Description
本発明の実施形態は、血流機能等を検査する為の血流機能検査装置、及び、該血流機能検査装置を具備するX線診断装置に関する。
近年、例えば血管狭窄が発生した場合等には、外科的な治療ではなく、カテーテルと称される管状部材を用いて内科的な治療を行う例が増加している。カテーテルを用いた治療においては、カテーテルを例えば鼠蹊部から被検体に挿入し、当該カテーテルを血管に沿って狭窄部まで進め、当該狭窄部でバルーンを膨らませることにより治療をする。
さらに、再狭窄が発生することを防ぐ為に、ステントと称される金属から成る網目状の筒を狭窄部に挿入することで、血管壁を血管の内側から支えて再度の狭窄化を防止することもある。
しかしながら、上述したカテーテルによる内科的治療は、当然ながら症状の改善を100%保障するものではない。
例えば、一つの血管に複数の狭窄部が存在する場合、実際には治療を施した狭窄部とは別の狭窄部が血流低下により大きく関与している可能性も充分に考えられる。このような場合には、血流低下に関与している他の狭窄部をも治療しなければ症状は改善しない。
さらに、より悪い状態の場合には、血管の先の組織が壊死している可能性も考えられる。このような場合においても、狭窄部の治療に関わらず、既に組織自体が壊死している為、症状は改善しない。
ところで、狭窄部を治療することによる経済的負担及び治療に伴うリスクが現実に存在する以上、上述したように治療が症状改善に寄与しないような狭窄部の治療は回避すべきであると言える。
以上説明したような事情から、狭窄部の治療効果を予め予測する為に冠血流予備量比(FFR;Fractional Flow Reserve)と称される指標が用いられている。FFRは、下記式によって算出される。

ここで、Pd及びPpは、圧センサ付ガイドワイヤを狭窄部まで挿入して測定して得た圧力の値であり、狭窄部から心臓に近い側の部位での圧力をPpとし、狭窄部から末梢側の部位での圧力をPdとしている。つまり、FFRの値は、狭窄部の前後部における圧力(血圧)の比を算出することで得られる。このFFRの値に基づいて、種々の治療の必要性等が判断される。換言すれば、FFRは、冠動脈病変の機能的評価の指標として冠動脈病変評価に用いられている。なお、上述した圧センサ付ガイドワイヤ自体については従来より種々のものが提案されている(例えば特許文献1乃至特許文献3参照)。
ここで、Pd及びPpは、圧センサ付ガイドワイヤを狭窄部まで挿入して測定して得た圧力の値であり、狭窄部から心臓に近い側の部位での圧力をPpとし、狭窄部から末梢側の部位での圧力をPdとしている。つまり、FFRの値は、狭窄部の前後部における圧力(血圧)の比を算出することで得られる。このFFRの値に基づいて、種々の治療の必要性等が判断される。換言すれば、FFRは、冠動脈病変の機能的評価の指標として冠動脈病変評価に用いられている。なお、上述した圧センサ付ガイドワイヤ自体については従来より種々のものが提案されている(例えば特許文献1乃至特許文献3参照)。
ところで、FFRを算出する為には、圧センサ付ガイドワイヤを狭窄部の前後に位置するように配置し、且つ、ガイドワイヤを安定させるように操作しつつ、圧センサによる計測結果(圧力データ)を記録する為の操作(記録ボタンの押圧)及び補正操作等を全て手動で行うという煩雑な操作が要求される。さらには、病変が複数存在する場合には、各々の病変部毎に、上述の操作を繰り返し実行しなくてはならない。また、ガイドワイヤは血管内壁などを傷つけたりしないよう常にコントロールしていなければならないので、FFRを計測する装置の操作は別の人間が行うことになる。
従って、不適切な位置での計測や計測結果の記録忘れ等が発生する可能性も充分に考えられ、全ての操作を適切に行うことができたとしても作業時間として相当な時間が掛かってしまう。
前記の事情に鑑みて為されたものであり、例えばFFRの計測等の血流機能の検査における、計測操作手順の簡略化及び作業時間の短縮化を実現した血流機能検査装置及びX線診断装置を提供することを目的とする。
本実施形態に係るX線診断装置は、被検体にX線を照射するX線発生ユニットと、前記X線発生ユニットによって照射され前記被検体を透過したX線を検出するX線検出ユニットと、前記X線検出ユニットによって検出されたX線に基づいてX線画像を生成する画像生成ユニットと、ガイドワイヤに設けられた圧力センサを用いて収集された圧力データを記録する記録ユニットと、前記X線画像を用いて、前記圧力センサによる圧力の計測位置を設定する計測位置設定ユニットと、前記X線画像を略リアルタイムに表示すると共に計測位置設定ユニットによって設定された計測位置を重ねて表示する表示ユニットと、を具備するものである。

