Abstract
Description
The invention relates to surgery, namely to the replenishment of bone defects using a new plastic biomaterial.
2000. - №2. О. Опыт пластики ороантрального соустья у больных одонтогенным верхнечелюстным синуситом / International journal of applied and fundamental research №11, 2011. - С.99-100).Specialists of various fields of medicine: traumatologists and orthopedists, dental surgeons, maxillofacial surgeons, otorhinolaryngologists face bone deficiency during reconstructive operations. With extensive bone defects, it is quite difficult to use autologous bone tissue due to its deficiency in the donor zone. The implementation of osteoplastic operations with other types of autografts (fascial-muscular, mucous-periosteal, fascial-periosteal flaps, etc.), as a rule, does not lead to the formation of an adequate (bone) regenerate (Belchenko V.A., Prityko A. G., Gonchalov GV The use of autografts of the cranial vault to eliminate defects of the alveolar ridge of the upper jaw // New in Dentistry. 2000. - No. 2. - P.32-34; Gyusan AO. Experience of plastic surgery of an oroantral anastomosis in patients with odontogenic maxillary sinusitis / International journal o f applied and fundamental research No. 11, 2011. - S.99-100).
- М., 2006. - 23 с.).The use of synthetic materials to fill bone defects is widespread. In particular, wollastonitapatite bioceramics are used to close bone defects in the maxillofacial region. The latter is introduced into the bone defect after its preliminary preparation, which consists in the removal of granulations, connective tissue, etc. from the future field of implantation (Chugunov A.A. Plastic surgery of bone defects of the jaws with wollastonite-apatite bioceramics: abstract of dissertation ... Candidate of Medical Sciences. - M., 2006. - 23 pp.).
К., Ковалевский A.M. - 255 с.).For the purpose of contour plasty and to increase the height of the alveolar arches of the edentulous jaws, porous blocks of hydroxylapatite or its mixture with tricalcium phosphate are used. Successful use of this type of bioceramics requires careful adaptation of the porous block to the contours of the perceiving bone bed (Balin V.N., Iordanishvili A.K., Kovalevsky A.M. Practical periodontology. St. Petersburg: Peter, 1995. - 255 p.).
However, these methods have the following disadvantages:
- 2004. - №1. - С.23-31; Narins R.S. Injectable Skin Fillers / R.S.Narins, P.H.Bowman // Clin. Plast. Surg. - 2005. - Vol.32, №2. - P.151-162).1. Synthetic implants are foreign to the human body and support a chronic inflammatory process in the surrounding tissues at the border of the living environment and the polymer. At the end of these processes, a coarse-fibrous connective tissue capsule is formed around the implant (Results of correction of soft tissue defects with polyacrylamide gel / A.A. Adamyan, M.A.Sulamanidze, Yu.V. Romashov [et al.] // Annals of plastic, reconstructive and aesthetic Surgery. - 2004. - No. 1. - P.23-31; Narins RS Injectable Skin Fillers / RSNarins, PHBowman // Clin. Plast. Surg. - 2005. - Vol.32, No. 2. - P.151. -162).
- 22 с.).2. Sorption of a large number of microorganisms on synthetic material can lead to inflammatory complications during surgical intervention, for example, in the specific conditions of an infected periodontal surgical wound (I. Bolonkin. Justification for the use of a combined implant in patients with atrophy of the alveolar processes of the jaw (clinical experiment. Research. ): abstract of thesis ... Candidate of Medical Sciences. - Samara, 2008. - 22 p.).
К., Ковалевский A.M. - 255 с.).3. The pronounced radiopacity of many synthetic materials makes it practically impossible to observe the dynamics of bone tissue restoration (Balin V.N., Iordanishvili A.K., Kovalevsky A.M. Practical periodontology. St. Petersburg: Peter, 1995. - 255 p.).
Known methods for filling bone deficiency with crushed bone graft. In clinical practice, such biomaterials can be used in the form of large fragments, crumbs, powder, paste (Bone grafting in dentistry and maxillofacial surgery. Osteoplastic materials / Edited by A.S. Pankratov. - M.: Publishing House of BINOM, 2011 .-- 86-92 pp.). The essence of the method is that after preparation of the bone bed, by excising pathologically changed soft tissues, the resulting bone defect is filled with the necessary amount of crushed bone allograft or paste based on it. The paste is prepared by adding blood products (platelet mass, fibrin film, etc.) or whole blood.
The disadvantages of the prototype are:
1. The need to combine large fragments of a bone allograft with finely divided biomaterials to better adapt to the shape of the tissue bed and to avoid the traumatic effect of the edges and surfaces of bone fragments on the soft tissues surrounding the transplantation area.
2. The replacement of the bone graft occurs according to the peripheral type, which implies the initial resorption of the transplanted biomaterial with the subsequent gradual formation of bone regenerate.
The objective of the invention is to increase the effectiveness of osteoplastic operations through the stimulation of osteogenesis.
