demenz durch Schädigung von Gehirnzellen verursacht wird, die während Ischämie durch cerebrale Infarzierung oder cerebrale Blutung stattfindet.
- Kürzlich wurde bestätigt, daß die Schädigung von Zellen (oder Degeneration von Gehirnzellen) nicht nur als direktes Ergebnis von Ischämie (ischämische Zelländerung) auftritt, sondern ebenso als verzögerter Effekt (verzögerter neuronaler Tod), der selbst nach Beenden der Blutung beobachtet wird. Kirino et al. haben in Brain Res. 239: 57-69 (1982) berichtet, daß wenn Meriones Unguiculatus kurzzeitig zu Ischämie am Vorhirn behandelt und der Blutfluß beendet wird, pyramidale Zellen geschädigt und in dem CA1 Bereich von Hippocampus nach einer gewissen Zeitperiode geschädigt und verloren werden. Das heißt, daß die kurze Ischämie den verzögerten neuronalen Schaden verursacht. Hippocampus ist der Bereich des Gehirns, in dem die intellektuelle Aktivität bezüglich Emotion und Gedächtnis geregelt wird. Entsprechend wird vermutet, daß Schädigung des Hippocampus ein Grund der Verursachung der Demenz ist.
- Es ist deshalb ein ernstes Anliegen, Demenz, die nach cerebraler Infarzierung und cerebraler Blutung beobachtet wird, zu verhindern oder zu behandeln.
- Diesbezüglich haben Kirino et al. überdies in Progress in Brain Research, Band. 63: 39-58 (1985) berichtet, daß Pentobarbital mit Zellmembran stabilisierender Wirkung einen Schutzeffekt für Gehirnzellen zeigt und wesentlich den erwähnten verzögerten neuronalen Schaden unterdrückt.
- Wie berichtet, ist Pentobarbital zur Überwindung verzögerter neuronaler Schädigung von Wert. Weil jedoch Pentobarbital nicht selektiv wirkt, und weil die Aktion des zentralen Nervensystems ebenfalls stark nachläßt, kann Pentobarbital in der Praxis nicht als Schutzmittel für Gehirnzellen verwendet werden.
- Erfindungsgemäß wurde nun eine Verbindung gefunden, die wirkungsvolle Schutzaktivität für Gehirnzellen aufweist. Erfindungsgemäß haben wir gefunden, daß ein Piperazinderivat der Formel (I) als aktive Komponente bei der Herstellung eines Medikaments zur Verminderung verzögerter neuronaler Schädigung oder der Gehirntodrate nach Ischämie verwendet werden kann, wobei in der folgenden Formel (I)
- R¹ Wasserstoff oder eine niedrige Alkylgruppe, R² Hydroxyl, eine Aralkyloxygruppe, eine niedere Alkoxygruppe mit 1-5 Kohlenstoffatomen oder eine niedere Alkenyloxygruppe mit 3-5 Kohlenstoffatomen, R³ Wasserstoff, eine Aralkyloxygruppe, eine niedrige Alkoxygruppe mit 1-5 Kohlenstoffatomen oder eine niedrige Alkenyloxygruppe mit 3-5 Kohlenstoffatomen, und R&sup4; Wasserstoff oder eine niedere Alkoxygruppe mit 1-5 Kohlenstoffatome sind.
- Von dem Piperazinderivat gemäß Formel (I) ist bereits bekannt, daß es als wirkungsvolles Mittel bei der Verbesserung der Gehirnzirkulation dient, wobei es eine Wirkung auf Erhöhung des Blutstromes bei peripheraler Vasodilation besitzt, insbesondere vertebrale Gefäßweitung insbesondere der vertebralen Arterie und einen Inhibierungseffekt der Plättchenaggregation (japanische Patentveröffentlichung Nr. 58(1983)-124776 und Nr. 59(1984)-101475).
