Abstract
Una composición de materia para su uso en el tratamiento de cáncer colorrectal (CCR) en un paciente que tiene expresión disminuida de la proteína hMSH2 (complejo de proteína de reconocimiento de reparación de desapareamiento (MMR)) y que responde más al tratamiento con 5-fluorouracilo, comprendiendo la composición: una cantidad efectiva de miR-21 antisentido y una formulación farmacéuticamente aceptable del mismo, en donde miR-21 tiene la SEQ ID. No.12.
Description
En las micromatrices de oligonucleótidos prefabricadas o micromatrices de un solo canal, las sondas están diseñadas para hacer coincidir las secuencias de miARN conocidos o predichos. Existen diseños comercialmente disponibles que cubren genomas completos (por ejemplo, de Affymetrix o Agilent). Estas micromatrices dan
5 estimaciones del valor absoluto de la expresión génica y, por lo tanto, la comparación de dos estados requiere el uso de dos micromatrices separadas.
Las matrices de oligonucleótidos largas por puntos están compuestas por sondas de captura de oligonucleótido de 50 a 70meros, y se producen mediante impresión o bien por chorro de tinta o bien robótica. Las matrices de oligonucleótidos cortas están compuestas por sondas de oligonucleótido de 2025meros, y se producen mediante síntesis fotolitográfica (Affymetrix) o mediante impresión robótica.
En algunos casos, es deseable el uso de RTPCR cuantitativa. RTPCR cuantitativa (qRTPCR) es una modificación de la reacción en cadena de la polimerasa usada para medir rápidamente la cantidad de un producto de la reacción
15 en cadena de la polimerasa. La qRTPCR se usa comúnmente para el fin de determinar sin una secuencia genética, tal como un miR, está presente en una muestra, y si está presente, el número de copias en la muestra. Cualquier método de PCR que puede determinar la expresión de una molécula de ácido nucleico, incluyendo un miARN, entra dentro del alcance de la presente descripción. Hay varias variaciones del método de qRTPCR conocido en la técnica, tres de los cuales se describen a continuación.
Los métodos para la reacción en cadena de la polimerasa cuantitativa incluyen, pero no se limitan a, a través de de electroforesis en gel de agarosa, el uso de SYBR verde (un colorante de ADN bicatenario), y el uso de una sonda indicadora fluorescente. Estos dos últimos pueden analizarse en tiempo real.
25 Con la electroforesis en gel de agarosa, la muestra desconocida y una muestra conocida se preparan con una concentración conocida de una sección de tamaño similar de ADN diana para su amplificación. Ambas reacciones se ejecutan durante el mismo periodo de tiempo en condiciones idénticas (preferiblemente usando los mismos cebadores, o al menos cebadores de temperaturas de hibridación similares). La electroforesis en gel de agarosa se usa para separar los productos de la reacción de su ADN original y ahorrar cebadores. Las cantidades relativas de las muestras conocidas y desconocidas se miden para determinar la cantidad de las desconocidas.
El uso de colorante SYBR verde es más preciso que el método de gel de agarosa, y puede dar resultados en tiempo real. Un colorante de unión a ADN se une a todo el ADN bicatenario recién sintetizado y se mide un aumento en la intensidad de fluorescencia, permitiendo así que se determinen las concentraciones iniciales. Sin embargo, SYBR
35 verde marcará todo el ADN bicatenario, incluyendo cualquier producto de PCR inesperado así como dímeros de cebador, conduciendo a potenciales complicaciones y artefactos. La reacción se prepara como habitualmente, con la adición de colorante de ADN bicatenario fluorescente. La reacción se ejecuta, y se monitorizan los niveles de fluorescencia (el colorante solo fluoresce cuando está unido al ADN bicatenario). Con referencia a una muestra patrón o una curva patrón, puede determinarse la concentración de ADN bicatenario en la PCR.
El método de sonda indicadora fluorescente usa una sonda a base de ácido nucleico específica de secuencia para cuantificar únicamente la secuencia sonda y no todo el ADN bicatenario. Se lleva a cabo comúnmente con sondas a base de ADN con un indicador fluorescente y un extintor colocado en posiciones adyacentes (denominadas sondas doblemente marcadas). La estrecha proximidad del indicador con el extintor impide su fluorescencia; solo con la
45 rotura de la sonda se detecta la fluorescencia. Este proceso depende la actividad enxonucleasa en 5’ a 3’ de la polimerasa implicada.
La reacción PCR cuantitativa en tiempo real se prepara con la adición de la sonda doblemente marcada. Con la desnaturalización del molde de ADN bicatenario, la sonda puede unirse a su secuencia complementaria en la región de interés del ADN de molde. Cuando la mezcla de reacción de PCR se calienta para activar la polimerasa, la polimerasa empieza a sintetizar la hebra complementaria con el ADN de molde monocatenario cebado. A medida que continúa la polimerización, esta alcanza la sonda unida a su secuencia complementaria, que se hidroliza entonces debido a la actividad exonucleasa 5’3’ de la polimerasa, separando de ese modo las moléculas indicadoras fluorescentes y las moléculas extintoras. Esto da como resultado un aumento en la fluorescencia, que se
55 detecta. Durante los ciclos térmicos de la reacción PCR en tiempo real, el aumento en la fluorescencia, a medida que se libera de la sonda doblemente marcada hidrolizada en cada ciclo de PCR se monitoriza, que permite la determinación precisa de las cantidades finales, y así iniciales, de ADN.
