Abstract
Description
本発明は、神経学的症状の評価に関し、より具体的には、痴呆症の進行の診断およびモニタリング、うつ病の評価およびうつ病処置の有効性の予測に関する。本発明はまた、てんかん、パーキンソン病、注意疾患(多動性)障害、脳卒中、譫妄、覚醒状態および睡眠状態の評価の診断およびモニタリングに適用することもできる。
本発明は、疾患もしくは症状の有無または疾患もしくは症状の進行の有無を示す特徴および指標を作成する系および方法である。また本発明の系および方法は、前投薬ベースラインから投薬に対する応答性を予測する特徴および指標も作成する。さらに本発明の系および方法は、特徴または指標の性能特性を改良する検定方法論を包含する。このような特徴および指標を得るために、被検者より取得される生体電位シグナルから、パワースペクトル、時間領域、バイスペクトルおよび高次スペクトル値が導かれる。
図1に示す本発明の好ましい態様では、被検者(10)からEEGシグナルを取得するデータ取得装置(DAU)(20)が組み込まれており、このデータが以後プロセッシングされる。DAU(20)は典型的に、アナログ・デジタル(A−D)・コンバータ(25)が組み込まれているコンピュータシステムおよび一組の電極(被検者(10)の頭皮に設置して表示されている)からなる。単一の電極(15)のみが示されているが、EEGシグナルを得るために用いられる任意の構成(montage)の電極を本発明に利用しても良い。A−Dコンバータ(25)は、電極(15)から得られるアナログEEGシグナルを、サンプリングされた一組のシグナル値に変換するために用いられ、このシグナル値は次いでデータ演算処理装置(DCU)(30)のプロセッサ(processor)(35)によって分析することができる。DCU(30)はプロセッサ(35)および、DAU(20)からサンプリング値を受信する通信装置を組み込んでいる。記載されている態様では、DAU(20)およびDCU(30)のプロセッサは同一である。しかし別の態様では、DAU(20)はEEGシグナルを取得し、サンプリングされたEEGシグナルを通信リンクを介して遠隔のDCU(30)に送信することができる。このような通信リンクはシリアルまたはパラレルのデータライン、ローカルまたはワイドエリアネットワーク、電話線、インターネットまたは無線接続であってよい。評価を行う臨床医はキーボード(40)およびディスプレイ装置(50)を用いてDCU(30)と通信することができる。DAU(20)から遠隔のDCU(30)を利用する別の態様では、臨床医が使用する追加のキーボードおよびディスプレイ装置をDAU(20)に取り付けてもよい。
バイスペクトルアレイの計算
バイスペクトルアレイは周波数領域(フーリエ変換)法ならびに時間領域(自己回帰)法で計算することができる。用語バイスペクトルアレイまたはバイスペクトルには、自己および相互両形式に関して、すべてあるいは任意の以下のアレイが含まれる:複素三重積、実三重積、バイスペクトル密度、バイフェーズおよびバイコヒーレンスアレイ。さらにパワースペクトルアレイが中間工程として計算され、ある指標の特徴として用いられるパラメータを得るために利用可能である。両方法が以下に説明されているが、当業者であれば他の方法を導き出すことが同様に可能であることを理解するであろう。本発明はすべての演算処理方法を包含することを意図している。

工程818では、系は、以下の方程式を用いてバイフェーズアレイφ(f1,f2) を計算する:
[式中、Mは記録数であり、Nは記録当たりのサンプル数であり(好ましい態様では、それぞれ128および256)、Lは見込みのARフィルター次数よりずっと大きい(好ましい態様ではL=50)]。ARフィルターの次数を求めるため、工程910では、各自己相関数列に対してレビンソン(Levinson)再帰関数を実行することにより、全次数、m=0,1,2,...Lについて最終予測誤差、FPEX(m) およびFPEY(m) が計算される。それぞれFPEX(m) およびFPEY(m) の最小値の場所(location)、QXおよびQYが選択され、これがそれぞれXi(t) およびYi(t) のパワースペクトルのARフィルターの次数である。すなわち

