材料及び方法
臨床評価
代表的な一実施例では、過去に治療歴のない低リスク型AMLを有する高齢者においてチピファルニブ(R115777、ZARNESTRA(登録商標))によるファルネシルトランスフェラーゼ阻害の有効性及び安全性を調べる非盲検、多施設共同、非比較対照の第2相試験において、骨髄試料を採取した。
同意を得た患者から、チピファルニブによる治療を行う前に骨髄試料を採取した後、好ましくはその場で単核細胞を処理した。骨髄穿刺液をPBSで希釈し、フィコール-ジアトリゾ酸(1.077g/mL)とともに遠心した。濃縮された白血病血液細胞をPBSで2回洗い、10% DMSOを含むFBSに再懸濁し、-70℃~-80℃で直ちに凍結した。全RNAを、Trizolキット(キアゲン社(Qiagen)カリフォルニア州サンタクララ)を使用して細胞試料から抽出した。RNAの品質は、Agilent Bioanalyzer上でリボソームのバンドの存在を評価することによって調べることができる。良好な品質の試料を更にマイクロアレイ分析用に処理した。製造者の指示(キアゲン社(Qiagen)カリフォルニア州サンタクララ)に従って、Trizol処理した骨髄の同じ試料からDNAを単離した。試料を特定の遺伝子の網羅的遺伝子発現、N-RAS変異、及び/又はqPCRについてアッセイした(図1)。
N-RASの突然変異の活性化の分析を、上記に述べたようなPCR及びRFLP分析によって調べた。エンド(End)ら(2001)。N-RAS遺伝子のエクソン1及び2を1回の多重反応において同時に増幅し、アリコートを2ラウンド目のPCRで使用した。2ラウンド目のアンプリコンの天然の制限酵素部位又はプライマーにより誘導した制限酵素部位における切断に対する耐性は、分析される遺伝子座のその部位を破壊した突然変異の存在を示している。特定の遺伝子座の分析に用いた制限酵素は、Bsl I(N-rasコドン12及び13)、Msc I(N-rasコドン61、1番目及び2番目)、並びにBfa I(N-rasコドン61、3番目)であった。反応物を一晩消化し、PCR産物をAgilent Bioanalyzerで分析した。
アフィメトリックス社(Affymetrix)(カリフォルニア州サンタクララ)のプロトコールに従ってcDNA及びRNAの合成を行った。多くの試料の収率は低かったため、米国特許出願公開第20070048782号に以前に述べられているようにして2ラウンドの線形増幅を行った。ハイブリダイゼーションを行うため、11μgのcRNAを、40mMのTris-酢酸(pH8.1)、100mMの酢酸カリウム、及び30mMの酢酸マグネシウム中、94℃で35分間インキュベートすることによってランダムにフラグメント化した。フラグメント化されたcRNAを、60rpmに設定したロティサリーオーブン中で、45℃で16時間、U133Aアレイとハイブリダイズさせた。ハイブリダイゼーション後、アレイを洗い(Triton X-100(0.005%)を含む6×SSPE及び0.5×SSPEにより)、ストレプトアビジン-フィコエリスリン(SAPE、モレキュラープローブズ社(Molecular Probes)オレゴン州ユージーン)により染色した。Agilent G2500A GeneArrayスキャナー(アジレント・テクノロジーズ社(Agilent Technologies)カリフォルニア州パロアルト)を使用して、結合した標識プローブの定量化を行った。
チピファルニブに対する応答は、完全応答(CR)、部分応答(PR)、及び血液学的改善(HI)を有する患者として定義した。簡単に言うと、HIは、5%未満の任意の骨髄芽球数又は骨髄芽球の少なくとも半分の減少として定義した。進行疾患状態(PD)は、骨髄又は循環芽球の比率(%)の、ベースラインからの50%よりも大きい増大、又は循環芽球の新たな出現(少なくとも連続して2回)によって定義した。安定疾患状態(SD)は、CR、PR、HI、又はPDの基準を満たさない任意の応答として定義した。