En algunos casos, es deseable el uso de hibridación in situ. La hibridación in situ (ISH) aplica y extrapola la tecnología de la hibridación de ácidos nucleicos a nivel de la célula individual, y, en combinación con el tipo de citoquímica, inmunocitoquímica y inmunohistoquímica, permite el mantenimiento de la morfología y la identificación de marcadores celulares que van a mantenerse e identificarse, y permite la localización de secuencias en células específicas dentro de poblaciones, tales como tejidos y muestras sanguíneas. ISH es un tipo de hibridación que usa un ácido nucleico complementario para localizar una o más secuencias de ácido nucleico específicas en una porción 65 o sección de tejido (in situ), o, si el tejido es suficientemente pequeño, en todo el tejido (ISH de embriones completos). ARN ISH puede usarse para evaluar los patrones de expresión en un tejido, tal como la expresión de

células se incubaron con PI 1 µg/ml durante 3 h a 25 °C antes del análisis de FACS mediante el citómetro de flujo Coulter Epics XL (Beckman Coulter, Fullerton, CA). Las células se consideraron apoptóticas cuando su contenido de ADN era <2N. Tinción con anexina V: Las células se separaron con tripsina, se lavaron con PBSFCS al 5 % y entonces se colocaron en tampón de unión que contenía NaCl 0,14 M, CaCl2 2,5 mM y ácido N2
5 hidroxietilpiperazinaN’2etanosulfónico 0,01 M (pH 7,4) a lo que se añadieron 7aminoactinomicina D (7AAD) y anexina VFITC (Pharmingen, San Diego, CA) antes del análisis de FACS. Las células se consideraron apoptóticas cuando eran anexina VFITC positivas y 7AAD negativas.
Los experimentos de sincronización se ejecutaron tal como sigue: células LovoMSH2 positivas, LovoMSH2 negativas, SW620 y Colo320DM se sincronizaron mediante parada en G0G1 a través de confluencia y tratamientos de suero bajo durante 48 horas 4, las células se disociaron entonces con tripsina, se contaron, se transfectaron con PremiRcontrol, PremiR21, ARNip frente a hMSH2, ARNipcontrol y vectores que codifican para el ADNc de MSH2 de longitud completa (con o sin región semilla de miR21) usando el kit Cell Nucleofector® (Lonza Walkersville, Inc ME) o Lipofectamine 2000TM (Invitrogen, Carlsbad, CA) siguiendo las instrucciones del fabricante y se volvieron a
15 sembrar en placa en medio que contenía FBS al 10 %. La adición de 5fluorouracilo (50ug/ml 5) se produjo a las 16 h tras la liberación, correspondiendo al momento justo antes de la entrada en la fase S pero después del punto de control del ciclo celular G1S mediado por p53.
Generación de clones estables que sobreexpresan niiR21
Se infectaron de forma estable células LovoMSH2 positivas y LovoMSH2 negativas con el plásmido de expresión pCDHCMVMCSEF1miARN que contenían el miR21 de longitud completa y el gen de GFP bajo el control de dos promotores diferentes (System Biosciences, Mountain View, CA). Se usó un vector vacío como control. Se empaquetaron los constructos control y de expresión de premiR21 con la mezcla de plásmido pPACKH1
25 Lentivector Packaging (System Biosciences) en la línea celular de empaquetamiento 293TN. Los virus se concentraron usando la solución de precipitación de virus PEGit™ y se analizaron los títulos usando el kit de titulación UltraRapid Lentiviral (System Biosciences). Las células infectadas se seleccionaron mediante análisis de FACS (FACS Calibur, Becton Dickinson Inmunocytometry Systems). La eficiencia de infección >90 % se verificó mediante microscopía fluorescente y se confirmó adicionalmente mediante PCR en tiempo real para la expresión de miR21.
Estudios de xenoinjerto.
Se realizaron estudios en animales de acuerdo con las directrices institucionales. Células Lovo MSH2positivas
35 infectadas con vectores lentivirales que codifican para o bien miR21, ARNip para hMSH2 o vector vacío como control y Lovo hMSH2negativas infectadas con vector vacío se inyectaron en el costado de ratones atímicos (5x106). Cuando los xenoinjertos (6 animales para cada grupo) alcanzaron un volumen palpable, se administró 5FU (50 mg/kg/día) mediante inyección intraperitoneal durante 5 días consecutivos a la semana durante 2 semanas. El volumen tumoral se midió al inicio del tratamiento y entonces una vez a la semana. El volumen tumoral estimado (V) se calculó mediante la siguiente fórmula: V = W2 x L x 0,5, en la que W representa el mayor diámetro tumoral en centímetros y L representa el siguiente diámetro tumoral más grande. El volumen tumoral relativo individual (RTV) se calculó tal como sigue RTV= Vx/V1 donde Vx es el volumen en milímetros cúbicos en un instante dado y V1 es el volumen al inicio del tratamiento.
