Abstract
【解決手段】本明細書には、角膜の前面の形状を変化させることにより視力低下を矯正する小径のインレーが記載されている。1つの実施の形態において、瞳孔よりも小さい直径を有するインレーは、老眼を矯正するため提供される。近視力を提供するため、インレーはその内部のジオプトリー度が増大した角膜内面の「実効」領域を誘発させるよう角膜の中央に植え込まれる。「実効」領域の周縁方向の角膜領域により遠視力が提供される。別の実施の形態において、インレーよりも遥かに大きい直径である、有効な光学領域を角膜の前面に誘発させる小径のインレーが提供される。少なくとも部分的にドレーピング効果に起因する有効な光学領域の増大はインレーがインレーの直径よりも遥かに大きい臨床的効果を患者の視力に発生させることを許容する。

Description
1つの実施の形態において、瞳孔の直径よりも小さい直径を有するインレーは、老眼を矯正するため提供される。近視力を提供するため、インレーは、角膜内の中心に植え込まれて、角膜の光学領域よりも小さい「実効」領域を角膜の前面上に誘発させ、この「実効」領域は、インレーによる影響を受ける角膜の前面の領域である。植え込んだインレーは、「実効」領域内における角膜の前面の曲率を増大させ、これにより「実効」領域内の角膜のジオプトリー度を増大させる。インレーは、瞳孔の直径よりも小さいため、遠方の対物からの光線は、インレーを迂回し且つ「実効」領域に対する周方向の角膜領域を使用して屈折し、網膜上に遠方の対物の像を形成する。
老眼用インレー
この段落では、老眼を矯正するため、瞳孔と比較して小さい直径を有する小さい角膜内インレーについて説明する。好ましい実施の形態において、小さいインレー(例えば、直径1から2mm)は、角膜の中心に植え込まれて、角膜の光学領域よりも小さい「実効」領域を角膜の前面上にて誘発させ近視力を提供する。この場合、「実効」領域は、インレーによる影響を受ける角膜の前面である。植え込んだインレーは、「実効」領域内における角膜の前面の曲率を増大させ、これにより「実効」領域内の角膜のジオプトリー度を増大させる。遠視力は、「実効」領域に対して周縁方向の角膜領域により提供される。
(1) 最初のステップは、近視力の改善と遠視力の損失との間の許容可能な折り合いである最大光学領域(dz)を決定することである。考慮事項は、近くの物を読む間の特定の被験者及び特徴的な被験者の群(例えば、特定の年令範囲の被験者)の瞳孔の寸法、及び特に、夜間に遠視力のための瞳孔の寸法を含む。一例としての適用例において、一方の眼にインレーが配置されて近視力を提供し、また、その他方の眼にてその他の手段による遠視力の矯正が実行される。この例において、両方の眼が遠視力に寄与し、インレーの入っていない眼は最も鮮明な遠視力を提供する。インレーを有する眼は、近視力を提供する。
(3)以下に詳細に記載した方法を使用してインレーを設計する。この方法は、その内容の全体を参考として引用し本明細書に含めた、2005年12月1日付けで出願された、「角膜内インレーの設計(Design Of Intracorneal Inlays)」という名称の、米国特許出願明細書11/293,644に記載された設計方法と同様である。
図3及び図4には、インレーの設計による影響を受ける2つの領域、すなわちインレーの直径(dI)により規定される「中央領域」260及びインレー直径と光学領域(dz)との間に含まれる「ドレープ領域」255とが示されている。以下に記載した設計方法を使用して、老眼を矯正するため中央領域内にて角膜の前面に所望の形状を生じさせるようインレーを設計する。この設計方法は、インレー材料が角膜と同一の屈折率を有するものと想定する。