受信者操作特性(ROC)分析を用いて、個々の遺伝子及び/又は多重遺伝子分類指標の全体の予測値を検定した。以下の遺伝子フィルタリング基準を用いて、応答患者と進行疾患状態の患者との間で発現に差が見られる遺伝子を同定した。すなわち、感度100%で「応答患者」を同定する特異度≧40%、T検定のp値(不等分散によりlog2変換したデータ)<0.05、倍率変化(fold change)>2。これらの基準をパスした遺伝比をAUC(ROC曲線下面積)によってランク付けした。
各試料について1μgの全RNA(OD260により評価したもの)を、製造者の指示に従ってHigh Capacity cDNA Reverse Transcriptionキット(アプライド・バイオシステムズ社(Applied Biosystems)カリフォルニア州フォスターシティー)を使用して逆転写した。次いで試料を25℃で10分間、更に37℃で30分間インキュベートして、RNAの転換率を最適化した。ABI Prism 7900HT配列検出システム(アプライド・バイオシステムズ社(Applied Biosystems)カリフォルニア州フォスターシティー)を使用してQPCRを行い、すべての試料について3重で行った。各反応液は、UNGを含む5μLのTaqMan(登録商標)Universal PCR Master Mix(アプライド・バイオシステムズ社(Applied Biosystems)カリフォルニア州フォスターシティー)、4.5μLのcDNAテンプレート、及び0.5μLの20×Assay on Demand Gene Expression Assay Mix、又は9pmolの順方向及び逆方向プライマーの両方、及び2.5pmolのプローブ(アプライド・バイオシステムズ社(Applied Biosystems)カリフォルニア州フォスターシティー)を、全反応体積10μL中に含んでいた。すべてのプライマー、プローブセットは、小さいアンプリコンのサイズ(100ヌクレオチド未満)のために選択されたものであり、FAM蛍光性プローブを使用した。使用したプライマー及びプローブは、APTX(製品番号4331182、アプライド・バイオシステムズ社(Applied Biosystems))及びRASGRP1(製品番号4351372、アプライド・バイオシステムズ社(Applied Biosystems))である。RASGRP1:APTX発現比は、試料セットから平均Ctを引き、標準偏差で割ることにより生のCt値を正規化し、更に各遺伝子の正規化されたCt値の差(APTX-RASGRP1)を計算することによって計算した。
本研究では、過去に治療歴のない低リスク型の急性骨髄性白血病を有する高齢患者における、ファルネシルトランスフェラーゼ阻害剤チピファルニブの第2相臨床試験に組み入れられた患者からの白血病骨髄試料の遺伝子発現プロファイルを調べた。ランセット(Lancet)ら(2006)67人の患者からチピファルニブによる治療を行う前に骨髄を採取し、白血病骨髄細胞をフィコール密度遠心分離によって濃縮した(表1)。13人の応答患者(9人のCR、4人のHI)、8人の安定疾患状態、及び13人の進行性疾患状態の患者から得た良好な品質の全RNAを、増幅、標識して、Affymetrix U133A GeneChipにハイブリダイズした。全部で30個の試料をqPCRにより特異的遺伝子の検証について評価し、32個の試料をN-RAS変異状態について評価した。
32人のAML患者の骨髄からのDNAを、N-RAS活性化変異(コドン12、13、61)についてスクリーニングした。患者の34%(11/32)がN-Ras変異を示し、1人の患者が複数のコドンに変異を有していた(表2)。N-RAS変異状態と、チピファルニブに対する応答又は全生存期間との間に統計的に有意な相関は認められなかった。
