Description

The invention relates to an operating table according to the preamble of claim 1.

Such an operating table is known from the company brochure of the Stierlen-Maquet AG company "MAQUET 1415.", October 1975. Here, a single vertical receiving opening is provided at the free end of the support arm for receiving the counter-pulling rod, which lies in the longitudinal center plane of the support plate. The counter-pulling rod is only used for a treatment of the femoral neck and other operations in which the patient lies flat on the bed plate. In the case of thigh intramedullary nailing and similar interventions, in which the patient lies in a lateral position, in the known operating table, a vertical groin support is attached to the operating table frame at the transition point from the end section to the section adjoining it towards the head end, on which the front, the patient's upper iliac crest is supported. In this case, a separate spar, which is articulated on the lower part and carries an extension device at its free end, is used together with the last support. The conversion of the operating table from use with a femoral neck position, d. H. in the patient lying on the back, for use with thigh positioning, i.e. for patients lying on the side is therefore relatively complex.

In osteosynthetic operations, it is necessary to be able to record the operating field by means of an X-ray device, usually an X-ray image intensifier, and to be able to continuously monitor it appropriately. Therefore, the support plate is made radiopaque, and the frame of the operating table is designed so that it does not hinder the use of suitable X-ray machines. This is also the reason why the support arm forming part of the frame has only a small width. In spite of this, it can be difficult with femoral neck supports to completely cover the surgical field, especially in the vertical direction, since this is only offset to the side by a small amount in relation to the support arm. On the other hand, in the case of thigh positioning, the additional spar, which is attached together with the groin support, lies close to the operating field and therefore complicates the use of the X-ray apparatus.

The invention has for its object to develop an operating table of the type mentioned in a structurally uncomplicated manner such that a conversion between the femoral neck position and thigh position is possible in a simple manner and in both cases the operating field for image intensifiers or other X-ray devices is easily accessible.

The object is achieved according to the invention in an operating table of the type mentioned by the features specified in claim 1.

In the operating table according to the invention, the counter-tension rod inserted into one of the vertical receiving openings is laterally offset outwards with respect to the longitudinal center plane of the mounting plate, so that when the patient rests on the neck of the patient with the back lying on the mounting plate, the operating field, i. H. the area of the femoral neck has moved relatively far outwards, which facilitates its supply and fluoroscopy. In addition, the counter-tension rod can in many cases be used as a groin support with the patient lying on the side of the arm, or instead of the counter-tension rod, a similarly designed, longer compared to the counter-tension rod support can be inserted into one of the vertical receiving openings without the lateral attachment of a separate last support and one special spars would be required, since in these cases one of the existing spars can be used to attach the extension device. In this case, retrofitting is avoided, or the retrofitting is limited to replacing the counter-tensioning rod with a longer groin support inserted in the same place.

The operating table shown in FIGS. 1 and 2 comprises a frame 10, which is either adjustable in height on a fixed column indicated in FIG. 2 and can be tilted on all sides, or which can at least be taken over by a suitable chassis for transport purposes, which can then be carried out with three above directed pin engages provided in the underside of the frame 10, indicated in Fig.1 openings 14,15,16. A mounting plate 18 for the upper body of a patient is arranged on the frame 10 and can be adjusted against one another by two swivel joints 20 Base plates 22, 24 and a cushion pad 26 covering them and to whose left end in FIGS. 1 and 2 there is a mounting plate 28 for the patient's head. The mounting plate 18, which can be X-rayed, comprises an end section 30, which is used to support the pelvis of the patient in femoral neck nailing and similar operations in which the patient is supine. In this case, a perpendicular counter-pull rod 32 is then provided, which has a rigid axis 34 and, in its sections lying at the height of the bearing plate 18 and above the bearing plate 18, a padding 36 concentrically surrounding the axis 34. Below the end section 30, the frame 10 is designed as a support arm 42 which extends along the longitudinal central plane 38 of the mounting plate 18 up to the end face 40 of the end section 30, at the free end of which the counter-pulling rod 32 is inserted and removed in a manner to be described. Furthermore, at the free end of the support arm 42, two horizontal bars 44, 46 are each pivotably articulated about a vertical axis, each of which carries at their free ends remote from the frame 10 an extension device 48 designed as a tension spindle unit. With the latter, one leg of the patient supported on the counter-pull rod 32 in the pubic region can be extended in the case of a patient lying on his back on the support plate 18. Both legs can be extended at the same time, for example with Küntscher femoral nailing, while with one-sided thigh nailing it can also be sufficient to extend the affected leg while the healthy leg is being stored using a Goepel leg holder.

