Abstract
Ein Lithotripsiegerät (2) umfasst ein Röntgensystem (3) mit einem in einer Mittelebene des Röntgensystems (3) angeordneten Isozentrum (I) und einen auf das Isozentrum (I) ausrichtbaren Stoßwellenkopf (6), der an einer Tragvorrichtung (8) gelagert ist. Eine besonders einfache und stabile Verstellmechanik des Stoßwellenkopfs ist erreicht, indem die Tragvorrichtung (8) eine erste linearverfahrbare Verstelleinheit (20) zur horizontalen Verstellung des Stoßwellenkopfs (6), eine zweite linearverfahrbare Verstelleinheit (24) zur Höhenverstellung des Stoßwellenkopfs (6) und eine rotatorische 15 Verstelleinheit (22) für eine Rotation des Stoßwellenkopfs (6) aufweist.

Lithotripsiegerät
Die Erfindung betrifft ein Lithotripsiegerät umfassend ein
Röntgensystem mit einem in der Mittelebene des Röntgensystems angeordneten Isozentrum und einen auf das Isozentrum ausrichtbaren Stoßwellenkopf, der an einer Tragvorrichtung gelagert ist.
Die Lithotripsie stellt ein medizinisches Verfahren zur ex- trakorpularen Zertrümmerung von in den Nieren oder in den Harnwegen abgelagerten Konkrementen dar. Ein Lithotripsiegerät besteht in der Regel aus einem Röntgensystem zur Ortung und Visualisierung des Konkrements und einem Therapiesystem zur Zertrümmerung des Konkrements. Das Röntgensystem umfasst üblicherweise einen Röntgen-C-Bogen, der um sein Isozentrum orbital verfahrbar ist. An den Schenkelenden des Röntgen-C- Bogens sind eine Röntgenquelle und ein Bildverstärker an- geordnet. Der C-Bogen umgreift bei seinem Einsatz teilweise einen Patiententisch, so dass das Konkrement im Isozentrum liegt. Das Therapiesystem umfasst einen Stoßwellenkopf mit dessen Hilfe Stoßwellen (Schalldruckwellen) erzeugt werden. Um Schaden in dem das Konkrement umgebenden Gewebe weitgehend zu vermeiden, ist es erforderlich, den Fokuspunkt des Sto߬ wellenkopfs im Konkrement bzw. im Isozentrum des Röntgen-C- Bogens zu positionieren.
Aus der DE 103 42 016 Al ist eine Lithotripsie-Einrichtung bekannt, bei der der Stoßwellenkopf von einem Gelenkarm ge¬ tragen ist. Der Gelenkarm weist drei zueinander parallele Drehachsen bzw. drei Drehgelenke auf, durch welche die Posi¬ tionierung des Stoßwellenkopfs erfolgt.
Der Erfindung liegt die Aufgabe zugrunde ein Lithotripsiege¬ rät anzugeben, das sich durch eine steife Bauweise und eine einfache Verstellmechanik auszeichnet.
Die Aufgabe wird erfindungsgemäß gelöst durch ein Litho- tripsiegerät umfassend ein Röntgensystem mit einem in einer Mittelebene des Röntgensystems angeordneten Isozentrum und einen auf das Isozentrum ausrichtbaren Stoßwellenkopf, der an einer Tragvorrichtung gelagert ist, wobei die Tragvorrichtung eine erste linearverfahrbare Verstelleinheit zur horizontalen Verstellung des Stoßwellenkopfs, eine zweite linearver- fahrbare Verstelleinheit zur Höhenverstellung des Stoßwellenkopfs und eine rotatorische Verstelleinheit für eine Rotation des Stoßwellenkopfs aufweist.
