因此,本发明的一个目的是实现治疗女性小便失禁的外科方法,该方法包括的步骤为:在尿道平面之上的水平线上确定手术穿出点,在该点处手术针将在病人的腹股沟穿出;在***壁上切一个切口;朝向坐骨耻骨支进行侧尿道切开,同时避免***壁穿孔;绕坐骨耻骨支旋转手术针,并用手术针的尖端从病人体内穿刺先前确定的手术穿出点。
本发明的另一个目的是采用手术针来治疗女性小便失禁,该手术针包括一个手柄,一个具有两端的直段和一个具有两端的螺旋段,所述直段的一端与所述手柄相连,所述螺旋段的一端在其结点处与所述直段的另一端相连,而所述螺旋段的另一端为具有尖端的自由端。
参考下面结合附图的说明将更好地展示本发明的这些和其它目的,以及由此具有的许多优点。

优选实施例的详细说明
在对附图中示出的发明的优选实施例的描述中,为了清楚起见将采用特定的术语。但是,本发明将不受所选特定术语的限制,并且可以理解每个特定术语包括所有的以类似方式操作,以达到类似目的的技术等同物。
采用本发明的外科手术时,病人首先摆成妇产科医学姿势,腿放在镫形架上,大腿处于屈曲过度状态。病人的臀部(i)伸到手术台的边缘。用标准防腐剂清洗手术区,用多条手术单覆盖,而不是用单条裤状手术单,小心地保持腹股沟褶在手术区内。用尼龙缝合线将小***(j)悬吊固定到大腿褶内的皮肤上以高于外阴口几厘米,以便于暴露出外阴,并保持腹股沟褶(k)在手术区内。将一只16 Fr Foley导管***到膀胱中。
在尿道口(n)水平位置描出一根水平线,以确定针将从皮肤水平位置穿出的点(m)。穿出点位于高于该线约1至3厘米,优选为2厘米处,并向外距离腹股沟1-4厘米,优选为2厘米(参见图1和10A)。在每个穿出点处切一个2至10毫米,优选为5毫米的皮肤切口。
在接近尿道口1厘米处中线的两侧用两个Allis夹具悬吊起前***壁。***壁的中央矢状切口开始位于该水平面上,并(朝向***凹口)延伸1厘米的距离(图10B)。将***黏膜和黏膜下组织均切开。然后用刀刃在每一侧最小限度地横向切割尿道侧面和***下部几毫米深(图11A)。
用一个Allis夹具抓住右侧大小***,同时用另一个Allis夹具保持尿道***道切口的左边缘,以清楚地暴露右外阴前庭的最后方。将精细解剖剪引导穿过由刀刃初始化的解剖通道,然后接下来以相对于尿道矢状面45度角朝向坐骨耻骨支的上部伸进(图11B和11C)。重要的是应正确暴露外阴前庭,并注意解剖的特定方向,以避免发生任何***壁穿孔。
一旦到达了坐骨耻骨支的上部,感觉到与骨接触时,用剪刀的尖端刺穿右闭孔膜,使之略微张开。在解剖时,可能会出血,但这不重要,仅仅是有时候需要止血装置。然后将导引器推入预先形成的切开通道中,直到其触到闭孔膜并刺穿它。导引器槽的开口侧应朝向操作者(图11D)。管的远端安装到针的螺旋段上,并且组装好的装置轻轻地沿导引器的槽滑动,以穿过闭合孔(图11E)。将导引器和Allis夹具撤走。在该步骤中,针的手柄应以与***裂隙的矢状轴平行的方式操作。然后,通过穿过件的旋转移动,管的尖锐的尖端出现在位于腹股沟水平面的先前切开的皮肤穿出点上(图11F)。通过向后旋转移动管,将管从支撑针中拉出,直到带子的头1厘米变得露至外面。