Damit auch bereits bei der Vorbereitung der Operation eine Durchleuchtung des Operationsfeldes möglich ist, sind sowohl die Stützplatten 50, 52 wie auch die Beinauflageplatten 56, 58 in ähnlicher Weise wie die Lagerungsplatte 18, jedoch mit Ausnahme ihrer metallischen Tragestangen, beispielsweise der Tragestangen 60 (Fig. 2), durchleuchtbar ausgebildet.1 and 2, two triangular, padded support plates 50, 52 are provided as additional devices, which complement the essentially trapezoidal end section 30 in the plan view of FIG. 1 to a rectangular basic shape, the width of which is at least approximately as large as that Width of the section 54 of the mounting plate 18 adjoining the end section 30. Two additional leg support plates 56, 58 are shown as additional plates, which in their illustrated use position extend parallel to one another from the end section 30 of the mounting plate 18 and each have a support rod 60 (FIG. 2). have, which extends near the longitudinal center plane 38 (FIG. 1) of the mounting plate 18 parallel to this. The support plates 50, 52 as well as the leg support plates 56, 58 have, as can be seen from FIG. 2, in their use position an upper surface which is at least approximately flush with the upper side of the end section 30 of the bearing plate 18 and in the exemplary embodiment. These support plates 50, 52 and leg support plates 56, 58 enable the patient, including his hip and legs, to be gently supported before the operation; After the extension of at least one leg, the leg support plates 56, 58 and at least one support plate 50 or 52 are removed in order to be able to carry out the operation without hindrance. For this purpose, the support plates 50, 52 and the leg support plates 56, 58 are held in a plug-in connection in an easily removable manner, to be described in more detail, at the free end of the support arm 42. So that the surgical field can be illuminated even during preparation for the operation, both the support plates 50, 52 and the leg support plates 56, 58 are similar to the support plate 18, but with the exception of their metallic support rods, for example the support rods 60 (FIG . 2), designed to be X-ray.

When used, the counter-tension rod 32 is in a position offset to the outside relative to the longitudinal center plane 38 (FIG. 1) of the mounting plate 18. This is achieved in that two vertical receiving openings 64 are provided at the free end of the support arm 42 for pluggable reception of the axis 34 of the counter rod 32, which are located on both sides of the longitudinal central plane 38 of the support plate 18 and which are less than half the largest width relative to this longitudinal central axis 38 the storage plate 18 are offset to the outside; the counter-tension rod 32 is inserted with its axis 34 into one of these vertical receiving openings 64 and is offset outwards by a corresponding distance with respect to the longitudinal center plane 38. The distance of this outward displacement is expediently chosen such that the distance between the center of a vertical receiving opening 64 and the longitudinal center plane 38 is between 5 cm and 15 cm. In the exemplary embodiment, this center distance is 11 cm, while the greatest width of the mounting plate 18 is approximately 50 cm.

In Fig. 1, the counter-pull rod 32 is upwards relative to the longitudinal median plane 38, i. H. offset to the left relative to the longitudinal axis of a patient lying on his back on the support plate 18. The left thigh neck of the patient is thereby moved outward relative to a positioning of the patient oriented along the longitudinal center plane 38 of the support plate 18 and is therefore particularly easily accessible to the operating doctor. The support plate 52 is removed here and therefore does not hinder the operation or its metallic support rod, which will be described with reference to FIG. 6, the fluoroscopy of the operating area.

The same applies correspondingly to an operation in the area of the right thigh neck of the patient, in which the counter-pull rod 32 is in the lower one in FIG vertical receiving opening 64 is inserted and the support plate 50 is removed.