45 Ejemplo 2.
MiR21 selecciona como diana directamente la proteína de expresión hMSH2 y hMSH6
El análisis en sílico mostró que miR21 podría seleccionar como diana hMSH2 y hMSH6 ARNm (TargetScan, Whitehead Institute, MIT, Fig. 1A). Los inventores identificaron sitios de unión putativos para miR21 en la 3’UTR tanto de hMSH2 (NCBINM_000249.2) como de hMSH6 (NCBI NM_000179.2). Los inventores examinaron el efecto de la expresión de miR21 sobre la expresión de ARNm de hMSH2 y hMSH6 endógenas en células de CCR Colo320DM y SW620. Ambas líneas celulares presentan una baja expresión basal de miR21. Los inventores transfectaron estas líneas celulares con precursor de miR21 (miR21) o un control de precursor de miR codificado 55 (Figura 6). La sobreexpresión de un ARN de interferencia pequeño específico (ARNip) frente a hMSH2 (antiMSH2)
o hMSH6 (antiMSH6) no afectó a los niveles de miR21 (Fig. 6A). Los niveles de ARNm de hMSH2 y hMSH6 no se vieron afectados por la sobreexpresión de miR21 (Fig. 1B). Por el contrario, ARNip antiMSH2 y antiMSH6 redujo específicamente la expresión de ARNm de hMSH2 y hMSH6 respectivamente (Fig. 1B). Los inventores observan una reducción consistente en la expresión de ARNm de hMSH6 con el ARNip antiMSH2. Esta reducción podría ser un resultado de degenerar la hibridación del ARNip antiMSH2 con el ARNm de hMSH6 o estabilidad reducida de ARNm de hMSH6 resultante de compañero de proteína heterodímero disminuido hMSH2. Los presentes resultados muestran que la sobreexpresión de miR21 no afecta a los niveles de ARNm de hMSH2 o hMSH6.
Los inventores examinaron los niveles de proteína de de hMSH2 y hMSH6 tras la transfección de miR21 en células

Ejemplo 7.
Método de tratamiento de pacientes con cáncer.
Este ejemplo describe un método de selección y tratamiento de pacientes que es probable que tengan una respuesta favorable a tratamientos con composiciones en el presente documento.
15 Un paciente al que se ha diagnosticado cáncer, normalmente se somete en primer lugar a resección de tejido en un intento de curación. Se obtiene muestras de tumor de la porción del tejido extraída del paciente. El ARN se aísla entonces de las muestras de tejido usando cualquier método apropiado para la extracción de ARN pequeños que sea bien conocido en la técnica, tal como mediante el uso de TRIZOLTM. El ARN purificado se somete entonces a RTPCR usando cebadores específicos para miR21 u otros miARN expresados diferencialmente divulgados, opcionalmente junto con análisis genético. Estos ensayos se ejecutan para determinar el nivel de expresión del ARN pertinente en el tumor. Si se determina el patrón de expresión de miR expresado diferencialmente, especialmente si se establece el estado mutante, el paciente es un candidato para el tratamiento con las composiciones en el presente documento.

Métodos de diagnóstico de los pacientes con cáncer
35 En un aspecto particular, se proporciona en el presente documento un método para diagnosticar si un sujeto tiene, o corre el riesgo de desarrollar, cáncer. El método incluye generalmente medir el patrón de expresión diferencial de miR de la expresión de miR21 y/o la proteína MMR en comparación con el control. Si se establece un patrón de expresión de miR/proteína MMR diferencial, los resultados son indicativos del sujeto o bien que tiene, o bien que corre el riesgo de desarrollar cáncer colorrectal. El nivel del al menos un producto génico se mide usando análisis de transferencia de tipo Northern. También, el nivel del al menos un producto génico en la muestra de ensayo es inferior del nivel del producto génico de miR correspondiente y/o la expresión de proteína de MMR en la muestra control, y/o el nivel del al menos un producto génico de miR y/o la expresión de proteína de MMR en la muestra de prueba es mayor que el nivel del producto génico de miR correspondiente y/o la expresión de proteína de MMR en la muestra control.

El nivel del al menos un producto génico de miR puede medirse mediante transcripción inversa de ARN a partir de una muestra de prueba obtenida del sujeto para proporcionar un conjunto de oligodesoxinucleótidos diana; hibridación de los oligodesoxinucleótidos diana con una micromatriz que comprende oligonucleótidos sonda específicos de miARN para proporcionar un perfil de hibridación para la muestra de prueba; y, comparar el perfil de hibridación de la muestra de prueba con un perfil de hibridación generado a partir de una muestra control.