次の目的は、新たに診断されたAML集団においてチピファルニブに対する応答を予測する遺伝子を同定することである。そのため、13人の応答患者(9人のCR及び4人のHI)及び進行疾患状態の13人の患者において発見実験を行った。安定疾患状態の患者は、応答患者とも非応答患者とも明確に定義することができないことから、この分析では用いなかった。本発明者らは、再発性及び不応性AMLに対するマーカーを同定するために用いられたもの(米国特許出願公開第20070048782号)と同じアプローチを用いて、応答を予測する45個のプローブセット(38個の固有の遺伝子に対応する)を同定した(表3)。この選択基準は、特異度のカットオフ値が40%であり、平均遺伝子発現差が少なくとも2倍となる高い感度(100%に近づく)で応答患者を予測する遺伝子を同定することを狙ったものである。これらの遺伝子を、訓練セットの受信者操作特性(ROC)分析から定義される曲線下面積(AUC)に基づいてランク付けした。この値は、完全な分類を表示するAUCが1.0の遺伝子の全体的予測値を表す。各遺伝子を、LOOCV法を用いて訓練セットで最初に検定した。最上位の遺伝子であるRASグアニル放出タンパク質1(RASGRP1)は、AUC 0.95を示した。
RASGRP1の予測値は、直線的な手法により分類指標に更なる遺伝子を追加した場合に改善されなかった。代替的な遺伝子選択アルゴリズムを用いて、RASGRP1単独の予測値を高めると思われる遺伝子を選択した。そのため、最上位スコアペア(TSP)アルゴリズムを用いて、最大の予測精度をもたらす最良の遺伝子のペアを同定した。ゲマン(Geman)ら(2004)。この手法は、2遺伝子間の発現の最大の差を利用するために用いられ、qPCRに基づく診断アッセイの開発を目的とした場合に有用であり得る。訓練セットからのTSPは、RASGRP1及びアプラタキシン(APTX)とした。RASGRP1及びAPTXは、応答患者においてそれぞれ過剰発現及び発現低下した。ロバストなLOOCVにより、この最上位スコアペア(TSP)によって試料の訓練セットにおいて85.7%のNPV及び91.7%のPPVが得られ、全体誤差率はわずか8%であることが示された(図2A)。予測される応答患者と非応答患者との間の全体生存期間の差は、357日であった(図2B)。これらのデータは、このモデル構築アルゴリズムはこれにともなう予測誤差率が低いことを示している。
54人の再発性/不応性AML患者からの試料からなる独立したマイクロアレイデータセットにおいて、TSP分類指標の外部検証を行った。癌治療に対する応答が高確率であることを予測する診断アッセイは、できるかぎり多くの潜在的応答患者を拾うことが重要であることから、高い感度(及び陰性予測値)を有するものでなければならない。したがって、TSP分類指標を試験するための適当なカットオフを定義するうえで、特異度を許容可能なレベルに維持しながら、応答患者を予測する高い感度を得る必要性を考慮した。訓練セットでは、実現可能な特異度のレベルは、感度が100%~80%に設定されている場合にそれぞれ約30%~100%の範囲であった。分類指標ができるだけ多くの応答患者を予測するようにするため、訓練セットにおいて約60%の特異度を与えた控えめなカットオフを検定した。このカットオフを再発性/不応性AMLの独立した試験セットに適用したところ、RASGRP1:APTX遺伝子分類指標により、応答患者は92%NPV及び27.6%PPVで層別化された(18.5%有病率に対し)(図3C)。応答患者であることにともなうオッズ比は4.38であった。これは、RASGRP1単独の予測精度と同様の値であるが、TSP分類指標を適用することにより、より良好なNPVが得られ、また、予測される応答患者と進行患者との全生存期間の差は98日であり(図3A)、RASGRP1の場合のわずか56日(図4)と比較して改善が示された。
2遺伝子発現比は、より臨床的に関連のあるqPCR検出システムの使用を可能とするものである。30人分の試料(20人のPD、6人のCR、3人のHI、及び1人のPR)より、qPCRを行ううえで充分な全RNAを得た。したがって、新たに診断されたAMLの臨床試験からのこれら30人分の試料(10人の応答患者、20人の進行疾患状態)においてTaqMan(登録商標)qPCRを使用して、RASGRP1:APTX遺伝子発現比を、チピファルニブに対する応答の予測指標として評価した。これらの試料のうちの9個はマイクロアレイプラットフォーム上でアッセイを行ったが、21個は低品質のRNAのために発見セット(discovery set)において用いなかった。したがって、この試験セットの2/3は、完全に独立した試料からなるものであった。
