Abstract
Det beskrives metabolitter av [2-(3-metoksyfenyl)-cykloheksylmetyl]-dimetylamin som frie baser og/eller i form av fysiologisk akseptable salter, de tilsvarende medikamenter, anvendelsen av [2-(3- metoksyfenyl)-cykloheksylmetyl]-dimetylamin og metabolittene derav for fremstilling av et medikament for anvendelse av depresjoner, samt metoder for behandling av depresjoner.
Description
Foreliggende oppfinnelse angår metabolitter av [2-(3-metoksyfenyl)-cykloheksylmetyl]-dimetylamin som frie baser og/eller i form av fysiologisk akseptable salter, tilsvarende legemiddel samt anvendelse av [2-(3-metoksyfenyl)-cykloheksylmetyl]-dimetylamin og dennes metabolitter for fremstilling av et legemiddel for behandling av depresjoner.
Depresjon er en forstyrrelse av affektiviteten der et depressivt syndrom står i forgrunnen, hvorved depressivt er forbundet med negativt stemning hhv. betyr tristhet. De for terapi anvendte antidepressiva er også viktige adjuvanter for smerteterapi og (R. van Schayck et al. 1998, MMP, 10, 304-313; Jung et al. 1997, J. Gen. Intern. Med. 12, 384-389; Onghena og Van Houdenhove 1992, Pain, 49, 205-219; Feuerstein 1997, Diagnostik und Therapie, 3,213-225; Rowbotham 1997, The Pain Medicine Journal Club Journal, 3, 119-122), særlig ved kroniske smertetilstander da den vedvarende smertebelastning kan føre til en depressiv stemning hos pasienten. Dette er ofte tilfelle hos kreft-smertepasienter (Berard 1996, Int. Med. J., 3,257-259). Da det til nu ikke finnes noe middel mot smerte med klinisk relevant, antidepressiv aktivstoffkomponent må antidepressiva føyes til som ytterligere medikasjon ved analgetikum administrering. Da kroniske smertepasienter trenger et antall medikamenter, fører en ytterligere administrering av et antidepressiva til en ytterligere belastning av organismen. Av denne grunn og for å forbedre compliance ville en allerede tilstedeværende antidepressiv aktiv bestanddel være av særlig fordel i et analgetisk virkende middel.
Grunnlag for den antidepressive virkning er gjenopptaksinhiberingen av noradrenalin og serotonin. Opioider med en middels opptaksinhibering som for eksempel tramadol har vært kjent i lang tid og anvendes som potente analgetika med lav avhengighets- og tilbøylighetspotensiale (Raffa et al. 1992, Journal of Pharmacology og experimental Therapeutics, 260,278-285; Raffa og Friderichs 1996, Pain Review, 3,249-271). Opptakshemmingkomponenten er dog ikke sterk nok til å kunne utløse en klinisk relevant, antidepressiv effekt.
Ved siden av den egentlige, antidepressive virkning fører gjenopptaksinhiberingen av noradrenalin og serotonin også til en alenestående, analgetisk virkning der synkende smerteinhiberingsbaner aktiveres på ryggmargnivå. I kombinasjon med opiater som også på ryggmargsnivå hemmer videreføring av smerte, fører dette til en vekselsidig potensiering av smerteinhiberingsmekanismen og derved til en spesielt effektiv analgesi.
Spesielt gunstig ville det derfor i denne indikasjon være, særlig behandlingen av depresjon nettopp også i kombinasjon med smerteterapi, å ha en opioidsubstans med en relativt sterk NA- og/eller 5HT-opptaksinhiberingskomponent som viser en antidepressiv virkning (for eksempel påvist i en egnet, dyreforsøksmodell).
Oppgave for foreliggende oppfinnelse er derfor å tilveiebringe stoffer og særlig opioidstoffer som er egnet for behandling av depresjon og særlig slike som kombinerer denne virkning med analgetisk virkning.
Overraskende er det nu funnet at 2-(3-metoksyfenyl)-cykloheksylmetyl)-dimetylamin og samt også denne metabolitter og derved særlig 3-(2-dimetylaminometyl-cykloheksyl)-fenol, har en slik terapeutisk relevant, antidepressiv aktiv stoffkomponent.
