Abstract

Description
본 발명은 특히 낭포성 섬유증(cystic fibrosis; CF), 만성 폐쇄성 호흡기 질환 및 부비강염(sinusitis)의 치료에서 높은 점막 점도(mucosal viscosity)를 제거하기 위하여, 예를 들어 특히 만성 폐쇄성 호흡기 질환(chronic obstructive pulmonary diseases; COPD's)의 경우에서와 같은 기도(respiratory tract), 더욱 상세하게는 부비동(sinuses) 및 폐(lungs)로부터의 점액 제거율(mucus clearance)을 강화시키기 위하여 올리고우로네이트(oligouronate)를 사용하는 인간 또는 비인간 동물, 특히 포유류의 치료 방법에 관한 것이다.
낭포성 섬유증은 유럽 사람들에게 가장 흔한 치명적인 유전병이다. 이 병은 신체의 분비 세포 및 다른 세포에 존재하는 염소 이온채널(chloride channel)인 낭포성 섬유증 막통과 전도성 조절자(cystic fibrosis transmembrane regulator; CFTR)를 암호화하는 유전자에서의 변이에 의해 발병된다. 이 병은 신체에서 폐질환, 소화 문제 및 불임(infertility)을 야기할 수 있는 진하고(thick) 치료가 어려운 점액 분비(intractaqble mucus secretions)의 존재에 의해 특징지워진다. 폐에서 점액섬모 제거(mucociliary clearance)의 일반적인 패턴은 미생물에 의해 군집 화된 과도한 점착성의 점액을 제거하는데 실패하여 그 결과 점액 점도를 증가시키고, 만성 폐 염증 및 폐쇄(obstruction)를 초래할 수 있다. 따라서, 폐 질환은 다수의 낭포성 섬유증 환자들에게 있어서 가장 큰 건강상의 문제이며 사망의 주 원인이다.
점액은 폐를 포함한 전체 기도에서 정상적으로 분비된다. 점액은 주로 폐를 깨끗하게 유지하고 감염으로부터 보호하는 점액섬모 제거 시스템의 일부분으로서 작용한다. 점액섬모 제거 시스템은 점액(mucus); 섬모(cilia); 및 기도 표면액(airway surface liquid)의 3가지 주요 성분을 가진다. 따라서, 상피 표면은 점액을 분비하는 배상세포(goblet cells) 및 기도 표면액의 상부층(overlying layer)과 섬모 돌출(cilia protrude)의 끝부분 그 위에 점액층을 가지는 섬모상피세포(ciliated epithelial cells)를 포함한다. 점액은 주로 물(약 95% wt.) 및 뮤신(mucins), 점액의 물리적 성질을 담당하는 겔 형성 분자로 구성된 끈적거리는 겔 물질이다. 섬모는 끝부분이 점액층에 묻혀있고 물과 같은 비 점액성 기도 표면액에서 리드미컬하게 운동하는, 상피 세포 표면으로부터의 작은 털같은 돌기이다. 상기 점액층은 폐의 표면상에서 박테리아, 바이러스, 흡입된 입자들, 환경오염 물질 및 세포 잔해물들을 포착하는 끈적한 융단(sticky blanket)을 형성한다. 섬모의 운동(beating)은 이러한 점액 융단을 이동시키며, 융단에 포착된 어떤 것들은 입과 폐 바깥으로 향한다. 정상적인 조건 하에서 점액섬모 제거 시스템은 효과적으로 작용하며, 폐는 깨끗하게 유지되어 감염으로부터 벗어난다. 상기 시스템이 작용하지 않을 경우에는, 제2차로 기침 방어가 있다. 따라서, 증가된 수준의 점액 은 예를 들어 흡입된 입자들 또는 병균 감염 때문에 자극 또는 염증에 반응하여 분비되고, 상기 점액은 기침반사(cough reflex)에 의해 폐 바깥으로 나온다.