在左侧实施相同的技术,注意不要使带子扭转。当两个管均拔出穿过皮肤切口后,切去带子的端部。接下来使带子在中尿道和远端尿道之间的结点下方对准,在带子的两端施加拉伸力,并且在带子和尿道之间***一剪刀来调节带子的张力,以留下防止带子有任何张力的空间。然后,同时撤去塑料套(图12A)。用于使带子在尿道下方正确对准的另一操作过程是,用Babcock镊夹住带子的中间,以产生小的、5毫米长的带环(图12B)。如上所述,此时在带子远端施加了拉伸力,从而使Babcock镊夹着与尿道接触。将塑料套和Babcock镊先后撤去,这样在带子和中尿道的腹侧方之间产生了小的尿道下方空间。在皮下层内切去带子端部,并缝合切口。
可采用四种不同的特殊设计的手术针来使尿道下带子滑动穿过闭合孔(o)。在图2A~2D中示出了针4的内外穿过闭孔通道。针4通过简单的旋转移动,绕坐骨耻骨支(p)从内至外(具体地说为从尿道下方的***创口朝向腹股沟褶)旋转(图2A,2B和2C)。针刺穿填充闭合孔(o)的各种肌肉结构和膜,并利用其螺旋设计,使针的尖端5从位于腹股沟褶旁边的先前确定的皮肤穿出点露出。然后用针的类型将带子(t)或者直接***或者间接***(图2D)。
四种器械具有共同的方面(参见图3A~3D),其螺旋形针具有由共同的手柄支撑的确定设计(长度、弯曲度、直径、形状)。所有这些装置制造出来用于或者间接(“线穿过”和“管穿过”针),或者直接(“管支撑”和“针保持器”针)将带子从内至外(具体地说为从尿道下方的***创口朝向腹股沟褶)***。
四种手术针包括扁平螺旋段4,该螺旋段具有确定的直径,并如图3A所示通过结点6附着到垂直段3上。扁平螺旋段4包括一个开口环状段或近似环状段,其具有2/4至3/4圆周的长度,并有一个末端5,该末端距垂直段和螺旋段之间的结点6有一特定距离b和d(Ab@和Ad@)(图3C)。b和d的距离可以变化,b可以是小于3.5cm,d可以为4至8cm。扁平螺旋段4还可包括长度为1至3.5cm的两个直线段8和9,如图4至7所示。
在图4A~4C的线穿过(Astring-passing@)针中,装置的下段A包括由固定螺钉2固定到装置上段的,长度为5-15cm,优选为10cm的手柄。装置的上段B直径为2-5mm,优选为4mm,包括有长度为6-18cm,优选为12.5cm的扁平螺旋段4,该螺旋段在结点6处附着到长度为5-15cm,优选为9cm的垂直段3上。扁平螺旋段为开口的螺旋形,接近于环形段,其端部或尖端5位于距点6的距离为d的位置。螺旋段的末端包括3-15mm,优选为8mm长的针眼7,扎带、线或带子可穿过该针眼。扁平螺旋段可包括两个长度为1-3.5cm,优选为2cm的直线段8和9。距离d可以在4至8cm范围内变化。
另外,器械的上段B和下段A可制成为一整体段,没有结点部分。
在图5A~5C所示的“管穿过”针中,装置的下段A包括长度为5-15cm,优选为10cm的手柄1,该手柄通过固定螺钉2固定在装置的上段B上。装置的上段B直径为2-5mm,优选为4mm,包括长度为6-18cm,优选为12.5cm的扁平螺旋段4,该螺旋段在结点6处附着到长度为5-15cm,优选为9cm的垂直段3上。扁平螺旋段为开口的螺旋形,接近于环形段,其末端或尖端5′具有5-15mm,优选为7mm长的锥形,由1至5个,优选为3个弯曲段形成,使管能够附着到螺旋段上。扁平螺旋段可包括两个长度为1-3.5cm,优选为2cm的直线段8和9。
另外,器械的上段B和下段A可制成为一整体段,没有结点部分。
在图6A~6D所示的“管支撑”针中,管支撑装置能够以一种独特的旋转移动使带子直接从内至外通过。该器械支撑一新型部件,该部件为一箭状管,其远端封闭并形成尖状。带子粘在管的近端的内部。