I henhold til dette er oppfinnelsens gjenstand anvendelsen av

i fremstilt form per se eller i form av deres syrer eller baser eller i form av deres salter, særlig de fysiologisk godtagbare salter eller i form av deres solvater og særlig hydrater; for fremstilling av et legemiddel for behandling av depresjoner.
Derved er det spesielt foretrukket når de anvendte forbindelser foreligger som R,R-, fortrinnsvis lR,2R-stereoisomerer.
Med begrepet salt menes enhver form av oppfinnelsens aktive bestanddeler der dette inntar en ionisk form hhv. er ladet og er koplet med et mot-ion (et kation eller anion) hhv. befinner seg i oppløsning. Med dette menes også komplekser av de aktive bestanddeler med andre molekyler og også ioner, særlig i kompleks som er kompleksdannet via ioniske vekselvirkninger. Særlig mener han også (og dette er også en foretrukket utførelsesform av oppfinnelsen) fysiologisk godtagbare salter, særlig fysiologisk godtagbare salter med kationer eller baser og fysiologisk godtagbare salter med anioner eller syrer og salter som er dannet med en fysiologisk godtagbar syre og et fysiologisk godtagbart kation.
Foretrukne salt av de anvendte forbindelser er hydrokloridet.
Med fysiologisk godtagbar menes at substansen og særlig saltet som sådan tåles ved anvendelse i mennesker eller pattedyr, altså virker ikke fysiologisk (for eksempel giftig)-
Med begrepet fysiologisk godtagbart salt med anioner eller syrer, menes innenfor rammen av oppfinnelsen salter av minst en av oppfinnelsens forbindelser, som regel protonert for eksempel på nitrogen, som kation, med minst ett anion, som er fysiologisk godtagbart, særlig ved anvendelse i mennesker og/eller pattedyr. Særlig menes innenfor rammen av oppfinnelsen et salt som er dannet med en fysiologisk godtagbar syre, nemlig saltet av den angjeldende aktive bestanddel med uorganiske hhv. organiske syrer som er fysiologisk godtagbare, særlig ved anvendelse i mennesker og/eller pattedyr. Eksempler på fysiologisk godtagbare salter av spesielle syrer er salter av: salt-, bromhydrogen-, svovel-, metansulfon-, maur-, eddik-, oksal-, rav-, eple-, vin-, mandel-, fumar-, melke-, sitron- eller glutaminsyre, videre l,l-diokso-l,2-dihydrolb6-benzo[d]isotiazol-3-on (sakkarinsyre), monometylsebasinsyre, 5-okso-prolin, heksan-1-sulfonsyre, nikotinsyre, 2-, 3- eller 4-aminobenzsyre, 2,4,6-trimetyl-benzosyre, a-liponsyre, acetylglycin, acetylsalicylsyre, hippursyre og/eller asparaginsyre. Særlig foretrukket er hydrokloridsaltet.
Med begrepet saltet som er dannet med en fysiologisk godtagbar syre, menes innenfor rammen av oppfinnelsen den angjeldende aktive bestanddel med uorganiske hhv. organiske syrer som er fysiologisk godtagbare, særlig ved anvendelsen i mennesker og/eller pattedyr. Spesielt foretrukket er hydrid. Eksempler på fysiologisk godtagbare syrer er: salt-, bromhydrogen-, svovel-, metansulfon-, maur-, eddik-, oksal-, rav-, eple-, vin-, mandel-, fumar-, melke-, sitron- eller glutaminsyre, l,l-diokso-l,2-dihydrolX<6->benzo[</]isotiazol-3-on (sakkarinsyre), monometylsebasinsyre, 5-okso-prolin, heksan-1-sulfonsyre, nikotinsyre, 2-, 3- eller 4-aminobenzoinsyre, 2,4,6-trimetyl-benzoinsyre, a-liponsyre, acetylglycin, acetylsalicylinsyre, hippurrsyre og/eller asparaginsyre.