2遺伝子発現比を、化学療法レジメンによる治療を受けた116人のAML患者の独立したマイクロアレイデータセットにおいて検定した。RASGRP1:APTX分類指標を、チピファルニブ治療群の場合と同様のカットオフを用いてこの患者群に適用したところ、全生存期間に有意な乖離は認められなかった(図5)。他の様々なカットオフを用いた場合にも、有意な生存期間の差は認められなかった(表4)。このことは、RASGRP1:APTX分類指標がチピファルニブによる治療を受けた患者を特異的に層別化するものであり、非FTIとは関係がないことを示すものである。これに対して、この予後特性サインを本研究の再発性及び不応性AML患者の群に適用した場合、全生存期間に関して明確な層別化が見られた。
本実施例では、2遺伝子アッセイをFTI併用療法に適用するのに適した改良されたRT-PCRアッセイについて述べる。分析アッセイの開発に際して、RASGRP1(RASを活性化するグアニンヌクレオチド交換因子)、APTX(DNAの除去修復に関与しているアプラタキシン)、及びHMBS(内因性コントロールとして使用する)の3つのマーカーについてTaqmanアッセイを設計した。HMBSは一実施形態においてコントロールとして使用することができるか、又は他の実施形態では省略してもよい。Taqmanプライマープローブセット及びそれらのアンプリコンの配列を上記の「課題を解決するための手段」の項、及び本開示の配列表に示す。すなわち、APTXについては配列番号3~4及び2、RASGRP1プライマープローブセットについては配列番号56及び1、及びHMBSについては配列番号7~9である。

50ngの全RNAを3重qRT-PCRにおいて標的インプットとして使用し、これを25μLの反応液中、Applied Biosystems Prism 7500 Sequence Detection Systemで行った。RNA試料(正規化コントロールRNAを含む)を氷上で解凍し、10ng/μLにまで希釈した。qRT-PCRは、VERIDEX(登録商標)BLN RT-PCR Kit(GeneSearch(登録商標)Breast Lymph Node(BLN)Test Kit、IVD、カタログ番号2900004):GeneSearch(登録商標)BLN Enzyme Mix、IVD、P/N 7700040及びBLN Base Master Mix、P/N 7700031からの試薬を使用して行った。
1.組み入れ基準
ABI7500データアウトプットファイルから得られたサイクル閾値(Ct)値をデータ分析に使用した。各マーカーについて「試験せず」のCtカットオフを以下のように選択した。
HMBS(内因性コントロールマーカー):Ctは30以下、
RASGRP1 Ct:35以下、
APTX Ct:35以下。
本実施例では、2遺伝子アッセイが新たに診断されたAML患者において有効であることを示す。2遺伝子応答予測アッセイを、チピファルニブ及びエトポシドの第1相投与試験に組み込まれた84人の新たに診断されたAML患者のサブセットから得た白血病芽球で、Blood 2008,111:2589に述べられる技術的RT-PCRプロトコールを用いて最初に試験した。第1相試験の臨床計画は、Blood 2009,113:4841に述べられている。
RUO及び最適化GMP RT-PCRフォーマットを用い、臨床応答について評価可能な新たに診断されたAML患者33人からの骨髄試料から遺伝子発現プロファイルを分析した。RUO RT-PCRデータセットからの1人の患者試料(HI)が分析組み入れ基準を満たさなかったため、両方のデータセットから除外した。最も良好な応答による患者の内訳を表12に示す。
