ESTO TIENE LA VENTAJA, DE QUE ESPACIOS INTERNOS DE DIFERENTE TAMAÑO SE PUEDEN LLENAR CON LA MISMA ESPIRAL.
Description
Prótesis intervertebral.
El invento se refiere a un implante
intervertebral, compuesto por un cuerpo elástico alargado. Una
prótesis de este género es conocida por el documento
EP-A-0 453 393. Sirve para sustituir
un núcleo de disco intervertebral (núcleo pulposo) dañado. En esto
el anillo externo (anillo fibroso) de un disco intervertebral
natural se conserva en gran parte. La prótesis conocida se compone
de un cuerpo hueco estanco a los líquidos, que está lleno de un
medio incompresible, capaz de fluir. Esta prótesis requiere para su
implantación un instrumento de introducción especialmente
configurado, con el cual en el curso de la operación el cuerpo hueco
puede ser arrollado en forma de espiral y a continuación llenado.
Esto plantea grandes exigencias en la habilidad del cirujano por
una parte, así como en la fabricación del instrumento de
introducción en lo que se refiere al mecanismo de arrollado, así
como en cuanto al dispositivo de llenado. Además los criterios de
estanqueidad del cuerpo hueco, habida cuenta de las grandes cargas
en la zona de la columna vertebral, representan a la larga un
riesgo. Después de que el cuerpo ha sido arrollado y llenado en la
zona intervertebral el extremo libre se apoya en el anillo fibroso,
en caso de que el instrumento auxiliar, que para el llenado y el
cierre hermético ataca en el centro de la espiral, pueda ser
retirado sin producir espacios huecos. Sólo cuando no puede
producirse ningún desplazamiento de partes de la espiral a otros
espacios huecos pueden ser compensadas las fuerzas dirigidas
radialmente que se presentan en caso de carga y la espiral obtiene
la función de apoyo

En el documento
FR-A-2 712 486 se muestran implantes
de forma espiral para la zona de núcleo de un disco intervertebral,
que se componen de aleaciones con memoria de forma (metal con
memoria), las cuales debido a un fenómeno físico se deforman desde
un estado libre de tensiones a otra forma con estado libre de
tensiones. Como fenómeno físico se cita la aparición de una tensión
por tracción y un cambio de la temperatura, que modifican las
condiciones físicas originales. Otros ejemplos muestran cintas de
transporte ocupadas por ganchitos, que son arrastradas a través de
un tubo, para arrollar una cinta arrastrada ocupada por ganchitos
formando una espiral.
El documento US 3.867.728 describe, entre otros,
un implante intervertebral en forma de un disco ovalado, lográndose
esta forma al variar el espesor de un cuerpo alargado enrollado.
El problema del invento es proporcionar un
implante que permita una manipulación sencilla.
Este problema es solucionado mediante un
implante según la reivindicación 1. La sección transversal del
cuerpo es preferentemente rectangular, pudiendo también estar
redondeados los bordes. También es posible emplear cuerpos de
sección transversal redonda, en tanto que en caso de carga se
conserve una posición de la espiral en esencia con acoplamiento de
forma, asegurada contra el ladeado. La sección transversal del
cuerpo elástico no es constante a través de su longitud total, más
bien el espesor disminuye hacia el exterior. Una espiral semejante
tiene la ventaja de que se coloca por sí misma, en caso de empuje no
se pandea y puede ser separada en una longitud arrollada
conveniente.
El implante como tal puede tomar la forma de un
disco cilíndrico, es decir, puede estar configurado plano, o bien
presentar abombamientos hacia fuera y/o hacia dentro que sigan los
contornos de las superficies de las vértebras. De esta manera se
consigue que las fuerzas producidas en caso de carga se distribuyan
más uniformemente sobre las superficies de las vértebras. El
diámetro de la espiral está dimensionado preferentemente de manera
que se llene el espacio dentro del anillo fibroso.