Med begrepet fysiologisk godtagbart salt med kationer eller baser menes innenfor rammen av oppfinnelsen minst en av oppfinnelsens forbindelser, som oftest en (deprotonert) syre, som anion med minst ett, fortrinnsvis uorganisk, kation som er fysiologisk godtagbart, særlig ved anvendelse i mennesker og/eller pattedyr. Spesielt foretrukket er saltene av alkali- og jordalkalimetallene, men også med NFLt<+>, særlig imidlertid (mono-) eller (di-)natrium-, (mono-) eller (di-)kalium-, magnesium hhv. kalsiumsalter.
Med begrepet salt som er dannet med et fysiologisk godtagbart kation menes innenfor rammen av oppfinnelsen en av oppfinnelsens angjeldende forbindelser som anion med minst et organisk kation som er fysiologisk godtagbart, særlig ved anvendelse i mennesker og/eller pattedyr. Særlig foretrukket er salter av alkali- og jordalkalimetallene, men også med NFLt<+>, særlig imidlertid (mono-) eller (di-)natrium-, (mono-) eller (di-)kalium-, magnesium hhv. kalsiumsalter.
I henhold til foretatte undersøkelser er de anvendte substanser og særlig (lR,2R)-3-(2-dimetylaminometyl-cykloheksyl)-fenol, potente analgetika og antidepressiva, men har også en ytterligere og klinisk relevant, antidepressiv aktiv stoffkomponent. Særlig (lR,2R)-3-(2-dimetylaminometyl-cykloheksyl)-fenol kan anvendes med middels til sterkt akutt og kronisk smerte og muliggjør behandling av den depressive følgesymptomatikk ved kliniske smertetilstander. De fører derved til en tydelig forbedring av den kroniske smertebehandling, da antidepressiva til nu har måttet føyes til som ytterligere medikamenter til den egentlige smerteterapi. (lR,2R)-3-(2-dimetylaminometyl-cykloheksyl)-fenol og de andre substanser som anvendes ifølge oppfinnelsen kan også anvendes uavhengig av deres analgetiske virkning som genuine antidepressiva.
En ytterligere gjenstand for oppfinnelsen er metabolitter, særlig av [2-(3-metoksyfenyl)-cykloheksylmetyl]-dimetylamin hhv. 3-(2-dimetylaminometyl-cykloheksyl)-fenol valgt blant

Særlig foretrukket er oppfinnelsens metabolitter som for eksempel foreligger som R,R, fortrinnsvis 1R,2R stereoisomerer.
Oppfinnelsens metabolitter er fysiologisk problemfrie. Derfor er en ytterligere gjenstand for oppfinnelsen et legemiddel som som aktiv bestanddel inneholder minst en metabolitt ifølge oppfinnelsen samt eventuelt tilsetnings- og/eller hjelpestoffer. Særlig er den tilsvarende gjenstand for oppfinnelsen et legemiddel som som aktiv bestanddel inneholder minst en forbindelse som er valgt blant: svovelsyremono-[3-(2-dimetylaminometyl-cykloheksyl)-fenyl] ester, svovelsyremono-( 1 R,2R)- [3-(2-dimetylaminometyl-cykloheksyl)-fenyl] ester, 3-(2-metylaminometyl-cykloheksyl)-fenol,

samt eventuelt tilsetnings- og/eller hjelpestoffer.
Særlig foretrukket er det når de oppnådde forbindelser foreligger som R,R, og fortrinnsvis 1R,2R stereoisomerer.
