Description
慢性肝炎の末期症状である肝硬変は高率に肝細胞癌を発症するが、肝移植を除いて根治療法はなく、肝硬変に至る前の肝線維症の段階で脱線維化を誘導する治療法開発が急務である。
そして、当該ウイルス性慢性肝炎モデルマウスを用いて肝線維症の予防・治療薬候補化合物を探索した結果、意外なことにCBP/β-カテニン阻害剤が優れた肝線維症の予防・治療作用を示すことを見出し、本発明を完成するに至った。

特に言及しなければ、本明細書及び請求の範囲で用いられる以下の用語は、本出願のために次の意味を有するものである。
肝線維症は、ウイルス性肝炎、アルコール性肝障害、自己免疫性肝障害および代謝性肝障害などの様々な肝障害により壊死した肝細胞が修復される過程でコラーゲンや複合糖質からなる細胞外マトリックスと呼ばれる線維組織の産生が促進され、炎症の進行とともに徐々にこれらの線維組織が蓄積した状態である。
ウイルス性肝炎の病原ウイルスは特に限定されず、A型肝炎ウイルス、B型肝炎ウイルス、C型肝炎ウイルス、E型肝炎ウイルス等が挙げられ、中でもC型肝炎ウイルスに起因するC型肝炎を経て肝線維症に至る態様が好ましい。
「肝線維症の予防」とは肝線維症の病態の発症を防ぐこと、抑制することまたは遅延させることを意味する。
「肝線維症を治療するための有効量」は、肝線維症を治療するために動物に投与された場合に、当該肝線維症について、そのような治療を達成するのに十分な量を意味する。
「肝線維症を予防するための有効量」は、肝線維症を予防するために動物に投与された場合に、当該肝線維症について、そのような予防を達成するのに十分な量を意味する。

本発明者らは、ヒトにおける慢性C型肝炎を的確に評価できるモデル動物として、Cre/loxPシステムでHCV遺伝子を導入したトランスジェニック(Cre/loxP/HCV-Tg)マウスと、IFN誘導性のCreを発現するトランスジェニック(Mx-Cre-Tg)マウスを交配させることにより、任意の時期にHCV遺伝子をスイッチング発現するトランスジェニックマウス(Cre/loxP/HCV-MxCreTgマウス)を作製した。このTgマウスにポリイノシン酸-ポリシチジル酸(Poly(I:C))を投与することでIFNα誘導性のCre酵素が発現し、loxP配列に挟まれているネオマイシン耐性遺伝子をループアウトすることで、任意の時期にHCV蛋白を発現させることを可能にした。すなわち、スイッチング発現システムを樹立したことにより、発生段階でのHCV遺伝子の発現はなくなり、HCV感染に似た免疫反応状態をつくることに成功した。
このモデルマウスにPoly(I:C)を投与することにより発現させた肝臓内のHCV蛋白は、完全に排除されることなく、600日以上の持続的な発現が確認された。形態学的には肝細胞の膨化、脂肪変性、グリコーゲン変性、肝細胞索の乱れ、線維化、肝細胞癌およびリンパ腫などがみられた。
したがって、Cre/loxP/HCV-MxCreTgマウスは、C型肝炎から肝線維症、肝硬変、並びに肝癌に至る病態を的確に評価できる病態モデルである。
本発明のCBP/β-カテニン阻害剤は、常法により製剤化した医薬製剤(例えば、注射剤、カプセル剤、錠剤、散剤、顆粒剤など)として、ヒト又は動物(例えば、マウス、ラット、ハムスター、ウサギ、ネコ、イヌ、ウシ、ヒツジ、サル等の哺乳動物)に投与することができる。例えば、有効成分の量に換算して、1日あたり約0.01~1000mg/kg(体重)、好ましくは1日あたり約0.1~500mg/kg(体重)の投与量で、1回または数回に分けて投与するとよいが、その投与量、投与方法や投与回数は、症状、年齢などにより適宜変更しうる。例えば注射剤に製剤化する場合には、蒸留水、生理食塩水などの担体を用いるとよく、カプセル剤、錠剤、散剤、顆粒剤に製剤化する場合には、デンプン、乳糖、白糖、炭酸カルシウムなどの賦形剤、デンプンのり液、アラビアゴム、ゼラチン、アルギン酸ナトリウム、カルボキシメチルセルロース、ヒドロキシプロピルセルロースなどの結合剤、ステアリン酸マグネシウム、タルクなどの滑沢剤、デンプン、寒天、結晶セルロース、炭酸カルシウム、炭酸水素ナトリウム、アルギン酸ナトリウムなどの崩壊剤などを用いるとよい。製剤中の有効成分の含有率は、1~99重量%の間で変動させることができる。例えば、錠剤、カプセル剤、顆粒剤、散剤などの形態をとる場合には、有効成分を5~80重量%含有させるのが好ましく、注射剤の場合には、有効成分を1~10重量%含有させるのが好ましい。
投与方法としては特に限定されないが、経口投与、静脈内投与、静脈内持続投与、腹腔内投与などが挙げられる。
また、本発明の医薬組成物を充填した浸透圧ポンプを、例えば、皮下に埋め込み、持続的に投与する態様も好ましい。
R6CN2 HCV cDNA(nt 294-3435)及び全ゲノムHCV cDNA(nt 1-9611)は、慢性活動性肝炎に罹患した患者(#R6)から採取した血液サンプルからクローニングした。この血液サンプルの感染測定は、公知文献(Proc.Natl.Acad.Sci.USA,90:6037-6041)に報告された方法に従って行った。R6CN2HCV及びR6CN5HCVトランスジェニックマウスはMx1-Creトランスジェニックマウス(Jackson Laboratoryから購入)と交配させ、R6CN2HCV-MxCreトランスジェニックマウス及びR6CN5HCV-MxCreトランスジェニックマウスを作製し、それぞれCN2-29(+/-)/MxCre(+/-)マウス及びRzCN5-15(+/-)/MxCre(+/-)マウスと名付けた。
これらマウスの肝臓でのCre発現は、ポリイノシン酸-ポリシチジル酸[Poly(I:C)](GE Healthcare UK Ltd.(英国バッキンガムシャー州)より購入)の腹腔内注射により誘導した。すなわち、300μLのpoly(I:C)溶液(1mg/mLリン酸緩衝生理食塩水[PBS])を生後2ヶ月経過後、48時間間隔で3回腹腔内注射した。このpoly(I:C)投与操作を1回行うことでほぼすべての肝実質細胞でHCV遺伝子発現を誘導することができる。この発現は600日以上にわたって持続し、慢性肝炎から肝線維化に至る肝線維症モデル動物を樹立することができた。
全ての動物の世話及び実験手順は、東京都臨床医学総合研究所動物実験専門委員会により確立されたガイドラインに従って行われた。
実施例1で得られたCN2-29(+/-)/MxCre(+/-)マウスにCBP/β-カテニン阻害剤を腹腔内投与して、CBP/β-カテニン阻害剤が肝線維症の治療効果を有するか否かをモデルマウスから採取した肝組織に組織学的染色を行うことで調べた。
CBP/β-カテニン阻害剤を市販のPBSpH7.6に溶解し、水酸化ナトリウム水溶液にてpHを7.6に再調整し、40mg/mlのCBP/β-カテニン阻害剤溶液を得た。この溶液を浸透圧ポンプ(ALZET mini-osmotic pump model 2006)に250μl充填し、マウス背皮下に埋め込んだ。これにより0.15μl/hrで42日間に渡り持続的にCBP/β-カテニン阻害剤を投与した。対照群には、市販のPBSpH7.6を同様に浸透圧ポンプに充填し、マウス背皮下に埋め込んでvehicleとして投与した。モデル作製及びCBP/β-カテニン阻害剤投与のスケジュールを図1に示す。
本実施例で使用したCBP/β-カテニン阻害剤は、以下の通りである:

