The complex, perfectly coordinated interplay of several anatomical elements / physiological units of the so-called aortic root ensures that the aortic valve functions in a physiological manner. The anatomical structure of the aortic root is correspondingly complex.
The "aortic root" is composed of aortic valve and aortic bulb. The aortic root is delimited proximal to the blood flow by the so-called annulus (surgical annulus) and distal to the blood flow by the so-called sinutubular junction (sinutubular junction, STJ).
The three sinus valsalvae, which are also called sinus aortae, form the cavities that are enclosed by the three pockets of the aortic valve, i.e. are limited by the commissures of the respective pocket and the aortic wall (coat) and are referred to in their entirety as "aortic bulb" (bulbus aortae).
The architectural relationships of the aortic root elements to each other, in particular the aortic bulb and the aortic valve, guarantee a physiological mode of operation by ensuring a competent closure of the aortic valve pockets through the even distribution of pressure and tension on the individual elements, especially pockets, sinuses and commissures.
One of the functions of the sinus valsalvae is the rapid closure of the aortic valve, which is caused by the vortices that develop in the blood column. In addition, the sinus valsalvae absorb the pressure created by the blood column and the tension when the valve closes.
There is great disagreement in the state of the art and science about the exact dimensions and the exact functioning of the aortic root or its elements. However, it seems certain that the dimensions and the relationships of the individual elements to each other change with increasing age, as well as with the presence of certain diseases such as high blood pressure, arteriosclerosis and connective tissue diseases. These changes lead to a mismatch in the pressure and stress distribution on the individual elements. This causes pathological changes at the tissue level.
Framework-free valve prosthesis systems, in particular aortic valve prosthesis systems or framework-less closing organ prosthesis systems for implantation in the body of a mammal are known in the prior art, in particular also for total aortic valve and aortic bulb replacement.
Various indications compel (total) aortic valve and aortic bulb replacement or their reconstruction, such as aortic root aneurysm, aortic dissection, endocarditis, congenital aortic valve abnormalities and connective tissue diseases.
In the state of the art, aortic valve and aortic bulbus reconstruction represents an enormous challenge. Its success depends heavily on the subjective experience of the surgeon. The surgical implementation of the reconstructions disadvantageously takes a great deal of time, in particular also during the ongoing operation of the patient with corresponding risks, the longer the operation takes. Another disadvantage is that the reproducibility with regard to the reconstructions is usually not given.
A disadvantage of known prior art aortic valve and aortic bulb prostheses that are currently on the market is that they have the physiological function or physiological function Do not adequately enable the capacity of the replaced natural organic structures. Furthermore, the lifespan and the availability of such known prostheses are limited. Last but not least, due to the foreign material frequently used in the prior art of the prosthesis used, there is an increased risk of inflammation for the patient, which should not be underestimated, in particular an increased risk of endocarditis when using such prior art prostheses.
The object of the present invention is to at least partially overcome the disadvantages known in the prior art. The above object is achieved by a scaffold-free prosthetic valve system according to the invention. Preferred embodiments of the scaffold-less valve prosthesis system are the subject of the dependent claims.

The scaffold-free prosthetic valve system for implantation into the body of a mammal according to the invention, which can be designed in particular as a scaffold-free aortic valve prosthesis system, has the following features:
- i) three flexible, sail-shaped closing elements made of one material, the three closing elements each being offset from one another by 120 ° C. and having a common central axis; and wherein each closing element has two parallel, substantially straight and coaxial to the central axis side edges and an upper edge and a lower edge, wherein the upper and lower edges are substantially partially elliptical and run parallel to each other; and the three closing elements can each be connected to an essentially cylindrical jacket, which can be arranged coaxially to the central axis, to form three pockets and three sines; and

The scaffold-free prosthetic valve system according to the invention is characterized in that the circumference of the essentially partially elliptical lower edges of the closing elements is essentially the same size and the height of each commissure is essentially the same size. In the first position of the closing elements, the central coaptation point is essentially centered between an annulus (basal ring), which defines a circular plane between a deepest point of each of the closing elements on the part elliptical lower edges of the closing elements and a sinutubular transition that defines a circular plane between a highest point of each of the commissures on the side of the partially elliptical upper edges of the closing elements, arranged in the region of the central axis.