Egende tilsetnings- og/eller hjelpestoffer innenfor rammen av oppfinnelsen er alle de velkjente stoffer for å oppnå galeniske formuleringer. Utvalget av disse hjelpestoffer samt de anvendte mengder avhenger av om hvorvidt legemidlet skal administreres oralt, intravenøst, intrapeirtonealt, intradermalt, intramuskulært, intranasalt, bukkalt eller lokalt. For oral anvendelse egner seg tilberedninger i form av tabletter, tyggetabletter, drageer, kapsler, granulater, dråper, safter eller siruper, for parenetral, topisk og inhalative anvendelser benyttes oppløsninger, suspensjoner, lett rekonstituerbare tørrstoffpreparater samt spray. En ytterligere mulighet er suppositorier for anvendelse i rektum. Anvendelsen av et depot i oppløst form, en bærefolie eller et plaster, eventuelt under tilsetning av hudpenetreringsfremmende midler, er eksempler på egnede, perkutane anvendelsesformer. Eksempler på hjelpe- og tilsetningsmidler for orale anvendelsesformer er disintegreringsmidler, glidemidler, bindemidler, fyllstoffer, formskyllemidler, eventuelt oppløsningsmidler, smaksstoffer, sukkere, spesielle bærere, fortynnere eller farvestoffer, antioksidanter osv. For suppositorier kan det anvendes blant annet voks, hhv. fettsyreestere og for parenteral administrering kan det anvendes bærere, konserveringsmidler, suspensjonshjelpestoffer osv. Mengden av aktive bestanddeler som gis til pasientene varierer i avhengighet av pasientens vekt, applikasjonstype og sykdommens alvor. Fra oralt, rektalt eller perkutant anvendbare tilberedningsformer kan oppfinnelsens anvendte forbindelser settes fri på forsinket måte. Ved oppfinnelsens indikasjoner foretrekkes spesielt de tilsvarende retardformuleringer, særlig i form av et "en gang pr. dag" preparat som kun må inntas en gang daglig.
Videre foretrukket er legemidler som inneholder minst 0,05 til 90,0% av den aktive bestanddel, særlig lave, virksomme doseringer, for å unngå bi- eller analgetiske virkninger. Vanligvis anvendes 0,1 til 5000 mg/kg, særlig 1 til 500 mg/kg og spesielt 2 til 250 mg/kg kroppsvekt av minst en av forbindelsene som anvendes ifølge oppfinnelsen.
Like foretrukket og vanlig er imidlertid også anvendelse av 0,01-5 mg/kg, fortrinnsvis 0,03 til 2 mg/kg og særlig 0,05 til 1 mg/kg kroppsvekt.
Hjelpestoffer kan for eksempel være: vann, etanol, 2-propanol, glyserol, etylenglykol, propylenglykol, polyetylenglykol, polypropylenglykol, glukose, fruktose, laktose, sukrose, dekstrose, molasse, stivelse, modifisert stivelse, gelatin, sorbitol, inositol, mannitol, mikrokrystallinsk cellulose, metylcellulose, karboksymetylcellulose, celluloseacetat, skjellakk, cetylalkohol, polyvinylpyrrolidon, paraffiner, voks, naturlige og syntetiske gummier, aksiagummi, alginater, dekstran, mettede og umettede fettsyrer, stearinsyre, magnesiumstearat, sinkstearat, glyserylstearat, natriumlaurylsulfat, spiselige oljer, sesam-, kokosnøtt-, jordnøtt- eller soyabønneolje, lecitin, natriumlaktat, polyoksyetylen og -propylenfettsyreestere, sorbitanfettsyreestere, sorbin-, benzo-, sitron-, askorbin- eller tanninsyre, natriumklorid, kaliumklorid, magnesiumklorid, kalsiumklorid, magnesiumoksid, sinkoksid, silisiumdioksid, titanoksid eller titandioksid, magnesiumsulfat, sinksulfat, kalsiumsulfat, potteaske, kalsiumfosfat, dikalsiumfosfat, kaliumbromid eller kaliumiodid, talkum, kaolin, pektin, krospovidon, agar og bentonitt.
Fremstillingen av oppfinnelsens legemidler og farmasøytisk preparater skjer fra de farmasøytiske formuleringer ved hjelp av i og for seg kjente hjelpemidler, innretninger, metoder og prosess slik de for eksempel er beskrevet i "Remington's Pharmaceutical Sciences", ed. A. R. Gennaro, 17th ed., Mack Publishing Company, Easton, Pa. (1985), særlig i del 8, kapittel 76 til 93.
