Abstract
本发明涉及一种提高吞咽障碍患者进食安全性的组合物,所述组合物中麦芽糊精或淀粉的含量为30‑85%(w/w)、食用胶的含量为10‑60%(w/w)、赤藓糖醇的含量为1‑15%(w/w)和药学上可接受的载体。本发明组合物单位质量的粘度高,利于药物或营养成分的均匀分散,为吞咽困难患者精准提供所需的药物或营养成分,显著降低患者因吞咽不便而导致的误吸、呛咳或由此引发的感染发生率。

技术领域
本发明涉及生物医药领域,具体涉及一种提高吞咽障碍患者进食安全性的组合物及其制备方法和其应用。
背景技术
因多种原因引起的、发生于不同部位的咽下困难吞咽障碍(deglutitiondisorders)严重影响患者的摄食、营养吸收及口服给药,甚至导致食物误吸入气管而引发吸入性肺炎或感染,严重者甚至危及生命。2017年《中国特定人群吞咽功能障碍的流行病学调查报告》显示,中国吞咽障碍的患病率为38.7%。
吞咽障碍的病因包括生理原因、外科手术、遗传因素和疾病原因等。如衰老而引起的吞咽能力下降、放疗和化疗使得患者的肌肉变弱,与生理学和吞咽反射神经支配相关的神经退化等,以及与神经学疾病、传染性疾病、自身免疫疾病、代谢性疾病、肌病性疾病、医源性疾病、外科手术后肌肉或神经性疾病、放射疗法、腐蚀性和结构性疾病相关的疾病等。
食物质地与性状的调配对于能经口进食的吞咽障碍患者而言,是确保安全有效进食的先决条件之一。食物的性状影响吞咽过程,通过调节食物的性状,可以让部分吞咽患者安全有效地进食。研究表明,在液体食物中加入增稠组件能够增加液体食物的黏稠度、降低其流动性,给患者充分时间做出反应并控制会厌软骨盖住气管,是延长液体食物吞咽时间、降低吞咽安全风险的重要方法,也是解决吞咽功能障碍患者进食困难的有效措施之一。
CN105831711B公开了一种提供舒适粘稠度的速溶型增稠剂及其制备方法,所述增稠剂由黄原胶20.00%和麦芽糊精80.00%组成。
CN 110326792 A公开了一种基于罗望子多糖的非全营养流食,含有0.2%~0.35%罗望子多糖、0.15%~0.3%黄原胶、2%~12%赤藓糖醇、2%~10%抗性糊精和1%~15%营养组分,余量为水。
CN110522011A公开了一种改善吞咽障碍的增稠组件,含有亲水胶30%~45%,淀粉10%~30%,麦芽糊精40%~60%,其中,所述亲水胶包括黄原胶、瓜尔胶、刺云实胶、刺槐豆胶。
上述产品配置时容易产生团块或疙瘩,水合速率较慢,使用不方便,且增加吞咽后咽部残留不下的风险。为此,临床急需开发粘度适中且分散性好、均一性的粘度组合物。
发明内容
本发明的目的在于提供一种提高吞咽障碍患者进食安全性的组合物,组合物中麦芽糊精或淀粉的含量为30-85%(w/w)、食用胶的含量为10-60%(w/w)、赤藓糖醇的含量为1-15%(w/w)和任选地药学上可接受的载体。
本发明优选的技术方案中,组合物中麦芽糊精或淀粉的含量为35-80%(w/w),优选为40-78%(w/w)。
本发明优选的技术方案中,所述淀粉选自改性玉米淀粉、木薯淀粉、羟丙基二淀粉磷酸酯、马铃薯淀粉、抗性糊精中的任一种或其组合。
本发明优选的技术方案中,组合物中食用胶的含量为15-55%(w/w),优选为20-50%(w/w)。
本发明优选的技术方案中,所述食用胶选自黄原胶、瓜尔胶、海藻酸钠、刺槐豆胶、羧甲基纤维素钠、刺云豆胶、亚麻籽胶、秋葵胶、魔芋甘露聚糖、卡拉胶、豆角胶、葫芦巴胶、罗望子胶、肉桂胶、***胶、印度胶、果胶类、纤维质、黄蓍胶、刺梧桐树胶中的任一种或其组合。
本发明优选的技术方案中,所述组合物中赤藓糖醇的含量为1.5-12%(w/w),优选为2-10%(w/w)。
本发明的优选技术方案中,所述药学上可接受的载体选自酸碱调节剂、防腐剂、稳定剂、矫味剂或赋香剂的任一种或其组合。
本发明优选的技术方案中,本发明的制剂选自颗粒剂、散剂、合剂、露剂、糊剂、乳剂的任一种。本发明的组合物采用本领域熟知的各种剂型。
本发明优选的技术方案中,所述矫味剂选自山梨醇、木糖醇、甘露醇、乳糖醇、麦芽糖醇、赤藓糖醇中的任一种或其组合。
本发明优选的技术方案中,组合物中含有麦芽糊精40-78%、食用胶20-50%和赤藓糖醇2-10%,余量为水。
本发明优选的技术方案中,组合物含有麦芽糊精20-76%、黄原胶20-50%、瓜尔胶1-15%、海藻酸钠1-10%和赤藓糖醇2-10%,余量为水。
本发明优选的技术方案中,组合物含有麦芽糊精40-78%、黄原胶10-15%、瓜尔胶10-15%、海藻酸钠10-15%和赤藓糖醇2-10%,余量为水。
本发明优选的技术方案中,所述含量为2%的组合物水溶液的粘度不低于2500CP,优选不低于3000CP,更优选不低于3500CP。
本发明的另一目的在于提供一种提高吞咽障碍患者进食安全性的组合物的制备方法,组合物中麦芽糊精或淀粉的含量为30-85%(w/w)、食用胶的含量为10-60%(w/w)、赤藓糖醇的含量为1-15%(w/w)和任选地药学上可接受的载体,所述组合物的制备包括下述步骤:称取所需量的麦芽糊精或淀粉、食用胶和任选地药学上可接受的载体,混合均匀后,制粒,干燥,即得。
本发明优选的技术方案中,所述干燥选自流化床干燥、减压干燥、真空干燥、喷雾干燥、常压干燥的任一种。
本发明的另一目的在于提供本发明组合物用于制备提高吞咽障碍患者进食安全性的制品中的应用。
本发明的组合物利于减轻吞咽障碍患者吞咽中发生的误吸或呛咳风险。
本发明优选的技术方案中,所述的组合物用于吞咽障碍患者中促进安全吞咽的用途。