図1は、本発明の第1実施形態に係る血流機能検査装置の一構成例を示すブロック図である。同図に示すように、本第1実施形態に係る血流機能検査装置100には、X線診断装置50と、圧センサ付ガイドワイヤ90と、心電計(不図示)と、が通信可能に接続されている。なお、以下の説明においては、X線診断装置50とは別体として構成された血流機能検査装置100により、後述するFFR算出に係る処理を実現する場合を例とする。しかしながら、当該例に拘泥されず、血流機能検査装置100の機能を内蔵したX線診断装置50により、FFR算出に係る処理を実現するようにしてもよい。
図1は、本発明の第1実施形態に係る血流機能検査装置の一構成例を示すブロック図である。同図に示すように、本第1実施形態に係る血流機能検査装置100には、X線診断装置50と、圧センサ付ガイドワイヤ90と、心電計(不図示)と、が通信可能に接続されている。なお、以下の説明においては、X線診断装置50とは別体として構成された血流機能検査装置100により、後述するFFR算出に係る処理を実現する場合を例とする。しかしながら、当該例に拘泥されず、血流機能検査装置100の機能を内蔵したX線診断装置50により、FFR算出に係る処理を実現するようにしてもよい。
X線診断装置50は、血流機能検査装置100にX線診断画像を提供する為の装置であり、X線撮影機構10と画像処理装置20とを具備する。
前記X線撮影機構10は、X線管球12と、検出システム14と、寝台(不図示)の天板16と、C形アーム160と、を有する。
前記X線管球12は、検出システム14と共に、C形アーム160にマウントされる。前記検出システム14はFPD(Flat Panel Detector)が一般的である。しかしながら、検出システム14はイメージインテンシファイアとTVカメラとを備えたものであっても良い。C形アーム160は、天井から吊り下げられる支柱に支持される。C形アーム160は、直交3軸に関して回転可能である。寝台(不図示)の天板16上に載置された被検体Pは、X線管球12と検出システム14との間に配置される。
画像処理装置20は、制御部37と、A/D変換器38と、画像メモリ24と、サブトラクション部31と、アフィン変換部36と、表示部41と、ルックアップテーブル(以降、LUTと略称する)45と、フィルタリング部32と、ファイバチャネルカード47と、を有する。
前記制御部37は、画像処理装置20の各部を統括的に制御する。前記A/D変換器38は、X線撮影機構10に接続されており、X線撮影機構10からアナログ情報として供給される投影データをデジタル化する。前記画像メモリ24は、X線撮影機構10から供給され、A/D変換器38によりデジタル化された投影データを記憶する。前記サブトラクション部31は、画像データのサブトラクト(DSA;Digital Subtraction Angiography)を実行する。前記フィルタリング部32は、例えば高周波強調処理等のフィルタリング処理を行う。前記アフィン変換部36は、画像データに対して拡大処理、移動処理を行う。前記LUT45は、画像データに対して階調変換を行う。前記ファイバチャネルカード47は、ファイバチャネル転送のI/Fである。
前記血流機能検査装置100は、圧力計測部101と、ファイバチャネルカード103と、ワイヤ先端部検出部105と、目標位置設定部107と、記録タイミング制御部109と、記録部111と、FFR算出部113と、表示部141と、を有する。
前記圧力計測部101は、圧センサ付ガイドワイヤ90の圧センサからの出力信号(圧力データ)を受信し、該信号から圧力を算出(計測)する。
前記ファイバチャネルカード103は、ファイバチャネル転送のI/Fである。ここで、画像処理装置20と血流機能検査装置100とは、ファイバチャネルカード47,103を介してファイバチャネルケーブルで接続されている。X線撮影機構10によって収集されたデータは、ファイバチャネルケーブルを用いた高速データ通信により、略リアルタイムに血流機能検査装置100に転送されるように構成されている。
前記ワイヤ先端部検出部105は、画像処理装置20から転送されたX線診断画像から圧センサ付ガイドワイヤ90の先端部を検出する。具体的には、圧センサ付ガイドワイヤ90の先端部には例えば放射線不透過性マーカ(例えばタングステン等から成るマーカ)が設けられている。また、圧センサ付ガイドワイヤ90の先端部は、可視性を高める構造を有している。前記ワイヤ先端部検出部105はこのマーカをX線診断画像から検出することで、圧センサ付ガイドワイヤ90の先端部を検出する。また、ワイヤ先端部検出部105は、例えば相関演算等の画像処理技術によって、圧センサ付ガイドワイヤ90の先端部を追跡する。
前記目標位置設定部107は、画像処理装置20から転送されたX線診断画像に基づいて、予め圧力を計測(記録)する目標位置(後述する計測位置)を設定する。