Para introducir el disco intervertebral se ha
mostrado ventajoso colocar medios de refuerzo en forma de una banda
de plástico o metálica en el extremo libre de la espiral, mediante
la cual un dispositivo de separación puede ser guiado hasta un
punto de corte previsto. La banda en estado libre de fuerzas
preferentemente no presenta ninguna curvatura. Tras la implantación
realizada la banda junto con la parte de espiral no necesaria es
separada de la espiral colocada por medio del dispositivo de
separación.
Para poder controlar la posición de la espiral
en el curso y después de la operación se recomienda la previsión de
marcaciones positivas a los rayos X en forma de esferas o de un hilo
sobre o a lo largo en el interior del cuerpo elástico.
El material del que está fabricado el cuerpo
elástico corresponde en esencia a las propiedades elásticas de un
disco intervertebral, de manera que las fuerzas que se presentan
puedan ser transmitidas lo más uniformemente posible sobre la
superficie de las vértebras. Ha resultado ventajoso el empleo de
poliuretano o mezclas de poliuretano-silicona, que
presentan una dureza Shore en el campo de aproximadamente 80A.
También pueden emplearse espumas de los más diversos plásticos, en
tanto que satisfagan las exigencias elásticas.
En una realización particularmente preferida la
espiral presenta una pieza media central, alrededor de la cual se
colocan apretadas las demás espiras. Se forman sólo pequeños
espacios intermedios, que dado el caso en estado implantado se
llenan de líquido del disco intervertebral y no permiten un ladeado
del cuerpo elástico bajo fuerzas radiales. Mediante una relación
apropiada de altura a ancho se impide en general un ladeado del
cuerpo elástico. La pieza media puede presentar en una forma
cilíndrica circular, pero también se han mostrado útiles formas
como la esfera o de elipsoide. A ella se une de manera ventajosa una
parte de transición corta, más delgada, mediante la cual está
realizada la unión hacia otras espirales. Esto tiene la ventaja de
que la espiral queda en la más compacta empaquetadura con
acoplamiento de superficies y se obtiene la mayor distribución
superficial posible con reducción de picos de fuerza. En una
configuración más sencilla del implante también puede suprimirse la
pieza media central, puesto que su aportación de superficie es
pequeña con relación a la superficie total.
La altura de la espiral corresponde
esencialmente a la distancia intervertebral fisiológica. A partir de
las propiedades elásticas del material empleado se deduce el
espesor preferido en el que tiene que estar configurado el cuerpo
elástico. En la periferia de la espiral, después de unas tres a
cinco espiras, según material y ancho de la espira, el espesor del
cuerpo puede disminuir, puesto que las fuerzas adyacentes deben ser
absorbidas principalmente en el centro y puesto que por otra parte
el cuerpo debe poder ser separado en el punto adecuado. En otra
configuración del invento es posible adaptar la altura del implante
a las distancias vertebrales individuales.
La separación debe realizarse preferentemente
dentro del espacio hueco, para lo cual deben aplicarse las fuerzas
adecuadas por medio de un instrumento todavía a describir en
detalle.
Con esto se describe también un procedimiento
para fabricar un implante intervertebral de forma espiral.
Como procedimiento de fabricación se emplea un
proceso de moldeo por inyección, en el cual el molde en forma de
caracol se llena en el interior de la voluta. El implante así
obtenido presenta una forma espiral con espiras con fuerte
acoplamiento de forma.
Además se describe un dispositivo para
introducir el implante, que en esencia presenta un tubo de sección
transversal rectangular, en el que puede hacerse entrar el cuerpo
elástico y precisamente hasta tal punto que la pieza media todavía
sobresalga fuera del extremo más distante del tubo. La sección
transversal del tubo corresponde en esencia a la sección
transversal del cuerpo elástico. El avance del cuerpo puede
efectuarse a mano. Preferentemente sin embargo con el
correspondiente aumento de la sección transversal del tubo están
previstos medios de avance en forma de un empujador, los cuales
pueden ser activados por ejemplo mediante un mecanismo de palanca.