The lateral displacement of the counter-pull rod 32 with respect to the longitudinal center plane 38 has the consequence that the counter-pull rod 32 can serve as a groin support for the patient during operations with a patient lying on the side, for example in the case of thigh medullary nailing. At best, the counter-tension rod 32 shown in FIG. 1 is to be replaced by a longer, otherwise similarly shaped, strip support 66 if necessary. It is therefore not necessary to mount a separate strip support with a separate spar for an extension device in the usual way, to the side of the bearing plate 18, for example at the transition between the end section 30 and the subsequent section 54.

On the other hand, it is also possible, if necessary, to provide a height-adjustable, vertical groin support for carrying out thigh osteosynthesis in the case of a patient lying on the side and with the counter-pull rod 32 removed Patient. So that the downwardly projecting housing in such a height-adjustable ledge support, in which the spindle for the height adjustment is accommodated, does not limit the pivoting range of the spar 44 or 46 used in such a way that this impedes the use of an image converter device or other X-ray apparatus, the extension device 48 is provided Spars 44, 46 are expedient, as in the exemplary embodiment, of two sections 68, 70 connected to one another in an articulated manner, the section 68 lying towards the support arm 42 being noticeably shorter and the longer section 70 being telescopically extendable in the usual manner and in the length desired in each case in a manner not shown can be locked. The swivel joint 72 connecting the sections 68, 70 and allowing pivoting about a vertical axis can be locked in the respectively desired swivel position by means of a lever 74. The pivotability of the sections 68, 70 against each other is also useful when using the operating table for femoral neck surgery and similar procedures in which the patient lies on the back, as this makes it particularly easy to bring the bars 44, 46 into a position in which they do not hinder the use of an image intensifier or other X-ray apparatus.

As can be seen from the top view of FIG. 1, the vertical receiving openings 64 have a center distance, measured parallel to the longitudinal center plane 38 of the mounting plate 18, from the end face 40 of the end section 30, which is approximately equal to the radius of the counter-pulling rod 32 in its section above the mounting plate 18 . Furthermore, the end section 30 has above the vertical receiving opening 64 to its front side 40 and to its longitudinal sides 76, 78 open recesses 80, 82 in the exemplary embodiment, the cross-section of which is slightly larger than the cross-section of the counter-pulling rod 32 in its section passing through the mounting plate 18 is. In other words, the end section 30 extends in the longitudinal direction so far that its end face 40 is approximately aligned with the side of the counter-pulling rod 32 facing away from the mounting plate 18. Because of this relatively large longitudinal extent of the end section 30, it is possible to use the counter-tension rod 32 or a groin support 66 to still support the pelvis of the patient lying on the side on the end section 30. Only in special cases, such as when using a groin support laterally attached instead of a support plate 50, 52, may it be necessary to provide a separate pelvic support plate at the location of the leg support plates 56, 58 which then fell away, which in a corresponding, yet to be described manner, like the leg support plates 56 , 58 is plugged into the free end of the carrier 42 and which then acts as an extension of the end section 30 beyond its end face 40.

The trapezoidal taper of the emd section 30, which, when the support plate 50, 52 is removed during the operation, facilitates access to the operating area, in particular to the femoral neck area, is made in the exemplary embodiment in such a way that the imaginary intersections of the long sides 76, 78 of the end section 30 with its end face 40 have a distance from the longitudinal center plane 38 of the bearing plate 18 that is approximately as large as the center distance of a vertical receiving opening 64 from the longitudinal center plane 58 plus the radius of the counter-pulling rod 32 in its section lying above the bearing plate 18. This results in the aforementioned position of the counter-pull rod 32 in a recess 80 or 82 designed as an inner corner, which has 64 advantages with regard to the accessibility of the vertical receiving openings.

The vertical receiving openings 64 are formed in an adapter 84, the construction of which can be seen in more detail in FIGS. 3 to 5.