前記記録タイミング制御部109は、ワイヤ先端部検出部105によって検出された圧センサ付ガイドワイヤ90の先端部の位置と、目標位置設定部107によって設定された計測位置とに基づいて、所定のタイミングで前記圧力データを記録部111に記録する。換言すれば、前記記録タイミング制御部109は、圧センサ付ガイドワイヤ90の先端部の位置と計測位置とに基づいて、圧力データを記録部111に記録するタイミングを制御する(記録タイミング信号を生成する)。
具体的には、記録タイミング制御部109は、圧センサ付ガイドワイヤ90の先端部が予め設定された計測位置に到達し、且つ、当該計測位置で所定時間静止したことを検出すると、当該計測位置において圧センサが収集した圧力データを記録部111に記録する。なお予め設定された計測位置は一定の範囲を持つものとする。
前記記録部111は、記録タイミング制御部109による制御で、各計測位置において収集された圧力データを記録する。
前記FFR算出部113は、記録部111に記録された圧力データに基づいて、冠血流予備量比(以降、FFRと略称する)を算出する。FFRは、狭窄部の治療効果を予め予測する為の指標である。ここで、Pd及びPpを、圧センサ付ガイドワイヤを狭窄部まで挿入して測定して得た圧力の値とし、狭窄部から心臓に近い側の部位での圧力をPp、狭窄部から末梢側の部位での圧力をPdとすると、前記FFRは、下記式によって算出される。

以下、本発明の第1実施形態に係る血流機能検査装置100によるFFR算出に係る一連の処理の一例を説明する。
なお、実際に圧センサ付ガイドワイヤ90を使用してFFRを算出する場合には、空気圧でのキャリブレーション等の様々なキャリブレーションが必要である。しかしながら、それらキャリブレーションは本第1実施形態に係る血流機能検査装置に特有の処理ではない為、説明を省略する。
まず、ユーザは、狭窄部が生じている血管の心臓側にカテーテルを誘導し、そこから造影剤を注入しつつX線撮影機構10及び画像処理装置20によって、X線診断画像の動画像(以降、X線動画像と略称する)を収集する。このとき、検査対象の血管が心臓血管である場合には、X線動画像だけでなく、心電計(不図示)によって心電図も同時に収集する。X線撮影機構10及び画像処理装置20によって収集された画像は、略リアルタイムで血流機能検査装置100に送信される。
ユーザは、血流機能検査装置100の表示部141に表示されたX線動画像を参照し、狭窄部が明瞭に視認でき、且つ、動き(例えば心臓の動き)が少ない画像を選択し、『目標位置設定ボタン』を押圧する。
図2は、FFRを算出する際の狭窄部における計測位置を示す図である。目標位置設定部107は、図2に示すように、狭窄部の直近の心臓側から末梢側へという順で計測対象の血管に対して略垂直な線を描いて計測位置を設定する。このとき、検査対象の血管が心臓血管である場合には、心臓がほぼ静止しているときの心電位相を指定し、その心電位相に対応するX線画像上で計測位置を設定する。設定された計測位置は、例えば図2に例示するように、X線画像上において明示的に表示される。
ここで、複数の狭窄部を有する病変については、この操作を狭窄の個数分繰り返す。図2に示す例では狭窄部(1)S1及び狭窄部(2)S2が存在するので、2か所の狭窄部の心臓側と末梢側とにそれぞれ1本ずつ直線が描かれている。ここで狭窄部には、計測位置が設定された順に番号(本例では(1)、(2))が振られる。
図2において、狭窄部(1)S1の心臓側の線がPp1(狭窄部(1)S1の心臓側の血圧)の計測位置、末梢側の線がPd1(狭窄部(1)S1の末梢側の血圧)の計測位置を示している。同様に、図2において、狭窄部(2)S2の心臓側の線がPp2(狭窄部(2)S2の心臓側の血圧)の計測位置、末梢側の線がPd2(狭窄部(2)S2の末梢側の血圧)の計測位置を示している。
ユーザは、目標位置設定部107によって計測位置が適切に設定されたと判断すると、『設定完了ボタン』を押圧する。これにより、計測位置情報と心電図位相情報(R-R信号の何%位置)とが、記録タイミング制御部109に送信される。 なお、設定された計測位置は、心電図の位相情報を元に、計測位置を重ねて表示する状態と表示しない状態とを区別するようにしてもよい。また、心電図の位相情報を元に、計測位置を重ねて表示するリアルタイム画像を逐次更新することも可能である。 続いて、ユーザは、表示部141に表示されたX線動画像を参照しながら、カテーテルの中に圧センサ付ガイドワイヤ90を通し、狭窄部(2)S2よりも末端側の所定の位置においてカテーテルの先端部から圧センサ付ガイドワイヤ90を露出させる。なお、少なくともカテーテルや圧センサ付ガイドワイヤ90の操作中においては、ワイヤ先端部検出部105は、ファイバチャネルケーブル経由でX線診断装置50からX線動画像を随時リアルタイムで取得し、或る時点で収集した画像と、その直前に収集した画像とをサブトラクションし、圧センサ付ガイドワイヤ90の移動が抽出されたサブトラクション画像を生成する。このとき、検査対象の血管が心臓血管である場合には、計測位置の設定に使用した心電位相で収集した画像と、その1周期前に収集した画像と、をサブトラクションし、圧センサ付ガイドワイヤ90の移動が抽出されたサブトラクション画像を生成する。