El empujador presenta en su extremo fijo más distante, en unión
efectiva con el cuerpo elástico o el refuerzo, medios que facilitan
un avance del cuerpo. Los medios pueden consistir en hacer rugosa
la superficie, o si no también en resaltes en forma de dentado con
orientación preferente en la dirección más distante. De esta manera
se consigue que con el avance del empujador los resaltes engranen en
el cuerpo elástico o en el refuerzo y le hagan desplazarse con
ellos, mientras que con el retroceso del empujador resbalen hacia
atrás sobre la superficie. De esta manera es posible un avance
controlado del cuerpo elástico. Es de ventaja adicional configurar
el tubo en su extremo más distante con un estrechamiento de sección
transversal por un lado, el cual en su extremo delgado tan sólo
pueda alojar al cuerpo elástico. Esto hace que el empujador al
empujar hacia delante sea a la vez presionado hacia abajo y así
aumenta el cierre de fuerza entre empujador y cuerpo.
El tubo del dispositivo de introducción también
puede de manera ventajosa estar curvado, puesto que la introducción
del implante en el espacio intervertebral se efectúa de preferencia
lateralmente desde la espalda. La curvatura puede estar realizada
tanto hacia arriba o abajo como opcionalmente hacia la izquierda o
la derecha, según desde qué lado deba tener lugar el acceso. Además
el dispositivo de introducción también puede acabar por delante en
forma de cuña, para con la parte en forma de cuña distanciar de
nuevo suficientemente los cuerpos vertebrales adyacentes según la
disminución de distancia que se haya producido.
Se describe además un dispositivo para separar
el extremo no necesario de la espiral. Después de que la prótesis
intervertebral ha sido implantada, el dispositivo de introducción es
retirado a lo largo del refuerzo, que al menos parcialmente
permanece fuera de la zona del disco intervertebral, y se hace
entrar el instrumento de separación. Este instrumento presenta
preferentemente un mecanismo de accionamiento similar a unas pinzas,
que comprende dos partes de agarre que están en unión entre sí
mediante una articulación. Una mordaza de la parte de corte está
configurada hueca, de tal manera que puede alojar al refuerzo. La
otra mordaza presenta un filo cortante, que en esencia es
transversal al eje longitudinal de las pinzas. La longitud de las
mordazas así como la disposición del filo cortante en la dirección
más distante se eligen preferentemente de manera que por una parte
sea posible un corte en la zona intervertebral, y por otra parte
todavía puedan aplicarse con seguridad las fuerzas de separación
necesarias. Además el cuerpo elástico está rodeado de forma que al
separar el extremo no necesario de la espiral no sean cortadas junto
con él partes de tejido ajenas. Naturalmente según el invento
también pueden emplearse pinzas cuyas partes de agarre estén en un
ángulo con respecto al eje longitudinal.
En una realización particularmente preferida el
dispositivo de introducción también puede contener un instrumento
de corte. Esto puede realizarse por ejemplo estando otro empujador
provisto en su extremo más distante de una cuchilla de corte, que
se desarrolla transversal con respecto al eje longitudinal del
dispositivo. El efecto de corte se produce mediante el
estrechamiento de la sección transversal más distante del tubo, de
modo similar como en el avance.
El implante tiene la ventaja de ser de
construcción lo más sencilla posible y de asegurar uniformemente un
efecto de apoyo sobre una gran parte de su superficie. El implante
tiene además la ventaja de no tener que ser fijado en forma de
espiral por medios adicionales, sino que puede ser introducido por
medio de instrumentos comparativamente sencillos y seguros de
manejar.
A continuación el invento se explica en detalle
con ayuda de un ejemplo de realización particularmente preferido en
relación con los dibujos.
La Figura 1 muestra esquemáticamente y muy
aumentado el cuerpo elástico en forma de una espiral;
la Figura 2 muestra una sección transversal de
la espiral según la Figura 1;
la Figura 3 muestra en representación
esquemática un dispositivo de introducción;
la Figura 4 muestra en representación
esquemática un dispositivo de separación y
la Figura 5 muestra esquemáticamente una vista
en planta del dispositivo de separación según la Figura 4.