The problem is solved by the method of filling bone defects, including preparing the bone bed by excising pathologically changed tissues from the future transplantation area and thorough hemostasis, then filling the bone defect with a transplant - allogeneic biomaterial, crushed to a finely dispersed or finely divided state, or a paste prepared by mixing finely dispersed biomaterial blood in the form of platelet mass, or fibrin film, or other blood products and, or with whole blood, in which, unlike the prototype, allogeneic costal cartilage is used as biomaterial, which is treated with anionic and rolled detergents for the purpose of membranolysis, then washed with water until a neutral (phenolphthalein) reaction is placed in cold acetone (4 ° C) for 24 hours for dehydration, after which it is dried under vacuum to constant weight, and dispersion is carried out in a rotary impact mill to particle sizes from 30 μm to 5 mm, then a finely divided transplant is isolated in the fractionation unit atm with a particle size of 30-300 m or coarse particle size of 500 microns - 5 mm.
The specified technical result of the invention is achieved due to the following. The use of a cartilaginous autograft for osteogenesis is known (A. A. Radkevich. Reconstructive surgery of the alveolar processes of the jaws: abstract of thesis ... Doct. Medical Sciences. - Irkutsk, 2002. - 33 p.). This biomaterial, when replanted into the bone bed, is replaced by lamellar bone tissue according to the type of enchondral ossification. Moreover, even large fragments of the indicated transplant are devoid of surfaces and edges capable of injuring the recipient zone. However, the known cartilage autograft does not provide effective osteogenesis, since it does not have high regenerative properties. The proposed allogeneic cartilage graft is made from donor costal (hyaline) cartilage using Alloplant biomaterial technology, which provides high regenerative properties (RF Patent No. 2189257 IPC A61L 27/00, published on September 20, 2002). The biomaterial processed in this way has extremely low antigenic properties and provides selective tissue growth at the site of the implanted biomaterial and stimulation of osteogenesis.
The method is as follows. In the presence of bone defects, for example, at the preliminary stage of joint prosthetics, a bone bed is prepared by excising pathologically changed soft tissues from the area of subsequent transplantation, and thorough hemostasis is performed. Then, to fill in the resulting bone defect, the required amount of coarse-grained (for filling large bone cavities: during joint prosthetics, after antromastoidotomy or radical surgery on the middle ear) or fine-grained (when closing the oroantral fistula or performing sinus lift), allogeneic cartilage graft or paste made from fine-grained is introduced. cartilage graft (to fill in small bone defects). The paste is prepared by adding blood preparations in the form of platelet mass, fibrin film, etc. or whole blood.
An allogeneic cartilage graft is made as follows. Allogeneic costal cartilage is treated with anionic and cationic detergents for the purpose of membranolysis using Alloplant biomaterial technology (RF Patent No. 2189257 IPC A61L 27/00, published on September 20, 2002). Then the tissues are washed with water until neutral (according to phenolphthalein) reaction, placed in cold acetone (4 ° C) for 24 hours for dehydration, and then dried under vacuum to constant weight.
Dispersion is carried out on a rotary impact mill to particle sizes from 30 microns to 5 mm. To prevent thermal effects on the crushed tissues during the grinding process and to give them additional fragility, cooling with liquid nitrogen is used. Then, a crushed graft of two forms is isolated at the fractionation unit: 1) finely divided with a particle size of 30-300 microns; 2) coarse with a particle size of 500 microns - 5 mm. The dispersed material is Packed in glass bottles, corked with rubber stoppers, wrapped in aluminum caps and sterilized with radiation at a dose of 1.5 Mrad.
В отдаленные сроки (на 180-е сутки) в области трансплантации формируется органоспецифический костный регенерат, имеющий достаточный объем (66,9±2% от изначального введенного) и плотность (49,7±7% - в пределах нормы в пластинчатой костной ткани) (Фиг.1).Before proceeding to the clinical application of the method, a series of experimental studies on laboratory animals was carried out. Various models of allotransplantation into defects of tubular and mixed bones were created. When filling bone defects using a dispersed allogeneic cartilage graft in the early stages, a weakly expressed reaction of the tissue bed is noted. In the long term (on the 180th day), an organ-specific bone regenerate is formed in the transplantation area, having a sufficient volume (66.9 ± 2% of the initial amount injected) and density (49.7 ± 7% - within the normal range in lamellar bone tissue) (Figure 1).
5%.The proposed method operated on 10 patients with oroantral fistulas, 23 patients who underwent sinus lift surgery (raising the bottom of the maxillary sinus), 18 patients after antromastoidotomy or radical surgery on the middle ear (to fill the mastoid cavity and restore the posterior wall of the external auditory meatus), 25 patients when performing prosthetics of large joints (knee, hip). In the early postoperative period, the following complications were observed. Pain was moderately expressed in 4 patients, edema in 3 patients, suppuration of the wound was not in any patient. Long-term results of treatment were evaluated one year and two years after the operation. Good results were achieved in 85% of cases, satisfactory in 10%, unsatisfactory 5%.
On the example of an operation to raise the bottom of the maxillary sinus using an allogeneic cartilage graft up to 3 months after the intervention, the regenerate bone tissue has a higher density (634.1 ± 58.7 HU), which is 1.6 times higher than normal in individuals the second period of adulthood in the alveolar process. In the distant period, there is a tendency to a decrease in bone density in this region, apparently as a result of the processes of replacement and remodeling of a cartilaginous allograft. After 6 months, the studied parameter was 471 ± 68.3, which indicates the preservation of the structure of bone regenerate.
Thus, the proposed invention improves the efficiency of surgical operations to replace bone defects due to the use of a new plastic biomaterial from allogeneic costal cartilage as a transplant, which stimulates osteogenesis.
