本願は、その全体を本明細書に援用する2016年5月3日に出願された米国仮特許出願第62/331,070号の優先権を主張するものである。
一態様において本発明は以下を提供する。

100ng未満の無細胞DNAを含む試料から無細胞メチル化DNAを捕捉する方法であって、

ドナー補充及び試料取得
膵臓腺癌(PDAC)患者試料をUniversity Health Network BioBankから入手し、健常対照をカナダ、トロントのMount Sinai Hospital(MSH)のFamily Medicine Centreから補充した。患者が同意して収集された全試料を、Research Ethics Board、University Health Network及びカナダ、トロントのMount Sinai Hospitalからの施設承認の下に得た。
原発性PDAC試料の場合、検体を切除後すぐに処理し、代表的な切片を使用して、診断を確認した。新たな液体窒素凍結組織試料のレーザーキャプチャーマイクロダイセクション(LCM:Laser capture microdissection)をLeica LMD7000機器上で行った。手短に述べると、気相液体窒素中で維持された凍結組織をOCT切断媒体に包埋し、クライオトームで8μm厚切片に切り出した。切片をPEN膜スライド(Leica)に載せ、ヘマトキシリンで軽く染色して、腫瘍領域の顕微鏡同定を容易にした。切片を切断して核酸の劣化を最小にした同じ日にLCMを行った。
EDTA及びACD血漿試料をBioBank及びカナダ、トロントのMount Sinai Hospital(MSH)のFamily Medicine Centreから入手した。全試料を使用まで-80℃又は気相液体窒素中で貯蔵した。無細胞DNAを0.5~3.5mlの血漿からQIAamp Circulating Nucleic Acid Kit(Qiagen)を用いて抽出した。抽出DNAを使用前にQubitによって定量化した。
University Health NetworkのResearch Ethics Boardによって認可されたようにUniversity Health Network Biobankから患者の同意の下で得られたヒト直腸結腸腫瘍組織を、コラゲナーゼAを用いて単細胞に消化した。単細胞を4~6週齢NOD/SCID雄性マウスに皮下注射した。マウスをCO2吸入によって安楽死させた後、心臓穿刺によって採血し、EDTA管に貯蔵した。収集血液試料から、血漿を単離し、-80℃で貯蔵した。無細胞DNAを0.3~0.7mlの血漿からQIAamp Circulating Nucleic Acid Kit(Qiagen)を用いて抽出した。すべての動物の作業をUniversity Health NetworkのAnimal Care Committeeによって認可された倫理規定に従って行った。
無細胞DNAが得られた同じ患者由来のLCM濃縮腫瘍及び正常試料から抽出されたゲノムDNAを軽微な変更を加えたGuら、2011年18の手順に従ってRRBSに供した。手短に述べると、Qubitによって測定された10ngのゲノムDNAを制限酵素MspIで消化し、次いで末端修復、Aテーリング、及びIllumina TruSeqメチル化アダプターとのアダプター連結に供した。次いで、調製されたライブラリをZymo EZ DNAメチル化キットを用いて製造者の手順に従って亜硫酸水素塩変換に供し、続いて160bp~300bpの断片のゲルサイズ選択に供した。各精製ライブラリを増幅する最適サイクル数をqPCRを使用して決定し、その後、試料をKAPA HiFi Uracil+Mastermix(Kapa Biosystems)を用いて増幅し、AMPureビーズ(Beckman Coulter)を用いて精製した。最終ライブラリをBioAnalyzer分析に供した後、UHN Princess Margaret Genomic CentreにおいてIllumina HiSeq 2000で配列決定した。
