En ciertas realizaciones, un método no actúa reduciendo la letalidad causada por una enfermedad, tal como cáncer.
En otra realización, un compuesto modulador de sirtuina que aumenta el nivel y/o la actividad de una proteína sirtuina puede administrarse a un sujeto para aumentar generalmente la longevidad de sus células y proteger sus células contra estrés y/o contra apoptosis. Se cree que tratar un sujeto con un compuesto descrito en la presente es similar a someter al sujeto a hormesis, es decir, leve estrés que es beneficioso para organismos y puede extender su longevidad.
Los compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína sirtuina pueden administrarse a un sujeto para prevenir el envejecimiento y consecuencias o enfermedades relacionadas con el envejecimiento, tales como accidente cerebrovascular, enfermedad cardíaca, falla cardíaca, artritis, presión sanguínea alta y enfermedad de Alzheimer. Enfermedad cardiovascular
En otra divulgación, la invención proporciona un método para tratar y/o prevenir una enfermedad cardiovascular al administrar a un sujeto que lo necesita un compuesto modulador de sirtuina que aumenta el nivel y/o la actividad de una proteína sirtuina.
Las enfermedades cardiovasculares que pueden ser tratadas o prevenidas utilizando los compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína sirtuina incluyen cardiomiopatía o miocarditis; tal como cardiomiopatía idiopática, cardiomiopatía metabólica, cardiomiopatía alcohólica, cardiomiopatía inducida por fármacos, cardiomiopatía isquémica y cardiomiopatía hipertensiva. También son tratables o prevenibles utilizando los compuestos y métodos descritos en la presente los trastornos ateromatosos de los principales vasos sanguíneos (enfermedad macrovascular) tales como la aorta, las arterias coronarias, las arterias carótidas, las arterias cerebrovasculares, las arterias renales, las arterias ilíacas, las arterias femorales y las arterias poplíteas. Otras enfermedades vasculares que pueden ser tratadas o prevenidas incluyen aquellas relacionadas con la agregación de plaquetas, arteriolas retinales, las arteriolas glomerulares, el vasa nervorum, arteriolas cardíacas y lechos capilares asociados del ojo, del riñón, del corazón, y los sistemas nerviosos central y periférico. Los compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína también pueden utilizarse para aumentar los niveles de HDL en plasma de un individuo.
Otros trastornos que pueden ser tratados con compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína sirtuina incluyen restenosis, por ejemplo, luego de intervención coronaria y trastornos relacionados con un nivel anormal de colesterol de alta densidad y baja densidad.
En una realización, un compuesto modulador de sirtuina que aumenta el nivel y/o la actividad de una proteína sirtuina puede administrarse como parte de una combinación terapéutica con otro agente cardiovascular. En una realización, un compuesto modulador de sirtuina que aumenta el nivel y/o la actividad de una proteína sirtuina puede administrarse como parte de una combinación terapéutica con un agente antiarritmia. En una realización, un compuesto modulador de sirtuina que aumenta el nivel y/o la actividad de una proteína sirtuina puede administrarse como parte de una combinación terapéutica con otro agente cardiovascular.

Se divulga un método para tratar una célula del sistema nervioso central para prevenir daño en respuesta a una disminución en el flujo de sangre a la célula. Típicamente la gravedad del daño que puede prevenirse dependerá en gran parte del grado de reducción en el flujo de sangre a la célula y la duración de la reducción. En una realización, puede prevenirse la muerte celular apoptótica o necrótica. En otra realización, puede prevenirse el daño mediado isquémico, tal como edema citotóxico o anoxemia del tejido del sistema nervioso central. En cada realización, la célula del sistema nervioso central puede ser una célula de la médula o una célula del cerebro.