- Bei Piperazinderivaten der Formel (I) sind Beispiele für die niedere Alkylgruppe R¹ Methyl, Ethyl, n-Propyl und Isopropyl. Beispiele für die niedere Alkenyloxygruppe R² und R&sub3; sind Propenyloxy, Isopropenyloxy und Allyloxy. Beispiele für Aralkyloxygruppe R² und R³ sind Benzyloxy, Phenethyloxy und p-Methoxybenzyloxy.
- Die Piperazinderivate der Formel (I) können in Form eines Salzes mit einer pharmazeutisch verträglichen Säure vorliegen. Solche Säuren können sein: Salzsäure, Schwefelsäure, Weinsäure, Fumarsäure, Maleinsäure, p-Toluolsulfonsäure und Methansulfonsäure.
- Die Piperazinderivate gemäß Formel (I) können beispielsweise hergestellt werden durch Reduktion einer Carbonylverbindung gemäß folgender Formel (II):
- (Hierbei haben R¹, R², R³ und R&sup4; die zuvor gegebene Bedeutung.)
- Representative Beispiele für Piperazinderivate der Formel (I), die als die aktiven Komponenten in der pharmazeutischen Zusammensetzung gemäß Erfindung dienen, sind beispielsweise die folgenden Verbindungen.

- Andere wertvolle Verbindungen gemäß Formel (I) können gemäß der erwähnten japanischen Patentveröffentlichungen Nr. 58(1983)- 124776 und Nr. 59(1984)-101475 hergestellt werden.
- Die pharmakologische Wirkung der Verminderung der verzögerten neuronalen Schädigung oder des Todes, die durch die aktive Komponente gemäß Formel (I) erzielt wird, wird durch die folgenden Ergebnisse pharmakologischer Experimente belegt, bei denen die Testverbindung in ein größeres Kreislaufsystem eingegeben wird. Ergebnisse der Giftigkeitstests werden ebenfalls wiedergegeben.
- Ein männliches Meriones Unguiculatus (ungefähr 12 bis 16 Wochen alt) wurde anesthisiert. Dessen bilaterale gemeinsamen Carotidarterien im Nacken wurden freigelegt und mit Klammern 5 Min. verschlossen, um im Vorhirn Ischämie zu verursachen. In dem Moment, in dem der Blutfluß wiederhergestellt wurde, wurde 1- (3,4-Dimethoxyphenyl)-2-(4-diphenylmethylpiperazinyl)ethanoldihydrochlorid (im folgenden als Medikament A bezeichnet) oder 0,2% Methylzellulose (das als Träger für obige Testverbindung verwendet wurde), intraperitoneal mit einer Dosis von 30 mg/kg (n = 10) jedeweils verabreicht.
- Nach 7 Tagen wurde das behandelte Tier durch Perfusionfixierung fixiert, die durch Einführung von etwa 300 ml 10%iger wäßriger Formalinlösung in das linke Ventrikel bei 120 cm·H&sub2;O durchgeführt wurde, und enthauptet. Nach 1stündigem Belassen des enthaupteten Teils bei Zimmertemperatur wurde das Gehirn herausgenommen und in die gleiche Lösung 48 Std. getaucht. Nach sezieren der Gehirne und koronares Zerschneiden in kleine Blocks wurden diese entwässert und in üblicher Weise in Parafin eingebettet. Von den Blocks wurden koronare Schnitte 5 um Dicke senkrecht zur Längsachse des Gehirns bei 2 mm hinter dem Bregma genommen. Dann wurden sie nach der Nissle Färbungsmethode gefärbt.
- Gleiche Schnitte wurden ebenfalls aus einer normalen Gruppe aus 6 Tieren genommen, die nicht zur Verursachung von Ischämie behandelt wurden.
- Die Schnitte wurden mittels optischem Mikroskop betrachtet, um die Anzahl gutgeformter pyramidaler Zellen (beispielsweise Neuronen) im CA1 Bereich der Spezien zu messen; photographisch wurde die Länge der pyramidalen Zellschicht in dem Bereich gemessen. Dann wurde die Anzahl pyramidaler Zellen je 1 mm berechnet und untersucht.