Die Erfindung gestattet eine wesentliche Vereinfachung der Trag- und Bewegungsmechanik des Stoßwellenkopfs, indem die Positionierung des Stoßwellenkopfs nun über zwei Transla- tionsbewegungen und eine Rotationsbewegung erfolgt. Dies ermöglicht einen besonders einfachen und robusten Aufbau der linearverfahrbaren Verstelleinheiten, der zu einer hohen Steifigkeit der Tragvorrichtung und somit des gesamten Litho- tripsiegeräts führt. Darüber hinaus lässt sich der Stoß- wellenkopf aufgrund seiner Rotierbarkeit um bis zu 360° in zahlreiche Behandlungspositionen verfahren, so dass eine maximale Präzision der Behandlung gewährleistet ist.
Betrachtet in einem kartesischen Koordinatensystem erfolgt die Höhenverstellung des Stoßwellenkopfs entlang einer z-
Achse und die horizontale Verstellung entlang einer y-Achse. Die Drehbeweglichkeit des Stoßwellenkopfs erfolgt um eine Drehachse, die parallel zur x-Achse des Koordinatensystems verläuft. Das Röntgensystem ist derart gelagert, dass es kei- ne Bewegung entlang einer x-Achse ausführt. Somit erübrigt sich eine horizontale Verstellung des Stoßwellenkopfs in die x-Richtung. Die Mittelebende ist hierbei definiert durch die z-y-Ebene in der das Isozentrum liegt.
Gemäß einer bevorzugten Ausgestaltung sind die linearverfahrbaren Verstelleinheiten derart ausgebildet, dass die Bewegung
zur Höhenverstellung entkoppelt von der Bewegung zur horizontalen Verstellung des Stoßwellenkopfs ist. Der Vorteil dieser Ausgestaltung ist, dass das Verfahren des Stoßwellenkopfs über mechanisch voneinander unabhängige Achsen erfolgt, so dass die an der ersten linearen Verstelleinheit angreif¬ ende Gewichtskräfte des Stoßwellenkopfs auf einem sehr kurzen Weg in die Tragvorrichtung geleitet wird. Weiterhin vorteil¬ haft ist, dass die voneinander entkoppelten Bewegungen eine besonders einfache Steuerung der Verstelleinheiten ermöglich- en.
Gemäß einer weiteren bevorzugten Ausgestaltung stehen die Ii- nearverfahrbaren Verstelleinheiten senkrecht zueinander. Diese Ausgestaltung ist konstruktiv besonders einfach zu rea- lisieren. Zudem wird somit gewährleistet, dass beim Verstell¬ en einer der Verstelleinheiten ein Verfahren des Stoßwellenkopfs in ausschließlich horizontaler oder vertikaler Richtung erfolgt. Dies macht die Steuerung der Verstelleinheiten besonders leicht.
Vorzugsweise ist das Röntgensystem an der Tragvorrichtung gelagert. Dies ermöglicht eine besonders kompakte Bauweise des Lithotripsiegerät . Hierbei sind die Positionen des Röntgen- systems relativ zum Stoßwellenkopf mechanisch festgelegt, so dass keine aufwändigen Berechnungen oder Ortungsvorgänge erforderlich sind, um die Position des Fokuspunkts des Sto߬ wellenkopfs in Bezug auf das Isozentrum zu ermitteln und zu ändern .
Um eine besonders präzise Fokussierung der Stoßwellen im Isozentrum zu erreichen, ist das Röntgensystem derart an der Tragvorrichtung gelagert, dass der Fokuspunkt des Stoßwel¬ lenkopfs in der Mittelebene liegt . Somit wird ein Verlagern des Isozentrums innerhalb der Mittelebene auf keinerlei Weise die Fokussiergenauigkeit des Stoßwellenkopfs beeinträchtigen. Hierdurch ist insbesondere eine orbitale Verfahrbarkeit des Röntgensystems realisierbar, die eine bequeme Erfassung und Überwachung des Konkrements aus unterschiedlichen Richtungen
ermöglicht. In jeder Position entlang der orbitalen Fahrbahn des Röntgensystems bleibt das Isozentrum in der Mittelebene, ebenso wie der Fokuspunkt des Stoßwellenkopfs und die Sto߬ wellen können genau auf des Isozentrum gerichtet werden.