When implanted in the patient, the upper edges of the closing elements of the frame-less prosthetic valve system designed as a scaffold-free aortic valve prosthesis system are oriented to the so-called aorta ascendens and the lower edges of the closing elements are oriented to the so-called outflow tract.
For the purposes of the present invention, an “essentially annular opening” is also to be understood as the geometric shape of a truncated cone. Alternative geometric shapes of the opening are also conceivable, for example that of a triangle.
For the purposes of the present invention, a “mammal” is to be understood as a mammal of any species which is of social, economic or research importance, including but not limited to mice, rats, dogs, cats, sheep, goats, cows, Horses, pigs, non-human primates and humans. In a preferred embodiment, the mammal is a human.
In the sense of the present invention, a “patient” is understood to mean a mammal in the sense of the invention that is provided for an implantation of a prosthesis system according to the invention or that it has in the implanted state.
In the sense of the present invention, a “recipient” is to be understood as a patient who is provided for a prosthesis system according to the invention.
The inventors of the present invention have surprisingly succeeded in providing a scaffold-free prosthetic valve system, in particular a scaffold-free aortic valve prosthesis system for implantation in the body of a mammal, the prosthesis system, while its structure does not correspond to the individual physiological-organic conditions of the patient to whom it is to be implanted, but because of its novel construction and its new shape essentially corresponds to or essentially comes close to the physiological function of the native aortic valve.
Understanding the physiological function of the aortic root requires precise knowledge of the mathematical and architectural relationships between the aortic root structures. These are not in a random relationship to each other, but are perfectly coordinated.
The construction of the prosthesis system is based on solid mathematical and architectural foundations, which are calculated individually according to the anatomical conditions of the patient. For this purpose, it is expedient that the prosthesis system is prefabricated in different sizes so that it can be selected according to the anatomical conditions of a patient and used for the patient. In particular, this has a time-saving advantage during the implantation operation of the prosthesis system. This also reduces the risk of surgery that is always present. increases with increasing operation time. It is also possible to make further adjustments to the prosthesis system before the implantation of the prosthesis system before it is implanted in the patient, for example to improve the individual fit of the prosthesis system to the organic conditions of the patient.
The aortic valve prosthesis system according to the invention has the advantage that its closing elements close very well and open very widely, corresponding essentially to the native conditions of an aortic valve. The prosthesis system, when installed in the patient, enables hemodynamic conditions in the closed state as well as in the open state (systole and diastole in the heart valve) of the annular opening, which essentially correspond to those of the native aortic valve. The opening area and the pressure gradients essentially correspond to that of the native aortic valve. Due to the special design of the prosthesis system, the tension distribution in the area of the pockets, the commissures and the aortic wall is advantageously essentially physiologically distributed. As a result, the design according to the invention in the implanted state enables essentially undisturbed flow of the blood flow in the open state of the annular opening of the aortic valve prosthesis system and thus avoids disadvantageous high shear stresses generated by blood vertebrae and the pressure difference across the heart valve during diastole is particularly advantageously absorbed. In addition to the simulated native physiological hemodynamics, this also favors the longevity of the prosthesis system in use, since pressure and shear stresses attack and stress the elements of the prosthesis system less strongly due to the described favorable distribution of forces, and thereby the wear is drastically reduced, especially compared to the prior art prostheses.
According to one embodiment of the scaffold-free prosthetic prosthesis system according to the invention, the first material, non-positive and / or positive connections are connected to one another by sewing and / or welding and / or gluing and / or combinations of the aforementioned.
According to a further embodiment of the scaffold-free prosthetic valve system according to the invention, the material essentially consists of biological material, in particular of pericardial tissue, fascia lata or dura mater, for example of animal origin.
Tissue material and / or cell material which is of human or animal origin, for example originating from pork, cattle or horse, can be considered as “biological material” in the sense of the invention. The valve prosthesis system according to the invention can either represent a homograft in which the tissue and / or cell material donor belongs to the same species as the recipient, or it can represent a xenograft in which the tissue and / or cell material donor does not belong to the same species as the recipient listened to. Tissue material and / or cell material is either originally biocompatible (compatible with blood and tissue) or is made biocompatible by appropriate modifications, in particular surface modifications, or the existing original biocompatibility of the material is increased accordingly by such modifications.