Las Figuras tratan de un implante, en particular
una prótesis intervertebral, que se compone de un cuerpo elástico
alargado, que es elástico en su forma y en estado libre de fuerzas
adopta la forma de una espiral S. La espiral mediante tracción
hacia atrás puede hacerse entrar en un instrumento de introducción,
que en la zona de introducción es sólo insignificantemente mayor
que la sección transversal del cuerpo elástico alargado, para a
través de una pequeña abertura en el anillo fibroso alcanzar el
espacio interno de un disco intervertebral e insertar la espiral,
que se arrolla por sí misma, y separarla cuando el espacio interno
está lleno. Esto tiene la ventaja de que con la misma espiral
pueden llenarse espacios internos de diferente tamaño.
La Figura 1 muestra el implante en forma de
espiral con una pieza media central 1, que en esencia presenta la
forma de un cilindro. A ella se une lateralmente una zona más
delgada 2, que posibilita, en esencia sin espacios intermedios, un
fuerte acodado y por lo tanto una transición con acoplamiento de
forma hacia la vuelta espiral 3 contigua a ella.
Para el caso en el que se suprima la pieza media
se recomienda configurar el extremo interno de la vuelta espiral de
manera que con la carga no puedan producirse picos de esfuerzo. Esto
puede conseguirse por ejemplo mediante un extremo redondeado.
Las vueltas espirales contiguas a la zona de
transición son con acoplamiento de forma y presentan un ancho
constante en el intervalo de 2 a 5 mm, de manera que un ladeado bajo
la carga a esperar debe excluirse en el más amplio grado. El
implante comprende preferentemente 2-4 espiras
aseguradas contra el ladeado. A ellas se une zona de transición 4,
en la que el ancho del cuerpo elástico se reduce hasta el punto que
aproximadamente en la zona 5 de la Figura 1 se hace posible una
fácil separación. Mediante el número de espiras en el curso de la
operación se adapta la espiral al tamaño deseado. El efecto de apoyo
con respecto al basculamiento está reducido como tal en las
estrechas espiras exteriores, pero junto con las espiras interiores
de forma estable es suficientemente grande.
El espesor del cuerpo elástico que forma la
espiral también puede disminuir sin embargo constantemente desde el
interior, de manera que no sea necesaria ninguna zona de transición
especialmente configurada. En la Figura 2 está representada la
espiral en sección transversal. Tiene un diámetro en el intervalo de
20 a 30 mm, preferentemente en el intervalo de 23 a 25,5 mm. La
altura es preferentemente de 5-10 mm. La altura
también puede aumentar de forma continua en dirección central y en
el centro tomar como máximo 10 mm. La altura también puede estar
adaptada al desarrollo individual de las superficies de las
vértebras, lo que puede ser de interés particularmente en caso de
deformaciones del cuerpo vertebral.
En el extremo libre 6 de la espiral está fijado
un refuerzo 7 en forma de banda, que sobresale sobre el extremo
libre y como prolongación es útil para el manejo de la espiral. La
transición debe ser lo más lisa posible. La banda debe estar en
condiciones de transmitir las fuerzas de empuje y por razones de
biocompatibilidad y de esterilización se fabrica preferentemente de
titanio. Su longitud es prácticamente de unos 20 cm y posibilita un
guiado del instrumento de separación hasta el punto de separación en
la espiral.
El cuerpo elástico se compone de plástico, por
ejemplo poliuretano, viniendo a quedar situadas las vueltas
espirales, en estado libre de fuerzas, con acoplamiento de forma en
grado máximo. Las propiedades elásticas del material deben ser
tales que incluso después de un desenrollado sea adoptada de nuevo
la forma espiral. Las propiedades de deformación del material
tienen que ser similares en el más amplio grado a las del disco
intervertebral y estar situadas en el intervalo de dureza Shore de
70A a 90A, preferentemente alrededor de 80A.