式において、Vは、眼鏡の角膜頂点間距離、例えば、眼鏡から角膜の前面までの0.012mである。角膜の屈折度の測定値を考慮した、眼鏡の角膜頂点間距離Vは、典型的に、眼鏡が角膜の前面からある距離に配置された状態にて測定し、また、これらの度の測定値を角膜の前面における等価的な度に変換することを考慮するものである。
設計及び分析の目的のため、Kポストの近似値は、−6ジオプトリーとすることができる。植え込み前の曲率半径r植え込み前は、次式により近似値を求めることができる。
2つの曲率半径は、同一の原点から開始する必要はない。
図5には、望まれる角膜の前面540と植え込み前の角膜の前面535との間の差により特定された厚さプロファイル510の断面図が示されている。図5において、植え込み前の前面535から望まれる前面540までを向く矢印550は、光軸線zに対して実質的に垂直なr軸線に沿った異なる位置における厚さプロファイル510の軸方向厚さL(r)を表わす。二重矢印560は、厚さプロファイルの中央厚さLcを表わす。この実施の形態において、厚さプロファイル510は、z軸線の周りにて回転対称である。このように、全体の厚さプロファイルは、図5に示した断面図をz軸線の周りにて回転させることにより規定することができる。

ここで、Lcは、厚さプロファイルの中央厚さであり、Z植え込み(r)は、rの関数として手術前の角膜の前面であり、Z新しい(r)は、rの関数として、望ましい角膜の前面であり、dIは、インレーの直径である。上述した例において、前面Z新しい及びZ植え込み前は、球形であると想定した。これは、必ずしもそうでなくてもよい。前面は、非球形であってもよい。より全体的に、望まれる前面Z新しいは、望まれるADDの関数とし、また、例えば、高次収差の矯正のため、球形面のような、より複雑な設計パラメータでもある。また、植え込み前の前面Z植え込み前は、全体として非球形である。非球形面を必要とする設計の場合、表面関数Z(r)は、全体として非球形の形態により与えることができる。

a4及びa6は、高次の非球形定数である。
非球形面に対し、k=0、a4=0、a6=0である。人間の角膜は、k=−0.16、a4=0、a6=0により近似値を得ることができる。曲率半径rcは、老眼の矯正のため、ADD度により特定することができ、また、その他のパラメータは、高次収差に対する矯正値を特定することができる。
増大した実効光学領域を有するインレー
幾つかの適用例において、インレーは、インレー直径よりも遥かに大きい実効光学領域を角膜の前面に誘発させることが望ましい。実効光学領域の増大は、インレーが実際のインレーの直径よりも遥かに大きい臨床的効果を患者の視力に対して発生させることを許容する。一例において、1.5mm−2mm範囲の直径のインレーは,4mm−5mmの増大した実効光学領域を有し、この場合、インレーの光学効果は、その直径よりも2xから3x大きい。増大した実効光学領域は、また、上述した範囲外のインレーの直径により実現することもできる。例えば、インレーの直径は、望まれる光学効果を実現しつつ、幾つかの設計に対して1mmよりも小さくすることができる。
ここで、ncは、角膜の屈折率。この場合にも、インレーの実効光学領域は、インレーの直径よりも大きかった。
Claims (36)
- 角膜内インレーを使用して老眼を治療する方法において、
インレーを角膜内に植え込むことにより近視力を矯正し得るよう角膜の前面の形状を変化させるステップを備え、インレーは、約2.5mm以下の直径を有し、遠視力はインレーによる影響を受ける前面の領域の周縁方向の角膜の領域により提供される、老眼を治療する方法。 - 請求項1に記載の方法において、インレーは、角膜の屈折率に実質的に等しい屈折率を有する、方法。
- 請求項1に記載の方法において、インレーは、角膜の屈折率より大きい屈折率を有する、方法。
- 請求項1に記載の方法において、インレーは、角膜の50%以下の深さにて植え込まれる、方法。
- 請求項1に記載の方法において、インレーは、角膜の25%以下の深さにて植え込まれる、方法。