腸内細菌ファージλDNA(ThermoFischer Scientific)を表1に示したプライマーを用いて増幅し、6種の異なるPCRアンプリコン産物を生成した。PCR反応をKAPA HiFi Hotstart ReadyMixを用いて以下の条件で行った:95℃3分間の酵素の活性化、30サイクルの98℃20秒間、60℃15秒間、72℃30秒間、及び72℃1分間の最終伸長。PCRアンプリコンをQIAQuick PCR精製キット(Qiagen)を用いて精製し、ゲル上に流して、サイズ及び増幅を検証した。1CpG、5CpG、10CpG、15CpG及び20CpGLのアンプリコンをCpG Methyltransferase(M.SssI)(ThermoFischer Scientific)を用いてメチル化し、QIAQuick PCR精製キットを用いて精製した。PCRアンプリコンのメチル化を制限酵素HpyCH4IV(New England Biolabs Canada)を用いて試験し、ゲル上に流して、そのメチル化を確実にした。非メチル化(20CpGS)及びメチル化(1CpG、5CpG、10CpG、15CpG、20CpGL)アンプリコンのDNA濃度をPicoGreenを用いて測定した後、50%メチル化及び50%非メチル化λPCR産物と一緒に貯蔵した。
cfMeDIP-seq手順の略図を図2に示す。cfMeDIP前に、DNA試料をKapa Hyper Prep Kit(Kapa Biosystems)を用いてライブラリ調製に供した。一部変更した製造者手順に従った。手短に述べると、目的DNAを0.2mL PCR管に添加し、末端修復及びAテーリングに供した。アダプター連結は、NEBNextアダプター(Illuminaキット用NEBNext Multiplex Oligos、New England Biolabs)を用いて最終濃度0.181μMで従い、20℃で20分間インキュベートし、AMPure XPビーズを用いて精製した。溶出ライブラリをUSER酵素(New England Biolabs Canada)で消化し、続いてMeDIP前にQiagen MinElute PCR Purification Kitを用いて精製した。
cfMeDIP-seqを手順のフィラー成分におけるメチル化と非メチル化ラムダDNAの異なる%を用いて以下のように実施した。
本発明者らは、QIAgen Circulating Nucleic Acidキットを使用して、Princess Margaret Cancer Centreにおいて初期臨床試験登録前に作成された対応腫瘍組織分子プロファイリングデータを有する患者由来の約20mLの血漿(4~5×10mL EDTA採血管)から無細胞DNAを単離した。DNAを細胞系(CRC及びMM細胞系の希釈)からPureGene Gentraキットを用いて抽出し、Covaris超音波処理器を用いて約180bpに断片化し、より大きいサイズ断片をAmpureビーズを用いて排除して、無細胞DNAの断片サイズを模倣した。DNA塩基配列決定ライブラリを83ngの断片化DNAからNEXTflex-96 DNA Barcodeアダプター(Bio Scientific、オースティン、TX)アダプターを利用するKAPA Hyper Prep Kit(Kapa Biosystems、ウィルミントン、MA)を用いて構築した。既知の変異を含むDNA断片を単離するために、本発明者らは、Illumina TruSeq Amplicon Cancer Panelを用いて臨床検査室によって試験された48の遺伝子から変異ホットスポットを標的にしたビオチン化DNA捕捉プローブ(xGen Lockdown Custom Probes Mini Pool、Integrated DNA Technologies、Coralville、IA)を設計した。バーコードライブラリをプールし、次いでカスタムハイブリッド捕捉ライブラリを製造者の指示(IDT xGEN Lockdown手順バージョン2.1)に従って適用した。これらの断片をIllumina HiSeq 2000機器を用いて>10,000×リードカバー度まで配列決定した。得られたリードをbwa-memを用いて整列させ、変異をSAMtools及びmuTectバージョン1.1.4を用いて検出した。