Otro aspecto abarca administrar un compuesto activador de sirtuina a un sujeto para tratar una afección isquémica del sistema nervioso central. Un número de afecciones isquémicas del sistema nervioso central pueden ser tratadas por los compuestos activadores de sirtuina descritos en la presente. En una realización, la condición isquémica es un accidente cerebrovascular que resulta en cualquier tipo de daño al sistema nervioso central isquémico, tal como muerte celular apoptótica o necrótica, edema citotóxico o anoxia de tejido del sistema nervioso central. El accidente cerebrovascular puede impactar cualquier área del cerebro o puede ser causado por cualquier etiología que se conoce que comúnmente resulta en la ocurrencia de un accidente cerebrovascular. En una alternativa de la presente realización, el accidente cerebrovascular es un accidente cerebrovascular troncoencefálico. En otra realización, el accidente cerebrovascular es un accidente cerebrovascular embólico. En otra alternativa, el accidente cerebrovascular puede ser un accidente cerebrovascular hemorrágico. En otra realización, el accidente cerebrovascular es un accidente cerebrovascular trombótico.
En otro aspecto, un compuesto activador de sirtuina puede administrarse para reducir el tamaño del infarto del núcleo isquémico que sigue a la afección isquémica del sistema nervioso central. Más aun, un compuesto activador de sirtuina también puede administrarse de forma beneficiosa para reducir el tamaño de la penumbra isquémica o zona de transición que sigue a la afección isquémica del sistema nervioso central.
En una realización, un régimen de fármaco de combinación puede incluir fármacos o compuestos para el tratamiento
o la prevención de trastornos neurodegenerativos o afecciones secundarias asociadas con estas condiciones. Por lo tanto, un régimen de fármaco de combinación puede incluir uno o más activadores de sirtuina y uno o más agentes antineurodegeneración.
Trastornos de coagulación de la sangre
En otros aspectos, los compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína sirtuina pueden utilizarse para tratar o prevenir trastornos de coagulación de la sangre (o trastornos hemostáticos). Tal como se utiliza en la presente de manera intercambiable, las expresiones "hemostasia" y "coagulación de la sangre" se refieren al control de sangrado, incluyendo las propiedades fisiológicas de vasoconstricción y coagulación. La coagulación de la sangre ayuda a mantener la integridad de la circulación mamífera después de una lesión, inflamación, enfermedad, defecto congénito, disfunción u otra interrupción. Además, la formación de coágulos de sangre no sólo limita el sangrado en caso de una lesión (hemostasia), sino que puede conducir a daño de órganos serios y muerte en el contexto de enfermedades ateroescleróticas mediante oclusión de una arteria o vena importante. La trombosis es de este modo la formación de un coágulo de sangre en el momento y lugar equivocados.
Por consiguiente, la presente invención proporciona tratamientos de anticoagulación y antitrombóticos con el objetivo de inhibir la formación de coágulos de sangre para prevenir o tratar trastornos de coagulación de la sangre, tales como infarto del miocardio, accidente cerebrovascular, pérdida de una extremidad por enfermedad arterial periférica
o embolismo pulmonar.
Tal como se utiliza de manera intercambiable en la presente, "modular o modulación de la hemostasia" y "regular o regulación de la hemostasia" incluye la inducción (por ejemplo, estimulación o aumento) de la hemostasia, así como la inhibición (por ejemplo, reducción o disminución) de la hemostasia.
Se divulga un método para reducir o inhibir la hemostasia en un sujeto al administrar un compuesto modulador de sirtuina que aumenta el nivel y/o la actividad de una proteína sirtuina. Las composiciones y métodos divulgados en la presente son útiles para el tratamiento o la prevención de trastornos trombóticos. Tal como se utiliza en la presente, la expresión "trastorno trombótico" incluye cualquier trastorno o afección caracterizado por coagulación o actividad hemostática excesiva o no deseada o un estado hipercoagulable. Los trastornos trombóticos incluyen enfermedades
o trastornos que implican la adhesión de plaquetas y formación de trombos y puede manifestarse como una propensión aumentada a formar trombosis, por ejemplo, un mayor número de trombosis, trombosis a una temprana edad, una tendencia familiar a la trombosis y trombosis en sitios inusuales.
En otra realización, un régimen de fármacos de combinación puede incluir fármacos o compuestos para el tratamiento o la prevención de trastornos de coagulación de la sangre o afecciones secundarias asociadas con estas condiciones. Control del peso
En otro aspecto, los compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína sirtuina pueden utilizarse para tratar o prevenir el aumento de peso u obesidad en un sujeto. Por ejemplo, pueden utilizarse compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína sirtuina, por ejemplo, para tratar o prevenir la obesidad hereditaria, obesidad dietética, obesidad relacionada con hormonas, obesidad relacionada con la administración de medicación, para reducir el peso de un sujeto o para reducir o prevenir el aumento de peso en un sujeto. Un sujeto que necesita dicho tratamiento puede ser un sujeto que es obeso, que probablemente se vuelva obeso, con sobrepeso o que probablemente vaya a tener sobrepeso. Los sujetos que probablemente se vuelvan obesos o que vayan a tener sobrepeso pueden identificarse, por ejemplo, en base a los antecedentes familiares, genérica, dieta, nivel de actividad, ingesta de medicamentos o varias combinaciones de los mismos.