The adapter 84 comprises two transverse, namely perpendicular to the longitudinal center plane 38, horizontal, mutually aligned pipe sections 86, 88. In these, the vertical receiving openings 64 are formed as bores which penetrate the respective upper and lower walls, so that one at the lower end of the Axis 34 (FIG. 2) of the counter-pull rod 32 formed pin 90, the length of which is slightly greater than the outer dimensions of the pipe sections 86, 88, can be kept inserted. The outside Directed, open mouths of the transversely directed pipe sections 86, 88 serve at the same time as below the mounting plate 18 laterally outside the vertical receiving openings 64, transversely to the longitudinal central plane 38 and horizontally oriented receiving openings 92, 94 of square cross section, in which an additional device such as the support plate 50 or 52 (Fig. 1) can be kept inserted with a suitable horizontal pin. In order not to hinder fluoroscopy, the mutual outer distance of the transverse receiving openings 92, 94 is expediently, as in the exemplary embodiment, only slightly larger than the mutual center distance of the vertical receiving openings 64.

The height of the transversely directed receiving openings 92.94 is selected so that it lies between the underside of the mounting plate 18 and the swivel joints 96, 98 of the spars 44, 46 located on the support arm 42; the perpendicular center-to-center distance between the spars 44, 46 and the transversely directed receiving openings 92, 94 is expediently between 8 cm and 15 cm or more in order to allow the spars 44, 46 to pivot through under additional devices, the horizontal pins of which are in the transversely directed receiving openings 92 , 94 are inserted.

The transverse pipe sections 86, 88 are welded to the outside of a further pipe section 100, 102 at their inner ends lying towards the longitudinal central plane 38. These pipe sections 100, 102 extend on both sides of the longitudinal center plane 38 parallel to this and thus also parallel to one another in the horizontal direction. Their inside and outside cross-sections resemble the inside or outside. External cross sections of the transverse pipe sections 86, 88. The longitudinal pipe sections 100, 102 are aligned with their end faces pointing away from the mounting plate 18 approximately with the vertical outer sides of the transverse pipe sections 86, 88 pointing away from the mounting plate 18 and thus approximately with the end face 40 of the end section 30. Next The longitudinal tube sections 100, 102 open at their end faces and thus form two longitudinal and horizontal receiving openings 104, 106 lying at the same height as the transverse receiving openings 92, 94.

The longitudinal receiving openings 104, 106 are used for the pluggable receiving of suitably dimensioned, cross-sectionally square, horizontal pins of additional plates, which in their position of use have an upper side which is at least approximately flush with the upper side of the end section 30 of the bearing plate 18. Such additional plates are the leg support plates 56, 58 shown in FIGS. 1 and 2, which in the position of use extend parallel to one another from the end section 30 of the bearing plate 18 and each have a support rod 108 (FIG. 2) which is parallel to the longitudinal central plane 38 extends to this horizontally and ends in a longitudinally adapted receiving opening 104, 106, not shown in the pin. Likewise, the above-mentioned pelvic support plate can also be provided as an additional plate, which can serve to extend the end section 30 when the groin support is held laterally in one of the transverse receiving openings 92, 94 and which can have two horizontal pins that can be inserted into the longitudinal receiving openings 104, 106.

The longitudinal pipe sections 100, 102 have a suitably dimensioned length for receiving the aforementioned pins and, as can be seen from FIGS. 3 and 5, extend for this purpose with their ends facing the mounting plate 18 over the vertical side of the transverse pipe sections 86, lying there there. 88 beyond. The center distance between the longitudinally oriented receptacle openings 104, 106 formed by the longitudinal pipe sections 100, 102 is relatively small and should in any case be less than the mutual center distance of the vertical receptacle openings 64. In the exemplary embodiment, the mutual center distance of the longitudinally receiving openings 104, 106 is approximately 10 cm. This small center distance and the corresponding small distance between the support rods 108 of the leg support plates 56, 58 ensure that the latter can be easily examined.

The mutually facing vertical sides of the longitudinally directed pipe sections 100, 102 are welded in their regions aligned with the transverse pipe sections 86, 88 to an intermediate part which is formed by a vertical pipe section 110 which has the same square cross section as the pipe sections 86, 88, 100 and 102, which extends from the height of the common upper sides of the tube sections 86, 88, 100, 102 down to the free end of the support arm 42 and is fastened to it by means of screws 112, 114. As a result, the adapter 84 essentially has an upright T-shaped configuration.

