本願は、一般に心臓リズム障害に関する。より詳しくは、本願は、心臓リズム障害と関連した心臓の活性化情報(活性化開始;activation onset)を再建する(reconstructing)ためのシステムおよび方法に関する。
心臓(心臓の)リズム障害は、普通にみられて、世界中で羅病率および死の重要な原因をなす。心臓における電気系の不具合は、心臓リズム障害の主因をなす。心臓リズム障害は、多くの形態において存在する。そのうち治療するのが最も複雑でかつ困難なものは、心房細動(AF;atrial fibrillation)、心室頻拍(VT;ventricular tachycardia)および心室細動(VF;ventricular fibrillation)である。他のリズム障害は、より治療しやすいが、心房頻拍(AT;atrial tachycardia)、上室性頻拍(SVT;supraventricular tachycardia)、心房粗動(AFL;atrial flutter)、上室性異所性複合体/鼓動(SVE;supraventricular ectopic complexes/beats)および心室性期外錯体/鼓動(PVC;premature ventricular complexes/beats)を含んで、臨床的に重要でもよい。通常の状況下では洞結節が心臓を洞リズムに保つとはいえ、特定の状況下で、正常な洞結節の急速な活性化によって不適切な洞性頻拍または洞結節リエントリが生じることがありえる。その両方もまた、心臓リズム障害を表す。
Claims (19)
- コンピューティングデバイスを用いて心臓の活性化情報を再建する方法であって、前記コンピューティングデバイスは、
第2の心臓信号の微分に関して第1の心臓信号の微分において閾値を上回る変化点があるかどうか決定するために、前記第1の心臓信号および前記第2の心臓信号を処理し、および、
前記変化点が前記閾値を上回ると決定される場合、前記第1の心臓信号において鼓動を示す心臓活性化を定めるために前記変化点で前記第1の心臓信号における活性化開始時期を割り当てる、方法。 - 前記変化点は、前記第1の心臓信号および前記第2の心臓信号にとって同じ時点について決定される、請求項1に記載の方法。
- 前記変化点は、前記第1の心臓信号および前記第2の心臓信号にとって傾斜、振幅、タイミングおよび形状の1つ以上から決定される、請求項1に記載の方法。
- 請求項1に記載の方法であって、前記変化点の決定は、前記コンピューティングデバイスが、
前記第1の心臓信号および前記第2の心臓信号から合成心臓信号を形成し、
前記第1の心臓信号における複数の位置での比率値であって、各比率値は、前記第1の心臓信号の微分と前記合成心臓信号の微分との違いに対する、前記第2の心臓信号の微分と前記合成心臓信号の微分との違いを表す、比率値を決定し、および、
前記第1の心臓信号における前記変化点として、前記決定された比率値から最大の比率値を有する1点を選択すること、
を含む方法。 - 前記閾値は、前記第1の心臓信号および前記第2の心臓信号に関連したノイズレベルよりも高い、請求項1に記載の方法。
- 前記ノイズレベル以下の変化点は、心臓の他の領域、呼吸器系、消化管、神経系、および電子妨害からの1つ以上の信号と関係する、請求項5に記載の方法。
- 請求項1に記載の方法であって、前記コンピューティングデバイスが、
前記閾値を上回る変化点がないと決定される場合、心臓信号のカタログにおいて前記第1の心臓信号の少なくとも1つの特性を参照心臓信号の少なくとも1つの特性にマッチさせ、および、
前記第1の心臓信号において鼓動を示す心臓活性化を定めるために、前記参照心臓信号の活性化開始時期として前記第1の心臓信号における活性化開始時期を割り当てること、
をさらに含む方法。 - 前記第1の心臓信号において鼓動を示す複数の心臓活性化を定めるために、前記コンピューティングデバイスが、処理および割り当てを実行することをさらに含む、請求項1に記載の方法。
- 前記コンピューティングデバイスが、複数の心臓信号から前記第1の心臓信号および前記第2の心臓信号を反復的に選択することをさらに含む、請求項1に記載の方法。