更なる改良の後、GMPフォーマットの1チューブ多重型RT-PCRアッセイを、33人分の骨髄試料の既存のセットに4人分の評価可能な全血試料を追加することにより、37人の患者で試験した。骨髄及び全血試料では3つのマーカーについて同等の生のCt値が得られたことから、これらを1つのデータセットに合わせて表18にまとめる。受信者操作特性(ROC)分析により、CR(9)/全患者(37)として計算された、全体奏功率(24%)に一致する応答基準として用いた完全寛解(CR)患者群における完全寛解率を予測するための2遺伝子比の差別値として80%(AUC=0.80)が示された。これを図9及び14に示す。2遺伝子比カットオフ5.2で、アッセイの感度は67%、特異度は93%であった。
本実施例では、上記の実施例と一致して、2遺伝子アッセイがFTIを含む治療に対して特異的であることを示す。別の相乗的な薬剤が存在することで、2遺伝子アッセイが有効性を示すためにFTIに求められる条件が大きく低減されることはない。
本実施例では、すべての試料採取法が同等ではないことを示す。過剰なRNA分解、及び試料の採取プロトコールに依存した他の変動要因によって、2遺伝子アッセイは影響され得る。
本実施例は、実施例3の一部として上記に述べた患者の一部に基づいたものであり、FTI併用療法を実施するうえで2遺伝子アッセイの助けにより毒性と患者の効果的治療とのバランスをとるために、チピファルニブの用量を調節することについて述べる。この後日の分析は、チピファルニブ及びエトポシドのより適当な投与計画を決定するうえで行われたものであり、CRを応答患者としてみなし、残りを非応答患者としてみなすことにより、RUO及びGMP RT-PCRに関する上記の分析をやはり改変するものである。この後日の分析の結果は、上記に述べた実施例に示した。
治療応答を予測するための患者集団の層別化は、癌患者の臨床管理におけるその重要性が益々高まっている。例えば、質の高い治療を行ううえで、転移性乳癌におけるトラスツズマブ(ヘルセプチン、ジェネンテック社(Genentech))及び直腸結腸癌におけるセツキシマブ(エルビタックス、メルク社(Merck))などの標的療法の候補となる患者を層別化するためにコンパニオン診断が必要とされる。消化管間質腫瘍におけるイマチニブ(グリベック、ノバルティス社(Novartis))、並びに肺癌におけるエルロチニブ(タルセバ、オー・エス・アイ・ファーマシューティカルズ社(OSI Pharmaceuticals))及びゲフィニチブ(イレッサ、アストラ・ゼネカ社(Astra-Zeneca))では、予測用バイオマーカーが更に利用されている。現在のところ、FTIを含む併用療法に対する応答を予測する方法は存在していない。

(2) 前記RASGRP1及びAPTXの発現の比が、
(i)5’-CGCTTCCGATTGGGCTAC-3’、
(ii)5’-AGAATCAAAATCCTGGCTGATC-3’、
(iii)5’-CTGGACGATCTCATTGACAGC-3’、及び
(iv)5’-CTTGCAACAGTTGGTTACTTCG-3’からなる群からの少なくとも1つのプライマーを用いて決定される、実施態様1に記載の方法。
(3) RASGRP1の発現のレベルが、APTX、βアクチン及びHMBSからなる群のうちの1つ又は2つ以上に対して推定される、実施態様1に記載の方法。
(4) 前記ΔΔCt閾値が約5.2である、実施態様1に記載の方法。
(5) 前記ΔΔCt閾値が、約70%以上の曲線下面積に対応している、実施態様1に記載の方法。
(6) アッセイは、1個のチューブ内で多重フォーマットで行われる、実施態様1に記載の方法。
(7) 外因性コントロールを用いて、APTXに対するRASGRP1の発現レベルの比を決定することを更に含む、実施態様2に記載の方法。
(8) 前記外因性コントロールが、1つ又は2つ以上の参照細胞系統、JY RNA、Universal RNA、標準化された参照RNA、及び参照患者試料からなる群から選択される、実施態様7に記載の方法。
(9) 試料が、
(i)骨髄試料、及び
(ii)血液試料、の少なくとも一方を含む、実施態様1に記載の方法。
(10) 前記患者が、チピファルニブ及びチピファルニブと相乗作用を示す別の薬剤により治療され、前記チピファルニブと相乗作用を示す別の薬剤が、エトポシド、テニポシド、タモキシフェン、ソラフェニブ、パクリタキセル、テモゾロミド、トポテカン、トラスツズマブ、及びシスプラチナムからなる群から選択される要素の1つ又はその誘導体である、実施態様1に記載の方法。