さらに、ワイヤ先端部検出部105は、前記サブトラクション画像から圧センサ付ガイドワイヤ90の両エッジを抽出し、該抽出した両エッジから、圧センサ付ガイドワイヤ90の先端部の特徴量を利用して当該先端部を検出する。
ユーザは、カテーテルを引きながら、血管内において圧センサ付ガイドワイヤ90をゆっくりと移動させる。記録タイミング制御部109は、前記検出された圧センサ付ガイドワイヤ90の先端部と、前記計測位置情報とを比較し、圧センサ付ガイドワイヤ90の先端部が計測位置から所定距離範囲内に入り且つ所定時間静止したか否かを判断する。このとき、検査対象の血管が心臓血管である場合には、計測位置の設定に使用した心電位相で収集した画像から検出された圧センサ付ガイドワイヤ90の先端部と、前記計測位置情報とを比較し、圧センサ付ガイドワイヤ90の先端部が計測位置から所定距離範囲内に入り且つ所定時間静止したか否かを判断する。ここで、圧センサ付ガイドワイヤ90の先端部が計測位置から所定距離範囲内に入り且つ所定時間静止したと判断した場合、記録タイミング制御部109は、血流機能検査装置100の表示部141に例えば『狭窄部(2)の心臓側位置での血圧記録を開始します。ガイドワイヤを固定して動かさないようにして下さい。』との旨のメッセージを表示させる。
さらに、記録タイミング制御部109は、圧センサ付ガイドワイヤ90の先端部と計測位置との距離が変化していないことを確認した上で、例えば『狭窄部(2)の心臓側位置での血圧記録を行っています。ガイドワイヤを固定して動かさないようにして下さい。』との旨のメッセージを表示部141に表示させ、当該計測位置における圧力データを記録部111に記録する(記録タイミング信号を生成する)。すなわち、記録部111は、記録タイミング制御部109の制御で(記録タイミング信号に従って)、当該計測位置において圧力センサが収集した圧力データを記録する。
所定時間の記録が完了すると、記録タイミング制御部109は、『狭窄部(2)の心臓側位置での血圧記録を完了しました。ガイドワイヤを次の計測位置に移動して下さい。』との旨のメッセージを表示部141に表示させる。記録が完了すると、記録タイミング制御部109は、記録した圧力データを示すグラフ(計測位置における血圧変動のグラフ)を、表示部141に表示させる。
そして、ユーザは、この表示部141に表示されたグラフを視認し、正常に血圧が計測されたか否かを判断する。ここで、例えば血圧変動が周期毎に大きく変化している場合には、当該計測が正しく行われていない可能性があるので、ユーザは『再計測ボタン』を押圧し、再度の計測を実行する。
上述した一連の操作・処理を、残りの計測位置(すなわち、本例の場合、狭窄部(2)S1の末梢側位置、狭窄部(1)S2の心臓側位置、末梢側位置)についてそれぞれ計測を実行する。
そして、Pp1,Pd1,Pp2,Pd2の全ての算出・記録が完了すると、記録タイミング制御部109は、それら圧力データを記録部111から読み出してFFR算出部113に出力する。そして、FFR算出部113は、それら記録部111に記録された圧力データ(本例ではPp1,Pd1,Pp2,Pd2)を、上述の(式2)に代入することによって、全ての狭窄部(本例では、狭窄部(1)S1及び狭窄部(2)S2)に係るFFRの値を算出する。
また、本実施例ではX線診断装置50と血流機能検査装置100とを別体構成としているが、血流機能検査装置100をX線診断装置50内部に組み込んでも勿論良い。また、血流機能検査装置100の一部の機能だけをX線診断装置50内部に組み込んでも良い。具体的にはワイヤ先端部検出部105と、目標位置設定部107と、記録タイミング制御部109とをX線診断装置50内部に組み込み、X線診断装置50から血流機能検査装置100に記録タイミングを指示するような構成としても良い。
さらには、本第1実施形態は、FFRの算出のみならず、FFRと同様な手順で算出する冠血流予備能(CFR)の算出においても適用できることは勿論である。
以上説明したように、本第1実施形態によれば、例えばFFRの計測等の血流機能の検査における、計測操作手順の簡略化及び作業時間の短縮化を実現した血流機能検査装置及びX線診断装置を提供することができる。
上述した第1実施形態に係る血流機能検査装置及びX線診断装置は、例えば下記のように変形して実施することができる。
《変形例1》
例えば、上述した例では、FFR算出部113は、全ての狭窄部(狭窄部(1)及び狭窄部(2))に係る圧力データが算出・記録された時点でFFRを算出している。しかしながら、一つの狭窄部に係るPp及びPdが算出・記録された時点で順次FFRを算出してもよい。