- Mikroskopische Beobachtung bei niedriger Vergrößerung zeigte, daß die Färbung der pyramidalen Zellen des CA1 Bereichs gleich der des anderen Bereichs in der normalen Gruppe war. Mikroskopische Beobachtung bei hoher Vergrößerung zeigte, daß die Zellformen gut erhalten waren. In der Gruppe, der 30 mg/kg Medikament A verabreicht worden war, wurde fast das gleiche histologische Bild erhalten wie bei der normalen Gruppe. Beide mikroskopischen Beobachtungen bei niederer und hoher Vergrößerung ergaben das gleiche Ergebnis wie in der normalen Gruppe, das heißt, daß die Zellen am Leben blieben.
- In der mit dem Träger behandelten Vergleichsgruppe änderten sich die Zellen zu irreversibler Degeneration, nämlich Fusionegrozytosis. Die Analyse der Anzahl Zellen in 1 mm pyrimidaler Zellschicht im CA1 Bereich jeder Gruppe zeigte, daß der Ausfall von Zellen in der Gruppe begrenzt war, der 30 mg/kg Komponente A verabreicht wurde, und zwar im Vergleich mit denen, denen der Träger verabreicht wurde. Dieses zeigt offensichtlich die Wirksamkeit des Medikaments an (Tabelle 1). Zusätzlich wurde erkannt, daß die Anzahl Zellen bei der mit Träger behandelten Gruppe im Vergleich mit der Normalvergleichsgruppe zur Abnahme tendierte. Es wurde jedoch kein statistisch relevanter Unterschied der Anzahl Zellen zwischen diesen Gruppen festgestellt; und Zellschädigungen waren vermindert. - Der ddN männlichen Maus mit einem Gewicht von etwa 20 g wurde oral die erfindungsgemäße aktive Komponente verabreicht; sie wurde 7 Tage beobachtet. Die Ergebnisse werden in Tabelle 2 wiedergegeben. Tabelle 2 Aktive Komponente LD&sub5;&sub0; (mg/kg)

- Die obigen pharmakologischen Versuche zeigen, daß Piperazinderivate der Formel (I) eine Schutzfunktion für Gehirnzellen aufweisen, die gleich der von Pentobarbital ist, und daß die Derivate sehr sichere Medikamente sind.
- Darüberhinaus zeigen Piperazinderivate der Formel (I) im wesentlichen keine anesthetische Wirkung, solange sie in normalen Dosen verabreicht werden. Entsprechend sind Piperazinderivate der Formel (I) derart wirksam, daß sie wesentlich verzögerte neuronale Schädigung vermindern, die manchmal durch cerebrale Infarktion oder cerebrale Blutungen, insbesondere bei älteren Patienten, verursacht wird. Demnach sind Piperazinderivate der Formel (I) wirksame Mittel zur Verhinderung der Demenz.
- Darüberhinaus können Piperazinderivate der Formel (I) eingesetzt werden, um Folgeeffekten von vorübergehender Ischämie vorzubeugen, die verursacht werden können durch zeitweilige Blutdruckverminderung durch Ersticken oder Ertrinken, Unfälle bei anesthetischer Behandlung, externe Wunden, Medikamente oder Drogen u.ä..
- Normalerweise ist bekannt, daß ein Mittel zur Behandlung des Gehirns die Blut-Gehirnbarriere (BBB) passieren sollte. Die Piperazinderivate der Formel (I) passieren leicht BBB, weil sie durch die Verabreichung in das größere Kreislaufsystem wirksam sind.
- Entsprechend können die aktiven Komponenten der Erfindung, nämlich Piperazinderivate der Formel (I), auf normalen Wegen, wie durch orale Verabreichung oder parenterale Verabreichung mittels Suppositorium oder Injektion verabreicht werden.