According to an alternative embodiment of the scaffold-free prosthetic valve system according to the invention, the material is essentially selected from the group consisting of highly flexible biocompatible polymers, in particular the polyester, consisting of polyurethane, polytetrafluoroethylene (PTFE), polyethylene terephthalate (PET), preferably polyurethane. Polyethylene terephthalate (PET) is known, for example, under the brand name Dacron.
„Hochflexibel biokompatible Polymere“ im Sinne der vorliegenden Erfindung sind solche biokompatiblen Polymere, die im implantierten Zustand den enormen und stetigen Druckbelastungen aufgrund der gegebenen physiologischen Hämodynamik, d.h. unter entsprechender Zug- und/oder Druckbelastung flexibel und biegsam bis zu einem Biegewinkel von ca. 90° bei entsprechender hoher Materialfestigkeit standhalten."Highly flexible biocompatible polymers" in the sense of the present invention are those biocompatible polymers which, in the implanted state, can withstand the enormous and constant pressure loads due to the given physiological hemodynamics, i.e. withstand a corresponding tensile and / or compressive load flexibly and bendably up to a bending angle of approx. 90 ° with a correspondingly high material strength.
According to a further embodiment of the scaffold-free prosthetic valve system according to the invention, the material consists both partly of highly flexible biocompatible polymers and partly of biological material.
For the purposes of the invention, parts or the entire surface of the valve prosthesis system according to the invention can be surface-modified. The material according to the invention can in this case be coated with one or more medically active substances in order to increase the biocompatibility of the prosthesis system for the recipient and to have a favorable influence on the implantation of the prosthesis system at the implantation site. Such medically active substances, for example those which prevent stenosis and / or inflammation and / or improve tissue healing at the implantation edges, are known to the person skilled in the art and are hereby fully included.
Furthermore, it is within the meaning of the invention to provide a scaffold-free closure organ prosthesis system, in particular a scaffold-free aortic valve and aortic bulbus prosthesis system for implantation in the body of a mammal, comprising a scaffold-free prosthesis system according to the invention with an essentially cylindrical jacket, made of a first material with an outer and an inner wall, an inlet end and an outlet end, a predetermined height and a predetermined extent; and wherein the inner wall of the jacket has three sines, each with a partially elliptical lower edge for receiving the three closing elements made of a second material, each of which forms three pockets connected to the jacket; and wherein the partially elliptical lower edge of the sine and the lower edge of the pockets are essentially of the same size and the side edges of each closing element on the side edges of the adjacent closing element in the region of the jacket and the side edges of the closing elements each with the adjoining sections of the inner wall of the jacket first material, have non-positive and / or positive connections and determine commissures with a predetermined height; and the lower edges of the closing elements each have second material, non-positive and / or positive connections with the adjacent sections of the inner wall of the jacket.
The inventors of the present invention surprisingly succeeded in providing a framework-less closing organ prosthesis system, in particular a framework-less aortic valve and aortic bulb prosthesis system for implantation into the body of a mammal, the structure of the prosthesis system not corresponding to the individual physiological-organic conditions of the patient to whom it is built should be implanted, but due to its new construction and its new shape essentially corresponds to or essentially comes close to the physiological function of the native aortic valve and the aortic bulb.
The construction of the prosthesis system is based on mathematical and architectural principles, which can preferably also be calculated individually in accordance with the anatomical conditions of the patient. It is also possible to make further adjustments to the prosthesis system before the implantation of the prosthesis system before it is implanted in the patient, for example to adapt the individual fit of the prosthesis system to the organic conditions of the patient.
The prosthesis system allows hemodynamic conditions built into the patient, which essentially approximate those of the native aortic valve. The opening area and the pressure gradients correspond to those of the native aortic valve with aortic bulb. Due to the special construction of the prosthesis system, the tension distribution in the area of the pockets, the commissures and the aortic wall is advantageously physiologically distributed.
According to a further embodiment of the closing organ prosthesis system, the jacket has a greater height than the side edges of the closing elements.
According to a further embodiment, the greater height in the sense of the present invention can be such that the edge surface of the jacket projecting beyond the side edges of the closing elements on the side of the upper edges of the closing elements is 0.5-5.0 cm, preferably 1.0-3. 0 cm and particularly preferably 1.0 - 2.0 cm and the edge surface of the jacket projecting over the side edges of the closing elements on the side of the lower edges of the closing elements is either the same size as that on the side of the upper edges of the closing elements or 0.5-1, Is 0 cm.