Således kan, for en fast formulering som en tablett, den aktive bestanddel i legemidlet, dvs. en forbindelse med den generelle struktur I eller et farmasøytisk godtagbart salt derav, granuleres med en farmasøytisk bærer, for eksempel vanlige tablettutgangsstoffer som maisstivelse, laktose, sakkarose, sorbitol, talkum, magnesiumstearat, dikalsiumfosfat eller farmasøytisk akseptable gummier, og farmasøytiske fortynningsmidler som vann, for å danne en fast blanding som inneholder oppfinnelsens forbindelse eller et farmasøytisk godtagbart salt derav i homogen fordeling. Ved homogen fordeling menes her at den aktive bestanddel er fordelt jevnt over hele blandingen slik at denne uten videre kan deles opp i like virksomme enhetsdoseformer som tabletter, piller eller kapsler. Den faste blanding blir så delt opp i enhetsdoseformer. Tablettene eller pillene av oppfinnelsens legemiddel hhv. oppfinnelsens preparat kan også overtrekkes eller kompounderes på annen måte for å tilveiebringe en doseringsform med forsinket frisetting. Egnede beleggsmidler er blant annet polymere syrer og blandinger av polymere syrer med stoffer som for eksempel skjellakk, cetylalkohol og/eller celluloseacetat.
Også når oppfinnelsens legemidler kun viser små bivirkninger kan det, hvor dette overhode skulle være nødvendig, for eksempel for å unngå spesielle former for avhengighet, ved siden av de anvendte forbindelser også være fordelaktig å benytte morfinantagonister og særlig nalokson, naltrekson og/eller levallorfan.
Da oppfinnelsens forbindelser, slik det er angitt ovenfor, er analgetisk virksomme er en ytterligere gjenstand for oppfinnelsen anvendelsen av en forbindelse ifølge oppfinnelsen hhv. en metabolitt ifølge oppfinnelsen for fremstilling av et legemiddel for behandling av smerte og særlig akutt, visceral, kronisk eller neuropatisk smerte eller kreftsmerter.
Da oppfinnelsens forbindelser utover dette også viser virksomhet ved urininkontinens, vil det være mulig å anvende forbindelsene, hhv. en metabolitt ifølge oppfinnelsen for fremstilling av et legemiddel for behandling av øket urinpress hhv. urininkontinens.
2-(3-Metoksyfenyl)-cykloheksylmetyl)-dimetylamin og (lR,2R)-[2-(3-metoksyfenyl)-cykloheksylmetyl)-dimetylamin] og fremstilling derav er kjent fra DE 195 25 137 Al eksempel 8 hhv. US 5,733,936 eksempel 8, hvorved den absolutte stereokjemi for den forbindelse (-6) som fremstilles i henhold til eksempel 8 vel korrekt burde hete (1R,2R) og ikke (1R,2S). 3-(2-dimetylaminometyl-cykloheksyl)-fenol og lR,2R)-3-(2-dimetylaminometyl-cykloheksyl)-fenol og fremstilling derav er kjent fra DE 195 25 137 Al eksempel 10 hhv. US 5,733,936, eksempel 10, hvorved den absolutte stereokjemi for den forbindelse (-7) som fremstilles i henhold til eksempel 10 vel korrekt må hete (1R,2R) og ikke (1R,2S).
Forbindelsene og deres fremstilling som ennå ikke er kjent fra DE 195 25 137 Al eller US 5,733,936, er fremstilt i henhold til eksemplene.
De følgende eksempler skal forklare oppfinnelsen uten at oppfinnelsens gjenstand derved skal ansees begrenset.
Eksempler
Generelt gjennomføres rensing og enantiomerseparering av alle som eksempel nevnte prosesser på forskjellige trinn med søylekromatografi og overveiende HPLC.