本発明者らは、145,000個のシミュレートされたゲノムを作製した。癌特異的メチル化DMRの割合は、0.001%、0.01%、0.1%、1%及び10%に設定され、それぞれ1、10、100、1000及び10000個の独立したDMRからなった。本発明者らは、14,500個の二倍体ゲノム(100ngのDNAに相当する)をこれらの最初の混合物から採取し、10、100、1000及び10000リード/遺伝子座を更に採取して、これらの深さにおける配列決定カバー度を表した。このプロセスをカバー度、存在量及び特徴数の各組合せで100回繰り返した。本発明者らは、パラメータの各組合せで少なくとも1つのDMRの検出成功頻度を推定し、確率曲線をプロットして(図1A)、配列決定深さを条件とする検出成功確率に対する特徴数の影響を視覚的に評価した。
24名の膵癌(PC:Pancreatic Cancer)患者及び24名の健常ドナーからのcfDNA試料間の差次的メチル化領域(DMR)をMEDIPS Rパッケージを用いて計算した25。各試料について、BAMアラインメント(対ヒトゲノムhg19)ファイルを使用して、MEDIPS R対象を作製した。次に、2セットの試料からのRPKMをt検定で比較することによってDMRを計算した。t検定からの生のp値をBenjamini-Hochberg手順によって調節した。次いで、DMRを調節p値が0.1未満であるすべての窓として定義した。38,085個の総DMRが見いだされ、6,651個が膵癌患者におけるHyper、31,544個がHypoであった。これらのDMRからのスケール付きRPKM値をヒートマップとして示した(図5C)。このヒートマップは、距離関数「ユークリッド」、列方向クラスタリングに対するクラスタリング関数「ward」、及び行方向クラスタリングの「average」で作成された。
RRBSによってプロファイルされた5個の正常PBMC試料をGEOからダウンロードして(受託ID GSE89473の全対照試料)、それらのメチル化プロファイルを24個の膵癌組織RRBS試料と比較した。ダウンロードしたBEDファイルをR methylKitパッケージで構文解析し、処理した26。次いで、これらの5個の試料を24名の膵癌患者からの同様に処理されたRRBS試料と比較した。カスタム機能を使用して、24個のPC試料のうち少なくとも18個、及び5個のPBMC試料のうち4個中に存在するCpGを抽出し、常染色体中のCpGのみが保持され、1,806,808個のCpGのバックグラウンドセットを作製した。これらから、Benjamini-Hochbergで調節されたp値<0.01及びデルタベータ>0.25の基準を用いてDMCを得た。134,021個のDMCは、膵癌においてPBMCに比べてHyperであることがわかった。同様に、同じq値カットオフ及びデルタベータ<-0.25を用いて、本発明者らは179,662個のHypo DMCを得た。合計313,683個のDMCを対応するボルケーノプロットにおいて赤い点で示し(図7F)、q値の負のlog10をデルタベータに対してプロットした(負のlog10q値=2における水平線は、DMCをコールするためのq値カットオフであり、垂直点線はデルタベータカットオフである)。
順列分析を行って、血漿中で同定されたDMR(本発明者らのcfMeDIP-seq手順に供された循環cfDNA)と原発腫瘍組織中で同定された癌特異的DMC(RRBS)の予想対観察重複の頻度を比較した。本発明者らは、可能性のある4つの例を調べた:Hyper DMRと重複するHyper DMC、Hypo DMRと重複するHyper DMC、Hypo DMRと重複するHypo DMC、及び最後にHyper DMRと重複するHypo DMC。各例では、Hyper又はHypo DMCをHyper又はHypo DMRと重ねて、「生物学的交差」数を得た。次いで、DMCの各セットを1,806,808個のCpGのバックグラウンドセット全体にわたって無作為に1000回混合し、DMRの各セットと再度重複させた。これらの無作為な生物学的交差をZスコアを用いて同じスケール上に置き、それぞれボックスプロット及び菱形で示した(図5E)。これらのプロットにおける水平の点線は、ボンフェローニ補正から誘導されるq値0.05に関連するカットオフZスコアである。