Eine besonders hohe Stabilität der ersten linearverfahrbaren Verstelleinheit ist dadurch erreicht, dass diese bevorzugt aus zwei Verstellarmen ausgebildet ist, zwischen denen der Stoßwellenkopf drehbar gelagert ist. Durch die beiden Ver- stellarme ist eine spiegelsymmetrische Ausgestaltung der La¬ gerung des Stoßwellenkopfs erreicht, die eine sehr große Sta¬ bilität nach sich zieht . Insbesondere wird dadurch einer Ver¬ formung der Verstelleinheit oder gar eine seitliche Verkipp¬ ung des Geräts aufgrund unsymmetrischer Beanspruchung verhin- dert . Der Stoßwellenkopfs wird in einer besonders stabilen
Lage gehalten und die Kraft, mit der er auf die Verstellarme wirkt, wird beidseitig auf dem kürzesten Weg in die Tragvor¬ richtung eingeleitet.
Zweckdienlicherweise sind die Verstellarme teleskopartig aus¬ gebildet. Das teleskopartige Ein- und Ausfahren der Verstell¬ arme bietet eine platzsparende Lösung.
Weitere herkömmliche und bewährte Baukomponente werden einge- setzt, um eine einfache Verfahrbarkeit der zweiten linearver- fahrbaren Verstelleinheit zu gewährleisten. Deshalb umfasst die zweite linearverfahrbare Verstelleinheit vorteilhafter¬ weise zwei Führungen, die voneinander beabstandet an der Tragvorrichtung angeordnet sind. Die Führungen, die insbeson- dere Linearführungen sind, gewährleisten eine Bewegung des
Stoßwellenkopfs nur in vertikaler Richtung und somit ist die Höhenverstellung von der horizontalen Bewegung des Stoßwellenkopfs entkoppelt. Das Führungs-/Antriebssystem ist insbe¬ sondere nach Art vertikal ausgerichteter Kugelumlaufspindein ausgebildet, die sich durch geringe Reibung bzw. Verschleiß auszeichnen.
Um eine besonders einfache Ankopplung der ersten mit der zweiten Verstelleinheit zu gewährleisten, sind die Verstell¬ arme zweckdienlicherweise jeweils in einer der Führungen ge¬ halten. Zudem ist jeder der Verstellarme insbesondere in meh- reren Punkten oder geradlinig in den Führungen gelagert, so dass eine besonders feste Konstruktion vorliegt, bei der die am anderen Ende des Verstellarms wirkende Gewichtskraft des Stoßwellenkopfs eine vernachlässigbar geringe Biegung des Verstellarms verursacht .
Eine Ausführungsvariante des Lithotripsiegeräts ist anhand einer Zeichnung näher erläutert. Hierin zeigt die einzige Figur eine perspektivische Darstellung eines Lithotripsiegeräts mit einer rotatorischen und zwei linearverfahrbaren Verstell- einheiten.
In der Figur ist ein Lithotripsiegerät 2 zur extrakorpularen Zertrümmerung von einem Konkrement, z.B. einem Nierenstein, dargestellt. Das Lithotripsiegerät 2 umfasst ein Röntgen- System 3 mit einem Röntgen-C-Bogen 4, einen Stoßwellenkopf 6 und eine Tragvorrichtung 8. Sowohl der C-Bogen 4, als auch der Stoßwellenkopf 6 sind an der Tragvorrichtung 8 gelagert. Der C-Bogen 4 ist orbital verfahrbar und dessen Verfahrrichtung ist durch den Pfeil 10 angezeigt. An den Schenkelenden des Röntgen-C-Bogens 4 sind eine Röntgenquelle 12 und eine Bildgebende Einheit 14 angeordnet, die ebenfalls Teil des Röntgensystems 3 sind. Die Bildgebende Einheit 14 umfasst in diesem Ausführungsbeispiel einen Bildverstärker und weitere Komponente zur Detektion eines das Konkrement durchstrahl- enden Röntgenstrahls und einer Bildgebung des Konkrements. Durch die Röntgenquelle 12 und die Bildgebende Einheit 14 verläuft eine Strahlachse 16, die das Isozentrum I des Rönt¬ gen-C-Bogens 4 enthält. Im Betrieb des Lithotripsiegeräts 2 umgreift der C-Bogen 4 einen hier nicht dargestellten Pati- ententisch, auf dem ein Patient gelagert ist. Dabei wird das zu zertrümmernde Konkrement genau im Isozentrum I positio¬ niert .