- 請求項1に記載の方法であって、前記コンピューティングデバイスが、
複数の心臓信号から、第1の心臓信号および異なる第2の心臓信号を有するペアの心臓信号を反復的に選択し、
前記ペアの各々において前記第1の心臓信号にとっての鼓動を示す複数の心臓活性化を定めるために、前記ペアの各々に対して処理および割り当てを実行し、および、
心臓リズム障害の出所を示すために、前記複数の心臓信号から心臓活性化の割り当てられた活性化開始時期に基づいて心臓活性化パターンを再建すること、
をさらに含む方法。 - 心臓の活性化情報を再建するシステムであって、
少なくとも1つのコンピューティングデバイスを備え、前記少なくとも1つのコンピューティングデバイスは、
第2の心臓信号の微分に関して第1の心臓信号の微分において閾値を上回る変化点があるかどうか決定するために、前記第1の心臓信号および前記第2の心臓信号を処理して、そして、
前記変化点が前記閾値を上回ると決定される場合、前記第1の心臓信号において鼓動を示す心臓活性化を定めるために前記変化点で前記第1の心臓信号における活性化開始時期を割り当てる、
ように構成される、システム。 - 前記少なくとも1つのコンピューティングデバイスによって実行されるときに、前記少なくとも1つのコンピューティングデバイスに処理させておよび割り当てさせる命令を含むコンピュータ可読媒体をさらに備える、請求項11に記載のシステム。
- 前記変化点は、前記第1の心臓信号および前記第2の心臓信号の両方にとって同じ時点について決定される、請求項11に記載のシステム。
- 請求項11に記載のシステムであって、前記少なくとも1つのコンピューティングデバイスは、
前記第1の心臓信号および前記第2の心臓信号から合成心臓信号を形成して、
前記第1の心臓信号における複数の位置での比率値であって、各比率値は、前記第1の心臓信号の微分と前記合成心臓信号の微分との違いに対する、前記第2の心臓信号の微分と前記合成心臓信号の微分との違いを表す、比率値を決定して、そして、
前記第1の心臓信号における前記変化点として、前記決定された比率値から最大の比率値を有する1点を選択する、
ようにさらに構成される、システム。 - 請求項11に記載のシステムであって、前記少なくとも1つのコンピューティングデバイスは、
前記閾値を上回る変化点がないと決定される場合、心臓信号のカタログにおいて前記第1の心臓信号の少なくとも1つの特性を参照心臓信号の少なくとも1つの特性にマッチさせて、そして、
前記第1の心臓信号において鼓動を示す心臓活性化を定めるために、前記参照心臓信号の活性化開始時期として前記第1の心臓信号における活性化開始時期を割り当てる、
ようにさらに構成される、システム。 - 前記少なくとも1つのコンピューティングデバイスは、前記第1の心臓信号において鼓動を示す複数の心臓活性化を定めるために、処理および割り当てを実行するようにさらに構成される、請求項11に記載のシステム。
- 前記少なくとも1つのコンピューティングデバイスは、複数の心臓信号から前記第1の心臓信号および前記第2の心臓信号を反復的に選択するようにさらに構成される、請求項11に記載のシステム。
- 前記第1の心臓信号および前記第2の心臓信号を検出するために少なくとも第1のセンサおよび第2センサをそれぞれ有するカテーテルをさらに備える、請求項11に記載のシステム。
- 請求項11に記載のシステムであって、前記少なくとも1つのコンピューティングデバイスは、
複数の心臓信号から、第1の心臓信号および異なる第2の心臓信号を有するペアの心臓信号を反復的に選択して、
前記ペアの各々において前記第1の心臓信号にとっての鼓動を示す複数の心臓活性化を定めるために、前記ペアの各々に対して処理しておよび割り当てて、
心臓リズム障害の出所を示すために、前記複数の心臓信号から心臓活性化の割り当てられた活性化開始時期に基づいて心臓活性化パターンを再建して、そして、
心臓リズム障害を抑制するか、減少させるかまたは除去するために、出所での心臓組織の処置を容易にするために、再建された心臓活性化パターンを表示する、
ようにさらに構成される、システム。