The free end of the support arm 42 is, as can be seen in particular from FIGS. 4 and 5, reverse T-shaped in cross section. Here, it has a central part 116 extending along the longitudinal central plane 38, on the front side of which facing away from the bearing plate 18, the vertical pipe section 110 is fastened by means of the screw 112. The latter passes through both walls of the vertical pipe section 110 and is surrounded in the interior thereof by a sleeve 118 which prevents the vertical pipe section 110 from being compressed. The end of the support arm 42 also points on both sides of the central part 116 two flanges 120, 122 which project outwards in the transverse direction and which project slightly beyond the central part 116 also in the longitudinal direction. The wall of the vertical pipe section 110 lying away from the mounting plate 18 continues downward in a flange 124 which is fastened by means of the screws 114 to the region of the flange extensions 120, 122 which projects beyond the central part 116 in the longitudinal direction. As a result, the adapter 84 is overall very rigidly connected to the free end of the support arm 42, which is important in view of the bending moments exerted by the leg support plates 56, 58 on the adapter 84, for example. The top of the adapter 84 is also connected via support columns 126, 128 to the end section 30 of the bearing split 18, whereby the bending stiffness is further increased and the end section 30 is additionally supported in the vertical direction. To increase the strength of the transverse pipe sections 88 with bending moments acting on the counter-pull rod 32, the upper and lower sides of the transverse pipe sections 86, 88 are each reinforced in the vicinity of the vertical receiving openings 64 by means of welded-on metal rings 130, 132 and 134, 136.

The swivel joints 96, 98, by means of which the spars 44, 46 are articulated at the free end of the support arm 42, are arranged on both sides of the longitudinal median plane 38 at a mutual distance, namely they are located on both sides of the central part 116 and vertical pipe section 110 on the upper sides of the flange extensions 120, 122. They can be locked in the desired position using levers 138, 140. Since the swivel joints 96, 98 lie below the end section 30 without projecting beyond the end face 40 in the longitudinal direction, fluoroscopy 44, 46 can be carried out from the outside to the end face 40 of the end section 30 in the case of suitably pivoted spars without the swivel joints 96 , 98, the spars 44, 46 or the adapter 84 interfere.

Der Zapfen 90 des Gegenzugstabes 32 und damit die dieIn Fig. 8 the formation of the triangular support plate 52 is shown in more detail; the support plate 50 is of a corresponding, mirror-symmetrical design. The support plate 52 has a radiolucent, padded horizontal upper part 142, a horizontal support rod 144 lying below the upper part 42, the vertical part 146 and 148 connecting the upper part 142 and the support rod 144 and a horizontal pin 150 of square cross section, which is an extension of the Support rod 144 is formed and in the plan view projects above the upper part 142 to the outside. In order to bring the support plate 52 into its use position laterally of the end section 30 (FIG. 1), the pin 150 is inserted into the transversely directed receiving opening 94 of the transversely directed pipe section 88. The pin 90 of the counter rod 32 and thus the

The vertical receptacle opening 64 (FIG. 4) has a smaller diameter than the clear width of the transverse pipe section 88 measured in the horizontal direction, and the pin 150, the length of which is greater than the length of the pipe section 88 between its mouth end and the one passing through it vertical receiving opening 84 has a vertical, slit-shaped recess 152 which is open towards its end and the width of which is at least as large as the diameter of the pin 90 and the vertical receiving openings 64. It is thus possible, at any time, for the support plate 52 to be inserted despite the inserted counter-pulling rod 32 to attach or remove, and vice versa to insert or remove the counter-pull rod 32 or a strip support 66 (Fig. 2) at any time, regardless of whether the support split 52 is installed or not.

For additional devices that can be inserted instead of the support plates 50, 52 with horizontal pins into the transverse receiving openings 92, 94 and which are to be secured against a displacement of their pin out of the transverse receiving opening 92, 94 when the counter-pull rod 32 is removed, it can deviate from the construction of the support plates 50, 52 may be expedient to provide the pin with a vertical bore which is aligned with a vertical receiving opening 64 when the pin is inserted into a transversely directed receiving opening 92, 94, and to insert a bolt into the latter which defines the vertical receiving opening 64 and penetrates said hole of the pin to lock the latter. This construction is particularly useful for use in special cases, in place of a support plate 50, 52 laterally attached last supports.
