Abstract
Description
The invention relates to medicine, namely to dentistry, and can be used for regeneration of the jaw with atrophy of the alveolar process.
Deficiency of bone tissue of the alveolar ridge of the jaw is an urgent problem in restorative dentistry.
Conditions like: post-extraction atrophy of the alveolar process of the upper and lower jaw, cortical plate defect in the form of digiscence, fenestration, intraosseous furcation defect of the jaw during implantation require bone augmentation.
Cawood J.I. & Howell R.A. [1].In accordance with the classification of Cawood J.I. & Howell R.A. (1988) to eliminate atrophy of the alveolar ridge of the jaw III-VI class, bone augmentation is required horizontally and vertically [1].
[2].Targeted bone regeneration in case of atrophy of the alveolar bone of the jaw involves increasing the height and width of the alveolar bone due to the use of a mixture of purified recombinant human platelet growth factor in combination with autogenic bone, organic minerals obtained from beef bone, and a barrier membrane after mucosal removal of the mucosa skeletons of the bone are placed in the area of the bone defect covered with a collagen membrane and overlap with the mucoperiosteal l with a patch [2].
[3].Closest to the technical solution to the proposed method is a method of plastic surgery of the alveolar process of the upper jaw during its atrophy [3].
For plasty of the alveolar process of the upper jaw, a bone autograft is taken using a piezosurgical apparatus, with which a special U-shape is given to the bone autograft using a vertical longitudinal cut corresponding to the size of the recipient zone. Under infiltration anesthesia, a mucosal incision is made along the crest of the alveolar process of the upper jaw and two vertical incisions. Peel off a full mucoperiosteal flap. A rectangular autologous bone autograft is collected along the length of the recipient zone in the retro-molar region of the lower jaw using a piezosurgical apparatus. The edges of the resulting bone defect are smoothed using a piezosurgical apparatus. A rectangular bone autograft with a piezosurgical apparatus is given a U-shape by means of a vertical longitudinal cut, corresponding in size to the recipient zone. The recipient area is perforated in several places, a bone autograft is fitted in and fixed with titanium microscrews. The resulting undercuts are filled with a mixture of autogenous bone chips and synthetic osteoplastic material, and the top is closed with a collagen resorbable membrane. They mobilize the mucoperiosteal flap. The flap is laid on the wound without tension. The wound is sutured with interrupted sutures. The formed defect in the donor zone is filled with osteoplastic material and covered with a collagen membrane. The wound is sutured with interrupted sutures
The disadvantages of this solution are two vertical incisions in the upper jaw, an additional operation to collect a bone autograft in the retro-molar region of the lower jaw, and fixation of the bone autograft with titanium microscrews, which must be removed before implantation.
The proposed solution is based on the clinical results of the method of directed bone regeneration of the jaw with atrophy of the alveolar bone.
The aim of the invention is the reduction of areas of the surgical fields, augmentation during atrophy of the alveolar process of the lower jaw without the use of titanium microscrews.
The goal is achieved by reconstructive reconstruction of the alveolar ridge with secondary, partial or complete adentia of post-extraction atrophy of the alveolar ridge.
The method is as follows.
In the preoperative period, the patient undergoes clinical, laboratory and x-ray examinations (targeted intraoral images, orthopantomograms, computer tomograms with three-dimensional reconstruction of the image).
Augmentation of the height and width of the alveolar process of the lower jaw is carried out on the basis of modeling the clinical situation in the oral cavity and the condition of the bone tissue of the perceiving bed.
On gypsum models of the upper and lower jaw determine:
- прикус;- bite;
- окклюзионную поверхность;- occlusal surface;
- высоту, ширину и степень атрофии альвеолярного отростка.- the height, width and degree of atrophy of the alveolar process.
According to computed tomography determine:
- высоту и ширину альвеолярного отростка;- the height and width of the alveolar ridge;
- состояние альвеолярного отростка нижней челюсти по отношению к нижнечелюстному каналу;- the state of the alveolar process of the lower jaw in relation to the mandibular canal;
- состояние краевого отдела альвеолярного отростка вокруг сохранившихся зубов;- the state of the marginal division of the alveolar process around the remaining teeth;
- форму височно-нижнечелюстного сустава.- the shape of the temporomandibular joint.
After examination and diagnosis, make up a treatment plan. The stage of the operation of directed bone regeneration of the jaw is carried out during atrophy of the alveolar process to create optimal conditions for the subsequent operation of dental implants.
Under infiltration and conduction anesthesia along the crest of the alveolar process of the lower jaw, a longitudinal incision of the mucous membrane and periosteum is performed.
The mucous membrane is exfoliated from the periosteum from the lingual and buccal sides.
For soft tissue mobility, the mucoperiosteal flaps exfoliate as coronally as possible to the line of transition of the alveolar process to the basal bone.
In the area of augmentation of the alveolar process of the lower jaw, decortication of the external cortical plate is carried out, and a receptive bed is formed.
The formed receiving bed is filled with a mixture of allogeneic graft with autogenous bone chips.
The entire augmentate is covered with a collagen membrane, its edges are tucked under the periosteum and the periosteum is sutured with resorbable suture material.