本发明优选的技术方案中,所述制品选自药物制剂、营养制剂、饮食补充剂、功能性食品、饮料产品的任一种或其组合。

除非另有说明,本发明的粘度检测使用NDJ旋转粘度计测量,粘度计的技术参数控制:3#转子,转速12rpm-60rpm。选择扭矩在30%-90%之间。
与现有技术相比,本发明的有益技术效果包括:
1、本发明组合物科学选用食用胶和麦芽糊精,综合利用食用胶的增稠效果和麦芽糊精优良的分散性,并具有易于冲调、快速分散溶解、水合速度快,无味,稳定性好,口服方便且灵活,无任何副作用等优点。
2、本发明组合物可应用于医用食品、普通食品、保健食品等领域,添加到饮品(如水、牛奶、果汁、营养液、咖啡等)中,增加粘稠度,提高患者的吞咽安全性,改善患者的吞咽功能、利于患者摄入充足营养和所需物质。
3、本发明组合物的制备操作简便,具有安全性高、成本低,绿色环保、适合工业化大生产等优点。

实施例1本发明组合物的制备
组合物的组成为:麦芽糊精41g、赤藓糖醇5g、黄原胶40g、海藻酸钠4g、瓜尔胶10g。
将原料混合均匀,装入喷雾干燥制粒机原料容器内,设定温控仪上进风温度控制范围(80℃±15℃),加热换热室内空气,使物料沸腾混合,升温干燥。称取干物质总量30%的纯化水作为粘合剂,设定蠕动泵转速150rpm、喷雾压力0.40MPa进行制粒20min,即得。
实施例2本发明组合物的制备
组合物的组成为:麦芽糊精70g、赤藓糖醇5g、黄原胶10g、海藻酸钠10g、瓜尔胶10g。
将原料混合均匀,装入喷雾干燥制粒机原料容器内,设定温控仪上进风温度控制范围(80℃±15℃),加热换热室内空气,使物料沸腾混合,升温干燥。称取干物质总量30%的纯化水作为粘合剂,设定蠕动泵转速150rpm、喷雾压力0.40MPa进行制粒20min,即得。