例えば、上述した例では、FFR算出部113は、全ての狭窄部(狭窄部(1)及び狭窄部(2))に係る圧力データが算出・記録された時点でFFRを算出している。しかしながら、一つの狭窄部に係るPp及びPdが算出・記録された時点で順次FFRを算出してもよい。
さらに、或る狭窄部に係るFFRの値が“虚血閾値”を下回った場合(例えば0.75を“虚血閾値”として、FFRの値が0.63と算出・記録された場合等)には、ユーザによる確認の為に当該狭窄部に係る圧力データのグラフを表示部141に表示するように構成してもよい。
また、或る狭窄部に係るFFRの値が“虚血閾値”を下回った場合(例えば0.75を “虚血閾値”として、FFRの値が0.63と算出・記録された場合等)には、当該狭窄部に係る計測位置における圧力データを再収集し、FFRを再度算出するように構成してもよい。
《変形例2》
上記説明においては、圧センサ付ガイドワイヤ90の先端部が計測位置から所定距離範囲内に入り且つ所定時間静止したと判断した場合に、計測位置における圧力データを記録部111に自動的に記録する構成とした。しかしながら、当該例に拘泥されず、例えばユーザ自身のマニュアル操作によって、計測位置における圧力データの記録開始を指示するようにしてもよい。この場合、圧センサ付ガイドワイヤ90の先端部が計測位置に到達したこと(或いは所定距離範囲内に入ったこと)をトリガとして、血流機能検査装置100の表示部141に例えば『狭窄部(2)の心臓側位置での血圧記録開始ボタンを押してください。ガイドワイヤを固定して動かさないようにして下さい。』等の記録開始指示を促すメッセージを表示する(或いは音声等にて出力する)ことが好ましい。また、上述した計測中であること、計測が完了したことを知らせるメッセージも表示することができる。さらに、仮に記録開始指示が入力されることなく圧センサ付ガイドワイヤ90の先端部が計測位置を通過した場合には、表示部141に例えば『『狭窄部(2)の心臓側位置での血圧が記録されていません。』等の警告メッセージを表示する(或いは音声等にて出力する)ことが好ましい。
上記説明においては、圧センサ付ガイドワイヤ90の先端部が計測位置から所定距離範囲内に入り且つ所定時間静止したと判断した場合に、計測位置における圧力データを記録部111に自動的に記録する構成とした。しかしながら、当該例に拘泥されず、例えばユーザ自身のマニュアル操作によって、計測位置における圧力データの記録開始を指示するようにしてもよい。この場合、圧センサ付ガイドワイヤ90の先端部が計測位置に到達したこと(或いは所定距離範囲内に入ったこと)をトリガとして、血流機能検査装置100の表示部141に例えば『狭窄部(2)の心臓側位置での血圧記録開始ボタンを押してください。ガイドワイヤを固定して動かさないようにして下さい。』等の記録開始指示を促すメッセージを表示する(或いは音声等にて出力する)ことが好ましい。また、上述した計測中であること、計測が完了したことを知らせるメッセージも表示することができる。さらに、仮に記録開始指示が入力されることなく圧センサ付ガイドワイヤ90の先端部が計測位置を通過した場合には、表示部141に例えば『『狭窄部(2)の心臓側位置での血圧が記録されていません。』等の警告メッセージを表示する(或いは音声等にて出力する)ことが好ましい。
《変形例3》
圧センサ付ガイドワイヤ90の先端部の位置(すなわち、圧力センサの位置)を、X線画像上において、周囲とは異なる様態(例えば、異なる色、異なる濃度、輝度の反転、点滅、或いはこれらの組み合わせ)で重ね合わせて表示するようにしてもよい。

[第2実施形態]
以下、本発明の第2実施形態に係る血流機能検査装置及びX線診断装置について説明する。説明の重複を避ける為、上述した第1実施形態との相違点を説明する。なお、本実施形態においても、第1の実施形態と同様に、X線診断装置50とは別体として構成された血流機能検査装置100により、後述するFFR算出に係る処理を実現する場合を例とする。しかしながら、当該例に拘泥されず、血流機能検査装置100の機能を内蔵したX線診断装置50により、FFR算出に係る処理を実現するようにしてもよい。
以下、本発明の第2実施形態に係る血流機能検査装置及びX線診断装置について説明する。説明の重複を避ける為、上述した第1実施形態との相違点を説明する。なお、本実施形態においても、第1の実施形態と同様に、X線診断装置50とは別体として構成された血流機能検査装置100により、後述するFFR算出に係る処理を実現する場合を例とする。しかしながら、当該例に拘泥されず、血流機能検査装置100の機能を内蔵したX線診断装置50により、FFR算出に係る処理を実現するようにしてもよい。
本第2実施形態においては、圧センサ付ガイドワイヤ90の先端部には位置センサ(例えばGPSセンサ)が設けられている。