5個の正常PBMC試料と比較した24個のPC試料を同じ患者由来の24個の正常組織とも別々に比較した。バックグラウンドセット(763,874個のCpG)及びPC中のDMC Hyper&Hypo(それぞれ34,013&11,160)を同じ方法で計算し、これらを使用して、ボルケーノプロット(図7C)とボックスプロット(図5D)を同様に作成した。
本発明者らは、上位百万の最も変わりやすい全ゲノム窓を用いた24個のPC及び24個の健常cfDNA試料についてのPCAによる目的変数なしのクラスタリング分析を行った(図7A~B)。各窓について、可変性を平均絶対偏差(MAD)測定規準によって計算した。これは、データの中央値から絶対偏差の中央値を返す堅牢な測定であり、データは、所与の窓に対するこれら48試料にわたるRPKM値である。
RNA-SeqデータをGTExデータベースから全ヒト遺伝子に対する組織による中央値RPKMの形で得た(ファイルGTEx_Analysis_v6p_RNA-seq_RNA-SeQCv1.1.8_gene_median_rpkm.gct.gz under https://gtexportal.org/home/datasetsから得た)。目的TFをそれらの遺伝子名と適合させ、ヒートマップ(図8A、8C)を全組織にわたってスケール付けされた各TFの中央値RPKMを用いて作成した。距離関数「manhattan」及びクラスタリング関数「average」を行方向クラスタリングと列方向クラスタリングの両方に使用した。
本発明者らが症例対対照における低メチル化領域において有意に濃縮されたモチーフを検出したTFが、膵癌試料において有意に上方制御されるかどうか推定するために、本発明者らはssGSEAスコアを検定統計量とした無作為化検定を使用した。各試料について、本発明者らは、低メチル化モチーフに有意に関連することが判明した85個のTF、及び85個のTFの無作為な1,000セットを用いてスコアを計算した(全ヒトTFのリストをhttp://www.tfcheckpoint.org/data/にあるファイルTFCheckpoint_download_180515.txtから得た)。TCGA上の178名の膵臓腺癌患者からの発現レベルを使用した。
cfDNAメチル化マッピングに適切な全ゲノム方法
ここで記述するcfMeDIP-seq法は、100ngの入力DNAまで堅牢である既存の低入力MeDIP-seq手順17の改変によって開発された。しかし、血漿試料の大部分は、100ngよりはるかに少ないDNAを生成する。この難題を克服するために、本発明者らは、開始DNAの量を100ngに人為的に膨張させるために、外来λDNA(フィラーDNA)をアダプター連結cfDNAライブラリに添加した(図2)。これは、抗体による非特異的結合の量を最小にし、プラスチック製品との結合のために失われるDNAの量も最小にする。フィラーDNAは、アダプター連結cfDNAライブラリにサイズが類似したアンプリコンからなり、非メチル化DNA及びCpG密度の異なるインビトロメチル化DNAで構成された。異なる患者は異なる量のcfDNAを生成するので、このフィラーDNAの添加は、実用にも役立ち、入力DNA量を100ngに規準化することができる。これによって、利用可能なcfDNAの量にかかわらず、下流の手順がすべての試料で確実に正確に同じままである。
