Abstract
세바스산 또는 그 유도체를 장 경유 영양 및 비경구적 영양에 이용하는 용도및 이들 물질을 활성물질로 함유하는 의약 조성물을 제공한다.
세바스산의 유도체로는 세바스산의 염, 세바스산 트리글리세리드 및 세바스산 트리글리세리드의 염을 사용한다.
Description
본 발명은 세바스산및 약리학적으로 허용되는 세바스산 유도체의 장 경유 영양(enteral nutrition)및 비경구적 영양에 있어서의 용도와 이들 세바스산 및 그 유도체를 함유한 의약 조성물에 관한 것이다.
본 발명에 있어서 약리학적으로 허용되는 세바스산 유도체라 함은 약리학적으로 허용되는 세바스산의 염, 특히 알칼리 금속염, 바람직하게는 나트륨염및 칼륨 염과, 세바스산 트리글리세리드및 약리학적으로 허용되는 그 염, 바람직하게는 나트륨 염과 칼륨 염을 뜻한다. 세바스산 트리글리세드의 나트륨염과 칼륨 염은 신규 화합물이다.
임상적으로 사용함에 있어서 사슬길이가 긴 모노카르복실 지방산의 트리글리세리드의 지질(lipid) 에멀젼이 전(total) 비경구적 영양용으로 수년동안 사용되고 있는 것이 일반적이기는 하지만 근래에 와서는 사슬길이가 중간 정도되는 트리글리세리드 몇가지만이 그 결과가 만족스럽게 나타날 뿐이었다. 그러나 이들 에멀젼은 심각한 결점을 가지고 있다. 실제로 패혈증등과 같은 번번히 발생하는 몇가지 병리학적 조건에 있어서 사슬이 긴 활성의 트리글리세리드를 효과적으로 이용함에 대한 실험적인 증거가 논쟁의 대상이 되고 있다. 그 이유는 추측컨대, 카르니틴(carnitine)의 상대적인 결핍으로 인하여 그 사용이 제한을 받고, 또한 사슬이 긴 지방산의 미토콘드리아 산화가 저해되기 때문이라고 할 수 있다. 또한 몇가지 실험 결과에 의하면 케토산 성증(ketoacidosis) 이 유발된 실험 동물에게 사슬길이가 중간정도인 트리글리세리드를 많은 양으로 투여하면 마취작용을 수반하는 독성효과를 나타낸 다는 것이다. 따라서 현재까지 사용되고 있는 이들 이외의 다른 물질을 개발해야 할 필요성이 있었다.
현재 확인된 바로는 세바스산과 약리학적으로 허용되는 이들의 유도체는 공지의 기질(substrate)이 가지고 있는 결함을 나타내지 아니하는 장 경유 영양 및 비경구적 영양에 적합한 조성물을 제조시 우수한 기질들이다.
따라서 본 발명에서는 활성물질로서 세바스산 또는 그 유도체를 함유함을 특징으로 하는 장(腸) 투여및 비경구 투여에 적합한 의약 조성물을 제공하고 있다. 특히 세바스산 유도체는 약리학적으로 허용되는 세바스산 염, 세바스산 트리글리세리드 및 세바스산 트리글리세리드의 약리학적으로 허용되는 염 중에서 선택된다. 바람직하게는 세바스산 또는 세바스산 트리글리세리드의 약리학적으로 허용되는 염은 나트륨염과 칼륨염이다.
이들을 수용액 형태로 제제화할 경우 의약 조성물 중에는 약리학적으로 허용되는 세바스산 염 0.2∼0.6몰/ℓ 또는 세바스산 트리글리세리드 또는 세바스산 트리글리세리드의 약리학적으로 허용되는 염 0.2∼1몰/ℓ를 함유하는데, 바람직하게는 세바스산의 염, 트리글리세리드 또는 트리글리 세리드 염 0.5몰/ℓ를 함유하는 것이다.
경구 투여용 단위제형 형태의 조성물, 예를 들자면 캡슐의 경우에서는 세바스산의 염, 트리글리세리드 또는 트리글리세리드 염을 0.5∼1.5g 함유한다.
세바스산 나트륨 및 세바스산 칼륨은 세바스산 (C10)을 NaOH 또는 KOH로 중화(몰비 OH- : C10 = 2 : 1 )하여 제조한다.
세바스산 트리글리세리드의 나트륨 염과 칼륨 염을 제조하자면 세바스산을 수산화 리튬과 수산화 알루미늄 존재하에 갖 증류한 디옥산중의 티오닐글로라이드(b.p. 79℃)에 가하고 (몰비 C10 : 티오닐 클로라이드 = 1 : 2 ), 이 혼합물을 촉매인 염화 세슘 존재하에 환류온도에서 5∼6시간 유지한다. 디옥산을 감압하에 제거함으로써 세바스산의 산 염화물을 얻는다.