装置的下段A包括由固定螺钉2固定到装置上段B的,长度为5-15cm,优选为10cm的手柄。装置的上段B直径为2-5mm,优选为3mm,包括有长度为4.5-17.6cm,优选为11.8cm的扁平螺旋段4,该螺旋段附着到长度为5-15cm,优选为9cm的垂直段3上。“管支撑”针的扁平螺旋段比“线穿过”针或“管穿过”针的螺旋段短0.4-1.5cm,优选0.7cm。扁平螺旋段为开口的螺旋形,接近于环形段,其端部或尖端5″为扁平的。扁平螺旋段可包括两个长度为1-3.5cm,优选为2cm的直线段8和9。
另外,器械的上段B和下段A可制成为一整体段,没有结点部分。
由如图6E和8E所示管支撑针支撑的部件为箭状管C,它通常由高密度聚乙烯制成(但不排除其它材料),长度为10-25cm,优选为17cm。管的内径为2.0-4.5mm,优选为3.3mm,外径为3.0-5.5mm,优选为4.3mm。管的壁厚为0.3-0.8mm,优选为0.5mm。管的远端长度为3-40mm,优选为7mm,呈具有尖锐部分的尖端,长度为3-20mm,优选为5mm。管支承一横向开口,长度为5-20mm,优选为10mm,位于距管的远端10-20cm,优选为12.5cm之处。该横向开口能够使针的螺旋段4导引进入管中,如图6D所示。管C的近端20在其内部与带子22粘在一起。
在“针保持器”装置中,如图7A-7F所示,“针保持器”装置包括一新型段,该新型段有一个手柄1和一对针,在针的端部附着有带子。这种装置能够在一个单一旋转移动中使带子从尿道下方朝向闭合孔滑动(具体地说为从内至外)。
装置的下段A包括由固定螺钉2固定到装置上段3的,长度为5-15cm,优选为10cm的手柄1。装置的上段B分为两个部件:垂直段3和螺旋段4。垂直段在其远端24包括槽26,图7D中所示的针的螺旋段4的近端可***该槽中,并由销钉***固定在该位置。呈开口螺旋状的螺旋段4接近环状段,长度为6.0-18.0cm,优选为12.5cm,直径为2-5mm,优选为4mm。它的远端或尖端5为尖锐的。扁平螺旋段可包括两个长度为1-3.5cm,优选为2cm的远端直线段8和9。螺旋段的近段8也可为直线形的,包括一个能使销钉30***的小槽28,以及一个设计用来将带子附着到螺旋段上的针眼32。
如图7E和7F所示移动螺旋段3的其中一段34将销钉30***到槽28中。图7E的静止位置所示为,在螺旋段4回缩之后***到槽26中时,销钉30准备啮合到槽28中,然后如图7F所示,段34伸展。
带子附着在其上的“针保持器”针的螺旋段在其通过一个单一旋转移动从内(尿道下方)至外(腹股沟褶)穿过闭合孔和皮肤后,可以将其从其支撑手柄上移去。
一旦用剪刀在闭孔膜上形成了从尿道下方朝向坐骨耻骨支的预留通道,操作者可能在将这种新设计的装置的针滑入穿过该预留通道时会遇到一些困难。如图9A-9C所示的被称为“导引器”的器械可容易地使针朝向闭孔膜通过***区域。这种器械长度为9-15cm,优选为11cm,其包括两段:近端的长度为2-7cm,优选为4cm,直径为3-6mm,优选为4mm的管状段34和远端半圆形的,长度为5-11cm,优选为7cm,直径为3-6mm,优选为4mm的槽g。导引器的槽被引导与坐骨耻骨支的上部接触。针经导引与槽并置,该槽扮演着鞋拔的角色,使针易于朝向闭孔膜滑动。