図3は、本発明の第2実施形態に係る血流機能検査装置の一構成例を示すブロック図である。
第1実施形態に係る血流機能検査装置100と相違する構成要件としては、Ethernet(登録商標)等の外部との通信を行う為のI/Fであるネットワークインターフェイス(Network I/F)125と、前記位置センサからの信号(例えばGPS信号)に基づいて圧センサ付ガイドワイヤ90の先端部を検出する位置同定部121と、CT画像やMRI画像等の三次元画像に基づいて予め前記計測位置を設定する為の目標位置設定部127と、CT画像やMRI画像等の三次元画像と位置センサとのキャリブレーションを行う位置補正部123と、位置同定部121からの出力情報と目標位置設定部127からの出力情報とに基づいて記録タイミングを判定・制御する記録タイミング制御部129と、を主に挙げることができる。
本第2実施形態に係る血流機能検査装置100は、ネットワークインターフェイス125を介して、CT装置161、MRI装置171、及びPACS151が通信可能に接続されている。
以下、本発明の第2実施形態に係る血流機能検査装置によるFFR算出に係る一連の処理の一例を説明する。
なお、実際に圧センサ付ガイドワイヤ90を使用してFFRを算出する場合には、空気圧でのキャリブレーション等の様々なキャリブレーションが必要である。しかしながら、それらキャリブレーションは本第2実施形態に係る血流機能検査装置に特有の処理ではない為、説明を省略する。
まず、ユーザは、検査対象の血管の三次元画像を、CT装置161、MRI装置171、及びPACS151等からネットワークインターフェイス125経由で取得する。この三次元画像は、検査対象の血管について狭窄部を含む領域が描出されている医用画像である。
ユーザが『目標位置設定ボタン』を押圧し、目標位置設定部127が三次元画像上で狭窄部の直近の心臓側から末梢側への順で、図4に示すように計測位置を同定(検出)する。図4は、FFRを算出する際の狭窄部における計測位置を示す図である。
ここで、複数の狭窄部を有する病変については、この操作を狭窄部の個数分だけ繰り返し実行する。図4に示す例では狭窄部(1)S1及び狭窄部(2)S2が存在するので、2か所の狭窄部の心臓側と末梢側とにそれぞれ1個ずつ印が描かれている。ここで狭窄部には、ユーザによって計測位置が設定された順に番号が振られる。
図4において、狭窄部(1)S1の心臓側の印がPp1(狭窄部(1)S1の心臓側の血圧)の計測位置、末梢側の印がPd1(狭窄部(1)S1の末梢側の血圧)の計測位置を示している。同様に狭窄部(2)S2の心臓側の印がPp2(狭窄部(2)S2の心臓側の血圧)の計測位置、末梢側の印がPd2(狭窄部(2)S2の末梢側の血圧)の計測位置を示している。
ユーザは、目標位置設定部107によって計測位置が適切に設定されたと判断すると、『設定完了ボタン』を押圧する。これにより、計測位置情報と、三次元画像が収集された心電図位相情報(R-R信号の何%位置)とが、記録タイミング制御部109に送信される。
続いて、ユーザは、カテーテルの中を通して圧センサ付ガイドワイヤ90を進め、『Calibrationボタン』を押圧する。位置同定部121は、前記位置センサからの信号(例えばGPS信号)に基づいて圧センサ付ガイドワイヤ90の先端部を検出し、圧センサ付ガイドワイヤ90の先端部が血管の特徴的な構造に到達した時点で、当該時点における圧センサ付ガイドワイヤ90の先端部の位置に対応する三次元画像上の位置を特定する。この一連の操作・処理は3回実行する。
そして、上述の処理によって特定された検査対象の血管の三次元画像上の座標と、位置センサからの信号(例えばGPS信号)で検出された三次元座標と、が位置補正部123に送信される。位置補正部123は、これら送信されてきた情報に基づいて、三次元画像と位置センサとのキャリブレーションを行う。これにより、圧センサ付ガイドワイヤ90の先端部に設けられた位置センサで検出される三次元座標と、検査対象の血管の三次元画像上の三次元座標と、の対応付け(位置補正処理)が完了する。
上述の処理によって位置補正処理が完了した後、ユーザは、表示部141に表示されたX線動画像を参照しながら、カテーテルの中に圧センサ付ガイドワイヤ90を通し、狭窄部(2)S2よりも末端側の所定の位置においてカテーテルの先端部から圧センサ付ガイドワイヤ90を露出させる。
圧センサ付ガイドワイヤ90の先端部に設けられた位置センサからの信号(例えばGPS信号)は、位置同定部121によって三次元座標に変換され、且つ、位置補正部123によって血管の三次元画像上の三次元座標に変換(補正)される。この変換後の三次元座標は、表示部141に表示された血管の三次元画像上において圧センサ付ガイドワイヤ90の位置として表示され、略リアルタイムで記録タイミング制御部109に送信される。