촉매인 마그네슘과 NaOH 존재하에 트리글리세리드를 증류수에 용해시켜 용액의 pH를 약 8로 유지하고 C10산 염화물을 반응 혼합물에 가하여 에스테르 반응으로 부터 방출되는 HC1과 반응시킴으로써 NaC1과 H2O를 형성시킨다.
디옥산에 C10 산 염화물 과잉량(글리세롤양의 5∼8배, 생성된 화합물의 중합반응이 일어나지 않도록 함)을 용해시킨 용액을 질소 분위기에서 위에서 얻은 수용액에 서서히 가하는데, 이때 소량의 NaOH를 조금씩 가하여 pH를 염기성으로 유지하도록 주의해야 한다. 시약을 소량 사용하면 반응은 약 30분 이내에 종료한다. 이어서 반복 증류된 증류수를 가하여 투명용액을 얻고, 디에틸 에테르로 3회 추출하여 과잉의 유리산을 제거한다. 용액중에 용해된 미량의 에테르는 질소가스를 주입하여 기포를 생성시킴으로써 제거한다.
마지막으로 반복 증류된 증류수에 대하여 이 용액을 24시간 투석(dialysis)하여 알칼리 금속의 염화물을 제거한다.
본 발명에 의한 세바스산 또는 그 유도체는 어느 것이나 무독성이다.
위스타 래트(Wistar rat, 128 마리중 64마리는 수컷이고 64마리는 암컷)와 뉴질랜드산 토끼(128마리중 64마리는 수컷이고 64마리는 암컷)에 대하여 세바스산 이나트륨을 경구투여및 복강내 투여하여 급성 독성 시험 결과 LD50를 평가할 수 없었다. 마찬가지로 래트 40마리 (암컷 20마리, 수컷 20마리)와 토끼 40마리 (암컷 20마리, 수컷 20마리)에 대하여 세바스산을 펠릿(pellet) 형태로 제제화한 것을 투여하여 아급성 (subacute) 독성 및 만성독성을 시험하였다. 대조 동물군에 비하여 처리된 동물군에 있어서는 생물학적 파라메터 (혈액치, 성장곡선, 각종 기관의 조직학적 시료의 흡수)에 있어서 큰 차이는 없었다.
더욱이 래트와 토끼에 대하여 태아독성 시험, 기형발생시험 및 신생아 독성 시험을 한 결과 독성작용은 발견되지 않았다. 특히 태아 발육은 절대적으로 정상이었고 기형 발생작용은 전혀 나타나지 않았다.
세바스산 나트륨과 세바스산 칼륨 역시 래트와 토끼에 있어서 특별한 급성 독성이나 만성 독성등의 작용을 나타내지 않았다. 나트륨을 많이 함유한 염수만을 투여한 동물에 있어서는 동일한 치사율을 나타내었기 때문에 독성의 원인은 나트륨 때문인 것이 분명하다.
마지막으로 임신중의 어미에게 세바스산 염을 다량 투여하더라도 태어나기 전의 동물과 갓 태어난 동물에 대해서는 알칼리 금속의 세바스산 염은 독성이 없었다.
사람에 있어서 장기간 치료시에 있어서도 독성 작용은 나타나지 않았다.
한편, 세바스산과 그 유도체들은 현재까지 사용되고 있는 지방산에서 나타나는 미토콘드리아에서 뿐만 아니라 세포질에서 유리되어 카르니틴 아실트란스퍼라아제와는 관련이 없는 경로를 통해 미토콘드리아로 유입되는 C6 레벨까지의 퍼옥시좀(peroxisome)에 있어서도 베타 산화와 CoA 활성화되기 때문에 대사가 잘된다.
이러한 대사작용은 카르니틴 감소에 의하여 영향을 받는 일이 거의 없기 때문에 이러한 특수한 대사 경로로 인하여 이들 화합물은 패혈증에 극히 적합하다.
더욱이 현재 사용되고 있는 지방기질(fatty substrate)과 비교하면 본 발명의 화합물들은 ATP/CO2의 몰비가 큰 것인데, 즉 부수되는 CO2생성이 적은 반면 에너지 발생은 크다. 이러한 현상은 호흡 파라메터의 변화를 나타내는 환자에 있어서 특히 유리하다.
나트륨 투여양을 감소시키는 것이 중요한 경우에 있어서 트리글리세리드 염은 상응한 세바스산 염의 나트륨 양의 절반을 함유하기 때문에 선택할 수 있는 화합물이다.