当“线穿过”针(图4)或“管穿过”针(图5)用于手术中时,在针通过之前需要用剪刀在闭孔膜上做出一个预留通道。然后,针绕坐骨耻骨支(p)旋转。针刺穿填充闭孔(o)的各种肌肉结构和膜,并借助其螺旋设计,使针的尖端从在腹股沟褶侧向的先前确定的皮肤穿出点处露出。采用相反弯曲的针,在相对侧重复相同的操作。
在两侧,线或管分别附着到“线穿过”针或“管穿过”针上。针与线或管一起反向朝着尿道***道口旋转。然后将通常由聚丙烯(也不排除其它材料)制成的合成(或生物)带子系到线或在尿道侧的管子上。接下来,从皮肤端牵接线或管子,使带子在两侧由内至外滑动。
当采用“管支撑”针(图6)或“针保持器”针(图7)时,可实施相同的技术。在这两种针变型中,通过在两侧支撑针的手柄上施加简单地旋转移动,使直接附着在管或针上的带子可直接从尿道的下方穿过而到达穿出点。另外,一旦穿过闭孔之后,针的端部可以从手柄上脱离(参见图7)。
无论采用哪一种设计的针,一旦手术进行完毕,带子将正确位于中尿道下方,同时注意不要使带子扭转或使之变形成勒带。在悬带的端部不要施加张力,这样在带子和尿道后方之间留有空间。在空隙中***一剪刀可易于控制尿道和带子之间的上述空间。
每次在带子缠绕在塑料套上时,一旦带子已穿过每一侧,就必须将这些缠绕件撤去。将带子在皮肤穿出点水平面上剪去,并且不必固定。用缝合线缝合***和皮肤切口伤口。
图8A-8F所示的“直管支撑”针是前面描述的“管支撑”针的变型,能够在图6E和8E所描述的附着到带子上的箭状管中滑动。该装置已设计成将带子从下侧(具体地说为尿道下方)***到上侧(具体地说为耻骨前部或后部区域)。该装置包括一手柄,该手柄附着到呈矢状弯曲,长度为12-18cm,优选为15cm,厚度为2.5-3.5mm,优选为3mm的上段B上,该上段支撑着附着到带子C上的管。直管支撑(Astraight tube supporting@)针与这里描述的其它针类似,可为一新型的一次性段结构。
实施例
采用上述外科手术对总计107例病人进行了连续治疗。手术分别在82例脊柱麻醉、24例全麻和1例局麻的情况下进行。
病人的平均年龄为62±12.6岁(中间值=62,范围为29-88),平均经产状况为2.54±1.7次(中间值=2.5,范围为0-9)。17例病人(15.9%)曾做过手术治疗失禁和/或***脱垂。
详细的病历、身体检查、内窥镜评估和排尿疼痛试验证明,大多数病人(n=74;69.2%)具有典型的SUI症状。每个病例的Ulmsten试验呈阳性。其它病人(n=33;30.8%)具有相应的骨盆器官脱垂。在该组中,45.4%的病人具有真性SUI,54.6%具有潜在SUI。
所有病人手术后均进行了每月一次的随访,并进行了详细的面谈、诊所检查、小便分析以及postvoid残留测定。
对所有107例连续病例对象进行手术,不考虑病人的身高和体重。将每个214针穿过闭孔膜,并精确地从已标记和切口的皮肤水平位置处穿出。
在隔离SUI治疗病例中,手术的平均时间为14分钟(中间值=13;范围为7-20)。进行TVT手术的病人的住院时间仅平均为1.8天(范围为0.5-8天)。
没有出现主要的术后并发症。没有伤及尿道、膀胱、神经或肠的记录。没有发生手术中明显出血(>100ml)。没有手术中***穿孔的病例。手术后没有癍痕或血肿的报道。
仅观察到有很少的并发症或几乎没有。在一例病人中记录有术后少量***糜烂。3例病人(2.8%)具有完全性潴留;其中两例进行了相关的脱垂外科治疗。在这些病人中,在局部麻醉和静脉镇静(根据需要进行)的情况下,在手术后立即进行了带子释放操作。没有需要切开的带子。经过带子释放操作之后没有病人出现失禁或瘘。