cfMeDIP-seq手順の感度を評価するために、本発明者らは、結腸直腸癌(CRC)HCT116細胞系DNAを多発性骨髄腫(MM)MM1.S細胞系DNAに段階希釈した。どちらもcfDNAサイズを模倣するように剪断された。本発明者らは、CRC DNAを100%、10%、1%、0.1%、0.01%、0.001%から0%に希釈し、これらの希釈の各々についてcfMeDIP-seqを行った(図4A~D)。本発明者らは、同じ試料における3点変異の検出のために超深度(10,000×中央値カバー度)標的配列決定も行った。5%偽陽性率(FDR:False Discovery rate)閾値を用いた各CRC希釈ポイント対純粋MM DNAで同定されたDMRの観察数は、希釈係数に基づくDMRの予想数と0.001%希釈までほぼ完全に線形(r2=0.99、p<0.0001)であった(図1D)。さらに、これらのDMR内のDNAメチル化シグナルも、観察対予想シグナルでほぼ完全な線形性を示す(r2=0.99、p<0.0001)(図1E)。それに対して、1%希釈を超えると、超深度標的配列決定は、PCR又は配列決定誤差のために、CRC特異的変異体と偽変異体を確実に識別することができなかった(図1F)。したがって、cfMeDIP-seqは、癌由来のDNAの検出感度に優れ、標準手順を用いた超深度標的配列決定による変異体検出の性能を超える。
本発明者らは、血漿cfDNAのメチローム分析を初期癌におけるctDNAの検出に使用できるかどうか検討することにした。本発明者らは、24名の初期膵癌患者(症例)並びに24名の年齢及び性別対応健常ドナー(対照)の手術前血漿のメチローム分析を行った(表4A、4B及び5)。各患者について、高腫瘍純度のレーザーキャプチャー顕微解剖(LCM:laser-capture microdissected)腫瘍試料及び正常組織試料を調べた。cfMeDIP-seqを腫瘍及び正常組織上の循環cfDNA及びRRBSに対して行った(図5A及び図6、表6A及び6B)。t検定及びBenjamini-Hochberg補正を多重検定に使用して、本発明者らは、症例と対照cfDNAの間で38,085個のDMRを得た(p<0.01、q<0.1)(図5B~C)。
症例と対照の間のDMRは、腫瘍由来のDMRでは高度に濃縮されるので(図5D~E)、本発明者らは、cfDNAメチロームが腫瘍特異的又は組織関連活性転写因子に関連したモチーフの濃縮を明らかにすると仮定した。これらのcfDNAメチロームを使用して、これらのDNA分子の起源組織における活性転写ネットワークを推測することができた。TFの大部分が標的配列のDNAメチル化状態に基づく可変の結合を示すので28、活性転写ネットワークを推測するために、本発明者らは、cfDNA中のDMRが転写因子(TF)フットプリントの濃縮を明らかにし得るかどうか検討した。モチーフ分析をHOMERソフトウェア20を用いて低メチル化DMRに対して、健常ドナー(図8A)と膵癌患者(図8C)で別々に行って20、潜在的TFフットプリントを明らかにした。
Claims (22)
- (a)(i)対象の核酸試料に由来する核酸分子及び(ii)フィラーデオキシリボ核酸(DNA)分子を含む混合物を生成することであって、前記フィラーDNA分子は種々異なるCpG密度を有し、前記フィラーDNA分子が、前記核酸試料中の全DNAに対して少なくとも約9倍多い量で添加され、及び
(b)前記核酸分子のメチル化領域を濃縮するのに十分な条件で前記混合物を結合剤とインキュベートすること
を含む対象の核酸試料を処理する方法であって、前記フィラーDNA分子が、濃縮倍比(fold enrichment ratio)を増加する、方法。 - 前記核酸試料が無細胞DNA(cfDNA:cell-free DNA)試料である、請求項1に記載の方法。
- (a)における前記混合物において、(i)前記核酸分子を複数含み、かつ(ii)前記フィラーDNA分子を複数含み、
(b)が前記複数の核酸分子の複数のメチル化領域を濃縮するために前記複数のフィラーDNA分子を使用することを含む、請求項1に記載の方法。 - 前記複数のメチル化領域が少なくとも約99%の特異性で濃縮される、請求項3に記載の方法。
- 前記複数のフィラーDNA分子が約5%のメチル化フィラーDNA分子を含む、請求項3に記載の方法。
- 前記フィラーDNA分子が約50bp~約800bpの長さを有する、請求項1に記載の方法。
- 前記長さが約100bp~約600bp長である、請求項6に記載の方法。
- 前記長さが約200bp~約600bp長である、請求項6に記載の方法。
- 前記混合物が少なくとも約50ngの核酸分子を含む、請求項3に記載の方法。