Eksempel 1: Fremstilling av (lR,2R)-[2-(3-metoksyfenyl)-cykloheksylmetyl]-metylamin, hydroklorid
[2-(3-Metoksyfenyl)-cykloheksylmetyl]-metylamin, hhv. (lR,2R)-[2-(3-metoksyfenyl)-cykloheksylmetyl]-metylamin, og særlig hydrokloridsaltet, fremstilles som følger: 5,67 g (22,9 mmol) (lR,2R)-[2-(3-metoksyfenyl)-cykloheksylmetyl]-dimetylamin som fri base oppvarmes i 389,64 ml toluen med 3,16 ml (25,2 mmol) fenylklorformat i 3 timer. Etter avkjøling og vasking dampes den organiske rest inn og det omrøres med 192,53 ml etylenglykol og til sammen 45,84 ml 5 N NaOH i til sammen 8,5 time under leilighetsvis avkjøling ved 110°C. Det oppnås (lR,2R)-[2-(3-metoksyfenyl)-cykloheksylmetyl]-metylamin som ble opparbeidet og felt ut som hydroklorid ved hjelp av TMCS.
Eksempel 2: Fremstilling av (lR,2R)-3-(2-metylaminometyl-cykloheksyl)-fenol, hydroklorid
3,55 g (15,2 mmol) av hydrokloridsaltet av (lR,2R)-[2-(3-metoksy-fenyl)-cykloheksylmetyl]-metyl-aminer ifølge eksempel 1 ble omført med 4,59 ml (47-48 %) vandig HBr i 7,5 time under tilbakeløp og så avkjølt over natten. Det ble oppnådd (lR,2R)-3-(2-Metylaminometyl-cykloheksyl)-fenol som ble opparbeidet og felt ut som hydroklorid med TMCS.
Eksempel 3: Fremstilling av svovelsyremono-(lR,2R)-[3-(2-dimetylaminometyl-cykloheksyl)-fenyl] ester
Svovelsyremono-[3-(2-dimetylaminometyl-cykloheksyl)-fenyl] ester eller svovelsyrernono-(lR,2R)-[3-(2-dimetylarninornetyl-cykloheksyl)-fenyl] ester ble fremstilt som følger: (lR,2R)-3-(2-Dimetylaminometyl-cykloheksyl)-fenol; hydroklorid ble behandlet med dicykloheksylkarbodiimid (DCC) i H2SO4. Det ble oppnådd svovelsyremono-(lR,2R)-[3-(2-dimetylaminometyl-cykloheksyl)-fenyl] ester. Eksempel 4: Fremstilling av 6-[(lR,2R)-3-(2-dimetylaminometyl-cykloheksyl)-fenoksy]-3,4,5-trihydroksy-tetrahydropyran-2-karboksylsyre 6-[3-(2-Dimetylaminometyl-cykloheksyl)-fenoksy]-3,4,5-trihydroksy-tetrahydropyran-2-karboksylsyre hhv. 6-[(1 R,2R)-3-(2-dimetylaminometyl-cykloheksyl)-fenoksy]-3,4,5-trihydroksy-tetrahydropyran-2-karboksylsyre fremstilles som følger: (lR,2R)-3-(2-Dimetylaminometyl-cykloheksyl)-fenol; hydroklorid behandles med 3,4,5-tri-O-acetyl-l-a-bromo-D-glukoronsyremetylester med 1. LiOH og 2. HOAc. Det oppnås en 6-[(lR,2R)-3-(2-Dimetylaminometyl-cykloheksyl)-fenoksy]-3,4,5-trihydroksy-tetrahydropyran-2-karboksylsyre. Rensingen skjedde via en Lobar-Lichoprep RP128 søyle med et MeOH:H20 system og derpå følgende HPLC separering. Eksempel 5: Fremstilling av [2-(3-metoksy-fenyl)-cykloheksylmetyl]-dimetyl-amin; N-oksid [2-(3-Metoksy-fenyl)-cykloheksylmetyl]-dimetyl-amine; N-oksid hhv. (lR,2R)-[2-(3-metoksy-fenyl)-cykloheksylmetyl]-dimetyl-amine; N-oksid fremstilles som følger: (1 R,2R)-[2-(3-Metoksy-fenyl)-cykloheksylmetyl]-dimetyl-amin; hydroklorid oppløses og behandles ved romtemperatur med H2O2. Det oppnås (lR,2R)-[2-(3-Metoksy-fenyl)-cykloheksylmetyl]-dimetyl-amin; N-oksid. Eksempel 6: Fremstilling av (lR,2R)-3-(2-dimetylaminometyl-cychloheksyl)-fenol N-oksid 3-(2-Dimetylaminometyl-cychloheksyl)-fenol N-oksid hhv. (lR,2R)-3-(2-dimetylaminometyl-cychloheksyl)-fenol N-oksid fremstilles som følger: (lR,2R)-3-(2-Dimetylaminometyl-cychloheksyl)-fenol; hydroklorid oppløses og behandles ved romtemperatur med H2O2. Det oppnås (lR,2R)-3-(2-Dimetylaminometyl-cychloheksyl)-fenol N-oksid.