낭포성 섬유증 환자들에서, 폐의 점액은 정상인들보다 더 진하고 더 농후하며, 이러한 더 진한 점액은 섬모에 의하여 쉽게 수송되지 않는다. 그 결과 점액섬모 제거 시스템이 제대로 발휘되지 못하며, 폐는 더욱 감염에 공격받기 쉽다. 게다가, 낭포성 섬유증 환자의 폐는 지속되는 낮은 수준의 염증 및 일반적으로 염증을 야기하는 약품(agent)에 대하여 증가된 반응을 보이는 과염증성 상태(hyperinflammatory state)처럼 보인다. 증가된 점액 생성은 염증에 대한 반응의 일부분으로서 해결하기 어렵다. 점액이 너무 진한 경우 증가된 점액은 점액섬모 시스템 또는 기침에 의해 제거되며, 폐용량(lung capacity)은 감소되고, 점막을 가로지르는 산소 교환은 감소된다. 이것은 박테리아의 군집형성에 이상적인 환경을 제공하고, 또한 염증을 야기하고 면역 반응을 활성화시킴으로써 낭포성 섬유증 환자에게 심각한 문제를 제공한다. 이는 점액 분비를 증가시킬 뿐만 아니라 면역 반응 세포 및 대식세포와 라이소자임과 같은 물질의 존재 또한 증가시킨다. 박테리아와 대식세포가 사멸함으로써 이들의 세포 물질들이 점액 안으로 방출되고, 이것들은 DNA와 같은 점액성 분자들을 포함한다. 게다가, 예를 들어 버크홀데리아 속(Burkholderia sp .) 및 슈도모나스 애루기노사(Pseudomonas aeruginosa)와 같은 몇몇의 박테리아는 또한 매우 점성이 높은 다당류를 분비하며, 후자의 경우에는 알긴산염(alginates)를 분비한다. 이러한 분자들은 또한 점액의 점도를 증가시키며, 알긴산염의 경우에는 분명 점액의 뮤신 매트릭스와의 상호작용에 의한 것이지만 DNA의 경우에는 분명 뮤신 매트릭스 내에서 졸 층(sol phase)의 점성 증가에 의한 것이다.
따라서, 섬모에 의해 수송될 수 있는 형태로 점액을 유지하는 것이 낭포성 섬유증 환자 치료의 핵심 목적이다. 쉽게 파괴되는 겔과 같은 구조의 물질은 낭포성 섬유증 환자의 고점액성 점액과 같이 수송될 수 없는 유동체가 된다. 그러므로 어떠한 치료제도 당단백질 뮤신에 의해 형성되는 겔 매트릭스를 파괴해서는 안된다는 것이 중요하다.
우리들은 이것이 올리고우로네이트, 즉 다가의 하이드록실기(multiply charged hydroxyl group)를 운반하는 올리고머, 특히 2개 내지 30개의 모노머 잔기를 함유하는 것을 이용하여 달성될 수 있다는 것을 발견하였다.
기도, 특히 공동(sinuses), 주로 부비동(paranasal sinuses)의 이외의 곳에서도 지나치게 점성이 있는 점액은 문제를 일으킬 수 있다. 올리고우로네이트는 또한 이러한 문제를 해결하는데 사용될 수 있다.
따라서, 하나의 양상에서 본 발명은 기도에서의 점막 고점도를 제거하기 위한, 예를 들어 만성 폐쇄성 호흡기 질환, 부비강염 또는 낭포성 섬유증 환자를 치료하기 위한 인간 또는 비인간(예를 들어, 포유동물)의 치료 방법을 제공하며, 상기 방법은 상기 환자에게 생리학적으로 허용가능한 올리고우로네이트 폴리올(oligouronate polyol)을 유효량으로 상기 기도에서의 점막 표면에 적용하는 것을 포함한다.
또 다른 양상에서 본 발명은 기도에서의 점막 고점도의 치료에 사용하기 위한 약제의 제조를 위하여 생리학적으로 허용가능한 올리고우로네이트의 용도를 제공한다.
올리고우로네이트의 적용은 환자의 기도, 예를 들어 부비동 또는 더욱 바람직하게는 폐에서 점액을 분비하거나 점액을 수송하는 표면으로 임의의 방법에 의해 올리고우로네이트를 수송할 수 있다. 도입은 전형적으로 흡입, 예를 들어 에어로졸과 같은 액체 방울(liquid droplets) 또는 파우더일 수 있다.