指標はバイスペクトルアレイおよび/または他の周波数領域および時間領域の特徴を用いて構築することができる。このような指標はある疾患状態の有無を予測するように設計することができる。指標は分類子として設計することができ、評価対象である個人がこの疾患を有するか否かが予測され、あるいは罹患の確率が疾患進行の尺度として利用される。また指標は、神経学的機能または疾患状態/進行の継続的予測因子として設計することができる。このような指標の開発には、種々の進行段階での特定病理学的症状を有する個体ならびにこの特定症状を有しないコントロールの個体から得られるEEGデータからなるデータセットが必要とされる。またデータセットは指標設計上の予測対象である疾患状態の、他に依存しない評価を含まなければならない。
式中、c3は min および max の値(それぞれ最小値および最大値である)を用いて定義され、これによりIndex(痴呆症_進行) の範囲は0から100まで(両端を含む)となる。この実施例を得るために用いられるデータベースによれば、min(F3)=0.9、max(F3)=2.6であり、これによりc0=−52.9およびc3=58.8が得られる。MMSEと、F3の、すなわちIndex(痴呆症_進行) の相関は−0.64であり、これはIndex(痴呆症_進行) が痴呆症の程度についての高感度の尺度であることを示す。
上と同様に、c0およびc6は、Index(コントロール_痴呆症) の範囲が0から100まで(両端を含む)となるように定義される。この実施例を得るために用いられるデータベースによれば、min(F6)=0.4、max(F6)=1.1であり、これによりc0=−57.1およびc6=142.9が得られる。閾値50を用いると、Index(コントロール_痴呆症) は、感度94%、特異性83%および受信者動作曲線下領域(AUC)95%で、正常コントロール被検者から痴呆症患者を識別した。
上と同様に、c0およびc7 は、Index(ATD_MID) の範囲が0から100まで(両端を含む)となるように定義される。この実施例を得るために用いられるデータベースによれば、min(F7)=0.5、max(F7)=6.5であり、これによりc0=−333.3およびc7=66.7が得られる。閾値50を用いると、Index(ATD_MID) は、感度82%、特異性86%およびAUC91%で、MID患者とATD患者を識別した。
上と同様に、c0およびc10は、Index(うつ病_重篤度) の範囲が0から100まで(両端を含む)となるように定義されている。この実施例を得るために用いられるデータベースによれば、min(F10)=3.9、max(F10)=6.1であり、これによりc0=−177.3およびc10=45.5が得られる。Hamilton-D スコアとF10の、すわなちIndex(うつ病_重篤度) の相関は0.31(p<0.001)である。Index(うつ病_重篤度) はうつ病のレベルを評価する客観的方法として、診断を作成する方法として、あるいは処置の有効性を評価する方法として用いることができる。
上と同様に、c0およびc13は、ベンラファクシン応答指標(Index(ベンラファクシン_応答))が0から100まで(両端を含む)となるように定義されている。この実施例を得るために用いられるデータベースによれば、min(F13)=0.4、max(F13)=0.9であり、これによりc0=−80.0およびc13=200.0が得られる。閾値50を用いると、Index(ベンラファクシン_応答) は、感度75%、特異性77%およびAUC81%で応答者を予測した。
感度および特異性を改良する検定方法論
本発明の感度および特異性は比較検定方法論(differential testing methodologies)を利用して高めることができる。比較検定方法論では、2回またはそれ以上の連続評価を行い、各評価時の実際の値とともに検定の測定基準の値の変化を分析する。一般に評価は種々の条件下、例えば睡眠時または精神的作業のようなストレスの影響下で行われ、これらはベースライン評価と比較される。痴呆症、うつ病および他の神経学的障害の患者は、比較検定方法論にて、正常被検者と異なるEEG応答を示す。取得する指標の性能を高めるために、いくつかの比較検定方法論を用いることが出来る。検定の測定基準はEEGバイスペクトルアレイならびに他の非バイスペクトルパラメータから得られる指標であるのが好ましい。以下の記載ではこれをINDEXと示す。