Eksempel 7: Fremstilling av 4-(2-dimetylaminometyl-cykloheksyl)-katekol eller 4-(2-dimetylaminometyl-cykloheksyl)-benzen-l,2-diol.
Fremstillingen av 4-(2-Dimetylaminometyl-cykloheksyl)-katekol eller 4-(2-dimetylaminometyl-cykloheksyl)-benzen-l,2-diol skjedde i henhold til følgende reaksjonsskjema:
Derved angir den ovenfor nevnte metode 1 BuLi den for fagmannen godt kjente syntese via BuLi reagens og metode 2 Grignard den for fagmannen godt kjente syntese via Mg reagenser.
Eksempel 8: Fremstilling av C-[2-(3-metoksy-fenyl)-cykloheksyl]-metylamin.
Fremstillingen av C-[2-(3-metoksy-fenyl)-cykloheksyl]-metylamin skjedde i henhold til følgende reaksjonsskjema:
Eksempel 9: Fremstilling av (lR,2R)-3-(2-aminometyl-cykloheksyl)-fenol.
3-(2-Aminometyl-cykloheksyl)-fenol eller (lR,2R)-3-(2-aminometyl-cykloheksyl)-fenol fremstilles som følger:
Eksempel 10 In vitro isolering av metabolitten: [2-(3-Metoksyfenyl)-cykloheksylmetyl]-dimetylamin; hydroklorid og, i et annet eksempel, (lR,2R)-3-(2-dimetylaminometyl-cykloheksyl)-fenol; hydroklorid ble oppløst i TRIS/HC1 buffer pH 7,4. Deretter ble det tilsatt MgCl samt eventuelt de andre i henhold til litteraturen nødvendige kofaktorer for cytokrom P450 (CytP450) og det hele ble så inkubert med CytP450 3A4 (N-demetylering) og/eller CytP450 2D6 (O-demetylering) ved 37°C. Deretter ble satsen separert ved HPLC, metabolittene i fraksjonene identifisert via NMR og så isolert fra fraksjonene.
Eksempel 11 In vivo isolering av metabolittene:
Et pattedyr ble injisert med [2-(3-Metoksyfenyl)-cykloheksylmetyl]-dimetylamin; hydroklorid og i et ytterligere eksempel (lR,2R)-3-(2-dimetylaminometyl-cykloheksyl)fenol; hydroklorid. Blod ble tappet og dette separert etter separering av korpuskulære bestanddeler via HPLC hvoretter metabolittene i fraksjonen ble identifisert ved NMR og så isolert fra fraksjonene.
Eksempel 12 ji-Opioid reseptoraffinitet og NA og 5HT opptak-inhibitorkomponenter: Ved siden av den for en avgjørende binding til (j,-opioidreseptoren undersøkte man også NA og 5HT opptaksinhiberingskomponentene for to klinisk anvendte antidepressiva, fluoksetin og desipramin. Resultatene fra disse studier, gjennomført i henhold til kjente standardtester i faglitteraturen er oppsummert i tabell 1.
5HT gjenopptaksinhibering nettopp av (lR,2R)-3-(2-dimetylaminometyl-cykloheksyl)-fenol ligger i samme størrelsesorden som (j,-opioidreseptorbindingen for substansen. Dermed har substansen et nøyaktig ubalansert forhold av u-opioidkomponent og opptakshemming og har derved også potensiale for en spesielt effektiv inhibering av smerte. Opptakshemmingen er i størrelsesorden til det klinisk anvendte antidepressiva og fører derved til en selvstendig antidepressiv aktiv stoffkomponent hvorved substansen har en relativt sterk inhiberingsvirkning på noradrenalin- og særlig serotoningj enopptaket.