올리고우로네이트는 대전된 우론산 잔기, 예를 들어 글루로네이트 및 갈락투로네이트 잔기와 같은 잔기를 포함하는 많은 천연 다당류 때문에 천연원(natural sources)로부터 쉽게 얻을 수 있다.
본 발명에 따라 이용할 수 있는 올리고우로네이트를 생산하기 위하여 다당류를 올리고당으로 분해하는 것은 효소적 분해 및 산 가수분해와 같은 전통적인 다당류의 용해(lysis) 기술을 사용하여 수행될 수 있다. 올리고우로네이트는 그 후 이온 교환 수지를 사용한 크로마토그래피나 분별 침전(fractionated precipitation) 또는 가용화(solubilization)에 의한 다당류 분해산물(polysaccharide breakdown products)로부터 분리될 수 있다.
올리고우로네이트는 또한 카르복실화되거나 카르복시메틸화된 글리칸과 같은 대전된 기를 첨가하는 변경을 포함하지만 이에 제한되지는 않고 화학적으로 변경될 수 있으며, 올리고우로네이트는 예를 들어, 과요오드 산화(periodate oxidation)에 의해 다루기 쉽게 변경된다.
적합한 다당류는 "하이드로콜로이드 핸드북(Handbook of Hydrocolloids; Ed. Phillips and Williams, CRC, Boca Raton, Florida, USA, 2000)"에 예시적으로 논의되어 있다. 그러나, 알긴산의 사용은 만누론산(manuronic acid; M) 및 글루론산(guluronic acid; G)의 블록 공중합체와 같이 천연적으로 생성된 것이 특히 바람직하며, G-블록 올리고머는 알긴산 원 물질(source materials)로부터 쉽게 생산될 수 있다. 실제, 일반적으로 올리고우로네이트는 올리고글루로네이트가 바람직하고 또는 올리고갈락투로네이트는 덜 바람직하다.
알긴산이 올리고우로네이트를 제조하기 위한 시작 물질로서 사용되는 경우, 글루로네이트 함량은 필요한 경우 아조토박터 비네란디(A.vinelandii)로부터 만노우로난 C-5 에피머라아제(mannouronan C-5 epimerases)를 사용하여 에피화(epimerization)시킴으로써 증가될 수 있다.
본 발명에 따라 사용하기에 적합한 올리고글루로네이트는 라미나리아 하이퍼보레아(Laminaria hyperborea)로부터 알긴산의 산 가수분해, 중성 pH 에서의 용해, 올리고글루로네이트를 침전시키기 위하여 pH를 3.4로 감소시키기 위한 무기산의 첨가, 약산을 사용한 수세, 중성 pH에서의 재현탁 및 동결 건조에 의해 전통적으로 생산될 수 있다.
또 다른 양상으로부터, 본 발명은 또한 약제학적 조성물, 특히 생리학적으로 허용가능한 담체 또는 첨가제와 함께 생리학적으로 허용가능한 올리고우로네이트를 포함하는 예를 들어, 용액, 현탁액(suspension), 분산액(dispersion) 또는 파우더와 같은 흡입가능한 조성물을 제공하고, 또한 바람직하게는 예를 들어, 핵산 분해 효소(예를 들어, DNase Ⅰ과 같은 DNAse), 겔솔린(gelsolin), 티올 감소제, 아세틸시스테인, 소디움 클로라이드, 비대전된 저분자량 다당류(예를 들어, dextran), 아르기닌(또는 다른 산화질소(nitric oxide) 전구체 또는 합성 자극제), 또는 음이온성 폴리아미노산(예를 들어, 폴리 ASP 또는 폴리 GLU)와 같은 생리학적으로 허용가능한 점막 점도 감소제를 더 제공한다. DNase의 사용은 특히 바람직하다. 올리고우로네이트는 성인에 대하여 전형적으로 1 내지 10000mg, 특히 10 내지 1000mg의 투여량으로 투여될 수 있다. 최적의 투여량은 일련의 투약 범위 실험, 선택적으로 하기의 동물 모델(예를 들어, 개 모델) 초기 연구에 의해 쉽게 결정될 수 있다.