(12) 疾患と診断された患者にチピファルニブを処方するための方法であって、
(i)5’-CGCTTCCGATTGGGCTAC-3’、
(ii)5’-AGAATCAAAATCCTGGCTGATC-3’、
(iii)5’-CTGGACGATCTCATTGACAGC-3’、及び
(iv)5’-CTTGCAACAGTTGGTTACTTCG-3’からなる群からの少なくとも1つのプライマーを用いた標的リボ核酸からのシグナルの増幅においてRASGRP1及びAPTXの発現を評価することを含む、方法。
(13) RASGRP1遺伝子の発現の比が、APTXの発現レベルに対して推定される、実施態様12に記載の方法。
(14) 前記アッセイが、1個のチューブ内で多重フォーマットで行われる、実施態様12に記載の方法。
(15) 外因性コントロールを使用して、APTXに対するRASGRP1の発現レベルの比を決定することを更に含む、実施態様13に記載の方法。
(16) 前記APTXに対するRASGRP1の発現レベルの比を、約5.2のΔΔCt閾値と比較する、実施態様15に記載の方法。
(17) 前記APTXに対するRASGRP1の発現レベルの比を、約4.7のΔΔCt閾値と比較する、実施態様15に記載の方法。
(18) 前記APTXに対するRASGRP1の発現レベルの比を、特定の感度若しくは特異度又はROC分析における感度と特異度との最大の和に対応した閾値と比較する、実施態様15に記載の方法。
(19) 曲線下面積が約70%以上である、実施態様18に記載の方法。
(20) 前記外因性コントロールが、1つ又は2つ以上の参照細胞系統、JY RNA、Universal RNA、標準化された参照RNA、及び参照患者試料からなる群から選択される、実施態様15に記載の方法。
(i)骨髄試料、及び
(ii)血液試料、の少なくとも一方を含む、実施態様12に記載の方法。
(22) 患者における前記APTXに対するRASGRP1の発現レベルの比が閾値よりも大きい場合に、前記患者にチピファルニブを処方する、実施態様15に記載の方法。
(23) チピファルニブが、チピファルニブと相乗作用を示す別の薬剤とともに処方され、前記チピファルニブと相乗作用を示す別の薬剤が、エトポシド、テニポシド、タモキシフェン、ソラフェニブ、パクリタキセル、テモゾロミド、トポテカン、トラスツズマブ、及びシスプラチナムからなる群から選択される要素の1つ又はその誘導体である、実施態様17に記載の方法。
(24) ファルネシルトランスフェラーゼ阻害剤と相乗作用を示す前記別の薬剤が、エトポシドである、実施態様19に記載のアッセイ。
(25) 骨髄性疾患と診断された患者に対する治療を同定するための方法であって、
少なくとも標的リボ核酸の増幅に基づいて、第1の治療成績を有する第1のアッセイを投与することと、
前記第1の治療成績及び所定の閾値に基づいて前記患者を、治療群のうちの1つ又は2つ以上による効果が得られる確率が低いものとしてフラグ付けすることであって、前記治療群のそれぞれが、ファルネシルトランスフェラーゼ阻害剤と、エトポシド、テニポシド、タモキシフェン、ソラフェニブ、パクリタキセル、テモゾロミド、トポテカン、トラスツズマブ、及びシスプラチナムからなる群から選択される相乗作用を有する薬剤との投与を必要とする、フラグ付けすることと、
前記患者がフラグ付けされない場合に、前記治療群から治療を選択することと、を含む、方法。
(26) 前記所定の閾値が、前記治療群のうちの1つ又は2つ以上に対する応答について高い陰性予測値を有するように選択される、実施態様25に記載の方法。
(27) 前記所定の閾値が、前記治療群のうちの1つ又は2つ以上に対して高い陽性予測値を有するように選択される、実施態様25に記載の方法。
(28) 前記治療の相対陽性予測値に基づいて前記治療を選択することを更に含む、実施態様25に記載の方法。
(29) 前記骨髄性疾患が急性骨髄性白血病である、実施態様25に記載の方法。
(30) 過去の治療に対する応答の低下が検出されることに応じて、前記過去の治療から今後の治療に変えることを更に含む、実施態様25に記載の方法。