본 발명에 의한 장 경유 영양및 비경구적 영양의 기질이 가지는 효능과 장점에 대하여 몇 가지 임상 실험으로 평가하였는데, 그 한가지를 아래와 같이 간단히 요약할 수 있다.
건강한 남자 지원자를 두 무리로 나누어 각 무리에 대하여 임상 실험을 하였다.
즉, 첫째 무리(6명)에 대하여는 세바스산염 1000mg을 대형 경구용 환제(丸劑)로 하여 경구 투여하였고, 나머지 무리(6명)에 대하여는 세바스산염 10g을 증류수 500ml에 용해한 것을 3.33g/h의 정맥내 주입속도로 3시간에 걸쳐 정맥내에 투여하였다.
2-콤파트먼트 오픈 모델(2-compartment open model)에 상응하는 바이익스 포넨셜 피트(biexponential fit)를 이용하여 사람 각각에 대한 혈청 세바케이트(serum sebacate) 데이터를 컴퓨터로 분석하였다. 반감기(t1/2) 분포는 0.34±0.06h이었고 배제상(elimination phase)은 오히려 빨랐다. (Ke=2.10±0.38h-1). 중앙 콤파트먼트의 체적은 2.79±0.54ℓ이었고, 조직 콤파트먼트의 체적은 3.72±0.14ℓ이었다. 이들 데이터로 부터 세바스산 염의 조직 고정은 양호함을 할 수 있다. 혈장 정화치는 5.96±2.19ℓ/h이었고 신장 정화치는 19.22±10.69ℓ/h이었는데, 이로 부터 세바스산의 요세관 배설(tubular excretion)이 일어났음을 알 수 있다.
주입(180분)이 끝났을 때 세바스산 염의 혈창 농도는 최고치에 도달하였는데 이것은 480.50±43.02㎍/ml에 상응한 것이었다.
직접 열량 측정법으로 210분 동안의 주입 초기에서 시작하여 호흡 및 대사파라메터를 측정하였다. 실험이 끝나기까지 O2소모량(VO2ml/min/m2)은 거의 변하지 않았고 (시간 0일때 154.3±28.3, 180분일때 155.3±39.5), CO2생성량(VCO2ml/min/m2)은 주입이 끝났을 때 기초값(147.7±27.3) 이하의 값 (123.7±25.0)으로 감소하였다. 따라서 호흡지수 (RQ : respiratory quotient)는 상당히 감소(0.96±0.04으로 부터 0.81±0.06로) 하였고 주입도중과 주입후에 있어서 지질로 부터 유도된 열량 %는 증가하였다.
(-0.13±13.3으로 부터 52.1±26.2로). 대사율(MR : metabolic rate, Kcal/h/m2)은 전 실험시간 동안 변함없이 일정하였다.
이 실험에 있어서 세바스산의 요(尿) 배설과 베타 산화 생성물 [수베르산(C8) 및 아디프산(C6)]은 낮았음이 확인되었다. (투여량의 16% 이하).
에너지 생성은 많았고 (6.64Kcal/g) 세바스산은 체내에서 완전히 산화되어 CO2와 H2O로 되었다.
Claims (7)
- 영양적으로 활성기질로서 세바스산 또는 약리학적으로 허용되는 그 유도체를 함유하는 장 경유 영양용 또는 비경구직 영양용의 의약 조성물.
- 제 1항에 있어서, 세바스산 또는 세바스산 트리글리세리드의 약리학적으로 허용되는 염은 나트륨 염및 칼륨 염으로부터 선택되는 것인 의약 조성물.
- 제 1항에 있어서, 세바스산 트리글리세리드 또는 세바스산 트리글리세리드의 약리학적으로 허용되는 염을 0.2∼1몰/ℓ 함유하는 수용액 상태의 의약 조성물.
- 제 3항에 있어서, 세바스산의 트리글리세리드 또는 트리글리세리드 염을 약 0.5몰/ℓ 함유하는 의약 조성물.
- 제 1항에 있어서, 세바스산의 트리글리세리드 또는 트리글리세리드 염을 0.5∼1.5g 함유하는 단위제형 형태의 경구 투여용의 의약 조성물.
- 제 5항에 있어서, 캡슐형태의 의약 조성물.
- 제 1항에 있어서, 세바스산의 유도체는 세바스산의 약리학적으로 허용되는 염, 세바스산 트리글리세리드및 세바스산 트리글리세리드의 약리학적으로 허용되는 염으로 된 군으로부터 선택되는 것인 의약 조성물.