올리고우로네이트는 바람직하게는 스프레이로, 더욱 바람직하게는 수용액으로부터 제조된 에어로졸 스프레이 또는 부피의 크기에 대하여 대략 나노미터 내지 마이크로미터(예를 들어, 10 내지 50000nm) 상태의 입자 크기의 파우더 형태로 투여된다.
또 다른 양상에서, 본 발명은 저장기(reservoir) 및 작은 물방울 생성기(droplet generator)를 포함하는 스프레이 기구(spray applicator)를 제공하며, 상기 저장기는 생리학적으로 허용가능한 올리고우로네이트 수용액을 포함한다. 이러한 스프레이 기구와 실제 올리고우로네이트는 공동 또는 코막힘(nasal blockage) 또는 감기(common cols) 또는 다른 이유로부터 발생하는 충혈(congestion)의 치료에 사용될 수 있다.

실시예 1
뮤신-알긴산 겔 모델 상에서 5개의 올리고머성 물질의 효과를 테스트하였다:

물질

유동 테스트(
Rheological
tests
)
젤라틴 - 샘플을 혼합한 후 즉시 37℃에서 유량계(rheometer)에 로딩하였다. 온도를 10℃로 냉각(0.5 degree/min)시킨 후, 10℃에서 18시간 동안 보관하였다. 샘플 및 겔 성장 거동을 지속적으로 측정하였다.
주파수 스윕(Frequency sweep) - 젤화 후 샘플의 온도를 37℃로 올리고, 유동학적 거동을 주파수의 함수로서 관찰하였다.
응력 스윕(Stress sweep) - 37℃에서, 겔을 거동을 증가하는 부가응력(applied stress)의 함수로서 관찰하였다.
겔화
이 연구는 소디움 글루로네이트 올리고머(G 블록)가 초기 겔화 후 시간에 따른 겔의 복합 모듈(G*)이 감소되는 것에서 나타난 것과 같이 뮤신-알긴산 상호작용에서 알긴산이 배치될 수 있음을 증명하였다. 5개의 테스트 겔과 대조군 겔에 대한 결과를 각각 도 1a 내지 1f에 나타내었다.
단지 소디움 갈락투로네이트 올리고머만으로 테스트한 다른 올리고머성 물질들은 시간에 따라 절대 계수(moduli value)를 감소시키는 것으로 나타나는 유사한 효과를 가진다. 다른 물질들(DNA, PEG, 덱스트란) 겔화에 지연을 야기시키지만, 겔은 복합 모듈의 증가에서 보듯이 시간에 따라 지속적으로 성장한다.
겔화 단계의 마지막에서 겔의 복합 모듈 및 위상각을 도 2 및 도3에 나타내었다. 상기 복합 모듈은 총 반응 또는 시스템의 세기를 측정한 것이고, 위상각은 액체와 비슷한(liquid like) 거동과 더 큰 액체와 비슷한(liquid-like) 거동을 나타내는 더욱 높은 위상각을 가지는 고체와 비슷한(solid like) 거동간의 균형에 관한 것이다. 소디움 글루로네이트 올리고머(G 블록)는 G*에 매우 큰 급경사와 위상각에서의 가장 큰 증가를 야기한다.

실시예 2
가래상의
효과
슈도모나스 애루기노사(Pseudomonas aeruginosa) 감염에 대하여 양성 진단을 받은 낭포성 섬유증 환자로부터 가래 샘플을 수집하였다. 상기 샘플을 부드럽게 흔들어 부분으로 나누어 동결시킨 후 -40℃에 저장하였다.
사전 테스트 과정:
상기 가래 샘플을 냉동고로부터 꺼내어 테스트 전에 1시간 동안 냉장고(4℃)에 넣어두고, 약 10분간 마그네틱 교반기로 교반하고 0.5g으로 나누었다. 테스트 매질을 하나의 샘플, 순수한 가래 샘플을 제외한 모든 샘플에 첨가하였다. 상기 결과 샘플은:
A : 0.5g 가래 (순수)
B : 0.5g 가래 + 65μl 350mM NaCl 용액 (첨가된 글루로네이트 올리고머의 이온강도)
C : 0.5g 가래 + 75μl 150mg/ml 소디움 글루로네이트 올리고머 Dp19
D : 0.5g 가래 + 75μl 150mg/ml 소디움 글루로네이트 올리고머 Dp10
상기 소디움 글루로네이트 올리고머 Dp10(중합도)는 하기 특성을 가진다.

그 후 혼합된 샘플을 1분간 부드럽게 흔들어준 후, 테스트하기 위한 유량계에 옮기기 전에 5분간 냉장고에 넣어두었다.
유동 테스트
단백질 가수분해 효소의 효과를 최소화시키기 위하여 10℃에서 유량계(Rheologica Stress-Tech general purpose rheometer)를 사용하여 플레이트/콘(40mm/10) 상에서 순수 가래 및 혼합된 가래의 유동 측정을 수행하였다.
순수 가래 및 테스트 매질과 혼합된 가래의 유동 특성화를 4가지의 서로 다른 단계에서 수행하였다.
- 일정 변형율(constant strain) 0.03 및 1Hz의 주파수를 사용하여 2분간 진동 변형 제어(Oscillating strain control)