(32) 完全寛解が、5%未満の骨髄芽球が存在し、いずれの細胞系統も正常に成熟していること、ANCが少なくとも1000/μLであり、血小板数が少なくとも100,000/μLであること、末梢血中に芽球が存在しないこと、骨髄中に同定可能な白血病細胞が存在しないこと、疾患に関連した細胞遺伝学的異常が認められないこと、及び以前に存在していたあらゆる髄外疾患が認められないことによって定義される、実施態様31に記載の方法。
(33) 前記陽性応答が、部分寛解及び血液学的改善の1つ又は2つ以上を更に含む、実施態様32に記載の方法。
(34) 部分寛解が、ANC及び血小板の、上記に述べたレベルまでの回復をともなうが、骨髄芽球は5~25%である、前記骨髄中の三血球系造血の存在、及び骨髄芽球の比率のベースラインから少なくとも50%の減少により定義され、血液学的改善が、骨髄芽球の少なくとも50%の減少若しくは任意の測定可能な髄外疾患の減少、ANCの500~1000/μLへの回復、血小板数の20,000~100,000/μLへの回復、又は輸血の必要性における改善によって定義される、実施態様33に記載の方法。
(35) (i)5’-CGCTTCCGATTGGGCTAC-3’、
(ii)5’-AGAATCAAAATCCTGGCTGATC-3’、
(iii)5’-CTGGACGATCTCATTGACAGC-3’、及び
(iv)5’-CTTGCAACAGTTGGTTACTTCG-3’からなる群からの少なくとも1つのプライマーを用いた標的リボ核酸からのシグナルの増幅においてRASGRP1及びAPTXの発現を評価することによって、ファルネシルトランスフェラーゼ阻害剤の投与を含む治療に対して応答する確率が高い患者を同定するためのアッセイ。
(36) RASGRP1遺伝子の発現のレベルが、APTX、βアクチン及びHMBSからなる群のうちの1つ又は2つ以上に対して推定される、実施態様35に記載のアッセイ。
(37) 前記アッセイが、1個のチューブ内で多重フォーマットで行われる、実施態様35に記載のアッセイ。
(38) 外因性コントロールを用いて、APTXに対するRASGRP1の発現レベルの比を決定することを更に含む、実施態様35に記載のアッセイ。
(39) 前記外因性コントロールが、1つ又は2つ以上の参照細胞系統、JY RNA、Universal RNA、標準化された参照RNA、及び参照患者試料からなる群から選択される、実施態様37に記載のアッセイ。
(40) 試料が、
(i)骨髄試料、及び
(ii)血液試料、の少なくとも一方を含む、実施態様35に記載のアッセイ。
(42) 前記ファルネシルトランスフェラーゼ阻害剤がチピファルニブであり、前記ファルネシルトランスフェラーゼ阻害剤と相乗作用を示す前記別の薬剤がエトポシドである、実施態様41に記載のアッセイ。
(43) ファルネシル阻害剤と、エトポシド、テニポシド、タモキシフェン、ソラフェニブ、パクリタキセル、テモゾロミド、トポテカン、トラスツズマブ、及びシスプラチナムからなる群から選択されるか又はこれらの群から選択される要素の誘導体である、別の薬剤との組み合わせによる治療に対する応答患者を同定するためのアッセイにおける閾値を選択する方法であって、
血液試料を処理してリボ核酸が濃縮された調製物を調製することと、
RASGRP1及びAPTXの発現レベルの比を得ることと、
データセットにおいて、前記治療の陽性予測値、応答患者を同定する陰性予測値、ROC分析におけるAUC、感度及び特異度からなるセットからの測定値の1つ又は2つ以上を最大化することであって、前記RASGRP1及びAPTXの発現レベルの比が、前記閾値と比較される、最大化することと、を含む、方法。
(44) 前記RASGRP1の発現レベルが、RT-PCRを用いて測定される、実施態様43に記載の方法。
(45) 前記RASGRP1の発現レベルが、特定の参照標準RNAと一致するように決定される前記閾値に対して評価される、実施態様44に記載の方法。
(46) 急性骨髄性白血病と診断された患者にチピファルニブ及びエトポシドを投与するための方法であって、