El cuerpo elástico para su fabricación puede ser
cortado a partir del material, lo que particularmente es posible
mediante láseres controlados por ordenador. Son preferidos los
procesos de corte que no carguen térmicamente el material, puesto
que así se evitan subproductos tóxicos y tampoco se ejerce ninguna
influencia sobre las propiedades elásticas del material. No debe
omitirse en especial que el material como tal tiene que ser
esterilizable.
Para poder controlar la posición del implante en
el curso y después de la operación se recomienda la previsión de
marcaciones positivas a los rayos X en forma de puntos 8 o de un
hilo no representado en las ilustraciones sobre o a lo largo y en
el interior del cuerpo elástico. Como material marcador puede
emplearse bario, tantalio o similares.
El dispositivo para introducir el cuerpo
elástico está representado en detalle en la Figura 3. Comprende un
tubo rectangular 10 para el alojamiento de la espiral desenrollada
al menos hasta tal punto que su parte S todavía saliente pueda ser
introducida sin problemas a través de la abertura practicada en la
operación en la zona intervertebral. El tubo 10 presenta
preferentemente en el extremo más distante un estrechamiento de
sección transversal 11 por un lado, que está configurado cónico o en
forma de pico. El tubo también puede presentar una curvatura, que
facilita el acceso a la zona intervertebral lateralmente desde la
espalda y está dispuesta según que el usuario utilice la mano
derecha o la izquierda. La curvatura sin embargo también puede
estar dirigida en sentido caudal o craneal, según la exigencia.
El avance del cuerpo también puede realizarse a
mano. En ello son útiles medios de agarre para apoyar el tubo.
Éstos pueden estar configurados coaxiales con el tubo en forma de un
engrosamiento o desarrollarse en sentido radial al eje del tubo.
Preferentemente sin embargo están previstos medios de avance en
forma de un empujador elástico 12, que pueden ser accionados por
ejemplo mediante un mecanismo de palanca 13. El empujador presenta
en su extremo fijo más distante, en unión efectiva con el cuerpo
elástico o con el refuerzo, medios 14 que facilitan un avance del
cuerpo. Los medios pueden consistir en hacer rugosa la superficie, o
si no también en resaltes en forma de dentado con orientación
preferente en la dirección más distante. De esta manera se consigue
que con el avance del empujador los resaltes engranen en el cuerpo
elástico o en el refuerzo y le hagan desplazarse con ellos,
mientras que con el retroceso del empujador resbalen hacia atrás
sobre la superficie. Con ello es posible un avance controlado del
cuerpo elástico. El estrechamiento de sección transversal 11 por un
lado del tubo rectangular 10 hace que el empujador al empujar hacia
delante sea a la vez presionado hacia abajo y así aumenta el cierre
de fuerza entre empujador y cuerpo. Debido a que el empujador ataca
muy distante por delante en la espiral desenrollada, ésta
prácticamente es arrastrada a través del tubo, de manera que no se
autobloquea bajo su tensión propia, como fácilmente podría ser el
caso con empuje desde atrás.
Las Figuras 4 y 5 muestran un dispositivo para
separar el extremo sobrante de la espiral. Después de que la
prótesis intervertebral ha sido implantada, el dispositivo de
introducción es retirado a lo largo del refuerzo 7, que permanece
fuera de la zona del disco intervertebral, y se hace entrar el
instrumento de separación 20. Este instrumento presenta
preferentemente un mecanismo de accionamiento similar a unas pinzas
que comprende dos partes de agarre 21, 22 que están en unión entre
sí mediante una articulación 23. Una mordaza 24 está provista en
dirección longitudinal de una escotadura 25, en la que puede
alojarse el refuerzo 7 y/o el cuerpo elástico 5, 6. La otra mordaza
26 presenta un filo cortante 27, que en esencia es móvil
transversalmente con respecto a la escotadura 25. La longitud de
las mordazas así como la disposición del filo cortante en la
dirección más distante se eligen preferentemente de manera que por
una parte sea posible un corte dentro de la zona intervertebral, y
por otra parte todavía puedan aplicarse con seguridad las fuerzas de
separación necesarias. La cuchilla de corte en sí puede estar
configurada como cuchilla recambiable. Naturalmente según el invento
también pueden emplearse pinzas cuyas partes de agarre estén en un
ángulo con respecto al eje longitudinal.