将原料混合均匀,装入喷雾干燥制粒机原料容器内,设定温控仪上进风温度控制范围(80℃±15℃),加热换热室内空气,使物料沸腾混合,升温干燥。称取干物质总量30%的纯化水作为粘合剂,设定蠕动泵转速150rpm、喷雾压力0.40MPa进行制粒20min,即得。
对比例2
组合物的组成为:黄原胶30g,瓜尔胶15g,淀粉10g,麦芽糊精45g。
将原料混合均匀,装入喷雾干燥制粒机原料容器内,设定温控仪上进风温度控制范围(80℃±15℃),加热换热室内空气,使物料沸腾混合,升温干燥。称取干物质总量30%的纯化水作为粘合剂,设定蠕动泵转速150rpm、喷雾压力0.40MPa进行制粒20min,即得。
试验例1快速增稠试验
将实施例1的组合物分别加水配成1.25%、2.50%、3.75%、5.00%的溶液,室温下测试粘度。结果显示,随着样品浓度的增加,产品粘度迅速增加,呈现非牛顿流体特征。少量添加即可有明显的增稠效果。见图1。
试验例2稳定性实验
将浓度为2%的实施例1的组合物水溶液室温下静置,4小时内粘度基本没有变化。结果见图2。
试验例3组合物在不同温度、不同液体中增稠效果实验
将实施例1的组合物与不同温度的液体制品按比例混合(组合物浓度为2%),结果表明,在不同温度、不同液体条件下,产品均具有良好的增稠效果,不受液体种类、液体温度的影响,产品适用范围广泛。在同一液体、不同温度(如低温水6℃、常温水20℃、热水60℃)下的粘度基本一致,增稠一致性良好。结果见图3-图4。
试验例4组合物凝胶性观察实验
取8个300ML烧杯,分别配制0.1%-6%各浓度下的实施例1的组合物水溶液,将烧杯口朝下倾斜45°观察,各样品流动性良好,未发现凝胶结块现象。采用旋转流变仪对各个样品进行动态黏弹性温度扫描,在动态黏弹性温度扫描范围内,黏性模量均大于弹性模量,两者没有出现相交点,体系固态性质不显著。
试验例5吞咽造影观察
将实施例1的组合物配制成2.5%浓度水溶液,选择10名不同年龄的志愿者,通过吞咽造影设备观察发现,样品在吞咽过程中在咽喉及食管部位无残留,结果见图6。
试验例6感官评价试验
选取年龄40-70岁之间的不同洼田饮水试验等级的志愿者,根据他们的吞咽障碍等级配制不同浓度的实施例1的组合物水溶液。受试者服用后对样品的感官、整体接受能力评分。结果见表1。

1.一种提高吞咽障碍患者进食安全性的组合物,其特征在于,组合物中麦芽糊精或淀粉的含量为30-85%(w/w)、食用胶的含量为10-60%(w/w)、赤藓糖醇的含量为1-15%(w/w)和药学上可接受的载体。
2.如权利要求1所述的组合物,其特征在于,所述组合物中麦芽糊精或淀粉的含量为35-80%(w/w),优选为40-78%(w/w),优选所述淀粉选自改性玉米淀粉、木薯淀粉、羟丙基二淀粉磷酸酯、马铃薯淀粉、抗性糊精中的任一种或其组合。
3.如权利要求1-2任一项所述的组合物,其特征在于,所述组合物中食用胶的含量为15-55%(w/w),优选为20-50%(w/w),优选所述食用胶选自黄原胶、瓜尔胶、海藻酸钠、刺槐豆胶、羧甲基纤维素钠、刺云豆胶、亚麻籽胶、秋葵胶、魔芋甘露聚糖、卡拉胶、豆角胶、葫芦巴胶、罗望子胶、肉桂胶、***胶、印度胶、果胶类、纤维质、黄蓍胶、刺梧桐树胶中的任一种或其组合。
4.如权利要求1-3任一项所述的组合物,其特征在于,所述组合物中赤藓糖醇的含量为1.5-12%(w/w),优选为2-10%(w/w)。
5.如权利要求1-4任一项所述的组合物,其特征在于,所述药学上可接受的载体选自酸碱调节剂、防腐剂、稳定剂、矫味剂或赋香剂的任一种或其组合。
6.如权利要求1-5任一项所述的组合物,其特征在于,所述组合物含有麦芽糊精40-78%、黄原胶20-50%、瓜尔胶1-15%、海藻酸钠0-10%和赤藓糖醇2-10%,余量为水。
7.如权利要求1-6任一项所述的组合物,其特征在于,所述含2%组合物的水溶液的粘度不低于2500CP,优选不低于3000CP,更优选不低于3500CP。
8.如权利要求1-7中任一项所述的组合物的制备方法,其特征在于,所述组合物中麦芽糊精或淀粉的含量为30-85%(w/w)、食用胶的含量为10-60%(w/w)、赤藓糖醇的含量为1-15%(w/w)和药学上可接受的载体,所述组合物的制备包括下述步骤:称取所需量的麦芽糊精或淀粉、食用胶和药学上可接受的载体,混合均匀后,采用流化床制粒,筛分后即得。
9.如权利要求1-7任一项所述的组合物或如权利要求8所述的方法制备的组合物制备利于吞咽障碍患者安全吞咽的制品中的应用。