それぞれのレベルがΔΔCt法を用いて計算されたRASGRP1及びAPTXの発現の比が、試験集団における感度若しくは特異度又はROC分析における感度と特異度との最大の和に対応したΔΔCt閾値を上回るか否かを判定することと、
前記比が前記ΔΔCt閾値を上回る場合に、治療上の有効量のチピファルニブを投与することと、を含む、方法。
(47) 前記RASGRP1及びAPTXの発現の比が、
(i)5’-CGCTTCCGATTGGGCTAC-3’、
(ii)5’-AGAATCAAAATCCTGGCTGATC-3’、
(iii)5’-CTGGACGATCTCATTGACAGC-3’、及び
(iv)5’-CTTGCAACAGTTGGTTACTTCG-3’からなる群からの少なくとも1つのプライマーを用いて決定される、実施態様46に記載の方法。
(48) RASGRP1の発現のレベルが、APTX、βアクチン及びHMBSからなる群のうちの1つ又は2つ以上に対して推定される、実施態様46に記載の方法。
(49) 前記ΔΔCt閾値が約5.2である、実施態様46に記載の方法。
(50) 前記ΔΔCt閾値が、約70%以上の曲線下面積に対応している、実施態様46に記載の方法。
(52) 外因性コントロールを用いて、APTXに対するRASGRP1の発現レベルの比を決定することを更に含む、実施態様47に記載の方法。
(53) 前記外因性コントロールが、1つ又は2つ以上の参照細胞系統、JY RNA、Universal RNA、標準化された参照RNA、及び参照患者試料からなる群から選択される、実施態様42に記載の方法。
(54) 試料が、
(i)骨髄試料、及び
(ii)血液試料、の少なくとも一方を含む、実施態様46に記載の方法。
(55) 前記患者が、チピファルニブ及びチピファルニブと相乗作用を示す別の薬剤により治療され、前記チピファルニブと相乗作用を示す別の薬剤が、エトポシド、テニポシド、タモキシフェン、ソラフェニブ、パクリタキセル、テモゾロミド、トポテカン、トラスツズマブ、及びシスプラチナムからなる群から選択される要素の1つ又はその誘導体である、実施態様46に記載の方法。
(56) 急性骨髄性白血病と診断された前記患者が、年齢55才以上、好ましくは年齢60才以上、更により好ましくは年齢65才以上である、実施態様46に記載の方法。
Claims (6)
- 疾患と診断された患者がチピファルニブとエトポシドの投与を含む治療に対し高確率で応答することを前記患者からの試料が示すかどうかを決定するための方法であって、
(i)5’-CGCTTCCGATTGGGCTAC-3’、
(ii)5’-AGAATCAAAATCCTGGCTGATC-3’、
(iii)5’-CTGGACGATCTCATTGACAGC-3’、及び
(iv)5’-CTTGCAACAGTTGGTTACTTCG-3’からなる群からの少なくとも1つのプライマーを用いた標的リボ核酸からのシグナルの増幅においてRASGRP1及びAPTXの発現を評価することを含み、
APTXの発現レベルとRASGRP1の発現レベルのそれぞれがΔΔCt法を用いて計算され、当該APTXの発現レベルに対する当該RASGRP1の発現レベルの比が、試験集団のROC分析における特定の感度または特異性または感度と特異性との最大の和に対応するΔΔCt閾値を超えるかどうかが決定され、
RASGRP1遺伝子の発現の比が、APTXの発現レベルに対して推定され、
前記方法が、外因性コントロールを使用して、APTXの発現レベルに対するRASGRP1の発現レベルの比を決定することを更に含み、
前記外因性コントロールがUniversal RNAを使用して、前記APTXの発現レベルに対するRASGRP1の発現レベルの比を、7.3のΔΔCt閾値と比較する、
方法。 - 前記アッセイが、1個のチューブ内で多重フォーマットで行われる、請求項1に記載の方法。
- 曲線下面積が70%以上である、請求項1に記載の方法。
- 試料が、
(i)骨髄試料、及び
(ii)血液試料、の少なくとも一方を含む、請求項1に記載の方法。 - 前記疾患が骨髄性疾患である、請求項1に記載の方法。
- 前記疾患がAMLである、請求項1に記載の方法。