そして、記録タイミング制御部109は、前記検出された圧センサ付ガイドワイヤ90先端部と、前記計測位置情報とを比較し、圧センサ付ガイドワイヤ90の先端部が計測位置から所定距離範囲内に入り且つ所定時間静止したか否かを判断する。
ここで、圧センサ付ガイドワイヤ90の先端部が計測位置から所定距離範囲内に入り且つ所定時間静止したと判断した場合、記録タイミング制御部109は、血流機能検査装置100の表示部141に例えば『狭窄部(1)の心臓側位置での血圧記録を開始します。ガイドワイヤを固定して動かさないようにして下さい。』との旨のメッセージを表示させる。
さらに、記録タイミング制御部109は、圧センサ付ガイドワイヤ90の先端部と計測位置との距離が変化していないことを確認した上で、例えば『狭窄部(1)の心臓側位置での血圧記録を行っています。ガイドワイヤを固定して動かさないようにして下さい。』との旨のメッセージを表示部141に表示させ、当該計測位置における圧力データを記録部111に記録する(記録タイミング信号を生成する)。すなわち、記録部111は、記録タイミング制御部109の制御で(記録タイミング信号に従って)、当該計測位置において圧力センサによって収集された圧力データを記録する。
所定時間の記録が完了すると、記録タイミング制御部109は、例えば『狭窄部(1)の心臓側位置での血圧記録を完了しました。ガイドワイヤを次の計測位置に移動して下さい。』との旨のメッセージを表示部141に表示させる。記録が完了すると、記録タイミング制御部109は、記録した圧力データを示すグラフ(計測位置における血圧変動のグラフ)を、表示部141に表示させる。
そして、ユーザは、この表示部141に表示されたグラフを視認し、正常に血圧が計測されたか否かを判断する。ここで、例えば血圧変動が周期毎に大きく変化している場合には、当該計測が正しく行われていない可能性があるので、『再計測ボタン』を押圧し、再度の計測を実行する。
上述した一連の操作・処理を、残りの計測位置(本例では、狭窄部(1)S1の末梢側位置、狭窄部(2)S2の心臓側位置、末梢側位置)についてそれぞれ実行する。そして、Pp1,Pd1,Pp2,Pd2の全ての算出・記録が完了すると、記録タイミング制御部109は、それら圧力データを記録部111から読み出してFFR算出部113に出力する。そして、FFR算出部113は、それら記録部111に記録された圧力データ(本例ではPp1,Pd1,Pp2,Pd2)を、上述の(式2)に代入することによって、全ての狭窄部(本例では、狭窄部(1)S1及び狭窄部(2)S2)に係るFFRの値を算出する。
《変形例》
当然ながら、第1の実施形態において説明したFFRの算出タイミング(変形例1)、マニュアル操作による圧力データの記録開始制御(変形例2)、圧力センサの位置の表示形態(変形例3)についても、本第2の実施形態に適用可能である。

以上説明したように、本第2実施形態によれば、第1実施形態に係る血流機能検査装置及びX線診断装置と同様の効果を奏する血流機能検査装置及びX線診断装置を提供することができる。なお、検査対象の血管が心臓血管である場合には、計測位置の設定に使用した心電位相で収集した画像から検出された圧センサ付ガイドワイヤ90の先端部と、前記計測位置情報とを比較し、圧センサ付ガイドワイヤ90の先端部が計測位置から所定距離範囲内に入り且つ所定時間静止したか否かを判断すれば良い。
本発明のいくつかの実施形態を説明したが、これらの実施形態は、例として提示したものであり、発明の範囲を限定することは意図していない。
例えば、上記各実施形態においては、医用画像診断装置としてX線診断装置を利用する場合を例示した。しかしながら、当該例に構成されず、例えば、X線コンピュータ断層撮像装置。磁気共鳴イメージング装置、超音波診断装置等の他の医用画像診断装置を用いて、記述の血流機能検査を実現するようにしてもよい。
また、これら新規な実施形態は、その他の様々な形態で実施されることが可能であり、発明の要旨を逸脱しない範囲で、種々の省略、置き換え、変更を行うことができる。これら実施形態やその変形は、発明の範囲や要旨に含まれるとともに、特許請求の範囲に記載された発明とその均等の範囲に含まれる。

Claims (23)
- 被検体にX線を照射するX線発生ユニットと、
前記X線発生ユニットによって照射され前記被検体を透過したX線を検出するX線検出ユニットと、
前記X線検出ユニットによって検出されたX線に基づいてX線画像を生成する画像生成ユニットと、
ガイドワイヤに設けられた圧力センサを用いて収集された圧力データを記録する記録ユニットと、
前記X線画像を用いて、前記圧力センサによる圧力の計測位置を設定する計測位置設定ユニットと、
前記X線画像を略リアルタイムに表示すると共に計測位置設定ユニットによって設定された計測位置を重ねて表示する表示ユニットと、