Die y-z-Ebene, in der die Strahlachse 16 liegt, ist die Mit¬ telebene, die das Isozentrum I enthält. Der Stoßwellenkopf 6 ist derart an der Tragvorrichtung 8 befestigt, dass sein Fo¬ kuspunkt F sich ebenfalls immer in der Mittelebene befindet. Somit wird der Fokuspunkt F während der Therapie nur über ein Drehen und ein Verfahren des Stoßwellenkopfs 6 in der Mit¬ telebene zum Isozentrum I positioniert.
Die Zertrümmerung des Konkrements erfolgt über gebündelte Schallwellen / Stoßwellen, die vom Stoßwellenkopf 6 erzeugt werden. Die erzeugten Stoßwellen sind in einem Fokuspunkt F fokussiert, der während der Behandlung mit dem Isozentrum I zusammenfällt, in dem auch das Konkrement liegt, um Schaden im Gewebe außerhalb des Konkrements zu vermeiden.
Um den Fokuspunkt F in die korrekte Position zu bringen, ist eine translatorische Beweglichkeit des Stoßwellenkopfs 6 ent¬ lang der z-, und y-Achsen und eine Rotation um eine Drehachse 18 parallel zur x-Achse eines kartesischen Koordinatensystems ermöglicht .
Die horizontale Verstellung des Stoßwellenkopfs 6 erfolgt ü- ber eine linearverfahrbare Verstelleinheit 20, die in diesem Ausführungsbeispiel nach Art zwei Verstellarme ausgebildet ist. Die Verstellarme 20 sind an der Tragvorrichtung 8 gela¬ gert und sind über ein Antriebssystem teleskopartig ein- und ausfahrbar. Somit werden die auf die Verstelleinheit 20 wir¬ kenden Kräfte symmetrisch verteilt, so dass die Beanspruchung jedes Verstellarms 20 gering ist. Darüber hinaus werden diese Kräfte auf einem besonders kurzem linearen Weg in die Trag¬ vorrichtung 8 eingeleitet.
Der Stoßwellenkopf 6 ist über eine rotorische Verstelleinheit 22, die in dieser Ausführung aus zwei Drehlagern 22 und einer nicht dargestellten Welle besteht, zwischen den zwei Verstellarmen drehbar gehalten. Dabei kann der Stoßwellenkopf 6 um eine durch die Drehlager verlaufende Drehachse 18 um bis zu 360° rotieren. Die Drehlagerung des Stoßwellenkopfs 6 er-
folgt insbesondere über ein Getriebe, z.B. ein Planetenge¬ triebe oder ein Harmonic-Drive-Getriebe . Zur Arretierung des Stoßkopfs 6 in der gewünschten Position ist ein in diesem Ausführungsbeispiel nicht dargestelltes Arretierelement, bei- spielweise eine Bremsbacke, vorgesehen, das zum Festhalten des Stoßwellenkopfs 6 gegen die Welle gedrückt wird. Die zum Erzeugen der Stoßwelle erforderliche elektrische Energie wird im Bereich der Drehlager 22 beispielweise über einen Schleifring oder eine Flüssig-Metall-Verbindung in den drehbaren Stoßwellenkopfs 6 eingeleitet.
