Суть способа заключается в гальванизации области поражения (бедро, голень), проводимой во время приема больными лекарственных капельниц. Курс лечения 12-15 процедур ежедневно по 30 мин. Через вторую канюлю 2 в просвет катетера вводится серебряная проволока 6, конец которой через резиновую заглушку выведен наружу и соединен с клеммой аппарата для гальванизации 4 с соблюдением правил электроизоляции 8. Эта проволока в просвете катетера играет роль активного электрода. Кроме того, воздействие постоянного тока непосредственно на ишемизированные ткани стимулирует трофические и энергетические процессы в них, уменьшает отек, оказывает анальгезирующее действие. Сила тока 6-8 мА (как при гальванизации стандартным методом). Количество процедур на курс лечения 5-7 (в качестве предоперационной подготовки), 12-28 (в качестве самостоятельного метода лечения). Продолжительность процедуры 15-30 мин (стандартное время гальванизации). Данный способ может быть использован в качестве подготовки больного в реконструктивной операции путем более быстрой стабилизации ишемии и адаптации тканей к предстоящей реваскуляризации, а также как самостоятельный метод лечения, если отсутствуют условия для проведения реконструктивной операции. Кроме того, способ может быть использован при лечении диабетических флегмон стопы и длительно не заживающих трофических язв на почве варикозной или посттромботической болезни вен нижних конечностей. Срок предоперационного лечения больных известным способом составляет 10-15 дней, а при использовании предложенного способа 5-7 дней. Количество ранних послеоперационных тромбозов после известных способов предоперационной подготовки достигает 40%, а при использовании предложенного способа 14%. Всего больных, которым лечение в предоперационном периоде проводилось с использованием предложенного способа 32, известным способом 54. Осложнений, связанных с самим способом, не было. Белгорода 27 января 1998 г. с жалобами на постоянные боли в пальцах левой стопы, онемения их, язву на первом пальце левой стопы, отек стопы и нижней трети голени. Болен около двух месяцев. Обследование: УЗДГ - ПЛИ 0,3 слева, 1,0 справа. Артериография: не контрастируется левая поверхностная бедренная артерия и передняя берцовая артерия. Диагноз: атеросклероз. Оклюзия левой ПБА и передней большеберцовой артерии. ХАН III. Трофическая язва первого пальца левой стопы. 28 января операция: катетеризация левой нижней надчревной артерии. Проводилась длительная внутриартериальная инфузия (трентал, ксантинола никотинат, гордокс, ампиокс, солкосерил) на фоне воздействия постоянного электрического тока предложенным методом в течение 5-ти дней. На второй день больной отметил исчезновение болей покоя. Отек исчез к третьему дню, язва подсохла и уменьшилась в размерах. 3-го февраля больному произведена операция левосторонняя бедренно-подколенное аутовенозное шунтирование. В послеоперационном периоде отчетливая пульсация на задней берцовой артерии. ПЛИ 0,9. Раны зажили первичным натяжением. Швы сняты на восьмой день. Больной выписан на десятый день после реконструктивной операции. PLI 0.9. The wounds healed by primary intention. Sutures were removed on the eighth day. The patient was discharged on the tenth day after reconstructive surgery.