を具備するX線診断装置。 - 前記表示ユニットは心電図の位相情報を元に計測位置を重ねて表示する状態と表示しない状態とを制御する請求項1記載のX線診断装置。
- 前記表示ユニットは心電図の位相情報を元に計測位置を重ねて表示するリアルタイム画像を更新するタイミングを制御する請求項1記載のX線診断装置。
- 前記X線診断装置はさらに前記被検体内の圧力センサの位置を検出する位置検出ユニットを具備する請求項1記載のX線診断装置。
- 前記表示ユニットはさらに位置検出ユニットで検出された圧力センサの位置を異なる様態で重ね合わせて表示する請求項1記載のX線診断装置。
- 前記異なる様態は異なる色、異なる濃度、輝度の反転、あるいは点滅のどれか一つあるいは組み合わせである請求項1記載のX線診断装置。
- 前記X線診断装置はさらに前記設定された計測位置と前記検出された圧力センサの位置との位置関係に基づいて、前記圧力データの前記記録ユニットへの記録を制御する制御ユニットを具備する請求項1記載のX線診断装置。
- 前記制御ユニットは、前記圧力センサが前記計測位置に到達したことをトリガとして、前記圧力データの前記記録ユニットへの記録指示入力を促すメッセージを出力する請求項1記載のX線診断装置。
- 前記制御ユニットは、ユーザによって入力される記録指示をトリガとして、前記圧力データの前記記録ユニットへの記録を開始する請求項8記載のX線診断装置。
- 前記制御ユニットは、前記指示が記録入力されることなく前記圧力センサが前記計測位置を通過した場合には、所定のメッセージをユーザに提示する請求項9記載のX線診断装置。
- 前記制御ユニットは、前記圧力センサが前記計測位置で所定時間静止したことをトリガとして、前記圧力データを前記記録ユニットへ記録する請求項1記載のX線診断装置。
- 前記位置検出ユニットは、前記ガイドワイヤに設けられた位置センサが出力する位置情報信号と前記X線画像の座標系とに基づいて、前記検出された圧力センサの位置を補正し、
前記制御ユニットは、前記補正された圧力センサの位置に基づいて、前記圧力センサが前記計測位置に所定時間静止したか否かを判定する請求項1記載のX線診断装置。 - 前記制御ユニットは、同一の心電位相で収集された前記X線画像を用いて前記圧力センサの経時的な位置を特定することで、前記圧力センサが前記計測位置に所定時間静止したか否かを判定する請求項1記載のX線診断装置。
- 前記制御ユニットは、同一の心電位相で収集された前記X線画像を用いて前記圧力センサの経時的な位置を特定することで、前記圧力センサが前記計測位置に所定時間静止したか否かを判定する請求項12に記載のX線診断装置。
- 前記制御ユニットは、前記圧力データの前記記録ユニットへの記録開始時にその旨を示す第1のメッセージをユーザに提示し、記録処理中にはその旨を示す第2のメッセージをユーザに提示する請求項1に記載のX線診断装置。
- 前記第1のメッセージは、短時間で記録を開始する旨を報知するメッセージ、及び、前記ガイドワイヤの位置固定を促すメッセージのうち少なくとも何れか一つを含むメッセージである請求項15に記載のX線診断装置。
- 前記第2のメッセージは、記録中である旨を示すメッセージ、及び、前記ガイドワイヤを動かさないことを要請するメッセージのうち少なくとも何れか一つを含むメッセージである請求項15に記載のX線診断装置。
- 前記制御ユニットは、前記記録ユニットへの前記圧力変化の記録を完了すると、記録が完了した旨を示すメッセージ、及び、前記ガイドワイヤを動かしてよい旨を報知するメッセージのうち少なくとも一つをユーザに提示する請求項1に記載のX線診断装置。
- 前記制御ユニットは、前記記録ユニットへの前記圧力データの記録を完了すると、該記録に係る圧力データを示すグラフを前記表示ユニットに表示させる請求項1に記載のX線診断装置。
- 前記制御ユニットは、前記記録ユニットへの前記圧力データの記録を完了すると、該記録に基づいて冠血流予備量比の値を算出し、該値が所定の閾値以下の場合には、再度、同一の前記計測位置における前記圧力データを前記記録ユニットに記録する請求項1に記載のX線診断装置。
- 前記制御ユニットは、前記計測位置に係る前記圧力データの記録を完了すると、前記表示ユニットに表示された前記X線画像上の当該計測位置に対応する部位の表示態様を変化させる請求項1に記載のX線診断装置。
- 前記制御ユニットは、前記計測位置に係る前記圧力データの記録が未完了の場合、前記表示ユニットに表示される前記X線画像上において、当該計測位置に対応する部位に係る前記圧力データを記録するようユーザに促す警告表示を行う請求項1に記載のX線診断装置。
- ガイドワイヤに設けられた圧力センサを用いて収集された圧力データを記録する記録ユニットと、
画像上において指定された計測位置と前記圧力センサの位置との位置関係に基づいて、前記圧力データの前記記録ユニットへの記録を制御する制御ユニットと、
を具備する血流機能検査装置。