Such dimensions of the edges of the jacket protruding on both sides make it easier for the surgeon when implanting the prosthesis system to adapt it to the patient's organic conditions and to connect it to the patient's native vascular system. The surgeon can adapt the protruding edges of the jacket according to the given needs before or during the implantation, in particular shorten it, for example by cutting it to a certain height.
According to a further embodiment of the closing organ prosthesis system, a suitable suture ring can be provided on the cylindrical jacket described at one or both of its ends, which enables attachment to the corresponding tissue during implantation.
According to a further embodiment of the closing organ prosthesis system, the ratio of the circumference of the annulus (basal ring) and sinutubular transition is 1: 0.8 -1: 1.4, preferably 1: 1 - 1: 1.2 and particularly preferably 1: 1.
The inventors of the present invention surprisingly found that such conditions advantageously enable the prosthesis system implanted in the patient, which is preferably designed as a frame-less aortic valve and aortic bulb prosthesis system, to reproduce the physiology of the native aortic valve and aortic bulb in a particularly precise manner.
According to a further embodiment of the closure organ prosthesis system, it has three inter-commissural triangles, which are each arranged between immediately adjacent closure elements in the area of a commissure and extend with the base from the annulus perpendicularly in the direction of the highest point of a commissure with a predetermined height, this height 1/3 - 2/3 and preferably 1/3 - 1/2 the height of the commissures.
The relationship between the annulus and sinutubular junction as well as the height of the inter-commissural triangles in the described relationship to the commissures can vary. The geometric height of the bag changes accordingly.
According to a further embodiment of the closing organ prosthesis system, the first and / or the second material, force and / or positive connections are connected to one another by sewing and / or welding and / or gluing and / or combinations of the aforementioned.
According to a further embodiment of the closing organ prosthesis system, the first material of the sheath and the second material of the closing elements consist of the same material, preferably essentially of biological material, in particular of pericardial tissue, fascia lata or dura mater, for example of animal origin, or are essentially from the group selected from highly flexible biocompatible polymers, in particular the polyester, consisting of polyurethane, polytetrafluoroethylene (PTFE), polyethylene terephthalate (PET), preferably polyurethane.

The material for a closing organ prosthesis system according to the invention is either inherently biocompatible or else made biocompatible by processing, for example by coating with a biocompatible material. Corresponding biocompatible materials that come into contact with tissue and body fluids, and corresponding coating processes for generating biocompatible materials are known to the person skilled in the art.
Tissue material and / or cell material that is of human or animal origin, for example from pork, cattle or horses, can be considered as “biological material” in the sense of the invention. The closing organ prosthesis system according to the invention can either represent a homograft in which the tissue and / or cell material donor belongs to the same species as the recipient, or it can represent a xenograft in which the tissue and / or cell material donor does not belong to the same species as the recipient listened to. Tissue material and / or cell material is either originally biocompatible (compatible with blood and tissue) or is made biocompatible by appropriate modifications, in particular surface modifications, or the existing original biocompatibility of the material is increased accordingly by such modifications.
According to a further embodiment of the closing organ prosthesis system, the first material or the second material is essentially selected from the group consisting of highly flexible biocompatible polymers, in particular the polyester, consisting of polyurethane, polytetrafluoroethylene (PTFE), polyethylene terephthalate (PET), preferably polyurethane.
According to a further embodiment of the closing organ prosthesis system according to the invention, the material consists both partly of highly flexible biocompatible polymers and partly of biological material.
- Level A: This level spans the level of the base of the commissures, is ring-shaped, its circumference is equal to 3 · Y (see
1B , Reference numerals:36 indexed in1B the corresponding proportion of a closing element, which is y). This level corresponds anatomically to the ventriculo-aortic transition. - Level B: This level spans at the level of the central coaptation in the area of the central axis of the closing elements, is ring-shaped, its circumference is equal to 3y (
1B ), Reference number:37 indexed in1B the corresponding proportion of a closing element, which is y).