Eksempel 13 "Halesuspensjonstest":
"Halesuspensjonstesten" detekterer antidepressiv og anksiolyttisk aktivitet og skjer i henhold til metoden ifølge Porsolt et al. (Porsolt R. D., Bertin A., Jalfre M., Behaviourial despair in mice: a primary screening test for antidepressants. Arch. Int. Pharmacodyn. Ther., 229, 327-336,1977) og Stéru at al. (Stéru L., Chermat R., Thierry B, Simon P. The Tail Suspension Test: a new metod for screening antidepressants in mice. Psychopharmacology, 85, 367-370,1985). Gnagere som henges opp etter halen blir hurtig immobile. Anti-depressiva forringer immobilitetsvarigheten, mens trankvilansier forlenger immobilitetsvarighetene. Dyrenes oppførsel ble observert i 6 minutter, fortrinnsvis ved hjelp av datamaskinutstyr (Itemac-TST) utviklet av Stéru et al. (1985) (se ovenfor). Flere mus ble undersøkt parallelt og immobilitetsvarigheten bestemt. Denne parameter er analog den som ble benyttet i "behavioural despair" testen (Stéru L., Chermat R., Thierry B., Mico J. A., Lenégre A., Stéru M., Simon P. Porsolt R. D., The automated Tail Suspension Test: a computerized device which differentiates psychotropic drugs. Prog. Neuropsychopharmacol. Exp. Psychiatry, 11, 659-671,1987). 10-20 mus ble undersøkt pr. gruppe. Morfin (lR,2R)-3-(2-dimetylaminometyl-cykloheksyl)-fenol; hydroklorid og desipramin ble gitt 30 minutter før testen intraperitonealt og nalokson intravenøst.
Denne oppførselsmodell er i stand til å vise den anti-depressive virkning for opptaksinhiberingskomponenten ved samtidig nærvær av en opioid virkning. I denne oppførselsmodell fører anti-depressiva, på samme måte som i andre oppførselsmodeller, til en forkortelse av immobilitetsfasen, mens opioider som morfin forlenger denne immobilitetsfase. Ved nærvær av begge aktiv stoff komponentene overlapper effektene hverandre slik at man kun ser en liten motilitetsforandring. Dette ble opphevet ved at opiatkomponenten ble blokkert vekk ved forbehandling med nalokson. Etter naloksonforbehandlingen trådte den stimulerende virkning av (lR,2R)-3-(2-dimetylaminometyl-cykloheksyl)-fenol, som uten forbehandling kun var meget liten, tydelig frem. Derfor er substansen, særlig i kombinasjon med en smerteterapi, meget vel egnet. Resultatene av disse undersøkelser sammen med referansesubstanser er observert i tabell 2.
Tab. 2 viser at (lR,2R)-3-(2-dimetylaminometyl-cykloheksyl)-fenol; hydroklorid først etter forbehandling med nalokson bevirker en sterk reduksjon av immobilitetsvarigheten slik dette også ble funnet ved imipramin. Opioid fører, slik det er vist for morfin, til en forlengelse av immobilitetsvarigheten. Dette forklarer hvorfor (lR,2R)-3-(2-dimetylaminometyl-cykloheksyl)-fenol uten nalokson forbehandling, som nettoeffekt av opptaksinhibering og opioidvirkning kun fremkaller en liten forandring i immobilitetsvarigheten sammenlignet med oppløsningsmiddelkontrollen.
I henhold til de funn som er skissert ovenfor er overraskende (lR,2R)-3-(2-dimetylaminometyl-cykloheksyl)-fenol det første opioidanalgetika som har en relevant og åpenbart også klinisk utnyttbar, antidepressiv aktiv bestanddelskomponent.
Eksempel 14: Parenteral applikeringsform
1 g (lR,2R)-3-(2-dimetylarninornetyl-cykloheksyl)-fenol; hydroklorid ble oppløst i 11 vann for injeksjon ved romtemperatur og deretter innstilt til isotone betingelser ved tilsetning av NaCl.