- Beispiele der Herstellungsformen für orale Verabreichung sind Tabletten, Kapseln, Pulver, Granulat und Sirup. Beispiele für die Preparationen für parenterale Verabreichung sind Suppositorien und Injektion. Bei der Formulierung dieser Preparationen können Exzipientien, desintegrierende Mittel, Bindemittel, Gleitmittel, Pigmente und Verdünnungsmittel verwendet werden, die auf diesem Gebiet üblich sind. Zu beispielhaften Exzipientien zählen Glukose, Laktose und mikrokristalline Zellulose. Beispiele für desintegrierende Mittel sind Stärke und Carboxymethylzellulosecalzium. Zu Beispielen für Gleitmittel zählt Magnesiumstearat und Talg. Beispielhafte Bindemittel sind Hydroxypropylzellulose, Gelatin und Polyvinylpyrrolidon.
- Die Injektionsdosis liegt normalerweise im Bereich von 0,1 mg bis 100 mg/Tag, vorzugsweise 0,5 mg bis 100 mg/Tag für Erwachsene. Die Dosis zur oralen Verabreichung liegt normalerweise im Bereich von 1 mg bis 1000 mg/Tag, vorzugsweise 5 mg bis 1000 mg/Tag bei Erwachsenen. Diese Werte sind ausgedrückt in Form der Mengen der physiologisch aktiven Verbindung, nämlich des Piperazinderivats der Formel (I). Diese Dosen können in Abhängigkeit vom Alter und Zustand der Patienten erhöht oder erniedrigt werden.
- Die Erfindung wird anhand der folgenden Beispiele näher erläutert.
- In einer Mischung von 100 ml Ethanol und 30 ml Chloroform wurden 13,8 g (30 mmol) 2-(4-Diphenylmethylpiperazinyl)-1-(2,- 3,4-trimethoxyphenyl)ethanon gelöst; der gebildeten Lösung wurde unter Kühlung mit Eis 2,27 g (60 mmol) Natriumborhydrid über einen Zeitraum von 20 min. zugegeben. Die Lösung wurde bei Zimmertemperatur 2 Std. gerührt und dann mit 50 ml gesättigter wäßriger Ammoniumchloridlösung und 100 ml Wasser versetzt. Die erhaltene Lösung wurde mit 200 ml Ethylacetat extrahiert. Die organische Lösung wurde zunächst mit Wasser und dann mit gesättigter Salzlösung gewaschen und über wasserfreiem Natriumsulfat getrocknet. Das Lösungsmittel wurde abdestilliert; der Rückstand wurde aus Chloroform/Ethanol umkristallisiert und ergab 9,70 g 2-(4-Diphenylmethylpiperazinyl)-1-(2,3,4-trimethoxyphenyl)ethanol als weißes kristallines Produkt (Ausbeute: 70%). Schmelzpunkt 128ºC.

- In 23 ml Aceton wurden 2.31 g (5 mmol) der freien Base unter Erhitzen gelöst. Der Lösung wurden 10 ml 1-N HCl in Aceton zugesetzt; es wurden 2.19 g 2-(4-Diphenylmethylpiperazinyl)-1- (2,3,4-trimethoxyphenyl)ethanoldihydrochlorid als weißes kristallines Pulver (Ausbeute 82%) erhalten. Schmelzpunkt 189ºC (Zersetzung).

- 1-(3,4-Dimethoxyphenyl)-2-(4-diphenylmethylpiperazinyl)ethanon wurden gemäß Referenzbeispiel 1 behandelt. Das gebildete Produkt wurde säulenchromatographisch über Silicagel gereinigt und aus Äther umkristallisiert; es wurde 1-(3,4-Dimethoxyphenyl)-2-(4-diphenylmethylpiperazinyl)ethanol (Ausbeute 75%) erhalten. Schmelzpunkt 85ºC.