Claims (27)
- 以下を含む、神経学的症状を評価する系:
生体電位シグナルを取得するための1つまたはそれ以上の電極ならびに
生体電位シグナルからK次スペクトル値(Kは1より大きい整数である)を得るため、およびK次スペクトル値から、神経学的症状を示す少なくとも1つの特徴を得るためのプロセッサ。 - 以下を含む、神経学的症状を評価する系:
生体電位シグナルを取得するための1つまたはそれ以上の電極、
(a)生体電位シグナルからK次スペクトル値(Kは1より大きい整数である)を得、
(b)K次スペクトル値から神経学的症状を示す特徴を得、ならびに
(c)その特徴を、神経学的症状を示す指標に結び付けるためのプロセッサ。 - 神経学的症状が治療的介入の有効性を予測するものであり、治療的介入の開始前に上記のプロセッサが上記の予測を算出する、請求項1に記載の系。
- 神経学的症状が薬理学的処置の成功を予測するものであり、臨床処置の効果が現れる前に上記のプロセッサが上記の予測を算出する、請求項1に記載の系。
- 以下を含む、神経学的症状を評価する系:
生体電位シグナルを取得するための1つまたはそれ以上の電極、
(a)検定精度を高め、
(b)生体電位シグナルからK次スペクトル値(Kは1より大きい整数である)を得、ならびに
(c)K次スペクトル値から神経学的症状を示す特徴を得るためのプロセッサ。 - 上記の検定精度を高めるために比較検定方法を利用する、請求項5に記載の系。
- 上記のプロセッサに連結された、制御された用量の薬理学的物質を投与するための注入装置をさらに含む、請求項5に記載の系。
- 以下の工程を含む、神経学的症状を評価する方法:
ヒト被検者から生体電位シグナルを取得する工程、および、
上記の生体電位シグナルからK次スペクトル値(Kは1より大きい整数である)を得る工程、および、
K次スペクトル値から、神経学的症状を示す少なくとも1つの特徴を得る工程。 - 以下の工程を含む、神経学的症状を評価する方法:
ヒト被検者から生体電位シグナルを取得する工程、
生体電位シグナルからK次スペクトル値(Kは1より大きい整数である)を得る工程、
K次スペクトル値から、神経学的症状を示す少なくとも1つの特徴を得る工程、および、
特徴を、神経学的症状を示す指標と結び付ける工程。 - 神経学的症状を示す少なくとも1つの特徴がアルツハイマー性痴呆症の診断である、請求項8に記載の方法。
- 神経学的症状を示す少なくとも1つの特徴が痴呆症の比較診断痴呆症である、請求項8に記載の方法。
- 神経学的症状を示す少なくとも1つの特徴が痴呆症の程度についての評価である、請求項8に記載の方法。
- 神経学的症状を示す少なくとも1つの特徴がうつ病のレベルについての評価である、請求項8に記載の方法。
- 神経学的症状を示す少なくとも1つの特徴が抗うつ剤処置の有効性についての評価である、請求項8に記載の方法。
- 神経学的症状を示す少なくとも1つの特徴が特定治療的介入の有効性についての予測である、請求項8に記載の方法。
- 治療的介入の開始前に予測が算出される、請求項15に記載の方法。
- 臨床処置の効果が現れる前に予測が算出される、請求項15に記載の方法。
- 神経学的症状の評価が、最も有効な抗うつ剤処置の予測である、請求項8に記載の方法。
- 以下の工程を含む、神経学的症状を評価する方法:
上記の生体電位シグナルを取得する工程、
検定精度を高める工程、
生体電位シグナルからK次スペクトル値(Kは1より大きい整数である)を得る工程、
K次スペクトル値から、神経学的症状を示す少なくとも1つの特徴を得る工程。 - 検定精度が高めるために比較検定方法論を用いる、請求項19に記載の方法。
- 制御された用量の薬理学的物質を投与する工程をさらに含む、請求項19に記載の方法。