An ihrem anderen Ende ist jeder der Verstellarme 20 über ein Führungs-/Antriebssystem mit Linearführungen 24 in zwei Punkten an der Tragvorrichtung 8 befestigt. Die Führungen 24 sind senkrecht zu den Verstellarmen 20 ausgebildet und ermöglichen eine Höhenverstellung der Verstellarme 20 bzw. des Stoßwel¬ lenkopfs 6. Die Führungen 24 auf jeder Seite der Trag¬ vorrichtung 8 sind insbesondere Elemente einer hier nicht dargestellten motorisch angetriebenen Kugelumlaufspindel, die im Wesentlichen ein vertikal gerichtetes Gewinde und eine Schraubenmutter aufweist. Die Verstellarme 20 sind an der Schraubenmutter befestigt und gleichzeitig über die Führungen 24 gelagert. Die Rotation des Gewindes wird in eine transla¬ torische Bewegung der Mutter umgewandelt, wodurch die Positi- onierung der Verstellarme 20 in Vertikalrichtung erfolgt.
Durch die Art der Lagerung der Verstellarme 20 ist die Höhen¬ verstellung des Stoßwellenkopfs 6 entkoppelt von seiner hori¬ zontalen Verstellung, d.h. das vertikale Verfahren des Stoß- wellenkopfs 6 erfolgt unabhängig von dem Aus- oder Einfahren der Verstellarme 20. Die Rotation des Stoßwellenkopfs 6 er¬ folgt ebenfalls unabhängig von den translatorischen Bewegungen der Verstellarme 20. Somit wird der Stoßwellenkopf 6 über mechanisch unabhängige Achsen verfahren, was die Ansteuerung des Bewegungsablaufs deutlich vereinfacht .
Claims
1. Lithotripsiegerät (2) umfassend ein Röntgensystem (3) mit einem in einer Mittelebene des Röntgensystems (3) angeordneten Isozentrum (I) und einen auf das Isozentrum (I) ausrichtbaren Stoßwellenkopf (6), der an einer Trag¬ vorrichtung (8) gelagert ist, dadurch gekennzeichnet, dass die Tragvorrichtung (8) eine erste linearverfahrbare Verstelleinheit (20) zur horizontalen Verstellung des Stoßwellenkopfs (6), eine zweite linearverfahrbare Verstelleinheit (24) zur Hö¬ henverstellung des Stoßwellenkopfs (6) und eine rota¬ torische Verstelleinheit (22) für eine Rotation des Stoßwellenkopfs (6) aufweist.
2. Lithotripsiegerät (2) nach Anspruch 1, dadurch gekennzeichnet, dass die linearverfahrbaren Verstelleinheiten (20, 24) derart ausgebildet sind, dass die Bewegung zur Höhenverstellung entkoppelt von der Be- wegung zur horizontalen Verstellung des Stoßwellenkopfs (6) ist.
3. Lithotripsiegerät (2) nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass die linearverfahrbaren Ver- Stelleinheiten (20, 24) senkrecht zueinander stehen.
4. Lithotripsiegerät (2) nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass das Röntgensystem (3) an der Tragvorrichtung (8) gelagert ist.
5. Lithotripsiegerät (2) nach Anspruch 4, dadurch gekennzeichnet, dass das Röntgensystem (3) der¬ art an der Tragvorrichtung (8) gelagert ist, dass ein Fokuspunkt (F) des Stoßwellenkopfs (6) in der Mittel¬ ebene liegt .
6. Lithotripsiegerät (2) nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die erste linearverfahrbare Verstelleinheit (20) aus zwei Verstellarmen ausgebildet ist, zwischen denen der Stoßwellenkopf (6) drehbar gela¬ gert ist.
7. Lithotripsiegerät (2) nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Verstellarme (20) tele¬ skopartig ausgebildet sind.
8. Lithotripsiegerät (2) nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die zweite linearverfahr¬ bare Verstelleinheit (24) zwei Führungen umfasst, die voneinander beabstandet der Tragvorrichtung (8) angeordnet sind.
9. Lithotripsiegerät (2) nach einem der Ansprüche 6 oder 7 und Anspruch 8, dadurch gekennzeichnet, dass die Verstellarme (20) je¬ weils in einer der Führungen (24) gehalten sind.