27例病人(15.9%)在手术后直接抱怨说他们的腹股沟有中等疼痛或不适。这种现象常常在两天内缓解,并且所有病例用非类***止痛药控制。2例病人(1.9%)中有持续一周的更严重的疼痛,并伴有髋部关节痛,这可能是由于手术中采用的妇产科姿势造成的。
1例病人手术后第8天出现了浅静脉血栓形成,并且继发出现脓肿,需要引流。该病人的演化是有益的,因为出现这种不幸事件,使操作者更小心地加大预防力度,在外科治疗中给所有病人使用有效力的抗生素。
上述分析主要集中于本发明新近发展的外科手术和工具的再现性以及早期并发症出现率。在手术后的一个月,术后评估显示,107例病人中的95例(88.8%)治愈了SUI。另外8例有改善,而4例失败(3.7%)。手术未治愈的病人在手术后立即失禁。少数病人(n=9;8.4%)干预治疗后一个月重新呈现尿急。相反,一些手术前尿急的病人(n=33;30.8%)据称,手术后这种重点症状得以减轻。
以上的描述应看作为仅用于显示本发明的原理。因为对所属领域的技术人员来说,很容易进行各种变化和变型。不能够用上面图示和描述的精确结构和操作过程来限定本发明。因此,所有合适的修改和等同物均落入本发明的范围之中。
Claims (22)
1.一种用于治疗女性小便失禁的手术方法,所述方法包括:
在高于尿道平面的水平线上确定手术穿出点,手术针将在病人的股穿出,
在***壁上切开一个切口,
朝向坐骨耻骨支切开尿道侧面,同时避免***壁穿孔,
绕坐骨耻骨支旋转手术针,以及
将手术针的尖端从病人体内刺穿先前确定的手术穿出点。
2.如权利要求1所述的手术方法,其中在手术针穿过穿出点之前,将带子附着到该手术针上。
3.如权利要求1所述的手术方法,其中在手术针的尖端穿过穿出点之后,将线和管中之一附着到该手术针上。
4.如权利要求3所述的手术方法,其中在线和管之一附着到手术针上之后,使手术针向后朝向下尿道***开口旋转。
5.如权利要求4所述的手术方法,其中将带子附着到线和管之一上,并通过从穿出点移动线和管之一而拉动在病人体内的带子。
6.如权利要求1所述的手术方法,其中在手术针穿过闭孔之后,将手术针从手柄上取下。
7.一种用于治疗女性小便失禁的手术针,所述手术针包括:
手柄,
具有两个端部的直线段,所述直接段的一端与所述手柄相连,以及
具有两个端部的螺旋段,所述螺旋段的一端在结点处与所述直线段的另一端相连,
所述螺旋段的另一端为具有尖端的自由端。
8.如权利要求7所述的手术针,其中所述螺旋段的长度限定为圆周的1/2至3/4。
9.如权利要求7所述的手术针,其中所述螺旋段的尖端位于距结点水平距离为4至8cm的垂直平面中。
10.如权利要求7所述的手术针,其中所述螺旋段的尖端在结点上方间隔最多3.5cm。
11.如权利要求7所述的手术针,其中所述螺旋段的直径为2至5mm,长度为6至18cm。
12.如权利要求11所述的手术针,其中所述螺旋段的尖端包括一针眼。
13.如权利要求11所述的手术针,其中所述螺旋段的尖端包括多个卷曲段。
14.如权利要求7所述的手术针,其中所述螺旋段还包括一个直线段。
15.如权利要求7所述的手术针,其中所述螺旋段的直径为2至5mm,长度为4.5至17.6cm。

20.一种如权利要求19所述的箭状管,其由高密度聚乙烯制成。
21.一种如权利要求19所述的箭状管,其近端粘接到带子上。
22.一种导引器,包括用于接纳如权利要求14所述的手术针的直线段的开口管状段。