Después de que el cuerpo elástico ha sido
acortado a la longitud deseada, es retirado el dispositivo de
separación junto con el refuerzo 7. El implante, debido a sus
propiedades de estabilidad de forma, adopta la forma espiral. El
implante conserva también bajo carga su forma espiral, sin que sea
necesaria una fijación especial del extremo libre.
El antes descrito procedimiento para introducir
un implante de forma espiral en un espacio hueco se emplea
ventajosamente también para fines de enseñanza, prácticas y
demostración.
Claims (10)
1. Implante intervertebral, en particular que
adopta la forma de un doisco de cilindro, compuesto por un cuerpo
alargado, de una sola pieza y elástico, de plástico con una dureza
Shore en el intervalo de 70A a 90A, por ejemplo poliuretano, con
propiedades de deformación que son similares en gran parte a las de
un disco intervertebral, siendo el cuerpo elástico en su forma y
adoptando en estado libre de fuerzas la forma de una espiral (S),
siendo el ancho (B) del cuerpo en el interior de la espiral mayor
que en la parte exterior, en donde el ancho disminuye
constantemente desde el interior o es constante el ancho en el
interior de la espiral a la que se une una zona de transición que
reduce el ancho formando vueltas espirales estrechas en el exterior,
y obteniéndose el implante al verter la espiral en el procedimiento
de moldeo por inyección en el interior de la voluta en un molde en
forma de caracol, para que después de un desenrollado de la espiral
las vueltas espirales, en estado libre de fuerzas, vengan a quedar
situadas con acoplamiento de forma en grado máximo alrededor de una
pieza media central.
2. Implante según la reivindicación 1,
caracterizado porque el cuerpo en esencia presenta una
sección transversal rectangular (H, B), cuya altura (H) corresponde
a la zona intervertebral a sustituir.
3. Implante según la reivindicación 1,
caracterizado porque la espiral presenta una superficie de
base plana y/o una superficie de tapa plana.
4. Implante según la reivindicación 1,
caracterizado porque la espiral presenta al menos una
superficie de base y/o de tapa abombada hacia fuera.
5. Implante según la reivindicación 2,
caracterizado porque la relación de altura a ancho de la
sección transversal del cuerpo elástico en su forma está
configurada al menos en el centro de la espiral de manera que bajo
carga el cuerpo no se ladea ni bascula.
6. Implante según la reivindicación 1,
caracterizado porque la espiral presenta en su centro una
pieza media (1) aproximadamente cilíndrica, a la cual, dado el caso
a través de un elemento de transición (2) de menor ancho, se une
lateralmente el cuerpo elástico en su forma, para en estado libre de
fuerzas generar una espiral con acoplamiento de forma en esencia
sin espacios huecos.
7. Implante según la reivindicación 1,
caracterizado porque el cuerpo se compone de material
esterilizable, siendo el material preferentemente poliuretano o una
mezcla de poliuretano-silicona y presentando una
dureza Shore en el campo de aproximadamente 80A.
8. Implante según la reivindicación 1,
caracterizado porque el cuerpo en forma de espiral adopta un
diámetro exterior (D) que llena en esencia totalmente el espacio
dentro del anillo fibroso, estando el diámetro exterior situado en
el intervalo entre 20 y 30 mm, preferentemente en el intervalo entre
23 y 25,5 mm, así como presentando preferentemente una altura de 5
a 10 mm y presentando la espiral al menos 3, preferentemente 5
espiras.
9. Implante según la reivindicación 1,
caracterizado porque en el extremo libre (6) de la espiral
están previstos adicionalmente medios de refuerzo (7),
preferentemente en forma de una banda, mediante la cual pueden ser
transmitidas fuerzas de empuje, estando fabricada la banda de
plástico o de metal, preferentemente de titanio.
10. Implante según la reivindicación 1,
caracterizado porque están previstos marcadores (8) de rayos
X, preferentemente en forma de esferas o hilos.