本CBP/β-カテニン阻害化合物は、WO2009/148192の実施例I-7に記載された化合物であり、同文献記載の方法で合成することができる。また、本CBP/β-カテニン阻害化合物がCBP/β-カテニン阻害作用を有することは、同文献記載のレポーターアッセイ法により確認されている。
(1)のCBP/β-カテニン阻害化合物投与の42日目に剖検を行い、肝臓を採取して組織学的検索を行った。
染色はヘマトキシリン・エオジン染色(HE staining)により組織の全体像を、銀染色及びアザン染色(Azan staining)により線維を染色・観察した。
ヘマトキシリン・エオジン染色は病理組織の最も基本的で重要な一般染色法であり、組織構造の光顕レベルでの全体像の把握を目的とする。ヘマトキシリン、エオジン各々の染色液により、一般に次のように染め分ける。
ヘマトキシリンにより青藍色~淡青色:細胞核・石灰部・軟骨組織・細菌・粘液。
エオジンにより赤~濃赤色:細胞質・間質・各種線維・赤血球・角化細胞。
銀染色は結合組織線維の1つである細網線維を染める事が目的である。
アザン染色は膠原線維(濃青色)と筋線維(青色)を染め分ける染色法である。
組織サンプルは、PBS中の4%パラホルムアルデヒドで固定し、パラフィン中に封埋し、薄片にした(厚さ4μm)。得られた切片をヘマトキシリン・エオジン染色(HE)、銀染色及びアザン染色を行った。
ヘマトキシリン・エオジン染色は、病理組織染色ハンドブック(高橋清之・医学書院)に記載の方法に従い行った。

CBP/β-カテニン阻害化合物は、42日間の投与で慢性C型肝炎モデルマウス(Cre/loxP/HCV-MxCre Tgマウス)の肝小葉構造を改善し、脱線維化作用を示した(図2、図3、図4)。
慢性肝炎により異常増殖し、肝細胞索構造が無くなっていた肝臓組織がCBP/β-カテニン阻害化合物の投与により索状形態を取るように正常化した(図2)。
慢性肝炎により肝線維化が亢進して鍍銀染色により細網線維が強く染まった(図3左)。CBP/β-カテニン阻害化合物の投与により細網線維の染色が弱くなり正常化した(図3右)。
同様に慢性肝炎により肝線維化が亢進してアザン染色により膠原線維が強く染まっていたのがCBP/β-カテニン阻害化合物の投与により染色が弱くなり正常化した(図4)。
以上の結果より、CBP/β-カテニン阻害剤には慢性肝炎・肝線維症・肝硬変からの脱線維化作用があることを明らかになった。

- 肝線維症が、肝炎ウイルスの感染に起因する肝線維症である、請求項10に記載の方法。
- 肝線維症が、C型肝炎またはC型肝炎ウイルスの感染に起因する肝線維症である、請求項10に記載の方法。