- Berechnet (%) C 74.97 H 7.46 N 6.48
- Gefunden (%) C 75.19 H 7.62 N 6.68
- Die oben erwähnte freie Base wurde in ihr Hydrochlorid gemäß Referenzbeispiel 1 umgewandelt und aus Ethanol/Äther umkristallisiert. Es wurde 1-(3,4-Dimethoxyphenylmethylpiperazinyl)ethanoldihydrochlorid (Ausbeute 74%) erhalten. Schmelzpunkt 175ºC (Zersetzung).

- Eine Tablette (220 mg) enthielt die folgenden Komponenten:
- Aktive Komponente 50 mg
- Lactose 100 mg
- Stärke 50 mg
- Magnesiumstearat 5 mg
- Hydroxypropylzellulose 15 mg
- Eine Kapsel enthielt 350 mg der folgenden Komponenten:
- Aktive Komponente 40 mg
- Lactose 200 mg
- Stärke 70 mg
- Polyvinylpyrrolidon 5 mg
- Kristalline Zellulose 35 mg
- Das Granulat enthielt die folgenden Komponenten je 1 g:
- Aktive Komponente 200 mg
- Lactose 450 mg
- Maisstärke 300 mg
- Hydroxypropylzellulose 50 mg
- In ungefähr 600 ml destilliertem Wasser wurden unter Rühren 2.5 g der aktiven Komponente und 3.5 g D-Mannitol gelöst. Der gebildeten Lösung wurde ein Phosphatpuffer zugesetzt, um den pH- Wert der Lösung auf 4.0 bis 5.5 festzulegen. Die Lösung wurde mit destilliertem Wasser auf 1000 ml Lösung aufgefüllt. Die Lösung wurde dann gefiltert und in Ampullen in Mengen von 4 ml je Ampulle in üblicher Weise abgefüllt.
- Die 1000 ml Lösung gemäß Beispiel 4 wurden in Glasröhrchen eingebracht und in üblicher Weise gefriergetrocknet.
- In etwa 600 ml destilliertem Wasser wurden unter Rühren 0.3 g der aktiven Komponente und 8.7 g NaCl gelöst. Der gebildeten Lösung wurde ein Phosphatpuffer bis zu einem pH-Wert der Lösung von 4.0 bis 5.5 zugesetzt. Die Lösung wurde mit destilliertem Wasser auf 1000 ml Lösung aufgefüllt. Die Lösung wurde dann gefiltert und in Ampullen eingegeben.
- Die 1000 ml Lösung gemäß Beispiel 6 wurden in Glasröhren eingebracht und in üblicher Weise gefriergetrocknet.
Claims (4)
1. Verwendung eines Piperazinderivats der Formel
in der R¹ für Wasserstoff oder eine niedrige Alkylgruppe, R² für
Hydroxyl, eine Aralkyloxygruppe, eine niedere Alkoxygruppe mit
1-5 Kohlenstoffatomen oder eine niedere Alkenyloxygruppe mit 3-5
Kohlenstoffatomen, R³ für Wasserstoff, eine Aralkyloxygruppe,
eine niedrige Alkoxygruppe mit 1-5 Kohlenstoffatomen oder eine
niedrige Alkenyloxygruppe mit 3-5 Kohlenstoffatomen, und R&sup4; für
Wasserstoff oder eine niedere Alkoxygruppe mit 1-5
Kohlenstoffatome stehen, als aktive Komponente in der Zubereitung eines
Arzneimittels zur Verminderung verzögerter neuronaler Schädigung
oder Tods, einem Ichemia im Gehirn folgend.
2. Verwendung des Derivats gemäß Anspruch 1, wobei die
aktive Komponente
1-(3,4-Dimethoxyphenyl)-2-(4-diphenylmethylpiperazinyl)ethanol ist.
3. Verwendung des Derivats nach Anspruch 1, wobei das
Derivat als Salz einer pharmazeutisch verträglichen Säure vorliegt.
4. Verwendung des Derivats nach den Ansprüchen 1 bis 3,
wobei das Arzneimittel in Form von Tabletten, Kapseln, Granulen
oder als injezierbare Zubereitung vorliegt.