In
- r = radius of the pocket, Y = chord. The tendon stretches between two radii whose angle Θ to one another is 120 degrees and y is equal to one third of the circumference of the essentially cylindrical sheath (not shown) which forms the aortic base and has the same circumference as the aortic valve annulus. In
2nd one sees the three sines of the opened closing elements of an aortic valve prosthesis system according to the invention, which clarifies the geometric conditions and relationships of the aortic root.

Für die Taschen gilt hierbei:The following applies to the bags:
Für die Sinus gilt hierbei:The following applies to the sine:
Commissure = radius r of the elliptical part of the sine, which is equal to the radius r of the elliptical part of the pocket,
Y = long diameter of the elliptical part of the sinus = 2r · sin Θ = 2r · Sin 120 = free edge of the sinus = 1/3 of the circumference of the aortic wall or the mantle.

Without being bound by theory, this zone contributes differently to coaptation in the native aortic valve depending on the size of the inter-commissural triangle (English ICT).
In the connected state, with the formation of a functional aortic valve and aortic bulbus prosthesis system according to the invention, an essentially cylindrical jacket is formed with the three levels: annulus level, sinus level and the level of the sinutubular junction. Depending on the hemodynamics of the heart (systole and diastole), these 3 levels behave differently.
The movements of the aortic root elements on the different levels and their geometric behavior, particularly but not exclusively on the annulus level, are of essential importance for an essentially native physiological function of the aortic valve. These are subject to two important phases: first, the systole, in which the blood is ejected via the outflow tract and the aortic valve, and second, the diastole, in which the blood pressure acts on the closed aortic valve and root.
Veränderungen auf SinusebeneChanges at the sinus level
Veränderungen auf EbeneALevel A changes
Veränderungen auf Ebene B - Betrifft nur die Ausführungsform mit größerem Sinus nach Fig. 1D, Fig. 3B: Changes at level B - only affects the embodiment with a larger sine according to FIGS. 1D, 3B:
The ratio rB to rA is constant and independent of the annulus size.
The aortic diameter at this level is equal to 2r, where r is the radius of the pocket.
Auf beiden Ebenen
Die Veränderungen auf Anulusebene (Anulusphysiologie)The changes at the annulus level (annulus physiology)
Without being bound by theory, the free mobility of the annulus guarantees a physiological function of the aortic valve. The shape of the aortic valve annulus changes in systole and diastole.
Die Veränderung auf der Ebene des sinutubulären Übergangs (STJ)The change at the level of the sinutubular junction (STJ)
Die Veränderung auf der Ebene der gesamten KoaptationsflächeThe change on the level of the entire coaptation area
Die gesamte Koaptationsfläche wird in drei Zonen unterteilt: The entire coaptation area is divided into three zones:
-
I. Adaptationszone
Adaptation zoneA (Reference number:40 , please refer17B ,19th , dashed zone) This zone is formed by the portion of the pockets that connect the base of the commissures and the central coaptation point Z and is equal to r and the elliptical area of the pocket that forms a convex connecting line between the bases of the commissures and the central coaptation point and is equal to ½ X. With the native aortic valve, this zone contributes differently to coaptation depending on the size of the inter-commissural triangle (ICT). -
II. Basale Koaptationszone
Basal coaptation zoneB (Reference number:41 , please refer17B ,19th , dotted zone) This zone is formed by the portion of the pockets which connect the base of the commissures and the central coaptation point Z and form a concave connecting line and is equal to ½ X, and the area below the planeB the sine plane (the horizontal connecting line between the central coaptation point and the center of the commissures). -
III.
Coaptation bridgeC. (Reference number: 42, see17B ,19th , checkered zone) This zone is formed by the portion of the pockets which connects the highest point of the commissures and the central coaptation point Z and is equal to ½ X and the area above the planeB the sine plane (the horizontal connecting line between the central coaptation point and the center of the commissures), which is a concave connecting line between the highest point of the commissures and the central coaptation point. According to the inventors, this zone functions according to the laws of a suspension bridge.
Physiologie der KoaptationPhysiology of coaptation
19 ).
- Zone
A andB : The force F (blood pressure) acts on the elliptical part of the respective sinus and the respective pocket and causes the sinus to move outwards and the pocket inwards. - Thus, the minor vertex of the ellipse
A longer and the main vertex of the ellipse b shorter. - This mechanism pulls the base of the commissioner inwards (see
19th ).
