Поставленная цель достигается тем, что в послеоперационном периоде больному осуществляют люмбальную пункцию, катетеризируют субарахноидальное пространство, забирают 5 мл ликвора, инкубируют ликвор с нимустином в количестве 5 мг in vitro при температуре 38°С в течение 30 минут и вводят через катетер в субарахноидальное пространство, процедуру осуществляют 2 раза с промежутком в 7 дней. На 2-й день начинают дистанционную гамматерапию (ДГТ) на ложе удаленной опухоли ежедневно в течение 20 дней до суммарной очаговой дозы 60 Гр. Забирают ликвор больного в объеме 5 мл и инкубируют его с химиопрепаратом - нимустин - 5 мг in vitro при температуре 38°С в течение 30 минут, а затем эндолюмбально вводят больному. Процедуру осуществляют двукратно с промежутком 7 дней. Больная Л-ц Н. С.,1989 г/р, история болезни №483/т, поступила в отд. ОЦНС РНИОИ 20.01.2004 с диагнозом: медуллобзастома IV желудочка. Состояние после субокципитальной краниотомии с субтотальным удалением опухоли (23.12.2003 г., РГМУ, г/а №25924 - 31/03 - медуллобластома). При поступлении: умеренный общемозговой с-м, выраженная мозжечковая атаксия; индекс по Карновскому составил 50%, общесоматический статус по ВОЗ - 3 балла. 20.01.04 г. - 12.02.2004 г. проведен курс ДГТ СОД 60 Гр с интратекальной аутоликворохимиотерапией (АЛХТ) нимустином 10 мг. Перенесла АЛХТ удовлетворительно - осложнений и побочных эффектов нет. С 30.03. по 15.09.2004 г. проведено еще 3 курса интратекальной АЛХТ - по 10 мг нимустина. Переносимость АЛХТ удовлетворительная - осложнений и побочных эффектов нет. На РКТ головного мозга 14.12.2004 г. - в зоне удаления опухоли левой гемисферы мозжечка рецидива нет, ликворная полость, сообщающаяся с IV желудочком. Остальные отделы мозга - без очагов. Срединные структуры без смещения. После 4 курсов интратекальной химиотерапии состояние больной удовлетворительное, отмечается незначительная общемозговая симптоматика.