En otras realizaciones, los compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína sirtuina pueden ser administrados a sujetos que sufren de una variedad de otras enfermedades y condiciones que pueden ser tratadas o prevenidas al promover la pérdida de peso en el sujeto. Dichas enfermedades incluyen, por ejemplo, presión sanguínea alta, hipertensión, colesterol en sangre alto, dislipidemia, diabetes tipo 2, resistencia a la insulina, intolerancia a la glucosa, hiperinsulinemia, enfermedad coronaria, angina de pecho, falla cardíaca congestiva, accidente cerebrovascular, cálculos biliares, colecistitis y colelitiasis, gota, osteoartritis, apnea del sueño obstructiva y problemas respiratorios, algunos tipos de cáncer (tales como endometrial, de mama, de próstata y de colon), complicaciones en el embarazo, mala salud reproductiva femenina (tal como irregularidades menstruales, infertilidad, ovulación irregular), problemas de control de la vejiga (tal como inconsistencia por estrés); nefrolitiasis de ácido úrico; trastornos psicológicos (tal como depresión, trastornos alimenticios, imagen corporal distorsionada y baja autoestima). Un sujeto que necesita dicho tratamiento puede ser un sujeto que tiene sobrepeso, es obeso o un sujeto que probablemente vaya a tener sobrepeso o se vuelva obeso. El método puede comprender administrar diariamente, o cada dos días, o una vez por semana, una dosis, por ejemplo, en la forma de una pastilla, a un sujeto. La dosis puede ser una "dosis que reduce el apetito".
En una realización ejemplar, los compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína sirtuina pueden administrarse como una terapia de combinación para tratar o prevenir el aumento de peso u obesidad. Por ejemplo, uno o más compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína sirtuina pueden administrarse en combinación con uno o más agentes antiobesidad.
En otra realización, los compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína sirtuina pueden administrarse para reducir el aumento de peso inducido por fármacos. Por ejemplo, un compuesto modulador de sirtuina que aumenta el nivel y/o la actividad de una proteína sirtuina puede administrarse como una terapia de combinación con medicaciones que pueden estimular el apetito o provocar aumento de peso, en particular, aumento de peso debido a factores que no sean retención de agua.
Trastornos metabólicos/diabetes
En otro aspecto, los compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína sirtuina pueden utilizarse para tratar o prevenir un trastorno metabólico, tal como resistencia a la insulina, un estado pre-diabético, diabetes tipo II y/o complicaciones de los mismos. La administración de los compuestos moduladores de sirtuina que aumenta el nivel y/o la actividad de una proteína sirtuina puede aumentar la sensibilidad a la insulina y/o disminuir los niveles de insulina en un sujeto. Un sujeto que necesita dicho tratamiento puede ser un sujeto que tiene resistencia a la insulina u otro síntoma precursor de diabetes tipo II, que tiene diabetes tipo II, o que es probable que desarrolle cualquiera de estas condiciones. Por ejemplo, el sujeto puede ser un sujeto que tiene resistencia a la insulina, por ejemplo, que tiene altos niveles circulantes de insulina y/o afecciones asociadas, tales como hiperlipidemia, dislipogénesis, hipercolesterolemia, tolerancia a la glucosa alterada, alto nivel de glucosa en sangre, otras manifestaciones del síndrome X, hipertensión, aterosclerosis y lipodistrofia.
En una realización ejemplar, los compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína sirtuina pueden administrarse como una terapia de combinación para tratar o prevenir un trastorno metabólico. Por ejemplo, uno o más compuestos moduladores de sirtuina que aumentan el nivel y/o la actividad de una proteína sirtuina pueden administrarse en combinación con uno o más agentes antidiabéticos.