글루로네이트 올리고머의 첨가는 생채 외(ex vivo)에서 낭포성 섬유증 환자 가래의 물리적 특성을 명백하게 감소시킨다. 복합 역학 점도(complex dynamic viscosity) 뿐만 아니라 복합 역학 모듈(complex dynamic modulus) 모두 5개의 인자 만큼의 몇몇의 경우에서 상당히 감소되었다. 중합도 10은 19의 평균 중합도를 가지는 샘플보다 더욱 효과적이라는 것을 나타낸다. 상기 기재된 바와 같이 가래 샘플을 제조하는 것으로부터 얻어진 결과는 또한 낭포성 섬유증 환자의 가래가 다른 성분(G*과 η*는 시간과 관계 없음)의 존재 하에서보다 중합도 10의 G 블록의 존재하에서 더욱 빠른 평형상태와 유사한 물리적 특성을 얻었음을 암시한다. 그러나, 위상각에서의 변화는 G*과 η* 정도의 범위만큼까지는 효과적이지 않았다. 상기 결과는 뮤신 네트워크의 물리적 특성이 유지되며, 따라서 여전히 공기중의 오염물질에 대한 방어의 첫 선으로서 작용할 수 있음을 명백하게 보여주기 때문에 매우 중요하다.
실시예 3
에어로졸
약 75%의 글루로네이트 순도 및 5와 20 사이의 중합도를 가지는 글론산 블록의 나트륨염 3~10% 및 필요하다면 에어로졸의 작은 물방울 크기를 안정화시키기 위한 표면 활성 화합물을 포함하는 액체로부터 염분무기(nebulizer)의 도움으로 투여되는 멸균 에어로졸 스프레이를 만들었다.
Claims (17)
- 생리학적으로 허용가능한 올리고우로네이트를 유효량으로 환자 기도의 점막 표면에 적용하는 것을 포함하는, 기도에서의 점막 고점액을 제거하기 위한 인간 또는 비인간 환자의 치료 방법.
- 제1항에 있어서,낭포성 섬유증(cystic fibrosis)의 치료를 위한 것임을 특징으로 하는 방법.
- 제1항 또는 제2항에 있어서,부비강염(sinusitis)의 치료를 위한 것임을 특징으로 하는 방법.
- 제1항에 있어서,만성 폐쇄성 호흡기 질환(chronic obstructive pulmonary diseases)의 치료를 위한 것임을 특징으로 하는 방법.
- 제1항 내지 제4항 중 어느 한 항에 있어서,상기 올리고우로네이트는 환자의 기도(airway) 안으로 투여되는 것을 특징으로 하는 방법.
- 제1항 내지 제5항 중 어느 한 항에 있어서,상기 올리고우로네이트는 모노머로서 글루론산(guluronic acid)을 포함하는 것을 특징으로 하는 방법.
- 제1항 내지 제6항 중 어느 한 항에 있어서,상기 올리고우로네이트는 4 내지 25-mer인 것을 특징으로 하는 방법.
- 기도에서의 점막 고점액 치료에 사용하기 위한 약제를 제조하기 위한 생리학적으로 허용가능한 올리고우로네이트의 용도.
- 제8항에 있어서,상기 올리고우로네이트는 제6항 및 제7항 중 하나로 정의되는 올리고우로네이트임을 특징으로 하는 올리고우로네이트의 용도.
- 제8항 또는 제9항에 있어서,낭포성 섬유증의 치료에 사용하기 위한 흡입가능한 약제의 제조를 위한 것임을 특징으로 하는 올리고우로네이트의 용도.
- 제8항 내지 제10항 중 어느 한 항에 있어서,부비강염의 치료에 사용하기 위한 흡입가능한 약제의 제조를 위한 것임을 특 징으로 하는 올리고우로네이트의 용도.
- 제8항 또는 제9항에 있어서,만성 폐쇄성 호흡기 질환의 치료에 사용하기 위한 흡입가능한 약제의 제조를 위한 것임을 특징으로 하는 올리고우로네이트의 용도.
- 생리학적으로 허용가능한 올리고우로네이트와 함께 생리학적으로 허용가능한 담체 또는 첨가제를 포함하는 것을 특징으로 하는 흡입가능한 약제학적 조성물.
- 제13항에 있어서,생리학적으로 허용가능한 점막 점도 감소제를 더 포함하는 것을 특징으로 하는 조성물.
- 제13항 또는 제14항에 있어서,제6항 또는 제7항에서 정의되는 올리고우로네이트를 포함하는 것을 특징으로 하는 조성물.
- 제13항 내지 제15항 중 어느 한 항에 있어서,DNase를 포함하는 것을 특징으로 하는 조성물.
- 저장기(reservoir) 및 작은 물방울 생성기(droplet generator)를 포함하며, 상기 저장기가 생리학적으로 허용가능한 올리고우로네이트 수용액을 포함하고 있는 것을 특징으로 하는 스프레이 기구.
